The International Atomic Energy Agency has started a few years ago a project to enhance the existing framework for safety goals for nuclear installations and currently is finalizing a publication aimed to provide guidance for a more holistic approach to establishing and utilizing safety goals for nuclear installations. The intention was to develop a more consistent and holistic framework for safety goals that would be composed of a hierarchical structure of qualitative concepts (e.g. defense in depth, various safety requirements) and quantitative risk metrics. The safety goals should be technology neutral at the higher level and should cover both normal operation and accident conditions as well as the risk to workers, public, and the environment. In the paper the development of a hierarchical framework for safety goals is described including a methodology to derive lower-(more technical) safety goals from the higher goals. Moreover a process for assessing the degree of compliance with the safety goals and their consistent use is provided. As an example the developed framework is applied to the German regulatory framework for nuclear installations but focused on nuclear power plants.

This paper aims to show a practical case study of safety assessment concerning possible injuries and fire occurring in the engine room on-board a merchant ship, due to fatigue. Methods The methodology was centred in literature hazard identification, measurements in real ships and heat stress indices definition. Results Some corrections are proposed in order to obtain a safe environment for workers in the engine control room and to prevent fatigue risks at the engine room. This will allow adequate oil leakage detection and, as a consequence, improve fire prevention. Discussion Training is a preventive control option, but this study allows the understanding of new corrective control options like design corrections.

In parallel with the dynamic development of rail transport in terms of vehicle design, control systems, infrastructure issues, the development of processes and procedures in the area of safety management must also progress. This growing awareness was confirmed, among other things, by harmonising the content of the regulations on railway safety and interoperability, the safety certification of railway undertakings and the definition of the tasks and roles of national safety authorities. In effect, this was to enable the development of a single European railway area. An implementation tool that allows for a systematic approach to safety management processes and that enables the above requirements to be met is, among other things, RAMS analyses. A key step in the safety management process for specifying RAMS is hazard identification, which is particularly highlighted in PN EN 50126-2:2018 through the holistic model for risk assessment and control of railway system hazards (hourglass model). It places the hazard identification process in two stages, i.e. in the early development stage of the analysis (similar to other known risk management models) and in the hazard control stage. This positioning alone indicates the importance of the hazard identification process. On the comprehensiveness and detail of its implementation depends the validity of the final outcome of the RAMS analysis. The development of such a process for real technical facilities in a way that is consistent with the needs of RAMS analyses and, at the same time, ensures that satisfactory results of these analyses are achieved, has become the subject of this article.

The objective of this work is to evaluate the safety of adult and child passengers exposed to a radio frequency (RF) source, i.e., a leaky coaxial cable (LCX) on the subway platform. An adult model, a child model, and an LCX model have been numerically designed in COMSOL Multiphysics software. The distributions of the induced electric field (E-field), specific absorption rate (SAR), magnetic field ( H-field) and the head temperature increase in adult and child passenger models were calculated at 900 MHz. The induced fields in the passengers were compared with that without screen doors. The results show that the E-field, SAR and H-field in the whole body of the child are 2.00 × 10 -2 V/m, 1.07 × 10 -7 W/kg, and 2.94 × 10 -4 A/m, respectively. The E-field, SAR and H-field in the central nervous system of the child are 1.00e × 10 -2 V/m, 2.44 × 10 -8 W/kg, and 2.41 × 10 -4 A/m, respectively. The maximum values of the E-field, SAR and H-field in the adult passenger are 1.49–2.34 times higher than those of the child. The E-field, SAR, and H-field in the passenger models without a screen door are larger than those with a screen door. The screen door has a partial shielding effect on the RF electromagnetic field. The values of the maximum temperature that increases in adult and child head tissue are 0.2114 and 0.2111℃ after waiting 6 minutes exposure, respectively. All calculated results are well below the International Commission on Non-Ionizing Radiation Protection (ICNIRP) limits for general public exposure, indicating that RF electromagnetic exposure caused by the LCX on the subway platform is not a threat to passenger’s health.

The necessity of verifying manoeuvring mathematical models in shallow water for studying the safety of ships using a ship-handling simulator is pointed out in this report. Several instances of verification of mathematical models in shallow water are introduced here based on measurements of motion conditions of full-scale ships and shallow water tank tests of models. Results of safety assessment tests of five manoeuvring phases are given using the verified manoeuvring mathematical models to discuss manoeuvring criteria in shallow water. Objective manoeuvring criteria for safety assessment in shallow water are proposed based on subjective judgement related to control margin assessed by more than 3,200 masters and pilots for over 325 simulation tests and by analysis of the study results of 15 full-scale ships.

"Safe voyage from berth to berth" — this is the goal of all e-navigation strains, driven by new technologies, new infrastructures and new organizational structures on bridge, on shore as well as in the cloud. To facilitate these efforts suitable engineering and safety/risk assessment methods have to be applied. Understanding maritime transportation as a sociotechnical system allows system engineering methods to be applied. Formal and simulation based verification and validation of e-navigation technologies are important methods to obtain system safety and reliability. The modelling and simulation toolset HAGGIS provides methods for system specification and formal risk analysis. It provides a modelling framework for processes, fault trees and generic hazard specification and a physical world and maritime traffic simulation system. HAGGIS is accompanied by the physical test bed LABSKAUS which implements a reference port and waterway. Additionally, it contains an experimental Vessel Traffic Services (VTS) implementation and a mobile integrated bridge enabling in situ experiments for technology evaluation, testing, ground research and demonstration. This paper describes an integrated seamless approach for developing new e-navigation technologies starting with virtual simulation based assessment and ending in physical real world demonstrations.
„Bezpieczna podróż od nabrzeża do nabrzeża” — to cel całego procesu e-nawigacji, napędzanego nowymi technologiami, nową infrastrukturą i nową organizacją pracy na mostku, na lądzie, ale także w technologii chmury znanej z informatyki. By ułatwić te wysiłki, należy zastosować również odpowiednie metody ocena ryzyka. Rozumiejąc transport morski jako system socjotechniczny, można do tych celów zastosować metody inżynierii systemów. Formalna ocena ryzyka, a także oparta na symulacjach weryfikacja i walidacja technologii e-nawigacji to ważne metody oceny bezpieczeństwa i niezawodności systemu. Modelowanie i zestaw narzędzi symulacyjnych HAGGIS okazują się odpowiednimi instrumentami do opisu systemu i formalnej analizy ryzyka. Wyznaczają ramy dla modelowania procesu, sporządzenia drzewa błędów i generowania wykazu zagrożeń, jak również otoczenia fizycznego i symulacji ruchu morskiego. HAGGIS jest wspierany przez rzeczywisty poligon testowy LABSKAUS, który implementuje port i drogi wodne. Dodatkowo obejmuje on eksperymentalny system nadzoru ruchu (VTS) oraz mobilny zintegrowany mostek umożliwiający realne eksperymenty dla oceny technologii, testowania i demonstracji. W artykule opisano zintegrowane, spójne podejście do rozwijania nowych technologii e-nawigacji, zaczynając od wirtualnej symulacji, a kończąc na demonstracjach w fizycznej rzeczywistości.

Industrial safety is an important issue in Thailand, and attempts have been made to improve safety performance and accident records. This paper examines key criteria influencing safety improvement. Exploratory factor analysis confirms 9 safety criteria, including 5 “enablers” and 4 “results”, with a total of 47 associated attributes. A safety assessment approach is developed, using those 9 key criteria, to measure an organization’s current safety maturity level. Organizations can use the assessment approach to plan its safety improvement, and progress through to higher maturity levels by focusing on the weakest criteria shown in the assessment results with the lowest scores.

International experience has shown that external hazards (e.g. aircraft crash, flooding) can be safety significant contributors to the risk in case of nuclear power plants` operation. This is due to the fact that such hazards have the potential to reduce simultaneously the level of redundancy by damaging redundant systems or their supporting systems. In this paper, the procedure for the external hazard aircraft crash is described In more detail, starting with the screening procedure in order to determine scope and content of the assessment and the approach for those cases where a full scope analysis has to be performed. The consideration regarding this hazard does as not cover an intended aircraft crash.

Celem autorów jest również uczynienie Wytycznych FSA bardziej dostępnymi, tj. zrozumiałymi dla potencjalnych użytkowników. W tekście tym zamieszczono zatem wiele interpretacji mniej oczywistych pojęć, procedur i zasad dotyczących bezpieczeństwa morskiego i ochrony.
The paper attempts to illustrate principles of Maritime Formal Security Assessment to assess the security in Polish maritime areas. The authors’ aim is also to make the Guidelines of FSA more accessible, i.e. easier to understand by potential users. Therefore the text also includes several interpretations of less obvious notions, procedures and principles related to maritime security and protection.

The paper presents some current results of the research on development of a method for safety assessment of ships in critical conditions at the preliminary design stage. The meŹthod is expected to make it possible to perform the multi-objective and multi-parametric investigations to achieve the required and optimum levels of safety by using the risk-based and formal approaches to ship safety. During the design analysis influence of parameters associated with the hull form, arrangement of internal spaces, loading conditions, position and extent of damage, cargo shift and weather impacts on safety of ships in critical condiŹtions will be taken into account. Current capabilities of the method are illustrated by several calculation examples of influence of different arrangements of internal spaces of a cargo ship on values of the IMO formal subdivision indices such as the "Attained Subdivision Index A" and "Local Subdivision Indices ?Aj". The method is still under development and its final structure will be presented in the near future.

The article presents an exemplary application of the multi-criteria navigational safety assessment method for the proposed variants of the designed waterway in order to verify it. This method comprehensively considers the most important environmental and technical aspects related to the implementation of the planned investment. The method can be used for the purposes of navigational analysis, i.e. an engineering document that meets the requirements of the regulation of the competent, which subject to agreement with the Director of the territorially competent Maritime Office.

Formalna Analiza Ryzyka (FSA) jest skuteczną metodą mającą na celu zwiększenie bezpieczeństwa morskiego. Szczególnie zwraca uwagę na ochronę życia, zdrowia, środowiska morskiego i mienia, wykorzystując analizę ryzyka, metody jego kontroli i ocenę kosztów. Celem FSA jest wspieranie procesu zarządzania bezpieczeństwem, standaryzacji bezpieczeństwa oraz ustalanie polityki działań na rzecz bezpieczeństwa. Metoda ma charakter ogólny i może być wykorzystana, jako narzędzie wspomagające podejmowanie decyzji w zakresie tworzenia nowych regulacji prawnych, projektowania, klasyfikacji, budowy i eksploatacji statków. Metoda nadaje się do analizy pojedynczego statku lub typu statków, akwenów, np.: tankowców, zatok, portów, itp. W artykule przeanalizowano dokumenty międzynarodowe dotyczące FSA.
A Formal Safety Assessment (FSA) is an effective method that is aimed to increase maritime safety. Particularly draws attention to the protection of life, health, property and the marine environment, using risk analysis, methods of its control and the assessment of costs. The main purpose of FSA is to support the process of security management, the standardization of safety and determining safety action policy. The method is general and can be used as a tool to assist decision making in the field of the creation of new legal regulations, classification, design, construction and operation of ships. Method is suitable for the analysis of a single vessel or type of vessel, such as: oil tankers, gas tankers or different areas, for example bays, ports, jetties, etc. The article analyses the international instruments relating to the FSA.

This paper addresses the improvements to the reliability of the safety systems of nuclear reactors using redundancy allocation technique. The study has been carried out using the Probabilistic Safety Assessment (PSA). PSA involves, among others, the use of fault and event tree tools in the evaluation of the safety system failure probabilities and the quantification of annual occurrence probability of the accidental conditions postulated in the design of the nuclear reactors. The PSA has been presented and discussed. The Egypt Second Research Reactor, ETRR-2, has been used as a case study. The failure probability of the already existing safety systems has been reviewed. The effect of the allocation of more redundant components to the existing safety systems on the failure probability of the systems has been evaluated. The event trees for two selected initiating events, from those events postulated in the ETRR-2 design, have been studied considering the allocation of more redundant components to the safety systems. The result of the study showed that further improvement could be introduced to the reliability of the Confinement Ventilation System (CVS).

Formal safety assessment of ships has attracted great attention over the last few years. This paper, following a brief review of the current status of marine safety assessment is focused on the hazards identification (HAZID) and prioritisation process. A multicriteria decision making framework, which is based on experts‟ estimation, is then proposed for hazards evaluation. Additionally in this paper many aspects of the evaluation framework are presented including the synthesis of evaluation teams, the assessment of the importance of criteria, the evaluation of the consequences of the alternative hazards and the final ranking of the hazards. The proposed methodology has the innovative feature of embodying techniques of fuzzy logic theory into the classical multicriteria decision analysis. The paper concludes by exploring the potentiality of the above methodology in providing a robust and flexible evaluation framework suitable to the characteristics of a hazard evaluation problem.

Opis:
The regulatory body, established to ensure safety of nuclear facilities, is expected to make right decisions and provide appropriate regulations for the nuclear industry. The traditional manner of its activity has been based on a deterministic approach to safety analyses. However, increased maturity of Probabilistic Safety Assessment (PSA) makes it complementary to deterministic studies. The new IAEA concept, described in this article, is to apply an integrated approach by combining both deterministic and probabilistic insights with other requirements affecting the decision making process.
Organ regulacyjny, powołany w celu zapewnienia bezpieczeństwa jądrowego, jest odpowiedzialny za podejmowanie decyzji i wprowadzanie rozporządzeń dla przemysłu jądrowego. Tradycyjny sposób jego funkcjonowania opiera się na deterministycznym podejściu do analiz bezpieczeństwa. Rozwój analiz probabilistycznych (PSA) sprawia jednak, iż są one traktowane jako podejście komplementarne. Nowa koncepcja IAEA, opisana w tym artykule, polega na zintegrowanym podejściu, uwzględniającym analizy deterministyczne, probabilistyczne i inne aspekty procesu decyzyjnego.

W releracie przedstawiono problematykę bezpiecznego użytkowania żelbetowych silosów. Opisano wartości częściowych współczynników bezpieczeństwa związanych z obciążeniami konstrukcji silosów i z samą konstrukcją żelbetową. Pokazano zakres bezpieczeństwa na tle obliczanych współczynników bezpieczeństwa i awarii. Wykorzystano metody 2 Poziomu probabilistycznej oceny niezawodności. Wskazano w jakich miejscach i w jakich przypadkach można liczyć na większy poziom niezawodności żelbetowej konstrukcji silosów.
The researches include the evaluation parameters connected with silo strength and geometry etc. These experimental data were subjected to statistical accounts. According to the data of the mentioned above parameters values, the evaluation and estimation ol density function of these random values, statistics and silo reliability were calculated based on Level-1 (FORM), Level-2 (SORM) and Level-3 Reliability Methods. This results were used to estimate safety of existing, old silo construction and in designing new.

The paper deals with safety and reliability assessment as an integral part of the development process for modern aviation products with potentially critical functions. Focus is on digital engine control units, their development process and tools offering potential savings in otherwise time demanding and expensive safety assessment processes. The paper shows application of several approaches, which together form an innovative way for safety assessment of aerospace products (otherwise strictly limited by regulation procedures). It is focused on practical ways towards reduction of development costs during safety assessment, which do not compromise its comprehensiveness. Described approaches are based on experience from development of numerous aerospace products in last nearly 20 years. As an addition, possibility to further enhance the proposed innovative effect classification by application of FMECA was shown. Possible methods for quantitative assessment using Fuzzy logic and/or multiple-criteria decision analysis were discussed.

Risks relating to external hazards, either natural or man-made, have to be taken into consideration in the design of nuclear and other industrial facilities. These risks have to be studied to guarantee the availability and efficiency of safety functions which, e.g. in the case of power reactors, enable a safe shutdown, maintain the reactor in a safe shutdown state, ensure the residual heat removal and the containment of radioactive products. With a view to design protection against risks related to external hazards, these hazards have to be assessed in an appropriate manner. The methods used can be either deterministic or probabilistic. In both cases, the method strongly relies on observations (e.g. flood records) that are processed to define a maximum event for the respective facility design. Moreover, the validity of these records over a certain time frame like 100 years has to be checked. Coping with external hazards such as flooding in the future requires an in-depth assessment taking into account new data, further developed methodologies and criteria. Some of these ideas, developments and applications are provided.

Internal and external hazards analysis methods have been used recently to evaluate operating nuclear power plants (NPP) and to identify the resilience of NPP in case of hazards and potential needs for modification of plant systems and procedures as well as to support design of new plants The external hazards risk contribution has been modeled in many NPP PSA (at least for some external hazards) over the last decade, including events occurring during shutdown and low power operations. Recently, a revision of the German nuclear safety regulations has been successfully completed and these regulations entitled “Safety Requirements for Nuclear Power Plants”, also requiring a detailed investigation of internal and external hazards and combinations of different hazards applying deterministic and probabilistic safety assessment methods.

Because of the growing operational age of nuclear power plants, the ageing management of structures, systems and components used in these plants is gaining an important role. Technical systems are subject to timedependent and operationally caused ageing phenomena with modifications of originally given characteristics and, thus, of relevance in terms of safety. Especially physical ageing is of importance. Therefore, a comprehensive ageing management is required. In the context of an integrated safety management it has to be shown how to integrate the safety related issues of ageing into probabilistic safety assessment (PSA). In particular the question is to be answered whether the effort for the execution of an ageing PSA is justified, in particular if the safety significant effects of ageing can be identified and quantitatively estimated. Method for prioritization of the components in the nuclear power plant considering implication of their ageing on safety of the nuclear power plant is presented. On the basis of an actual report on ageing management in German nuclear power plants and a literature survey, this paper tries to estimate the necessity and value for the introduction of an ageing PSA in Germany.

Cruise ships arriving in the port of Koper carry approximately 1000 to 3000 passengers and crew members. Such a concentration of people presents a high degree of risk in the event of a major disaster, because it is difficult to control, due to limited space, the dynamics of people in the event of a general panic, the presence of large amounts of fuel, proximity of the city center and other vessels and cargo at the port. To avoid the possibility of hazard events, a good safety assessment must be done prior to a ship’s arrival. One of the methodologies for systematically assessing the risk is a Formal Safety Assessment, a tool for determing and evaluating the risk of potential hazards at a cruise ship terminal. This paper discusses the diverse aspects of safety analysis.

W artykule przedstawiono logiczno-probabilistyczne podejście do modelowania rozwoju wypadków lotniczych na podstawie analizy czynników ryzyka oraz czynników zapobiegających powstawaniu wypadków lotniczych. Przedstawiona metoda może być elementem proaktywnej strategii zapobiegania powstawaniu wypadkom lotniczym poprzez zarządzanie poziomem bezpieczeństwa lotów.
The paper has been intended to present a probabilistic approach to the a priori modelling of air-accident development, based on the analysis of risk factors and factors conducive to preventing air accidents. Methods of the assessment of air-accident risk are an indispensible component of the pro-active strategy for the safety-oriented management. The presented method may be such a component of the pro-active strategy for preventing air accidents, since it allows of what follows: - development of undertakings such as to prevent (reduce probability) of an air accident, based on the elimination of or reduction in the probability that risk factors occur; - preliminary, competent assessment of the effectiveness of the planned undertakings, based on the cost effectiveness - probability' rate; - selection of optimal variants of controlling (managing) the forecasted air-accident risk. Implementation of the pro-active strategy for safety management gives a chance to reduce accident risk by means of initiating indispensable actions (corrective, preventive, improving ones) to minimise the hazards.

Probabilistic Safety Analyses (PSA) are a supplementary analytical tool used worldwide more and more in order to quantitatively assess the effect of hazards on the overall result regarding the safety of industrial installations, in particular nuclear power plants. In that way PSA provides a reliable basis for decisions on the necessity and the benefits of safety improvements. In the recent past, the existing methods and tools with respect to determining the site-specific risk of nuclear power plants have been comprehensively extended and further enhanced. The focus of extending the existing PSA methods was on external hydrological hazards with flooding potential. For systematically considering hydrological hazards within PSA a systematic approach has been developed. The paper demonstrates the extended approach in the example of a nuclear power plant site with different flooding risks.

External hazards such as explosions can be safety significant contributors to the risk in case of operation of industrial plants. The procedure to assess external hazard explosion pressure waves within probabilistic safety assessment starts with a screening procedure in order to determine scope and content of the assessment. The second step is to choose an appropriate approach in case that a full scope analysis has to be performed. Several methods can be applied to evaluate the probability of occurrence of an external explosion event. The presented results indicate that the probability of occurrence of external explosion pressure waves can be successfully assessed by means of the Monte Carlo simulation, in particular in difficult site-specific conditions.

Dependent failures are extremely important in reliability analysis and must be given adequate treatment so as to minimize gross underestimation of reliability. German regulatory guidance documents for PSA stipulate that model parameters used for calculating frequencies should be derived from operating experience in a transparent manner. Progress has been made with the process oriented simulation (POS) model for common cause failure (CCF) quantification. A number of applications are presented for which results obtained from established CCF models are available, focusing on cases with high degree of redundancy and small numbers of observed events.

The paper presents the assumptions of a simulation model for assessing the navigational safety of fishing vessels. The model consist of submodels of merchant and fishing vessels traffic, submodel of external condition and submodel of accident consequences. Paper present also a results of simulation experiment which was carried out for Polish EEZ. Model takes into account all Polish fishing vessels operating in the Polish EEZ with length more than 15 meters and the merchant vessels traffic in South Baltic Sea area. As a results positions of simulated collisions of fishing and merchant vessels, positions and sizes of simulated bunker spills and the positions of losses of fishing equipment were achieved.
W artykule przedstawiono założenia złożonego modelu symulacyjnego dla szacowania bezpieczeństwa nawigacyjnego statków rybackich. Model składa się z modeli ruchu statków handlowych i statków rybackich, modelu warunków środowiskowych oraz modelu skutków zdarzenia. Ponadto w artykule przedstawiono rezultaty eksperymentu symulacyjnego przeprowadzonego dla polskiej wyłącznej strefy ekonomicznej. Model uwzględnia wszystkie polskie statki rybackie operujące w polskiej wyłącznej strefie ekonomicznej o długości większej niż piętnaście metrów oraz ruch statków handlowych na południowej części Morza Bałtyckiego. W rezultacie otrzymano pozycje prawdopodobnych kolizji statków transportowych oraz rybackich, pozycje i rozmiary prawdopodobnych rozlewów olejowych oraz pozycje utraty narzędzi połowowych.

Opis:
Transport safety is the primary standard in the functioning of transport systems. Most of all, it is true in the case of passenger transport systems. This article introduces classification of transport systems into land transport systems (road and rail) as well as land and water transport systems (inland and sea), depending on the type of environment in which these systems carry out their tasks. Such systems fall under the class of social engineering systems of the Man – Technological Object – Environment (M – TO – E) type. Such systems are influenced by forcing factors, leading to changes in their states. Such factors may be divided into operational, outside and anthropotechnical and they influence the system on various levels, including degradation of the degree of its safety. The article attempts to evaluate the safety of the operation of transport systems on the basis of the evaluation of the safety of the transport process carried out over a defined time interval Δt. The evaluation of the safety of the carried out transport process was prepared on the basis of a set of calculated index values determined depending on the type of transport. The article presents the principles of the construction of the model of evaluation of the safety of the operation of the transport system on the basis of the evaluation of the safety of the transport process carried out by the system.
Bezpieczeństwo jest priorytetowym kryterium funkcjonowania systemów transportowych. Dotyczy to w szczególności systemów transportu pasażerskiego. W pracy dokonano szczegółowej klasyfikacji systemów transportowych. Systemy tego typu należą do klasy systemów socjotechnicznych (C – OT - O), Człowiek – Obiekt Techniczny – Otoczenie. Bezpieczeństwo tych systemów uzależnione jest od niepożądanych oddziaływań ludzi usytuowanych w systemie i jego otoczeniu, niepożądanych oddziaływań obiektu technicznego oraz niepożądanych oddziaływań otoczenia. W pracy podjęto próbę oceny bezpieczeństwa działania systemów transportowych na podstawie oceny zrealizowanego procesu transportowego w określonym przedziale czasowym Δt. Artykuł przedstawia zasadę budowy modelu oceny bezpieczeństwa funkcjonowania systemu transportowego, na podstawie oceny bezpieczeństwa procesu transportu realizowanego za pomocą systemu.

The estimation of leak and break frequencies in piping systems is part of the probabilistic safety assessment of technical plants. In this paper, the statistical method based on the evaluation of the German operational experience for piping systems with different diameters is described because an earlier estimation has been updated and extended introducing new methodical aspects and data. Major point is the inclusion of structure reliability models based on fracture mechanics calculation procedures. As an example of application the statistical estimation method for leak and break frequencies of piping systems with a nominal diameter of 50 mm (the volume control system of a German pressurized water reactor) was updated. Moreover, the evaluation of the operational experience was extended to 341 years with respect to cracks, leaks and breaks in the volume control system of German pressurized water reactors (PWR). Using the actual data base, new calculations of leak and break frequencies have been performed and the results have been compared with the previous values.

Disposal facilities for radioactive wastes comprise a series of engineered barriers whose purpose is to contain the radionuclides until their radiation hazard has decreased to acceptable levels. In this regard, it is required that the long-term functionality of the system of barriers be evaluated by a quantitative risk analysis procedure, also called performance assessment. In this paper, a Monte Carlo simulation-based reliability model is propounded for the preliminary analysis of the safety performance of a radioactive waste repository, accounting also for barrier degradation processes. The model strengths are: simplicity, chich allows ease of computation, and flexibility, which allows modification to account for various physical aspects and inter-comparison of their effects. An application to a case study of literature is presented to validate the approach and demonstrate its flexibility.

Rambøll is on behalf of Femern A/S carrying out navigational studies of the vessel traffic conditions in the Fehmarnbelt in connection with the establishment of a future Fehmarnbelt fixed link, a 19 km long bridge or immersed tunnel connection crossing the Fehmarnbelt. The focus is on documenting safety and efficiency for the vessel traffic when a fixed link is crossing the Fehmarnbelt. Rambøll has developed the ShipRisk software package to perform the quantitative risk assessments in the Fehmarnbelt fixed link project. Focus in this article is on describing the background for estimating frequencies of ship accidents in ShipRisk and describe factors influencing the accident scenarios and present the work performed for testing and verifying the model.

Risk assessment and management was established as a scientific field some 30–40 years ago. Principles and methods were developed for how to conceptualize, assess, and manage risk. These principles and methods still represent largely the foundation of this field today, but many advances have been made, linked to both the theoretical platform and practical models and procedures. The purpose of the thesis is to perform a review of these advances, with a special focus on the fundamental ideas and thinking on which these are based. We have looked for trends in perspectives and approaches, and we reflect on where further development of the risk field is needed and should be encouraged. The present study is written for readers with different types of background, not only for experts on risk. However, there is a conflict between the cost impact and safety aspect. E&P managers as well as government supervisor authorities are constantly faced with decisions to be made regarding of safety. In order to ensure comparability and to set priorities application of QRA is a useful tool to justify choices made with regard to personnel safety, environmental protection, asset damage and business reputation, it is recommended to apply the systematic cause analysis method and develop the risk management models which contains an integral approach toward the health, safety and environmental aspect.

Purpose: The aim of this paper was the development of a finite element model of the soldier’s head to assess injuries suffered by soldiers during blast under a light armoured vehicle. Methods: The application of a multibody wheeled armoured vehicle model, including the crew and their equipment, aenabled the researchers to analyse the most dangerous scenarios of the head injury. These scenarios have been selected for a detailed analysis using the finite element head model which allowed for the examination of dynamic effects on individual head structures. In this paper, the authors described stages of the development of the anatomical finite element head model. Results: The results of the simulations made it possible to assess parameters determining the head injury of the soldier during the IED explosion. The developed model allows the determination of the parameters of stress, strain and pressure acting on the structures of the human head. Conclusion: In future studies, the model will be used to carry out simulations which will improve the construction of the headgear in order to minimize the possibility of the head injury.

The aim of this work was to perform the real case study for the US Surry Nuclear Power Plant which was touched down by tornado in 2011 causing the electrical switch yard destruction and loss of offsite power. Probabilistic methods have been applied to assess the reliability of the reactor shutdown and effective heat removal after this accident. The reactor protection system and auxiliary feedwater system were thoroughly analysed in the context of their safety features designed to prevent the reactor core damage. The emergency power system reliability has been also considered due to the fact that some components of the safety systems are electrically operated. Moreover, time-dependent analysis has been performed in order to address the level of damages after an extreme external event like tornado. Depending on the severity of such events the time required to restore the electrical grid may be significantly different and longer than 24 hours. The reliability and requirements for safety systems are changing with time and these changes have been taken into account as well.

Safety, along with the issue of security, is the most substantial and critical issue to any society and nation of the mankind. The fact that human elements contribute dominantly, in particular, to the transportation – marine, air and road traffic – accidents requires not only the technical approaches, but also cultural configuration inherent to the accidents. This paper tries to incorporate Hofstede’s dimensions of national culture to identify possible interaction with accidents. It will be of help to figure out the need to consider the aspect of national culture to be efficient in dealing with policies for safer communities and nations.

In the process of testing an electrosensitive protective system, determining its response time is of crucial importance. A unique double penetration method of measuring electrosensitive protective device (ESPD) response time has been worked out in the Central Institute for Labour Protection. In the first step, low speed penetration enables the detection zone border to be localised. In the second step of measurement, the probe is injected at a high speed and response time is measured. Three different ways have been taken for validation of the method: theoretical analysis, calibration of the stand, and taking a series of measurements. The double penetration method enables ESPD response time measurement results to be obtained, the accuracy, repeatability, and reproducibility of which is satisfactory enough to be assessed objectively.

Increasing cost of rotorcraft maintenance forces transport companies to utilize their rotorcraft fleet to the fullest. This means that the most successful rotorcraft production company is the one that can provide rotorcraft that can operate in wider range of weather conditions than competition. Air transport authorities define requirements for both rotorcraft performance during flight in icing conditions and reliability of ice accretion protection systems. At the same time production company management requires that production, and development costs are as low as possible. This paper will focus on problems of meeting requirements of rotorcraft ice protection systems reliability using various types of reliability analyses that will help keep the system as simple and inexpensive as possible and are required in certification process of ice protection system.
Ciągłe zwiększanie się kosztów utrzymania wiropłatów wymusza na firmach transportowych maksymalne wykorzystanie możliwości posiadanej floty. Oznacza to, że producent wiropłatów, aby osiągnąć sukces musi dostarczać produkt, który będzie w stanie operować w szerszym zakresie warunków pogodowych, niż firmy konkurencyjne. Organy nadzoru lotniczego określają zarówno wymagania dla własności lotnych wiropłatów w czasie lotu w warunkach oblodzenia oraz wymagania niezawodnościowe układu ochrony przed oblodzeniem. Jednocześnie zarządzający firmami produkującymi, wymagają, aby koszty produkcji i opracowania projektu były możliwie jak najniższe. Niniejsze opracowanie skupia się na problematyce sprostania wymaganiom niezawodnościowym przy wykorzystaniu różnych typów analiz niezawodnościowych, które pozwolą na zaprojektowanie możliwie najprostszych i najtańszych układów, i które są wymagane w procesie certyfikacyjnym układu ochrony wiropłata przed oblodzeniem.

The maritime industry plays a crucial role in the global economy, with roughly 90% of world trade being conducted through the use of merchant ships and more than a million seafarers. Despite recent efforts to improve reliability and ship structure, the heavy dependence on human performance has led to a high number of casualties in the industry. Decision errors are the primary cause of maritime accidents, with factors such as lack of situational awareness and attention deficit contributing to these errors. To address this issue, the study proposes an Ant Colony Optimization (ACO) based algorithm to design and validate a verified set of instructions for performing each daily operational task in a standardised manner. This AI-based approach can optimise the path for complex tasks, provide clear and sequential instructions, improve efficiency, and reduce the likelihood of human error by minimising personal preference and false assumptions. The proposed solution can be transformed into a globally accessible, standardised instructions manual, which can significantly contribute to minimising human error during daily operational tasks on ships.

The main area which Formal Safety Assessment (FSA) methodology was created for is maritime safety. Its model presents quantitative risk estimation and takes detailed information about accident characteristics into account. Nowadays, it is broadly used in shipping navigation around the world. It has already been shown that FSA can be widely used for the assessment of pilotage safety. On the basis of analysis and conclusion on the FSA approach, this paper attempts to show that the adaptation of this method to another area-risk evaluating in operating conditions of buildings-is possible and effective. It aims at building a mathematical model based on fuzzy logic risk assessment with different habitat factors included. The adopted approach lets us describe various situations and conditions that occur in creating and exploiting of buildings, allowing for automatic control of the risk connected to them.

W artykule przedstawiono szacunkowe zestawienia wyników analiz zagrożeń, awarii i katastrof, jakie wystąpiły na terenie Polski w latach 1962–2019. Wyniki zestawiono w zależności od charakteru obiektów, rodzaju uszkodzeń lub zniszczeń oraz rodzaju materiałów. Wskazano również najbardziej powszechne błędy projektowe wpływające na zagrożenia, awarie i katastrofy oraz wymieniono elementy, których powinna dotyczyć poprawa projektowania. Wnioski z analiz powinny stanowić podstawy do innowacyjnych rozwiązań w pracach badawczo-wdrożeniowych.
The paper presents the estimated results of the analyzes risk of failure and building damage that occurred in Poland in the years 1962-2019. The results were compiled depending on the nature of the objects, the type of damage or destruction and the type of materials. The most common design errors affecting failures and disasters were also indicated, and elements that should be improved in design were listed. Conclusions from the analyzes should constitute the basis for innovative solutions in research and implementation works.

According to Polish nuclear law, newly emerging nuclear facilities require probabilistic safety assessment (PSA). This article is intended to present the PSA method and to present the error tree method by which the probability of unavailability of the gravity reactor cooling system (GDCS) of the ESBWR power plant designed by GE Hitachi was determined. This work includes creatiion process of a damage tree and performing a quantitative analysis in SAPHIRE tool and estimating uncertainty using the Monte Carlo method. As a part of the work, it was shown that in the probability of failure of a single GDCS P LINE-A line, the most important element are the basic events related in particular to the operation of service valves.

The ship domain is one of the criteria for navigational safety assessment. It is particularly important in restricted areas with high intensity traffic, where the criteria of closest point of approach (CPA) and time to CPA are difficult to apply. This research continues to examine ship domains in Traffic Separation Schemes (TSS). We have analyzed precautionary areas established within TSSs in connection with changed arrangements of vessel traffic. Besides, we have defined ship domains in a precautionary area of a specific TSS, and compared them to domains of vessels proceeding along traffic lanes.

Efektem tego jest duże zagęszczenie podwodnej liniowej infrastruktury przesyłowej (LIP) - rurociągów czy kabli podmorskich. Obszarami szczególnie niebezpiecznymi w zakresie eksploatacji LIP są rejony przybrzeżne, gdzie wzmożona jest intensywność ruchu statków. W celu zminimalizowania ryzyka uszkodzeń sieci, należy już podczas projektowania, a następnie budowy, przeprowadzić szereg analiz dotyczących m.in. aspektów wytrzymałości i doboru materiałów, lokalizacji w aspekcie bezpiecznej nawigacji, wpływu warunków pogodowych oraz intensywności ruchu statków na powierzchni z uwzględnieniem ich ewentualnego wpływu podczas przepływania nad rurociągiem. W artykule skupiono się na ostatnim z wymienionych czynników. Autorzy omówili dostępne metody analizy ryzyka (FSA), wybrali optymalną do przedstawionego problemu, rozpatrzyli możliwe scenariusze zagrożeń i zaproponowali działania niezbędne do uniknięcia bądź zminimalizowania wystąpienia takich przypadków.
Recent technological advance combined with the minimization of costs and a raise in profitability of production of new investments, has in recent years caused an intensive development of the offshore sector in Poland, Europe and all over the world. The result of that, is a big density of underwater transmission pipelines and cables especially in coastal areas where traffic intensity is posing a potential threat to the safety of underwater infrastructure. To avoid or minimize the risk of pipeline damage, a significant number of analyses are needed to be conducted, for example aspects of material strength and its selection, location, impact of the weather conditions and intensity of the vessels movements on the sea surface, taking into account their possible impact when passing over the pipeline. This article focuses on the last of the above mentioned factors. The authors discussed the available methods of risk analysis (FSA), choose the optimal way to solve the problem, considered possible threats scenarios and suggested the necessary moves to avoid or minimize the occurrence of such cases.

Opis:
Transport of dangerous goods are always under critical observation of the public, in particular in case of transport of spent fuel or radioactive waste. In both cases transports are often crossing borders of countries, e.g., waste resulting from reprocessing of spent fuel. The transports could take place by ships, trucks, rails and airplanes and these options and the resulting risks are compared. Transport risk includes health and safety risks that arise from the exposures to workers and members of the public to radiation from shipments Moreover, it is shown that the more modern approach of risk-informed decision making elaborated for application to nuclear installations can also be applied to assess the risk of the transport of radioactive material.

Opis:
The MUNIN project is doing a feasibility study on an unmanned bulk carrier on an intercontinental voyage. To develop the technical and operational concepts, MUNIN has used a risk-based design method, based on the Formal Safety Analysis method which is also recommended by the International Mari-time Organization. Scenario analysis has been used to identify risks and to simplify operational scope. Systematic hazard identification has been used to find critical safety and security risks and how to address these. Technology and operational concept testing is using a hypothesis-based test method, where the hypotheses have been created as a result of the risk assessment. Finally, the cost-benefit assessment will also use results from the risk assessment. This paper describes the risk assessment method, some of the most important results and also describes how the results have been or will be used in the different parts of the project.
