Celem pracy jest prezentacja modelu hazardów proporcjonalnych Coxa(ang. Cox proportional hazards model), charakteryzujących go własności oraz metod esty-macji jego parametrów. Znajduje on zastosowanie w analizie przeżycia przy przewidywaniuszans przetrwania pewnych obiektów (najczęściej pacjentów w badaniach medycznych).Istotną zaletą modelu jest możliwość uwzględnienia w nim danych niepełnych, które częstopojawiają się w przeprowadzanych badaniach — zarówno w sposób losowy, jak i celowy.Model Coxa sprawdza się szczególnie dobrze w sytuacji, gdy interesujące jest określenieskuteczności sposobu leczenia w sensie porównawczym, czyli w odniesieniu do innych te-rapii. Terminologia i przykłady zaczerpnięte są na ogół z medycyny, ale opisany modelstosuje się również np. w socjologii, kryminalistyce czy inżynierii.Słowa kluczowe: model Coxa, hazard, analiza przeżycia.
The paper presents Cox proportional hazards model, its properties and methods of its parameters estimation. It is widely applicable in survival analysis – in prediction of survival chances of some objects (usually patients in medical studies). The essential advantage of the model is allowing of incomplete data, which often appear in studies –both in random and fixed way. Cox model works especially well when determination of treatment effectiveness in comparative sense (with reference to other therapies) is needed. Terminology and examples are usually taken from medicine but the model can be used also in sociology, crime detection or engineering.

Industrial large-scale urea production creates a number of hazards due to equipment and machine failures as well as service errors. Failures can cause material, environmental and human losses. The work presents the assessment of safety hazards and failure effects for selected emergency scenarios of Mitsui - Toatsu's urea synthesis reactor. A fragment of the process implemented in the synthesis node was covered by HAZOP (Hazard and Operability Study), and the critical points, from the security point of view, were identified. Several emergency scenarios for various types of failures and various effects are presented. The analyzed contingency scenarios include: blocking the valve on the outflow from the reactor and leaks caused by corrosion resulting from the maintenance of improper technological parameters of the process. The work suggested the need for new security measures and modernization of the installation by using the safety function for the selected critical point. The hazards analyzed and the contingency scenarios developed are essential for risk assessment and its possible reduction.

The task of the truck driver to transport cargo is subject to the regulations and safety rules. Activities performed by the driver are connected with the risk of injury, complications or even death. Taking into account employee safety, one of the most important aspects is to define safe working time standards that minimize the risk of fatigue during driving. The purpose of this article is to identify hazards and determine the occupational risk at the truck driver's workplace.

The paper analyzes the conditions of safe use of industrial gas systems and factors influencing gas hazards. Typical gas installation and its basic features have been characterized. The results of gas threat analysis in an industrial enterprise using FTA error tree method and ETA event tree method are presented. Compares selected methods of identifying hazards gas industry with respect to the scope of their use. The paper presents an analysis of two exemplary hazards: an industrial gas catastrophe (FTA) and an explosive gas explosion (ETA). In both cases, technical risks and human errors (human factor) were taken into account. The cause-effect relationships of hazards and their causes are presented in the form of diagrams in the drawings.

W artykule przedstawiono analizę zagrożeń metodą drzewa błędów (FTA) dla wybranych stanowisk pracy. Zawarte w artykule schematy drzewa błędów ilustrują dwa typy zagrożeń: wypadkiem podczas pracy z prasą hydrauliczną oraz zniszczeniem maszyny na skutek przegrzania. W obu przypadkach przeprowadzona analiza wskazuje przyczyny zagrożenia wynikające z faktu awaryjności elementów maszyny lub stanowiska pracy oraz możliwe do popełnienia błędy operatora. Za pomocą drzewa błędów wskazano wzajemne związki między zdarzeniami prowadzącymi do zagrożenia w analizowanych przykładach. Zidentyfikowano również działania korygujące dla analizowanych zagrożeń w celu zminimalizowania ryzyka zawodowego.
The paper presents an analysis of the Fault Tree Analysis (FTA) approach for selected workplaces. The fault tree diagrams included in this article illustrate two types of hazards: an accident when working with a hydraulic press and damage to a machine due to overheating. In both cases, the analysis indicates the cause of the hazard resulting from the failure of the machine or workpiece, and the operator's possible errors. The fault tree identifies the relationship between events leading to a threat in the analyzed examples. The corrective actions were also identified for the hazard analyzed to minimize occupational risk.

The Youd etal liquefaction resistance curves developed in 2001 to characterize the cyclic resistance of soil based on SPT test are the most used in the context of the Seed and Idriss simplified procedure as a deterministic model. These curves were developed from a modified database of Seed etal. in 1985 with the assumption that the actual peak shear stress (τd) induced at depth h is always less than that predicted by the simplified procedure (τr) of Seed and Idriss (rd= τd/τr<1). By using a suite of equivalent linear site response analyses to adjust the dynamic and the simplified shear stress at depth h, Filali and Sbartai showed in 2017 that the dynamic peak shear stress for some earthquakes is greater than the simplified peak shear stress (rd>1). As in this case, the assumption of the simplified procedure is not verified, Filali and Sbartai have proposed a corrector factor (RC) in the range where rd>1 to adjust the deformable and rigid body. In this paper, we will present a probabilistic study for the evaluation of the liquefaction potential using a database based on SPT measurement compiled after the Chi-Chi Taiwan earthquake, in which the cyclic stress ratio is evaluated using the proposed corrector factor. The objective of this study is to present a probabilistic shape of the cyclic resistance ratio (CRR) curves based on the original simplified method of Seed and Idriss and the corrected version and a new formulation for computing the probability of liquefaction.

Opis:
The event of the loss of propulsion function has been defined as hazardous event to a seagoing ship. It has been formalized. The procedure of acquisition of expert opinions on frequency of the event occurrence has been described. It may be considered to be of a numerical-fuzzy character. The fuzzy part was transferred to the numerical form by the pair comparison method. An example of the ship propulsion system comprising a low speed internal combustion engine and a solid propeller illustrates the method presented. It may be used wherever a hazard analysis has to be performed of a system involving human and technical aspects and there is a shortage of objective data on the investigated object.

The National Occupational Exposure Survey (NOES) was used to determine probabilities for 4 potential physical-agent and 10 potential ergonomic-related exposure hazards among a representative sample of U.S. industries. Potential physical-agent hazard exposures, principally whole-body and segmental vibration, were highest among railroad and heavy construction industries. Several construction industries had high probabilities of potential ergonomic-related exposure hazards, especially to the back and upper extremities. Establishments with 100 to 249 employees had the highest probability of potential exposures to the 2 types of hazards. Measures of safety and health climate did not differ consistently between high-hazard and low-hazard establishments. The approach taken in this paper may be used to help identify highrisk industries, evaluate interventions, and develop inspection strategies.

Introduction. A wheelchair is a special vehicle designed specifically for people with walking limitations. One of the types is an active wheelchair. This is a manually propelled wheelchair, the design and structure of which maximise the mobility of its given user at the expense of accepted instability. Objective. The aim of this study was to identify and present a hierarchy of potential incidents causing a health hazard and reducing the mobility of individuals with ambulatory disability who use active wheelchairs in non-urban settings. Materials and methods. Information about incidents connected with wheelchair use was collected from interviews with disabled individuals, based on a questionnaire. Recorded data were analysed using the Preliminary Hazard Analysis (PHA). Results. The results comprise a list of hazards for wheelchair user with ascribed priority numbers metrising their significance. Conclusions. An ordered list of hazards connected with wheelchair operation situations is useful when designing active wheelchairs and when learning the technique of wheelchair riding.

Business continuity depends on the quality of management of individual processes. The management team strives to anticipate all possible threats and define protective measures. In the event of the COVID-19 pandemic enterprises were not prepared for such threats. Therefore, an important role is played by the health and safety inspector who should be involved in the company management process. The impact of the safety aspects of the work environment on the quality and efficiency of work is even more important in the event of an unexpected hazard that is difficult to completely eliminate. Opis:
An unmanned merchant vessel seems to be escaping from the stage of idea exploration. Once the concept proofs its safety, it may become a part of maritime reality. Although the safety aspect of such a ship has been addressed by a handful of scholars, the problem remains open. This is mainly due to lack of knowledge regarding actual operational circumstances and design of unmanned ships, which are yet to be developed. In the attempt of bridging this gap, the risk analysis associated with unmanned ships needs to be carried out, where all relevant hazards and consequences are assessed and quantified in systematic manner. In this paper we present the results of a first step of such analysis, namely the hazard analysis associated with the unmanned ships. The list of hazards covers various aspects of unmanned shipping originating from both design and operational phases of vessel’s life. Subsequently the hazards and related consequences are organized in a casual manner, resulting in the development of a structure of a risk model.

Opis:
The primary purpose of the paper is to identify port areas most exposed to extreme hydrodynamic conditions (waves, sea currents, seabed level change). The results of modelling using SWAN wave model, MIKE 3D model, and reanalysis and measurement data were used in paper. Swell may exceed 0.8 m for winds exceeding 15 m s-1 from the west and south. During extreme conditions, sea currents can reach 0.4 ms-1 in the outer part of the bay adjacent to the port. Port basins do not show changes in the thickness of the seabed for the given maximum values of bottom currents. The most extensive deposition of the seabed and shore sediments (up to 0.04 m) is found on the Gdynia-Oksywie beach adjacent to the port and the approach fairway at the offshore currents. The outer area of the main breakwater is the most exposed to erosive activity (-0.012 m).

This paper presents the results of research on fire hazard coordinated by the Fire Safety Subcommittee of the International Maritime Organization (IMO). On the basis of statistical data, it identifies the main sources and frequency of fires in engine rooms. It also presents calculations of frequency of fires caused by self-ignition of flammable liquids in fuel oil and diesel oil systems, which constitute 60% of the overall hazard. In the theoretical analysis (frequency calculation), changes in construction protection of systems as well as changes in safety equipment of adequate rooms, have been taken into consideration. Fire hazard identification has been conducted on the basis of statistical data of 73 fires of merchant vessels engine rooms out of the overall number of 6,000 vessels in the 13-year-long period of use. As the final result of the research, fire prevention requirements for engine rooms and pumping stations have been developed. The requirements include, in particular: construction elements of systems preventing leakages with self-ignition, protection of ignition sources ('hot spots'), and ensuring safe engine room atmosphere. The results of the experiment have confirmed the possibility of conducting fire hazard quantitative assessment (estimation), the reliability of which is limited because of the nature of the model. As far as quality is concerned, results of risk sensitivity analysis confirm the effects expected when using safety requirements applied creatively.

Opis:
W artykule przedstawiono metody analizy zagrożeń z uwzględnieniem różnych rodzajów uszkodzeń. Opisano rozwiązania analityczne umożliwiające obliczenie intensywności zagrożeń złożonej struktury elektronicznej, charakteryzującej się różną dynamiką bloków przetwarzania i co za tym idzie różnymi czasami wykrywania poszczególnych usterek. Przedstawiono także sposób uwzględnienia intensywności zagrożeń pochodzącej od usterek niewykrywalnych.
In the presentation, the hazard analysis methods considering different types of failures and a difference between optimistic and pessimistic approach are presented. It shows analytic solutions helpful in calculation of hazard rate for complex electronic structure, which is characterized by different dynamic of processing blocks and different fault detection times. What is more the presentation shows a method of adding hazard rate of undetected failures.

Opis:
In the paper the required level of reliability is determined for several Unmanned Aerial Vehicles developed in Poland in order to get an achievement enabling these vehicles to operate within the Single European Sky. Calculations were made on the basis of an air crash model as well as the model capable to estimate the number of casualties resulting from an aircraft catastrophe. The provided examples allow us to specify Tactical and Technical Conditions pertaining in particular to the area of the operation of the aforementioned aircraft.
W pracy wyznaczono niezbędną niezawodności kilku opracowanych w Polsce samolotów bezpilotowych, której osiągnięcie umożliwia ich eksploatacje w połączonej przestrzeni powietrznej. Obliczenia prowadzone były wg modelu katastrofy powietrznej oraz modelu pozwalającego na oszacowanie liczby ofiar na skutek rozbicia się samolotu. Podane przykłady pozwalają na sprecyzowanie Warunków Taktyczno - Technicznych, w szczególności dotyczących obszaru eksploatacji tychże samolotów.

A non-ergodic probabilistic seismic hazard analysis (PSHA) utilizing the physics-based ground motion prediction was proposed in this study to minimize the increasing uncertainties in the use of empirical equations. The City of L’Aquila in Italy was used for illustrative purposes due to the availability of data and the historical seismicity of the site. A total of 28 seismic sources were identified in this study located within a 100 km radius from the city. Fault properties such as geometry and location were obtained from the literature, while the fault occurrence rates were obtained using the FiSH Code. A modified time-weakening friction law was proposed to model the seismic energy released by an earthquake. Uncertainties in different rupture scenarios were characterized through the Guttenberg-Richter Relations and the Characteristic Brownian Time Passage. Uncertainties in distances were characterized through probability mass functions, which were used to calculate the ground motion exceedance probabilities. The 1D elastodynamic equation coupled with the Hooke’s law was used to predict the peak ground acceleration (PGA), a measure of the ground shaking level. A hazard curve, which is a plot of PGA and its recurrence, was constructed and compared with the results of the study of Valentini, et al., AGU 100: Advancing Earth and Space Science (2019). The method proposed in this study predicts a higher hazard rates for PGAs less than 0.70 g, which implies that the ground motion was overestimated for very far sources. In contrast, lower hazard curves were observed for PGAs greater than 0.70g which can be attributed to fewer seismic sources considered in this study.
