Aim: To investigate the potential value of Doppler ultrasound and to assess cerebroplacental ratio (CPR) in the prediction of adverse perinatal outcome defined as Apgar score < 7 at 1 minute. Material and methods: This was a retrospective cross-sectional study in selected pregnant women undergoing an ultrasound examination between 36 and 42 weeks of gestation. We measured estimated fetal weight (EFW), mean umbilical artery pulsatility index (UA PI), mean middle cerebral artery pulsatility index (MCA PI), CPR, and Apgar score in 1 minute. Multiples of medians (MoM) were calculated for MCA PI and UA PI. Results: The study group consisted of 446 women, 236 were primipara and 210 were multipara. The average age was 29.6 years (range 16–46 years). The average week of delivery is 39.5 weeks of gestation (range 36–42). Mean MCA PI and UA PI were 1.3 (0.1–2.45) and 0.8 (0.39–1.66), respectively. The mean values were 1.03 (0.1–1.9) for MCA PI MoM and 1.04 (0.5–2.1) for UA PI MoM. Primiparas had lower values of MCA PI (1.27 vs. 1.34), MCA PI MoM (1.00 vs. 1.05), CPR (1.62 vs. 1.73), EFW (3479.53 g vs. 3579.25 g) and birth weight (3513.50 g vs. 3617.79 g). For CPR cut-off point of 1.08: sensitivity was (0.945), specificity 0.1, positive predictive values 0.979, negative predictive values 0.04 and accuracy 0.926. The ROC curves for CPR were: area under the curve was 0.52 at CI 95% (0.342–0.698), p = 0.8271. Conclusion: Screening in pregnancies with appropriate-for-gestational-age fetuses at 36–42 weeks of gestation using Doppler parameters is not useful in the prediction of adverse perinatal outcomes like an Apgar score < 7 at 1 minute.

Background: Peripheral arterial disease is an atherosclerotic disease characterized by an increase in morbidity and mortality. For these reasons early diagnosis of peripheral arterial disease is important. Ankle-brachial systolic pressure index measurement is frequently used in screening studies. Evaluating waveforms of distal lower extremities with Doppler ultrasound can be used as a screening program and provides more accurate information on peripheral arterial disease. Aim: We investigate the prevalence of peripheral arterial disease, compare the efficacy of Doppler ultrasound evaluation of distal lower extremity waveforms and ankle-brachial systolic pressure index measurement in screening programs, and discuss the importance of early diagnosis of asymptomatic cases. Material and methods: A total of 457 patients over the age of 65 (between 65 and 94, mean age: 71.4) including 270 males and 187 females were examined with Doppler ultrasound, had ankle-brachial systolic pressure index measurement taken and were screened for peripheral arterial disease. The correlation between Doppler ultrasound findings and ankle-brachial systolic pressure index was examined. Results: According to the Doppler ultrasound findings, in the aortoiliac (r = 0.648) and femoropopliteal (r = 0.564) area, there is a medium level of correlation between severe stenosis and occlusions and a low ankle-brachial systolic pressure index value, and a low level of correlation between such abnormalities in the tibioperoneal region (r = 0.116) and a low ankle-brachial systolic pressure index value. Therefore, while the sensitivity of ankle-brachial systolic pressure index increases in proximal stenosis, it decreases in distal stenosis. Conclusion: Despite the fact that ankle-brachial systolic pressure index is a diagnostic test commonly used in screening studies, evaluation of distal arteries by means of Doppler ultrasound provides more accurate information in terms of the identification of peripheral arterial disease.
Wprowadzenie: Choroba tętnic obwodowych to stan wywołany przez zmiany miażdżycowe, charakteryzujący się rosnącą zachorowalnością i umieralnością. Z tego powodu ważna jest wczesna diagnostyka. W badaniach przesiewowych często stosuje się wskaźnik ciśnienia skurczowego krwi kostka–ramię. Ocena zapisu badania przepływu krwi w dystalnych odcinkach kończyn dolnych za pomocą ultrasonografii dopplerowskiej może być wykorzystywana jako badanie przesiewowe i zapewnia dokładniejszą diagnostykę choroby tętnic obwodowych. Cel pracy: W pracy badano częstość występowania choroby tętnic obwodowych, porównywano skuteczność oceny przepływu krwi w dystalnych odcinkach kończyn dolnych za pomocą ultrasonografii dopplerowskiej i pomiaru wskaźnika kostka–ramię w badaniach przesiewowych oraz omówiono znaczenie wczesnej diagnostyki bezobjawowych przypadków choroby. Materiał i metody: W badaniu przesiewowym pod kątem choroby tętnic obwodowych z wykorzystaniem ultrasonografii dopplerowskiej oraz pomiaru wskaźnika kostka–ramię wzięło udział łącznie 457 chorych w wieku powyżej 65 lat (65–94, średnia wieku: 71,4) – 270 mężczyzn i 187 kobiet. Zbadano korelację pomiędzy wynikami badania ultrasonograficznego a pomiarem wskaźnika kostka–ramię. Wyniki: Na podstawie wyników badania ultrasonografii dopplerowskiej wykazano średni stopień korelacji pomiędzy znacznym zwężeniem lub niedrożnością w obszarze aortalno-biodrowym (r = 0,648) oraz udowo-podkolanowym (r = 0,564) a niską wartością wskaźnika kostka–ramię, z kolei w przypadku takich nieprawidłowości w obszarze piszczelowo-strzałkowym (r = 0,116) wykazano niski stopień korelacji z niską wartością wskaźnika kostka–ramię. Czułość wskaźnika kostka–ramię zwiększa się zatem w przypadku stenozy proksymalnej, natomiast w przypadku stenozy dystalnej jest niższa. Wniosek: Mimo że pomiar wskaźnika kostka–ramię to często stosowany test diagnostyczny w badaniach przesiewowych, ultrasonografia dopplerowska stosowana w ocenie tętnic dystalnych dostarcza dokładniejszych danych pod kątem diagnostyki choroby tętnic obwodowych. The study presented an approach to the morphometric image of atherosclerotic lesions of the final segment of the abdominal aorta, femoral and iliac arteries, considering possible endovascular intervention. The evaluation of these arteries is very important, because they are often used as a point of access for endovascular procedures performed on the peripheral arteries, or within the thoracic and abdominal aorta and its branches, as well as coronary arteries. The aim of the study was to determine morphometric measurements describing the atherosclerotic lesions, including the methodology of their surgical interpretation. Material and methods. The study group comprised 128 tomograms of patients qualified for surgery. An algorithm based on the mathematical morphology was designed to track the vessels, starting from the division of the common femoral artery, and ending at the bifurcation of the abdominal aorta. We proposed a set of numerical measurements of the observed arterial changes. Results and conclusions. We analysed 128 tomograms with a 94.5% efficiency, and with the assessment accuracy of the degree of lumen reduction (MAE- 1.5%). We observed much higher measurement values of local tortuosity of the atherosclerotic arteries (0.3 - 1 radians), as compared to their anatomical course in a healthy subject (0 - 0.2 radians). The presented method can be a very accurate and useful tool in the numerical analysis of the lumen distribution of the arteries and atherosclerosis, dedicated to surgeons elaborating management strategies.

Introduction. Although there are lots of varieties of aorta arch, they seem to appear relatively rarely. Anomalies of aortic arch departures are mainly concerned with its location, course, the place of departure and number of its main branches. However, they warrant attention due to their importance in operative, diagnostic, and endovascular procedures. Aim. We want to present here a case of a female cadaver with rare aortic arch origin of the left vertebral artery. The aim of this article is to complete the frame of anomalies in aortic arch and to explore rare variability shown in this specific case. Description of the case. The present report describes an anomalous case of the left vertebral artery arising from the aortic arch between the left common carotid artery and the left subclavian artery in a female cadaver during dissection in an anatomical laboratory. Aortic origin of the vertebral artery is a rare anatomic variant. Conclusion. Thorough knowledge of anomalous origin is important for patients who undergo operation of an aortic arch or inferior part of the neck. Normally, the vertebral artery arises from the first part of the subclavian artery on both sides.

This paper presents results of examinations of the intima-media thickness (IMT) in human common carotid arteries. Ultrasonic examinations were carried out on healthy volunteers with the use of the apparatus Vascular Echo Doppler (VED), designed in IFTR-PAS to measure the elasticity of arteries. Application of the PDA-14 PC-card (Signatec) allowed for the acquisition of ultrasonic RF signal from the output of the apparatus VED and for further analysis of dynamic changes of the IMT during a heart cycle. Changing of the IMT in time as a difference between the instantaneous position of the two tracking slopes of RF echoes was obtained. For this purpose the zero-crossing method, for tracking phase changes of two characteristic rising slopes of the RF ultrasonic echo, was used.

Introduction. Sciatic vessels most often accompany the sciatic nerve. Sciatic vessels are very rare. Aim. In this paper we determine the procedure in sciatic vessels surgical treatment. Description of the case. We present the case of a 75-year-old patient with symptoms of acute right lower limb ischemia. The patient was discharged home in good condition, and remains in outpatient control to this day. Conclusion. The popliteal artery proved to be available, but much deeper than usual.

Introduction: Ischemic heart disease is the most common cause of death in the world. The lives of patients with vascular defects can be saved by coronary artery bypass grafting (CABG). However, it is associated with an increased risk of developing depression after surgery. Meterial and Methods: The aim of the study is to present the results of the latest research on postoperative depression after CABG, including studies describing the course of the disease, its consequences for the patient’s prognosis and treatment. The publications available on the PubMed platform published after 2011 were reviewed. Results: Depression before and after CABG affects 30–40% of patients, mostly women. Established after surgery and untreated, it persists for many years. The level of anxiety in patients decreases systematically after surgery. Indicators that may correlate with the patient’s postoperative depression, including cortisol, high sensitivity C-reactive protein (hsCRP) and oxidative stress biomarkers, are being investigated. The occurrence of depression in patients after CABG has a number of negative consequences. Those include: weaker response to treatment, greater chance of relapse, and increased readmission frequency and mortality. Treatment of patients with this disorder involves the use of antidepressants (most often SSRIs – selective serotonin reuptake inhibitor) and/or various types of psychotherapy with cognitive behavioral therapy (CBT) at the forefront. Conclusions: Depression following CABG decreases the quality of life and worsens patient prognosis. It is necessary to detect this condition early after surgery and to apply treatment, taking into account the cardiological disorders of the patient.

Background. Currently, there is little documented research evaluating the effect of a high-mountain environment on patients with ischemic heart disease. Objectives. The main aim of the study was to assess the effect of normobaric hypoxia on exercise tolerance in patients diagnosed with stable coronary disease. Material and methods. 22 men aged 37 to 72 (55.68 ± 9.86 years of age) with coronary disease were qualified. In the pre-study, in a normobaric normoxia environment, each patient underwent: resting ECG, spiroergometric test using a treadmill, laboratory tests (gasometry, lactic acid concentration). The patients stayed in the cabinet for 3 hours at the: 1) normoxia, 2) hypoxia (2000 m a.s.l), 3) hypoxia (3000 m a.s.l.) levels. After the 3-hour period, patients underwent a spiroergometric exercise tolerance test combined with a blood lactic acid concentration test. Venous blood and capillary blood were drawn for gasometry testing purposes. Results. Under 2000 and 3000 m hypoxia noted a significantly shorter duration of the exercise test, distance travelled and MET values. An increase in resting blood pH and a decrease of resting and peak pCO2 and pO2 were observed. Conclusions. As a result of a 3-hour exposure to normobaric hypoxia, the exercise tolerance of patients after acute coronary syndrome treated with angioplasty combined with coronary stent implantation decreases. There is no clear information for patients as to whether high mountain conditions are safe for them. The presented research was a form of introduction to wider and more thorough experiments that can result in practical information for patients.

Opis:
Knowledge of mechanical behavior of healthy human arteries as the guidelines to target properties of vascular grafts deserves special attention. There is a lack of mathematical model to characterize mechanical behavior of biomaterial while many mathematical models to reflect mechanics of human arteries have been proposed. The objective of this paper was set to measure mechanical properties of healthy human arteries including Common Carotid Artery (CCA), Abdominal Aorta Artery (AAA), Subclavian Artery (SA), Common Iliac Artery (CIA) and Right and Left Iliac Artery (RIA and LIA) and compare them to those of commercial ePTFE and Dacron®. Methods: Series of stress relaxation and strain to failure tests vere performed on all samples. The experimental data was utilized to develop quasi-linear viscoelastic (QLV) model of both natural and artificial arteries. Results: ePTFE is the stiffest sample, while the CCA is the most compliant one among all. RIA and CIA are more viscous than the other natural arteries, while AA and CCA are less viscous. The proposed model demonstrated an accurate fit to the experimental results, a proof of its ability to model both nonlinear elasticity and viscoelasticity of the human arteries and commercial ones. Conclusions: ePTFE and Dacron® are much stiffer than human arteries that may lead to the disruption of blood hemodynamic and may not be biomechanically feasible as a replacement.

Classification and regression trees are very popular and attractive types of classifiers, widely used to solve decision-making problems in different fields of science. The study was conducted to identify preoperative risk factors associated with morbidity outcome among patients undergoing isolated Coronary Artery Bypass Grafting (CABG) and to develop some classification rules assigning patients to selected risk subgroups. Prediction rules were established on the basis of the selected tree-structured models. The following tree-based algorithms were used: QUEST, CRUISE, LOTUS and PLUS.
Drzewa klasyfikacyjne i regresyjne należą do bardzo popularnych metod klasyfikacji, przede wszystkim ze względu na prostotę interpretacji i przejrzystą formę wizualizacji wyników. Stąd też są one szeroko wykorzystywane do rozwiązywania problemów decyzyjnych w różnych dziedzinach nauki. Celem prowadzonych badań była identyfikacja przedoperacyjnych czynników ryzyka, związanych z wystąpieniem powikłań śród- i pooperacyjnych wśród pacjentów z chorobą wieńcową, leczonych w sposób operacyjny. Dodatkowo podjęto próbę zdefiniowania reguł decyzyjnych, które mogłyby umożliwić przydzielenie pacjenta do jednej z wyróżnionych grup ryzyka operacyjnego na podstawie opisujących go cech przedoperacyjnych. Reguły klasyfikacyjne budowano wykorzystując metodę rekurencyjnego podziału. W analizie uwzględniono algorytmy QUEST i CRUJSE, tworzące drzewa klasyfikacyjne oraz algorytmy LOTUS i PLUS, łączące rekurencyjny podział przestrzeni cech z analizą regresji logistycznej.

Knowledge of anatomical variants in hepatic vascular structures is obligatory in surgical practice. The purpose of this study is to describe anatomic variations in the Left Hepatic Artery (LHA). In ~60% of cases, LHA is a branch of the proper hepatic artery. The main anatomical variant is an Aberrant Left Hepatic Artery (ALHA) which stems from the Left Gastric Artery, also known as Hyrtl’s artery. The proper identification of ALHA is of importance in the planning and performance of all surgical procedures in the upper abdomen.
Znajomość zmienności anatomicznej struktur naczyniowych jest obowiązkowym elementem w praktyce chirurgicznej. Celem artykułu jest opisanie odmian anatomicznych lewej tętnicy wątrobowej (LHA). W ok. 60% przypadków LHA to gałąź tętnicy wątrobowej właściwej. Najczęstszą zmiennością anatomiczną jest wariant lewej tętnicy wątrobowej (ALHA) odchodzącej od lewej tętnicy żołądkowej, tzw. tętnica Hyrtla. Prawidłowa identyfikacja ALHA jest istotna w planowaniu i przeprowadzaniu wszystkich zabiegów chirurgicznych w górnym piętrze jamy brzusznej.

Proteoglycans (PGs) were dissociatively extracted from human umbilical cord arteries (UCAs) with 4 M guanidine hydrochloride containing Triton X-100 and protease inhibitors, purified by Q-Sepharose anion exchange chromatography and lyophilized. They were analysed by gel filtration, SDS/PAGE and agarose gel electrophoresis before and after treatment with chondroitinase ABC. It was found that the PG preparation was especially enriched in chondroitin/dermatan sulphate PGs. The predominant PG fraction included small PGs that emerged from Sepharose CL-2B with Kav = 0.74. Their molecular mass, estimated by SDS/PAGE, was 160-200 kDa and 90-150 kDa, i.e. it was typical for biglycan and decorin, respectively. Treatment with chondroitinase ABC yielded the core proteins of 45 and 47 kDa, characteristic for both small PGs. Remarkable amounts of the 45 kDa protein were detected in non-treated PG samples, suggesting the presence of free core proteins of biglycan and decorin. Large PGs were present in lower amounts. In intact form they were eluted from Sepharose CL-2B with Kav = 0.17 and 0.43. Digestion with chondroitinase ABC yielded the core proteins with a molecular mass within the range of 180-360 kDa but predominant were the bands of 200, 250 and 360 kDa. The large PGs probably represent various forms of versican or perlecan bearing chondroitin sulphate chains.

The aim of this paper is to compare the results of the mathematical modeling and experimental results of the ultrasonic waves scattering in the inhomogeneous dissipative medium. The research was carried out for an artery model (a pipe made of a latex), with internal diameter of 3, 5 and 8 mm and wall thickness of 0.75, 1.25 and 2 mm. The numerical solver was created for calculation of the fields of ultrasonic beams and scattered fields under different boundary conditions, different angles and transversal displacement of ultrasonic beams with respect to the position of the arterial wall. The investigations employed the VED ultrasonic apparatus. The frequency of the transmitted ultrasound was 6.75 MHz. The good agreement between the numerical calculation and experimental results was obtained. The numerical solver is used for verified proposed methods for determining of the IMT in the artery walls.

BACKGROUND Remifentanil is metabolized by non-specifi c esterases and is very shortacting. It is eliminated from all body compartments at the same time. AIM The aim of this study was to compare anesthetic induction with standard dose of etomidate and isofl urane combined either with fi xed rate remifentanil infusion or a single bolus dose of fentanyl. MATERIAL AND METHODS 54 patients (57.0±7.6 years) with stable CAD and EF > 40% scheduled for elective coronary revascularisation were recruited for this prospective, randomized trial. During induction, patients in group I received remifentanil infusion 0,5 mcg/kg/min., while group II received bolus dose 5 mcg/kg fentanyl. After initiation of remifentanil infusion or the injection of fentanyl, 0,2 mg/kg etomidate was given, followed by the injection of 0,1 mg/ kg pancuronium and the administration of 1% isofl urane. Haemodynamic parameters were measured before induction and after tracheal intubation. RESULTS Cardiac index decreased in both groups, heart rate and systemic blood pressure decreased only in remifentanil group, while systemic vascular resistance index increased only in fentanyl group. Heart rate, systemic blood pressure and systemic vascular resistance index after induction were significantly higher in fentanyl group. CONCLUSIONS Remifentanil is more potent than fentanyl in blunting a cardiovascular response to tracheal intubation in patients with coronary artery disease. Low dose of fentanyl, used for the anaesthetic induction, may result in a clinically important increase of systemic vascular resistance.

Arterial walls are a multilayer structures with nonlinear material characteristics. Furthermore, residua stresses exist in unloaded state (zero-pressure condition) and they affect arterial behavior. To investigate these phenomena a number of theoretical and numerical studies were performed, however no experimental validation was proposed and realized yet. We cannot get rid of residual stresses without damaging the arterial segment. In this paper we propose a novel experiment to validate a numerical model of artery with residual stresses. The inspiration for our study originates from experiments made by Dobrin on dogs’ arteries (1999). We applied the idea of turning the artery inside out. After such an operation the sequence of layer is reversed and the residual stresses are re-ordered. We performed several pressure-inflation tests on human Common Carotid Arteries (CCA) in normal and inverted configurations. The nonlinear responses of arterial behavior were obtained and compared to the numerical model. Computer simulations were carried out using the commercial software which applied the finite element method (FEM). Then, these results were discussed.

Primitive carotido-basilar anastomoses (PCA) are persistent fetal vessels. The aim of the study was to compare the clinical characteristics of patients operated on for internal carotid artery (ICA) stenosis with or without PCA in order to evaluate the impact of PCA on the treatment. Material and methods. Consecutive patients operated on for ICA stenosis at our university hospital were included. Surgical treatment consisted in carotid endarterectomy (CEA) with patch plastic. Results. Of the 380 CEA performed between 2006 and 2012, PCA were found in six patients (1.6%). All patients with PCA were symptomatic vs. 54% of patients without PCA (p=0.035). Significantly less posterior collateral flow was present in patients with PCA (33%) compared to those without PCA (85%, p=0.01). Only two of the six patients with PCA were diagnosed prior to surgery, none was ligated intraoperatively. PCA was not associated with stroke and restenosis at long-term follow up. Conclusions. PCA are rarely diagnosed prior to surgery in patients with ICA stenosis and need not to be ligated during CEA

The study presented a case of a 29 year-old male patient with a pseudoaneurysm of the splenic artery as a complication of acute pancreatitis. The pseudoaneurysm was incidentally diagnosed during control angio-CT. The patient underwent immediate surgery due to deterioration of his clinical condition. During laparotomy the pseudoaneurysm, spleen and part of the pancreatic tail were excised. The postoperative course proved uneventful and the patient was discharged from the hospital after seven days.

Smoking rates in coronary patients over the decade 1997–2007. Results of Cracovian Program for Secondary Prevention of Ischaemic Heart Disease and Polish parts of EUROASPIRE II and EUROASPIRE III surveysBackground: Smoking is one of the most important risk factors. Persisting smoking after an coronary event is related to significantly higher risk of the future cardiovascular complications. Studies performed in late nineties showed that a considerable percentage of patients continue with smoking after an coronary event. The aim of the present paper was to compare smoking rates in coronary patients in the post-discharge period in Krakow in 1997/1998, 1999/2000 and 2006/2007.Methods: Consecutive patients hospitalized from July 1, 1996 to September 31, 1997 (first survey), from March 1, 1998 to March 30, 1999 (second survey), and from April 1, 2005 to July 31, 2006 (third survey) due to acute myocardial infarction, unstable angina or for myocardial revascularization procedures, below the age of < 71 years were identified and then followed up, interviewed and examined 6–18 months after discharge. Self-reported smoking and breath carbon monoxide was analysed.Results: The number of patients who participated in the follow-up examinations were: 418 (78.0%) in the first survey, 427 (82.9%) in the second and 427 (79.1%) in the third survey. There was no significant change in smoking (self-reported) rates (16% vs 16% vs 19%; p = NS). When breath carbon monoxide was also analysed once again the difference did not reach significance (18% in 1999–2000 and 23% in 2006–2007; p = NS). Nicotine replacement therapy, bupropion or varenicline were also not used (0% vs 0.2% vs 0.5%; p = NS).Conclusion: The smoking rate in coronary patients over the decade from 1997/1998 to 2006/2007 did not changed significantly. The pharmacotherapy for smoking is almost not used in coronary patients.

The present work is aimed at comparing the effectiveness of two different methods of risk factor assessment used for prediction of the CAD (coronary artery disease): the logistic regression method and the application of artificial neural networks. The former is widely used in medical research, while the latter is relatively rare. In the logistic regression method hierarchical analysis was employed to select the significant variables of the classification process. In the neural network approach several strategies were proposed for selection of the discriminative variables, all based on weight analysis in the constructed networks. Both methods have produced a consistent set of discriminative variables (Glu0, Ins0, Ins30, BMI, apoA1 and HDL-Ch), belonging to three groups of risk factors associated with insulin resistance, obesity and lipid disorders.

Fast-tracking in cardiac care refers to the complex intervention including early extubation, care during anesthesia, mobilization and hospital discharge to reduce perioperative morbidity, costs, and length of stay in the intensive care unit and the hospital. This review was designed to evaluate early extubation protocols, the diefrences in early and late extubation, the safety and efficacy of early extubation among the patients in surgical intensive care aeftr coronary artery bypass graft (CABG) surgery. The analyzed studies showed many significant diefrences in the mortality and postoperavtie complicaotins of mtie-directed extubaotin praccties and low dose-based general anesthesia in patients with low to moderate risk undergoing early extubation (fast-track) and the conventional care methods. Diefrent fast-track interventions could diminish extubation time, costs, and the length of hospital stay. However, several factors including patient's stay in the intensive therapy ward vs general ward, patient selection, skills and experience of the sta,f and fast-track anesthesia methods could be considered to perform safe fast-tracking in patients undergoing cardiac surgery. On the other hand, to achieve this safety for high-risk cardiosurgery patients multidisciplinary coordination is needed.

Introduction. Isolated common iliac artery aneurysms (CIAs) are rare. The rarer are rupture of the common iliac artery. We can treat them surgically: classic or endovascular. Open surgery has a main role in the management of aneurysms not suitable for repair by endovascular techniques. Aim. The aim of the study is to present the possibility of occurrence of such a disease at a young age, to indicate non-characteristic symptoms that may predict the above-mentioned disease entity at the level of a primary care physician or SOR. Review of medical publications from 2000-2019 regarding guidelines or methods of conduct in that cases. Description of the case. A patient 39 years old, admitted to the Department of Vascular Surgery, in an interview: about 15 years ago severely beaten, as a result of injuries total loss of hearing, memory and basic skills such as: speech, writing, reading, hospitalized over 6 months. Conclusion. Good rehabilitation and patient persistence restored cognitive skills. Advances in surgical has lowered the mortality and morbidity rates even in patients.

Introduction. The technology of laser atherectomy has been known for about 20 years. Initial studies were not encouraging due to similar or lower efficacy compared to conventional angioplasty and greater complication rate. The introduction of new, low-profile catheters with high parameters of transmitted energy, a change in the procedure for performing angioplasty and the proper qualification of the lesions resulted in a reduction of adverse effects. Objective. The aim of the study was to present the current state of knowledge regarding the usage of excimer laser in coronary and peripheral arteries stenosis, its indications and contraindications as well as safety aspect. The article reviews 32 recent publications available on the PubMed and Google Scholar databases. Abbreviated description of the state of knowledge. The excimer laser emits ultraviolet energy and ablates the tissue in a vessel. The main indications for the use of the laser include: restenosis in the stent, ostial lesions, long, calcified lesions >20 mm and moderately and severely calcified lesions. There are isolated reports describing the use of ELCA in the treatment of acute coronary syndromes. Excimer laser also can be applied in a treatment of stenosis of peripheral arteries. Conclusion. Excimer laser coronary atherectomy is a safe and feasible technique for the treatment of ISR and is associated with a relatively low recurrent restenosis compared to scoring balloon dilatation alone. It is a very important and safe alternative to classic treatment methods, but further research assessing the usefulness of this procedure is required.

The aim of this paper is to describe a non-invasive method of examination of the local pulse wave velocity. The measurements were carried out in the model of the artery immersed in a water tank. Two synchronized ultrasonic apparatus VED with the ultrasonic radio frequency echoes acquisition system were used for evaluation of the arterial elasticity. The zero-crossing method was used for determination of the diameter changes of the artery model. The transit time between the waveforms of instant artery diameter was measured at two points of the artery model, separated by the distance of 5 cm. The transit time was determined using the criteria of similarity of the first derivatives of the raising slopes of the curves describing instant vessel’s diameter changes. The pulse wave velocity obtained by the proposed twopoint method was compared with the results obtained by the one-point method based on the modified Bramwell–Hill relation.

Peripheral artery disease (PAD) is associated with altered gait biomechanics. No previous research study has investigated the effect of activity on muscle activation in individuals with PAD. The purpose of this study was to investigate the effect of PAD on muscle activation in response to a ten-minute walking task. METHODS: Ten healthy young adults, ten healthy older adults and ten individuals with PAD performed a ten-minute treadmill walking trial at a self-selected velocity. Surface EMG was recorded from the vastus lateralis and medial gastrocnemius during five steps in the first and tenth minutes of the walking trial. EMG signals were rectified and smoothed using the root mean squared (RMS) with a 20 ms smoothing window. Peak RMS EMG and median frequencies (MdF) were calculated. Mixed-model ANOVAs with Tukey’s post-hoc was used to determine effects of group and activity on peak RMS EMG and MdF. RESULTS: PAD was associated with significantly greater reductions in MdF of the vastus lateralis compared to healthy young and healthy older adults. No significant differences were observed in peak RMS EMG. DISCUSSION: PAD is associated with exaggerated rates of fatigue in the quadriceps but not the gastrocnemius. Efficacy of evidence-based therapeutic interventions should be further investigated.

Background: The middle cerebral artery (MCA) supplies a major part of the brain and is of considerable clinical importance as a common location of intracranial aneurysms. The aim of the present study was to analyze the bilateral MCAs in patients with unilateral aneurysms. Material and methods: Images of computed tomography angiography of 45 patients were analyzed. The morphometric parameters of MCA were measured. The measured parameters of the normal MCAs were compared with MCAs containing aneurysms located in the main trunk division. Results: No significant differences in the length of trunk and angles between the middle and anterior cerebral arteries were found between the non-affected and aneurysmatic MCAs. Concerning arteries with aneurysms, more branches originating from the main trunk division were found more frequently. The coexistence of the more frequent trifurcation of the MCA and an aneurysm constitutes evidence of greater anatomical variability of this arterial area in cases with a concomitant vascular malformation. Conclusions: This finding is consistent with literature data, showing the relationship between aneurysms and arterial variability and the fact that aneurysms are most often located at arterial divisions.

Atrial arrhythmias and especially atrial fibrillation in the early phase (first 12 hours) of acute myocardial infarction (AMI) are rare. They are more common in the later stages of AMI and the most of the times as a sign of heart failure. The pathogenesis seems to be an underlined coronary ischemia. Among the causes an ischemia of sinus node artery (SNA) is previously described. SNA arise the most of the times from the initial part of right coronary artery while the remaining from the LCx or in a small proportion of them have double origin from RCA and LCx one. In this case we describe a case of atrial fibrillation in the early phase of AMI promptly resolved in the next one hour after successful percutaneous coronary angioplasty (PTCA) treatment of the culprit lesion in the LCx at site of the origin of SNA.
Zaburzenia rytmu przedsionków, a zwłaszcza migotanie przedsionków we wczesnej fazie (pierwsze 12 godzin) ostrego zespołu wieńcowego (AMI) są rzadkie. Są one bardziej powszechne w późniejszych stadiach AMI i przez większość czasu wustępują jako oznaka niewydolności serca. Patogeneza wydaje się być podyktowana niedokrwieniem wieńcowym. Wśród przyczyn wymienia się niedokrwienie tętnicy węzła zatokowego (SNA). SNA powstają najczęściej z początkowej części prawej tętnicy wieńcowej, podczas gdy pozostałe z LCx lub rzadziej mając podwójne pochodzenie (RCA i LCx). Autorzy omawiają przypadek migotania przedsionków we wczesnej fazie AMI, które natychmiast ustąpiło w ciągu następnej godziny po udanym leczeniu przezskórną angioplastyką wieńcową (PTCA) zmiany w LCx skąd pochodziło SNA.

INTRODUCTION: : Coronary artery disease (CAD) leads to mortality and morbidity globally. Approximately, 18 million deaths occurred due to cardiovascular diseases. Objective: To determine the association between knowledge about coronary angiography and anxiety level among patients undergoing coronary angiography. MATERIAL AND METHODS: Analytical cross-sectional study was conducted in two major public-sector hospitals between September 2017 to March 2018. A total of 264 patients were recruited in the study Rusing a non-probability convenience sampling technique. A modified questionnaire about coronary angiography was distributed among participants and the HADS scale was used to assess the anxiety level of the participants. SPSS version 22 was used for data analysis. RESULTS: The mean knowledge score of the participants about angiography was 17.88 ± 4.047. The total knowledge score of both hospitals was good knowledge (6%), fair knowledge (42%) and had poor knowledge (52%). Regarding anxiety level, the participants of both hospitals experienced a high level of anxiety i.e. mild (20%), moderate (34%) & sever (10%). A significant association was found between knowledge and anxiety levels among the participants of both hospitals with a p-value of <0.001. CONCLUSIONS:This study found a significant association between patient’s knowledge about coronary angiography and anxiety level among patients undergoing coronary angiography. The study puts forwards that nurses shall assess the patient’s knowledge before coronary angiography procedure. The provision of accurate knowledge renders categorical purpose to this procedure for salubrious patients’ outcomes.

Background: Transfemoral access is regarded as the TAVI gold standard for the transcatheter aortic valve implantation (TAVI) procedure. However, other options for vascular access have developed in the last few years. Access via the carotid artery is one such alternative. Materials and methods: The study included 9 elderly patients who underwent transcarotid TAVI procedure at the Cardiac and Vascular Surgery Department of the Medical University of Gdańsk. Procedures were performed by a local Heart Team in a hybrid operating room under general anesthesia. Data was collected before the implantation and at discharge. Results: The mean patients’ age was 81 years of age (64-88) and the mean logistic EuroSCORE was 10.8 (7-16). Implantations were performed with 100% device success rate. Intra-operative valve-in-valve procedure was performed in one patient; there were no access-related and valve-related complications during the surgery. Post-procedural complications included minor bleeding, hematoma and pneumothorax. Echocardiographic parameters were significantly improving after the procedure. The mean hospital stay was 5 days (2-7 days). Conclusions: Transcatheter aortic valve implantation via the carotid artery appears to be safe and effective alternative to standard TAVI vascular access.

In this study, the Soret and Dufour effects in a composite stenosed artery were combined with an analysis of the effect of varying viscosity on copper nanofluids in a porous medium. Blood viscosity, which changes with temperature, is taken into account using the Reynolds viscosity model. The finite difference approach is used to quantitatively solve the governing equations. For use in medical applications, the effects of the physical parameters on velocity, temperature and concentration along the radial axis have been investigated and physically interpreted. The results are graphically displayed and physically defined in order to facilitate comprehension of the various phenomena that occur in the artery when nanofluid is present. It is observed that the Soret effect increases the rate of heat transfer but decreases the rate of mass transfer. The new study enhances knowledge of non-surgical treatment options for stenosis and other abnormalities, hence reducing post-operative complications. Additionally, current research may have biomedical applications such as magnetic resonance angiography (MRA), which provide a picture of an artery and enable identification of any anomalies, and thus may be useful

Opis:
Massive bleeding from an extensive, exophytic tumour is a serious complication in treatment of advanced stages of cervical carcinoma. Ligation of the internal iliac artery is an effective, therapeutic procedure in controlling massive bleeding by process of tissue ischaemia. Although in a number of cases, surgical approach requires good anatomical knowledge and skills because of the highly deformed pelvic anatomy due to radiotherapy or to the recurrence of cancerous tissue. The aim of the study was to evaluate internal iliac artery ligation in patient with advanced cervical cancer with massive, life-threatening bleeding. The described patient’s case illustrates common clinical course of an advanced stage of a cervical cancer treated with internal iliac artery ligation, when the local methods of haemostasis are ineffective. The extensive cancer process extent unables introduction of haemostatics brachyteraphy. Massive bleeding and progressive anaemia could only be controlled by surgical internal iliac artery ligation. The above described proceeding enables administration of further treatment in a short period of time and improves the quality of patient’s life.
Masywne krwawienie ze zmienionej egzofitycznie szyjki macicy często stanowi poważne powikłanie w przebiegu leczenia zaawansowanej postaci raka szyjki macicy. Podwiązanie tętnic biodrowych wewnętrznych jest skutecznym zabiegiem pozwalającym na utrzymanie hemostazy, powodując niedokrwienie tkanek i wywołując szybki efekt terapeutyczny w postaci zahamowania krwawienia. Jednakże technika wykonania zabiegu wymaga dobrej znajomości anatomii i doświadczenia operacyjnego ze względu na występujące deformacje po radioterapii i zniekształcenia anatomiczne w przypadku wystąpienia wznowy procesu nowotworowego. Celem pracy była ocena zastosowania zabiegu podwiązania tętnic biodrowych wewnętrznych u pacjentki z zaawansowanym rakiem szyjki macicy i zagrażającym życiu krwotokiem. Opisany przypadek stanowi przykład zastosowania podwiązania tętnic biodrowych wewnętrznych w przypadkach obfitych krwawień ze zmienionej nowotworowo szyjki macicy, gdy zastosowanie miejscowych metod utrzymania hemostazy często okazuje się nieskuteczne. Rozległy naciek nowotworowy w obrębie szyjki macicy uniemożliwia również w wielu sytuacjach wdrożenie hemostatycznej brachyterapii. Masywne krwawienia i postępująca anemia mogły być skutecznie kontrolowane przez podwiązanie tętnic biodrowych wewnętrznych. Przedstawione przez nas postępowanie stworzyło warunki do dalszego leczenia onkologicznego i pozwoliło poprawić jakość życia chorej.

Although children are a minority among patients with cerebral artery aneurysms, aneurisms in this age group remain a problem not to be ignored. The cause of this is a much higher incidence of giant aneurisms in child - ren in comparison to adults, sometimes with dramatic manifestation. Two children with giant aneurysms of cerebral arteries, operated on at the Department of Neurosurgery Polish Mother Memorial Hospital Research Institute in Łódź are presented in this paper. Thirteen-year-old girl had an aneurysm on the left internal cerebral artery (25 mm in diameter). Ten-year-old boy had an aneurysm on the right medial cerebral artery (40 mm in diameter). Both aneurysms presented with subarachnoid hemorrhage. Surgical treatment consisted of aneurysm clipping in the case of the boy and proximal internal cerebral artery ligation in case of the girl. In spite of surgery, the treatment was unsuccessful. on the basis of presented cases some problems connected with the management of pediatric patients with giant cerebral aneurysms are discussed.
Wśród ogółu pacjentów z tętniakami naczyń mózgowych pacjenci pediatryczni stanowią zdecydowaną mniejszość, jednak w tej grupie wiekowej stanowią one istotny problem kliniczny. Wynika to między innymi z faktu znacznie częstszego w stosunku do populacji dorosłych występowania tętniaków olbrzymich, których przebieg kliniczny bywa niekiedy dramatyczny. W pracy przedstawiono przypadki dwojga dzieci z tętniakami olbrzymimi naczyń mózgowych leczonych operacyjnie w Klinice Neurochirurgii Instytutu Centrum Zdrowia Matki Polki w Łodzi. U 13-letniej dziewczynki stwierdzono obecność tętniaka o średnicy 25 mm położonego na lewej tętnicy szyjnej wewnętrznej. U 10-letniego chłopca rozpoznano tętniaka o średnicy 40 mm zlokalizowanego na prawej tętnicy środkowej mózgu. Chorzy zostali przyjęci do szpitala z objawami krwotoku podpajęczynówkowego, w stanie ciężkim. W obu przypadkach zastosowano leczenie chirurgiczne polegające na zaklipsowaniu szyi tętniaka u chłopca oraz zaklipsowaniu naczynia doprowadzającego u dziewczynki. U obojga dzieci dalszy przebieg leczenia był niepomyślny. W oparciu o prezentowane przypadki w pracy omówiono problemy związane z leczeniem dzieci z tętniakami olbrzymimi naczyń mózgowych.

The aim of this work is to determine mechanical properties of interfaces between layers of the human thoracic aortic wall with early stages of atherosclerosis lesions. Circumferential (n = 48) and axial (n = 15) specimens have been prepared and the mechanical properties of the interfaces between the layers have been determined on the basis of the peeling test. The results show that the mechanical and dissection properties of the interfaces between the layers depend on the direction of the tests. The results confirm that the early stage of atherosclerosis does not affect the mechanical parameters of the layer interfaces and does not affect resistance of the vessel wall to delamination.

A number of epidemiological studies conducted over the last decade indicate a relationship between specific pathogen infections and the development of atherosclerosis, although no pathogenetic pathways connecting these two have been determined. Recent reports support the role of heat shock proteins (HSPs) in atherogenesis. The HSPs are also believed to be a link between the infection and the development of atherosclerotic lesions. The aims of study: Immunohistochemical evaluation of carotid artery segments to show the relationship between the presence of heat shock proteins and the serum levels of anti-hsp60 antibodies. An attempt to demonstrate a relationship between an expression of chronic C. pneumoniae and CMV antigens. Material and Methods: The study included 41 patients qualified for carotid artery endarterectomy and 18 healthy volunteers of corresponding age. Levels of anti- hsp60, anti-C. pneumoniae IgA and IgG, anti-CMV IgG antibodies as well as hsCRP were determined. Results: The mean serum levels of anti-hsp60 antibodies were higher in patients with advanced atherosclerosis as compared to healthy volunteers (55.3±64.1 vs 32.8±29.8; p < 0.05). There was a strong correlation between anti-hsp60 antibodies and the expression of hsp60 in carotid arterial wall, as confirmed by immunohistochemical evaluation. The study group showed statistically significant higher levels of hsCRP. Furthermore, statistically significant higher serum levels of anti-C. pneumoniae IgG and IgA as well as anti-CMV IgG antibodies were found in the study group as compared to controls. No correlation was shown between the markers of chronic infection induced by the tested pathogens and serum levels of anti-HSP and hsCRP. Conclusions: Higher protein expression in vascular walls is closely correlated with the level of anti-hsp60. At the same time, no significant relationship between anti-hsp60 antibodies and serological markers of infection was observed, which may only indicate an indirect role of infection in the assessment of breaking the immunological tolerance against autologous HSPs.

Celem artykułu jest przedstawienie przemian przestrzeni w otoczeniu ciągu ulicy Mogilskiej w Krakowie. Arteria ta stanowi główny ciąg komunikacyjny łączący śródmieście Krakowa z Nową Hutą. Badania terenowe prowadzone przez autora obejmują ulicę Mogilską i Jana Pawła II. Od Ronda Mogilskiego, poprzez Rondo Czyżyńskie do Placu Centralnego im. Ronalda Reagana w Nowej Hucie. Otoczenie przedstawionej arterii komunikacyjnej zostało ukształtowane w różnych okresach, a jego struktura funkcjonalnoprzestrzenna stanowi interesujący przykład zróżnicowania formy oraz funkcji i jej wpływu na odbiór przestrzeni i charakter ulicy. Pomimo swojej kompozycyjnej roli w strukturze miasta łączącej dwie dzielnice Krakowa, dominująca jest przewaga czynnika komunikacji nad możliwościami stworzenia atrakcyjnej ulicy miejskiej, wyróżniającej się w przestrzeni miasta charakterem i jakością otoczenia. Przeprowadzone analizy użytkowania terenu, formy zabudowy oraz relacji kompozycyjnych wskazują na nie wykorzystanie potencjału tej arterii związanego przede wszystkim z jej unikalnym w skali miasta powiązaniu dwóch początkowo niezależnych zdefi niowanych zespołów urbanistycznych, historycznego Krakowa i Nowej Huty. Trzy główne przestrzenie Rondo Mogilskie, Rondo Czyżyńskie i Plac Centralny w analizowanym ciągu ulic, pomimo podejmowanych działań inwestycyjnych stanowią dominujące w przestrzeni arterii węzły komunikacyjne z ograniczeniem ruchu pieszego. Nie sprzyja to integracji przestrzeni, a powstające nowe obiekty pomimo nowoczesnej formy tylko lokalnie wpływają na podniesienie atrakcyjności otoczenia i jakości tkanki miejskiej. Podjęty temat wskazuje na znaczenie kształtowania otoczenia miejskiej arterii komunikacyjnej dla podniesienia jej atrakcyjności i możliwości identyfi kacji kompozycyjnej w przestrzeni. Istotnym elementem jest sama arteria jako ciąg komunikacyjny, jej szerokość, przebieg wraz z kształtem jej infrastruktury, ale także skala, forma i funkcja jej otoczenia. Racjonalne jego tworzenie w aspekcie kompozycyjnym i funkcjonalnym może budować w efekcie atrakcyjną przestrzeń miejską związaną z powiązaniami funkcjonalnymi, ale także ożywiać poszczególne fragmenty miasta oraz sprzyjać integracji otoczenia.
The aim of the article is to present transformations of space in the surroundings of Mogilska street in Kraków. The artery is a main transportation route connecting central Kraków with Nowa Huta. The field research conducted by the author cover Mogilska and Jana Pawła II streets, from Mogilskie roundabout, to Czyżyńskie roundabout and further on up to Ronald Reagan Centralny Square in Nowa Huta. The surroundings of the presented transportation artery were created in different periods of time and their functional and spatial structure is an interesting example of form and function diversity and its influence on how the space and character of the street is perceived. Although the street plays an important compositional role in the structure of the city as it connects two districts of Kraków, the dominant transportation function prevents it from becoming an attractive city street standing out in the city space due to the character and quality of its surroundings. The analysis of land use, development form and compositional relations points out to the potential of this artery stemming primarily from its role – unique in the scale of the city – as a connection between two initially independent urban organisms: historic Kraków and Nowa Huta, which potential remains largely untapped. The three major spaces: Mogilskie roundabout, Czyżyńskie roundabout and Centralny Square still remain dominant transportation hubs in the space of the artery with limited pedestrian traffic, in spite of all the development effort to prevent this. The situation does not promote integration of space, and the newly erected facilities, despite their modern form, are able to improve the attraction of the surroundings and urban fabric quality only locally. The issue in question highlights the significance of designing the surroundings of an urban transportation artery for the improvement of its attraction and compositional identification in a space. An important component is the artery itself as a transportation route, its width, course and infrastructure, but also the scale, form and function of its surroundings. Rational development of such surroundings in the compositional and functional aspect may, as a result, produce an attractive and functionally interconnected urban space, as well as invigorate individual parts of the city and promote integration of the surrounding areas.

Coronary artery disease is multifactorial. In recent times due to the development of modern molecular biology techniques, candidate gene polymorphisms related to lipid metabolism the renin–angiotensin–aldosterone system and the regulation of coagulation and fibrinolysis as well as inflammatory response, have become a research topic. This article provides an overview of the most frequently studied polymorphisms.
Choroba niedokrwienna serca ma charakter wieloczynnikowy. W ostatnim czasie ze względu na rozwój technik z zakresu biologii molekularnej tematem badań stały się polimorfizmy genów kandydatów, związanych m.in. z metabolizmem lipidów, z układem renina–angiotesyna–aldosteron (RAA), regulacją krzepnięcia i fibrynolizy oraz reakcją zapalną. W pracy przedstawiono przegląd wyników najczęściej badanych polimorfizmów.

Background: The aim of this review is to present the available clinical and surrogate endpoints that may be used in future studies performed in patients with peripheral artery occlusive disease (PAOD). Importantly, we describe statistical limitations of the most commonly used endpoints and offer some guidance with respect to study design for a given sample size. The proposed endpoints may be used in studies using surgical or interventional revascularization and/or drug treatments. Methods: Considering recently published study endpoints and designs, the usefulness of these endpoints for reimbursement is evaluated. Based on these potential study endpoints and patient sample size estimates with different non-inferiority or tests for difference hypotheses, a rating relative to their corresponding reimbursement values is attempted. Results: As regards the benefit for the patients and for the payers, walking distance and the ankle brachial index (ABI) are the most feasible endpoints in a relatively small study samples given that other non-vascular impact factors can be controlled. Angiographic endpoints such as minimal lumen diameter (MLD) do not seem useful from a reimbursement standpoint despite their intuitiveness. Other surrogate endpoints, such as transcutaneous oxygen tension measurements, have yet to be established as useful endpoints in reasonably sized studies with patients with critical limb ischemia (CLI). Conclusions: From a reimbursement standpoint, WD and ABI are effective endpoints for a moderate study sample size given that non-vascular confounding factors can be controlled.

Background: The effect of sleeping positions during acclimatization to high altitude on Acute Mountain Sickness (AMS) and High Altitude Pulmonary Edema (HAPE) is unknown. We tested whether sleeping with the upper body raised by 5° reduces prevalence and severity of symptoms of AMS as well as of elevated pulmonary artery systolic pressure (PASP) values as a risk factor of HAPE. Methods: Randomly assigning trekking tourist volunteers n = 44 (25 m, 19 f; mean age 42.9 yr) sleeping at 4280 m or 5170 m to the experimental group (upper body elevated by 5°), or to the control group. After exclusion of other reasons for AMS-like symptoms those assumed to be related to AMS were rated by Lake Louise Score questionnaire in the evening and the following morning of the study. Transthoracic echocardiography was performed on both occasions to estimate PASP. Results: In the study group, symptoms of AMS were significantly reduced in younger subjects (p = 0.021), prevalence of AMS was reduced in women (p = 0.156), and PASP values were significantly reduced in older subjects and men (p = 0.032; p = 0.031 respectively). Conclusion: Results suggest that sleeping with the upper body in elevated position during a high altitude ascent may benefit those suffering from AMS or at risk of HAPE due to elevated PASP values.

The contractile effects of PGF₂α (3 × 10⁻⁶ to 10⁻⁴ M) and PGE₂ (10⁻⁷ to 10⁻⁵ M) were examined on isolated branches of ovarian artery (OA) and extramyometrial branches of uterine artery (UA) collected from pigs in the luteal (day 10-12) and follicular phase (day 17-20) of the estrous cycle, and during early pregnancy (day 10-12). Strong contraction was demonstrated in both arteries during all investigated periods in response to PGF2α, which was significantly higher (P < 0.01) than to PGE₂, being negligible in the follicular phase. In UA, the effective dose of PGF₂α (ED50) amounted 7.9 × 10⁻⁶ M and 6.3 × 10⁻⁶ M in the luteal and follicular phase, and 5.0 × 10⁻⁶ M in early pregnancy. ED50 for PGE₂ reached 5.0 × 10⁻⁷ M in the luteal phase, and 4.1 × 10⁻⁷ M in early pregnancy. For both prostaglandins, the contraction was much stronger (P < 0.01) in OA than in UA branches. In OA, the ED50 for PGF₂α was 1.2 × 10⁻⁵ M in the luteal phase and was significantly higher (P < 0.05) than in the follicular phase (3.1 × 10⁻⁶ M) and early pregnancy (2.7 × 10⁻⁶ M). ED50 for PGE2 amounted 7.3 × 10⁻⁷ M in the luteal phase and 1.7 × 10⁻⁷ M in early pregnancy. Studies showed the influence of the estrous cycle and early pregnancy on OA branches sensitivity to the contractile effect of PGF₂α and the lack of this effect on UA branches, and the influence of the estrous cycle on UA and OA branch contraction in response to PGE2.

Localisation and morphology of myocardial bridges in the heart of domestic pig remain an open issue. Since these structures significantly influence haemodynamics in the coronary arteries, their occurance may lead to numerous pathologies. In the examined group of 150 domestic pig’s hearts, myocardial bridges were diagnosed in 47.3% of the material, mostly in males. In majority of cases the bridges were present above the posterior interventricular branch of the right coronary artery, less often above the anterior interventricular branch of the left coronary artery, and seldom above other blood vessels. The presence of myocardial bridges usually referred to the medial and initial segments of the arteries examined.

The study presented a case of a 58-year-old male patient treated for retroperitoneal fibrosis, right hydronephrosis, and right common iliac artery stenosis and saccular aneurysm of the above-mentioned vessel. The patient was qualified for endovascular treatment. Stentgraft implantation was performed with good long-term patency during more than 3 years of follow-up. Complete relief of intermittent claudication was observed. However, the endovascular exclusion of the aneurysm did not influence the course of retroperitoneal fibrosis

Objectives Legislators and policymakers have expressed strong interest in intervention programs to reduce dependence on social disability benefits. Hybrid: ambulatory followed by home-based cardiac telerehabilitation – hybrid cardiac rehabilitation (HCR) seems to be a novel alternative for standard cardiac rehabilitation for patients with cardiovascular diseases (CVD) as a form of pension prevention paid by the Social Insurance Institution (SII). The kind of professional status may bias the motivation to return to work after HCR. The aim of our study was to evaluate whether the professional status can affect the effects of HCR. Material and Methods One hundred fifty-two patients with CVD referred by the SII for a 5-week HCR were qualified for the study. Patients (87.7% males), aged 57.31±5.61 years, were divided into 2 subgroups: W) white-collar employees (N = 22) and B) blue-collar employees (N = 130). To evaluate functional capacity, an exercise test on a treadmill was used. Results The number of days of absence in the cardiac rehabilitation program did not differ between the groups (mean ± standard deviation – B: 1.09±3.10 days, W: 1.95±3.64 days). There were significant improvements (p < 0.05) in measured variables after HCR in both (W and B) groups (max workload: 8.21±2.88 METs (measured in metabolic equivalents) vs. 9.6±2.49 METs, 7.76±2.51 METs vs. 8.73±2.7 METs, resting heart rate (RHR): 77±16.22 bpm vs. 69.94±12.93 bpm, 79.59±14 bpm vs. 75.24±11.87 bpm; double product, i.e., product of heart rate and systolic BP (DP rest) 10 815.22±2968.24 vs. 9242.94±1923.08, 10 927.62±2508.47 vs. 9929.7±2304.94). In group B, a decrease in systolic blood pressure (BP syst. – 137.03±17.14 mm Hg vs. 131.82±21.13 mm Hg), heart rate recovery in the 1st minute after the end of peak exercise (HRR1) (99.38±19.25 vs. 93.9±19.48) and New York Heart Association (NYHA) class (1.22±0.53 vs. 1.11±0.36) was observed. In group W, a decrease in diastolic blood pressure (BP diast.) at rest was observed (88.28±9.79 mm Hg vs. 83.39±8.95 mm Hg). The decrease in resting HR was significantly greater in group W (69.94±12.93 vs. 75.24±11.87, p = 0.034). Conclusions Hybrid cardiac rehabilitation is feasible and safe with high adherence to the program regardless of the patient’s professional status. Professional status did not influence the beneficial effect of HCR on exercise tolerance.

Aim. The primary concern of the study was to compare pro-health behaviors in participants with stable coronary artery disease (SCAD), also known as stable ischemic heart disease (SIHD), and in healthy controls in order to improve the effectiveness of preventive measures in that group of patients. Material & methods. The study comprised a total of 394 participants aged 18 to 88 years (mean age 49.5±14.9 yrs), including 294 patients of the Cardiology Outpatient Clinic who were treated due to SCAD and 100 healthy controls. All participants were asked to complete the Health Behavior Inventory (HBI) by Juczyński. Moreover, the measurements of their body weight, height and waist and hip circumference were taken, and their BMI and WHR values were calculated. Finally, the risk for developing metabolic complications was assessed. Results. Healthy body weight was discovered in 32.6% (n=96) of cardiology patients and in 45.0% of controls. Abdominal obesity was more often found in cardiology patients (55.0%) than in controls (49.0%). Increased risk for obesity complications was disclosed in 60.2% of SCAD patients and in 37.0% of controls. Cardiology patients maintained health-related attitudes to a greater extent than controls on each subscale (p<0.05). Conclusions. Secondary prevention of coronary artery disease (CAD) should include both maximization of effective treatments and constant support of and encouragement for patients to reduce CAD risk factors and develop and implement pro-health behaviors.
Cel badań. Celem pracy było porównanie zachowań zdrowotnych pacjentów ze stabilną chorobą wieńcową oraz osób zdrowych, co pozwoli na optymalizację działań profilaktycznych na rzecz pacjentów. Materiał i metody. Badaniami objęto 394 osoby w wieku od 18 do 88 lat (49,5±14,9 lat), w tym 294 pacjentów Poradni Kardiologicznej, leczących się z powodu stabilnej choroby wieńcowej i 100 osób zdrowych. Posłużono się Inwentarzem Zachowań Zdrowotnych Juczyńskiego. Przeprowadzono pomiar masy, wysokości ciała, obwodu talii i bioder, obliczono wskaźnik BMI oraz WHR. Oszacowano ryzyko powikłań metabolicznych. Wyniki. Prawidłową masę ciała miało 32,6% (n=96) pacjentów kardiologicznych i 45,0% osób z grupy kontrolnej. Nadmierne gromadzenie tkanki tłuszczowej w okolicy brzucha obserwowano częściej u pacjentów kardiologicznych (55,0%) niż u osób zdrowych (49,0%). Podwyższone ryzyko powikłań otyłości obserwowano u 60,2% pacjentów z CAD i 37,0% osób zdrowych. Pacjenci z CAD w większym stopniu cechowali się pozytywnymi zachowaniami zdrowotnymi w każdej podskali,w porównaniu z osobami zdrowymi (p<0,05). Wnioski. W ramach prewencji wtórnej choroby wieńcowej istotna jest zarówno maksymalizacja efektywnego leczenia, jak i zakrojona na szeroką skalę promocja zwalczania czynników ryzyka i przyjęcia pozytywnych zachowań zdrowotnych.

Cardiovascular mortality remains a leading health and social problem in many countries throughout the world. Its main cause is related to atherosclerosis of coronary and cerebral vessels with their most severe consequences: heart attack and stroke. Therefore, it is obvious that current preventive measures include early detection of atherosclerosis process. Multi-detector computed tomography (MDCT) is one of imaging modalities allowing for noninvasive detection of atherosclerotic lesion within coronary arteries in subjects with accumulation of risk factors (smoking, high lipids, hypertension, male gender, family history) or with suspicion of coronary artery disease (CAD). In is very important that the tomographic images are taken in synchronization with cardiac cycle so that, during few heartbeats, an appropriate series of images can be recorded. Commonly, cardiac MDCT is used for visualization of cardiac and vessels morphology. Heart function can also be determined, however, this MDCT potential is only rarely applied, as current echocardiographic modalities are sufficient. Functional analysis of coronary arteries (flow, reserve) is usually approached by means of invasive procedures. We aimed at finding solution for evaluation of another kind of functional analysis of coronary arteries, namely vessel's wall compliance by means of MDCT coronary angiography. Under the proposed procedure, on basis of serial CT images of the vessels over entire cardiac cycles, the internal area of the blood vessel is measured and its changes during various phases of heartbeat (systole, diastole) are calculated. If the vessel wall has been changed by atherosclerotic plaque, either calcified or non-calcified, then its compliance will be reduced due to its stiffness. Calculation of coronary artery compliance requires a series of measurements, which is unreliable and impractical for doing manually. One component of the method described herein involves the images being converted into binary representations and the Hough Transform then applied. The overall methodology proposed in this paper assists in the preparation of a medical diagnosis.

Cardiovascular diseases represent the leading cause of death in Western Countries. Among them, a key role is played by arterial hypertension, which causes macro- and microvascular alterations. Specifically, hypertension is associated with structural alterations in the microvessels, such as an increased ratio of the tunica media thickness to internal lumen (M/L ratio) in small resistance arteries and a reduction of capillary density. In order to evaluate the small resistance artery structure, the direct measurements of M/L ratio through wire or pressure micromyography has been considered the gold-standard method. Despite the availability of convincing evidence about the prognostic relevance of the M/L ratio, the invasiveness of these methods has limited its implementation in the daily clinical practice. Therefore, non-invasive techniques have been developed to evaluate microvascular morphology, particularly in the retina, since it is perhaps the most accessible microvasculature. Scanner laser Doppler flowmetry (SLDF) and adaptive optics (AO) represent the most promising approaches for the evaluation of morphological characteristics of retinal arterioles, in particular for the measurement of their wall-to-lumen ratio (W/L ratio). The possibility to evaluate microvascular morphology by non-invasive techniques represents a major clinical advancement, with possibly favorable implications in research and in stratification of cardiovascular risk. In this review we will address the different methods to investigate the microcirculation as well as their clinical usefulness.

Opis:
Pre-eclampsia, the most common pregnancy associated syndrome, is connected with remodelling of extracellular matrix of the umbilical cord tissues. Since the fibroblast growth factor (FGF) is known to be a stimulator of collagen and glycosaminoglycan biosynthesis, one may expect that it plays an important role in such a remodelling. Studies performed on the umbilical cords of 10 control and 10 pre-eclamptic newborns demonstrated that both the umbilical cord arterial wall and Wharton's jelly contain FGF mainly in complexes with the components of different molecular mass. Pre-eclampsia is associated with a decrease of endogenous FGF-binding by soluble high molecular mass components of the umbilical cord. It is suggested that FGF released from these complexes may be actively bound by fibroblasts of the umbilical cord, stimulating them to produce collagen and sulphated glycosaminoglycans.
