Lung cancer is associated with one of the highest mortality rates among malignant tumors. It is the main death cause of men and women in Poland where over 15 000 men and 7 000 women die with this diagnosis annually. 1.7 million people die every year due to lung cancer in the world. Two main types of lung cancer are small cell lung cancer (SCLC) and non-small cell lung cancer (NSCLC). The history of lung cancer treatment begins with surgical approach followed by addition of chemotherapy and radiotherapy, which were used either separately or in combination depending on the stage of the lung cancer. Many somatic mutations were identified and molecularly targeted therapies could have been established. One of the oldest and the best known group of molecularly targeted drugs used in lung cancer treatment are tyrosine kinase inhibitors (TKIs) of epidermal growth factor receptor (EGFR). First EGFR-TKI was gefitinib, which has been examined in clinical trials before erlotinib, afatinib, dacomitinib and osimertinib. EGFR-TKIs increased overall survival (OS) with significantly less side effects when compared to standard chemotherapy. Another group of molecularly targeted drugs are anaplastic lymphoma (ALK) kinase inhibitors such as crizotinib, alectinib and ceritinib. Another innovation which was introduced in NSCLC treatment was immunotherapy. Its effect is based on modification of immune system leading to activation of cytotoxic T lymphocytes (CTL). Currently nivolumab and pembrolizumab (anti-PD1 antibodies) as well as atezoliumab (anti-PD-L1 antibody) are being used in NSCLC patients. The discovery of innovative therapies for NSCLC patients resulted in significant extension of patients' life expectancy while minimizing the side effects of such therapy. Moreover, the quality of patients’ life was significantly improved. However, important problems still remain to be solved: overcoming the resistance in the course of molecularly targeted therapies and the lack of predictive factors that determine the effectiveness of immunotherapy.

Introduction. Hyponatremia is a frequently observed electrolyte disorder among patients with cancer. In 1957, Schwartz et al. reported the first case of a patient with hyponatremia due to SIADH, secondary to lung cancer. From that moment on, there has been data published that indicates patients with SIADH are less responsive to chemotherapy, have greater predisposition to central nervous system metastases and are often characterized by an advanced stage of cancer during time of diagnosis. Hyponatremia has many possible causes, and the differential diagnosis can pose a challenge. Aim. The aim of the study was to consider the occurrence of secondary hyponatremia in the course of cancer and the significance of this disorder in the prognosis of the disease. Methods. An analysis of the clinical history of the patient and a review of available literature. Results. A 66-year old patient with hyponatremia was admitted to the Department of Endocrinology, and lung cancer was determined as the cause of the aforementioned electrolyte disorder. Conclusion. SIADH secondary to cancers should be included in a differential diagnosis of every case of hyponatremia of undetermined etiology.

Pulmonary localisation represents only 15% of all cases of actinomycosis. The clinical symptoms and radiological changes of this disease are non-specific and sometimes it can be misdiagnosed, usually as tuberculosis, lung cancer or lung abscess. In the reported case, what might look like the lung cancer, finally turned out to be actinomycosis. The interesting case is presented of lung actinomycosis in a 77-year-old farmer, admitted to the Department of Pneumonology, Oncology and Allegology in Lublin due to a massive haemoptysis. CT scan of the chest showed, apart from other changes, the spicular consolidation in the right lung which aroused oncology vigilance. The diagnostic path, which was a real medical challenge, led to the diagnosis of actinomycosis. The process of diagnosis and consequent treatment, which led to the complete regression of clinical and radiological changes, is presented.

Objectives: Recent results of epidemiological and medical statistics studies of lung cancer and indoor radon in different regions of the world make a relevant new combined analysis of residential exposure health effects. In particular, new data were obtained by means of a meta-analysis of case-control studies as well as taking into account a confounding effect of human papillomavirus infection in studies of geographically aggregated data. Materials and methods: Two sources of epidemiological data are considered: (1) studies of ecological design and (2) case-control studies. Ecological studies included the analysis performed for the USA counties and Russian oblasts with adjusting for the main confounders. Data on the case-control studies were gained from the meta-analysis of 31 individual studies with a weighting of obtained odds ratios according to the quality of radon exposure reconstruction and size of the reference group. Estimations of lung cancer excess relative risk (ERR) associated with indoor radon exposure are combined. Results: Two types of epidemiological study design provided generally consistent EER estimations. The combined value of ERR due to radon exposure is 0.14 (90% CI: 0.10–0.18) per 100 Bq/m3 . Conclusion: Available geographically aggregated data in regions of Russia and the United States and the meta-analysis of case-control studies conducted in a large number of countries confirm the association of lung cancer with indoor radon exposure.

Introduction: Chest pain is one of the symptoms of lung cancer. Chest pain disrupts patient’s functioning in somatic and psychic area. Purpose: Whether the existence of chest pain affects the level of perioperative anxiety in lung cancer patients. Is there a relationship between everyday functioning due to the chest pain and the level of perioperative anxiety? Is there a relation between anxiety associated with pain and gender? What is the cause of anxiety in this group of the patiens? Methodology: The study was conducted among 150 patients with lung cancer before the scheduled surgery. Data was collected with the use of questionnaire assessment of perioperative anxiety level in patients with lung cancer. Results: The chest pain before the surgery was confirmed by 63 (42%) patients, in case of 87 (58%) patients it was not identified. Chest pain was in case of 48% women and 36% men. 50 (33,3%) research participants who experienced chest pain and 35 patients (23,3%) without this symptom declared experiencing perioperative anxiety. In patients with lung cancer, the correlation: between chest pain and perioperative anxiety was (Z = -4.67; p< .001); between the difficult daily functioning of pain and perioperative anxiety was (Z = -4.72; p< .001); between gender and perioperative anxiety associated with pain was (Z = -3.24; p = 001). Patients afraid of: pain (37,3%), breathing problems (24,0%), physical disability (16,0%), eating problems (12,0%) sleep disorders (9,3%), nothing (1,3%). Conclusions: Patients with chest pain exhibited significantly higher of perioperative anxiety levels than non-symptomatic patients, as well as higher anxiety levels due to deteriorated daily functioning caused by the disorder. Women had a higher level of anxiety associated with pain than men before the surgery. Patients with lung cancer were most afraid of pain in the perioperative period.

Introduction. Before cigarettes became popular, lung cancer was an extremely rare disease, but currently it is the most common cause worldwide of cancer death in both genders. Therefore, it constitutes a huge epidemiological, diagnostic and socio-economic challenge around the world. In Poland, lung cancer is the second most common carcinoma in both men and women, and it is also the most prevailing cause of carcinous death. Undoubtedly, although smoking classical cigarettes is the strongest risk factor of developing the cancer, unfortunately it is not the only one. Objective. The aim of this study was to present smoking-related factors, e.g. cigars, pipes, e-cigarettes or marijuana smoking, etiologically related to lung cancer incidence.

The association between the lung cancer and indoor radon exposure in Russian population was investigated. The average indoor radon concentration for each region was estimated using the annual reports issued by the Saint-Petersburg Ramzaev Research Institute of Radiation Hygiene for the period 2008–2013. The average standardized lung cancer mortalities among males and females were estimated using the reports of the Moscow Hertzen Cancer Research Institute for the period 2008–2012. The relative risk (RR) was estimated as a ratio between the average mortality within seven exposure intervals and background mortality. The slope factors of linear dependence between the indoor radon exposure and lung cancer RR are 0.026 (–0.11÷0.17) and 0.83 (0.52–1.12) per radon concentration 100 Bq/m3 for males and females, respectively (with 90% confidence interval). The obtained results can be explained by the confounding effect of tobacco smoking. Significant excess risk of lung cancer in female population can be associated with radon exposure and low prevalence of smoking.

Summary: Lung cancer is the most common malignant tumor in the world, as well as one of the cancers with the most fatal prognosis. The acceptance of the disease is the most important element of the adaptive process. The better the illness acceptance, the lower the stress level and the higher the self-esteem, which facilitates the adaptation to the health status. Aim: The aim of this study was to assess the acceptance of the disease in patients before and after lung cancer surgery. Materials and methods: The study was conducted in 2016 at the Center of Oncology in Bydgoszcz and the Kuyavian and Pomeranian Pulmonology Center in Bydgoszcz. The study involved 87 patients who were assessed both before and after lung cancer surgery. The original questionnaire, as well as the Acceptance of Illness Scale, were used. Results: Men accounted for 75% of the probands, 65% of the study population were 50-69 years old. The highest number of patients – 25 (28.7%) had a 5-pack-year history, and the lowest amount of patients – 8 (9.2%) had a 2.5-pack-year history. The level of acceptance of illness before and after surgery differed in 58 persons. In 29, the level of acceptance remained the same, in 45, the level of acceptance decreased, and in 13 – it increased. Before surgery, the mean acceptance of illness score was 26.2 points, and after surgery – 20.89 points. The patients both after and before surgery had acceptance of illness scores regardless of their gender, age, education, place of residence or occupational activity. Conclusions: In more than a half of the patients, the acceptance of illness decreases after surgery and is at an average level. Male patients, patients aged 50-69 years, with primary, middle or vocational education, employed persons show a significantly worse illness acceptance, regardless of their place of residence, and occupational activity do not influence the acceptance of the disease.

The aim of the present study was to perform comparative analysis of serum from patients with different stages of non-small cell lung cancer (NSCLC) using the three complementary proteomic approaches to identify proteome components associated with the progression of cancer. Serum samples were collected before any treatment from 200 patients with NSCLC, including 103 early stage, 64 locally advanced and 33 metastatic cancer samples, and from 200 donors without malignancy. The low-molecular-weight fraction of serum proteome was MALDI-profiled in all samples. Serum proteins were characterized using 2D-PAGE and LC-MS/MS approaches in a representative group of 30 donors. Several significant differences were detected between serum samples collected from patients with early stage cancer and patients with locally advanced cancer, as well as between patients with metastatic cancer and patients with local disease. Of note, serum components discriminating samples from early stage cancer and healthy persons were also detected. In general, about 70 differentiating serum proteins were identified, including inflammatory and acute phase proteins already reported to be associated with the progression of lung cancer (serum amyloid A or haptoglobin). Several differentiating proteins, including apolipoprotein H or apolipoprotein A1, were not previously associated with NSCLC. No significant differences in patterns of serum proteome components were detected between patients with adenocarcinoma and squamous cell carcinoma. In conclusion, we identified the biomarker candidates with potential importance for molecular proteomic staging of NSCLC. Additionally, several serum proteome components revealed their potential applicability in early detection of the lung cancer.

Wstęp. Rak płuc główną przyczyna zgonów z powodu nowotworów w populacji mężczyzn. Stanowi także istotną przyczynę śmierci kobiet. Cel. Celem pracy jest analiza wybranych aspektów opieki pielęgniarskiej nad pacjentem z chorobą nowotworową płuc. Materiały i metody. W pracy dokonano analizy literatury poświęconej zagadnieniom opieki nad pacjentem w przebiegu raka płuc oraz przeprowadzono mapowanie diagnoz i interwencji pielęgniarskich przy użyciu słownika ICNP® . Wnioski. Rak płuc istotnie modyfikuje jakość życia pacjenta. Klasyfikacja ICNP® pozwala na projektowanie opieki pielęgniarskiej w oparciu o ujednoliconą terminologię.
Introduction. Lung cancer is the leading cause of cancer-related deaths in the male population. It is also a significant cause of women's death. Aim. The aim of the study is to analyze selected aspects of nursing care for a patient with lung cancer. Materials and methods. The paper analyzes literature on patient care in the course of lung cancer and mapping nursing diagnoses and interventions using the ICNP® dictionary. Conclusions. Lung cancer significantly modifies the patient's quality of life. The ICNP® classification allows you to design nursing care based on standardized terminology.

Background: Lung cancer is one of the most common malignant diseases with high mortality. Patients diagnosed with lung cancer are most likely to exhibited psychiatric symptomatology while they experience poor quality of life. Purpose: To examine the relationship between psychological distress and quality of life (QoL) in lung cancer patients receiving chemotherapy. Methods: A cross sectional study was conducted in which 110 lung cancer patients were recruited to participate. Data was collected with an anonymous self-administrated question-naire consisted by three parts: a sheet concerning demographic information, the scales Missoula Vitas Quality of Life Index-15 and General Health Questionnaires (GHQ)-28. Results: Women accounted for 51.8% of the sample, 27.3% were ≥56 years old and 24.5% were in the age between 35-44. Moreover, 38.2% were high school graduates while 46.4% were married. Age, educational level, and marital status were found to be related to patients’ perceived QoL. QoL was found to be related to psychological variables for GHQ-28. Conclusions: Quality of life can be considered to be a result of disease and treatment, as perceived by the patient and is affected by factors such as injury, anxiety, perceptions and social opportunities. This has a direct effect on patients’ functioning and ability to self-care.

Non-small cell lung cancer is one of the most commonly diagnosed cancer with a very high mortality rate. Trying to understand the mechanisms underlying the progression of this type of cancer, it is necessary to evaluate the changes occurring at molecular level in cancer cells. Besides the widely studied signaling pathways and genes which are dysregulated in NSCLC, there is a large group of non-coding RNAs involved in cancer pathogenesis. Those RNAs are tissue specific heterogeneous class of RNAs that play many functions in physiological condition in cells, nevertheless current data has shown that lncRNAs are also functional in different types of cancer. Moreover, it has been suggest that lncRNAs are involved in cancer progression by controlling key signaling pathways involved in diverse types of tumors. Notch signaling is one of those pathways, very often deregulated in NSCLC. Therefore in this review I summarized recent outcomes according the importance of lncRNAs in regulation of Notch pathway in the pathogenesis of NSCLC.

Lung cancer is the most common fatal cancer disease in the world. A characteristic feature of lung cancer is genetic diversity. In the overwhelming majority of cases, smoking is the most important etiopathogenic factor. Lung cancer is a cancer with a very bad prognosis regarding long-term survival. The risk of lung cancer depends primarily on active or passive exposure to the carcinogenic components of tobacco smoke. According to available data, the development of lung cancer in addition to active and passive smoking is directly affected by environmental pollution such as smog and fumes, ionizing radiation, mycotoxins and long-term exposure to asbestos (occupational exposure). Research on the pharmacoprevention of lung cancer began over 30 years ago. The first nutrient that the researchers said could inhibit the development of lung cancer was beta carotene. Unfortunately, long-term regular supplementation with high doses of antioxidant in the form of beta-carotene brought the opposite effect. An increase in the incidence of lung cancer was found in people who received beta carotene in the form of a synthetic food supplement. The other component tested was N-acetylcysteine. It is a sulfur compound and a powerful antioxidant that supports the synthesis of glutathione and cysteine, with destructive effects on carcinogenic substances. N-acetyl-cysteine, used in the form of NAC adduct and epigallocatechin-3-gallate, showed efficacy in inhibiting the development of lung cancer only in animal models. In the pharmacoprevention of lung cancer, the use of vitamin E was also tested in the form of tocotrienol and tocopherol. The following work also shows the existence of a high concentration correlation which belongs to the steroid hormone, mainly estrogen, in the blood and the development of lung cancer in women. An increased risk of lung cancer has been observed in women undergoing long-term hormone replacement therapy. The results show that 2-methoxyestradiol, the endogenous metabolite of 17ß-estradiol, shows positive results that inhibit the growth of lung cancer cell lines. The aim of the work was to present the correlation between tobacco abuse and passive smoking and lung cancer, pharmacoprevention of lung cancer and the association of elevated estrogen concentration in women with an increased risk of lung cancer.

Introduction. Blood biomarkers may support early diagnosis of lung cancer by enabling pre-selection of candidates for computed tomography screening or discrimination between benign and malignant screening-detected nodules. We aimed to identify features of serum metabolome distinguishing individuals with early-detected lung cancer from healthy participants of the lung cancer screening program. Methods. Blood samples were collected in the course of a low-dose computed tomography screening program performed in the Gdansk district (Northern Poland). The analysis included 31 patients with screening-detected lung cancer and the pair-matched group of 92 healthy controls. The gas chromatography coupled to mass spectrometry (GC/MS) approach was used to identify and quantify small metabolites present in serum. Results. There were several metabolites detected in the sera whose abundances discriminated patients with lung cancer from controls. Majority of the differentiating components were downregulated in cancer samples, including amino acids, carboxylic acids and tocopherols, whereas benzaldehyde was the only compound significantly upregulated. A classifier including nine serum metabolites allowed separation of cancer and control samples with 100% sensitivity and 95% specificity. Conclusions. Signature of serum metabolites discriminating between cancer patients and healthy participants of the early lung cancer screening program was identified using a GC/MS metabolomics approach. This signature, though not validated in an independent dataset, deserves further investigation in a larger cohort study.

Introduction and objective. Barley (Hordeum vulgare L.) is known as a rich source of different bioactive compounds. At present, considerable attention of researchers is focused on young barley grass. It can be a good source of dietary minerals, vitamins, carbohydrates, amino acids, phenolic compounds and proteins. It is possible that the composition of chemical ingredients beneficial for health may induce an anticancer potential of young barley in human colon adenocarcinoma (HT-29) and human lung adenocarcinoma (A549) cell lines. Materials and method. Hordeum vulgare water extract (HWE) and Hordeum vulgare juice extract (HJE) were prepared. Cell proliferation and viability were examined with the use of MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) and NR (neutral red) methods. Induction of necrosis was assessed by propidium iodide/Hoechst staining. Progress of the cell cycle involved in cell proliferation, apoptosis, and regulation of transcription was estimated using flow cytometry analysis. Additionally, the capability of free radical scavenging was evaluated with the DPPH assay. Results. The study revealed that extracts inhibited the proliferation of cancer cells. The NR study confirmed the low cytotoxic activity of the tested extracts to normal human colon epithelial cells (CCD 841 CoTr) and human skin fibroblasts (HSF). Furthermore, a dose-dependent cytotoxicity against HT-29 cells, but not A549 cells, has been reported. The free radical scavenging activity was observed in the case of the HWE but not the HJE. Conclusions. The obtained results indicate a cancer chemopreventive potential of young barley as a safe dietary agent in colon carcinoma.
