Lung cancer is associated with one of the highest mortality rates among malignant tumors. It is the main death cause of men and women in Poland where over 15 000 men and 7 000 women die with this diagnosis annually. 1.7 million people die every year due to lung cancer in the world. Two main types of lung cancer are small cell lung cancer (SCLC) and non-small cell lung cancer (NSCLC). The history of lung cancer treatment begins with surgical approach followed by addition of chemotherapy and radiotherapy, which were used either separately or in combination depending on the stage of the lung cancer. Many somatic mutations were identified and molecularly targeted therapies could have been established. One of the oldest and the best known group of molecularly targeted drugs used in lung cancer treatment are tyrosine kinase inhibitors (TKIs) of epidermal growth factor receptor (EGFR). First EGFR-TKI was gefitinib, which has been examined in clinical trials before erlotinib, afatinib, dacomitinib and osimertinib. EGFR-TKIs increased overall survival (OS) with significantly less side effects when compared to standard chemotherapy. Another group of molecularly targeted drugs are anaplastic lymphoma (ALK) kinase inhibitors such as crizotinib, alectinib and ceritinib. Another innovation which was introduced in NSCLC treatment was immunotherapy. Its effect is based on modification of immune system leading to activation of cytotoxic T lymphocytes (CTL). Currently nivolumab and pembrolizumab (anti-PD1 antibodies) as well as atezoliumab (anti-PD-L1 antibody) are being used in NSCLC patients. The discovery of innovative therapies for NSCLC patients resulted in significant extension of patients' life expectancy while minimizing the side effects of such therapy. Moreover, the quality of patients’ life was significantly improved. However, important problems still remain to be solved: overcoming the resistance in the course of molecularly targeted therapies and the lack of predictive factors that determine the effectiveness of immunotherapy.

Introduction. Hyponatremia is a frequently observed electrolyte disorder among patients with cancer. In 1957, Schwartz et al. reported the first case of a patient with hyponatremia due to SIADH, secondary to lung cancer. From that moment on, there has been data published that indicates patients with SIADH are less responsive to chemotherapy, have greater predisposition to central nervous system metastases and are often characterized by an advanced stage of cancer during time of diagnosis. Hyponatremia has many possible causes, and the differential diagnosis can pose a challenge. Aim. The aim of the study was to consider the occurrence of secondary hyponatremia in the course of cancer and the significance of this disorder in the prognosis of the disease. Methods. An analysis of the clinical history of the patient and a review of available literature. Results. A 66-year old patient with hyponatremia was admitted to the Department of Endocrinology, and lung cancer was determined as the cause of the aforementioned electrolyte disorder. Conclusion. SIADH secondary to cancers should be included in a differential diagnosis of every case of hyponatremia of undetermined etiology.

Pulmonary localisation represents only 15% of all cases of actinomycosis. The clinical symptoms and radiological changes of this disease are non-specific and sometimes it can be misdiagnosed, usually as tuberculosis, lung cancer or lung abscess. In the reported case, what might look like the lung cancer, finally turned out to be actinomycosis. The interesting case is presented of lung actinomycosis in a 77-year-old farmer, admitted to the Department of Pneumonology, Oncology and Allegology in Lublin due to a massive haemoptysis. CT scan of the chest showed, apart from other changes, the spicular consolidation in the right lung which aroused oncology vigilance. The diagnostic path, which was a real medical challenge, led to the diagnosis of actinomycosis. The process of diagnosis and consequent treatment, which led to the complete regression of clinical and radiological changes, is presented.

Objectives: Recent results of epidemiological and medical statistics studies of lung cancer and indoor radon in different regions of the world make a relevant new combined analysis of residential exposure health effects. In particular, new data were obtained by means of a meta-analysis of case-control studies as well as taking into account a confounding effect of human papillomavirus infection in studies of geographically aggregated data. Materials and methods: Two sources of epidemiological data are considered: (1) studies of ecological design and (2) case-control studies. Ecological studies included the analysis performed for the USA counties and Russian oblasts with adjusting for the main confounders. Data on the case-control studies were gained from the meta-analysis of 31 individual studies with a weighting of obtained odds ratios according to the quality of radon exposure reconstruction and size of the reference group. Estimations of lung cancer excess relative risk (ERR) associated with indoor radon exposure are combined. Results: Two types of epidemiological study design provided generally consistent EER estimations. The combined value of ERR due to radon exposure is 0.14 (90% CI: 0.10–0.18) per 100 Bq/m3 . Conclusion: Available geographically aggregated data in regions of Russia and the United States and the meta-analysis of case-control studies conducted in a large number of countries confirm the association of lung cancer with indoor radon exposure.

Introduction: Chest pain is one of the symptoms of lung cancer. Chest pain disrupts patient’s functioning in somatic and psychic area. Purpose: Whether the existence of chest pain affects the level of perioperative anxiety in lung cancer patients. Is there a relationship between everyday functioning due to the chest pain and the level of perioperative anxiety? Is there a relation between anxiety associated with pain and gender? What is the cause of anxiety in this group of the patiens? Methodology: The study was conducted among 150 patients with lung cancer before the scheduled surgery. Data was collected with the use of questionnaire assessment of perioperative anxiety level in patients with lung cancer. Results: The chest pain before the surgery was confirmed by 63 (42%) patients, in case of 87 (58%) patients it was not identified. Chest pain was in case of 48% women and 36% men. 50 (33,3%) research participants who experienced chest pain and 35 patients (23,3%) without this symptom declared experiencing perioperative anxiety. In patients with lung cancer, the correlation: between chest pain and perioperative anxiety was (Z = -4.67; p< .001); between the difficult daily functioning of pain and perioperative anxiety was (Z = -4.72; p< .001); between gender and perioperative anxiety associated with pain was (Z = -3.24; p = 001). Patients afraid of: pain (37,3%), breathing problems (24,0%), physical disability (16,0%), eating problems (12,0%) sleep disorders (9,3%), nothing (1,3%). Conclusions: Patients with chest pain exhibited significantly higher of perioperative anxiety levels than non-symptomatic patients, as well as higher anxiety levels due to deteriorated daily functioning caused by the disorder. Women had a higher level of anxiety associated with pain than men before the surgery. Patients with lung cancer were most afraid of pain in the perioperative period.

Introduction. Before cigarettes became popular, lung cancer was an extremely rare disease, but currently it is the most common cause worldwide of cancer death in both genders. Therefore, it constitutes a huge epidemiological, diagnostic and socio-economic challenge around the world. In Poland, lung cancer is the second most common carcinoma in both men and women, and it is also the most prevailing cause of carcinous death. Undoubtedly, although smoking classical cigarettes is the strongest risk factor of developing the cancer, unfortunately it is not the only one. Objective. The aim of this study was to present smoking-related factors, e.g. cigars, pipes, e-cigarettes or marijuana smoking, etiologically related to lung cancer incidence.

The association between the lung cancer and indoor radon exposure in Russian population was investigated. The average indoor radon concentration for each region was estimated using the annual reports issued by the Saint-Petersburg Ramzaev Research Institute of Radiation Hygiene for the period 2008–2013. The average standardized lung cancer mortalities among males and females were estimated using the reports of the Moscow Hertzen Cancer Research Institute for the period 2008–2012. The relative risk (RR) was estimated as a ratio between the average mortality within seven exposure intervals and background mortality. The slope factors of linear dependence between the indoor radon exposure and lung cancer RR are 0.026 (–0.11÷0.17) and 0.83 (0.52–1.12) per radon concentration 100 Bq/m3 for males and females, respectively (with 90% confidence interval). The obtained results can be explained by the confounding effect of tobacco smoking. Significant excess risk of lung cancer in female population can be associated with radon exposure and low prevalence of smoking.

Summary: Lung cancer is the most common malignant tumor in the world, as well as one of the cancers with the most fatal prognosis. The acceptance of the disease is the most important element of the adaptive process. The better the illness acceptance, the lower the stress level and the higher the self-esteem, which facilitates the adaptation to the health status. Aim: The aim of this study was to assess the acceptance of the disease in patients before and after lung cancer surgery. Materials and methods: The study was conducted in 2016 at the Center of Oncology in Bydgoszcz and the Kuyavian and Pomeranian Pulmonology Center in Bydgoszcz. The study involved 87 patients who were assessed both before and after lung cancer surgery. The original questionnaire, as well as the Acceptance of Illness Scale, were used. Results: Men accounted for 75% of the probands, 65% of the study population were 50-69 years old. The highest number of patients – 25 (28.7%) had a 5-pack-year history, and the lowest amount of patients – 8 (9.2%) had a 2.5-pack-year history. The level of acceptance of illness before and after surgery differed in 58 persons. In 29, the level of acceptance remained the same, in 45, the level of acceptance decreased, and in 13 – it increased. Before surgery, the mean acceptance of illness score was 26.2 points, and after surgery – 20.89 points. The patients both after and before surgery had acceptance of illness scores regardless of their gender, age, education, place of residence or occupational activity. Conclusions: In more than a half of the patients, the acceptance of illness decreases after surgery and is at an average level. Male patients, patients aged 50-69 years, with primary, middle or vocational education, employed persons show a significantly worse illness acceptance, regardless of their place of residence, and occupational activity do not influence the acceptance of the disease.

The aim of the present study was to perform comparative analysis of serum from patients with different stages of non-small cell lung cancer (NSCLC) using the three complementary proteomic approaches to identify proteome components associated with the progression of cancer. Serum samples were collected before any treatment from 200 patients with NSCLC, including 103 early stage, 64 locally advanced and 33 metastatic cancer samples, and from 200 donors without malignancy. The low-molecular-weight fraction of serum proteome was MALDI-profiled in all samples. Serum proteins were characterized using 2D-PAGE and LC-MS/MS approaches in a representative group of 30 donors. Several significant differences were detected between serum samples collected from patients with early stage cancer and patients with locally advanced cancer, as well as between patients with metastatic cancer and patients with local disease. Of note, serum components discriminating samples from early stage cancer and healthy persons were also detected. In general, about 70 differentiating serum proteins were identified, including inflammatory and acute phase proteins already reported to be associated with the progression of lung cancer (serum amyloid A or haptoglobin). Several differentiating proteins, including apolipoprotein H or apolipoprotein A1, were not previously associated with NSCLC. No significant differences in patterns of serum proteome components were detected between patients with adenocarcinoma and squamous cell carcinoma. In conclusion, we identified the biomarker candidates with potential importance for molecular proteomic staging of NSCLC. Additionally, several serum proteome components revealed their potential applicability in early detection of the lung cancer.

Wstęp. Rak płuc główną przyczyna zgonów z powodu nowotworów w populacji mężczyzn. Stanowi także istotną przyczynę śmierci kobiet. Cel. Celem pracy jest analiza wybranych aspektów opieki pielęgniarskiej nad pacjentem z chorobą nowotworową płuc. Materiały i metody. W pracy dokonano analizy literatury poświęconej zagadnieniom opieki nad pacjentem w przebiegu raka płuc oraz przeprowadzono mapowanie diagnoz i interwencji pielęgniarskich przy użyciu słownika ICNP® . Wnioski. Rak płuc istotnie modyfikuje jakość życia pacjenta. Klasyfikacja ICNP® pozwala na projektowanie opieki pielęgniarskiej w oparciu o ujednoliconą terminologię.
Introduction. Lung cancer is the leading cause of cancer-related deaths in the male population. It is also a significant cause of women's death. Aim. The aim of the study is to analyze selected aspects of nursing care for a patient with lung cancer. Materials and methods. The paper analyzes literature on patient care in the course of lung cancer and mapping nursing diagnoses and interventions using the ICNP® dictionary. Conclusions. Lung cancer significantly modifies the patient's quality of life. The ICNP® classification allows you to design nursing care based on standardized terminology.

Background: Lung cancer is one of the most common malignant diseases with high mortality. Patients diagnosed with lung cancer are most likely to exhibited psychiatric symptomatology while they experience poor quality of life. Purpose: To examine the relationship between psychological distress and quality of life (QoL) in lung cancer patients receiving chemotherapy. Methods: A cross sectional study was conducted in which 110 lung cancer patients were recruited to participate. Data was collected with an anonymous self-administrated question-naire consisted by three parts: a sheet concerning demographic information, the scales Missoula Vitas Quality of Life Index-15 and General Health Questionnaires (GHQ)-28. Results: Women accounted for 51.8% of the sample, 27.3% were ≥56 years old and 24.5% were in the age between 35-44. Moreover, 38.2% were high school graduates while 46.4% were married. Age, educational level, and marital status were found to be related to patients’ perceived QoL. QoL was found to be related to psychological variables for GHQ-28. Conclusions: Quality of life can be considered to be a result of disease and treatment, as perceived by the patient and is affected by factors such as injury, anxiety, perceptions and social opportunities. This has a direct effect on patients’ functioning and ability to self-care.

Non-small cell lung cancer is one of the most commonly diagnosed cancer with a very high mortality rate. Trying to understand the mechanisms underlying the progression of this type of cancer, it is necessary to evaluate the changes occurring at molecular level in cancer cells. Besides the widely studied signaling pathways and genes which are dysregulated in NSCLC, there is a large group of non-coding RNAs involved in cancer pathogenesis. Those RNAs are tissue specific heterogeneous class of RNAs that play many functions in physiological condition in cells, nevertheless current data has shown that lncRNAs are also functional in different types of cancer. Moreover, it has been suggest that lncRNAs are involved in cancer progression by controlling key signaling pathways involved in diverse types of tumors. Notch signaling is one of those pathways, very often deregulated in NSCLC. Therefore in this review I summarized recent outcomes according the importance of lncRNAs in regulation of Notch pathway in the pathogenesis of NSCLC.

Lung cancer is the most common fatal cancer disease in the world. A characteristic feature of lung cancer is genetic diversity. In the overwhelming majority of cases, smoking is the most important etiopathogenic factor. Lung cancer is a cancer with a very bad prognosis regarding long-term survival. The risk of lung cancer depends primarily on active or passive exposure to the carcinogenic components of tobacco smoke. According to available data, the development of lung cancer in addition to active and passive smoking is directly affected by environmental pollution such as smog and fumes, ionizing radiation, mycotoxins and long-term exposure to asbestos (occupational exposure). Research on the pharmacoprevention of lung cancer began over 30 years ago. The first nutrient that the researchers said could inhibit the development of lung cancer was beta carotene. Unfortunately, long-term regular supplementation with high doses of antioxidant in the form of beta-carotene brought the opposite effect. An increase in the incidence of lung cancer was found in people who received beta carotene in the form of a synthetic food supplement. The other component tested was N-acetylcysteine. It is a sulfur compound and a powerful antioxidant that supports the synthesis of glutathione and cysteine, with destructive effects on carcinogenic substances. N-acetyl-cysteine, used in the form of NAC adduct and epigallocatechin-3-gallate, showed efficacy in inhibiting the development of lung cancer only in animal models. In the pharmacoprevention of lung cancer, the use of vitamin E was also tested in the form of tocotrienol and tocopherol. The following work also shows the existence of a high concentration correlation which belongs to the steroid hormone, mainly estrogen, in the blood and the development of lung cancer in women. An increased risk of lung cancer has been observed in women undergoing long-term hormone replacement therapy. The results show that 2-methoxyestradiol, the endogenous metabolite of 17ß-estradiol, shows positive results that inhibit the growth of lung cancer cell lines. The aim of the work was to present the correlation between tobacco abuse and passive smoking and lung cancer, pharmacoprevention of lung cancer and the association of elevated estrogen concentration in women with an increased risk of lung cancer.

Introduction. Blood biomarkers may support early diagnosis of lung cancer by enabling pre-selection of candidates for computed tomography screening or discrimination between benign and malignant screening-detected nodules. We aimed to identify features of serum metabolome distinguishing individuals with early-detected lung cancer from healthy participants of the lung cancer screening program. Methods. Blood samples were collected in the course of a low-dose computed tomography screening program performed in the Gdansk district (Northern Poland). The analysis included 31 patients with screening-detected lung cancer and the pair-matched group of 92 healthy controls. The gas chromatography coupled to mass spectrometry (GC/MS) approach was used to identify and quantify small metabolites present in serum. Results. There were several metabolites detected in the sera whose abundances discriminated patients with lung cancer from controls. Majority of the differentiating components were downregulated in cancer samples, including amino acids, carboxylic acids and tocopherols, whereas benzaldehyde was the only compound significantly upregulated. A classifier including nine serum metabolites allowed separation of cancer and control samples with 100% sensitivity and 95% specificity. Conclusions. Signature of serum metabolites discriminating between cancer patients and healthy participants of the early lung cancer screening program was identified using a GC/MS metabolomics approach. This signature, though not validated in an independent dataset, deserves further investigation in a larger cohort study.

Introduction and objective. Barley (Hordeum vulgare L.) is known as a rich source of different bioactive compounds. At present, considerable attention of researchers is focused on young barley grass. It can be a good source of dietary minerals, vitamins, carbohydrates, amino acids, phenolic compounds and proteins. It is possible that the composition of chemical ingredients beneficial for health may induce an anticancer potential of young barley in human colon adenocarcinoma (HT-29) and human lung adenocarcinoma (A549) cell lines. Materials and method. Hordeum vulgare water extract (HWE) and Hordeum vulgare juice extract (HJE) were prepared. Cell proliferation and viability were examined with the use of MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) and NR (neutral red) methods. Induction of necrosis was assessed by propidium iodide/Hoechst staining. Progress of the cell cycle involved in cell proliferation, apoptosis, and regulation of transcription was estimated using flow cytometry analysis. Additionally, the capability of free radical scavenging was evaluated with the DPPH assay. Results. The study revealed that extracts inhibited the proliferation of cancer cells. The NR study confirmed the low cytotoxic activity of the tested extracts to normal human colon epithelial cells (CCD 841 CoTr) and human skin fibroblasts (HSF). Furthermore, a dose-dependent cytotoxicity against HT-29 cells, but not A549 cells, has been reported. The free radical scavenging activity was observed in the case of the HWE but not the HJE. Conclusions. The obtained results indicate a cancer chemopreventive potential of young barley as a safe dietary agent in colon carcinoma.

Objective: The aim of the study was comparison of cancer incidence risk of lungs, stomach, colon, bladder and kidneys from ex-miners of black coal mines and the general male population of the Czech Republic. Materials and methods: The analysis was conducted in two cohorts of ex-miners according to the presence of coalworkers’ pneumoconiosis (CWP). The first cohort included the miners without CWP (N = 6705), and the second cohort included the miners who were compensated for CWP (N = 2158). Personal and occupational data was merged with the data in the National Population Register and the National Oncological Register for the period from 1992 to 2006. Cancer risk in miners in comparison to the general male population of the Czech Republic was evaluated by SIR (Standardized Incidence Ratio) and 95% confidence interval (CI). Results: About twice as high risk of lung cancer was found in miners with CWP (SIR = 2.21; 95% CI: 1.75–2.76). Lung cancer risk correlated with the severity of CWP (simple CWP SIR=1.96; 95% CI: 1.48–2.56, progressive massive fibrosis SIR = 4.29; 95% CI: 2.09–7.87). No increased risk of lung cancer was found in ex-miners without CWP. The risk of malignant neoplasm at the other selected sites was comparable with the risk in the general male population of the Czech Republic. Conclusions: This study found increased lung cancer risk in coal miners with CWP, but not in those without CWP, comparing with the general population. These results served as the basis for the inclusion of lung cancer in association with CWP into a new Czech list of occupational diseases.

Lung cancer is one of the most lethal types of cancer due to a lack of proper treatment. The rare presence of molecular therapy targets forces the use of platinum-based drugs. Cisplatin, approved by the USA as an anticancer therapy in the 1970s, is still one of the most prominent therapies against lung cancer. Unfortunately, the biggest limitation of cisplatin-based therapy is the development of cisplatin resistance. Cancer cells overcome the vast DNA damage caused by the drug in a variety of ways such as detoxication and extracellular transport of the drug, enhanced repair mechanisms, omitting apoptosis and epigenetic alterations. Chemotherapy resistance is an issue that so far cannot be dealt with. Nevertheless, better understanding of the molecular pathways behind cisplatin resistance brings hope for better therapy outcomes in lung cancer patients.

Introduction. Lung cancer is the most common cancer in the Western world. Annually there are approximately 1.8 million new cases worldwide. It is characterized by poor prognosis with a 5-year survival of 10-17% depending on the country. Contributing to this poor prognosis is a mainly late diagnosis, as well as a fairly frequent recurrence despite radical surgery. Over the years, scientists have been searching for a tumor marker that would be useful for patients with lung cancer. Aim. The aim of this study is to discuss the significance of carcinoembryonic antigen (CEA) in the diagnosis, prognosis of the disease course, and monitoring patients with lung cancer. Methods. Review of the literature using the PubMed database, Termedia, Via Medica and the key issue: carcinoembryonic antigen as a tumor marker in lung cancer. Conclusions. Serum CEA level can be a reliable complement to the diagnosis of lung cancer. It can be helpful in preoperative prediction of disease course and qualification for adjuvant treatment of non-small cell lung cancer especially adenocarcinoma. Trends and normalization of CEA during chemotherapy have an impact on progression-free survival and overall survival (OS) of patients. Various available publications describe CEA as a marker for metastatic lung cancer, which is the most specific for metastasis in the liver and brain.

There was presented an exstremely rare case of adenoma oxyphillicum of the larynx coexisting with the lung carcinoma. Diagnostical and medical conduct was throughly analyzed. It consisted such matters as detailed graphical examination, spliting of larynx, and radiotherapy and chemiotherapy of lung cancer. The authors want to call attention to limited use fullness of competent lungs X-ray in a-p projection in recognizing the early stadium of lung cancer.

Angioedema is a localized and self-limiting oedema of the subcutaneous and submucosal tissue. Case presentation: 73-year-old man was admitted to the Division of Allergology with the diagnosis of angioedema. Detailed physical examination led to the suspicion of superior vena cava syndrome with secondary face swelling. Biopsy revealed metastasis of small cell carcinoma in superclavicular lymph nodes. Conclusions: Facial swelling, especially recurrent and non-inflammatory, is often, but not always caused by angioedema. Insightful differential diagnosis is crucial before making the final one.

Non-small cell lung cancer (NSCLC) accounts for the majority (85%) of all lung cancers. Although many therapies are available, 35–50% of patients with stage I or II NSCLC develop recurrence and metastasis. This study was designed to investigate the anti-tumor activity of cyanidin (Cy) and peonidin (Pn) on NSCLC cells (SPCA-1). SPCA-1 cell proliferation, cell cycle and early apoptosis were investigated after treatment with Cy and Pn. The underlying signaling mechanism was also explored by detecting the levels of apoptosis-related proteins using enzyme-linked immunosorbent assay (ELISA). Cy and Pn inhibited the viability of SPCA-1 cells with an IC50 of 141.08 μg/mL and 161.31 μg/mL, respectively. Meanwhile, Cy and Pn induced cell cycle arrest at G2/M phase. Cy and Pn treatment significantly increased the levels of Bax, P53, and Caspase-3, while decreasing that of Bcl-2, thereby inhibiting the growth of SPCA-1 cells. In conclusion, Cy and Pn induced early apoptosis of NSCLC cells through regulation of the levels on Caspase-3, Bax, Bcl-2, and P53. These results suggest Cy and Pn as potential anticancer drugs for the treatment of lung cancer.

Lung cancer is associated with an increased risk of venous thromboembolism, including pulmonary embolism. In some situations, clinical deterioration in patients with lung cancer accompanied by elevated pressure in the right ventricle, usually measured by commonly used echocardiography, may support the diagnosis of pulmonary embolism. However, there are many other causes of increased pressure in the right ventricle in such patients, for example: progression of cancer, pre-existing lung diseases, surgical resection of pulmonary tissue, pnemotoxicity of radiotherapy or concomitant diseases of the left heart. The article presents 2 clinical cases of patients with lung cancer, in which elevated pressure in the right ventricle was resulted from other causes and accompanied the progression of cancer, despite the clinical picture suggesting a pulmonary embolism. Increased pressure in the right ventricle and usually associated pulmonary hypertension, significantly worsen already poor prognosis of patients with lung cancer. The differential diagnosis should, therefore, take into account the whole clinical picture, excluding venous thromboembolism as an important cause of pulmonary hypertension, but also take into account other potential factors to be able to make the right diagnosis and implement optimal treatment as early as possible.

Significant, unexplained disparities in the incidence of lung cancer in men and breast cancer among some neighbouring populations of the Pomorskie voivodeship poviats were revealed. They were not found for lung cancer in women. Examinations were conducted on data from the Pomeranian Cancer Registry. A hypothesis was put forward that the observed accumulation of certain surnames in certain poviats argues for the repetitive occurrence of certain features of the genotype in populations inhabiting these areas, which may have an influence on the emergence of certain cancers and play a role in the emergence of the above mentioned disparities.

Introduction. The paper presents the results of a study of cell cultures of lung cancer and kidney cancer using lycopene performed using clinical magnetic resonance imaging. Aim. The aim of the study was to evaluate lycopene activity on tumor cell cultures. Material and methods. For this purpose, MR tests were performed using the technique of determining transverse relaxation. Results. Described here studies demonstrated that lycopene may inhibit the growth of A549 and ACHN cell lines. Conclusion. We determine changes in spin lattice relaxavity T2 to monitor treatment of lung cancer cell line A549 and kidney cancer cell line ACHN cells treatment with lycopene.

Vena cava superior syndrome (VCSS) is a sudden life-threatening condition encountered in patients with neoplasms. The prognosis depends on histopathological diagnosis, severity of clinical symptoms and administered treatment. Depending on the type of neoplasm, the treatment of choice may be radiotherapy or chemotherapy. In patients with rapidly increasing clinical symptoms the justified management is percutaneous balloon angioplasty of the superior vena cava with the placement of stents.

Objectives The correlation between quartz dust concentrations in the Swiss enterprises and the incidence of silicosis and other related diseases acknowledged as occupational diseases (OD) was investigated. Material and Methods Quartz dust concentrations were obtained from Suva’s databases of occupational health surveillance measurements between 2005 and 2014. Information on quartz dust-related diseases was from medical dossiers of workers with OD acknowledged by Suva between 2005 and 2014. Results The median quartz dust concentration of the 2579 measurements between 2005 and 2014 was 0.09 mg/m³ (alveolar fraction). Out of all measurements, 28% were above the Swiss occupational exposure limit (OEL) of 0.15 mg/m³ (alveolar fraction). One hundred eighty-one individuals suffered from acknowledged quartz dust-related disease (179 silicosis and 2 chronic obstructive pulmonary disease (COPD)). Additionally, 8 out of these workers were diagnosed with lung cancer and 55 with COPD of a non-specified cause. Out of all workers, 46% were exposed to silica dust for the first time before 1975 when the current Swiss OEL was introduced. Out of the foreign workers, 63% began to work abroad, during which they could have at least partly acquired their silicosis. Out of all workers, 75% were ever-smokers. Conclusions The incidence of silicosis decreased drastically from approximately 300 cases/year in the 1970s to fewer than 20 cases/year 20 years ago. Several findings of this study that could help to interpret the ongoing occurrence of the disease include excessive exposure in or outside of Switzerland in former or current times, vulnerability to the development of silicosis due to cigarette smoke, or poor compliance with wearing breathing masks. Wstęp. Rak płuc jest to nowotwór złośliwy o bardzo heterogennej biologii i złym rokowaniu. Nowotwór ten jest najczęstszą przyczyną zgonów. Rak ten ma objawy o charakterze nieswoistym, które pojawiają się późno. Nowotwór płuc dotyczy osób starszych. Głównymi czynnikami ryzyka powstawania raka płuc są: aktywne palenie tytoniu, bierne palenie tytoniu. Rak płuca przez bardzo długi czas nie daje objawów i dolegliwości. Głównymi problemami, z którymi musi uporać się pielęgniarka są: ból w klatce piersiowej, nasilająca się duszność spoczynkowa, kaszel, krwioplucie, gorączka. Podstawowymi badaniami służącymi do diagnostyki tego nowotworu są badania krwi oraz RTG klatki piersiowej. Najczęstsze metody leczenia to chemioterapia, radioterapia oraz leczenie chirurgiczne. Cel. Celem pracy jest określenie zadań sprawowanych przez pielęgniarkę w planie opieki wdrażanej wobec pacjenta z rakiem płuc. Prezentacja przypadku. Pacjent lat 66 przyjęty do oddziału chorób płuc w trybie pilnym w celu pogłębienia diagnostyki zmiany guzowatej płuca lewego w wykonanym ambulatoryjnie RTG klatki piersiowej. Jest to druga doba pobytu chorego w oddziale. Jest nałogowym palaczem tytoniu. Dodatkowymi chorobami są refluks żołądkowo-przełykowy oraz przerost gruczołu krokowego i nadciśnienie tętnicze. Wnioski. Dominującymi problemami pielęgnacyjnymi pacjenta z rakiem płuc są: duszność spoczynkowa, męczący kaszel z odkrztuszaniem krwistej wydzieliny, ból w klatce piersiowej, wzmożona potliwość, zaparcia. Zadaniem pielęgniarki w opiece nad pacjentem z rakiem płuc jest udział w minimalizowaniu wszelkich dolegliwości wynikających z choroby, asystowanie przy zaspokajaniu potrzeb pacjenta, wsparcie psychiczne dla pacjenta oraz jego rodziny w przebiegu choroby o niepomyślnym rokowaniu.
Introduction. Lung cancer is a malignant tumor with a very heterogeneous biology and poor prognosis. It is derived from the epithelial cells of the bronchial tree and alveoli. The aim. This work is aimed to analyze the tasks of a nurse in the care of a lung cancer patient based on a case-by-case study. Case study. Patient aged 66, was admitted to the lung diseases ward as a matter of urgency in order to deepen the diagnosis of nodular lesions of the left lung on an outpatient chest X-ray. It is the second day of the patient's stay in the ward. He is a heavy smoker. Additional diseases are gastroesophageal reflux, prostatic hyperplasia and arterial hypertension. Conclusions. The dominant nursing problems of a patient with lung cancer are: dyspnea at rest, exhausting cough with coughing up bloody discharge, chest pain, increased sweating, constipation. The nurse's task in caring for a patient with lung cancer is to participate in minimizing any ailments resulting from the disease, assisting in satisfying the patient's needs, providing psychological support to the patient and his family in the course of an unfavorable disease.

Lung cancer is the most common neoplasm on over the world and remains the leading cause of cancer death in both men and women. The predominant cause of lung cancer is exposure to tobacco smoke. The methods using for diagnosis and monitoring of the disease are based on radiological, cytological and histopathological examinations. The prognosis for survival in lung cancer is very poor which is mostly caused by late diagnosis when the disease gets the last phases. The prevention methods used for early detection of lung cancer include X-ray chest examination, cytological sputum examination and chest computer tomography (CT). In that article we present the case of history of 55 years old patient, heavy smoker, who was admitted to the hospital because of tumour of right lung and right hilus showed on chest X-ray. The bronchoscope revealed extended changes in bronchium for the left lower lobe, which were not present on chest X-ray but only in later done CT. If there were no changes in right lung, probably the diagnostic examinations in that patient, with high risk of lung cancer, would be performed much later. That is because the present guidelines do not recommend that low-dose CT should be used to screen for lung cancer.
Rak płuca jest najczęstszym nowotworem złośliwym na świecie, występuje kilkakrotnie częściej u mężczyzn niż u kobiet. Stanowi pierwszą przyczynę zgonów z powodu chorób nowotworowych u mężczyzn i drugą u kobiet. Najbardziej znanym czynnikiem ryzyka zachorowania na raka płuca jest palenie tytoniu. W rozpoznawaniu i monitorowaniu przebiegu choroby wykorzystuje się badania obrazowe i badania morfologiczne, które służą do wykrywania komórek nowotworowych. Rak płuca należy do najgorzej rokujących nowotworów, wynika to głównie z późnego rozpoznawania choroby. Profilaktyka wtórna raka płuca obejmuje badania, które polegają na wykrywaniu wczesnych stadiów nowotworu. Do badań tych należą okresowe badania RTG klatki piersiowej, badania cytologiczne plwociny oraz tomografia komputerowa. W pracy przedstawiamy przypadek diagnozowanego 55-letniego chorego, wieloletniego palacza papierosów. Chory został przyjęty do kliniki z powodu widocznych na zdjęciu RTG klatki piersiowej zmian guzowatych w okolicy wnęki oraz w samej wnęce płuca prawego. Badanie bronchoskopowe ujawniało obecność rozległych zmian naciekowych w oskrzelu dolnopłatowym płuca lewego, co potwierdziło radiologicznie dopiero badanie komputerowe. Gdyby nie obecność zmian w płucu prawym, to omawiany chory, należący do grupy wysokiego ryzyka zachorowania na raka płuca, nie zostałby skierowany na badania diagnostyczne. Aktualne wytyczne dotyczące postępowania w raku płuca nie zalecają wykonywania tomografii komputerowej z użyciem małej dawki promieniowania jako badania przesiewowego w kierunku raka płuca.

Lung cancer is one of the most common types of cancer diagnosed, and the development of methods to image diseased lung tissue by MRI is of utmost importance. Contrast-Enhanced Magnetic Resonance Imaging (CE-MRI) was used to noninvasively evaluate spin-spin relaxation time, T1, of lung cell cultures infused with various clinical gadolinium-based contrast media for imaging. In this study we used a clinical 1.5 Tesla scanner and the contrast agents: Omniscan, MultiHance, Gadovist and ProHance. A significant five-fold reduction of T1 relaxation time was obtained.

Rak płuc jest najczęstszą przyczyną zgonów z powodu nowotworów złośliwych. Pomimo rozwoju medycyny wciąż głównymi metodami walki z rakiem płuc są operacje chirurgiczne oraz radioterapia i chemioterapia, które stanowią alternatywę dla pacjentów niekwalifikujących się do zabiegu operacyjnego. W podjęciu decyzji dotyczącej zastosowania poszczególnych technik leczenia pod uwagę są brane: stadium zaawansowania choroby, stan ogólny pacjenta i choroby współistniejące. W związku z licznymi powikłaniami występującymi w przebiegu stosowania głównych metod terapii oraz brakiem pewności co do całkowitego wyleczenia opracowywane są coraz to nowsze, skuteczniejsze rozwiązania. Artykuł zawiera krótkie opisy wybranych najnowszych sposobów w terapii niedrobnokomórkowego raka płuc (NDRP), tj. radioterapii stereotaktycznej z wykorzystaniem robota Cyber Knife, chemioterapii z zastosowaniem cysplatyny, leczenia za pomocą drobnocząsteczkowych inhibitorów tyrozynowej kinazy oraz ablacji prądem o wysokiej częstotliwości radiowej (radiofrequency ablation – RFA). Wymienione techniki są znacznie skuteczniejsze niż standardowe, a ponadto obserwuje się mniejsze zagrożenie wystąpienia powikłań po ich zastosowaniu. Z dostępnego piśmiennictwa wynika, że obecnie prowadzone są badania kliniczne nad nowymi obiecującymi metodami. Działania w kierunku poszukiwania coraz to nowszych sposobów leczenia rokują na przyszłość dając nadzieję na znalezienie niezawodnej metody, pozwalającej na całkowity powrót do zdrowia chorych ze zdiagnozowanym rakiem płuc.
Lung cancer is the leading cause of death from cancer. Despite the development of medicine is still the main methods of struggle with lung cancer are surgery and radiotherapy and chemotherapy, an alternative for patients ineligible for surgery. The decision regarding the use of various treatment techniques are taken into account: stage of disease, general condition of the patient and concomitant diseases. Following a number of complications occurring in the course of the main ways of treatment and the lack of certainty as to cure the disease are developed newer, more effective solutions. The article contains brief descriptions of the latest methods in the treatment of small cell lung cancer (NSCLC), ie, stereotactic radiotherapy using a robot Cyber Knife, chemotherapy with cisplatin, treatment with small molecule tyrosine kinase inhibitors and the ablation of high-frequency RF (Radiofrequency Ablation – RFA). These techniques are much more effective than standard, and also observed a lower risk of complications after their use. The available literature shows that current clinical trials are ongoing to develop new promising methods. Efforts to search for newer and newer therapies hold promise for the future by giving hope to find a reliable method that allows for complete recovery of patients diagnosed with lung cancer.

Objectives This study has researched the significance of histologically raised findings and lung dust analyses in the context of claiming the recognition of and thus compensation for an asbestos-associated occupational disease. Material and Methods For this approach, all findings from the German Mesothelioma Register in 2015 that included lung dust analyses were evaluated and were compared with information on asbestos fiber exposure at work based on fiber years, and with the results of radiological findings. Results For 68 insured persons, recognition of an asbestos-induced lung disease according to Section 4104 of the German Ordinance on Occupational Diseases (Berufskrankheitenverordnung – BKV) could be recommended solely on the basis of the histological examinations of lung tissues and complementary lung dust analyses. Neither did the calculation of the cumulative asbestos dust exposure at work yield 25 fiber years, nor could bridge findings (e.g., plaques) be identified. In addition, the autopsies of 12 patients revealed plaques that had not been diagnosed during radiological examinations. These results show that – irrespective of the prescribed working techniques and radiological diagnosis – pathological/anatomical and histological diagnostics are often the only way for the insureds to demonstrate the causal connection between asbestos and their disease. Even after long intervals of up to 40 years post last exposure, the asbestos fibers would still be easily detectable in the lung tissues evaluated. Conclusions Whenever suitable tissue is available, it should be examined for mild asbestosis with the aid of a lung dust analysis. Otherwise there is a risk that an occupational disease is wrongfully rejected. In the context of health insurance, the lung dust analysis and the resulting proof of the presence of asbestosis often constitute one option of providing evidence of an occupational disease. In the current study, thermodynamic dissolution of brigatinib in six pure solvents namely, water, ethanol, isopropyl myristate (IPM), poly(ethylene glycol)-400 (PEG-400), dichloromethane (DCM) and ethyl acetate (EA) at (298.2-323.2K) temperature and 0.1 MPa atm. pressure were carried out using flask shaking technique.The mole-fraction solubility of brigatinib was obtainedgreatest in IPM (6.09 x 10-2) and least in water (3.12 x 10-6) at298.2 K.Experimentally obtained solubilities values of brigatinib were regressed using adapted Apelblat equation with root mean square deviations in the range of 1.91 to 4.67 in all solvents and similar trends were observed with increased the temperature. The solubility data generated in this investigation could be helpful for investigator for analysis/dosage form development of brigatinib.

Introduction. Air pollution is one of the most important issues of our times. Air quality assessment is based on the measurement of the concentration of substances formed during the combustion process and micro-particles suspended in the air in the form of an aerosol. Microscopic atmospheric particulate matters (PM) 2.5 and 10 are mixtures of organic and inorganic pollutants smaller than 2.5 and 10 μm, respectively. They are the main cause of negative phenomena in the earth’s atmosphere of Earth and human health, especially on the respiratory and cardiovascular systems. Particulates have the ability to cause permanent mutations of tissue, leading to neoplasms and even premature deaths. Nitrogen dioxide (NO2) is one of the main pollutants which arises mainly during the burning of fossil fuels. Based on numerous scientific researches, it has been proved that long-term exposure to NO2 could increase morbidity of cancer due to inflammatory processes increasing abnormal mutations. Materials and method. Data available in the Polish National Cancer Registry, Chief Inspectorate for Environmental Protection and Map of Health Needs in the Field of Oncology for Poland, WHO Air Quality Guidelines 2005 were analyzed. Air pollution was also evaluated: PM2.5, PM10, NO2, and compared with lung cancer morbidity. Results and conclusions. Based on the available data and literature, it can be concluded that in 2009–2017, on average, each Pole smoked ten cigarettes a day +/- 2. Therefore, it can be estimated that after 60 years everyone had 30 package-years of smoking, leading to a high risk of lung cancer and other smoking related diseases. Additionally air quality in Poland is not satisfactory, exceeding the standards presented in the WHO Guidelines 2005. It can be assumed that this may translate into an additional, independent continuous increase in morbidity and mortality dependent on smoking.

Telomerase, undetectable in normal somatic cells, plays a critical role in carcinogenesis of the majority of human tumors including lung carcinoma. The aim of our study was to determine human telomerase reverse transcriptase (hTERT) mRNA expression in patients with non-small cell lung cancer (NSCLC) in order to estimate its usefulness as diagnostic and/or prognostic factor. hTERT expression was analyzed in a group of 12 females and 28 males with NSCLC using Quantitative Real-Time Polymerase Chain Reaction (QRT-PCR method) in cancerous and non-cancerous lung tissues. Results were analyzed according to clinical data and one-, two-, and five-year survival rates. hTERT expression in the cancerous tissue was significantly higher than in the lung parenchyma free from neoplasm infiltration (p<0.05). There was no significant association between hTERT expression in the tumor tissue and age, gender, grading or clinical stage. A significant difference in hTERT expression between two types of histopathological patterns (adenocarcinoma and squamous cell carcinoma) was detected (p=0.01). No association between hTERT expression in NSCLC specimens and survival rates was found. hTERT mRNA detection by QRT-PCR in tumor and corresponding cancer-free tissues can be used as a diagnostic marker in patients with NSCLC, but seems not to be a prognostic factor.

Sealed can technique using CR-39 plastic track detector strippable has been used in order to measure radon concentrations to calculate the annual effective dose and lung cancer cases per year per million person for twenty different brand tobacco cigarette were collected from Iraqi market. Lung cancer is the leading cause of cancer-related deaths worldwide. Radon exposure is the second leading cause of lung cancer, following tobacco smoke. Radon is not only an independent risk factor; it also increases the risk of lung cancer in smokers. It has been observed that tobacco smoking increases the risks of radon-induced lung cancer. Radon and cigarette smoking have synergistic, multiplicative effect on lung cancer rates. People who inhale tobacco smoke are exposed to higher concentrations of radioactivity. Ever since studies on the relation of smoking to cancer, particularly the lung cancer has been established, there had been a great interest in studies concerned with the monitoring of the alpha radioactivity in tobacco. The results showed that the 222Rn concentrations in cigarette tobacco samples ranged from (156.450 to 403.087) Bq/m3 with an average 284.751 Bq/m3, while the radon induced lung cancer risks was found to vary from 71.047 to 183.049 with an av¬erage value of 129.3107 per million person, and standard deviation 83.529 and 37.932, respectively. Excellent correlation has been observed between radon concentration and lung cancer cases per year per million person for different brand tobacco cigarettes.

Background and objectives: To justify the concept of validating conformal versus intensity-modulated approach in the treatment of non-small cell lung cancer (NSCLC). Materials and methods: For 10 patients representative of the spectrum of tumour sizes and locations, two plans were prepared: one with three-dimensional conformal radiation therapy (3DCRT) technique and the other with intensity-modulated radiation therapy (IMRT) technique. Preliminary measurements were performed in static conditions. For each of the fi eld angles considered, the motion kernel was generated to simulate tumour motion trajectories, with the largest amplitude in the cranio-caudal direction of 4, 6, and 8 mm. The measurement results determined the agreement between the planned and measured doses. Results: No statistically signifi cant differences were found between the motion patterns, with the smallest amplitudes for clinical target volume in 3DCRT. For IMRT, the signifi cant differences between 0 mm vs. 6 mm and 0 mm vs. 8 mm amplitudes were found. The motion impact on delivered vs. planned doses had less effect on the oesophagus in 3DCRT compared to that in IMRT. The observed differences were comparable for the heart. Interpretation and conclusions: For maximal amplitudes below 4 mm, the disagreement between planned and delivered doses can be neglected. However, the amplitudes above 5 mm and 7 mm lead to signifi cant changes in IMRT and 3DCRT dose distribution, respectively.

Opis:
Anatomical resection of the lung parenchyma is the treatment of choice at an early stage of lung cancer. The inevitable result of these operations is the loss of the respiratory surface causing varying degrees of respiratory dysfunction, depending on the extent of the procedure and the operated side. Therefore, from the first hours after the operation, patients should undergo comprehensive rehabilitation, which is fundamental to minimize the risk of complications and improve respiratory function. Due to the popularization of the concept of enhanced recovery after surgery (ERAS), the importance of effective rehabilitation is increasing. One of the more common complications after anatomical resection of the lung parenchyma is persistent air leakage. There are no guidelines in the literature regarding rehabilitation in this group of patients. This paper describes the physiotherapy of patients after resection of the lung parenchyma complicated by persistent air leakage.
Anatomiczna resekcja miąższu płuca jest leczeniem z wyboru we wczesnym stadium zaawansowania raka płuca. Nieuniknionym skutkiem tych operacji jest ubytek powierzchni wymiany gazowej powodujący różnego stopnia, w zależności od rozległości zabiegu oraz operowanej strony, zaburzenie czynności oddechowej. Dlatego od pierwszych godzin po operacji pacjenci powinni być poddawani kompleksowej rehabilitacji, która ma podstawowe znaczenie dla minimalizacji ryzyka powikłań oraz poprawy funkcji oddechowej. Ze względu na upowszechnianie się koncepcji kompleksowej opieki okołooperacyjnej (ang. Enhanced recovery after surgery – ERAS), znaczenie skutecznego postępowania rehabilitacyjnego jest coraz większe. Jednym z częstszych powikłań po anatomicznej resekcji miąższu płuca jest utrzymujący się po zabiegu przeciek powietrza. W piśmiennictwie brak jest wytycznych dotyczących postępowania rehabilitacyjnego w tej grupie pacjentów. W niniejszej pracy opisano fizjoterapię pacjentów po zabiegu resekcji miąższu płuca powikłanym utrzymującym się przeciekiem powietrza.

Background. It is recognised that both nutritional status and an improper diet have significant effects on weakening the outcomes of treatment in cancer patients. As a result, a lowered response to therapy and an increase in untoward side effects is often observed leading to a deteriorating quality of life. The role of an adequately balanced diet is thus regarded as being vital in supporting recovery. Objective. To assess the dietary consumption of calories, macro-elements and selected vitamins and minerals for subjects diagnosed with cancers of the breast, lungs and bones or soft tissue. Material and Methods. A survey was performed on 100 subjects diagnosed with various tumours between the September and December months of 2011 consisting of 34 with breast cancer, 33 lung cancer and 33 with bone or soft tissue cancer. The questionnaire was devised in-house, which included a three day dietary record. Results. The average daily calorific intake was found to be inadequate at 1608 kcal. In addition, abnormal proportions of energy derived from macro-elements was seen, where the contributions made by fats and proteins were somewhat high at respectively 35.1% and 16.5%, but too low in the case of carbohydrates at 52.1%. Up to 78% subjects had insufficient protein intakes, 88% showed deficiencies in consuming carbohydrates, as were 89% for fibre, 85% vitamin C, 99% calcium, 98% magnesium and 81% for iron. Conclusions. Many dietary shortcomings were observed in the studied subjects. There is therefore a need to educate persons suffering from cancer to adopt an adequate and balanced diet as means of providing vital support for treatment to be more effective.
Wprowadzenie. Nieprawidłowy sposób żywienia oraz stan odżywienia osób z chorobą nowotworową wpływa na słabszą odpowiedź na leczenie przeciwnowotworowe, może nasilać jego objawy uboczne oraz wpływać na jakość życia. Prawidłowo zbilansowana dieta pełni wspomagającą rolę w powrocie do zdrowia. Cel pracy. Celem badań była ocena spożycia energii, makroskładników oraz wybranych witamin i składników mineralnych u osób z diagnozowaną chorobą nowotworową piersi, płuc oraz kości i tkanek miękkich. Materiał i metody. Badanie przeprowadzono w okresie od września do grudnia 2011 roku. Badaną grupę stanowiło 100 osób ze zdiagnozowaną chorobą nowotworową (34 osoby z nowotworem piersi, 33 osoby z nowotworem płuc i 33 osoby z nowotworem kości i tkanek miękkich). Badania przeprowadzono przy użyciu autorskiej ankiety oraz metody 3-dniowego bieżącego notowania. Wyniki. Spożycie energii przez osoby z chorobą nowotworową było niewystarczające i średnio wynosiło 1608 kcal/dzień. Procentowy udział poszczególnych makroskładników w dostarczaniu energii był nieprawidłowy, w przypadku tłuszczu i białka był zbyt wysoki (odpowiednio 35,1% oraz 16,5%), a w przypadku węglowodanów zbyt niski (52,1%). Aż 78% badanych nie realizowało normy na białko, w przypadku węglowodanów wartość ta wynosiła 88%, a dla błonnika 89%. Spożycie witaminy C było zbyt niskie u 85% badanych, wapnia u 99%, magnezu u 89%, a żelaza u 81% badanych. Wnioski. W ocenianych racjach pokarmowych pacjentów obserwowano liczne nieprawidłowości. Istnieje potrzeba edukacji osób z chorobą nowotworową w zakresie zasad prawidłowego żywienia i znaczenia bilansowania diety.

Aims of the study: An assessment of the incidence and severity of suicidal thoughts and depression in a group of patients with lung cancer. Furthermore, we have investigated whether the increase in suicidal thoughts and depression depends on pain intensity and the overall physical function of patients. Material and methods: The study included 62 patients diagnosed with lung cancer, hospitalised in the Department of Thoracic Surgery at the Thoracic Surgery Clinic in Lublin, at the Independent Public Clinical Hospital No. 4 in Lublin. The patient population included 21 females (34%) and 41 males (66%). A total of 35 (56%) respondents came from rural regions, and the remaining 27 (44%) from the city. The average age was 59 years (standard deviation was 12.6). The severity of depression and suicidal thoughts was evaluated using the Beck Depression Inventory. In addition, Numeric Pain Intensity Scale and the Karnofsky Scale were used to assess the general functioning of the study population. Sociodemographic variables were analysed using a self-designed survey. Results: Suicidal thoughts occurred in 6.45% of respondents, exclusively in males. Statistical analysis showed that the group with depression received significantly less points in a scale measuring general functioning. There was a negative correlation between depression severity and the general functioning of patients. Depression was diagnosed in 32.25% of respondents, including mild in 25.8%, moderate in 6.45%, and severe in 4.83%. Depression affected 36.58% of men and 23.8% of women. No correlation between the severity of depression and the severity of pain was observed. Conclusions: The severity of depression increases with the decrease in overall physical functioning and is not related to pain severity.
Celem badań było określenie częstości występowania oraz stopnia nasilenia myśli samobójczych, a także stopnia występowania i nasilenia depresji w grupie osób z rozpoznanym rakiem płuc. Ponadto autorzy analizowali, czy stopień nasilenia myśli samobójczych oraz depresji zależy od stopnia nasilenia bólu, jak również stanu ogólnego funkcjonowania pacjentów. Materiał i metody: W badaniu wzięły udział 62 osoby, pacjenci ze zdiagnozowaną chorobą nowotworową płuc przebywający na Oddziale Torakochirurgii w Klinice Klatki Piersiowej w Lublinie, Samodzielnego Publicznego Szpitala Klinicznego nr 4 w Lublinie. Wśród nich było 21 kobiet (34%) oraz 41 mężczyzn (66%). Trzydzieści pięć (56%) osób pochodziło ze wsi, pozostałe 27 (44%) z miasta. Średnia wieku wynosiła 59 lat (odchylenie standardowe 12,6). Do zbadania stopnia nasilenia depresji i występowania myśli samobójczych posłużono się Inwentarzem Depresji Becka. Dodatkowo wykorzystano Numeryczną Skalą Nasilenia Bólu oraz Skalę Karnofsky’ego do oceny stanu ogólnego funkcjonowania badanych osób. W celu określenia zmiennych socjodemograficznych posłużono się ankietą autorską. Wyniki: Myśli samobójcze wystąpiły u 6,45% badanych, wyłącznie u mężczyzn. Analiza statystyczna wykazała, że grupa osób z depresją otrzymała istotnie mniej punktów w skali mierzącej ogólne funkcjonowanie (Skala Karnofsky’ego). Wystąpiła negatywna zależność między nasileniem depresji a ogólnym funkcjonowaniem pacjentów. Depresję rozpoznano u 32,25% badanych, z czego u 25,8% miała ona charakter łagodny, u 6,45% umiarkowany, a 4,83% ciężki. Depresja występowała u 36,58% mężczyzn i u 23,8% kobiet. Nie zaobserwowano zależności między nasileniem depresji a nasileniem odczuwanego bólu. Wnioski: Nasilenie depresji zwiększa się wraz ze spadkiem ogólnego funkcjonowania fizycznego i nie jest powiązane z nasileniem dolegliwości bólowych.

Introduction: Tyrosine kinase inhibitors (TKI) are the standard of treatment in patients with advanced non-small cell lung cancer (NSCLC) with EGFR (endothelial growth factor receptor) gene activating mutation. Objective: The evaluation of the efficacy and toxicity of TKI drugs in NSCLC patients treated in single centre. Material and methods: NSCLC patients treated with TKI (gefitinib, erlotynib, afatinib) between 2012– 2016 were retrospectively analysed. We evaluated: overall response rate (ORR) which is the sum of complete responses (CR) and partial remissions (PR), progression free survival (PFS), overall survival (OS) and adverse events (AE) according to CTCAE (Common Terminology Criteria for Adverse Events) scale. Results: The study group were 16 patients ORR was 50% (CR: 1, PR: 7). Median PFS and OS was 8,7 and 22,9 months respectively. Adverse events observed mainly in stage 1 and 2 were related to hyponatraemia, hyperbilirubinemia, skin toxicity and mucositis. There was one death reported due to infectious complications. Conclusion: The efficacy and toxicity of TKI in study group were found to be similar to those described in the literature.

Background To improve the effectiveness of lung cancer screening using low-dose computed tomography (LDCT), the presence of smoking-related comorbidities that may significantly affect mortality in this group should be taken into account. Material and methods A questionnaire survey and spirometry tests were conducted in a group of 730 respondents as part of a lung cancer screening study between 2016 and 2018. People diagnosed with COPD underwent a three-year follow-up to assess the incidence of medical events. Results Our study confirmed that cardiovascular diseases (CVDs) were the most common comorbidities in patients who were diagnosed with COPD and participated in LDCT lung cancer screening. Among the CVDs, the most common were arterial hypertension (45.8%) and coronary artery disease (12.5%). Tobacco-related diseases (e.g. CVD, lung cancer, and exacerbations of COPD) were the leading causes of emergency department visits and hospitalizations. The number of visits due to COPD in specialized clinics more than doubled in the observed period. Conclusions Properly planned screening tests allow not only for the detection of the disease for which they were designed but also for the assessment of comorbidities. In patients undergoing lung cancer screening, it is justified to extend the diagnostics to include spirometry.

Diagnosis of lung cancer (LC) has been fraught with difficulty and by the time of definitive diagnosis, most patients are in later stages of the disease. Epidemiological studies have demonstrated that lifestyle behaviors play an etiological role in LC risk; however data in the literature on this topic often appears inconclusive or require further study. Understanding of the mechanisms operating between lifestyle patterns and their impact on LC is important for the disease’s prevention and treatment. The purpose of this study was to review the current evidence on the role of diet, body mass index (BMI), physical activity, smoking, alcohol consumption, and sex hormone use in LC development based on meta-analyses, systematic reviews and previously published epidemiologic studies. Regarded as the foremost cause of LC, evidence from studies have indicated that tobacco smoking causes LC. Additionally, exposure to outdoor air pollution and/or occupational-related exposures increase LC risk. Further, frequent consumption of red meat, processed meat increases adenocarcinoma and squamous cell carcinoma. Inverse associations between the disease risk and BMI ≥25 kg/m2, higher level of physical activity, and fruit and vegetable consumption with a high frequency were reported. Future studies are warranted to validate the association between histologic subtypes of LC and lifestyle patterns.

According to the age-period-cohort projection of lung cancer incidence (ICD-10 codes C33-C34), smoking prevention will be crucial for both men and women longevity. Lung cancer remains the main cancer cause of death among males in Poland. In 2014 the number of deaths in Poland caused by lung cancer among men was more than twice as high as for women. The study is focused on identification of attitudes influencing the propensity to smoke, which is performed by structural equation modelling (SEM) based on weighted least squares means and variance adjusted (WLSMV) estimation. Good fit of the presented models is demonstrated by RMSEA, NNFI and CFI values. Reliability of scales is assessed by ordinal alpha and ordinal theta coefficients. Commitment to learning in childhood exhibits the highest standardized effect on propensity to smoke, which is an argument for youth smoking prevention. Structural equation model was estimated for men and women separately because of gender differences in smoking determinants. A strong character, individualism and self-reliance are the factors, which increase smoking propensity among males. These can be treated as features of masculinity and are not statistically important determinants of being a smoker for females. Depressive disorders, being a victim of violence as adults are statistically significant determinants of smoking among women.

Background. Persistently high incidence of ‘diseases of affluence’ (including cancer) motivates numerous research teams to look for causes of morbidity, as well as to search for preventive methods and effective therapeutic measures. The paper aims to present the literature on effects of selenium (Se) on prevention of gastrointestinal and lung cancer. Material and methods. Based on national and international literature, the paper presents information on the role of selenium (Se) in prevention of cancer – with special consideration given to gastrointestinal and lung cancer. Results. The results of national and international research show the importance of selenium in prevention and treatment of cancerous diseases, including digestive tract cancer and lung cancer. Numerous studies have shown that the risk of cancer for people with low selenium levels is twice as high as for people with high levels of selenium in blood serum. The most prominent relationship between low selenium level in serum and cancer is observed in gastrointestinal cancer, lung cancer, breast cancer in women, and prostate cancer. Conclusions. Many clinical and experimental studies carried out nationally and internationally gathered evidence which indicate the vital role played by selenium in prevention and treatment of diseases – especially of cancer. It should be emphasised that the intake of selenium in amounts necessary for proper functioning - given numerous deficiencies (first in the soil, then in food products coming from selenium-deficient soil) – is merely the first step, as it has already been proven in numerous publications that carbohydrates (especially simple sugars, sweets, cakes, etc.) ‘destroy’ selenium in the human body.
Wprowadzenie. Wysoka zachorowalność na choroby cywilizacyjne (w tym na choroby nowotworowe) skłania liczne zespoły badawcze do poszukiwania odpowiedzi dotyczącej przyczyn zachorowalności na nowotwory, jak i poszukiwania metod prewencji i skutecznych środków terapeutycznych. Celem niniejszej pracy jest przedstawienie w świetle literatury przedmiotu wpływu selenu (Se) na zapobieganie nowotworom przewodu pokarmowego i płuc. Materiał i metody. W pracy zaprezentowano na przykładzie dostępnej literatury przedmiotu (krajowej jak i zagranicznej) informacje dotyczące roli selenu (Se) w profilaktyce chorób nowotworowych – ze szczególnym uwzględnienie chorób nowotworowych przewodu pokarmowego i płuc.. Wyniki. Wyniki badań krajowych i światowych pokazują jak ogromne znaczenie ma wpływ selenu w profilaktyce i leczeniu chorób nowotworowych, w tym: przewodu pokarmowego i płuc. Liczne badania wykazały, że ryzyko raka dla osób z niskim poziomem selenu jest dwa razy większe niż dla osób z wysokim poziomem selenu w surowicy krwi. Najwyraźniejszy związek między niskim poziomem selenu w surowicy krwi a nowotworami jest dla nowotworów przewodu pokarmowego, płuc, gruczołu piersiowego u kobiet i prostaty. Wnioski. W wielu badaniach klinicznych i eksperymentalnych przeprowadzonych w kraju i na świecie zebrano dowody wyraźnie wskazujące na dużą rolę, jaką odgrywa Se dla profilaktyki i leczenia, zwłaszcza chorób nowotworowych. Podkreślić należy, że dostarczenie do organizmu Se w ilości niezbędnej do prawidłowego jego działania, w sytuacji licznych niedoborów (najpierw w glebie, potem w produktach spożywczych pochodzących z gleb z niedoborem Se) jest tylko pierwszym krokiem, gdyż udowodniono już w licznych publikacjach, że węglowodany (a zwłaszcza cukry proste, słodycze, ciasta itp.) spożywane – „niszczą” selen w organizmie człowieka.

A construction of a realistic statistical model of lung cancer risk and progression is proposed. The essential elements of the model are genetic and behavioral determinants of susceptibility, progression of the disease from precursor lesions through early (localized) tumors to disseminated disease, detection by various modalities, and medical intervention. Using model estimates as a foundation, mortality reduction caused by early-detection and intervention programs can be predicted under different scenarios. Genetic indicators of susceptibility to lung cancer should be used to define the highest-risk subgroups of the high-risk behavior population (smokers). The calibration and validation of the model requires applying our techniques to a variety of data sets available, including public registry data of the SEER type, data from the NCI lung cancer chest X-ray screening studies, and the recent ELCAP CT-scan screening study.

Recently, the diagnostic use of saliva in medicine has become a major goal for many researchers. There are many advantages of using saliva diagnostics. Its drawing is non-invasive, painless, quickly repeated, and straightforward. Its diagnostic potential has been demonstrated, in particular for infectious diseases and diseases of the oral cavity. Because of the unabated frequency of occurrence of malignant neoplastic diseases, especially in seniors, this paper aims at presenting an overview of chosen studies that have used saliva in the diagnostic and monitoring of selected malignant diseases. The potential use of saliva in the diagnosis of early-stage lung cancer, by detecting genetic material in saliva and analysis of characteristic mutations of the EGFR gene, has been described. Additionally, prostate cancer can be detected by positive identification of microRNA markers in saliva. Evaluation of PSA concentrations in saliva has also been carried out and obtained results were correlated with blood concentrations. There are also reports concerning the Ca15-3 marker in the saliva in women with breast cancer. The evaluation of lung cancer resistance protein in the saliva of these patients has also been investigated. The results of the cited studies are promising and give hope for the potential use for salivary diagnosis of these cancers.
W ostatnich latach diagnostyczne wykorzystanie śliny w medycynie stało się celem naukowym dla wielu badaczy. Ślina jako biologiczny materiał diagnostyczny ma wiele zalet: jej pobranie jest małoinwazyjne, niebolesne, szybko powtarzalne i łatwe. Udowodniono jej potencjał diagnostyczny przede wszystkim w chorobach infekcyjnych i jamy ustnej. Ze względu na niesłabnącą częstość występowania nowotworów złośliwych wśród populacji osób w starszym wieku, niniejsza praca ma na celu przedstawić przegląd wybranych badań naukowych wykorzystujących ślinę w diagnostyce oraz monitorowaniu wybranych chorób nowotworowych. Opisano potencjalne wykorzystanie śliny w rozpoznaniu wczesnego stadium raka płuca dzięki detekcji w ślinie materiału genetycznego i analizie charakterystycznych mutacji genu kodującego białko EGFR. Realną perspektywą w raku prostaty jest wykrywanie w ślinie wybranych mikroRNA. Dokonuje się także oceny stężenia antygenu PSA w ślinie, a otrzymane wyniki analizuje się względem jego stężenia we krwi. Istnieje wiele doniesień na temat oznaczeń antygenu Ca15-3 w ślinie wśród kobiet z rakiem piersi. Dokonuje się także oceny białka oporności raka płuc w ślinie u tych chorych. Wyniki przytoczonych badań są obiecujące oraz dają nadzieję na możliwość zastosowania wczesnej diagnostyki tych nowotworów.

Wstęp. Rak płuca stanowi w Polsce pierwszą przyczynę zgonów z powodu nowotworów. Najistotniejszymi czynnikami wpływającymi na ryzyko wystąpienia raka płuca są: palenie tytoniu (czynne i bierne), ekspozycja na pierwiastki rakotwórcze, czynniki genetyczne, choroby płuc (zwłóknienie płuc, przewlekła obturacyjna choroba płuc, krzemica) oraz nieodpowiednia dieta (mała ilość spożywanych warzyw i owoców). Cel. Celem niniejszej pracy jest przedstawienie zadań pielęgniarki w opiece nad seniorem w terminalnej fazie raka gruczołowego płuc Prezentacja przypadku. Pacjentka lat 74 w 5-tej dobie hospitalizacji przyjęta w oddział z rozpoznaniem raka gruczołowego płuc, powodu znacznych dolegliwości bólowych, postępującej kacheksji, zaparć oraz problemów z oddychaniem. Wnioski. W terminalnej fazie raka gruczołowego płuc dominującymi problemami pielęgnacyjnymi seniora są: ból, duszność, zaparcia spowodowane leczeniem opioidami, niedożywienie wynikające z postępu choroby nowotworowej, zmiany  w obrębie jamy ustnej. Rolą pielęgniarki w opiece nad seniorem w terminalnej fazie raka gruczołowego płuc jest przede wszystkim udział w łagodzeniu wszystkich dolegliwości wynikających z postępu choroby.
Introduction. Lung cancer is the main cause of death due to cancer in Poland. The most important factors that influence the risk of developing lung cancer are: smoking (active or passive), exposure to carcinogens, genetic factors, lung dieseses and bad diet (eating not enough vegetables) The aim. The aim of this study is to present the tasks of a nurse in the care of a senior in the terminal stage of lung adenocarcinoma. Case presentation. A 74-year-old patient on the 5th day of hospitalization was admitted to the ward diagnosed with lungs adenocarcinoma, which was the cause of significant pain, progressive cachexia, constipation and respiratory problems. Conclusion. In the terminal stage of lung adenocarcinoma, the dominant care problems for seniors are: pain, dyspnea, constipation caused by opioid treatment, malnutrition resulting from the progression of cancer, oral lesions. Main goal of a nurse in the care of a senior in the terminal stage of lung adenocarcinoma is mainly participation in alleviating ailments resulting from disease progression.
