The International Atomic Energy Agency has started a few years ago a project to enhance the existing framework for safety goals for nuclear installations and currently is finalizing a publication aimed to provide guidance for a more holistic approach to establishing and utilizing safety goals for nuclear installations. The intention was to develop a more consistent and holistic framework for safety goals that would be composed of a hierarchical structure of qualitative concepts (e.g. defense in depth, various safety requirements) and quantitative risk metrics. The safety goals should be technology neutral at the higher level and should cover both normal operation and accident conditions as well as the risk to workers, public, and the environment. In the paper the development of a hierarchical framework for safety goals is described including a methodology to derive lower-(more technical) safety goals from the higher goals. Moreover a process for assessing the degree of compliance with the safety goals and their consistent use is provided. As an example the developed framework is applied to the German regulatory framework for nuclear installations but focused on nuclear power plants.

This paper aims to show a practical case study of safety assessment concerning possible injuries and fire occurring in the engine room on-board a merchant ship, due to fatigue. Methods The methodology was centred in literature hazard identification, measurements in real ships and heat stress indices definition. Results Some corrections are proposed in order to obtain a safe environment for workers in the engine control room and to prevent fatigue risks at the engine room. This will allow adequate oil leakage detection and, as a consequence, improve fire prevention. Discussion Training is a preventive control option, but this study allows the understanding of new corrective control options like design corrections.

In parallel with the dynamic development of rail transport in terms of vehicle design, control systems, infrastructure issues, the development of processes and procedures in the area of safety management must also progress. This growing awareness was confirmed, among other things, by harmonising the content of the regulations on railway safety and interoperability, the safety certification of railway undertakings and the definition of the tasks and roles of national safety authorities. In effect, this was to enable the development of a single European railway area. An implementation tool that allows for a systematic approach to safety management processes and that enables the above requirements to be met is, among other things, RAMS analyses. A key step in the safety management process for specifying RAMS is hazard identification, which is particularly highlighted in PN EN 50126-2:2018 through the holistic model for risk assessment and control of railway system hazards (hourglass model). It places the hazard identification process in two stages, i.e. in the early development stage of the analysis (similar to other known risk management models) and in the hazard control stage. This positioning alone indicates the importance of the hazard identification process. On the comprehensiveness and detail of its implementation depends the validity of the final outcome of the RAMS analysis. The development of such a process for real technical facilities in a way that is consistent with the needs of RAMS analyses and, at the same time, ensures that satisfactory results of these analyses are achieved, has become the subject of this article.

The objective of this work is to evaluate the safety of adult and child passengers exposed to a radio frequency (RF) source, i.e., a leaky coaxial cable (LCX) on the subway platform. An adult model, a child model, and an LCX model have been numerically designed in COMSOL Multiphysics software. The distributions of the induced electric field (E-field), specific absorption rate (SAR), magnetic field ( H-field) and the head temperature increase in adult and child passenger models were calculated at 900 MHz. The induced fields in the passengers were compared with that without screen doors. The results show that the E-field, SAR and H-field in the whole body of the child are 2.00 × 10 -2 V/m, 1.07 × 10 -7 W/kg, and 2.94 × 10 -4 A/m, respectively. The E-field, SAR and H-field in the central nervous system of the child are 1.00e × 10 -2 V/m, 2.44 × 10 -8 W/kg, and 2.41 × 10 -4 A/m, respectively. The maximum values of the E-field, SAR and H-field in the adult passenger are 1.49–2.34 times higher than those of the child. The E-field, SAR, and H-field in the passenger models without a screen door are larger than those with a screen door. The screen door has a partial shielding effect on the RF electromagnetic field. The values of the maximum temperature that increases in adult and child head tissue are 0.2114 and 0.2111℃ after waiting 6 minutes exposure, respectively. All calculated results are well below the International Commission on Non-Ionizing Radiation Protection (ICNIRP) limits for general public exposure, indicating that RF electromagnetic exposure caused by the LCX on the subway platform is not a threat to passenger’s health.

The necessity of verifying manoeuvring mathematical models in shallow water for studying the safety of ships using a ship-handling simulator is pointed out in this report. Several instances of verification of mathematical models in shallow water are introduced here based on measurements of motion conditions of full-scale ships and shallow water tank tests of models. Results of safety assessment tests of five manoeuvring phases are given using the verified manoeuvring mathematical models to discuss manoeuvring criteria in shallow water. Objective manoeuvring criteria for safety assessment in shallow water are proposed based on subjective judgement related to control margin assessed by more than 3,200 masters and pilots for over 325 simulation tests and by analysis of the study results of 15 full-scale ships.

"Safe voyage from berth to berth" — this is the goal of all e-navigation strains, driven by new technologies, new infrastructures and new organizational structures on bridge, on shore as well as in the cloud. To facilitate these efforts suitable engineering and safety/risk assessment methods have to be applied. Understanding maritime transportation as a sociotechnical system allows system engineering methods to be applied. Formal and simulation based verification and validation of e-navigation technologies are important methods to obtain system safety and reliability. The modelling and simulation toolset HAGGIS provides methods for system specification and formal risk analysis. It provides a modelling framework for processes, fault trees and generic hazard specification and a physical world and maritime traffic simulation system. HAGGIS is accompanied by the physical test bed LABSKAUS which implements a reference port and waterway. Additionally, it contains an experimental Vessel Traffic Services (VTS) implementation and a mobile integrated bridge enabling in situ experiments for technology evaluation, testing, ground research and demonstration. This paper describes an integrated seamless approach for developing new e-navigation technologies starting with virtual simulation based assessment and ending in physical real world demonstrations.
„Bezpieczna podróż od nabrzeża do nabrzeża” — to cel całego procesu e-nawigacji, napędzanego nowymi technologiami, nową infrastrukturą i nową organizacją pracy na mostku, na lądzie, ale także w technologii chmury znanej z informatyki. By ułatwić te wysiłki, należy zastosować również odpowiednie metody ocena ryzyka. Rozumiejąc transport morski jako system socjotechniczny, można do tych celów zastosować metody inżynierii systemów. Formalna ocena ryzyka, a także oparta na symulacjach weryfikacja i walidacja technologii e-nawigacji to ważne metody oceny bezpieczeństwa i niezawodności systemu. Modelowanie i zestaw narzędzi symulacyjnych HAGGIS okazują się odpowiednimi instrumentami do opisu systemu i formalnej analizy ryzyka. Wyznaczają ramy dla modelowania procesu, sporządzenia drzewa błędów i generowania wykazu zagrożeń, jak również otoczenia fizycznego i symulacji ruchu morskiego. HAGGIS jest wspierany przez rzeczywisty poligon testowy LABSKAUS, który implementuje port i drogi wodne. Dodatkowo obejmuje on eksperymentalny system nadzoru ruchu (VTS) oraz mobilny zintegrowany mostek umożliwiający realne eksperymenty dla oceny technologii, testowania i demonstracji. W artykule opisano zintegrowane, spójne podejście do rozwijania nowych technologii e-nawigacji, zaczynając od wirtualnej symulacji, a kończąc na demonstracjach w fizycznej rzeczywistości.

Industrial safety is an important issue in Thailand, and attempts have been made to improve safety performance and accident records. This paper examines key criteria influencing safety improvement. Exploratory factor analysis confirms 9 safety criteria, including 5 “enablers” and 4 “results”, with a total of 47 associated attributes. A safety assessment approach is developed, using those 9 key criteria, to measure an organization’s current safety maturity level. Organizations can use the assessment approach to plan its safety improvement, and progress through to higher maturity levels by focusing on the weakest criteria shown in the assessment results with the lowest scores.

International experience has shown that external hazards (e.g. aircraft crash, flooding) can be safety significant contributors to the risk in case of nuclear power plants` operation. This is due to the fact that such hazards have the potential to reduce simultaneously the level of redundancy by damaging redundant systems or their supporting systems. In this paper, the procedure for the external hazard aircraft crash is described In more detail, starting with the screening procedure in order to determine scope and content of the assessment and the approach for those cases where a full scope analysis has to be performed. The consideration regarding this hazard does as not cover an intended aircraft crash.

Celem autorów jest również uczynienie Wytycznych FSA bardziej dostępnymi, tj. zrozumiałymi dla potencjalnych użytkowników. W tekście tym zamieszczono zatem wiele interpretacji mniej oczywistych pojęć, procedur i zasad dotyczących bezpieczeństwa morskiego i ochrony.
The paper attempts to illustrate principles of Maritime Formal Security Assessment to assess the security in Polish maritime areas. The authors’ aim is also to make the Guidelines of FSA more accessible, i.e. easier to understand by potential users. Therefore the text also includes several interpretations of less obvious notions, procedures and principles related to maritime security and protection.

The paper presents some current results of the research on development of a method for safety assessment of ships in critical conditions at the preliminary design stage. The meŹthod is expected to make it possible to perform the multi-objective and multi-parametric investigations to achieve the required and optimum levels of safety by using the risk-based and formal approaches to ship safety. During the design analysis influence of parameters associated with the hull form, arrangement of internal spaces, loading conditions, position and extent of damage, cargo shift and weather impacts on safety of ships in critical condiŹtions will be taken into account. Current capabilities of the method are illustrated by several calculation examples of influence of different arrangements of internal spaces of a cargo ship on values of the IMO formal subdivision indices such as the "Attained Subdivision Index A" and "Local Subdivision Indices ?Aj". The method is still under development and its final structure will be presented in the near future.

The article presents an exemplary application of the multi-criteria navigational safety assessment method for the proposed variants of the designed waterway in order to verify it. This method comprehensively considers the most important environmental and technical aspects related to the implementation of the planned investment. The method can be used for the purposes of navigational analysis, i.e. an engineering document that meets the requirements of the regulation of the competent, which subject to agreement with the Director of the territorially competent Maritime Office.

Formalna Analiza Ryzyka (FSA) jest skuteczną metodą mającą na celu zwiększenie bezpieczeństwa morskiego. Szczególnie zwraca uwagę na ochronę życia, zdrowia, środowiska morskiego i mienia, wykorzystując analizę ryzyka, metody jego kontroli i ocenę kosztów. Celem FSA jest wspieranie procesu zarządzania bezpieczeństwem, standaryzacji bezpieczeństwa oraz ustalanie polityki działań na rzecz bezpieczeństwa. Metoda ma charakter ogólny i może być wykorzystana, jako narzędzie wspomagające podejmowanie decyzji w zakresie tworzenia nowych regulacji prawnych, projektowania, klasyfikacji, budowy i eksploatacji statków. Metoda nadaje się do analizy pojedynczego statku lub typu statków, akwenów, np.: tankowców, zatok, portów, itp. W artykule przeanalizowano dokumenty międzynarodowe dotyczące FSA.
A Formal Safety Assessment (FSA) is an effective method that is aimed to increase maritime safety. Particularly draws attention to the protection of life, health, property and the marine environment, using risk analysis, methods of its control and the assessment of costs. The main purpose of FSA is to support the process of security management, the standardization of safety and determining safety action policy. The method is general and can be used as a tool to assist decision making in the field of the creation of new legal regulations, classification, design, construction and operation of ships. Method is suitable for the analysis of a single vessel or type of vessel, such as: oil tankers, gas tankers or different areas, for example bays, ports, jetties, etc. The article analyses the international instruments relating to the FSA.

This paper addresses the improvements to the reliability of the safety systems of nuclear reactors using redundancy allocation technique. The study has been carried out using the Probabilistic Safety Assessment (PSA). PSA involves, among others, the use of fault and event tree tools in the evaluation of the safety system failure probabilities and the quantification of annual occurrence probability of the accidental conditions postulated in the design of the nuclear reactors. The PSA has been presented and discussed. The Egypt Second Research Reactor, ETRR-2, has been used as a case study. The failure probability of the already existing safety systems has been reviewed. The effect of the allocation of more redundant components to the existing safety systems on the failure probability of the systems has been evaluated. The event trees for two selected initiating events, from those events postulated in the ETRR-2 design, have been studied considering the allocation of more redundant components to the safety systems. The result of the study showed that further improvement could be introduced to the reliability of the Confinement Ventilation System (CVS).
