The permanently growing interest in amateur and professional sports activities among young, middle-aged and elderly athletes raises serious concerns about athletes’ health, the safety of physical training, and the sports-related risk of sudden cardiac death and other cardiovascular complications during exercise. In recent years there has been an increasing number of cases of sudden cardiac death during physical effort. At present, life expectancy in the most developed countries grows rapidly and the number of people >65 years dramatically increases. Moreover, biological age of the population is lower. Subsequently, relevant increase in the number of elderly athletes involved in various types of sports activities has been reported in many countries. It was also demonstrated that physical activity has strong beneficial effect on cognitive functions, psychomotor performance and thus exercise capacity, which is very important for the elderly people in their everyday routine activities. Nonetheless, it should be remembered that participation in amateur and professional sports activities may be associated with risk of serious cardiovascular events in the elderly athletes often suffering from various civilization diseases. It is also reported that the number of elderly people after interventional cardiology procedures, open heart surgery and cancer treatment (chemotherapy, radiotherapy), who wish to participate in various sports activities, systematically grows. The authors of the paper conduct a literature review on cardiovascular risk assessment in the elderly athletes including the contemporary cardiology diagnostic methods and diagnostic schemes to prevent sudden cardiac death and other cardiovascular events during exercise. The controversies over efficacy of particular diagnostic tools to detect cardiovascular diseases in the elderly athletes and worldwide epidemiologic data concerning risk of sudden cardiac death during physical exercise have been also presented. Here, the authors have derived suggestions for establishment of comprehensive diagnostic schemes to prevent sudden cardiac death during sports activities.

The Mediterranean diet is characterized by abundance of plant foods, such as vegetables, fruit, bread and cereal products, legumes, nuts and seeds, as well as olive oil, herbs and spices. Moreover, moderate intakes of dairy products, fish, poultry and wine, and low consumption of red meat are recommended. This diet is low in saturated fat (<7% of energy) with total fat within the range of 25-35% of energy. To assess dietary compliance cardiovascular diseases with the recommendations of the Mediterranean diet a variety of indicators are used, of which the most common are: 9-point scale of alternate Mediterranean Diet Score (aMED) and 14-item Questionnaire of Mediterranean diet adherence (MDA). The results of the epidemiological research indicate that the nutrition model based on the assumptions of the Mediterranean diet is a crucial component of primary and secondary prevention of 25-35% of energy.

The most effective and the cheapest method of cardiovascular disease prevention is changing lifestyle. Cardiovascular disease is caused by many factors. They include: a diet rich in saturated fat and cholesterol, smoking, a sedentary lifestyle, physical inactivity, overweight and obesity. The aim of the research was to assess the level of knowledge of the students of the Medical University about cardiovascular disease prevention. Material and methods: Research was conducted on 200 students of the Medical University of Lublin, residing in the Student House No. 4 in Lublin. Self-authorship questionnaires were used to assess the level of knowledge of the students of the Medical University about cardiovascular disease prevention. Research results: The students of the Medical University have broad knowledge about the influence of physical activity, diet, cigarettes and alcohol use on cardiovascular disease prevention. 90.5 percent of the respondents know that smoking greatly increases the risk of cardiovascular disease. More than a half of the students are aware that excessive alcohol consumption has a negative impact on the cardiovascular system. 38.67 percent of the respondents recognize moderate alcohol consumption as protective for cardiovascular disease. The remaining 3.13 percent of the students have no knowledge about this issue. Almost half of the respondents (45 percent) is aware that psychosocial factors have an impact on the cardiovascular system. 86.5 percent of the students believe that there is a correlation between cardiovascular disease and factors such as: low socioeconomic status, social isolation, stress, negative emotions, depression. The remaining 13.5 percent of the respondents have incomplete knowledge about this issue. Almost the half of the respondents knows that type A personality increases the risk of cardiovascular disease, whereas the remaining 52 percent of the students have incomplete knowledge about this issue.

Objectives Of all work stressors, occupational stress is the leading cause of many disorders among workers. Drivers are classified as a high risk group for work related stress. This study set out to determine the relationship between risk factors of cardiovascular diseases and occupational stress among drivers. Material and Methods Two hundred and twenty two Ilam’s intercity drivers were selected for the study. For measuring work stress, the Osipow work stress questionnaire was used. After a 10-h fasting period, systolic and diastolic blood pressure was recorded. Intravenous blood samples were taken to determine cholesterol, triglyceride and blood glucose levels. The independent samples t-test and Pearson’s correlation test were used to assess the relationship between variables and occupational stress. Results Seventy-one percent of the intercity drivers suffered from average to acute stress, and 3.1% of them suffered from acute stress. There was no significant relationship between occupational stress and diastolic blood pressure (p = 0.254) among the drivers. Nevertheless, the Pearson’s correlation test demonstrated a strong relationship between work stress and blood glucose (p < 0.01), while no strong correlation was found for blood triglycerides and cholesterol levels. Conclusions Based on the results, high rates of occupational stress were observed in the Ilam’s intercity drivers. Occupational stress may have effect on blood glucose levels but the results did not suggest a considerable relationship between risk factors of cardiovascular diseases and occupational stress among intercity drivers. Background. Diseases of the cardiovascular system (CVS) are among the most common diseases of humankind (WHO). Monitoring of blood pressure (BP) is an accessible method for evaluating global hemodynamic processes. The functional reserves of the circulatory system are traditionally determined by the use of functional loading trials and tests. Objectives. The aim of the study is to enhance the information collected during the blood pressure measurement process by studying the levels of adaptation of the CVS to physical activity with morphological, temporal, spectral and correlation analyses of arterial oscillography (AO). Material and methods. In 178 healthy individuals aged 18–20 years, arterial oscillograms were recorded during blood pressure measurement and correlations of the functional reserve of the cardiovascular system at various stages of adaptation to a Ruffier test were investigated. Results. The proposed methods of AO analysis significantly increase the informativeness of the procedure for blood pressure measurement, provide an opportunity to conduct a visual analysis of AOs and to assess the state of the cardiovascular system, its reserve capabilities and its ways of adapting to shoulder compression at rest, after physical load and in the process of recovery. Conclusions. Using the information technologies proposed by the authors of the morphological, temporal, spectral and correlation analysis of arterial oscillograms, their evaluation and clinical interpretation significantly increase the informativeness of the blood pressure measuring process. They can be used for early detection of pre-morbid conditions and functional blood circulation reserves, which will help the physician to more effectively plan a preventative, diagnostic and therapeutic process.

Introduction. Cardiovascular disease is the first cause of morbidity and mortality worldwide. Among several known risk factors, researchers also focus their attention on the chronic exposure to air pollution. There is much evidence that exposure to air pollution, especially to ultrafine particles, can damage the endothelium and can favour cardiovascular diseases in the general population. Occupational exposition could be an additive risk factor for the cardiovascular system. This article presents a scientific review of the linkage between occupational exposure to air pollution and ischemic heart disease. Materials and method. A scientific review was undertaken, followed by PRISMA Statements. Observational studies were selected from several scientific databases, likesuch as Pubmed, Google Scholar, Nioshtic-2 and Reserchgate, searching for selected key words: police workers, professional drivers, mail carriers, filling station attendants, road cleaners, garage workers, motor vehicles and engine maintenance. All the key words were combined with “Boolean Operators” with the following words: cardiovascular (or cardiac) disease, cardiovascular function, cardiovascular system, ischemic heart disease, coronary disease, myocardial infarction. During the systematic research, the focus was on retrospective and prospective studies from January 1990 – December 2014. Results. Both the retrospective and prospective studies showed an increased risk of ischemic heart disease in occupationally occupied people exposed to air pollution. Only one study presented a ly minor risk. Conclusions. The findings of this systematic review suggest a possible linkage between occupational exposure to urban air pollution, especially to motor exhaust and particulate, and ischemic heart disease.

This is due to the narrow gap that is between the petal and the ring. Composite materials and a new valve design can minimize this problem. Injected polyurethane with a metallic, titanium bonded insert optimal microscale flexibility with macro-stiffness for ensuring appropriate mechanical functions of the valve.

Objectives: Limited research on the health situation of teachers on long-term sick leave is available. The aim of this study has been to describe the health status of female teachers on long-term sick leave (LSFT) in comparison to working female teachers (WFT) and to determine predictors for their state of mental health (MH) and cardiovascular fitness (CF). Material and Methods: Twenty-eight LSFT and 300 WFT (average age: 53±5 years old) participated in a screening diagnostic inventory. Mental health, CF, blood pressure (BP), body mass index (BMI), body fat mass (BFM), health behavior (smoking, alcohol intake, physical activity) and disease burden (DB – number of medical diagnoses) were analyzed for the purpose of characterization of the health status. The multiple linear regression analysis was performed to identify predictors for the state of MH and CF. Results: Adverse values for the MH but also for CF, BFM and the DB (median of medical diagnoses: LSFT: 5; WFT: 2) among the LSFT in comparison to the WFT were confirmed. Additionally, the part of smokers among LSFT (25%) was higher (WFT: 8%). In contrast, the WFT (61%) were much more affected by an elevated BP (LSFT: 26%). Disease burden proved as the strongest predictor for MH of the female teachers. Age, BMI and DB proved as predictors for CF. Conclusions: Health-related differences between long-term sick leave and working teachers were particularized and a link between physical and mental health among teachers was quantified. Therefore, health-related concepts for teachers should equally focus on physical and psychological aspects. The relevance of regular well-structured occupational health check-ups should be brought to the attention of the profession to prevent diseases and early retirements. The purpose of this study was to evaluate differences in heart rate recovery (HRr) in trained and untrained adults, while assessing the role of physiological and emotional factors. Eighteen untrained and 21 trained participants completed a maximal exercise test and a 20-min treadmill exercise at 55–70% heart rate reserve, and emotional state was assessed prior to exercise. Multiple regression was used to assess relationships between heart rate recovery and physiological and emotional assessments. The trained group had a higher relative maximal oxygen consumption (p < 0.001), lower resting heart rate (p < 0.001), and faster short- and long-term heart rate recovery (p < 0.05) than the untrained group. Resting heart rate was the most predictive measure with HRr for the trained group (R = 0.551–0.818), whereas resting heart rate, maximal heart rate, and fitness were predictors of recovery in the untrained group (R = 0.764–0.977). The results show the predominant parasympathetic influence on HRr in the trained group, but indicates influence of fitness and exercise intensity on recovery in the untrained group. Thus, fitness appears to influence HRr in those only with low fitness. This notion may help influence the behavior of untrained individuals to improve fitness to reduce risk of mortality and morbidity.

Background: Screening represents an important tool to improve detection of cardiovascular risk factors. Uniform standards for screening programs and for evaluation of their effectiveness are still lacking. Material and methods: The results of the Polish 400 Cities Project (P400CP) were analyzed by age and gender, and with regard to the prevalence and awareness of cardiovascular risk factors. The P400CP population were volunteers reporting for screening tests in small towns, and a representative sample was used as control. Results: Mean age in the screening group was 10 years more than in the representative sample. Two thirds of participants were women. First-time diagnosis rates for elevated arterial blood pressure, hyperglycemia and hypercholesterolemia in the screening were 28%, 15% and 51%, respectively. Almost 40% of participants reported previously diagnosed hypertension (vs. 25% of the control sample), 7% vs. 4% reported diabetes and 16% vs. 9% – hypercholesterolemia. Smokers represented 16.5% of participants (36% of the control sample). Conclusions: “Open” screening programs fail in targeting young adults, especially males, and people reporting for screening are often already diagnosed with arterial hypertension and/or diabetes. Despite these limitations, “open” screening is effective in detecting undiagnosed cases of hypercholesterolemia. Non-smokers report for screening tests more often.

Our program, partially based on the European SCORECARD program, takes into account parameters such as age, sex, systolic blood pressure, total cholesterol concentration and cigarette smoking, and enables internet users to observe themselves to determine the degree of their CVD risk.

Ventilatory threshold is one of the ways to measure cardiovasculatory fitness of the body. Therefore, in the present study it was decided to demonstrate which physiological parameters most accurately express the second ventilatory threshold (VAT2) depending on the cardiovasculatory fitness of different groups of athletes and untrained men. The study involved the following athletes: race walkers (n=14), weightlifters (n=16), powerlifters (n=16), runners (n=14), professional soccer players (n=13), amateur soccer players (n=16), martial arts (n=12), and untrained men (n=15). Subjects’ VAT2 and maximal load (ML) were recorded and at these levels were determined the value of achievable maximal power (P), oxygen uptake (VO2), heart rate (HR), the ratio for oxygen uptake and heart rate (VO2/HR) and the rate-pressure produkt (RPP). It was shown that subjects were of similar age but different body mass (BM) and BMI. There were also differences between athlete groups at VAT2 and ML in relation to: P, VO2, VO2/HR (p<0.001) and RPP only at VAT2 (p<0.023). Reached HR values at VAT2 as well as at ML have not differed between the groups. There were also intergroup differences at VAT2 in terms of relative values: %VO2max (p<0.002), %Pmax(p<0.016), %VO2max/%HRmax (p<0.03). Relatively expressed %HRmax and %RPPmax reached at VAT2 did not differ between the two groups. Runners, professional soccer players and race walkers achieved the most favorable indicators of physical performance. It has been demonstrated that VAT2 besides P i %Pmax was best described by VO2, %VO2max and by VO2/HR and %VO2max/%HRmax, as well as by RPP. On the other hand HR and %HRmax, as well as %RPPmax are not useful in this regard. Furthermore it should be recognized that athletes in whose structure of the training occurred running of varying intensity achieved the highest physical fitness.

Over the years, researchers have focused on identifying specific personality traits associated with specific disease manifestations. Mental health and psychological status, as well as personality characteristics, have been studied and identified as some of the prognostic factors of cardiovascular disease, along with physical ones, such as hypertension, hyperlipidemia and diabetes mellitus. This article focuses on Type D personality characteristics and the way it may affect the onset of cardiovascular disease, the prognosis and quality of life of patients. People with different personality characteristics adjust differently to disease. More specifically, people with type D personality have been found to have poor general health and suffer for a variety of health-threatening situations. Type D personality and psychological distress, in general, have been suggested as causes for poor prognosis for patients with cardiovascular disease, as these patients are less adherent to their therapy and have a number of unhealthy lifestyle habits, such as smoking and low activity levels. It seems necessary that attention should be paid to psychological characteristics and personality types, as risk or protective factors for cardiovascular disease.
Na przestrzeni lat naukowcy skupili się na określeniu specyficznych cech osobowości związanych z konkretnymi objawami choroby. Zdrowie psychiczne i stan psychiczny oraz cechy osobowościowe zostały zbadane i zidentyfikowane jako niektóre z czynników prognostycznych choroby sercowo-naczyniowej, jak i fizyczne, takie jak nadciśnienie tętnicze, hiperlipidemia i cukrzyca. W tym artykule skupiono się na charakterystyce osobowości typu D oraz sposobie jego wpływu na początek choroby sercowo-naczyniowej, rokowania i jakość życia pacjentów. Osoby o różnych cechach osobowości różnią się w zależności od choroby. Dokładniej mówiąc, u pacjentów z osobowością typu D stwierdzono, że mają słabe zdrowie ogólne i cierpią z powodu różnych stanów zagrażających życiu. Typowa osobowość typu D i psychiczne cierpienia są ogólnie sugerowane jako przyczyny złego rokowania u chorych na choroby sercowo-naczyniowe, ponieważ pacjenci są mniej przyzwyczajeni do terapii i mają wiele niezdrowych nawyków związanych ze stylem życia, takich jak palenie tytoniu i niskie poziomy aktywności. Wydaje się, że należy zwrócić uwagę na cechy psychiczne i typy osobowości, jako czynniki ryzyka lub czynniki chroniące chorobę układu sercowo-naczyniowego.

Introduction and objective. The aim of this study was to show the prevalence of hypertension and major risk factors of cardiovascular disease among healthy adults in an agricultural region of south-eastern Poland, and the changes which have occurred in this area during the 12-year follow-up. Materials and method. 1,233, mostly rural inhabitants of Zamosc County without previous history of diabetes and CVD were subjected to analysis. Prevalence of hypertension and major cardiovascular risk factors were evaluated. Changes in the prevalence of risk factors between 1997 – 2008 were analyzed. Results. 33.0% of the examined population are active smokers, and there was a 1.8-fold increase (p = 0.0009). The percentage of people with hypercholesterolemia between 1997 – 2008/9 increased almost 2-fold (p<0.0001) and now it is 62.3%. The number of people with high blood pressure decreased nearly by a half (46%, p <0001) and it is currently 25.8%. Improper waist circumference was observed in 32.8% of the population (33.1% women, 26.5% men). Abdominal obesity decreased among men (48%; p=0.0008) and rural residents (29%, p=0.01). In comparison with 1997, in 2008 2009, the cardiovascular risk assessed using SCORE tables increased. The percentage of people with high-risk (≥5%) almost tripled in the general population (p=0.0183) and increased 4-fold in men (p=0.0145). Conclusions. Detection of hypertension in the rural region in which the survey was carried out is still too low. Actions against tobacco addiction should be a major component of health-education programmes for the rural areas of south-eastern Poland.

Atherosclerotic cardiovascular disease has been recognized as the leading cause of death for both men and women in developed countries. During the past century epidemiological and observational studies established that inappropriate nutrition together with reduced physical activity and increased tobacco consumption are key factors for cardiovascular disease development. Recent epidemiological and clinical trial data provide evidence of the great effectiveness of dietary interventions in cardiovascular disease prevention and treatment. Therefore, the efforts to improve diet play an important role in shifting population disease risk. In this paper, the scientific background and current recommendations for dietary prevention of cardiovascular disease are summarized.

Introduction. Undergraduates in many institutions live a lifestyle that could predispose them to non-communicable diseases (NCDs) including cardiovascular-related. Previous studies on modifiable risk factors (MRFs) against NCDs had focused mainly on adults, whereas young people should be prime targets for prevention programmes. Aim. This study was aimed at investigating knowledge of MRFs against cardiovascular diseases (CVDs) among university undergraduates in Ibadan, Nigeria. Material and methods. A descriptive cross-sectional design was adopted, where 500 undergraduates participated. Data were collected using Focus Group Discussion (FGD) guide and validated questionnaire. Data analyses were conducted using thematic approach and bivariate methods at 5% levels of significance, respectively. Results. Mean age was 22.8±3.0 years and 51.4% were females. The majority reported that their fathers (82.8%) and mothers (83.0%) had no hypertension history. The FGD revealed that students were knowledgeable of MRFs and preventive behaviours but still engaged in unhealthy lifestyle practices. The MRFs identified included excessive alcohol consumption (85.4%), unhealthy diet (77.6%), physical inactivity (75.2%) and smoking (70.2%). Respondents (56.4%) had good knowledge of MRFs against CVDs; 57.0% had fair lifestyle practices and 54.8% had good knowledge of preventive behaviour against CVDs. Conclusion. Gender was significantly associated with knowledge of CVDs preventive behaviours. Institutional-based lifestyle modification programmes, using peer group education is suggested.195-205

Objectives: Health care workers constitute a high-risk occupational category owing to the character of their work that includes high-risk environment, shift work and mental as well as physical stress. In occupational medicine, caring for their health condition should be a priority and include measures aimed at preventing cardiovascular diseases. The study aimed at determining the prevalence of cardiovascular disease (CVD) risk factors in employees of a large hospital and assessing their effect on the incidence of cardiovascular events. Materials and Methods: The group comprised 3124 employees with a mean age of 36.1 years (SD = 11.4), out of whom 562 were males (mean age of 37.1 years, range: 18-72; SD = 12.26) and 2562 were females (mean age of 35.9 years, range: 18-68; SD = 11.24). At their initial examination, the employees filled in a questionnaire on basic CVD risk factors (according to valid recommendations). This was supplemented with objective data to determine the risk of CVD using valid charts. From this group, a subset of persons at a high or intermediate risk was selected, comprising 247 individuals with a mean age of 54.1 years (SD = 5.73). After 5-9 years (mean 7.24±1.38 years), they either underwent another examination or their health status was ascertained by phone or in a computer database. The end point was the incidence of cardiovascular events (sudden death, acute myocardial infarction, unstable angina pectoris, percutaneous coronary intervention, cardiac failure, stroke or transient ischemic attack). Results: The end point was noted in a total of 15 males (6.07%) and 6 females (2.42%), being statistically significantly present in managers (males p < 0.00007, females p < 0.00001), male physicians/surgeons (p < 0.025), tertiary-educated males (p < 0.0095), female smokers (p < 0.015), male ex-smokers (p < 0.007), overweight or obese males (p < 0.02) and those with the waist-to-hip ratio above 1.0 (p < 0.005). Conclusions: Cardiovascular events are most likely to occur in obese male physicians/surgeons holding managerial positions and in female managers.

Introduction and objective. There are some clinical situations where a high level of HDL cholesterol (HDL-C) may be unfavourable. In these situations, HDL-C may undergo some changes, and even if its quantity is within the reference range, its quality is no longer the same. Brief description of state of knowledge. Diabetes is the state of elevated oxidative stress. Studies conducted to-date have revealed an increased production of the reactive forms of oxygen as the result of tissue damage in diabetes patients. The expression ‘dysfunctional HDL’ has been coined in the literature to describe high-density lipoproteins that lose their antioxidative and anti-inflammatory properties, that is, HDL-C that loses its basic functions. Recent observational studies have confirmed that the atheroprotective activity of properly functioning HDL-C is frequently impaired in clinical situations associated with oxidative stress. The presented review lays the foundation for a new approach to understanding how the functional properties of HDL help reduce cardiovascular risk. Conclusions. In the light of presented findings it seems that there is a need to seek a better diagnostic marker than HDL-C level. This study presents some possible directions for future research to bring us closer to the full understanding of the HDL particle and its role in patients with ischemic heart disease and type 2 diabetes.

This study aimed to assess cardiovascular strain during sawmilling operations in terms of physical workload,based on heart rate changes. We measured resting and working heart rates and calculated cardiovascular load (%CVL), cardiovascular strain (%CVS) and relative heart rate (%RHR) in 35 sawmill workers. Based on heart rate only, the work in sawmills was classified as very heavy and extremely heavy. Similarly, a high-level category was recorded for %CVL and a very high range for %CVS. Thus, the workload in sawmill operations is usually very high and can lead to physiological strain of the workers. There is a need to redesign the work content of this occupation to prevent excessive strain in the workers, as this will increase their productivity and reduce their health risk.

Introduction. Cardiovascular disease (CVD) prophylaxis in general practice, especially in rural areas, is a major organizational challenge. Objective. The aim of this study was to determine the prevalence of CVD risk factors in the studied population, and to identify the effect of the number of planned prophylactic consultations on selected clinical parameters, risk factors, and total cardiovascular risk on the SCORE scale (Systemic Coronary Risk Evaluation). Materials and method. The study included patients of a rural general practice, aged 35–55 years, with at least one modifiable CVD risk factor. Medical history was obtained, a physical examination performed, blood glucose levels, lipid profile, BMI, waist circumference and blood pressure measured and the SCORE cardiovascular risk at baseline and at the end of the study calculated. All participants were provided with targeted specific education. Analysis was performed in two groups of patients (1 and 2), where group 1 had one more prophylactic consultation than Group 2. Results. The results at baseline and a year later were compared in pharmacologically untreated patients, 26 in Group 1 and 34 in Group 2. In Group 1, which had had more prophylactic consultations, a statistically significant decrease was found in the mean systolic blood pressure: 131.000–124.782 (p= 0.02721), mean diastolic blood pressure: 86.846–83.462 (p=0.01111), and a statistically significant decrease in total cardiovascular risk on the SCORE scale (p=0.0478). Conclusions. The higher number of preventive consultations had an impact on a statistically significant decrease in mean blood pressure and mean SCORE value. The year-long cardiovascular disease prophylaxis programme proved less effective than expected, and neither a decrease in body weight nor an improvement in lipid metabolism was achieved in any of the groups.

This article reports findings from a program of research on the systematic influence of implicitly perceived facial expressions of emotions on effort mobilization in cognitive tasks. Recently published research on the implicit-affect-primes-effort (IAPE) model (Gendolla, 2012) has revealed replicated evidence for this effect: implicitly perceived facial expressions of sadness, anger, fear, and happiness influence effort-related cardiac response during cognitive performance. In further support of the IAPE model, those studies revealed that the effects of implicitly processed emotional expressions on effort mobilization differ systematically: Implicit fear and sadness expressions that are processed online during task performance render tasks subjectively more difficult, resulting in relatively high effort. Implicit happiness and anger expressions have the opposite effect. Moreover, objective task difficulty and incentive moderated the effect of implicit affect, and especially controlled processing of affect primes turned out to be a boundary condition.

Anticancer therapies have extended the lives of patients with cancer, but for some, this benefit is tempered by cardiovascular complications. Their number is increasing as a result of an improvement in the early diagnosis of cardiotoxicity caused by chemo- and radiotherapy. Therefore prevention, detection, monitoring, and treatment of cancer patients at risk of cardiotoxicity with or without concomitant CV diseases are very important. Cardio-oncology is a new direction in Ukraine for improving clinical outcomes of cancer patients. This review aims to provide an overview of the rationale for setting up a Cardio-Oncology Unit and reflects our own experience establishing this service.

Introduction: High demands of modern social environment to the child’s individual development are connected with their health decline and wide variation of individual capabilities. Because of such inconsistency decreased functional abilities are being observed in preschoolers. It leads mainly to health problems and developmental disorders. Body adaptive properties are considered to be the integral indicator of health. Researches find high prevalence of the lack of adaptation among children of different age groups which might lead to low quality of life. Aim of the study: The aim of the research was to estimate preschooler’s cardiovascular system functioning and the adaptation abilities based on the examples of children from Tarnopol, Ukraine. Material and methods: 170 healthy preschoolers were among the participants of the research. Anthropometry and the assessment of some hemodynamic parameters were performed to draw conclusions about the level of their cardiovascular capacity. Results: Only 54.7% of the examined children reflect the satisfactory level of cardiovascular system adaptation. The deviations mainly result from high BMI, Rate Pressure Product and abnormal reaction of the autonomic nervous system after a squatting test. Conclusions: Simple noninvasive methods (functional changes index, pulse rate and blood pressure and the indicator of arteries’ pressure) help to identify children with reduced circulatory response to stress and physical burden.
Wstęp: Wysokie wymagania współczesnego środowiska społecznego wobec indywidualnego rozwoju dziecka są związane z ich niskim poziomem zdrowia i dużym zróżnicowaniem adaptacji poszczególnych funkcji organizmu. Z powodu takiej niezgodności u dzieci w wieku przedszkolnym obserwuje się zmniejszenie zdolności funkcjonalnych. Prowadzi to do problemów zdrowotnych i zaburzeń rozwojowych. Właściwości adaptacyjne organizmu są uważane za integralny wskaźnik zdrowia. Wczesne badania pozwoliły znaleźć wysoką częstość dysadaptacji dzieci z różnych grup wiekowych, co może prowadzić do zniżonej jakości życia. Cel badania: Celem przeprowadzonych badań była ocena funkcjonowania układu sercowo-naczyniowego i określenie zdolności adaptacyjnych na przykładzie dzieci z Tarnopola na Ukrainie. Materiał i metody: W badaniach uczestniczyło 170 zdrowych przedszkolaków (w wieku 3-5 lat). Badanie pomiarów antropometrycznych i niektórych parametrów hemodynamicznych przeprowadzono w celu określenia poziomu adaptacji i dyzadaptacji układu sercowo-naczyniowego dzieci. Wyniki:Jedynie 54,7% badanych dzieci posiada zadowalający poziom ogólnego dostosowania się układu krążenia. 45,3% przedszkolaków wykazuje ogólne odchylenia adaptacji (wysoki BMI, wysokie ciśnienie krwi, nieprawidłowe reakcje układu sercowo-naczyniowego i nerwowego w próbie wysiłkowej). Wnioski: Proste nieinwazyjne metody (definiowanie wskaźników antropometrycznych, pulsu i ciśnienia krwi oraz obliczenia indeksu zmian funkcjonalnych, wskaźnik ciśnienia tętniczego) mogą pomóc w identyfikacji dzieci z obniżonym poziomem odporności układu krążenia na stres i obciążenie fizyczne.

Disorders of the heart and blood vessels are the leading cause of health problems and death. Early detection of them is extremely valuable as it can prevent serious incidents (e.g. heart attack, stroke) and associated complications. This requires extending the typical mobile monitoring methods (e.g. Holter ECG, tele-ECG) by introduction of integrated, multiparametric solutions for continuous monitoring of the cardiovascular system. In this paper we propose the wearable system that integrates measurements of cardiac data with actual estimation of the cardiovascular risk level. It consists of two wirelessly connected devices, one designed in the form of a necklace, the another one in the form of a bracelet (wrist watch). These devices enable continuous measurement of electrocardiographic, plethysmographic (impedance-based and optical-based) and accelerometric signals. Collected signals and calculated parameters indicate the electrical and mechanical state of the heart and are processed to estimate a risk level. Depending on the risk level an appropriate alert is triggered and transmitted to predefined users (e.g. emergency departments, the family doctor, etc.).

Hungarian Americans share a unique culture of food traditions associated with their value system and way of life. Researchers, health care providers, and nutrition professionals counseling and treating a Hungarian-American population should develop a baseline of cultural understanding to achieve successful and long-lasting behavior change outcomes. The leading causes of death among Hungarians include ischemic heart disease (21.3%), stroke (13.4%), and cirrhosis (5.8%); all are directly or indirectly attributed to a traditional Hungarian diet coupled with a sedentary lifestyle. Health behaviors among Hungarian Americans can be partially explained by the Health Belief Model’s value-expectancy construct. Understanding cultural expectations and their associated values serve as a foundation for health promotion programming to reduce risk of cardiovascular disease and comorbidities. This review explored numerous facets of Hungarian-American dietary habits in psychosocial, economic, historical, and cultural contexts. Health education and health promotion considerations were also examined.

Cardiovascular Diseases (CD) are currently one of the most common causes of death. Because heart related deaths occur on such an enormous scale this phenomenon is referred to as an epidemic. Chronic and acute injury of the heart could be an effect of cardiac remodeling, which is a result of molecular, cellular and interstitial changes, influenced by hemodynamic load or neurohormonal activation (Cohn et al., 2000). These small deviations in cardiac activity and morphology may lead to an enormous negative effect. Despite a significant progress, knowledge of standard risk factors for cardiovascular diseases has become less and less effective, which is why predicting and seeking an appropriate treatment is very challenging. As a result, there is a growing interest in finding new markers of the CD. MicroRNAs (miRNAs), are short, non-coding RNAs responsible for regulation of gene expression at the post-transcriptional level. Among them that have the greatest potential are microRNA molecules that circulate in the blood plasma or serum, that are related to direct activation of signaling pathways, implicated in the aging process and thus for the development of cardiovascular disease. This paper is a summary of the current state of knowledge on miRNAs, their biogenesis and potential role as biomarkers to diagnose heart disease.

Wstęp. Zachowanie właściwej diety jest istotnym czynnikiem prewencji chorób układu krążenia (ChUK). Ważne w tym zakresie jest prowadzenie edukacji żywieniowej pacjentów. Cel pracy. Ocena sposobu żywienia, preferencji i częstotliwości spożycia produktów spożywczych przez pacjentów poradni kardiologicznej. Materiał i metody. Badania przeprowadzono w 2012 r. wśród 43 kobiet i 57 mężczyzn powyżej 40. roku życia. Wykorzystano ankietę dotyczącą sposobu żywienia oraz wyliczono wskaźnik BMI. Wyniki. Stwierdzono niski poziom spożycia warzyw i owoców oraz ryb – zwłaszcza wśród mężczyzn – a także nadmierne spożycie mięsa i wykorzystanie niewłaściwych technik przygotowania potraw mięsnych oraz błędne zastosowanie tłuszczów. Wnioski. Z przeprowadzonych badań wynika słaba znajomość zasad prawidłowego żywienia u osób z chorobami układu krążenia. Zatem istotna jest jak najwcześniejsza edukacja żywieniowa.
Background. Maintaining a proper diet is an important factor in the prevention of cardiovascular diseases (CVD). In this regard nutritional education is important for patients. Objectives. Evaluation of nutritional manner, preferences, and frequency of food products consumption by patients from cardiology department. Material and methods. The study was conducted in 2012 among 43 women and 57 men over 40 years old. A questionnaire concerning nutritional manner was used and BMI was calculated. Results. A low level of consumption of fruits, vegetables and fish especially among men, excessive consumption of meat were found. Apart from that improper dish preparation techniques of meat and wrong application of fats were identified. Conclusions. The study shows poor knowledge in regard of the nutritional principles among people with cardiovascular diseases. Therefore, preventive nutritional education seems to be necessary.

Proces zapalny jest bezpośrednio związany z sekrecją cytokin, np. czynnika martwicy nowotworu alfa (ang. Tumor Necrosis Factor alpha; TNF-α). Ta cząsteczka jest 1 z 22 białek należących do rodziny TNF i wydzielana jest głównie przez: makrofagi, monocyty, limfocyty T oraz komórki tuczne. Biologiczne efekty działania TNF-α zachodzą dzięki wiązaniu się tej cytokiny ze specyficznymi dla niej receptorami – TNFR1 i TNFR2. Duża liczba prac potwierdza kluczową rolę TNF-α w nowotworzeniu i chorobach układu sercowo-naczyniowego, będących chorobami o podłożu prozapalnym. Niestety, mimo postępu medycyny i wzrostu świadomości społeczeństwa, wymienione choroby stanowią główne przyczyny śmierci na świecie. Lepsze zrozumienie roli tej cytokiny w kancerogenezie i chorobach zapalnych może spowodować wykorzystanie TNF-α jako markera tych chorób oraz do monitorowania przeciwzapalnych efektów terapii.
The inflammatory process is directly associated with secretion of cytokines, e.g. tumor necrosis factor alpha (TNF-α). This molecule is one of the 22 proteins which belong to TNF family and is secreted mainly by: macrophages, monocytes, T lymphocyte and mast cells. The biological effects of TNF-α is possible through binding this cytokine to specific receptors – TNFR1 and TNFR2. The large number of reports provides that this cytokine plays extremely important role in cancers and cardiovascular disease – two groups of inflammatory diseases. Unfortunately, these diseases are the main cause of death in spite of advances in medicine and increasing public awareness of prevention. It is believed that better understanding both molecular potential of this cytokine and the impact in cancerogenesis and others inflammatory diseases may cause using TNF-α as a molecular marker in these diseases and will make it possible to observe the effects of anti-inflammatory therapy. It will be able to cause a drop in the incidence of these diseases and better monitoring of them.

Coronary Artery Disease is the type of cardiovascular disease (CVD) that happens when the blood vessels which stream the blood toward the heart, either become tapered or blocked. Of this, the heart is incapable to push sufficient blood to encounter its requirements. This would lead to angina (chest pain). CVDs are the leading cause of mortality worldwide. According to WHO, in the year 2019 17.9 million people deceased from CVD. Machine Learning is a type of artificial intelligence that uses algorithms to help analyse large datasets more efficiently. It can be used in medical research to help process large amounts of data quickly, such as patient records or medical images. By using Machine Learning techniques and methods, scientists can automate the analysis of complex and large datasets to gain deeper insights into the data. Machine Learning is a type of technology that helps with gathering data and understanding patterns. Recently, researchers in the healthcare industry have been using Machine Learning techniques to assist with diagnosing heart-related diseases. This means that the professionals involved in the diagnosis process can use Machine Learning to help them figure out what is wrong with a patient and provide appropriate treatment. This paper evaluates different machine learning models performances. The Supervised Learning algorithms are used commonly in Machine Learning which means that the training is done using labelled data, belonging to a particular classification. Such classification methods like Random Forest, Decision Tree, K-Nearest Neighbour, XGBoost algorithm, Naïve Bayes, and Support Vector Machine will be used to assess the cardiovascular disease by Machine Learning.

Objectives: In the general population, cadmium seems to be responsible for hypertension, atherosclerosis and an increase in acute coronary events. Therefore, the purpose of this meta-analysis was to analyze controlled studies conducted on cadmium and arterial pressure in occupationally-exposed workers. Materials and Methods: After analyzing all the relevant articles found in the literature, 6 publications were selected. Results: A higher prevalence of hypertension and higher values of systolic and diastolic blood pressure were recorded in the exposed subjects. Conclusions: Cadmium in occupationallyexposed individuals appears to induce an increase in systolic and diastolic blood pressure and an increase in the prevalence of hypertension.

Introduction. Education is a key tool in the prevention of cardiovascular disease (CVD). Education programmes require monitoring of their effectiveness. Objectives. 1) to introduce postal screening for the assessment of knowledge on CVD risk factors (RFs) for the Polish population, 2) to assess this knowledge in adult residents of Małopolska Voivodeship, and 3) to assess whether knowledge on RFs is related to age, gender, place of residence, level of education and family history of CVD. Materials and method. Anonymous questionnaires were posted to a random sample of 5,000 residents of Małopolska Voivodeship in Poland. Results were presented as proportions of participants who listed RFs correctly. A series of multiple logistic regression models was used to assess the associations of knowledge on RFs with the potential determinants. Results. 1,126 completed questionnaires were returned. Over 35% of respondents could not list a single RF and 14 % listed only 1–2 RFs. About 40% named 3–5 and only 12% listed 6 or more RFs. About a half of the respondents listed incorrectly from 1–8 characteristics as being associated with higher risk of CVD. In the multivariate analysis, knowledge on RFs was not significantly associated with age. Level of education was the strongest determinant of knowledge. Male rural and small town residents had less knowledge, whereas women with a family history of CVD had more knowledge on some CVD RFs. Conclusions. Using a postal questionnaire for the assessment of knowledge of CVD RFs in the population of Małopolska Voivodeship appeared to have serious limitations due to low participation in the study. Despite this, the results of the study indicate that knowledge on CVD RFs is insufficient. Female gender and higher education were related to more prevalent knowledge on RFs. Family history of CVD was related to better knowledge in women only. Male residents of rural areas and small towns had slightly less knowledge on CVD RFs.

Background: Cardiovascular diseases with the accompanied elevated level of total cholesterol have been a major problem in society for the last several decades. They belong to the diseases of civilization which affect people at an increasingly young age. For this reason, our aim was to investigate whether the concentrations of selected steroids are related to elevated total cholesterol in people without diagnosed cardiovascular diseases. Material and methods: The study involved 71 plasma samples. 19 of them were obtained from women and men with elevated cholesterol levels, whereas 52 samples were from healthy volunteers (control group). Liquid chromatography coupled with mass spectrometry (LC-MS) validated method followed by solid-phase extraction procedure were applied to measure the plasma concentrations of the three endogenous glucocorticosteroids (cortisol, corticosterone and cortisone). Results: Statistically significant differences between the concentration of cortisol were noted among healthy women and women with elevated cholesterol. The measured concentrations of cortisol in healthy women and men are comparable, 111.19 ng/mL and 112.22 ng/mL. respectively. However, the concentrations of cortisol in the elevated cholesterol group was significantly lower among women with elevated cholesterol than in healthy women (74.13 ng/mL and 111.19 ng/mL respectively). The concentration of cortisol for men with elevated cholesterol was 38.60 ng/mL. Hence, it is much higher than in women with elevated cholesterol and higher than in the case of healthy men. Distinctive changes can be observed also for corticosterone measured for both women and men. Conclusions: The observed differences on the level of steroids between healthy control group and patients with elevated cholesterol can be considered as worthy of further investigation from both biochemical as well as clinical points of view.

Modern antiandrogens: abiraterone acetate (inhibitor of CYP17 cytochrome) and enzalutamide (irreversible inhibitor of androgen receptor) are the drugs that are increasingly often administered in treatment of castration-resistant prostate cancer. Despite their clinical efficacy, especially in terms of prolonged survival and improved quality of life of patients, they pose of problem for a practicing oncologist such as possible cardiovascular complications (particularly arterial hypertension), which may lead to the cessation of this form of therapy. This article provides a brief overview of the mechanisms responsible for the above complications, including practical recommendations in the event such complications arise. A simple scheme of action for control of cardiovascular risk factors has been presented, which can improve the prognosis in this population of patients.

The use of tyrosine kinase inhibitors (TKIs) in the treatment of chronic myeloid leukemia has significantly improved the prognosis and outcomes for most patients. Clinical trials indicate that long-term CML therapy requires the introduction of second- or third-generation inhibitors in approximately 40–50% of cases. Effective in the case of imatinib resistance or intolerance, second generation TKI’s can also be used as a first-line treatment, leading to a faster, and deeper molecular response. TKIs, however, have also been observed to cause significant late adverse effects, including cardiovascular complications, which may raise certain safety concerns. The excellent treatment outcomes achieved with tyrosine kinase inhibitors have led to a gradual increase in the number and age of treated patients, and the associated higher incidence and severity of age-related co-morbidities such as diabetes, hypercholesterolemia, atherosclerosis, ischemic heart disease, hypertension, and congestive heart failure, which raise the risk of treatment-related cardiovascular complications. The article discusses the effects of individual TKIs on the pathogenesis of cardiovascular complications and presents the results of clinical trials that studied their impact on the incidence of such events.

Isoprostanes are a recently discovered group of prostaglandin isomers. Results of previous studies suggest that they can be used as oxidative stress markers, because in a number of cardiovascular, pulmonary and neurological diseases their levels in biological samples considerably increase. It has been found that people suffering from diabetes, obesity, homozygous familial hypercholesterolemia, moderate hypercholesterolemia, and smokers have higher levels of isoprostanes in urine. The same refers to patients with asthma, Alzheimer disease and Down syndrome. This paper reviews the results of relevant studies.

Opis:
Cardiovascular diseases are the most common causes of high mortality and invalidity in developed countries. That is why there is a great interest in preventive therapeutic methods that help to modify neural control of cardiovascular system by means of biological and behavioural practices. Behavioural medicine comes from the biopsychosocial model. The understanding of biopsychosocial factors helps to succeed in complex therapeutic care of cardiac patients. The aim of the presented article is to summarize theoretical knowledge in field of behavioural medicine and its possible practical use in rehabilitation of the patients with cardiovascular diseases.

We describe an emerging analysis of fatigue influence on behavioral restraint and consider its potential significance for two central concerns of contemporary restraint investigators: (1) adverse health effects of restraint, and (2) fatigue influence on self-control. Regarding health, the analysis provides a conceptual framework for anticipating restraint intensity and therefore possible risk. It also draws attention to a pathological pathway through which restraint could generate some adverse health outcomes. Regarding self-control, the analysis conveys a perspective that could markedly improve our prediction of relevant outcomes and has focused significance for crises that have been identified in relation to the influential limited resource analysis of self-control developed by Baumeister and colleagues. Crisis concerns have led some to conclude that the limited resource analysis should be discarded. The present behavioral restraint analysis goes against this grain, suggesting that the limited resource analysis might wisely be retained and afforded the chance to mature – among other things, incorporating elaborated effort components.

Wstęp. Choroby układu krążenia są najczęstszą przyczyną zgonów na świecie. Zmniejszenie poziomu tych czynników minimalizuje prawdopodobieństwo zachorowania, natomiast u chorych powoduje łagodniejszy przebieg dolegliwości i mniej powikłań (Kosobudzki, Bortkiewicz, 2012). Materiał i metody. Program profilaktyczny przeprowadzono wśród 2500 osób w wieku 40-60 lat z wysokim ryzykiem zachorowania na choroby układu krążenia. Do badań zakwalifikowano 1325 kobiet i 1175 mężczyzn. Wykorzystano kartę badania profilaktycznego składającą się z trzech etapów (pytania metryczkowe, badania obrazowe i analityczne oraz ankieta ewaluacyjna). Wyniki. U 76% osób biorących udział w projekcie potwierdzono wystąpienie zawału i udaru w rodzinie. Podejmowanie aktywności fizycznej potwierdziło jedynie 22% badanych. Niestety, aż 88,39% badanych mieszkańców powiatu bialskiego posiadało wskaźnik BMI mieszczący się w przedziale równym bądź przewyższający wartość 25. Badania analityczne wśród 88% badanych uznane zostały przez lekarzy za nieprawidłowe. Stosując algorytm SCORE blisko 60% osób poddanych badaniu zakwalifikowano do kategorii umiarkowanego ryzyka sercowo-naczyniowego. Wnioski. Zbadano, iż blisko 60% osób biorących udział w projekcie znajdowało się w umiarkowanej grupie ryzyka zgonu z powodu incydentu sercowo-naczyniowego. Najczęstszym zaleceniem lekarzy pracujących w poradniach POZ była kontrola w poradni kardiologicznej. Zalecenie to dotyczyło ponad 27% ogółu badanych.
Introduction. Cardiovascular diseases are the most common cause of death in the world. The reduction of those risk factors minimizes the probability of disease, while in ill patients it causes a milder course of ailments and fewer complications (Kosobudzki, Bortkiewicz, 2012). Material and methods. The preventive program was conducted among 2500 people aged 40- 60 with a high risk of developing cardiovascular disease. 1325 women and 1175 men were qualified for the study. A prophylactic examination card consisting of three stages was used (metrics questions, image and analytical tests, and an evaluation questionnaire). Results. 76% of people participating in the project confirmed the occurrence of heart attack and stroke in the family. Physical activity was confirmed only by 22% of respondents. Unfortunately, as much as 88.39% of the surveyed inhabitants of the Biała Podlaska County had BMI index in the range equal to or above the value of 25. Analytical studies among 88% of respondents were considered as being incorrect by the doctors. Using the SCORE algorithm, nearly 60% of subjects were classified to the category of moderate cardiovascular risk. Conclusions. It was examined that nearly 60% of people participating in the project were in a group of moderate risk of death due to a cardiovascular incident. The control in cardiology clinics was the most frequent recommendation from doctors working in outpatient counselling centres. This recommendation related to more than 27% of all respondents.

Introduction. The biggest threat to life are cardiovascular diseases which are a serious problem in Poland, Europe, and worldwide. Therefore, it has become important to have proper health behaviours which significantly eliminate the occurrence and development of cardiovascular diseases. Objective. The main aim of this study is to present factors that determine health behaviours of the 50+ population with cardiovascular diseases. Materials and metod. The study was carried out among 411 individuals aged over 50 with cardiovascular diseases. The method used was a diagnostic survey. The study involved an authors’ survey questionnaire and the Inventory of Health-Related Behaviours (IHB). A detailed statistical analysis was carried out in the R programme, version 3.5.1. Results. The most common disease in the study group was coronary heart disease – 63.75%. The majority of respondents showed (via Inventory of Health-Related Behaviour [IHB]) that the level of their health behaviours was moderate – 41.12%; the highest level of health behaviours in the study group were related to health practices, while a slightly lower level was observed in the area of positive mental attitude. Statistical significance was also found between the level of intensity of health behaviours and age, gender, BMI, place of residence, education, professional activity and marital status of the respondents. Conclusions. Anti-health behaviours were predominant among the respondents; significant demographic and social factors determining health behaviour were determined, such as age and gender; a high level of health behaviours was found in the group of women with higher education and correct body mass, living in cities and married.

Cardiovascular disease is at the forefront of global health issues and contributes to myocardial infarction, stroke, and even death. Lyme disease (LD), spread by ticks of the genus Ixodes, is a contributing factor to the potential development of abnormalities in the heart. The bacterial agents Mycoplasma pneumoniae, Chlamydia pneumoniae, and Borrelia burgdorferi are known to cause a variety of infections and we speculate that infection with Borrelia is a contributing factor to cardiac abnormalities, especially Lyme carditis (LC). Patients who demonstrate undiagnosed cardiac abnormalities should be monitored for potential infection with Lyme disease and, consequently LC. Lyme carditis affects up to 10% of patients that have been diagnosed with LD and additionally, it indiscriminately manifests across a range of ages, from the pediatric populace to the elderly. Given the grave cardiac abnormalities that can arise from LC and the propensity for misdiagnosis, it is critical that medical professionals be aware of the cardiovascular signs and symptoms of Atrioventricular Heart Block (AHB). AHB can lead to third degree heart block in patients and potentially lead to death if left not properly diagnosed and treated. Essential treatments are readily available for Lyme carditis, which include a range of antibiotics and surgical procedures. In this review, we highlight not only the true nature of Lyme disease, but more importantly, the positive correlation between LD and a variety of undiagnosed heart complications.

Opis:
Endothelial progenitor cells (EPCs) represent a small population of blood cells (5-40 cells/mm3), with an ability to differentiate into endothelial cells that form the lining of the blood vessels and contribute to postnatal angiogenesis. Abundant evidence shows that recruitment of EPCs from the bone marrow, the monocyte/macrophage lineage and the organs facilitate the endothelial regeneration and repair. Changes in the number of EPCs were observed in both, chronic kidney and cardiovascular diseases. Thus, these cells were tested for usage in diagnosis and therapy. In this paper, we review the current knowledge on the EPC biology and contribution of these cells to the kidney and cardiovascular diseases.

The efficiency in cross-country skiing commit considerable physiological challenges posed by the combined upper- and lower-body effort of varying intensity and duration. Extremely important for the effectiveness of the work is the ability to draw energy from the processes of aerobic and anaerobic metabolism maintenance work as low as possible during prolonged exercise. The workout should increase maximal aerobic power and the metabolic potential of the muscles. High-intensity exercise is the most important to initiate this change, while low- and moderate-intensity effort allow appropriate muscle adaptation. Monitoring and evaluation of these physiological changes is mandatory in the process of training, in amateur cross-country skier too. Amateur marathons are becoming increasingly popular. This has been an impulse for analyzing the results obtained by the amateur athlete during the preparatory period. The purpose of this retrospective case study was to describe changes in cardiovascular capacity for three months period of an amateur cross-country skier (age = 39, BMI = 20,9). This study based on analysis of source materials, obtained from conducted individually by the athlete registration process of training, the results from test to refuse and the results of measurements of body composition performed in the laboratory functional studies for three months period (preparation period). The results show inter alia increase in maximal oxygen uptake, lactate threshold shift towards higher loads and decrease body weight. This parameter was significantly lower than the performance in highly qualified competitors, but individual changes between the first measurement and the second one was similar. This review summarizes research on amateur cross-country skier. We have the chance to improve our understanding of the impact of various exercises on the human body by analyzing the results from workout monitoring of endurance athletes.

The purpose of physical training is to achieve the maximum capacity of the body. These changes are caused by numerous adaptive mechanisms taking place in the circulatory-respiratory, muscular, nervous, hormonal, and immunological systems, as well as in the mass and composition of the body. They are most visible in athletes of endurance disciplines such as cross-country skiing, skiing, long-distance running, roller skiing, triathlon or cycling. These disciplines are characterized by long-term effort and the highest intensity. This applies to both athletes and amateurs. The aim of the study was to assess the impact of physical activity during the period of main preparations on the circulatory and respiratory parameters of amateurs practicing cross-country skiing. This study was a pilot, and show the results of the assessment of the case. The purpose of this retrospective case study was to describe changes in cardiovascular endurance within three months of a male amateur cross country skier. This study was based on the analysis of training registration, ergospirometry test (treadmill test) results and body composition measurements. The results showed an increase in maximum oxygen uptake, a shift of the lactate threshold towards higher loads and a reduction in body weight. This parameter was lower than the performance of highly qualified competitors, but the individual changes between the first measurement and the second were similar. Monitoring and evaluation of these changes are mandatory in the training process, also in amateur sport. It helps to counteract states of fatigue, overtraining and designing pro-health training loads. In addition, they can provide protection for the health of those who make the marathon and ultra- marathon efforts. The popularity of ski marathons is growing. Assessment of participants' exercise capacity can be used to plan training and comparison with professionals.

Introduction and objective. Cardiovascular diseases are the leading cause of death in Europe and worldwide. One of the most important risk factors for atherosclerosis are lipid metabolism disorders, in particular hipercholesterolaemia. The aim of the study was to determine the correlation between gut microbiota composition and atherosclerosis risk factors, so in order that it might be used as a biomarker for coronary artery disease diagnosis. Material and method. The study involved middle-aged men in eastern Poland with central obesity (n=20), subjects with atherosclerosis (n=15) and those with no cardiovascular diseases (n=5). The gut microbiota composition was determined using tag-encoded 16S rRNA gene using Illuminal MiSeq. Data were analyzed with the use of t-test. Results. Firmicutes (49.26%) and Bacteroidetes (44.46%) were the dominant Phyla in the middle-aged men in eastern Poland. Subjects with improper levels of total cholesterol were enriched in Prevotella (p=0.03) and decreased level of Clostridium (p=0.02). They also showed a falling tendency in Faecalibacterium (p=0.07). An upward trend was observed in Prevotella (p=0.07) in subjects with improper LDL-C values. Conclusions. The study showed that intestinal microbiome is likely to play a role in the pathogenesis of atherosclerosis through its role in lipid metabolism. Bacterial genera of particular importance were Prevotella, Bacteroides, Clostridium, Faecalibacterium. However, further studies involving larger groups of subjects are required to confirm these observations.

Galanin is a 29 amino acid peptide acting as a neuromodulator in the central and peripheral nervous systems. It infl uences a variety of the central nervous system, functions, including food intake regulation, learning and memory, neuroendocrine control and pain transmission. In the present paper, we review the central and peripheral infl uences of galanin on the cardiovascular system. We discuss mechanisms involved in galanin action as well as interactions between galanin and other neuronal systems in the cardiovascular control. In conclusion, galanin, together with other neurotransmitters/neuromodulators, infl uences the central and peripheral przedzwocardiovascular regulation. Moreover, we suggest its involvement in the activation of compensatory mechanisms in the state of disturbed circulatory homeostasis.
Galanina jest 29-aminokwasowym peptydem działającym jako neuromodulator w ośrodkowym i obwodowym układzie nerwowym. Wpływa na wiele czynności ośrodkowego układu nerwowego, włączając regulację ośrodka głodu i sytości, uczenie się i zapamiętywanie, regulację neuroendokrynną i przewodnictwo impulsów z nocyceptorów. W pracy dokonano przeglądu najnowszych doniesień dotyczących ośrodkowego i obwodowego wpływu galaniny na układ krążenia. Omówiono mechanizmy działania galaniny oraz interakcje między galaniną i innymi układami neuronalnymi w regulacji układu krążenia. Przedstawione informacje wskazują, że galanina wraz z innymi neuroprzekaźnikami/neuromodulatorami wpływa na ośrodkową i obwodową regulację układu krążenia. Najnowsze badania sugerują możliwy udział galaniny w aktywacji mechanizmów kompensacyjnych w stanie stresu, w tym podczas zaburzenia homeostazy krążeniowej.

Cardiovascular disorders (CVDs) constitute a major burden for health of working populations throughout the world with as much as 50% of all causes of death and at least 25% of work disability. There are some changes in CVD risk factors among occupational classes. This is mainly due to the new types of work-related causes of morbidity associated with the recent developments in global work life, particularly in the industrialized countries. Meanwhile, in the developing countries or those in transition (e.g., in Eastern Europe), CVD mortality is increasing due to major socioeconomic changes, the demographic transition and rapid industrialisation and urbanisation, all leading to growing challenges to cardiovascular health. Better control of known risk factors (i.e., smoking, obesity, physical inactivity, high cholesterol, high blood pressure, and high blood glucose) is effective to prevent CVD incidence. But the expected improvement has not been achieved. The obstacles of achieving such impact are due to lack of awareness, lack of policies and their implementation into practice and shortage of infrastructures and human resources. These are needed for wide-scale and long-term programme implementation. Considering the WHO Global Strategy on Occupational Health for All, the WHO Global Action Plan on Workers’ Health, the WHO Programme on Prevention of Non-communicable Diseases and the ILO Decent Work agenda, the 6th ICOH International Conference on Work Environment and Cardiovascular Diseases adopted the Tokyo Declaration.

Objectives The aim of the study was to assess health status of regular and part-time mines rescue brigadesmen. Material and Methods A group of 685 mines rescue brigadesmen was examined within the preventive testing – a basic internal, biochemistry and anthropometric examination, physical fitness testing. Results The average age of the subjects was 41.96±7.18 years, the average exposure in mining was 20±8.1 years, out of that 11.95±7.85 years as mines rescue brigadesmen. Elevated levels of total serum cholesterol (T-CH) and low-density lipoprotein cholesterol (LDL-CH) were found in over 1/2 of the subjects. Systolic hypertension (systolic blood pressure (SBP) ≥ 140 mm Hg) was confirmed in 34%, overweight (body mass index (BMI) ≥ 25) in 62.3% and obesity (BMI ≥ 30) in 20.4% of the examined mines rescue brigadesmen. The metabolic syndrome was found in 15.2% of persons. The highest physical fitness was found in mines rescue brigadesmen and the lowest in mine officers. Limit values of maximum oxygen uptake ($\text{VO}_\text{2 max}$/kg) determined by the management of the mine rescue station were not reached by every 3rd of all mines rescue brigadesmen. Compared with the control group of the Czech and Slovak population, the rescuers are taller, have greater BMI, higher percentage of body fat in all age categories and proportionally to that they achieve a higher maximum minute oxygen uptake; however, in relative values per kg of body weight their physical fitness is practically the same as that of the controls. Conclusions The prevalence of risk factors of cardiovascular diseases and $\text{VO}_\text{2 max}$/kg in the group of the mines rescue brigadesmen is comparable with that in the general untrained Czech population.

Objective: The aim of this study was to establish the prevalence of cardiovascular disease (CVD) risk factors amongst South African colliery executives presenting normal blood pressure (normotensive), pre-hypertension and hypertension. Materials and Methods: Selected CVD risk factors of a non-randomized, available population of 143 Caucasian male executives from fi ve South African collieries situated in Mpumalanga and Gauteng Provinces were recorded. Results: Executives with pre-hypertension and hypertension exhibited a higher prevalence of CVD risk factors, compared to the persons with normal blood pressure levels. The percentage of executives with CVD risk factors, with the exception of BMI, was greater amongst those with pre-hypertension than those with hypertension. Conclusion: The current study showed that a workplace CVD risk screening process was effective in identifying the relatively high prevalence of CVD risk factors amongst SA colliery executives. In addition, out of all the studied risk factors, undesirable body composition (BMI, WHR and fat %) exhibited the highest prevalence amongst pre-hypertensive and hypertensive SA colliery executives.

The term “functional food” refers to modified food products that claim to provide an additional function besides basic nutrition needs. The consumption of functional food is known to exert a positive impact on health and to prevent the occurrence of pathological conditions, such as cardiovascular diseases, some types of cancer, and obesity. Functional food products should resemble conventional food in terms of appearance and taste. The goal is usually achieved by adding active ingredients to the traditional food products (e.g., phytosterols/stanols are added to margarine, dairy, and cereal products), removing or limiting the concentration of potentially harmful agents, or by agricultural and genetic modifications of already existing edible plants and animals (e.g., feeding hens on algae or fish in order to obtain n-3 PUFAs-enriched eggs, and inducing genetic and/or nutritional changes during animal production to obtain meat with lower cholesterol levels). Well-designed intervention trials are scarce in this field, and more effort should be directed toward conclusively proving the role of functional food in disease prevention and health improvement among the population. These associated benefits and the advances in food processing industry should stimulate the development of products that would match the requirements of a healthy diet, simultaneously reducing the risk of chronic diseases. The aim of the present review was to present the examples of functional foods that are essential for the prevention of obesity and cardiovascular disease, and thereby report on their putative mechanisms of action, health-promoting effects, and limitations by conducting various intervention studies.

Purpose: To determine effect of working in the night shift system on nutritional status, anthropometric measurements, and risk of cardiovascular disease of health staff. Materials and Methods: This cross-sectional study was conducted between August-November 2017 at Samsun Ondokuz Mayıs University among 111 health staff. Data was collected with a questionnaire form including questions about demographic characteristics, nutritional status and anthropometric measurements. SPSS 21.0 statistical package program was used for statistical analysis and p<0.05 was considered statistically significant. Results: Based on waist circumference, 14.3% of men and 31.1% of women were at high risk, and based on waist/hip ratio, 4.8% of men and 33.3% of women were at risk of cardiovascular diseases. It has been shown that eating patterns are disrupted during shifts (89.2%), and 73.9% of participants cannot eat because their meals cool down during shifts. The difference in the numbers of main and snack meals consumed by health staff during the night shift was statistically significant (p<0.05). Body mass index, waist circumference, and hip circumference measurements decreased with increasing shift time. However, body mass index, waist circumference, and hip circumference increased as time spent in the profession increased. Conclusion: It was seen that night shift health staff are at risk of cardiovascular disease due to insufficient and unhealthy nutrition. It is clear that nutrition education programs are required for health staff working night shifts to reduce excess weight and obesity in this population.

This study aims to determine the level of awareness and meeting recommendations regarding their physical activity in the selected disease risk groups in eastern Slovakia. Methods. The study comprised 893 participating patients (353 males and 540 females). The basic condition to fulfil for participating in the research was the occurrence of one or more diagnoses out of three underlying chronic diseases that do not prevent PA. For data collection, we used a nonstandardized questionnaire which was part of the questionnaire battery explicitly designed for this research. This study presents a selection of questions focusing on patients awareness concerning PA. Results. Within all three groups of chronic diseases, the patients acknowledged that they obtained only general information on the importance of PA in the treatment of their conditions. The patients received information on physical activity in their neighbourhood predominantly from the media and further on from the family members and acquaintances or their doctors or medical staff. The difference between the particular disease groups in terms of the acquired information is minimal. Statistically significant relation between the doctors recommendations and minimum requirements for the treatment of disease was only found in the group of oncology patients. Conclusion. Regular physical activity represents essential advantages for the health of patients suffering from chronic diseases. With this in mind, the doctor, being a trustworthy person to the patient, should be able to inform the patients on the frequency, intensity and the most suitable PA concerning their diagnosis and severity of their medical condition.

INTRODUCTION Amitriptyline, a tricyclic antidepressant used in the treatment of many psychiatric, neurological and gastrointestinal diseases, affects cardiovascular system functions. The aim of the present study was to examine the influence of chronically administered amitriptyline on cardiovascular responsiveness to a histamine vasodilatatory stimulus. MATERIAL AND METHODS The studies were carried out on male Wistar rats pre-treated with amitriptyline (2.7 and 5.4 mg/kg, subcutaneously for 7 days). RESULTS Histamine (5.0 and 10.0 μg/kg) administered intravenously in a bolus injection evoked dose-dependent decreases in systolic and diastolic blood pressure as well as in renal (RBF) and skeletal muscle microcirculatory flow (SMMF), with no significant changes in heart rate (HR) in the control, saline pre-treated group. Amitriptyline prevented histamine-evoked decreases in blood pressure (2.7 and 5.4 mg/kg) and produced increases in RBF and SMMF (5.4 mg/kg), but did not influence HR dynamics. CONCLUSION We demonstrate for the first time that chronically administered amitriptyline to rats influences the cardiovascular responsiveness to histamine with an effect on peripheral perfusion.
WSTĘP Amitryptylina to trójpierścieniowy lek przeciwdepresyjny stosowany w leczeniu wielu schorzeń psychicznych, neurologicznych oraz przewodu pokarmowego, wpływający także na czynność układu krążenia. Celem pracy było zbadanie wpływu przewlekle stosowanej amitryptyliny na reaktywność układu krążenia na działanie bodźca hipotensyjnego – histaminy. MATERIAŁ I METODY Badania przeprowadzono na samcach szczurów szczepu Wistar, którym podawano amitryptylinę (2,7 i 5,4 mg/kg) podskórnie przez 7 dni. WYNIKI Histamina (5,0 i 10,0 μg/kg) podawana dożylnie (bolus) w grupie kontrolnej wywoływała zależne od dawki spadki ciśnienia tętniczego skurczowego i rozkurczowego oraz przepływu w tętnicy nerkowej (RBF) i w mikrokrążeniu mięśnia szkieletowego (SMMF), nie wpływając na częstość rytmu serca (HR). Amitryptylina zapobiegała wywoływanym przez histaminę spadkom ciśnienia tętniczego krwi (2,7 i 5,4 mg/kg), powodowała wzrost RBF i SMMF (5,4 mg/kg), nie wpływając na HR. WNIOSEK Przewlekle stosowana amitryptylina wpływa na reaktywność układu krążenia i przepływy obwodowe w odpowiedzi na dożylne podanie histaminy u szczurów.

Objective: One of the practical models for assessment of stressful working conditions due to job strain is "job demand and control" or Karasek's job strain model. This model explains how adverse physical and psychological effects including cardiovascular disease risk factors can be established due to high work demand. The aim was to investigate how certain cardiovascular risk factors including body mass index (BMI), heart rate, blood pressure, serum total cholesterol levels, and cigarette smoking are associated with job demand and control in workers. Materials and Methods: In this cohort study, 500 subjects completed "job demand and control" questionnaires. Factor analysis method was used in order to specify the most important "job demand and control" questions. Health check-up records of the workers were applied to extract data about cardiovascular disease risk factors. Ultimately, hypothesis testing, based on Eta, was used to assess the relationship between separated working groups and cardiovascular risk factors (hypertension and serum total cholesterol level). Results: A significant relationship was found between the job demand-control model and cardiovascular risk factors. In terms of chisquared test results, the highest value was assessed for heart rate (Chi² = 145.078). The corresponding results for smoking and BMI were Chi² = 85.652 and Chi² = 30.941, respectively. Subsequently, Eta result for total cholesterol was 0.469, followed by hypertension equaling 0.684. Moreover, there was a significant difference between cardiovascular risk factors and job demand-control profiles among different working groups including the operational group, repairing group and servicing group. Conclusion: Job control and demand are significantly related to heart disease risk factors including hypertension, hyperlipidemia, and cigarette smoking.

Atherosclerosis remains the main cause of death worldwide. Most important issues concerning atherosclerosis are hemodynamics and how it affects plaque prevalence and distribution, as well as the symmetry and asymmetry of vasculature and plaques. To present the symmetry in the vascular system an analysis of PubMed and MEDLINE databases was performed. As of February 21, 2023, the results were as follows: for “symmetry” AND “atherosclerosis” there were 47 results; for “symmetry” AND “atherosclerotic lesions” – 20 results; for “symmetry” AND “artery stenosis” – 82 results; for “asymmetry” AND “atherosclerosis” – 87 results. Not without meaning are preventive measures. In the light of the Fourth Industrial Revolution artificial intelligence (AI) solutions help to develop new tools outperforming already existing cardiovascular risk scales. The aim of this paper is to present a current view on symmetry within vasculature and atherosclerosis as well as present a new approach to assess individuals’ cardiovascular risk in accordance with precision medicine assumptions. Symmetry and asymmetry within the human vascular system play a crucial role in understanding of arterial diseases, including atherosclerosis. Moreover, it is unavoidable to use AI in cardiovascular risk stratification.

The study determined physiological and psychophysical responses to fire fighters’ typical activities. Selfreported assessment of the most demanding fire fighting/rescue tasks were collected with a questionnaire. Then 19 voluntary fire fighters performed 3 simulated fire fighting/rescue tasks in protective clothing. Heart rate (HR), blood pressure and energy expenditure were measured; the rating of perceived exertion (RPE) was assessed with the Borg scale. The questionnaires showed that carrying out victims, fire suppression and resuscitation were classified as heavy load; climbing stairs with a hose as moderate load. According to RPE the subjects considered their effort during ladder climbing fairly light and only somewhat harder during stair climbing and carrying out injured people. The study demonstrated that typical fire fighting/rescue tasks were associated with high energy expenditure and imposed considerable cardiovascular stress. The Borg scale appeared not suitable for assessing perceived exertion in fire fighters during simulated tasks.

This study aimed to assess the incidence of coronary heart disease (CHD) risk factors and cardiovascular risk in physical workers and managers in Poland. There were 232 male subjects: 123 managers (48.9 ± 11.2 years old) and 109 physical workers (37.5 ± 11.1 years old). The family history of CHD was recorded and anthropometric and biochemical indices, i.e., body mass index, visceral obesity index, blood pressure, glucose, total cholesterol, fibrinogen, HDL (high density lipoprotein), LDL (low density lipoprotein) and triglycerides were measured. Cardiovascular risk was assessed with the Systematic Coronary Risk Evaluation (SCORE) table system. The factors that turned out to be the most common in the managers were obesity, hypertension, and elevated levels of blood glucose and LDL, whereas cigarette smoking, premature CHD in the family and a high level of fibrinogen were more common in physical workers. Very high cardiovascular risk was found in 35% of managers and in 16% of physical workers.

Introduction. Dietary pattern analysis is used to describe the dietary habits of a selected population. In many studies, dietary patterns (DPs) have been associated with risk factors for cardiovascular disease (CVD). The aim of the study was to assess the association between dietary patterns identified in the population of Lower Silesia, Poland, with anthropometric and biochemical risk factors for CVD. Materials and method. The study group included 2,025 participants of the Prospective Urban Rural Epidemiological (PURE) Study. Dietary intake was evaluated based on data from the Food Frequency Questionnaire (FFQ). Dietary patterns were derived using principal component analysis (PCA). The relationship between DPs and body mass index (BMI), waist circumference, waist-hip ratio, blood pressure, total cholesterol, HDL cholesterol, LDL cholesterol, triglycerides and fasting glucose level, was assessed. Results. Three dietary patterns identified in the study explained 35.6% of total variance. The ‘fruit, vegetables & dairy’ DP, characterized by a high intake of vegetables, fruits, nuts, seeds, raisins, milk and low-fat dairy, was associated with improved lipid profile and anthropometric measures, lower diastolic blood pressure and lower fasting glucose concentration. ‘Traditional’ and ‘fat & sugar’ DPs were unfavourably associated with most of the risk factors for CVD presented in this study. Conclusions. Dietary patterns identified in this study were differently related to selected anthropometric and biochemical risk factors for CVD. ‘Fruit, vegetables & dairy’ DP was favourably associated with the biochemical and anthropometric CVD risk factors, and was characterized by higher nutritional value in comparison with ‘traditional’ and ‘fat & sugar’ DPs.

Introduction and objective. Stress at the work place reduces efficiency, as well as increasing accidents and absences, which may cause various diseases, including cardiovascular diseases. The aim of the study is an analysis of the prevalence and causes of stress in intellectual work, and its correlation with the prevalence of cardiovascular diseases in women at non-mobility working age. Materials and method. The study was conducted in 2016–2017 in a group of 300 women aged 45–60 in non-manual employment. A questionnaire for subjective job evaluation was used. Logistic regression models for the occurrence of CVDs versus frequency of occurrence of individual causes of work stress among the respondents were estimated. Results. Women at non-mobility working age with non-manual employment are especially exposed to stress, half of them experience high level of stress at work, most often caused by social contacts and lack of rewards and support. Cardiovascular diseases were found in 26.5% of the women studied who were also significantly higher exposed to the occurrence of health hazards due to exposure to harmful factors, or due to an accident at work, changes in the workplace, the need to compete with others,and the need to perform the task despite the lack of appropriate material resources, compared with women without such diseases. Conclusions. The study revealed a high prevalence of occupational stress in non-manual employment, indicated its main causes that correlated with the alence of cardiovascular diseases. Actions are needed to reduce the level of stress in the work of women at non-mobility working age, to maintain their work ability and quality of life.

Introduction. There is a strong association between the occurrence of cardiovascular disease (CVD) and low socio-economic status (SES). It is interesting to consider whether high dietary glycaemic load (GL) is also associated with low SES or demographic factors. Objective. The aim of the study is to assess the relationship between dietary GL and demographic, socio-economic and lifestyle factors in a selected population of Polish adults in Lower Silesia. Materials and method. The study group consisted of 2,025 Polish adults (aged 35–70 years), enrolled in the 1st stage of the Prospective Urban Rural Epidemiological (PURE) study. Nutritional data were collected using a food frequency questionnaire (FFQ), which was developed and validated for the Polish population in Lower Silesia. The overall GL of each diet was computed as a sum of GL values of consumed foods. Results. Significantly higher GL and GL/1,000 kcal were observed in the diets of males compared to females and in the diets of rural compared to urban inhabitants. An adverse relationship between both GL and GL/1,000 kcal and level of education was found. The percentage of females, urban inhabitants, people with university education and not married decreased with quartiles of the overall dietary GL and GL/1,000 kcal. The percentage of former smokers, people who never used alcohol and people with low and moderate physical activity decreased with quartiles of GL and GL/1,000 kcal. Lower percentage of individuals aged 44–64 years was observed with quartiles of the overall dietary GL. Conclusions. Factors such as: male gender, rural place of residence, low level of education and smoking determine the group of people that is the most exposed on the effects of improper nutrition, according to the low quality and/or high amount of carbohydrates defined by GL and GL per 1,000 kcal.

Introduction: Repetitive or long-lasting activity of stressors can cause cardiovascular disease. The purpose of this paper was to assess dependence between the level of intensity of stress at work and in everyday life and the occurrence of cardiovascular disease. Material and methods: One hundred people (50 with cardiovascular disease and 50 healthy ones) were answering the questions from the HSE survey to assess stress at work, SRRS survey to assess stressful events and Mini-COPE survey to assess coping with stress. Results: It was stated that healthy people as well as treated people suffering from cardiovascular disease experience stress at work. However, the people from the comparison group were under less strain than the people from the test group (p = 0,0044). It was also indicated that the people from the test group more often resigned from undertaking efforts to cope with a difficult situation (p = 0,0459). Conclusions: People suffering from cardiovascular disease are significantly more often under strain at work and slightly more under strain in everyday life than healthy people. Simultaneously, healthy people were coping better with stress than people with cardiovascular disease.
Wstęp: Powtarzające się lub długotrwałe działanie stresorów może powodować choroby układu krążenia. Celem pracy była ocena zależności pomiędzy poziomem natężenia stresu w pracy i w życiu codziennym a występowaniem chorób układu krążenia. Materiał i metody: Sto osób (50 z chorobami układu krążenia i 50 zdrowych) odpowiadało na pytania kwestionariusza HSE do oceny stresu w pracy, kwestionariusza SRRS do oceny wydarzeń stresujących, kwestionariusza Mini-COPE do oceny radzenia sobie ze stresem. Wyniki: Stwierdzono, że osoby zdrowe jak i osoby badane, u których występują choroby układu krążenia doświadczają stresu w pracy, jednak mniejsze narażenie na stres wykazały osoby z grupy porównawczej niż osoby z grupy zasadniczej (p = 0,0044). Wykazano również, iż osoby z grupy zasadniczej znacznie częściej rezygnowały z prób poradzenia sobie z trudną sytuacją (p = 0,0459). Wnioski: Osoby u których występują choroby układu krążenia były istotnie częściej narażone na stres w pracy oraz nieznacznie częściej narażone były na stres w życiu codziennym aniżeli osoby zdrowe. Jednocześnie, osoby zdrowe lepiej radziły sobie ze stresem niż osoby z chorobami układu krążenia.

Remote patient monitoring (telemonitoring) has the potential to bring about reductions in the costs associated with caring for patients equipped with cardio-active implants. Partial replacement of classic very resource absorbance follow ups of the devices held in specialized clinics will reduce costs for patients and the healthcare system. Using BIOTRONIK Home Monitoring ® system can easily detect early changes in the patient's condition associated with atrial arrhythmias, or heart failure and technical problems as compared to the conventional model of periodic visits to the clinic. This ability to early detection may significantly reduce the incidence of atrial arrhythmias and heart failure and related hospitalizations, which translates into significant cost savings.

Epidemiological studies clearly indicate that both long- and short-term exposure to several environmental air pollutants cause significant increase in the risk of cardiovascular events, the observed strong relation between particulate matter air level and cardiovascular diseases may be explained by indirect or direct influence of these particles on different biological processes involved in disease development. Improvement of air quality standards and lowering of particulate matter exposure can significantly diminish cardiovascular disease risk and improve public health status.
Badania epidemiologiczne wskazuję, że zarówno długotrwała jak i krótkotrwała ekspozycja na wysokie stężenia różnego typu zanieczyszczeń powietrza powoduje istotny wzrost incydentów klinicznych związanych z chorobami układu sercowo-naczyniowego. U podstaw tego związku leży bezpośredni oraz pośredni wpływ cząstek tworzących te zanieczyszczenia na procesy odgrywające kluczową rolę w rozwoju tych chorób. Zaostrzenie standardów czystości powietrza, a w efekcie zmniejszenie narażenia na działanie zanieczyszczeń powietrza, może w istotnie obniżyć ryzyko rozwoju chorób układu sercowo-naczyniowego i poprawić stan zdrowia populacji.

In this study of the relationship between the state of health of the population and the level of infrastructure in the regional perspective Spearman coefficient was used. Factors analyzed included mortality rates due to diseases of civilization, average life expectancy in relation to, for example, the number of employed medical staff and the number of outpatient counseling. The highest correlation is observed in case of (a) infant mortality and the number of employed medical staff, (b) mortality due to external causes (including accidents) and the number of rescue teams to one resident. Thus it can be concluded that there are correlations between health and certain aspects of physical and social infrastructure.

Long-term exposure to hypercholesterolemia is the cause of atherosclerosis, which in turn causes cardiovascular and cerebrovascular events. In developed countries, including Poland, vascular diseases are the main cause of death. They affect an ever younger part of the population, including the working population. The authors address the problem of epidemiology of cardiovascular diseases, unsatisfactory detection and treatment, economic consequences for the health care system, and the possibilities of using occupational medicine services in the prevention of this health problem. Due to the fact that the early detection of diseases caused by high blood cholesterol levels is relatively low in Poland, obligatory occupational medicine examinations seem to be a key element of the second-line prevention. Therefore, it seems natural to consider the idea of extending the scope of obligatory examinations and introducing tests that allow lipid disorders to be detected at an early stage. This can contribute to a general improvement of the health of the population, and to economic benefits, such as a decrease in the costs of treatment of the disorders that have been detected too late. Broadening the scope of occupational examinations is also important from the perspective of public health and epidemiology of cardiovascular diseases, thus being an element of prevention of civilization diseases. It means improving health and building health awareness, and it should translate into regular health examinations. The performance of these examinations should result not only from the obligation, but also from the patient’s conviction about the importance of early detection of disorders, including lipid disorders, for an effective therapy. Objectives: There are few studies about the association between breathing polluted air and increased risk of cardiovascular diseases and cardiac death in the Middle East. This study aimed to investigate the relation between air pollutants and cardiovascular mortality (based on ICD-10) in Ahvaz. Material and methods: In this ecological study, the data about cardiovascular disease mortality and air pollutants from March 2008 until March 2015 was inquired from the Ahvaz City Authority and the Khuzestan Province Environmental Protection Agency. The quasi-Poisson, second degree polynomial constrained, distributed lag model; using single and cumulative lag structures, adjusted by trend, seasonality, temperature, relative humidity, weekdays and holidays was used for the data analysis purposes. Results: Findings indicated a direct significant relation between an interquartile range (IQR) increase in ozone and cardiovascular deaths among men after 3 days’ lag. There was also a significant relation between an IQR increase in particulate matter below 10 μm and cardiovascular deaths for all people, over 60 years old and under 18 years old after 3 and 13 days’ lags. There was a significant relation between an IQR increase in nitrogen dioxide and carbon monoxide, and cardiovascular deaths in the case of under 18-year-olds (in the lag 11) and over 60-year-olds (in the lag 9), respectively. We finally found a significant association between an IQR increase in sulfur dioxide and cardiovascular deaths in the case of men, under 18-year-olds and from 18- to 60-year-olds in the lag 9, 0, and 11, respectively (p-values < 0.05). Conclusions: It appears that air pollution is significantly associated with cardiovascular deaths in Ahvaz City. Int J Occup Med Environ Health 2018;31(4):459–473

Endothelial progenitor cell (EPC) dysfunction is defined as weak function and lowered number of endothelial precursors with pro-angiogenic phenotypes that are imbedded in vascular integrity maintenance, angiogenesis, and vascular reparation. There is large body of evidence that deficiency of EPC number in peripheral blood is considered as a marker of endothelial dysfunction, which is established risk factor and player in pathogenesis of cardiovascular disease. Moreover, recent clinical studies have shown that circulating EPCs had demonstrated vascular protection in several vascular diseases including pulmonary hypertension. However, the role of EPCs in pathogenesis chronic obstructive pulmonary disease bronchial asthma, emphysema appeared to be uncertain. The short communication is dedicated the controversies in abilities of EPCs to have tissue protective and play a pivotal role in nature evolution of pulmonary disease.

The aim of the work is the study of the dependence of cardiovascular system reaction to the vestibular stimulation on the peculiarities of movements in the above-mentioned kinds of sports. Vestibular reactions of 108 males were studied, 93 of which were involved in cyclic (middle and long distance jogging, cross-country skiing, swimming), situational (sports game) and difficult to coordinate movements of stereotypical sports (gymnastics). The above facts indicate that sports contribute to the adaptation to vestibular load and, as a consequence, the development of less severe autonomic reactions to vestibular stimulation. It is essential that the vestibular resistance develops not only in those sports in which the motor activity abounds in a corner acceleration but also in those species sports where such similar accelerations are minimal. The influence of the specifics of the motor activity in some sports is reflected in the value of response in tests with a change of a head position.

Recent medical advances suggest that the cellular natriuretic peptide/cGMP and NO/cGMP effector systems represent important signal transduction pathways especially in the cardiovascular system. These pathways also appear to be very interesting targets for the possible prevention of cardiovascular diseases. Exciting candidates for prevention include cGMP-dependent signaling networks initiated by natriuretic peptides (NP) and nitric oxide (NO) which are currently explored for their diagnostic and therapeutic potential. cGMP signaling contributes to the function and interaction of several vascular cell types, and its dysfunction is involved in the progression of major cardiovascular diseases such as atherosclerosis, hypertension and diabetic complications. This review will take a focussed look at key elements of the cGMP signaling cascade in vascular tissue. Recent advances in our knowledge of cGMP-dependent protein kinases (cGK, also known as PKG), the potential for assessing the functional status of cGMP signaling and the possible cross talk with insulin signaling will be reviewed.

The aim of the study is to demonstrate the relationship between volume and intensity of work that the cross country ski during the macrocycle training and supplementation, and changes in body composition. This work is based on analysis of source materials, obtained from conducted individually by the athlete registration process of training, maximal exercise test results and the results of measurements of body composition performed in the laboratory functional studies. The highest results in terms of oxygen uptake test obtained during the second measurement. VO2 max reached the 3.2 l / min and 54 mL / kg / min VO2 on the threshold LT 49 ml / kg / min,% VO2 max on the threshold LT - 90.7. The maximum values of blood lactate level in the course of further tests were progressive 8.4 mmol / l 4.33 mmol / l and 9.39 mmol / l. The fat within the macrocycle decreased by 2.6% (1.8 kg). The lowest value was recorded adipose tissue after a period of preparation of the main (measurement of 22/08/2013) and amounted to 8.2% (4.8 kg). The effect of endurance training in an increase of the ceiling of oxygen and lactate threshold shift towards higher loads. Has reduced the level of adipose tissue, which may be the result of physical exertion, but carried supplementation based on building lean body mass at the expense of body fat. This is a translation of my authorship the original source, which was publicated in an edition of the Scientific Publishing Sophia in 2015 . It aims to increase the accessibility of the text.

Introduction. Web information systems can serve as a diagnostic tool for Internet users and to support the epidemiological work of doctors and health care providers. As part of this work, a system for detecting and calculating cardiovascular risk has been created. Objectives. 1) Application of web-based risk assessment of cardiovascular death; 2) an attempt to evaluate the distribution of selected risk factors among the population of Polish Internet users; 3) implementation of the epidemiological imaging system of cardiovascular risk factors. Materials and method. The ‘Ryzyko’ programme (www.ryzyko.gumed.edu.pl.) available on the Internet. To assess cardiovascular death risk in a ten year period the algorithm of the SCORE project was used. 28,320 solutions of the algorithm have been registered. Results. Over 28,000 webpage visitors entered the required data and received the outcome. More than 71% of Internauts who entered the data received the recommendation for medical control. The result of the programme is a graphic presentation of the distribution of the calculated risk of death, based on previously gathered information given by the Internauts in particular provinces in Poland. Conclusion. Automatic monitoring of the incidence of cardiovascular risk factors in Poland provides information for epidemiological studies. The system meets the characteristics of diagnostic programmes that can assist epidemiologicbased and therapeutic decisions.

The purpose of the study was to determine the relation between overtime, job strain and life style, and cardiovascular risk (CVR) in 97 managers and 98 physical workers. CVR was measured with the Framingham method. Information about job strain, overtime, life style and extra-occupational activities was obtained with a self-administered questionnaire. The results showed that both groups had a similar, medium-level job stress. Being a manager and having extra-occupational activities (self-education) were significantly related with CVR (p = .000, p = .035, respectively), whereas other factors that were analysed (i.e., physical work and overtime) were not. The managers were older than the physical workers; that may be why the factor of being a manager was significantly related to CVR. The extra-occupational activities connected with improving workersʼ skills may play an important role in the development of workersʼ overload and an increase in CVR.

Objectives The role of leisure-time physical activity in reducing all-cause and cardiovascular mortality is well explored. The knowledge on occupational and commuting physical activity continues to be ambiguous and misleading. The aim of the study is to assess the influence of different kinds of physical activity on cardiovascular mortality risk in men. Material and Methods Data analysis on physical activity level and other selected cardiovascular risk factors acquired from 3577 men in the age between 50–80 years who participated in the National Multicenter Health Survey WOBASZ (Wieloośrodkowe Ogólnopolskie Badanie Stanu Zdrowia), Poland (2003–2005) was linked with male mortality in 2004–2009. Data about causes of deaths were obtained from the Central Statistical Office and the Population Electronic Register. Results Among males aged 50–59 years, the strongest risk factor was living in large settlements and provincial capitals as a place of residence and the most protective factor was occupational physical activity. In the age group 60–69 years and 70–80 years, the strongest protective effect was observed for leisure-time physical activity. In men aged between 70–80 years (unlike in the 50–59 years age group), the protective effect of large settlements and provincial capitals as a place of residence was noted. Conclusions Occupational physical activity significantly reduced cardiovascular mortality in men aged 50–69 years, while for leisure-time activity the positive effect was observed in age group 60–69 years and 70–80 years. On the other hand, for the inhabitants of large settlements and provincial capitals, significantly higher risk of cardiovascular mortality in the age group 50–69 years and lower risk in the age group ≥ 70 years was noted, both in comparison with smaller places of residence.

Objectives: To investigate the association between per capita pesticide consumption and infant mortality rates from CNS and CVS congenital malformations in microregions in the South and Southeast Region of Brazil. Material and Methods: An ecological study was conducted using data on pesticide expenditure in 1985 and 1996, and deaths caused by CNS and CVS malformations in infants under 1 year old in 1986-1990 and 1997-2001, respectively. Per capita pesticide consumption and infant mortality rates were calculated for each microregion. Microregions were grouped according to quintiles of pesticide consumption, taking the first quintile as reference. The association between pesticide consumption and infant mortality was examined by calculating Spearman correlation coefficients (r) and mortality rate ratios (RR), stratifying by gender and type of microregion (urban or rural). Results: Significant and positive correlations between per capita pesticide consumption and rates of mortality due to CNS and CVS defects were observed in rural but not urban microregions. In general, mortality RRs for the 2 types of malformations in rural microregions were significantly higher in each quintile of pesticide consumption compared to the lowest quintile in the 2 study periods, with elevations ranging between 10% and 30%. Likewise, mortality RRs in these microregions showed significant trends of increase across quintiles of pesticide consumption in both study periods. In urban areas, however, mortality RRs from both CNS and CVS malformations were weak and not statistically significant, and a trend of increase of mortality with increasing pesticide usage was not observed. Conclusions: The results show the relevance of pesticide exposure in rural areas with intense agricultural activity, suggesting that such prenatal exposures may be related with the occurrence of certain congenital defects.

Objectives: Cardiovascular — coronary heart disease, causing heart attack and heart failure is a leading cause of disability and death worldwide. The most important risk factors for CVD are well-established and physical activity has long been considered the cornerstone of interventions and has shown extremely important in reducing the burden of this disease. The aim of this study was to examine the association between activity and inactivity and non-lipid CVD risk factors in healthy military recruits. Materials and Methods: Selected hormones and variables such as testosterone, free testosterone, Vitamin D, homocysteine, folic acid, with the inflammatory cytokines such as hsCRP, TNF-α and IL-6 were assessed in two groups (active = 50, inactive = 50), randomly matched by a questionnaire. Relationships between some measured variables and CVD risk factors were determined using simple correlation procedures. Results: Physically active subjects had signifi cantly lower results of homocysteine (14.3±1.9 vs. 16.5±3.7, p < 0.000) and interleukin-6 (2.9.±5.3 vs. 6±8.2, p < 0.025) and tended to have a nearly signifi cantly higher free testosterone level (13.1±7.1 vs. 11±5.3, p = 0.08). Signifi cant correlations were found between BMI, WC, tHcy, and the corresponding risk factors; and also between hsCRP and IL-6, as well as TT and FT. Conclusions: Reducing the risk factors of developing CVD events such as homocysteine and interleukin-6, together with increasing free testosterone and signifi cantly preventing its progression, clearly serve to underscore the benefi cial properties of physical activity and to promote its effectiveness as a support means for healthier lifestyles in the community and, particularly, among military personnel.

Background. The appropriate nutrition is an important component of the secondary prevention of cardiovascular diseases (CVD). Objectives. The aim of the study was to investigate if the patients with cardiovascular disease were informed of the role of appropriate nutrition in prevention or received nutrition guidelines and to assess the dietary intake compared to recommendations for patients with cardiovascular disease who received or not nutrition guidelines. Material and Methods. The study was conducted among patients with cardiovascular disease (n = 127) of cardiological hospital clinic, aged 62 ± 11. The questionnaire was used to obtain personal and anthropometric details, information if patients had received nutrition guidelines. The method of 3-day food records was used for dietary assessment. Results. 20% of subjects had not received nutrition guidelines and almost 40% of subjects did not recognize the nutrition effect on cardiovascular disease development. Compared to the diets of the subjects who had not received nutrition guidelines, the diets of those who had received them were of significantly lower intake of: energy from saturated fatty acids (15%, p = 0.006), cholesterol (21%, p = 0.012) and higher intake (14-26%) of potassium (p = 0,003), sodium (p = 0.013), phosphorus (p = 0.044), magnesium (p = 0.003), iron (p = 0.005), copper (p = 0.001), zinc (p = 0.046). Among the patients who had received nutrition guidelines, percentage of the subjects whose intake of nutrients was consistent with recommendations was higher. Conclusions. Not all subjects had received nutrition guidelines. Diets of those who had received them were more balanced, but in neither group nutrition guidelines were complied with.
Wprowadzenie. Prawidłowe żywienie jest ważnym elementem w ochronie wtórnej u pacjentów z chorobami układu sercowo-naczyniowego (CVD). Cel. Celem badania była ocena czy pacjenci ze zdiagnozowanymi chorobami układu sercowo-naczyniowego zostali poinformowani o roli odżywiania podczas leczenia oraz czy otrzymali zalecenia żywieniowe w ramach profilaktyki wtórnej, jak też ocena porównawcza sposobu żywienia osób, które otrzymały lub nie zalecenia żywieniowe. Materiał i metody. Badanie przeprowadzono wśród 127 pacjentów kliniki kardiologicznej jednego z warszawskich szpitali, pacjenci byli w wieku 62 ± 11 lat. Informacje nt. danych socjo-demograficznych i antropometrycznych oraz otrzymania zaleceń żywieniowych zostały zebrane za pomocą autorskiego kwestionariusza. Badane osoby wypełniły również kwestionariusz 3-dniowego bieżącego notowania spożycia żywności w celu oceny sposobu żywienia. Wyniki. Dwadzieścia procent osób nie otrzymało zaleceń żywieniowych, a prawie 40% osób nie dostrzegło wpływu żywienia na rozwój choroby. Odnotowano statystycznie istotne różnice w sposobie żywienia pomiędzy osobami, które uzyskały zalecenia żywieniowe a osobami, które takich zaleceń nie otrzymały, tj. niższy udział energii z nasyconych kwasów tłuszczowych (SFA) o 15%, niższe o 21% średnie spożycie cholesterolu oraz wyższe o 14-26% spożycie składników mineralnych, tj. potasu, sodu, fosforu, magnezu, żelaza, miedzi i cynku. Ponadto grupa osób, która otrzymała zalecenia żywieniowe charakteryzowała się lepszą realizacją norm na składniki żywieniowe i większą prawidłowością sposobu żywienia. Wnioski. Pomimo kluczowej roli, jaką odgrywa żywienie w profilaktyce i leczeniu chorób układu sercowo-naczyniowego, nie wszystkie badane osoby otrzymały wskazówki jak powinny się odżywiać. Osoby, które otrzymały zalecenia żywieniowe odżywiały się lepiej od tych, którym takich zaleceń nie przekazano, jednak ich sposób żywienia również odbiegał od norm i zaleceń.

All living organisms conduct protein synthesis with a high degree of accuracy maintained in the transmission and flow of information from a gene to protein product. One crucial 'quality control' point in maintaining a high level of accuracy is the selectivity by which aminoacyl-tRNA synthetases furnish correctly activated amino acids, attached to tRNA species, as the building blocks for growing protein chains. When differences in binding energies of amino acids to an aminoacyl-tRNA synthetase are inadequate, editing is used as a major determinant of enzyme selectivity. Some incorrect amino acids are edited at the active site before the transfer to tRNA (pre-transfer editing), while others are edited after transfer to tRNA at a separate editing site (post-transfer editing). Access of natural non-protein amino acids, such as homocysteine, homoserine, or ornithine to the genetic code is prevented by the editing function of aminoacyl-tRNA synthetases. Disabling editing function leads to tRNA mischarging errors and incorporation of incorrect amino acids into protein, which is detrimental to cell homeostasis and inhibits growth. Continuous homocysteine editing by methionyl-tRNA synthetase, resulting in the synthesis of homocysteine thiolactone, is part of the process of tRNA aminoacylation in living organisms, from bacteria to man. Excessive homocysteine thiolactone synthesis in hyperhomocysteinemia caused by genetic or nutritional deficiencies is linked to human vascular and neurological diseases.

Environmental factors affect human organism in many ways. They can stabilize, modify or be the cause of many diseases, particularly of circulation system. Diseases of circulation system at the end of the last millennium in USA were the cause of 38% of all deaths. Such high degree of mortality causes necessity of undertaking prophylactic actions in order to reduce the negative influence of risk factors, particularly those environment factors which influence on pathogenesis of some diseases can be modified. Environment might be the factor which causes heart disease by: oxidation stress, inflammatory process, induction of coagulation system, dysfunction of autonomic system. Schetler (70) divided the factors favoring diseases of circulation system into two groups: risk factors (which have admitted participations in pathogenesis of heart and vascular diseases) and environmental factors which significance in pathogenesis of heart diseases still increases. It is commonly acknowledged that environmental factors which favor development of many diseases of circulation system belong to: physical factors polluting the air, water, soil and food, noise, electromagnetic field, ionizing radiation and electric energy. Most of them are industrial pollution – organic and non-organic. They include also six basic air pollution compounds (ozone, CO2, SO2, CO, PbO4, dust); chemical elements (Pb, Hg, Cd, CO, As, lack of Se); medicaments; alcohol and drug inducing stupor; smoking and passive smoking. Also malnutrition and low physical activity increase the risk of occurrence of many circulation system diseases. The described situation is magnified by the fact, that present civilization still increases the number of environmental factors negatively influencing heart-vascular system.

Introduction. Elevated uric acid (UA) is associated with arterial hypertension (AH), obesity, dyslipidemia and insulin resistance. However, its association with body components has not been previously investigated. Objective. The aim of this study was to evaluate the relationship between UA and cardiovascular risk factors, anthropometric parameters and body composition in patients with AH. Materials and method. In 138 patients with AH the following parameters were evaluated: UA, low and high density lipoproteins (LDL-C, HDL-C), triglycerides (TG), fasting glucose (FG), creatinine; body mass index (BMI), waist circumference (WC), fat mass (FM), fat free mass (FFM) and total body water (TBW). Results. Positive correlations were shown between UA and LDL-C (p=0.041), TG (p<0.001), FG (p=0.025) and creatinine (p<0.001) and negative between UA and HDL-C (p<0.001). Significant associations between UA and anthropometric parameters and body components, such as WC (p<0.001), BMI (p<0.001), FFM (p<0.001) and TBW (p<0.001), were also observed. In the multiple regression model, independent predictors of UA concentration were serum creatinine and TBW (R2=0.45; p<0.001). Conclusions. In patients with AH, uric acid was significantly related to cardiovascular risk factors, including obesity. However, the main anthropometric determinant of plasma UA concentration is FFM. The consideration of body composition in the interpretation of UA concentration appears to be justified, but the verification of this hypothesis requires further studies.

The generation of reactive oxygen and nitrogen species is a part of normal metabolism and under physiological conditions undergoes a strict control by a variety of enzymatic and non-enzymatic anti-oxidative mechanisms. However, under pathological conditions, the enhanced production of free radicals and non-radical oxidants may be overwhelm the anti-oxidative defence, leading to oxidative stress and irreversible damage of cells and tissues. It has been established, that increased production of reactive oxygen and nitrogen species has been implicated in the development of cardiovascular system-related disorders, including hypertension, atherosclerosis, diabetes, and ischemia-reperfusion injury. The presented review is a brief insight on the role of oxidative stress in the pathogenesis and progression of cardiovascular diseases, related to haemostasis disturbances and endothelium dysfunction.
Powstawanie reaktywnych form tlenu i azotu jest częścią prawidłowych przemian biochemicznych i w warunkach fizjologicznych podlega ścisłej kontroli przez system licznych mechanizmów antyoksydacyjnych. W warunkach patologicznych dochodzi jednak do zwiększenia generowania zarówno wolnych rodników, jak i nierodnikowych czynników utleniających, co przewyższa możliwości obrony antoksydacyjnej organizmu. Pojawiający się stres oksydacyjny prowadzi do nieodwracalnych uszkodzeń komórek i tkanek. Wiadomo, że reaktywne formy tlenu i azotu są zaangażowane w rozwój wielu zaburzeń związanych z układem sercowo-naczyniowym, takich jak miażdżyca, nadciśnienie, cukrzyca czy uszkodzenia związane z niedokrwieniem serca i reperfuzją. Prezentowana praca stanowi krótki przegląd dostępnych danych, dotyczących biochemicznych podstaw udziału stresu oksydacyjnego w patogenezie i rozwoju wybranych chorób układu sercowo-naczyniowego.

Praca w środowisku zimnym powoduje znaczne obciążenie układu krążenia Jest to szczególnie niebezpieczne dla osób z chorobami układu krążenia (chorobą wieńcową, nadciśnieniem tętniczym) oraz dla osób starszych. W Polsce od wielu lat - mimo postępów w diagnostyce i leczeniu - choroby te są odpowiedzialne za ok. 50% wszystkich zgonów, co stawia nasz kraj na jednym z pierwszych miejsc pod tym względem w Europie. Publikacje naukowe dotyczą przede wszystkim wpływu niskich temperatur otoczenia na zaostrzenie chorób układu krążenia w populacji ogólnej. Znany jest fakt, że przypadki zawału serca i zaostrzenia choroby niedokrwiennej serca występują częściej w chłodniejszych porach roku. Według badań, narażenie na zimno zwiększa również skurczowe i rozkurczowe ciśnienie tętnicze krwi. Powtarzająca się ekspozycja na Środowisko zimne w miejscu pracy może stanowić również ryzyko rozwoju nadciśnienia tętniczego i choroby niedokrwiennej serca.
Working in the cold causes a significant burden on the cardiovascular system It is especially dangerous for people with cardiovascular disease (coronary heart disease, hypertension) and for the elderly. In Poland, despite advances in diagnosis and treatment, these diseases have been responsible for about 50% of all deaths, which puts our country in one of the first places in Europe. Scientific publications mostly discuss the impact of low ambient temperatures on the risk of cardiovascular disease in the general population. The incidence of myocardial infarction and exacerbated coronary heart disease are more frequent in colder seasons. Exposure to the cold increases systolic and diastolic blood pressure. Repeated exposure to a cold environment m the workplace can also be a risk f actor for developing high blood pressure and coronary heart disease.

Introduction: Cardiovascular diseases constitute the most frequent cause of death, even exceeding cancer. The so--called risk factors, both modifiable and non-modifiable, play a significant role in their development. methods: 200 secondary school students, aged 16–19 years, participated in the study (137 girls and 63 boys). The study was conducted by means of the authors’ own survey questionnaire. Results: The vast majority of respondents (187; 93.5%) appropriately indicated obesity, hypercholesterolemia and smoking as modifiable risk factors. Nearly half of the respondents (96; 48%) appropriately recognised abdominal obesity based on waist size measurements and in the vast majority (157; 78.5%) correctly marked body mass index (BMI) for proper body weight. Most (136; 68%) knew about arteriosclerosis, the aftermaths of excessive salt consumption (158; 79%), fast food (184; 92%) and alcohol (139; 69.5%). Definitely fewer students (63; 31.5%) indicated the sources of cholesterol in food and appropriately defined the notion of HDL (72; 36%) and BMI (73; 36.5%). Conclusion: Secondary school students’ knowledge on the risk factors for cardiovascular diseases is incomplete. There is a need to implement health education aimed at reducing the prevalence of risk factors at a young age and/or their elimination, and as a result preventing the development and progression of cardiovascular diseases.
Wstęp: Choroby układu krążenia stanowią najczęstszą przyczynę zgonów, częstszą nawet niż choroby nowotworowe. W ich rozwoju istotne znaczenie mają zarówno modyfikowalne, jak i niemodyfikowalne czynniki ryzyka. METODY: Badaniem objęto 200 uczniów (137 dziewcząt i 63 chłopców) szkół licealnych w wieku 16–19 lat. Badanie przeprowadzono za pomocą autorskiego kwestionariusza ankiety. WYNIKI: Zdecydowana większość badanych (187; 93,5%) jako modyfikowalne czynniki ryzyka poprawnie wskazała otyłość, hipercholesterolemię i palenie tytoniu. Prawie połowa badanych (96; 48%) na podstawie obwodu pasa potrafiła rozpoznać otyłość brzuszną, a zdecydowana większość (157; 78,5%) poprawnie zaznaczyła zakres BMI (body mass index) dla prawidłowej masy ciała. Większość ankietowanych (136; 68%) wiedziała, czym jest miażdżyca, jakie są konsekwencje nadmiernego spożywania soli (158; 79%), posiłków typu fast food (184; 92%) oraz alkoholu (139; 69,5%). Zdecydowanie mniej ankietowanych (63; 31,5%) prawidłowo wskazało źródła cholesterolu w pokarmach oraz poprawnie zdefiniowało pojęcia HDL (72; 36%) i BMI (73; 36,5%). WNIOSKI: Wiedza uczniów szkół licealnych na temat czynników ryzyka chorób układu krążenia jest niepełna. Konieczne jest prowadzenie działalności w zakresie oświaty zdrowotnej, mającej na celu zmniejszenie rozpowszechnienia czynników ryzyka już w młodym wieku i/lub ich eliminację, a w konsekwencji zapobieganie rozwojowi i progresji chorób układu krążenia.

Opis:
Small-molecular tyrosine kinase inhibitors constitute an effective therapeutic option in patients with hematologic malignancies and solid tumours. On the other hand, the significance of cardiovascular adverse events associated with their use is often emphasised. The events include arterial hypertension, heart failure, coronary disease/acute coronary syndromes, and long QT syndrome. The paper discusses the underlying mechanisms behind cardiovascular events associated with the treatment that involves tyrosine kinase inhibitors, and presents preventive and therapeutic options available in clinical practice. Awareness of the potential cardiovascular complications, regular follow-up, early diagnosis and initiation of appropriate treatment, combined with close collaboration with cardiology specialists, may enhance the benefits of long-term TKI therapy.

ObjectivesAt the end of the 20th century, after years of negligence in the prevention of cardiovascular diseases, Poland was struggling with very high premature mortality. The period of 1991–2005 brought significant improvements since the general public introduced beneficial dietary modifications. This paper aims to analyze the changes in the rate of premature mortality due to tobacco-dependent cardiovascular diseases in Poland in 2008–2017.Material and MethodsThe time trends of deaths occurring under the age of 65 years caused by ischemic heart disease, cerebrovascular disease, atherosclerosis and aortic aneurysm were analyzed. Both standardized and crude premature mortality rates were used, as well as mortality rates for patients grouped into 5-year age ranges with a breakdown by gender. The joinpoint model was used to determine these time trends.ResultsPremature mortality due to the analyzed cardiovascular diseases decreased linearly in 2008–2017. In the case of ischemic heart disease and cerebrovascular diseases, the decrease amounted to approx. 5% per year, both in the female and male population. However, in the case of atherosclerosis and aortic aneurysms, the rate of mortality reduction ranged 4–7% per year. The reduction concerned all the examined age groups, but with different dynamics. The most considerable annual decrease was observed in the group of patients aged 40–44 years (7.9% for females and 8.9% for males). Along with the increase in age, the dynamics of reduction decreased.ConclusionsIn 2008–2017, Poland experienced a decline in premature mortality due to tobacco-related cardiovascular diseases, particularly in the age group of 40–44 years. The decline may have been associated, among other things, with a reduction in exposure to tobacco smoke, one of the cardiovascular risk factors.

Objectives To investigate a hypothesized positive association between employment in occupations where heavy lifting is likely to occur, and the risk of ischaemic heart disease (IHD). Material and Methods Male blue-collar workers from Denmark (N = 516 180) were monitored with respect to hospital treatment or death due to IHD, through national registers over the years 2001–2010. Poisson regression was used to estimate relative rates of IHD between “workers in occupations which, according to an expert opinion, are likely to involve heavy lifting” and “other blue-collar workers.” Prevalent cases were excluded from the analysis. Results The rate ratio was estimated at 0.97 (95% confidence interval (CI): 0.94–1.00) for deaths or hospitalizations due to IHD and 1.07 (95% CI: 0.94–1.21) for deaths due to IHD. Conclusions The results do not support the hypothesis that occupational heavy lifting is an important risk factor for IHD.

Objectives: Both hyperthyroidism and overt hypothyroidism are associated with increased prevalence of metabolic syndrome and its components, while data on subclinical hypothyroidism is currently limited especially in working populations. The aim of this study was to examine the association between subclinical hypothyroidism and metabolic syndrome components in workers; and to evaluate whether there are differences by sex and occupation. Material and Methods: A total of 1150 university employees (male - 792, female - 358) aged 30-60 years who came for an annual medical check-up were studied. Anthropometric measurements were taken, and blood pressure, fasting plasma glucose (FPG), lipid profiles, thyroid stimulating hormone (TSH), free thyroxin (FT4) and free triiodothyronine (FT3) levels were measured. Results: After adjustment for age and body mass index (BMI), TSH was positively associated with increased triglyceride (TG) levels (β = 0.108, p = 0.020) and FPG (β = 0.130, p = 0.006) in subclinical hypothyroid male workers. However, TSH was not associated (p > 0.05) with any component of metabolic syndrome (MS) in the euthyroid group. In females, TSH was not correlated with MS components in both euthyroid and subclinical hypothyroid groups. Furthermore, comparison by occupation showed higher TSH in subclinical hypothyroid male workers employed in administration (5.23±0.52 mU/l) than those working as academics (5.12±0.52 mU/l), which resulted in elevated systolic and diastolic blood pressure, FPG, total cholesterol, TG and high density lipoprotein cholesterol. In females, BMI, systolic and diastolic blood pressure, TG and FPG were significantly (p < 0.05) higher in subclinical hypothyroid administrators than those in academics. Conclusions: Subclinical hypothyroidism was associated with metabolic syndrome components in male workers and not in females. Administration workers showed increased metabolic risks compared to academics. The findings suggest that the assessment of thyroid function in individuals with metabolic syndrome in the workplace may be favorable especially among men.

Wstęp: Zmiany ciśnienia tętniczego krwi podczas wysiłku fzycznego są u poszczególnych osób indywidualnie różne. Ich przyczyny nie są dokładnie znane. Cel pracy: Celem prezentowanej pracy jest zbadanie reakcji krążeniowych w spoczynku i przy obciążeniu fzycznym powodującym wzrost skurczowego ciśnienia tętniczego (SBP) = 200 mmHg u osób o różnej wydolności fzycznej. Materiał i metody: W badaniach uczestniczyło 18 piłkarzy nożnych i 14 nietrenujących studentów nie różniących się wiekiem, masą i wysokością ciała i wskaźnikiem BMI, u których w warunkach spoczynkowych dokonywano pomiarów ciśnienia tętniczego skurczowego (SBPr), ciśnienia tętniczego rozkurczowego (DBPr) i częstości skurczów serca (HRr). Następnie badani wykonywali wysiłek fzyczny na cykloergometrze o wzrastającej intensywności, w którym przy ciśnieniu skurczowym = 200 mm Hg (SBP200) dokonywano pomiarów DBP200 i HR200. W warunkach spoczynkowych i wysiłkowych wyliczano wielkość ciśnienia średniego (MAPr, MAP200), ciśnienia tętna (PPr, PP200) i podwójnego iloczynu ciśnienie – tętno (DPr, DP200). Wyliczano także moc osiąganą w teście PWC170 (W i W/kg) oraz wielkość maksymalnego poboru tlenu VO2max (ml/min i ml/min/kg). Wyniki: Piłkarze osiągnęli wyższą moc w PWC170 i VO2max oraz niższe HR200 i DP200 (p<0,001), jak również niższe wartości HRr (p<0,01) niż studenci, a spoczynkowe jak i wysiłkowe SBP, DBP, MAP i PP nie różniły się między badanymi grupami. W grupie piłkarzy moc w PWC170 (W) korelowała ujemnie (p<0,05) z DP200, a VO2max (ml/min; ml/min/kg) również korelował ujemnie z HRr (p<0,05; p<0,01). Wśród studentów moc w PWC170 (W, W/kg) i VO2max (ml/min i ml/min/kg) korelowała ujemnie z HR200 i z DP200. Ponadto VO2max (ml/min/kg) korelował ujemnie z HRr. Wnioski: Badania wykazały, że pomimo różnic w wydolności fzycznej badanych grup nie stwierdza się istotnych zmian ciśnienia tętniczego krwi spoczynkowego i modyfkowanego wysiłkiem fzycznym. Jednak przy obciążeniu powodującym skrajnie wysoki wzrost SBP obserwuje się poprawę ekonomizacji pracy serca i układu krążenia niezależny od wielkości wydolności fzycznej badanych.
Background: Changes in blood pressure during subjects’ exercise are individually diferent. The reasons of these diferences are unknown. Aim of the study: The aim of this paper is to investigate the rate of changes in cardiovascular system at rest and at physical load which enhance systolic blood pressure (SBP) = 200 mmHg in people with diferent physical performance. Material and methods: The studies included 18 soccer players and 14 students who do not practice any sport, whose age, body mass, body height and BMI have been similar, and who at baseline were measured systolic blood pressure (SBPr), diastolic blood pressure (DBPr) and heart rate (HRr). Then, the subjects performed exercise on a cycloergometer with increasing intensity in which at the level of the systolic blood pressure = 200 mm Hg (SBP200) were measured DBP200 and HR200. In terms of resting and exercise time the size of the mean arterial pressure (MAPr, MAP200), pulse pressure (PPr, PP200) and rate-pressure product (DPr, DP200) were calculated. Moreover the power achieved in PWC170 test (W and W/kg) and maximal oxygen uptake (VO2max) (ml/min and ml/min/kg) were also calculated. Results: Soccer players have reached a higher power in PWC170 and higher VO2max and lower HR200 and DP200 (p<0,001), as well as lower values of HRr (p<0,01) than students however SBP, DBP, MAP and PP registered during rest and during exercise did not difer between the groups. Among the players power reached at PWC170 (W) correlate negatively with DP200 – (p<0,05) and VO2max (ml/min; ml/min/kg) also correlated negatively with HRr (p<0,05, p<0,01). Among students power reached at PWC170 (W, W/kg) and VO2max (ml/min, ml/min/kg) correlate negatively with HR200 and DP200. Moreover VO2max (ml/min/kg) correlate negatively with HRr. Conclusions: Studies have shown that, despite diferences in physical performance of both groups, there were no signifcant changes in blood pressure at rest or modified during exercise. However, at extremely high increase in SBP improved economization in function of the heart and circulatory system independent of the size of physical performance of the subjects were observed.

Otyłość jest czynnikiem ryzyka śmiertelności ogólnej, zgonów z przyczyn sercowo-naczyniowych i nowotworowych . Jest uznanym, niezależnym czynnikiem rozwoju miażdżycy naczyń. Ryzyko wystąpienia chorób somatycznych zwią- zanych z otyłością zależy nie tylko od stopnia otyłości, ale głównie od rozmieszczenia tkanki tłuszczowej. Kluczową rolę w rozwoju insulinooporności, nietolerancji glukozy, cukrzycy typu 2, nadciśnienia tętniczego oraz chorób układu sercowo naczyniowego odgrywa trzewna tkanka tłuszczowa. Przeprowadzone badania wskazały, że osoby otyłe nie są grupą jednorodną pod względem profiu metabolicznego, a BMI nie jest uniwersalnym wskaźnikiem otyłości. W latach 80. XX wieku Ruderman wyróżnił tzw. metaboliczną otyłość z prawidłową masą ciała, w której zwiększona ilość tkanki tłuszczowej trzewnej i podskórnej brzucha związana jest ze zwiększonym ryzykiem zaburzeń metabolicznych. Insulinooporność i hiperinsulinemia uznawane są za kluczowe zaburzenia w tej grupie osób. Wraz ze współistniejącymi zaburzeniami gospodarki węglowodanowej, lipidowej i podwyższonym ciśnieniem tętniczym wskazują na podwyższone ryzyko schorzeń układu sercowo-naczyniowego. Z kolei w populacji osób otyłych wyróżniono podgrupę osób metabolicznie zdrowych, u których akumulacja tkanki tłuszczowej nie prowadzi do rozwoju insulinooporności, zaburzeń gospodarki węglowodanowej i lipidowej oraz nadciśnienia tętniczego. Kilka z przeprowadzonych badań wskazało, że u osób otyłych prawidłowe wskaźniki metaboliczne mogą istotnie zmniejszać ryzyko chorobowe. Jednak w świetle obecnego stanu wiedzy otyłość z prawidłowymi wskaźnikami metabolicznymi nie jest w pełni bezpieczna.
The obesity is a risk factor for cardiovascular, cancer and general mortality. It is also an independent risk factor for atherosclerosis. The incidence of obesity-related diseases depends not only on the degree of obesity but mainly on the fat tissue deposition. It is the visceral fat tissue that plays the major role in the development of insulin resistance, impaired glucose tolerance, diabetes, hypertension and cardiovascular diseases. The research showed that the population of obese people is not homogenous in terms of metabolic profie and that a BMI is not the universal obesity indicator. In the 80s Ruderman distinguished metabolic obesity with normal weight. It is defied as an increased amount of visceral and subcutaneous abdominal fat and is associated with an increased risk of metabolic disorders. Insulin resistance and hyperinsulinemia are considered to be the fundamental disorders in this group. The coexistence of impaired carbohydrate and lipid metabolism and hypertension leads to a high risk of cardiovascular disease. The fat tissue deposition enabled also to identify the obese patients that are not at risk of insulin resistance, glucose or lipid metabolism disorders and hypertension (“metabolically healthy”). There are some studies that show the reduction of cardiovascular risk among obese patients with normal metabolic parameters. However, the current knowledge does not allow us to regard obesity with normal metabolic parameters as completely safe.

ObjectivesA method of continuous heart rate (HR) and blood pressure (BP) recording was used for the evaluation of the cardiovascular system parameters in participants of short-term (<1 month) high-latitude expeditions, in comparison with the parameters of residents of Central Russia and the Arctic region.Material and MethodsA dynamic examination of participants of Arctic expeditions (30 men, residents of middle-latitude regions, aged 46.7±1.7 years), workers permanently living in Central Russia (the Moscow region, 44 men, aged 46.7±1.0 years) and residents of the North (the Murmansk region, 35 men, aged 46.6±1.3 years) was performed. The authors used a spiroartheriocardiorythmograph allowing the parallel recording of HR, BP, spectral characteristics of HR variability (HRV) and the variability of systolic BP (sBP) and diastolic BP (dBP), cardiac performance parameters, and spontaneous baroreflex sensitivity (BRS). The parameters were recorded at rest, in a sitting position, over 2 min.ResultsThe basic clinical parameters (HR, BP and cardiac performance) did not differ in the workers living in different climatic zones. However, the residents of the North demonstrated a lower total power (TP) of the dBP variability spectrum and a lower relative power of the high-frequency (HF) range in both the sBP and dBP variability spectra. The participants of expeditions to the North had a lower TP of the HRV spectrum (in comparison with both control groups) that did not change during the expeditions; BRS was reduced, while the TP of the sBP spectrum was increased in comparison with the corresponding parameters obtained from the residents of circumpolar regions, and decreased during the expedition in parallel with a decrease in the sBP values. The TP of both the sBP and dBP variability spectra, as well as the power of the HF range in these spectra, were similar in the participants of expeditions to those obtained from the residents of Central Russia, and they considerably surpassed the corresponding parameters in the northerners surveyed.ConclusionsThe revealed peculiarities of the cardiovascular system in the participants of high-latitude expeditions can be considered as correlates of positive, and adequate in terms of the physiological value, adaptive shifts in the autonomous regulation of the cardiovascular system.
