Metformin is used as an anti-diabetic drug among oral hypoglycemic drugs, which produces many gastrointestinal problems. Study aims to investigate the effect of metformin induced gastrointestinal problems and its prevalence. A cross-sectional study design was adapted using convenience sampling technique, at different Diabetic Centers of Lahore and Faisalabad, Pakistan from, June-2017 to November-2017. A total of 300 male and female patients participated in the study between 26 to 85 years and diagnosed with type-II diabetes. Data was directly collected from the patients and prevalence of metformin-induced gastrointestinal intolerance was determined by the symptoms of the patients. Data was analyzed by SPSS version 21. Results showed a significant difference between gender and symptoms (p=0.029). Moreover, the gastrointestinal problems were found to be dose related. A significant difference existed between patients who were taking 500mg and those taking 850 mg of metformin (p=0.006), patients who were taking 500mg and those taking 1000mg of metformin (p=0.000) and patients who were taking 850mg with those taking 1000mg of metformin (p=0.022). The prevelance of metformin-induced gastrointestinal symptoms was 45.8%. Most commonly occurring symptoms were, constipation (41.35%) followed by dyspepsia (27.89%), abdominal pain (26.92%), bloating and heart burn (25%), indigestion (15.38%), anorexia (11.54%), diarrhea (6.58%), flatulence (7.69%), nausea (6.73%) and vomiting (2.88%). It was concluded that gastrointestinal intolerance was more in females as compared to males. The gastrointestinal problems increased with the increase in dose. The side effects occurred were irrespective of the age and the most common gastrointestinal symptom was found to be constipation.

Aim of the study: High-resolution ultrasonography of the tibial nerve is a fast and non invasive tool for diagnosis of diabetic peripheral neuropathy. Our study was aimed at finding out the correlation of the cross sectional area and maximum thickness of nerve fascicles of the tibial nerve with the presence and severity of diabetic peripheral neuropathy. Material and methods: 75 patients with type 2 diabetes mellitus clinically diagnosed with diabetic peripheral neuropathy were analysed, and the severity of neuropathy was determined using the Toronto Clinical Neuropathy Score. 58 diabetic patients with no clinical suspicion of diabetic peripheral neuropathy and 75 healthy non-diabetic subjects were taken as controls. The cross sectional area and maximum thickness of nerve fascicles of the tibial nerves were calculated 3 cm cranial to the medial malleolus in both lower limbs. Results: The mean cross sectional area (22.63 +/– 2.66 mm2 ) and maximum thickness of nerve fascicles (0.70 mm) of the tibial nerves in patients with diabetic peripheral neuropathy compared with both control groups was significantly larger, and statistically significant correlation was found with the Toronto Clinical Neuropathy Score (p < 0.001). The diabetic patients with no signs of peripheral neuropathy had a larger mean cross sectional area (14.40 +/– 1.72 mm2 ) and maximum thickness of nerve fascicles of the tibial nerve (0.40 mm) than healthy non-diabetic subjects (12.42 +/– 1.01 mm2 and 0.30 mm respectively). Conclusion: The cross sectional area and maximum thickness of nerve fascicles of the tibial nerve is larger in diabetic patients with or without peripheral neuropathy than in healthy control subjects, and ultrasonography can be used as a good screening tool in these patients.
Cel pracy: Ultrasonografia wysokiej rozdzielczości nerwu piszczelowego jest szybką i nieinwazyjną metodą możliwą do wykorzystania w rozpoznaniu cukrzycowej neuropatii obwodowej. Celem naszej pracy było ustalenie, czy istnieje zależność pomiędzy polem przekroju poprzecznego oraz maksymalną grubością pęczków nerwu piszczelowego a występowaniem i zaawansowaniem cukrzycowej neuropatii obwodowej. Materiał i metoda: Badaniu poddano 75 pacjentów cierpiących na cukrzycę typu 2 z klinicznym rozpoznaniem cukrzycowej neuropatii obwodowej. Stopień zaawansowania neuropatii oceniany był za pomocą Klinicznej skali neuropatii Toronto (Toronto Clinical Neuropathy Score). Grupę kontrolną w badaniu stanowiło 58 pacjentów z cukrzycą bez objawów klinicznych cukrzycowej neuropatii obwodowej oraz 75 zdrowych osób. Pole przekroju poprzecznego oraz maksymalna grubość pęczków nerwu piszczelowego wyliczane były na wysokości 3 cm powyżej kostki przyśrodkowej w obu kończynach dolnych. Wyniki: Średnie pole przekroju poprzecznego (2,63 +/− 2,66 mm2) oraz maksymalna grubość pęczków nerwu piszczelowego (0,70 mm) u pacjentów z cukrzycową neuropatią obwodową były istotnie statystycznie większe niż u osób z obu grup kontrolnych. Stwierdzono także występowanie istotnej statystycznie zależności pomiędzy wielkością tych parametrów a stopniem zaawansowania cukrzycowej neuropatii obwodowej według Klinicznej skali neuropatii Toronto (p < 0,001). U pacjentów chorujących na cukrzycę bez występowania objawów klinicznych cukrzycowej neuropatii obwodowej pole przekroju poprzecznego (14,40 +/− 1,72 mm2) oraz maksymalna grubość pęczków nerwu piszczelowego (0,40 mm) były większe niż u zdrowych uczestników badania (odpowiednio 12,42 +/− 1,01 mm2 i 0,30 mm). Wnioski: Przekrój oraz maksymalna grubość pęczków nerwu piszczelowego są większe niż u osób zdrowych zarówno u pacjentów z cukrzycą cierpiących na cukrzycową neuropatię obwodową, jak i bez klinicznych objawów neuropatii. Ultrasonografia może być z powodzeniem wykorzystywana u chorych z cukrzycą jako badanie przesiewowe pod kątem występowania cukrzycowej neuropatii obwodowej. Wstęp: Z badań epidemiologicznych wynika, że narażenie na pestycydy fosforoorganiczne zwiększa ryzyko zachorowania na niektóre choroby, m.in. neurologiczne (np. chorobę Parkinsona, chorobę Alzheimera) i układu krążenia, a także na cukrzycę. Celem niniejszej pracy była ocena stężenia glukozy w surowicy krwi szczurów przy podostrym zatruciu niskimi dawkami chlorpyrifosu - związku fosforoorganicznego stosowanego jako insektycyd. Materiały i metody: Badania wykonano na szczurach samcach szczepu Wistar, którym przez 14 lub 28 dni sondą dożołądkowo podawano raz dziennie roztwór olejowy chlorpyrifosu w dawce 0,2; 2 lub 5 mg/kg m.c./dzień. Zwierzęta z grupy kontrolnej otrzymywały olej. W surowicy oznaczano stężenie glukozy metodą kolorymetryczną przy użyciu gotowych zestawów. Wyniki: Stwierdzono wzrost stężenia glukozy w surowicy po 14 dniach podawania chlorpyrifosu w najwyższej badanej dawce oraz po 28 dniach podawania związku we wszystkich badanych dawkach. Wnioski: Zarówno uzyskane wyniki (wzrost stężenia glukozy w surowicy krwi w podostrym zatruciu chlorpyrifosem), jak i dane z piśmiennictwa sugerują, że narażenie na insektycydy fosforoorganiczne może sprzyjać rozwojowi cukrzycy. U osób mających zawodowy kontakt z tymi związkami konieczne jest więc częstsze wykonywanie badań diagnostycznych w kierunku jej wczesnego wykrycia. Med. Pr. 2013;64(4):527–531
Background: Epidemiological studies suggest that exposure to organophosphate insecticides enhances the risk of various diseases, including neurological disorders, e.g. Parkinson's or Alzheimer's disease, arteriosclerosis and diabetes mellitus. For this reason the aim of the presented study was to estimate serum concentration of glucose in subchronic intoxication with low doses of chlorpyrifos, an organophosphate insecticide. Materials and Methods: The rats received chlorpyrifos at a daily dose of 0.2, 2 or 5 mg/kg b.w./day for 14 or 28 days. For biochemical determinations of serum glucose in the rats ready-to-use kit was applied. Results: In subacute intoxication with chlorpyrifos the increased serum concentration of glucose was observed after 14 days of intoxication with the highest dose (5 mg/kg b.w.) and after 28 days of intoxication with all dose levels used. Conclusions: The results of this study showing the increased concentration of serum glucose in subacute intoxication with low doses of chlorpyrifos, as well as the literature data suggest that exposure to organophosphate insecticides can increase the risk of diabetes mellitus. It may thus be concluded that people occupationally exposed to these compounds should be subjected to diagnostic tests for early detection of diabetes. Med Pr 2013;64(4):527–531

Aim: The aim of the study was to retrospectively assess the relationship between the occurrence of polyps and colon cancer in patients with type 2 diabetes. Methods: In 2014–2015, 976 colonoscopies were performed in patients. We compared the number of polyps with high-grade dysplasia and colorectal cancers in patients with and without diabetes. In addition, in the diabetic group we documented the relationship between HbA1C and the occurrence of polyps with high-grade dysplasia, and colon cancer. The data were statistically analyzed. Results: 1. Patients with diabetes show a higher incidence of polyps with high-grade dysplasia/carcinoma – 32/91 (35.16%) in comparison to patients without diabetes – 136/885 (15.37%), P < 0.001; 2. Patients with diabetes show a higher incience of polyps with cancer – 9/91 (9.89%) as compared to patients without diabetes – 18/885 (2.03%), P < 0.001. 3) Colorectal cancer occurred significantly more often in uncontrolled diabetes (P = 0.022). Conclusion: The conducted study shows a significant association between type 2 diabetes and the incidence of colorectal adenomas. These findings may lead to a conclusion that diabetic patients are at a higher risk of developing colorectal cancer, thus are in higher need for controlled colonoscopy. Therefore, it may be worth considering a scheme for screening patients in the above-mentioned group with colonoscopy.

BackgroundCarbohydrate metabolism disorders, obesity and a severe course of gastroesophageal reflux correlate with more frequent development of esophageal complications. The aim of this study was to assess the influence of obesity and carbohydrate disorders on the characteristics of gastroesophageal reflux disease (GERD).Material and methodsThe study included 58 patients with excess weight. Anthropometric parameters (including the body mass index, BMI), data regarding GERD (severity of symptoms, gastroscopy and esophageal pH monitoring results) were included in the study. Correlations between obesity and GERD parameters were analyzed. Subjects were divided into a diabetic and a control group and the severity of GERD was compared.ResultsGERD was diagnosed in 40 patients and occurred more frequently in the obese group (73%) than in the overweight group (57%). Increased GERD severity was associated with increased BMI only for postprandial parameters. GERD was diagnosed in most of the group with carbohydrate disorders (78% vs 63% in the non-diabetic group). No differences in the severity of GERD were observed between groups depending on carbohydrate disorders.ConclusionsIn our study, GERD was common in obesity and in diabetic disorders. Increased severity of postprandial reflux was associated with an increased BMI. Diabetic disorders were not associated with more severe GERD.

Introduction and objective. There are some clinical situations where a high level of HDL cholesterol (HDL-C) may be unfavourable. In these situations, HDL-C may undergo some changes, and even if its quantity is within the reference range, its quality is no longer the same. Brief description of state of knowledge. Diabetes is the state of elevated oxidative stress. Studies conducted to-date have revealed an increased production of the reactive forms of oxygen as the result of tissue damage in diabetes patients. The expression ‘dysfunctional HDL’ has been coined in the literature to describe high-density lipoproteins that lose their antioxidative and anti-inflammatory properties, that is, HDL-C that loses its basic functions. Recent observational studies have confirmed that the atheroprotective activity of properly functioning HDL-C is frequently impaired in clinical situations associated with oxidative stress. The presented review lays the foundation for a new approach to understanding how the functional properties of HDL help reduce cardiovascular risk. Conclusions. In the light of presented findings it seems that there is a need to seek a better diagnostic marker than HDL-C level. This study presents some possible directions for future research to bring us closer to the full understanding of the HDL particle and its role in patients with ischemic heart disease and type 2 diabetes.

Introduction and objective. Recently, data has indicated a higher incidence of mild cognitive impairment (MCI) in patients with diabetes. Old age is a risk factor for cognitive deterioration and dementia. The aim of the study was to find the factors associated with poor glycaemic control in type 2 diabetic elderly patients with MCI. Materials and method. A cross-sectional study was conducted on 87 diabetic patients with MCI in an outpatient clinic. All subjects were screened for MCI using the Montreal Cognitive Assessment (MoCA). Detailed medical history and collection of blood test samples were performed. Results. 83.9% of participants had poor glycaemic control. A positive correlation was found between HbA1c level and number of visit to a doctor per year, number of co-morbidities, duration of T2DM, triglycerides and fasting glucose level; and a negative correlation between HbA1c level and years of education, HDL cholesterol level and MoCA score. The univariate logistic regression models revealed factors which are associated with poor glycemic control are: less years of education, higher no of visit to doctor per year, increased number of co-morbidities, presence of CVD, retinopathy, higher levels of triglycerides and fasting glucose, lower level of HDL cholesterol, lower MoCA score. Multivariable model revealed that higher plasma levels of fasting glucose and triglycerides are significant predictors. Conclusions. There is a high prevalence of poor glycemic control patients among elderly diabetics with MCI. Higher plasma levels of fasting glucose and triglycerides seems to be the most important predictors of poor glycemic control, however father larger studies are needed to elucidate these relationships.

The spinal muscular atrophy is a rare autosomal recessive genetic disease characterized by the progressive loss of muscular strength. In its natural course the disease leads to death. Diabetes mellitus type 1 is an autoimmune metabolic disorder characterized by the disturbed insulin synthesis. This is a case report of an 8-year-old girl suffering from Werdnig Hoffman disease in whom DM1 was diagnosed. The unspecific clinical manifestation and diagnostic difficulties are presented in this paper. To the authors' knowledge, this is the first publication concerning the co-existence of these two medical conditions.

Introduction. Inflammatory response in Diabetes Mellitus (DM) begins with chronic sub-clinical inflammations as a result of insulin resistance and activation of both innate and adaptive immune system as the disease progresses to complicated diabetes. Hence, the present study investigated the neutrophil phagocytic, complement function (CH50), and some cytokine profiles among diabetic and non-diabetic patients attending the National Hospital in Abuja, Nigeria. Aim. To evaluate the neutrophil phagocytic, complement function (CH50), and some cytokine profiles among post-operative septic diabetic and post-operative septic non-diabetic patients at the National Hospital in Abuja, Nigeria. Material and methods. Subjects were recruited by convenient sampling technique through interviewer-administered questionnaires. Subsequently, blood samples were collected. Fasting blood sugar (FBS) (mmol/L) was determined using glucose oxidase method. Neutrophil function test (Fmol/phag) was assayed using nitroblue tetrazolium reduction test (NBT). Hemolytic complement function (CH 50) test was conducted using serum harvested from sheep sensitized with human group (ORh D +ve) red blood cells. While serum Interleukin-4, -6, -10 and TNF- α were determined using Enzyme Linked Immunosorbent Assay (ELISA). Results. Mean ± Standard deviation (SD) of FBS concentration of 10.5 ± 1.3 (mmol/|L) among diabetic and 4.7 ± 0.9 (mmol/L) among non-diabetics was recorded. There is a decrease in neutrophil phagocytic function with a mean ± SD of 5.4 ± 2.1 (Fmol/ phag) in diabetics compared to 9.2 ± 2.1 (Fmol/phag) in non-diabetics. Similarly, complement (CH 50) function and C-reactive protein were significantly lower in diabetics when compared to non-diabetics (p<0.001). There was a significant difference in IL-6 concentration between diabetics and non-diabetics groups, but no significant difference was observed in TNF-α, IL-4 and IL-10 concentrations between study groups (p>0.05). TNF-α and IL-6 was significantly higher in diabetics with cardiovascular disorders compared to non-diabetics subjects with cardiovascular disorders (p<0.001). Conclusion. Findings from this study revealed the association of complement, neutrophil phagocytic function, CRP and IL-6 among septic diabetic patients,. In addition TNF-α and IL-6 expression was higher in DM patients with cardiovascular disorders.

common risk factors for high risk pregnancies and spontaneous or therapeutic abortions. Objectives. To investigate the legal abortion caused by heart disease, blood disorders, diabetes and hypertension as referred to forensic medicine centers in Fars Province from 2007 to 2013. Material and methods. In a retrospective, cross-sectional study, samples consisted of all documents of people referred to forensic medicine centers in Shiraz since 2007 to 2013, comprising of 1664 files. Data collection tools included a demographic forum and the checklist of abortion causes. SPSS.16.0 was applied to analyze the data through descriptive statistical analysis. Results. The most frequent age group was 25–29 years at 31.5% (n = 522) and the lowest was over 40 years old at 4.15% (n = 70). The statistical report of the reasons for legal abortion permission were 19% (n = 63), 24.4% (n = 81), 10.54% (n = 35), and 8.13% (n = 27) due to heart problems, blood disorders, hypertension, and diabetes mellitus, respectively. Most frequent legal abortion permits by forensic medicine due to maternal causes were between the years of 2011–2012 at 17.8–28% (n = 59–93). The relationship between legal abortion permission at The Forensic Medicine Center at different years and maternal ages was statistically significant (p < 0.00001). Conclusions. The most common prevalent reason of abortion was Blood Disorder – 81 patients (24.4%) and heart disease – 63 cases (19%). It is essential that family education and prevention of repeated pregnancies be done with high-risk women. Also, initiation of pregnancy care at lower gestational age in identifying risky pregnancies and timely control of complications must also be undertaken

Cukrzyca to „epidemia XXI wieku”, szacuje się, że w 2011 roku na cukrzycę chorowało 6,7% populacji Polski, z czego jedna czwarta nie miała świadomości choroby. Tak duże rozpowszechnienie cukrzycy sprawia, że coraz więcej lekarzy staje przed problemem prawidłowej kwalifikacji pacjenta z cukrzycą do amatorskiego uprawiania sportu, w tym nurkowania. W niniejszej publikacji przedstawiono najważniejsze informacje dotyczące oceny ryzyka, dopuszczenia do amatorskiego nurkowania pacjentów z cukrzycą oraz proponowanych protokołów postępowania przed i podczas uprawiania tego sportu.
Diabetes is an "epidemic of the 21st century" and it is estimated that in 2011 6.7% of Poland's population suffered from diabetes, one quarter of which was unaware of the disease. Such a high prevalence of diabetes mellitus results in an increasing number of physicians being confronted with the problem of qualifying patient with diabetes to partake in amateur sports, including diving. This publication presents the most important information concerning risk assessment, admission of diabetic patients to amateur diving and some proposed protocols of management before and during sport practice.

Background: The most effective way to prevent an increasing diabetic population lies in early detection of risk factors and diagnosis of carbohydrate metabolism disorders. Aim of the study: The study aimed at determining socio-economic variables, lifestyle behaviours and biological factors differentiating patients with newly diagnosed diabetes from diabetes-free individuals. Material and methods: Assessment of diabetic vs. non-diabetic individuals was performed according to the American criteria issued by the Commission on Social Determinants of Health as well as the FINDRISC form, which helps identify patients who are at risk of developing type 2 diabetes on the basis of multi-factorial determinants of its development. The research was conducted in 2018 among 1167 primary health care patients from Lubuskie Voivodeship using a diagnostic survey method which interviewed the respondents according to the FINDRISC standard questionnaire. Results: The group of healthy patients was similar to the group of patients with newly diagnosed diabetes with respect to variables such as age (p=0.713), sex (p=1), place of residence (p=1), level of education (p=0.076), professional activity (p=0.758), BMI (p=0.133), waist measurement (p=0.665), frequency of fruit and vegetables intake (p=0.572), frequency of taking hypotensive medications (p=0.176), frequency of diabetes occurrence in the family history (p=0.227) and physical activity (p=0.321). Conclusions: Early detection of carbohydrate metabolism disorders, with the use of standardised tools that assess diabetes development, appears to be essential in the prevention of this disorder. Therefore, there is a strong need to create a tool adjusted to socio-demographic factors such as geographical location, economic conditions and lifestyle. Additionally, active and massive screening for carbohydrate metabolism disorders in patients with a low risk of diabetes seems to be crucial in its prevention.

The aim of this study was evaluation of personality traits’ differences between adolescents with diabetes (type 1) and their healthy peers. 100 diabetics (age 16–22,17) and 103 healthy adolescents were assessed. They completed Cattell’s the 16 Personality Questionnaire. The diabetics are significantly different from their healthy peers in a factor H (resistance-lack of resistance) and factors of the second degree – introversion–extraversion and worry–integration. Girls with diabetes are different from healthy girls in a factor C (emotionally stability–neurosis). The diabetics are less resistant to stress, have bigger difficulty in making social contacts, are more shy and anxiously. The girls with diabetes are more neurotic than the healthy girls.

Background: Rhinocerebral mucormycosis is the most common form of mucormycosis in patients with diabetes mellitus; it is linked to poor prognosis, presenting most commonly in an acute setting, mimicking symptoms of sinusitis or periorbital cellulitis. The general survival rate in chronic cases is 83%, compared to 10–35% in acute. Aim of the study: To report a death due to rhinocerebral mucormycosis in a 45-year-old male patient. Case report: In this case report a 45-year-old male presented with acute rhinocerebral mucormycosis and was admitted in a state of unconsciousness with complaints of sudden onset weakness of right upper and lower limb, motor aphasia, right facial swelling, orbital swelling, and diminished distant vision. Upon primary diagnosis of stroke, treatment started immediately. However, past medical history from patient’s attendants revealed that the patient underwent a tooth extraction procedure 20 days prior, and had since developed redness of the right eye, diminished distant vision, and swelling of the right side of the face. Pus was drained, and reports revealed orbital cellulitis with an intracranial spread. By the time of admission to hospital, the patient had abnormal lab profiles (WBC, ESR, serum creatinine), acute kidney injury, with MRI revealing rhinocerebral mucormycosis. The patient developed septic shock and died during treatment. Conclusions: Acute mucormycosis carries a high mortality rate. Pleiotropic manifestations and organ dysfunction add to the further risk of mortality. Timely diagnosis and management may increase the chances of the survival rate of the patient.

Introduction: According to the WHO, an estimated 422 million people are suffering from diabetes worldwide. Among them, the incidence of cholelithiasis is higher than in the healthy population. The aim of this literature review was to summarize the available evidence about acute cholecystitis in patients with diabetes. Materials and methods: This study adhered to the PRISMA guidelines. The course of hospitalization of patients with and without diabetes who underwent cholecystectomy due to acute cholecystitis was compared. Following information was abstracted from original studies: general study information, patient characteristics, complications, and recommendations for patients with diabetes. Results: Initial search provided 1632 results. After full text assessment, 40 studies met the inclusions criteria. Operative and postoperative complication rates were significantly higher among the diabetic patients. Diabetes is a risk factor for conversion from laparoscopic to open cholecystectomy method. The authors' opinions on elective surgery before the onset of acute cholecystitis symptoms are divided. Conclusions: Diabetic patients are at greater risk of developing complications. An individualized screening and treatment approach, as well as proper preparation of the diabetic patient for an elective cholecystectomy could have a positive effect on the outcome. However, the low quality of the data from the systematic review does not allow for meta-analysis, which is why we cannot draw strong conclusions.

Hereditary hemochromatosis (HH) is a rare genetic disorder, developing secondary to the accumulation of iron in tissues, which may lead to multiple organ failure. If untreated, it may result in liver cirrhosis or cardiomyopathy. The damage to the pancreas and the anterior pituitary, on the other hand, leads to a decreased production and secretion of hormones that are essential to life. Common symptoms of HH, that are distressing for patients, include joint pain, particularly involving hands and wrists, as well as the chronic fatigue syndrome. Iron overload affects the skeletal system, leading to osteoporosis. The pathological accumulation of iron in the anterior pituitary impairs the gonadotropin synthesis, resulting in reduced serum levels of testosterone in men and estrogens in women. This, however, contributes to lower bone mass. In vivo tests have also revealed that abnormal iron accumulation is related to an increased activity and number of osteoclasts, as well as the influence on the differentiation and activity of osteoblast-lineage cells. Based on a systematic review of literature, hereditary hemochromatosis (HH) will be presented as a chronic disease, affecting most of the endocrine glands.

Introduction. Impaired hearing organ function including abnormalities in auditory brainstem response (ABR) are more frequent in diabetic subjects compared to the general population. The aim of our study was to assess the impact of selected modifiable factors on ABR latencies in diabetic subjects. Material and Methods. 58 patients with type 1 and type 2 diabetes, aged <45 years, with diabetes duration <10 years, and without clinically overt hearing impairment or diabetic neuropathy, were included. In all subjects vital signs and blood samples were obtained, and ABR audiometry was performed. Results. Significantly delayed latencies in ABR were found in patients with total cholesterol <192 mg/dL, with HDL-cholesterol <49.5 mg/dL, with triglycerides >89 mg/dL, with presence of hypertension, and with systolic and diastolic blood pressure >135 and >78 mm Hg respectively. A linear correlation between triglycerides and wave I and III latencies, and between systolic blood pressure and wave III latency were revealed. A relationship between ABR latencies and HbA1c, LDL-cholesterol or BMI was not found. Conclusions. Several modifiable factors affect functioning of the retrocochlear part of the auditory pathway. If these results were confirmed in further studies, a vast area of possible therapeutic interventions to preserve hearing function in diabetic patients would become available.

The aim of this study was to estimate the relationship between personality of 66 healthy women and 66 women with type 2 diabetes. The average age of both groups was 63. I use the 16th Personality Factors Test by Cattell. A lower level of intelligence characterizes the personality of a diabetic, they are more submissive, effusive and responsible. They are calmer and more practical in their behavior. Healthy women are more self-sufficient and unconventional.

Diabetes is an incurable disease. Recently researchers are focusing more often on the role of the social support in diabetes treatment. It seams that support from family and friends plays a big role in the results of the disease treatment. Lack of such support may decrease patients’ treatment motivations. The aim of this study was to estimate the relationship between social support and metabolic control. The group which was examined consisted of 35 people with type 2 diabetes not treated with insulin and 31 who were on insulin. The Social Support Inventory by Saranson and glycosylated hemoglobin (HbA1c) for glycemic control were used in this study. The research indicates that poorer metabolic control was correlated with the lack of social support.

Introduction and aim. This study aims to determine the level of stigma in patients with type 2 diabetes mellitus (T2DM) and evaluate its relationship with their life satisfaction. Material and methods. This is a descriptive and cross-sectional study conducted in a single tertiary care center. Two hundred and ten patients ≥18 years old diagnosed with T2DM were included in this study. The relationship between the “Type 2 diabetes stigma assessment scale” and the “Satisfaction with life scale” was assessed using Pearson correlation analysis. Results. The average age of the participants was 54.85±15.81 years. The majority of patients reported adherence to diabetes treatment and having comorbidities. Stigma levels were higher in patients ≤65 years, those with less than a high school education, and those receiving insulin or oral anti-diabetic treatment, but lower with a T2DM diagnosis duration of ≤ 5 years (p<0.05). Life satisfaction was influenced by age, education status, economic status, adherence to treatment, dietary compliance, and the presence of comorbidities. A moderately negative relationship between stigma level and life satisfaction was identified. Conclusion. The study’s results indicate that an increase in stigma level is associated with a decrease in life satisfaction in patients with type 2 diabetes mellitus.

ABSTRACT Introduction. Depression, a common psychiatric mood disorder, is a leading cause of disability and a significant contributor to the overall global burden of disease. Aim. To determine the prevalence of depression in patients with controlled and uncontrolled type-2 diabetes mellitus (T2DM) and/or hypertension (HTN) in India. The association of depression with socio-demographic profile and clinical risk factors was also assessed. Material and methods. In this cross-sectional epidemiological study, T2DM and/or HTN patients attending outpatient department at tertiary care hospitals and private clinics across 54 cities in India were enrolled. The primary outcome measure was to determine the prevalence of depression in T2DM, HTN and T2DM + HTN patients. Association of depression with patients’ demography, socio-economic status, anxiety, and clinically diagnosed insomnia were also investigated. Results. Of 1829 patients, the prevalence of depression in T2DM, HTN and T2DM+HTN cases were found to be 51.03%, 46.94% and 48.64%, respectively. A higher proportion of patients with uncontrolled T2DM and HTN reported depression (T2DM: 77.64% vs. 22.36%; HTN: 72.49% vs. 27.51%). There was a significant association between anxiety and severity of depression across all indications (p<0.0001). Depression was significantly associated with complications in T2DM (p=0.0001) and comorbidities in T2DM + HTN (p=0.0023) cases. Conclusion. Depression is highly prevalent and has a direct significant association with various socio-demographic variables and anxiety in Indian patients with T2DM and/or HTN.

Introduction. Late complications of diabetes mellitus are a serious challenge for physicians treating diabetic patients. Micro- and macro-angiopathic changes are of the most severe sequels. However, it should be remembered that the effects of long-term diabetes are also associated with changes in the peripheral nervous system. One of the complications most troublesome for patients is diabetic autonomic neuropathy of the gastrointestinal (GI) tract. Objective. The review article presents the pathogenesis of diabetic neuropathy of the autonomous gastrointestinal tract, and advances in the diagnosis and treatment of this disease. The study used materials in the database which demonstrate current standards of conduct, based on the principles of EBM. Brief description of the state of knowledge. Diabetic neuropathy is one of the most common complications of diabetes mellitus. According to epidemiologists, it affects 10% to even 100% of patients with long-standing disease. Damage to the nervous fibres mainly results from hyperglycaemia, which leads to the typical symptoms of dysfunction of GI tract segments. The main additional examinations include abdominal ultrasound (US) and abdominal X-ray. In cases with oesophageal disorders accompanied by dysphagia, gastroscopy is found useful for excluding possible neoplastic lesions. The gold standard for evaluation of gastric emptying is radioisotopic scintigraphy, however it is available only in some specialistic departments. Other recommended methods include: manometry, EGG, insulin tests, NMR and expiratory tests. To date, the problem has been managed symptomatically; in special cases, surgical interventions have been required. Conclusions. At present, a wide array of treatment options is available, including new forms of drugs and less radical surgical procedures carried out in numerous centres worldwide.

Opis:
Introduction This literature review explores the role of physical activity (PA) in managing and preventing type 2 diabetes mellitus (T2DM), synthesizing the latest guidelines for all T2DM patients. Following PRISMA guidelines, it identifies specific areas for further research. Material and Methods Indexing services (PubMed and Scopus) were used to identify relevant studies, emphasizing original research, review articles, and updated institutional guidelines spanning 2017-2023. The following keywords were used: diabetes, physical activity, type 2 diabetes mellitus, PA, and exercise. Proposal and opinion articles, non-English papers (or those lacking full-text translation), studies involving non-human subjects, pediatric populations or with skewed gender distribution were excluded. Results PA improves quality of life, BMI, glycemic control and well-being. Effects vary by activity domain, such as work or leisure time. Recommendations suggest 60 daily minutes of exercise for children, 150 weekly minutes of moderate intensity exercise for adults, and screening for adverse events. A “sit-less” approach is proposed for those unable to maintain regular activity, with glycemic monitoring for those with variability. Conclusions PA is crucial in T2DM management. Conflicting findings regarding glycemic control warrant further investigation to ascertain causes, whether related to bias or other factors.

Diabetes mellitus (DM) is a group of metabolic diseases which typically presents with frequent urination, increased thirst and increased hunger. DM be classified into three main types: type I (insulin-dependent DM), type-II (non-insulin dependent DM) and type III (gestational DM). Diabetes is a group of diseases of global health significance as 382 million people worldwide had diabetes in the year 2013 and this was projected to increase to an estimated 415 million in 2015. Damage to the nerves of the body (diabetic neuropathy) is the most common complication of diabetes. The signs and symptoms of diabetic neuropathy include numbness, diminished sensation, pain etc. Various types of electrotherapy, such as transcutaneous electrical nerve stimulation (TENS), pulsed-dose electrical stimulation, frequency-modulated electromagnetic neural stimulation, have been reported effective in managing diabetic neuropathy. This study is a systematic review of the evidence to enable the determination of the effectiveness of electrical stimulation and low-intensity laser therapy (LILT), and also aid their recommendation if proven to be effective. The outcome of this study was that TENS and other forms of electrical stimulation reviewed in this study may be effective and safe non-pharmacological treatment modalities in relieving the symptoms associated with diabetic neuropathy. The effectiveness of LILT couldn’t be determined due to the different parameters used to evaluate patients’ outcome and limited number of studies. Authors recommend that further randomized controlled trials with similar methodological parameters and studies with higher quality of evidences are needed to establish the true effectiveness of these modalities in diabetic neuropathy.

Objectives The objective of the study was to investigate the prevalence of undiagnosed dysglycaemia and the risk for type 2 diabetes using the Finnish Diabetes Risk Score (FINDRISC) in the working population of Belgium. Moreover, it was to evaluate performance and applicability of FINDRISC as a screening tool during occupational health surveillance. Material and Methods A cross-sectional analysis was carried out over the years 2010–2011 among 275 healthy employees who underwent a health check including fasting plasma glucose and the FINDRISC questionnaire. The sensitivity, specificity and predictive value of different FINDRISC cut-off values to detect dysglycaemia was revised in the literature and then calculated. Results The prevalence of unknown dysglycaemia was 1.8%. Twelve percent of the employees had a FINDRISC score of 12 to 14 corresponding to a moderate risk of 17% to develop diabetes within the next 10 years, and 5.5% had a score of 15 or more corresponding to a high – very high risk of 33% to 50%. All dysglycaemic individuals had a FINDRISC score of 12 or higher. The sensitivity and specificity for detecting dysglycaemia was respectively 100% and 84.1% for a FINDRISC cut-off value ≥ 12; and 80% and 95.9% for a cut-off value ≥ 15. Conclusions A considerable number of workers had dysglycaemia or was at risk for developing type 2 diabetes. The questionnaire is a reliable, valuable and easy to use screening tool in occupational health surveillance.

Introduction. Several studies have suggested, that diabetes is very important factor of the risk of the chronic a renal disease. The paper aims to present a retrospective analysis of incidence of end-stage renal disease in subgroups of patients with and without diabetes in Poland. Material and methods. For assessing this problem an electronic search was performed using Polish National Health Fund data base from 01.01.2011 until 31.12.2013 in general population and from 01.01.2012 until 31.12.2012 in 2 populations: with diabetes and without diabetes mellitus. Results. The 43.70 % patients with end-stage renal disease was diabetic. The incidence rate in 2012 was: 292.48 ± 90.97 diabetic men / 100,000 diabetic population; 203.10 ± 90.97 diabetic women / 100,000 diabetic population; 23.44 ± 6.34 non-diabetic men / 100,000 non-diabetic population; 17.88 ± 6.33 non-diabetic women / 100,000 non-diabetic population. Conclusions. The incidence rate of end-stage renal failure in diabetics was multiple times higher than the incidence rate in non-diabetics. The incidence rate of new ESRD cases in Poland estimated to be 36.17 per 100,000 of general populations in 2011, 35.28% in 2012 and 30.46 per 100,000 of general populations in 2013. In 2012, the incidence rate of new ESRD cases in male diabetics was 292.48 ± 90.97 per 100,000 of diabetes men population, and in women diabetics 203.10 ± 66.06 per 100,000 of diabetes women population. In the same 2012 year, the incidence of new ESRD cases in men non-diabetics was 3.44 ± 6.34 per 100,000 of non-diabetes, and in women non-diabetes 17.58 ± 6.33 per 100,000 of non-diabetes women population.

Introduction. Type 2 diabetes mellitus (T2DM) is a common metabolic disorder, which carries a risk for atherosclerosis and cardiovascular impairment. The purpose of this review is to demonstrate the role of acetylsalicylic acid (ASA) in primary cardiovascular prevention in T2DM patients, as well as present an outline of microRNAs (miRNA) relevant to ASA therapy and should be evaluated as targets to improve treatment. Brief description of state of knowledge. Although the etiology of hypercoagulable state in T2DM is considered multifactorial, attention mainly focuses on platelet disturbances. Platelets in T2DM not only demonstrate intensified adhesion, activation, aggregation, and thrombin generation, but are likely to deliver miRNAs at specific sites of action in the cardiovascular system, hence contributing to the pathogenesis of cardiovascular events. Objective. Since cardiovascular disease (CVD) is currently the leading cause of mortality among T2DM patients, appropriate risk stratification and management is necessary to reduce morbidity and mortality in this group. A large number of T2DM patients show inadequate response to antiplatelet therapy, which currently revolves around ASA, despite compliance with treatment regimens proposed by the guidelines. Conclusions. The review shows that the use of ASA for primary prevention is beneficial in patients at high cardiovascular risk. However, it is important to select patients in whom ASA therapy will bring the most beneficial outcome with minimal risk for adverse effects. This can be potentially achieved with the use of unique biomarkers. The biologically diverse characteristics of miRNA make them a promising novel biomarker and potential tool for better risk stratification, as well as antiplatelet therapy optimization.

Background: Diabetes mellitus is a chronic disease related to the occurrence of numerous metabolic disorders. Their common feature is hyperglycaemia, caused by insufficient insulin secretion or its malfunction. The World Health Organization states that 346 million people worldwide have diabetes. The studies in adults with type 2 diabetes clearly demonstrate that the change of lifestyle behaviours effectively improves glycemic control. The modifications of lifestyle habits, such as regular physical activity and healthier dietary habits prove to be beneficial for patients. The aim of this study was to determine the factors affecting the level of physical activity in people with diabetes mellitus. Methods: Research material consisted of the data gathered from a diagnostic survey, which was conducted in a form of a questionnaire. The study included 60 people with type 1 or type 2 diabetes. Results: Unfortunately, the results show that not all of them were active enough, which is so crucial for stabilizing the disease. Conclusions: The diagnosis of diabetes in the respondents resulted in more time being spent by them on physical activity, which resulted in the improvement of diabetes control and the normalization of body weight according to BMI, thus increasing the effectiveness of diabetes treatment and reducing complications. The frequency of the physical activity undertaken by the respondents prior to the disease was not related to gender, however, after the diagnosis, it was dependent on gender

The paper investigates whether the changes in cognitive appraisal and coping strategies related to initiation of insulin treatment onset mediate the effect of temperament on changes in positivity ratio among diabetic patients. Temperament, cognitive appraisal, coping strategies and positivity ratio (ratio of positive to negative affect) were assessed among 278 patients: just before conversion to insulin therapy and then one month later. Mediation analysis indicated that endurance and briskness were directly connected to changes in positivity ratio, whilst the effect of perseveration on positivity ratio was indirect via changes in negative appraisal, emotion- and problem-focused coping. The results confirm the stressful nature of the initiation of insulin treatment, and the assumptions of Lazarus’ model of stress and regulative role of temperament.

INTRODUCTION: Recent years have seen a significant increase in the incidence of diabetes mellitus, type 2 diabetes in particular, which closely correlates with the growing problem of overweight and obesity. The aim of the study was to compare eating habits among type 2 diabetic patients with varying body weight as well as to verify whether there was a relationship between dietary habits and the body mass index in these patients. MATERIAL AND METHODS: The study was conducted in diabetology outpatient clinics. A total of 353 diabetic patients participated in the study. The study consisted of two parts, i.e. authors' questionnaire and anthropometric measurements. RESULTS: The recommended number of 4–5 meals a day was declared by 48.6% of subjects with normal body weight, 50.3% of overweight and 48.4% of obese patients. Daily consumption of breakfast was confirmed by 77.1%, 73.9% and 77.8% of respondents, respectively. Daily consumption of whole-grain bread and coarse grits was mostly declared by subjects with normal body weight (62.8%), followed by overweight and obese patients (22.9% and 23%, respectively). Daily consumption of milk and fermented dairy beverages was reported by 24.3% and 15.7% of subjects with normal body weight, 22.3% and 14.7% of overweight patients as well as by 16.7% and 12% of obese patients, respectively. CONCLUSIONS: Eating habits among patients with type 2 diabetes are varied, with healthy eating habits dominating in subjects with normal body weight. A relationship was found between the patients’ BMI values and some of the eating habits as well as consumption rates of various food products.
WSTĘP: W ostatnich latach odnotowuje się znaczący wzrost zachorowalności na cukrzycę, zwłaszcza typu 2, co ściśle koreluje z narastającym problemem nadwagi i otyłości. Celem pracy było porównanie nawyków żywieniowych pacjentów z cukrzycą typu 2 o różnej masie ciała oraz odpowiedź na pytanie, czy występują zależności pomiędzy nawykami a wskaźnikiem masy ciała (BMI) badanych osób. MATERIAŁ I METODY: Badanie zostało przeprowadzone w poradniach diabetologicznych wśród 353 pacjentów z cukrzycą. Składało się z dwóch części – przeprowadzenia ankiety z wykorzystaniem autorskiego kwestionariusza oraz wykonania pomiarów antropometrycznych. WYNIKI: Spożywanie zalecanej liczby 4–5 posiłków w ciągu dnia zadeklarowało 48,6% osób z prawidłową masą ciała, 50,3% z nadwagą oraz 48,4% z otyłością. Codzienne spożywanie I śniadania potwierdziło odpowiednio 77,1%, 73,9% oraz 77,8% z nich. Codzienną konsumpcję pełnoziarnistego pieczywa i gruboziarnistych kasz najczęściej deklarowały osoby z prawidłową masą ciała (62,8%), rzadziej osoby z nadwagą i otyłością, odpowiednio 22,9% i 23% wskazań. Spożycie mleka i mlecznych napojów fermentowanych każdego dnia deklarowało odpowiednio 24,3% i 15,7% osób z prawidłową masą ciała, 22,3% i 14,7% z nadwagą oraz 16,7% i 12% z otyłością. WNIOSKI: Nawyki żywieniowe badanych pacjentów z cukrzycą typu 2 są zróżnicowane, przy czym największą liczbę prawidłowych nawyków żywieniowych zaobserwowano u osób z prawidłową masą ciała. Stwierdzono, iż występuje zależność pomiędzy BMI badanych pacjentów a niektórymi nawykami żywieniowymi i częstością spożycia poszczególnych produktów spożywczych.

Background: Knowledge about diabetes mellitus (DM)-related complications, the direct and indirect costs incur red by patients with DM is still insufficient in Poland. Economedica DM-PL (Economedica) survey was a cross-sectional observational study that aimed to assess these issues. Material and methods: The survey was carried out in 2016 on a representative sample of 2032 diabetic patients recruited by 195 general practioners (GP) and 48 diabetologists (D) randomly selected from the national registry of physicians. Each GP and D recruited 8 to 10 patients with DM according to the specified protocol. The study included 1938 medical questionnaires (comple ted by the GPs and Ds), 1938 questionnaires about costs (completed by the patients) and 1532 treatment adherence questionnaires. Results: The Economedica survey provided new insights into DM epidemiological and cost data. 70% of patients with type-1 DM were diagnosed with the disease before the age of 40. For 76% patients with type-2 DM, it was diagnosed aeftr the age of 50. The average monthly out-of-pocket medical costs for DM patients in 2016 reached 66.2 EUR, while their non-medical costs were 36.8 EUR. Conclusions: The methodology applied in our study proved useful for providing new dimensions of DM epidemiological and cost data in Poland.

Starch-iodine assay method for the determination of α-amylase activity is also used in screening extracts for α-amylase inhibitors. However, there are indications that this method may not be appropriate for screening some classes of compounds or plant extracts. The present study investigated the limitation(s) of this method in screening plant extracts/compounds for α-amylase inhibition. A crude methanol extract (CR) of Dacryodes edulis, its solvent fractions (ethyl acetate (EA), aqueous methanol (AM), and hexane (HX)), quercetin (QC), and benzoic acid (BA) were used for this study. The phytochemical content and antioxidant activity were screened spectrophotometrically. α-Amylase inhibition (expressed in percentage and as IC50) was determined by starch-iodine method approach I and II (ST-ID I and ST-ID II, respectively) and dinitrosalicylic acid (DNSA) as the control method. The results showed that the extracts/compounds (AM, EA, and QC) with significantly high polyphenolic content, antioxidant activity, and starch-iodine complex decolorization effect yielded contrary results of α-amylase inhibition when the results of ST-ID I and II methods were compared to that of the DNSA method. The other test samples (CR, HX, and BA) yielded similar results for all the three methods. The result also showed the decolorization (%) of starch-iodine complex by the test samples correlated significantly (r = 0.877, P < 0.05) with DPPH reduction (%). In conclusion, the present study showed that the starch-iodine method is not appropriate for screening antioxidative extracts/compounds for α-amylase inhibitors – they decolorize the assay reagent in a manner similar to DPPH reduction and hence confound the result.

Recreational diving is becoming an increasingly popular physical activity around the world. The vast majority of people who practice scuba diving are amateurs, often being treated for a number of chronic diseases and having cardiovascular burdens. Due to the high prevalence of diabetes mellitus in the world population and the potential for dangerous complications during diving, there is a need for unified recommendations for safe diving with this condition. Current guidelines from international diving associations are not entirely consistent. This review presents the positions of international diving associations concerning the requirements for diving with diabetes, the scope of diving and recommended management of life-threatening conditions during the dive.
Nurkowanie rekreacyjne cieszy się coraz większą popularnością. Znacząca większość osób praktykujących nurkowanie to amatorzy, często obciążeni zdrowotnie. Duże rozpowszechnienie cukrzycy wśród populacji oraz znaczące ryzyko wystąpienia u pacjenta poważnych powikłań w trakcie nurkowania niesie ze sobą konieczność ujednolicenia zaleceń dla nurków-cukrzyków. Obecne zalecenia organizacji nurkowych nie są ze sobą całkowicie zgodne. INTRODUCTION: Acute coronary syndromes (ACS) and sudden death cause most ischemic heart disease (IHD)- related deaths, which represent 1.8 million deaths per year, with similar numbers of men and women dying from coronary artery disease (CAD). It’s known that inflammation plays crucial role in atherosclerotic plaque formation and its destabilization. The purpose of this study is to evaluate of white blood cells count in its subpopulation in patients with ACS and modifiable cardiovascular risk factors – arterial hypertension and 2 type Diabetes Mellitus (DM). MATERIAL AND METHODS: In this observational cohort trial we observed of 184 patients with ACS. All patients were randomized into four groups: 1st group - 42 patients with ACS without arterial hypertension (AH) or DM; 2nd group – 56 patients with ACS and previous AH; 3rd group – 42 patients with ACS and 2 type DM; and 4th group – 44 patients with ACS and AH and DM. We studied of leukocytes count and their subpopulations in blood. RESULTS: The mean white blood cells count was significant higher in patients with ASC, compared with control group: 8.23 [6.50; 9.40] vs 5.49 [5.20; 5.70] (p<0.001). Similarly, ACS caused increase of leukocytes subpopulation count in blood. The significant higher count of white blood cells was observed in patients with ACS and co-morbidities: 2 type DM and its association with AH. In patients with ACS and previous AH we observed significant lower neutrophils count (p<0.05), but increased quantity of lymphocytes, compared with patients with ACS without co-morbidities (p<0.001). DM and its association with AH was characterized of neutrophils, lymphocytes and monocytes counts growth. CONCLUSIONS:ACS is characterized of raised white blood cells count and its population, especially in cases of association with 2 type Diabetes Mellitus.

Type 2 diabetes mellitus (T2DM) is a metabolic disease with a high risk of complications and mortality. Novel T2DM therapeutic interventions are needed to combat this disease. This study aimed to identify pathways involved in T2DM and investigate sesquiterpenoid compounds from Curcuma zanthorrhiza that could act as SIRT1 activators and NFκB inhibitors. Protein–protein interaction and bioactive compound analysis were conducted using the STRING and STITCH databases, respectively. Molecular docking was used to determine the compounds’ interactions with SIRT1 and NFκB, while toxicity prediction was performed using Protox II. The results showed that curcumin could act as a SIRT1 activator (4I5I, 4ZZJ, and 5BTR) and NFκB inhibitor on the p52 relB complex and p50–p65 heterodimer, while xanthorrhizol could function as an IκK inhibitor. The toxicity prediction indicated that the active compounds of C. zanthorrhiza were relatively nontoxic because beta-curcumene, curcumin, and xanthorrizol belong to toxicity classes 4 or 5. These findings suggest that the bioactive compounds of C. zanthorrhiza could be promising candidates for developing SIRT1 activators and NFκB inhibitors to combat T2DM.

The activity of Cu,Zn superoxide dismutase in the fluid obtained from eye lens capsules after cataract surgery was investigated in samples obtained from patients with senile cataract and with senile cataract combined with diabetes mellitus. Two parameters were measured and compared: the frequency of occurrence of detected superoxide dismutase activity and the relative activity of the enzyme in samples derived from senile cataract patients versus those from the patiens affected additionally by diabetes mellitus. It was confirmed that the decrease of superoxide dismutase activity during cataract was additionally promoted by diabetes mellitus.

Background: Paraoxonase-1 is an HDL-associated esterase that acts as an anti-atherogenic agent by protecting LDL from oxidation. This study investigates paraoxonase-1 activities in children and adolescents with type 1 diabetes mellitus and possible associations with other biochemical markers. Patients and methods: The study enrolled 82 children and adolescents with type 1 diabetes mellitus and 41 controls with similar age and gender distribution. Serum paraoxonase-1 arylesterase and salt-stimulated paraoxonase activities were assessed by measuring the rates of phenyl acetate and paraoxon hydrolysis, respectively; paraoxonase-1 lactonase activity and oxidized LDL were assessed by a pH-sensitive colorimetric assay and ELISA, respectively. Glycated haemoglobin HbA1c and lipid profile were assayed with an immunoturbidimetric method and commercially available kits, respectively. Results: We found lower paraoxonase-1 activities in diabetics when compared to controls. The decrease was statistically significant only for the lactonase activity, the difference being higher when referring to the subgroup with poor glycaemic control. The lactonase activity/HDL ratio was also lower in diabetics vs. controls, but without statistical significance. Both lactonase and arylesterase activities were negatively correlated with HbA1c in diabetics, but only the latter was statistically significant (ρ = -0.21, P = 0.055; ρ = -0.24, P = 0.03, respectively). A correlation coefficient of ρ = 0.196 (P = 0.078) was found between oxidized LDL and HbA1c. Conclusion: All paraoxonase-1 activities were lower in diabetic children and adolescents, but only the decrease in the lactonase activity was statistically significant. Although lipid profile and glycaemic control were altered in diabetics, no differences were observed between groups regarding oxidized LDL level.

Diabetes is one of the common complications during pregnancy. Insulin-dependent diabetes, lasting from childhood, is an outcome of 0,5% of pregnancies, while significantly more pregnant women is diagnosed with gestational diabetes. It is believed to be a risk factor of more frequent occurence of neonatal complications. The aim of our research was to present the most common complications in the early adaptation period of diabetic mothers’ neonates. Retrospective research included 222 neonates of mothers with GDM that were hospitalized in the Neonatal Unit in Katowice from 2009 to 2017. In the study group 199 newborns were born by caesarean section and 23 by natural labour. 23 (10,36%) neonates had features of hypertrophy, whereas 30 (13,51%) – hypotrophy. 33 children (14,86%) have been diagnosed with carbohydrate disorders. Complications in the early adaptation period occurred in 44,1% of neonates and hyperbilirubinaemia was the most frequent (55,1%). Among the developmental anomalies, the most common was the congenital heart disease - 10.81% of newborns. Complications in the early adaptation period occur often in diabetic mothers' neonates, in particular: carbohydrate disorders, pathological jaundice and PIVH features in additional examinations. 10,81% of neonates have been diagnosed with non-critical heart disease.

Introduction and objective. Gestational diabetes mellitus is a common disease among pregnant women. The aim of the study was to compare plasma concentrations of 24 amino acids in umbilical cord blood in newborns from normoglycaemic pregnancies versus those complicated by gestational diabetes. The relationship between birth weight and length of newborns and placental concentrations of individual amino acids were also assessed. Material and methods. The study comprised 76 pregnant women at the gestational age of 37 weeks and more. The control group consisted of 31 women whose 75-g glucose load test excluded gestational diabetes mellitus. The study group encompassed 45 women with diagnosed gestational diabetes mellitus. The placental plasma concentrations of 24 amino acids were determined using ion-exchange chromatography with an automated amino acid analyser. Results. The concentrations of cysteic acid, aspartic acid, threonine, glutamic acid, cystine, and alpha-aminobutyric acid in umbilical cord blood plasma were higher in the group complicated by gestational diabetes compared to normoglycemic pregnancies; otherwise, concentrations of glutamine, alanine, valine, phenylalanine, lysine and, arginine were lower in the group complicated by gestational diabetes than in the normoglycaemic group. Conclusions. The results show that despite optimal control of carbohydrate metabolism during gestational diabetes mellitus, there are many abnormalities in the amino acids metabolism. This suggests that research on the effects of amino acids concentration in umbilical cord blood on the foetal development in gestational diabetes-complicated pregnancies should be continued.

Due to its multi-systemic nature, the development of additional manifestations, such as musculoskeletal complications, is possible including, for example diabetic osteopathy, limited joint mobility, joint disorders, and other, many of which are subclinical and correlated with the disease duration and its inadequate control. They should be recognized and treated properly, because their management improves the patients’ quality of life.

Introduction and aim. The purpose of this study was to describe the profile of physical activity (PA) of Brazilian adults living with diabetes mellitus living in large Brazilian urban centers, as well as to determine whether the practice aligns with the physical activity guidelines recommended for people with diabetes. Material and methods. Cross-sectional data were acquired from the 2020 Surveillance System for Risk and Protective Factors for Chronic Non-communicable Diseases, in which about 54,000 persons aged 18 and older in all Brazilian state capitals were contacted in a telephone survey. Participants reported on their engagement in recreational physical activity and active commuting to school and/or work in the three months preceding the interview, as well as the weekly frequency and duration of these activities. They also stated whether they were living with diabetes. A descriptive analysis was performed, and statistical significance was determined using Pearson’s chi-squared test. Results. In 2020, 7.9% of the population identified themselves living with diabetes. There was a greater frequency among older women and those with less education. Walking, water aerobics, and general gymnastics were the most common kinds of physical activity reported by people with diabetes. Moreover, over half of them (54.5%) were inactive, and 15% matched the physical activity criteria. The majority (90%) practiced PA for 30 minutes or more per day, while 87% of those who were active and exercised 1 to 2 times per week did not meet the requirements of the Ministry of Health. Conclusion. In 2020, 7.9% of the population identified themselves as having diabetes. There was a higher frequency among older women and those with less schooling. In the sample as a whole, approximately 70% of people living with diabetes were inactive (54.5%) or did not meet the minimum BP recommendations for people with diabetes. The duration of each session seemed to be in line with the recommendations, however, the lack of regularity caused by the low weekly frequency meant that the minimum recommended target could not be achieved. Efforts involving the continued monitoring of people living with diabetes and counseling in Primary Health Care to opt for a more physically active life, seem to be promising acts for a healthier life, pending a definitive resolution to the disease.

Type 1 diabetes mellitus in children and adolescents is a question of medical and psychological field. It is relevant to treat and maintain proper glicemic control from the very beginning of the disease. This may prevent from early development of diabetic-related complications. Family functioning is of a high importance in the course of chld's diabetes care. Cooperation between parents, medical service and teachers is needed to protect children from emotional problems and early complications. The role of the family seems to be irreplaceable.

It is widely accepted that purinergic signaling is involved in the regulation of functions of all known tissues and organs. Extracellular purines activate two classes of receptors, P1-adenosine receptors and P2-nucleotide receptors, in a concentration-dependent manner. Ecto-enzymes metabolizing nucleotides outside the cell are involved in the termination of the nucleotide signaling pathway through the release of ligands from their receptors. The pancreas is a central organ in nutrient and energy homeostasis with endocrine, exocrine and immunoreactive functions. The disturbances in cellular metabolism in diabetes mellitus lead also to changes in concentrations of intra- and extracellular nucleotides. Purinergic receptors P1 and P2 are present on the pancreatic islet cells as well as on hepatocytes, adipocytes, pancreatic blood vessels and nerves. The ATP-dependent P2X receptor activation on pancreatic β-cells results in a positive autocrine signal and subsequent insulin secretion. Ecto-NTPDases play the key role in regulation of extracellular ATP concentration. These enzymes, in cooperation with 5'-nucleotidase can significantly increase ecto-adenosine concentration. It has been demonstrated that adenosine, through activation of P1 receptors present on adipocytes and pancreatic islets cells, inhibits the release of insulin. Even though we know for 50 years about the regulatory role of nucleotides in the secretion of insulin, an integrated understanding of the involvement of purinergic signaling in pancreas function is still required. This comprehensive review presents our current knowledge about purinergic signaling in physiology and pathology of the pancreas as well as its potential therapeutic relevance in diabetes.

Background. Diabetes is a set of metabolic diseases characterized by hyperglycemia caused by improper action and/ or secretion of insulin. Currently, diabetes is becoming a serious challenge in modern medicine, this disease affects 425 million people, and the forecasts indicate that by 2045 the number of cases will increase to 629 million. Objective. The aim of the study was to evaluate the knowledge about risk factors for type 2 diabetes among secondary school students in the Silesian Province in Poland and to determine whether there are differences between the level of knowledge between girls and boys and between first-, second- and third-grade students. Material and methods. The survey was conducted among 650 high school students. The research tool was the author's questionnaire. The obtained results were developed using Microsoft Excel 2010 and Statistica 13.3 (TIBCO Inc.). Results. The definitions of type 2 diabetes were known to 63.9%. 91.8% of high school students indicated excessive body mass as a risk factor for morbidity, while 18.8% of people indicated the appropriate type of obesity increasing the risk of type 2 diabetes. Most of the students considered abnormal eating habits as an important factor increasing the occurrence of the disease (92.4%). The most numerous group of high school students were people with average level of knowledge (89.6%). Conclusions. The knowledge of high school students about risk factors for type 2 diabetes was varied. The most numerous group were high school students characterised by the average level of knowledge. There were no statistically significant differences between the proportion of correctly provided responses by women and men. There were statistically significant differences between the proportion of correctly provided responses by first-, second- and third-graders. Our research shows that educational activities should be undertaken, especially about modifiable risk factors for type 2 diabetes.

Background. Education of diabetic patients is integral to effective treatment. Material and methods. In the presented work, we studied Eastern Slovakian patients with diabetes type 2 with or without insulin treatment. We focused on their diabetic self-care and compared their attitudes, education on their disease, and treatment provided by professionals. There were 411 patients in the insulin-treated group. We used the standardized Diabetes Care Profile questionnaire (DCP). Results. The groups had very different attitudes toward diabetic self-care. Patients with provided professional self-care education scored higher in all areas of diabetic care. Appropriate education influenced knowledge and consequently the management and attitudes of diabetic patients toward their disease. Educated patients scored higher in the categories ‘Self-care ability’, ‘Importance of care’, ‘Self-care adherence’, ‘Diet adherence’, ‘Medical barriers’, ‘Exercise barriers’, ‘Monitoring barriers’ and ‘Understanding practice’ (p < 0.01). Patients who had not received diabetes education presented higher scores in emotional areas, i.e., negative and positive attitudes toward diabetes mellitus (p < 0.01). Conclusions. We concluded that it is beneficial for a structured e ducational process to be integrated in diabetes treatment.
Wprowadzenie. Edukacja pacjentów chorych na cukrzycę stanowi integralną część ich skutecznego leczenia. Materiał i metody. W prezentowanym badaniu porównano postawy w zakresie samodzielnej profilaktyki cukrzycy w dwóch grupach pacjentów cierpiących na cukrzycę typu 2, która wymaga insulinoterapii, w odniesieniu do szkoleń edukacyjnych dotyczących ich choroby oraz leczenia prowadzonego przez specjalistów. Badaną grupę stanowiło 411 pacjentów cierpiących na cukrzycę typu 2 leczoną za pomocą insuliny, którzy pochodzą ze wschodniej części Słowacji. Zastosowano standardowy kwestionariusz DCP (ang. Diabetes Care Profile), wykorzystywany do oceny czynników społecznych i psychologicznych wśród cukrzyków. Wyniki. Postawy dwóch porównywanych grup pacjentów wobec cukrzycy różniły się istotnie w zakresie samodzielnej profilaktyki. Pacjenci z zapewnionym profesjonalnym szkoleniem dotyczącym samodzielniej profilaktyki uzyskiwali wyższe oceny we wszystkich obszarach związanych z monitorowaniem i leczeniem cukrzycy. Prawidłowy proces edukacyjny ma wpływ na poziom wiedzy, a tym samym na działania podejmowane przez pacjentów i ich postawy wobec choroby. Pacjenci, którzy otrzymali odpowiednie przeszkolenie, lepiej kontrolowali sfery takie jak „Umiejętności dot. samodzielnej profilaktyki”, „Znaczenie samodzielnej profilaktyki”, „Przestrzeganie zasad samodzielnej profilaktyki”, „Przestrzeganie diety”, „Ograniczenia natury medycznej”, „Ograniczenia w wykonywaniu ćwiczeń”, „Ograniczenia w monitorowaniu” oraz „Rozumienie procedur” (p < 0,01). Z kolei pacjenci nieposiadający odpowiedniej wiedzy prezentowali lepsze wyniki w obszarach związanych ze sferą emocjonalną – negatywnym i pozytywnym nastawieniu do cukrzycy (p < 0,01). Wnioski. W kontekście uzyskanych wyników, zintegrowanie ustrukturyzowanego procesu edukacyjnego z leczeniem osób chorujących na cukrzycę wydaje się konieczne.

The number of people suffering from type 2 diabetes has been growing recently. This chronic disease is connected with lower perceived quality of life and experiencing a lot of stressful situations. Some of these situations can be anticipated. Thus, it is possible to prepare oneself for future difficult situations by using proactive coping strategies. The aim of this research was to verify the level of satisfaction with various areas of life, the frequency of use of proactive coping strategies in the case of type 2 diabetes patients and healthy individuals, as well as mediation role of these strategies in the relationship between positive emotionality, cognitive curiosity and perceived quality of life. One hundred and seventy four persons took part in the research: 85 persons with diabetes and 89 healthy individuals. We used instruments with recognized psychometric properties: The Proactive Coping Inventory, The World Health Organization Quality of Life Instrument-BREF, PANAS Questionnaire and State-Trait Personality Inventory. The analysis of the results showed, among other things, that people suffering from type 2 diabetes are the least satisfied with their health, and the most with their treatment and knowledge about the disease. Healthy individuals are the most satisfied with the physical domain and one’s present life, whereas the least satisfied with the environmental domain. Both groups differ in terms of using proactive coping strategies. People with diabetes most often adopt a preventive strategy, whereas healthy individuals a reflective one. Two strategies turned out to be able to mediate in the relationship of positive emotionality and life satisfaction in the diabetes group. It was a strictly proactive and preventive coping strategy. No mediation effect was found in the group of healthy people. The results show that in the face of anticipated difficulties, people with type 2 diabetes try to protect their current resources by resorting to their existing knowledge, the level of which they are most satisfied with. The higher the positive emotionality/cognitive curiosity among people with type 2 diabetes, the higher their satisfaction with life, which is due to the fact, that they more frequently use proactive or preventive strategies to cope with stress.

Introduction: Time orientation can significantly improve health-related prevention behaviour and influence disease outcome through boosting health-oriented behaviour. This study aims to compare time perspective between diabetic and cancerous patients, and healthy people. Methods: A cross-sectional descriptive study was conducted on 300 patients (105 healthy, 195 patients: 108 type 2 diabetes and 87 breast and digestive system cancer) aged 20 to 70 years (average age of 45.4 years). The samples were separated into three groups based on a convenience sampling method and were matched in terms of gender, age, education and monthly income. Data was collected through the Zimbardo Time Perspective Inventory (ZTPI-56) questionnaire and then evaluated with analysis of covariance (F test) followed by Fisher’s Least Significant Difference (LSD) test. Results: The three groups showed different time perspectives (F = 4.213, p < 0.05) and different ranking in time orientation. Conclusion: Our findings show that the disease and its type can significantly impact the time orientation of patients. Therefore, to prevent potential subsequent outcomes, the patients’ time perspective towards disease should be improved, especially under disease conditions.

Na świecie notuje się coraz więcej zachorowań na cukrzycę, której częstym następstwem jest choroba wieńcowo-naczyniowa. Wyniki przezskórnej interwencji wieńcowej (PCI), a także operacji pomostowania aortalno-wieńcowego (CABG) u pacjentów chorych na cukrzycę typu 2 są w tym przypadku mniej korzystne. Implementacja stentów wieńcowych uwalniających leki pozwoliła poprawić efekty kliniczne, głównie dzięki zminimalizowaniu stopnia restenozy. Celem pracy jest podsumowanie aktualnego stanu wiedzy, dotyczącego efektywności oraz bezpieczeństwa stosowania stentów uwalniających leki antyproliferacyjne u pacjentów chorych na cukrzycę typu 2.
Cardiovascular disease is a major consequence of this chronic condition. Outcomes after percutaneous coronary interventions (PCI) and coronary artery bypass grafting (CABG) are worse in patients with diabetes mellitus (DM) compared with those without DM. Drugs eluting stents implantation has improved the clinical outcomes of diabetic patients, especially the reduction of restenosis rate. The purpose of this review is to summarize the current achievements relating to the effectiveness and safety of drug eluting stents in diabetic patients.

Aldose reductase gene polymorphisms has been indicated to be associated with diabetic retinopathy (DR). The research data were from PubMed and EMBASE. We identified -106C > T single nucleotide polymorphism (SNP). Pool odds ratio (OR) with 95% CI were calculated. Nine studies were included. ALR2 106C > T gene polymorphisms was associated with the increased risk of DR in T1DM (C vs. T, OR = 2.07, p = 0.001; CC vs. CT + TT, OR = 2.56, p = 0.005). T allele and TT genotype were associated with decreased risk of DR in T1DM (OR = 0.48, p = 0.0001 and OR = 0.12, p = 0.0005). In conclusion, C allele and CC genotype may be a risk factor, while T allele and TT genotype may serve as protective factor for DR in T1DM patient.

Oxidative stress, capable of eliciting damage to various biomolecules including DNA, is a recognized component of diabetes mellitus and its complications. Metabolic syndrome (MetS) is associated with the development of type 2 diabetes mellitus (T2DM), as well as other unfavorable outcomes. The aim of this study was to elucidate the role of oxidative stress in the development of T2DM, by investigating association of oxidative DNA damage with metabolic parameters in subjects with MetS and early T2DM. Selected anthropometric and biochemical parameters of MetS, inflammation and oxidative DNA damage: body mass index (BMI), fatty liver index (FLI), waist circumference (WC), total cholesterol, HDL and LDL-cholesterol, gamma-glutamyl transpeptidase (GGT), uric acid, C-reactive protein (CRP), total leukocyte/neutrophil count, and urinary 8-hidroxy-deoxyguanosine (u-8-OHdG) were assessed in male subjects with MetS and both younger (≤55 years) and older (>55 years) subjects with T2DM of short duration without complications. BMI, FLI, WC, total and LDL-cholesterol and uric acid were higher, while the u-8-OHdG was lower in MetS group, when compared to older T2DM subjects. None of these parameters were different neither between MetS and younger T2DM, nor between two sub-groups of subjects with T2DM. Values of CRP, HDL-cholesterol, triglycerides, GGT, leukocytes and neutrophils were not different between all examined groups of subjects. Higher 8-OHdG in older subjects with T2DM suggests that both aging process and diabetes could contribute to the development of DNA damage. Oxidative DNA damage cannot serve as an universal early marker of T2DM.

In middle adulthood the intensity of stress is significantly higher than in the preceding developmental period. This stress is particularly significant in the case of chronically ill women, including those with type 2 diabetes. In this group, the disease-related stress intensifies the difficulties generated by the decrease of age-related organismic resources and in many instances impairs the quality of life. Therefore, an ability to cope with difficult situations is of crucial importance. The aim of the research was to estimate the general level of life-satisfaction, as well as the frequency and effectiveness of proactive coping strategies in a group of middle adulthood women with type 2 diabetes compared to women not suffering from diabetes. The study was conducted with methods which have acknowledged psychometric properties (SWLS, WHOQoL - BREF, PCI) as well as a survey collecting sociodemographic data. Women suffering from type 2 diabetes are less satisfied with their prior and current life, health and the physical dimension of their lives and environment. Only partially do they employ proactive coping strategies different from those used by the women from the healthy group. For life-satisfaction in its various aspects it is particularly important for the diabetic women to more often use the strategy of reflective coping and to less often seek emotional support while using the strategy of preventive coping or avoidance. The proactive strategies directed at anticipated stress are connected with the level of life-satisfaction and considered to be health resources.

In the work, we studied the parameters of the psychological and emotional state of patients with type 2 diabetes mellitus on the background of excessive body weight. Psychological and emotional state studies were conducted using the Patient Health Questionnaire-PHQ-9, questionnaire the hospital scale of anxiety and depression (HADS). A comparison of the frequency of occurrence of anxiety-depressive disorders and the study of the degree of their severity in the patients under study. The study revealed that negative changes in the indicators of psychological and emotional state in patients with type 2 diabetes are six times more likely than in healthy people.

Diabetes in an incurable disease. The diabetes treatment consists of keeping the blood glucose at a low level. The first aim of this study was to estimate the relationship between anxiety of healthy women and women with type 2 diabetes The second aim was to estimate the relationship between anxiety and metabolic control in diabetic women. The first group which was examined consisted of 66 women with type 2 diabetes who were at hospital and second group was formed of 66 healthy women. The average age of both groups was 53. I used the State Trait anxiety Inventory (STAI) by C. D. Spilbergera and glycosylated hemoglobin for glycemic control. The research indicates that the diabetic women had a significantly higher level of trait anxiety and poorer metabolic control was correlated with state anxiety.

Background. Chronic complications of diabetes are presently the most vital problem in diabetology and the most significant challenge in the treatment of the diseases. Because of peripheral neuropathy and macroangiopathy, people with diabetes are particularly vulnerable to foot problems. The following study aims to evaluate the development of chronic complications in patients with type 2 diabetes, with a particular focus on the prevalence of limb ischemia symptoms. Material and methods. The study involved patients with type 2 diabetes mellitus and nondiabetic persons at a similar age. Diabetics were divided into 2 groups. Group 1 consisted of type 2 diabetic patients with no trophic lesions of the limbs; Group 2 included patients with diabetic foot disease; and the control group (Group 3) were the non-diabetic elderly. The study was to analyse the incidence of retinopathy, nephropathy and neuropathy in both diabetic groups. Besides, it was to evaluate changes in macroangiopathy among diabetic patients and the control group. Results. In all groups, symptoms of lower limb ischaemia were assessed, which allowed making comparisons between them. The studied material indicated that the occurrence of diabetic ulcers is mostly associated with macroangiopathy and its symptoms, polyneuropathy and particular types of neuropathy, retinopathy and horny foot skin. Conclusions. An interdisciplinary approach to the problem of the diabetic foot allows recognising its early clinical symptoms.
Wprowadzenie. Przewlekłe powikłania cukrzycy są obecnie najważniejszym problemem w diabetologii i stanowią największe wyzwanie w leczeniu cukrzycy. Z powodu obwodowej neuropatii i makroangiopatii chorzy na cukrzycę są grupą szczególnie narażoną na problemy zdrowotne stóp. Celem pracy jest ocena rozwoju przewlekłych powikłań u chorych na cukrzycę typu 2 ze szczególnym uwzględnieniem analizy częstości występowania objawów niedokrwienia kończyn. Materiał i metody. Badaniami objęto chorych z cukrzycą typu 2 oraz osoby niechorujące na cukrzycę w podobnym przedziale wiekowym. Diabetyków podzielono na 2 grupy. Grupę 1- stanowili chorzy na cukrzycę typu 2 bez zmian troficznych kończyn, grupę 2- stanowili chorzy z zespołem stopy cukrzycowej, grupę kontrolną (grupa 3)- stanowiły osoby w wieku podeszłym niechorujące na cukrzycę. Przeanalizowano występowanie retinopatii, nefropatii i neuropatii w obu grupach diabetyków. Oceny zmian o charakterze makroangiopatii dokonano wśród chorych na cukrzycę i w grupie kontrolnej. Wyniki. We wszystkich grupach oceniano objawy niedokrwienia kończyn dolnych. Dokonano porównań między grupami. W badanym materiale występowanie owrzodzeń cukrzycowych wiąże się w największym stopniu z makroangiopatią i jej objawami, polineuropatią i poszczególnymi typami neuropatii, retinopatią oraz występowaniem zrogowaciałego naskórka stóp. Wnioski. Interdyscyplinarne podejście do problemu stopy cukrzycowej umożliwia rozpoznawanie jej wczesnych objawów klinicznych.

Background: Functioning in a peer group plays an important role in child development. Building self-esteem of children depends on many factors such as physical health, self-evaluation of their physical performance, academic achievement, social support, family relationships, as well as relationships with peers and teachers. Importance of contacts with peers increases during school-age. At this time children are not able to reject the opinion of other people and subject their behaviour completely to the expectations of their peers. When the need for emotional contact is not satisfied, the complex of being different appears. Isolation within group causes a sense of inferiority, the child becomes passive and resigned. Children, who are chronically ill, rejected or isolated often follow negative emotions in their actions, they become aggressive towards other children or do not want to continue learning. Often the decision about an individual teaching plan results in further isolation of the child. The child’s position in the peer group affects the developing personality, self-confidence and self-esteem. Aim of the study: The aim of the study was to determine the interpersonal relationships of young school-aged children with type 1 diabetes in their peer groups. Material and methods: The study was conducted on children with type 1 diabetes and their parents, who are supervised by Clinic of Diabetes, Regional Medical Center in Opole. The research tool used in both groups was a questionnaire consisting of open and closed questions elaborated by the authors of this study. Results: 73% (41) of evaluated children declared that diabetes does not hinder their performance at school. 93% (55) children claimed that they do not hide their condition from their peers. In addition, 61% (34) talk about their illness with their peers and 69% (38) of them can count on their peers’ help in self-control of diabetes. 71.9% (46) of parents of children with type 1 diabetes allow their children to participate in school trips and 68.8% (44) to participate in school competitions. Conclusions: In most children, diabetes does not affect children’s performance at school. They participate in classes, are accepted and liked in their peer group. Chronically ill children should be able to participate in all types of activities as much as their healthy peers. It seems necessary to continue studies on performance of young school-aged children with type 1 diabetes in their peer group.

Introduction. High amputation of the lower limb not only causes immense physical disability but also the destruction of the patient’s mental state, and helps to shorten life of patients with diabetes. The incidence of amputations in diabetic patients is 10 times higher in comparison to non-diabetic subjects (2.8% vs. 0.29%). The purpose of the study is an analysis of the geographic variability of major non-traumatic lower limb amputation in diabetic and non-diabetic patients in Poland. Materials and method. All major non-traumatic lower limb amputations performed for the first time, in particular data between 1 January 2013 – 31 December 2013, and between 1 January 2014 – December 2014, were identified in the National Health Fund (NHF) database. In the presented study, the patients were grouped in relevant provincial departments of the NHF according to their place of residence, and not according to the hospital where lower limb amputation was performed. Results. In 2013 in Poland, 4,727 major non-traumatic lower limb amputation were performed in diabetic patients, and 4,350 in 2014. On the other hand, in non-diabetic patients, 3,469 major non-traumatic lower limb amputations were performed in 2013, and 3149 in 2014. The mean number indicator of major non-traumatic lower limb amputations in diabetic patients in Poland, compared to the average indicator of amputations in patients without diagnosed diabetes in Poland was 19.9-fold in 2013 and 19.4-fold higher in 2014. Conclusions. In populations of diabetic patients and individuals without diagnosed diabetes major non-traumatic lower limb amputations are performed over 19-fold more frequently.

Introduction. Diabetes mellitus is a chronic metabolic disorder that can lead to an increased length of hospital stay, delayed healing, and exacerbation of co-morbidities with complications arising if not properly managed. Better understanding and maintenance of normal blood glucose by nurses can positively influence care outcomes. The study aimed to assess nurses’ knowledge on the management of diabetes and skills in blood glucose monitoring. Materials and method. A structured pre- and post-questionnaire was used to assess nurses’ knowledge on the management of diabetes, and a structured checklist was used to assess their skills in blood glucose monitoring. Three hundred and thirty- eight (338) participants directly involved in the provision of care to diabetic patients were randomly selected for the study. Data were analyzed using descriptive and inferential statistics. Results. The study revealed that 58.9% of the respondents scored above the mean score of 33.1±3.5 during pre-assessment, which shows they had a good baseline knowledge of blood glucose. A post-intervention finding showed 64.8% scored above the mean score of 36.3±3.1. Assessment of nurses’ knowledge of blood glucose monitoring proficiency showed that 70.1% scored above the mean score of 11.4. Conclusions. The educational intervention programme with a practical training session was an effective method of approach for improving blood glucose management among the participants. However, it is recommended that protocols and practice guidelines should be placed at strategic locations, and methods of empowering nurses, such as providing them with equipment needed to sustain this practice, should be heightened.

With the improvement of people’s living standards and rapid economic development, the incidence of diabetes mellitus (DM) is increasing in most parts of the world. DM presents an important potential threat to human health. In the present study, a model of diabetes in female mice was established, and fasting blood glucose was detected at week 4, after which the biochemical profiles were evaluated by histopathological analysis. The success rate of modeling in the normal control (NC) group and the low/ middle/high-dose streptozotocin (STZ) group were 0, 0, 25% and 60%, respectively. In the middle-dose and high-dose STZ groups, the liver index was increased significantly compared with the NC group (p<0.05). The blood biochemical indicators of total cholesterol and low density lipoprotein cholesterol in three STZ injection groups were as follows: alanine aminotransferase and aspartate transaminase in middle- and high-dose STZ groups, high-density lipoprotein cholesterol and serum creatinine in the high-dose STZ group, and blood urea nitrogen in the middle-dose STZ group were significantly increased (p<0.05). The level of total triglycerides was lower, obviously, in the high-dose STZ group than in the NC group (p<0.05). The mice showed marked steatosis, green-dyed fiber tissue coloring in varying degrees, and the contour of the hepatic lobules basically disappeared in STZ injection groups. The results suggest that to establish a diabetes model for female ICR mice, the optimum dose of STZ is 100 mg/kg.

The primary objective of this paper is the custom design of an effective, yet relatively easy-to-implement, predictive control algorithm to maintain normoglycemia in patients with type 1 diabetes. The proposed patient-tailorable empirical model featuring the separated feedback dynamics to model the effect of insulin administration and carbohydrate intake was proven to be suitable for the synthesis of a high-performance predictive control algorithm for artificial pancreas. Within the introduced linear model predictive control law, the constraints were applied to the manipulated variable in order to reflect the technical limitations of insulin pumps and the typical nonnegative nature of the insulin administration. Similarly, inequalities constraints for the controlled variable were also assumed while anticipating suppression of hypoglycemia states during the automated insulin treatment. However, the problem of control infeasibility has emerged, especially if one uses too tight constraints of the manipulated and the controlled variable concurrently. To this end, exploiting the Farkas lemma, it was possible to formulate the helper linear programming problem based on the solution of which this infeasibility could be identified and the optimality of the control could be restored by adapting the constraints. This adaptation of constraints is asymmetrical, thus one can force to fully avoid hypoglycemia at the expense of mild hyperglycemia. Finally, a series of comprehensive in-silico experiments were carried out to validate the presented control algorithm and the proposed improvements. These simulations also addressed the control robustness in terms of the intersubject variability and the meal announcements uncertainty.

Objectives Almost nothing is known about the effect of occupational noise on diabetes, and this is particularly relevant given the wide spread of both noise exposure and diabetes. This study has aimed to determine whether occupational noise exposure is associated with higher risk of diabetes in Europe. Material and Methods This study is based on 7th European Social Survey (ESS7, 2014) – a multi-country population-based questionnaire survey, which covered 28 221 Europeans aged ≥ 15 years old. Data on self-reported noise exposure, diabetes and other sociodemographic and work-related factors was available. The odds of prevalent diabetes were explored using unconditional logistic regression. Results In the total sample (N = 23 486), participants ever exposed to very loud noise had no substantive increase in the odds of diabetes (odds ratio (OR) = 1.01, 95% confidence interval (CI): 0.78–1.32). There were subgroups with non-significantly increased odds: men (OR = 1.12, 95% CI: 0.87–1.45), the elderly (OR = 1.09, 95% CI: 0.91–1.31), ethnic minority members (OR = 1.55, 95% CI: 0.91–2.62), those with secondary education (OR = 1.05, 95% CI: 0.78–1.41) and those living in small cities/towns (OR = 1.07, 95% CI: 0.89–1.29). Low-skilled white-collar workers had OR = 1.34 (95% CI: 1.09–1.64). Among participants employed during the preceding 5 years the odds were OR = 1.24 (95% CI: 0.95–1.61). Conclusions Self-reported occupational noise was not associated with increased odds of prevalent diabetes in the total sample. Sensitivity analyses revealed some subgroups with non-significantly higher odds. Our results suggest that further delve into the relationship between occupational noise and diabetes is feasible and warranted. Int J Occup Med Environ Health 2017;30(4):537–551

Background: Cardiovascular proteomics investigation reveals the characterization and elucidation of the novel therapeutic targets and strategies to prevent the development of heart failure associated diabetic complication by using 2DE and MS. Methods: The experimental animals were made diabetic with a single intraperitoneal injection of alloxan (150 mg/kg of bw). Albino rats were randomly divided into four individual groups: Group-I control (n=6), group-II alloxan-induced diabetic rats, untreated (n=6), group-III (n=6) and group-IV (n=6) alloxan-induced diabetic rats were treated with aqueous and ethanolic extracts of Cynodon dactylon for 15 days, respectively. Animals were euthanized to collect the heart tissues and blood samples. 2DE sample preparation, gel running and staining (n=6: each groups) were performed at the same time to avoid variation. The result of six gel images from each group were analyzed and evaluated as one match set with 2D software (P<0.05). Results: The above experiment revealed two up-regulated proteins in group-II i.e. NTF4 and ETFB. Conclusions: NTF4 is a neuro-protective agent for neuro-degenerative diseases. It will prevent diabetic secondary complications, such as diabetic polyneuropathy and cardiomyopathy. ETFB is active in the mitochondria, the energy-producing centres in cells. It is clear from the experiment that because of up-regulation of ETFB more energy is availabile and the electron transfer for heart during diabetes is possible, what leads to reduce the oxidative stress and free-radical formation. The up-regulated proteins reduced CVD that occurred just before overt hyperglycaemia due to administration of C. dactylon. This approach established the preliminary reference map for decoding cellular mechanisms linked between pathogenesis CVD and diabetes.

Transplantation of the pig islets of Langerhans is considered as the future treatment for patients suffering from type I diabetes mellitus. Despite the adaptation of modified Ricordi method and highly purified collagenase, the results of pancreas digestions are precarious. Selection of proper donor and optimal digestion procedure are fundamental. The aim of this study was to assess the impact of pancreas procuring parameters on pig islets yield. The pancreata were harvested from 69 market sows weighting over 150 kg. After intraductal injection of cold collagenase solution pancreata were transported in UW solution or under conditions of two layer method (TLM). In laboratory pancreata were digested at 37℃ according to Ricordi isolation method or stationary in the bottle. The particular parameters of isolation procedure were considered as substantial. Pig weight, volume of infused collagenase solution, TLM application and pancreas dividing before digestion positively affected islet yield. Additionally, the influence of pancreatic islet tissue histomorphology on isolation outcome was studied. Proper donor selection as well as adequate digestion parameters could improve pig islet recovery during islet isolation.

Background. The aim of this work was to examine the awareness of patients with diabetes mellitus (DM) on the importance of physical activity (PA) in the prevention and treatment of their disease. Material and methods. The survey was completed by 178 respondents (47.3% women, 52.7% men), of whom 69.1% were dependent on exogenous insulin. Data was collected in April 2019 via a questionnaire given to patients at the Department of Diabetology and Internal Medicine at the Louis Pasteur University Hospital in Košice, Slovakia. Results. More than half of the respondents admitted having insufficient information about their health condition, and a similar proportion felt lack of knowledge regarding exercise for their illness. As many as 69.1% of the patients reported inadequate help by their health care providers with regard to the do’s and dont’s of their disease, and 41.9% of respondents reported having no knowledgeable health care provider with whom to share their concerns. Conclusions. Based on the results obtained, it is concluded that a substantial number of DM patients do not have adequate information about their health condition and how to improve it. On the other hand, nearly two-thirds of participants recognize that regular physical activity (PA) is an option to achieve positive changes.
Wprowadzenie. Celem pracy było zbadanie świadomości pacjentów z cukrzycą co do znaczenia aktywności fizycznej w procesie zapobiegania ich chorobie i jej leczenia. Materiał i metody. W badaniu wzięło udział 178 respondentów, z czego 47,3% stanowiły kobiety, a 52,7% mężczyźni. 69,1% badanych było zależnych od insuliny egzogennej. Dane zostały zebrane w kwietniu 2019 r. za pomocą kwestionariusza przekazanego pacjentom na Oddziale Diabetologii i Chorób Wewnętrznych Szpitala Uniwersyteckiego Louisa Pasteura w Koszycach na Słowacji. Wyniki. P onad połowa respondentów przyznała, ż e nie j est w ystarczająco p oinformowana w kwestii dotyczącej ich stanu zdrowia; podobna liczba ankietowanych odczuwała brak wiedzy o ćwiczeniach w związku z ich chorobą. Aż 69,1% pacjentów zgłosiło nieodpowiednią pomoc ze strony instytucji świadczących usługi medyczne w odniesieniu do nakazów i zakazów dotyczących ich stanu zdrowia. Ponadto 41,9% ankietowanych zgłosiło brak wykwalifikowanych pracowników służby zdrowia, z którymi mogliby podzielić się swoimi obawami. Wnioski. Na podstawie uzyskanych wyników stwierdzono, że znaczna liczba pacjentów cierpiących na cukrzycę nie ma wystarczającej wiedzy co do swojego stanu zdrowia i możliwości jego poprawy. Z drugiej strony prawie dwie trzecie uczestników badania postrzega regularną aktywność fizyczną jako drogę do osiągnięcia pozytywnych zmian.

UBE2E2 encodes ubiquitin-conjugating enzyme E2E2, which plays an important role in the synthesis and secretion of insulin. Two previous studies indicated that SNPs in UBE2E2 were associated with risk for type 2 diabetes mellitus (T2DM) in the Japanese and Korean populations, respectively. We examined the association of one SNP in this gene, rs7612463, with the risk of T2DM in 1957 Han participants in northeastern China, using an SNPscanTM Kit. rs7612463 genotype was significantly associated with risk for T2DM under various genetic models, including an additive model (P = 0.004), a dominant model (P = 0.024), and a recessive model (P = 0.008). The AA genotype was associated with a significantly decreased risk for T2DM (P = 0.004, OR = 0.513, 95% CI = 0.325-0.810) after adjustment for age, gender, and BMI. The heterozygous genotype, AC, was associated with increased risk for total cholesterol (mmol l-1; P = 0.031) and triglycerides (mmol l-1; P = 0.039) in control individuals. Our results show that rs7612463 is associated with T2DM, with homozygotes of the AA genotype at decreased risk for T2DM in the Chinese population. Additionally, heterozygotes may have decreased risk of T2DM due to insulin resistance.

This paper deals with an application of regression analysis to the regulation of the blood-sugar under diabetes mellitus. Section 2 gives a description of Gram-Schmidt orthogonalization, while Section 3 discusses the difference between Gauss-Markov estimation and Least Squares Estimation. Section 4 is devoted to the statistical analysis of the blood-sugar during the night. The response change of blood-sugar is explained by three variables: time, food and physical activity ("Bewegung"). At the beginning of the section it is shown that the proposed method was very successful in 2007.

Two recent studies found that RBMS1 gene rs7593730 and BCAR1 gene rs7202877 are related to type 2 diabetes. However, the association of these loci with type 2 diabetes mellitus (T2DM) has not been examined in Chinese. We performed a replication study to investigate the association of the 2 susceptibility loci with T2DM in the Chinese population. We genotyped 1961 Chinese participants (991 with T2DM and 970 controls) for each of the 2 single nucleotide polymorphisms (SNPs) rs7593730 in RBMS1 and rs7202877 near BCAR1 using SNPscan and examined their association with T2DM using logistic regression analysis. We also analyzed the correlation of the SNP alleles and clinical phenotypes. In additive model, genotype association analysis of BCAR1 rs7202877 loci revealed that the homozygous of rs7202877 GG carriers had significantly decreased T2DM risk compared to homozygous carriers of TT (P=0.038, OR 0.44, 95% CI 0.20-0.96). In the recessive model, the GG genotype GG had significantly decreased T2DM risk compared to GT+TT (P=0.043, OR 0.67, 95% CI 0.46-0.99). Allele G was statistically significantly correlated with TC (mmol/L) (P=0.036) and LDL-C (mmol/L) (P=0.007). As for rs7593730, the carriers of CT and TT genotype had significantly decreased T2DM risk compared to the carriers of CC genotype (CT: CC P=0.038, OR 0.71, 95% CI 0.51-0.98; TT: CC P=0.010, OR 0.32, 95% CI 0.13-0.76). In a dominant model, TT+CT: CC (P=0.013, OR 0.673, 95% CI 0.49-0.92) and in a recessive model, TT: CT+CC (P=0.019, OR 0.59, 95% CI 0.39-0.92). The T allele carriers had significantly decreased T2DM risk compared to the carriers of C (P=0.002, OR 0.65, 95% CI 0.50-0.86). Allele T was statistically correlated with FINS (P=0.010). In conclusion, our study showed that RBMS1 gene rs7593730 and BCAR1 gene rs7202877 were significantly associated with type 2 diabetes in the Chinese population.

The article is a report of studies on the relationship between social support and the effectiveness of treatment for diabetes in youth. The aim of this study was to answer the question whether between patients with different treatment outcome differences in perceived social support. The study were 100-strong group of diabetics aged 16–18 years (50 girls and 50 boys), which has been assigned to groups of varying effectiveness of diabetes management. Adopted two criteria for allocation of subjects into groups: 1) the level of self-control of diabetes measured by the scale of „Treatment of Diabetes”(LC), 2) the degree of metabolic control measured by HbA1c. To measure the social support Sarason’s Social Support Questionnaire was used. Analysis of test results does not reveal significant differences in social support among patients with high and low self-control. In patients with varying degrees of metabolic control of diabetes patients with poor treatment outcome perceive less social support network (p < 0.05), but they are more satisfied with it (p < 0.01) than patients with good control of diabetes. In addition, the length of morbidity of diabetes is moderately, negatively correlated with the network of support and the degree of satisfaction with the aid received.

This overview will discuss the smart polymers as drug nanocarries, their construction and shapes showing their using for controlled insulin release. The report will focus on diabetes mellitus as a disease unit, its etiology and treatment by injection and by using smart polymers. The ingredients described in this article are: poly- (methacrylic acid-g-ethylene glycol), hyaluronic acid, G-CSF-transferrin conjugate in cultured enterocyte-like Caco-2 monolayers, poly(hydroxybutyrate-co-hydroxyhexanoate) and chitosan. Such “intelligent” polymers and their use in a controlled insulin release in diabetic therapy are immensely promising.

Opis:
We propose a decision support framework (DSF) assisting insulin therapy of diabetic children. Our DSF relies on a medical treatment graph (MTG), which models and graphically represents clinical pathways. Using the MTG, it is possible to plan and adapt medical decisions dependent upon the current health state of a patient and the progress of the treatment. Our MTG fits well with the requirements of clinical practice. The presented work is a cooperative effort of researchers in computer science and medicine. The MTG model has been thoroughly tested and validated using real-world clinical data. The usefulness of the approach has been confirmed by physicians.

The purpose of the research was to propound an insulin activity model in a human body after a subcutaneous injection. A deterministic model in the form of a mathematical function was formulated. The research was based on pharmaceutic publicly available drug information published by the manufactures. The paper presents in detail the model. The obtained results can be used in computer simulations of diabetes mellitus therapy. They suggest that activity models may be assigned to types of insulin instead of separate products.
Celem badań było opracowanie modelu aktywności insuliny w organizmie człowieka po wstrzyknięciu podskórnym. Sformułowano model deterministyczny w postaci funkcji matematycznej. Badanie oparto na publicznie dostępnych informacjach farmaceutycznych opublikowanych przez producentów. Artykuł szczegółowo przedstawia model. Uzyskane wyniki mogą być wykorzystane w komputerowych symulacjach terapii cukrzycy. Sugerują one również, że modele aktywności mogą dotyczyć grup insulin zamiast konkretnych preparatów.

Cukrzyca ciążowa (gestational diabetes mellitus – GDM) jest jedną z najczęstszych chorób prowadzących do powikłań w trakcie ciąży. Zauważono, że GDM towarzyszy nasilenie stresu oksydacyjnego. Celem niniejszej pracy jest podsumowanie stanu wiedzy na temat wpływu stresu oksydacyjnego na występowanie i przebieg GDM oraz jej odległe skutki. Przegląd ma także na celu sprawdzenie potencjalnych implikacji klinicznych związanych z GDM. Przeszukano bazy danych takie jak Pubmed, Scopus i Google Scholar. Artykuły wyszukiwano m.in. z użyciem fraz „gestational diabetes mellitus” i „oxidative stress”. Wykazano istotnie wyższy po-ziom stresu oksydacyjnego u kobiet z GDM, zarówno we krwi, jak i w łożysku. Stres oksydacyjny może być ściśle związany z patogenezą GDM. Ze względu na istotną rolę stresu oksydacyjnego w patofizjologii GDM wiele badań skupia się na poszukiwaniu substancji, które mogą zmniejszyć stężenie prooksydantów i zwiększyć udział antyoksydantów, tym samym poprawiając potencjał oksydacyjny. Stres oksydacyjny odgrywa kluczową rolę w patogenezie i patofizjologii GDM. Ponieważ jest to choroba, która może prowadzić do wielu powikłań zarówno u matki, jak i u płodu, jej wczesne wykrycie i skuteczne leczenie wymaga szczególnej uwagi. Ocena możliwości zastosowania parametrów stresu oksydacyjnego jako biomarkerów do wczesnego wykrywania GDM wymaga dalszych szczegółowych badań.
Gestational diabetes mellitus (GDM) is one of the most common diseases that leads to complications during pregnancy. It has been noticed that GDM is accompanied by an increase in oxidative stress. The aim of this study is to summarize the state of knowledge on the influence of oxidative stress on the occurrence and course of gestational diabetes as well as its long-term effects. This review also aims to examine the potential clinical implications of GDM. Databases such as Pubmed, Scopus, and Google Scholar were searched. Articles were searched for using the phrases “gestational diabetes mellitus” and “oxidative stress”. A significantly higher level of oxidative stress was found in women with GDM, both in the blood and in the placenta. Oxidative stress may be closely related to the pathogenesis of GDM. Due to the important role of oxidative stress in the pathophysiology of GDM, many studies have focused on finding substances that can reduce the concentration of prooxidants and increase the proportion of antioxidants, thereby improving the oxidative potential. Oxidative stress plays a key role in the pathogenesis and pathophysiology of GDM. As it is a disease that can lead to a number of complications for both the mother and the fetus, its early detection and effective treatment require special attention. Further detailed research is required to evaluate the possibility of using the parameters of oxidative stress as biomarkers for the early detection of GDM.

Apart from the classic role of vitamin D, its hormonal active form, calcitriol is also characterized by pleiotropic effects on various organs and tissues. For the last several years, many researchers have shown an association between deficiency of vitamin D and the risk of type 2 diabetes mellitus (T2DM). Recent investigations suggested the need of vitamin D supplementation in T2DM prevention. It was shown that vitamin D deficiency decreases insulin secretion. It was also observed that proper vitamin D supplementation may improve the ability of the cells of the islets of Langerhans to synthesize many proteins de novo and to convert proinsulin to insulin. Apart from regulating bone metabolism and also calcium and phosphate homeostasis, 1,25(OH)2D3 exerts antiproliferative and pro-differentiating effects on a wide variety of cell types. It also induces apoptosis of cancer cells and slows their proliferation. In a number of major studies the relationship between low vitamin D levels and increased risk of various cancers was observed. It concerns colorectal, lung, prostate, breast and ovarian cancer. It was observed that in patients with low serum vitamin D concentrations such disorders as ischemic heart disease, heart attack, stroke, cardiac arrhythmia, and hypertension were more frequent and mortality was significantly higher. These results led the researchers to consider vitamin D deficiency as a potential risk factor for cardiovascular diseases. The possible mechanism in the pathogenesis of cardiovascular diseases that may be related to low levels of vitamin D, is its adverse effect on the renin-angiotensin-aldosterone system (RAAS). Calcitriol is also an important determinant of muscle cell proliferation and differentiation, as well as inhibition of apoptosis. Vitamin D is synthesized in the skin. However, there are only a few food products that are rich in vitamin D3, e.g.: fish oils, fish and fortified-products, such as dairy products and margarines. Individuals who are vulnerable to vitamin D deficiency should be supplemented.
Poza klasyczną rolą witaminy D, jej hormonalna postać – kalcytriol wykazuje plejotropowe działanie na różne tkanki i narządy w organizmie. W ciągu ostatnich lat, wielu badaczy wykazało związek pomiędzy niedoborem witaminy D a ryzykiem wystąpienia cukrzycy typu 2 (T2DM). Wyniki badań sugerują potrzebę jej suplementacji w profilaktyce T2DM. Udowodniono związek między niedoborem witaminy D a upośledzonym wydzielaniem insuliny. Zaobserwowano również, że prawidłowa jej suplementacja poprawia zdolność komórek wysp Langerhansa do syntezy de novo wielu białek oraz przekształcania proinsuliny w insulinę. Poza regulowaniem metabolizmu kości i utrzymaniem homeostazy wapniowo-fosforowej, 1,25(OH)2D3 reguluje procesy proliferacji i różnicowania różnych komórek. Ponadto indukuje apoptozę komórek nowotworowych i zwalnia tempo ich proliferacji. W wielu badaniach zaobserwowano związek pomiędzy niskim poziomem witaminy D a zwiększonym ryzykiem wystąpienia różnych nowotworów. Dotyczyło to raka jelita grubego, płuc, prostaty, piersi i jajników. U pacjentów z niskimi stężeniami witaminy D w surowicy zaobserwowano częstsze występowanie schorzeń takich jak choroba niedokrwienna serca, zawał serca, udar mózgu, zaburzenia rytmu serca, nadciśnienie tętnicze oraz wyższą umieralność. Wyniki tych badań skłoniły do uznania niedoboru witaminy D za potencjalny czynnik ryzyka rozwoju chorób sercowo-naczyniowych. Możliwym mechanizmem w ich patogenezie, mogącym tłumaczyć omawiane zjawisko, jest wpływ witaminy D na układ renina-angiotensyna-aldosteron (RAAS). Kalcytriol jest także ważnym czynnikiem, który determinuje proliferację i różnicowanie komórek mięśniowych, jak również hamowanie ich apoptozy. Witamina D jest syntetyzowana w skórze. Niewiele jest jednak produktów spożywczych, które są dobrymi jej źródłami. Są to głównie: oleje rybne, ryby oraz produkty wzbogacane w witaminę D, takie jak produkty mleczne i margaryny. Osoby, które są szczególnie narażone na niedobór tej witaminy powinny stosować jej suplementację.

The frequency of diabetes mellitus, which is an independent and recognized risk factor of cardiovascular disease and the main cause of mortality, is rapidly increasing. The estimated number of patients suffering from diabetes will reach 552 million in 2030, which constitutes 9.9% of the adult population. This prognosticated epidemic forces a revision of existing recommendations, especially a new approach to pharmacological treatment including incretins, drugs affecting the entero-pancreatic axis. Among the available drugs using the incretins effect, there are several oral DPP-4 inhibitors such as sitagliptin, vildagliptin, saxagliptin and linagliptin. There is considerable scientific research confirming the safety and efficacy of incretin therapy, mainly vildagliptin and sitagliptin. The aim of this study was to summarise the results of the above mentioned research with particular reference to elderly people, considering the newest standards and recommendations of world scientific associations.
Cukrzyca jest uznanym i niezależnym czynnikiem ryzyka chorób sercowo-naczyniowych oraz jedną z głównych przyczyn śmiertelności. Częstość jej występowania gwałtownie wzrasta, a szacowana przez ekspertów liczba chorych na cukrzycę osiągnie w 2030 r. 552 mln, co stanowi 9,9% populacji dorosłych. Prognozowana epidemia zachorowań wymusza weryfikację dotychczasowych zaleceń terapeutycznych, szczególnie zaś nowe spojrzenie na możliwości farmakoterapii, w której coraz istotniejszą rolę odgrywają leki wpływające na oś jelitowo-trzustkową. Wśród dostępnych leków wykorzystujących efekt inkretynowy znajdują się podawane doustnie inhibitory dipeptydylopeptydazy 4-gliptyny (DPP-4)-gliptyny: sitagliptyna, wildagliptyna, saksagliptyna oraz linagliptyna. Istnieją liczne prace potwierdzające bezpieczeństwo i skuteczność terapii inhibitorami DPP-4, zwłaszcza wildagliptyny i sitagliptyny, a celem niniejszej pracy było podsumowanie ich wyników, ze szczególnym uwzględnieniem populacji ludzi w podeszłym wieku, w świetle najnowszych standardów postępowania i zaleceń towarzystw naukowych.

Background. There are few updated data on rates of hospital mortality of diabetic patients and length of their hospital stay on a country level. To determine such rates we provided analysis using claims data from a Nationwide General Hospital Morbidity Study carried out by the National Institute of Public Health – National Institute of Hygiene (NIPH- NIH) in Warsaw from 2010 to 2018. Objective. The aim of the study was to analyze the nine years changes of in-hospital morbidity and mortality in diabetic patients and length of hospital stay using a comparative approach by gender, age and place of residence. Material and methods. The data on all patients from general hospitals in Poland treated because of diabetes were taken from a nationwide database, kept since 1979 by the Department for Monitoring and Analyses of Population Health of NIPH-NIH. This database contains information gathered under the Statistical Research Program of Public Statistics. Hospitalization rates were used to evaluate the ‘hospitalized’ incidence of diabetes (number of hospitalization cases due to diabetes per year by the analyzed unit of population). In-hospital mortality was calculated as the percentage of deceased patients out of all patients hospitalized due to diabetes. Results. The number of cases and hospitalization rates of diabetic patients was rapidly declining by 18.8% for type 2 (E11) and 23.7% for type 1 (E10) diabetes. The downward tendency in the scope of hospitalization affected mainly older women and rural residents. Hospital mortality due to diabetes rose up dangerously to 3.77% exceeding the rates recorded eight years earlier. Conclusions. The recent reduction in hospitalization rates of people with diabetes in Poland may be associated with an unexpected increase in hospital mortality.

Obesity is a global problem, more and more children have a weight above the norm. Obesity is related to circulatory system and metabolic disorders. Relations between obesity and metabolic disorders are known, however factors affecting the risk of such syndromes may extend beyond mentioned relations. Existence of vitamin D receptors in many of body cells, especially in pancreas islets of Langerhans indicates the possibility of significant role played by vitamin D in glucose homeostasis. Several epidemiological studies have reported that low 25-hydroxy vitamin D levels, the most common vitamin D indicator in plasma, very frequent in obese children, is correlated with insulin resistance. Moreover, mechanism of vitamin D is associated both with calcium level in tissues, that regulate insulin synthesis and insulin secretion, and direct secretion of pancreatic beta cells. Parathormone PTH, often defined as obesity indicator, is also crucial. Its high level indicates the risk of metabolic disorders. There is no doubt that reducing of body mass together with complementation of vitamin D insufficiency is the best solution for obese children to decrease the risk of incidence and limit the consequences of type 2 diabetes, one of most common civilization disease.
Otyłość jest problemem na poziomie globalnym i dotyczy w coraz większym stopniu dzieci. Prowadzi do wielu chorób, w tym głównie związanych z układem krążenia i zaburzeniami metabolicznymi. Zależności pomiędzy otyłością a chorobami metabolicznymi są powszechnie znane, jednak okazuje się, że czynniki wpływające na ryzyko zachorowalności mogą wykraczać poza wspomniane zależności. Obecność receptorów witaminy D w wielu komórkach ciała, w tym w obszarze trzustkowych wysp Langerhansa, wskazuje na możliwą istotną rolę tej witaminy w przemianach, które warunkują zachowanie właściwej glikemii. Wiele badań wskazuje, że niski poziom 25 hydroksywitaminy D, często spotykany u dzieci z nadwagą, jest skorelowany ze zwiększoną insulinoopornością. Mechanizm działania witaminy D jest nie tylko związany z regulacją poziomu wapnia w tkankach, ale także wiąże się z bezpośrednim wydzielniczym działaniem trzustkowych komórek beta, czyli regulacją syntezy i sekrecji insuliny. Nie bez znaczenia jest stężenie parathormonu PTH, często nazywanego wyznacznikiem otyłości, którego wysoki poziom świadczy o możliwym ryzyku zachorowania na choroby metaboliczne. Wątpliwości nie budzi fakt, iż obniżenie masy ciała wraz z uzupełnieniem niedoborów witaminy D u dzieci otyłych jest właściwym sposobem zminimalizowania ryzyka zachorowania lub ograniczenia skutków jednej z najczęstszych chorób cywilizacyjnych naszych czasów, cukrzycy typu 2.

Cukrzyca typu 2 jest globalnym problemem zdrowotnym, który dotyczy rosnącej liczby dzieci i dorastających młodych ludzi w szerokim spektrum krajów, szczególnie w grupach pochodzenia etnicznego o wysokim ryzyku. Szacunkowo 33% z nich to pacjenci bezobjawowi, dlatego tak istotne dla klinicystów jest przeprowadzanie testów przesiewowych wśród pacjentów chorujących na otyłość, z historią rodzinną cukrzycy typu 2, z grup etnicznych o wysokim ryzyku choroby i z klinicznymi objawami insulinooporności (nadciśnienie tętnicze, dyslipidemia, PCOS, rogowacenie ciemne skóry, historia cukrzycy w ciąży). Rosnące przypadki cukrzycy typu 2 w młodości mogą prowadzić do znaczących długofalowych skutków dla osób dotkniętych chorobą, ale również dla systemów zdrowia publicznego i dla społeczeństwa. Cukrzyca typu 2 wśród dzieci i młodzieży jest powiązana z wcześniejszym występowaniem komplikacji zdrowotnych u osób dotkniętych chorobą, obejmujących neuropatię cukrzycową, retinopatię cukrzycową, miażdżycę naczyń obwodowych, nefropatię cukrzycową i nadciśnienie tętnicze.
Type 2 diabetes is a global health problem affecting a growing number of children and adolescents across a wide range of countries, especially in ethnic groups at high risk. Approximately 33% of them are asymptomatic patients, making it crucial for clinicians to conduct screening tests among patients with obesity, a family history of type 2 diabetes, high-risk ethnic groups, and clinical symptoms of insulin resistance (hypertension, dyslipidemia, PCOS, acanthosis nigricans, history of gestational diabetes). The increasing cases of type 2 diabetes in youth can lead to significant long-term effects for individuals affected by the disease, as well as for public health systems and society. Type 2 diabetes in children and adolescents is associated with an earlier onset of health complications, including diabetic neuropathy, diabetic retinopathy, peripheral vascular disease, diabetic nephropathy, and hypertension.

Alloxan can generate diabetes in experimental animals and its action can be associated with the production of free radicals. It is therefore important to check how different substances often referred to as free radical scavengers may interact with alloxan, especially that some of these substance may show both pro- and antioxidant activities. Using the alkaline comet assay we showed that alloxan at concentrations 0.01-50 μM induced DNA damage in normal human lymphocytes in a dose-dependent manner. Treated cells were able to recover within a 120-min incubation. Vitamins C and E at 10 and 50 μM diminished the extent of DNA damage induced by 50 μM alloxan. Pre-treatment of the lymphocytes with a nitrone spin trap, α-(4-pyridil-1-oxide)- N-t-butylnitrone (POBN) or ebselen (2-phenyl-1,2-benzisoselenazol-3(2H)-one), which mimics glutathione peroxides, reduced the alloxan-evoked DNA damage. The cells exposed to alloxan and treated with formamidopyrimidine-DNA glycosylase (Fpg) and 3-methyladenine-DNA glycosylase II (AlkA), enzymes recognizing oxidized and alkylated bases, respectively, displayed greater extent of DNA damage than those not treated with these enzymes. The results confirmed that free radicals are involved in the formation of DNA lesions induced by alloxan. The results also suggest that alloxan can generate oxidized DNA bases with a preference for purines and contribute to their alkylation.

Several chemical compounds found in plant products have proven to possess beneficial properties, being currently pointed out due to their pharmacological potential in type 2 diabetes mellitus complications. In this context, we studied the effect of Geranium robertianum L. (herb Robert) leaf decoctions in Goto-Kakizaki (GK) rats, a model of type 2 diabetes. Our results showed that oral administration of G. robertianum leaf decoctions over a period of four weeks lowered the plasma glucose levels in diabetic rats. Furthermore, the treatment with G. robertianum extracts improved liver mitochondrial respiratory parameters (state 3, state 4 and FCCP-stimulated respiration) and increased oxidative phosphorylation efficiency.

Introduction. Correctness of insulin treatment depends on both the experience and skills of the doctor and knowledge and behaviour of the patient. Objective. Evaluation the adequacy of insulin doses administered to diabetes patients in ambulatory conditions. Material and methods. The treatment of 59 patients hospitalized in the Diabetology Ward was evaluated at admission, discharge and 3 months after hospitalization. Results. The mean daily doses of insulin significantly differed at times of evaluation and were: 53.90, 39.31 and 43.34 units, respectively (p≈0.000001). A significant reduction of body weight, 90.86 vs. 88.25 kg (p≈0.000001), was obtained only during hospitalization, and was maintained 3 months after discharge (87.86 kg). Significant differences were also noted in the body mass index (33.44 vs. 32.48 vs. 32.37 kg/m2, p≈0.000001). The change in waist circumference was not statistically significant (107.87 vs. 104.89 cm; p≈0.06). A decrease in the number of hypoglycaemia episodes was observed, but were statistically insignificant (25 vs. 23; p≈0.7). Three months after hospitalization an insignificant decrease of HbA1c level was noted (8.41% vs. 8.03%; p≈0.07). Conclusions. During treatment in the Diabetology Ward the procedure of choice was more frequently a reduction than an increase in insulin doses. This management led to the reduction of the patients’ body weight, improvement of glycaemia, without any significant effect on the diabetes control determined by the HbA1c level.

Background. The objectives of this study were to determine the prevalence of type 2 diabetes mellitus (T2DM) among patients with COVID-19, examining the relationship between COVID-19 severity and T2DM in hospitalized patients, detecting T2DM outcomes in COVID-19 patients, and identifying the vaccination rates of COVID-19 patients with T2DM. Material and methods. A prospective, cross-sectional study was conducted on Duhok (Iraqi Kurdistan) hospitalized COVID-19 patients. Demographic, clinical, and laboratory characteristics of all confirmed COVID-19 by RT-PCR with coexisting T2DM were collected between early November 2021 and late April 2022. T2DM patients with HbA1c levels ≥7% were considered a poor control group, while those <7% were considered a good control group. Results. Out of 530 hospitalized COVID-19 patients, 158 (29.81%) were T2DM. Among 158 patients, 23 (14.56%) were vaccinated, of whom 17 (10.76%) were fully vaccinated and 6 (3.80%) partially vaccinated. Considering patient outcomes, chronic pulmonary disease (p=0.0106), obesity (p=<0.0001), patients on combined oral antidiabetic and insulin (p=0.0204), and poorly controlled DM (p=<0.0001) were significant predictors of mortality. Conclusions. The prevalence of T2DM in hospitalized COVID-19 patients was relatively high in Duhok. In contrast with the previous studies reported in the literature, the COVID-19 vaccination coverage was unsatisfactory. Therefore, raising awareness concerning health education about the severity and mortality rates of COVID-19 should be mandatory to achieve better disease prognosis.
Wprowadzenie. Celem niniejszego badania było określenie częstości występowania cukrzycy typu 2 wśród pacjentów z COVID-19, zbadanie związku między nasileniem COVID-19 a cukrzycą typu 2 u pacjentów hospitalizowanych, stwierdzenie wyników leczenia cukrzycy typu 2 u pacjentów z COVID-19 oraz określenie wskaźników szczepienia pacjentów z COVID-19 z cukrzycą typu 2. Materiał i metody. Prospektywnym badaniem przekrojowym objęto pacjentów z COVID-19 hospitalizowanych w Duhok w irackim Kurdystanie. Charakterystyka demograficzna, kliniczna i laboratoryjna wszystkich potwierdzonych metodą RT-PCR przypadków COVID-19 ze współistniejącą cukrzycą typu 2 została zgromadzona między początkiem listopada 2021 a końcem kwietnia 2022. Pacjenci z cukrzycą typu 2 z poziomem HbA1c ≥7% zostali uznani za słabą grupę kontrolną, natomiast pacjenci z <7% za dobrą grupę kontrolną. Wyniki. Spośród 530 hospitalizowanych pacjentów z COVID-19, u 158 (29,81%) występowała cukrzyca typu 2. Wśród 158 pacjentów 23 (14,56%) było zaszczepionych, w tym 17 (10,76%) w pełni, a 6 (3,80%) częściowo. Biorąc pod uwagę wyniki leczenia, przewlekła choroba płuc (p=0,0106), otyłość (p=<0,0001), pacjenci przyjmujący doustne leki przeciwcukrzycowe w skojarzeniu z insuliną (p=0,0204) oraz źle kontrolowane przypadki cukrzycy (p=<0,0001) były istotnymi predyktorami śmiertelności. Wnioski. Częstość występowania cukrzycy typu 2 u pacjentów z COVID-19 hospitalizowanych w Duhok była stosunkowo wysoka. W przeciwieństwie do wcześniejszych danych, współczynniki szczepień przeciwko COVID-19 były niezadowalające. W związku z tym, podnoszenie świadomości w zakresie edukacji zdrowotnej na temat ciężkości i śmiertelności COVID-19 powinno być obowiązkowe w celu uzyskania lepszej prognostyki choroby.

Diabetes Mellitus is a chronic metabolic disease characterized by hyperglycemia and disturbances of carbohydrate, fat and protein metabolism. It is a major public health problem and disease burden worldwide, and especially in Nigeria. This study determined the gender differential in the incidence of Diabetes mellitus among the patients in Udi L.G.A of Enugu State. Three research questions and one hypothesis were formulated to guide the study. Descriptive survey design was employed. A sample of 343 diabetes mellitus patients were randomly selected and used for the study. Researchers design data documentary profoma was used to collect the data from selected health facilities in respect to the patients’ gender and their fasting sugar level as contained in their folder or hospital record/register. Frequency and percentages was used to answer research question while the null hypothesis was tested using Chi- square at 0.05 level of significance. The finding of the result revealed a high incidence of Diabetes Mellitus in 2012, females recorded a high incidence of DM from 2008-2012 and high percentage of fasting blood sugar level than male. The finding also revealed no difference in the fasting blood sugar based on gender. It is recommended among others that diagnostic investigation on human placental lactose is advocated to reduce or stop its opposition to insulin secretion during pregnancy.

Previous studies in other countries have shown that single nucleotide polymorphisms (SNPs) in the growth arrest-specific gene 6 (Gas6; rs8191974) and adapter-related protein complex 3 subunit sigma-2 (Ap3s2; rs2028299) were associated with an increasedrisk for type 2 diabetes mellitus (T2DM). However, the association of these loci with T2DM has not been examined in Chinese populations. We performed a replication study to investigate the association of these susceptibility loci with T2DM in the Chinese population.We genotyped 1968 Chinese participants (996 with T2DM and 972controls) for rs8191974 in Gas6 and rs2028299 near Ap3s2, and examined their association with T2DM using a logistic regression analysis. We also analyzed the correlation of genotypes and clinical phenotypes. The distribution of the T allele of SNP rs8191974 in the Gas6 gene was significantly different between T2DM cases and controls when compared with the C allele (P<0.05, OR: 0.80, 95% CI: 0.69-0.94). The occurrence of the CT genotype and the dominant model was also significantly less frequent in the T2DM cases vs. controls when compared with the CC genotype (CT vs. CC: P<0.05, OR: 0.75, 95% CI:0.62-0.90; TT+CT vs. CC: P<0.05, OR:0.75, 95% CI:0.63-0.90). In SNP rs2028299, the allele C showed no statistically significant differencein distribution between the control and T2DM groups when compared with allele A. However, in male populations, the dominant model was statistically more frequent when compared with genotype AA (CC+CA vs. AA: P<0.05, OR:1.29, 95% CI:1.02-1.64), and in obesity-stratified analysis, we also observed a significant difference in the distribution of the dominant model between the T2DM cases and controls in subjects with BMI≥24 kg/m2 and BMI<28kg/m2 (CC+CA vs. AA: P<0.05, OR: 6.33, 95% CI:4.17-9.61). In conclusion, our study shows that SNPsrs8191974 and rs2028299 are significantly associated with T2DM in the Chinese population.

In search for new markers of insulin-dependent diabetes (IDDM) susceptibility we studied the CATT tetranucleotide repeat in intron 1 of tyrosine hydroxylase (TH) gene on chromosome lip, the CA repeat at T-cell receptor a chain (TCRA) locus on chromosome 14q and two CA repeats at D10S211 and D10S213 loci in the chromosome 10 region containing glutamic acid decarboxylase (GAD2) gene. Alleles at these microsatellite loci were identified in a population of diabetic children and unrelated healthy controls originating from Wielkopolska, a midwestern region of Poland. We found significant association of certain alleles at TH, TCRA and D10S211 loci with diabetes in the population under study. On the contrary, none of the alleles at D10S213 locus was associated with the disease. Our findings indicate that typing of microsatellite markers may represent useful additional tool for identifying individuals at high risk of developing IDDM. Regarding loci on chromosome 10 our data and data published by other authors may suggest the extistence of two separate regions of association with IDDM susceptibility on this chromosome.

INTRODUCTION Nutrition is a key factor affecting the health of diabetics. The study is an attempt to determine if there are differences in salty, sweet and umami taste perception as well as in food preferences between children with diabetes mellitus type 1, treated for years or more, and their healthy peers. MATERIAL AND METHODS Group C (19 children aged 10–18, average age 15, suffering from diabetes mellitus type 1 with at least a five-year medical history, mean 7.2 ± 2.2 years) and Group K1 (42 healthy children of sex, age and BMI corresponding to those of the children from Group C) were examined for their taste perception of sodium chloride, sucrose and monosodium glutamate by means of specific gustometry. Their food preferences were also studied. The number of healthy children whose food preferences were examined was increased to include 105 participants (Group K2). RESULTS In Group C as compared to Group K1, the percentage of correct taste recognition of sodium chloride was lower (p < 0.05) whereas the percentage of incorrect recognition was higher (p < 0.01). The intensity of sucrose taste perception was lower in Group C than in Group K1 (p < 0.01). Group C as compared to Group K2 liked more the taste of: farinaceous dishes (p < 0.01), pasta (p < 0.05), beef, veal and pork (p < 0.01) and salty snacks (p < 0.01). CONCLUSIONS Children and teenagers affected with diabetes mellitus type 1, display disturbances in their perception of salty and sweet taste. Moreover, their food preferences differ from those of healthy children. It seems reasonable to expand the research program in order to confirm the observed relations. Understanding the relation between taste perception and food preferences in young patients with diabetes mellitus may contribute to composing a tastier and more attractive diet, which is a significant element in therapy.
WSTĘP Sposób odżywiania jest głównym czynnikiem wpływającym na zdrowie osób z cukrzycą. W niniejszej pracy podjęto próbę odpowiedzi na pytanie: czy dzieci i młodzież z cukrzycą typu 1, chorujące przez co najmniej 5 lat, różnią się odbiorem smaku słonego, słodkiego i umami oraz preferencjami pokarmowymi od swoich zdrowych rówieśników. MATERIAŁ I METODY W grupie C (19 dzieci w wieku 10–18 lat, średnio 15 lat, z cukrzycą typu 1, z co najmniej 5-letnią historią choroby, średnio 7,2 ± 2,2 roku) oraz w grupie K1 (42 dzieci zdrowych o odpowiednio dobranej płci, wieku i BMI) wykonano badania odbioru smaku chlorku sodu, sacharozy i glutaminianu sodu metodą gustometrii swoistej oraz zbadano preferencje pokarmowe. Liczbę osób zdrowych objętych badaniem preferencji pokarmowych zwiększono do 105 dzieci (grupa K2). WYNIKI W grupie C, w porównaniu z grupą K1, odsetek prawidłowych rozpoznań smaku chlorku sodu był niższy (p < 0,05), natomiast błędnych rozpoznań wyższy (p < 0,01). Intensywność odbioru smaku sacharozy była niższa w grupie C niż w grupie K1 (p < 0,01). W grupie C bardziej niż w grupie K2 były lubiane: potrawy mączne (p < 0,01), makarony (p < 0,05), wołowina, wieprzowina i cielęcina (p < 0,01) oraz słone przekąski (p < 0,01). WNIOSKI U dzieci i młodzieży chorujących na cukrzycę typu 1 występują zaburzenia percepcji smaków słonego i słodkiego. Również preferencje pokarmowe tych dzieci są nieco inne niż dzieci zdrowych. Zasadne wydaje się rozszerzenie badań w celu potwierdzenia zaobserwowanych zależności. Zrozumienie relacji między percepcją smaku a preferencjami pokarmowymi młodych osób chorych na cukrzycę może przyczynić się do uatrakcyjnienia ich diety, stanowiącej istotny element terapeutyczny.

INTRODUCTION: Because of technological development and easier accessibility to diagnostic imaging, incidentally detected adrenal tumors are a frequently diagnosed endocrine disorder. The aim of the paper was to present the characteristics of patients with newly incidentally detected adrenal tumors, with emphasis on the detected hormonal disorders. MATERIAL AND METHODS: A retrospective, single center study included 96 patients hospitalized due to newly detected adrenal tumor between 2007 and 2014. RESULTS: Adrenal tumors were detected in 92 patients. The final diagnoses were: incidentaloma – 73 (76%), nodular adrenal hyperplasia – 12 (12.5%), pheochromocytoma – 5 (5.2%), late-onset congenital adrenal hyperplasia – 5 (5.2%), Conn’s syndrome – 1 (1.1%). In 94 out of the 96 patients at least one hormonal disorder was detected. The most frequent abnormalities were: incorrect plasma renin activity (67.7% of patients), evening serum cortisol concentration (60.2%, mean level: 8.9 µg%) and morning (48.9%, mean level: 15.4 µg%). A considerable group of patients suffered from comorbidities, such as hypertension (70.8%), impaired glucose tolerance (18.7%) and type 2 diabetes (16.7%). Additionally, the diameter of the tumors was negatively (p < 0.05) associated with the serum ACTH level. CONCLUSIONS: In most of the patients with incidentaloma, despite the lack of characteristic symptoms, it is possible to detect hormonal disorders in laboratory tests. The prevalence of hypertension and diabetes is high in this population of patients.
WSTĘP: Wraz z gwałtownym postępem technicznym i zwiększoną dostępnością badań obrazowych przypadkowo wykryte guzy nadnerczy stały się częstym problemem endokrynologicznym. Celem pracy była charakterystyka pacjentów z przypadkowo wykrytymi guzami nadnerczy, ze szczególnym uwzględnieniem wykrywanych nieprawidłowości hormonalnych. MATERIAŁ I METODY: Retrospektywne jednoośrodkowe badanie objęło 96 pacjentów (21 mężczyzn i 75 kobiet w wieku 28–88 lat, mediana 62 lata), hospitalizowanych w latach 2007–2014 z powodu nowo wykrytych guzów nadnerczy. WYNIKI: Guzy nadnerczy zostały przypadkowo wykryte u 92 pacjentów. Po przeprowadzeniu diagnostyki rozpoznano: gruczolaki nadnercza – incydentaloma – w 73 przypadkach (76%), guzkowy przerost nadnerczy w 12 (12,5%), guzy chromochłonne w 5 (5,2%), późno wykryte wrodzone przerosty nadnerczy w 5 (5,2%) oraz zespół Conna w 1 przypadku (1,1%). Co najmniej jedno zaburzenie hormonalne wykryto u 94 spośród 96 pacjentów. Najczęstszymi zaburzeniami były: nieprawidłowa aktywność reninowa osocza (67,7% pacjentów), nieprawidłowe wieczorne stężenie kortyzolu (60,2%, średnie stężenie: 8,9 µg%) i poranne (48,9%, średnie stężenie: 15,4 µg%). Znaczna grupa pacjentów cierpiała na choroby współistniejące: nadciśnienie tętnicze (70,8%), nieprawidłową tolerancję glukozy (18,7%) i cukrzycę typu 2 (16,7%). Ponadto wykazano odwrotną korelację (p < 0,05) największego wymiaru guza ze stężeniem ACTH. WNIOSKI: U większości pacjentów z przypadkowo wykrytymi guzami nadnerczy, pomimo braku charakterystycznych objawów, można wykryć nieprawidłowości w badaniach hormonalnych. Nadciśnienie i cukrzyca występują u tych pacjentów częściej niż w populacji ogólnej.

Diabetes mellitus (DM), being a chronic and essentially incurable disease, is associated with a significant risk of mental disorders in persons affected. The aim of this paper was to answer the following questions: 1) do young people afflicted with type 1 DM are more depressive than healthy peers?; 2) is there a correlation between severity of depression and quality of diabetes control? The study encompassed a group of 100 patients (50 boys and 50 girls) aged 16-22.2 years (mean age 17.8 years; SD=1.6 years) with type 1 DM and 100 healthy age-matched peers. Symptoms of depression were assessed using the Beck Depression Inventory; quality of management of diabetes was assessed by measuring the level of glycosylated haemoglobin (HbA1c), a generally accepted indicator of quality of metabolic control. HbA1c level was a criterion used to assign each patient to one of three subgroups, depending on quality of DM control attained in his or her case: 1) poor, 2) acceptable, 3) good. Results obtained indicate, that patients with DM are significantly more depressive than their healthy peers (p<0.05). Analysis of gender-dependent differences revealed a significantly higher level of depression in diabetic boys as compared with healthy boys (p<0.05). Significant differences in the level of depression were noticed among subgroups depending on quality of metabolic control. Diabetics who have their DM better controlled, present significantly less depressive symptoms (p<0.01). These findings relate to the entire study population and to the girls’ group. DM may constitute one of risk factors for development of depression. Depression may result in inadequate metabolic control. Ineffective management of DM may contribute to the development of depressive symptoms in young diabetics.
Cukrzyca jako choroba przewlekła i nieuleczalna niesie ryzyko występowania zaburzeń psychicznych u osób na nią chorujących. Celem niniejszej pracy była odpowiedź na pytania: 1) czy młodzi pacjenci chorujący na cukrzycę typu 1 są bardziej depresyjni od zdrowych rówieśników?; 2) czy istnieje związek między nasileniem depresji a efektywnością leczenia cukrzycy? Do badania włączono 100-osobową grupę pacjentów (50 dziewcząt i 50 chłopców) w wieku 16-22,2 roku (średnia: 17,8; s=1,6) z cukrzycą typu 1 oraz 100 zdrowych rówieśników. Objawy depresyjne badano Inwentarzem depresji Becka, za wskaźnik efektywności leczenia przyjęto stężenie hemoglobiny glikozylowanej (HbA1c) określające stopień kontroli metabolicznej. HbA1c była kryterium przyporządkowania każdego pacjenta do jednej z trzech grup efektywności leczenia cukrzycy: 1) niewystarczająca; 2) do akceptacji; 3) dobra. Wyniki badań dowiodły, że pacjenci z cukrzycą są istotnie bardziej depresyjni niż zdrowi rówieśnicy (p<0,05). Analiza różnic w obrębie płci wskazała na istotnie wyższy poziom depresji u chorych chłopców w porównaniu z chłopcami zdrowymi (p<0,05). Między pacjentami o różnym stopniu efektywności leczenia cukrzycy występują istotne różnice w poziomie depresji. Diabetycy o lepszym wyrównaniu metabolicznym cukrzycy mają istotnie mniej objawów depresyjnych (p<0,01). Takie wyniki odnoszą się do ogółu badanych oraz grupy chorych dziewcząt. Cukrzyca może stanowić jeden z czynników ryzyka występowania depresji. Depresja może wpływać na niedostateczną kontrolę cukrzycy. Słabe wyniki leczenia cukrzycy mogą przyczyniać się do pojawiania się objawów depresyjnych u młodych diabetyków.

An effective launching of pro-health food on market should be based on a deep knowledge of the target consumers. The study aimed at investigation of attitudes of diabetics toward food enriched with bioactive compounds and effect of diabetes diagnosis on the issue was studied by their comparison with healthy people’s attitudes. The research was conducted by the survey method. The diabetic persons were randomly selected; purposive sampling was applied for healthy people to obtain similar socio-demographic characteristics of both groups. Diabetics claimed knowledge on health benefits of inulin, stevia and mulberry (70%) but their self-estimation of that knowledge did not correspond with the actual state. Fortified food aroused positive emotions in 28% of the participants; however, food was regarded as “unnatural” by over 30%. The low percentage of diabetics bought food with the compounds and searched for information about them on labels. Despite declarations concerning the increased interest in the food issue after diabetes diagnosis (94%), the limited actual knowledge of pro-health food benefits can cause distrust while purchasing such food.
Efektywne wprowadzanie na rynek produktów o cechach prozdrowotnych wymaga poznania ich docelowych konsumentów. Celem było zbadanie postaw diabetyków wobec dedykowanej im żywności wzbogacanej w składniki bioaktywne oraz ustalenie, jak rozpoznanie choroby wpływa na nie (przez porównanie z postawami osób zdrowych). Badania przeprowadzono metodą ankietową. Doboru próby diabetyków dokonano losowo, a zdrowych – celowo dla uzyskania zbliżonej struktury demograficznej prób. Diabetycy deklarowali znajomość właściwości zdrowotnych inuliny, stewii oraz morwy (70%). Samoocena wiedzy respondentów nie pokrywała się jednak z jej stanem faktycznym. Żywność wzbogacana wzbudzała emocje pozytywne u 28% badanych, ale ponad 30% uważało ją za nienaturalną. Niski odsetek diabetyków kupował żywność prozdrowotną zawierającą ww. składniki oraz poszukiwał informacji o nich na etykietach. Pomimo deklarowanego wzrostu zainteresowania informacjami o żywności po diagnozie cukrzycy (94%), ograniczona faktyczna wiedza diabetyków o walorach produktów prozdrowotnych może wywoływać obawy związane z ich zakupem.
Эффективный ввод на рынок продуктов со свойствами пользы для здоровья требует изучения их целевых потребителей. Цель – изучить отношение диабетиков к предлагаемой им пище с добавлением биоактивных компонентов, а также выявить, как диагноз болезни влияет на них (сравнивая с отношением здоровых лиц). Изучение провели по методу опроса. Отбор выборки диабетиков провели по жеребьевке, здоровых же лиц – целевым способом для получения сближенной демографической структуры выборок. Диабетики заявляли о знании свойств (с точки зрения здоровья) инулина, стевии и шелковицы (70%). Самооценка знаний респондентов однако не совпадала с их фактическим состоянием. Обогащенная пища возбуждала по- ложительные эмоции у 28% опрошенных, но более 30% считали ее неестественной. Низкий процент диабетиков покупал полезную для здоровья пищу, содержащую вышеуказанные компоненты, и искал информацию о них на этикетках. Несмотря на заявляемый рост интереса к информации о пище после диагноза диабета (94%), ограниченные фактические знания диабетиков о свой- ствах полезных для здоровья продуктов могут вызывать опасения, связанныес покупкой их.

Obesity is a chronic disease that constitutes a global problem and a serious challenge to public health. In the course of this disease, excessive accumulation of fat tissue occurs in the body, which not only leads to an increased risk of health complications, but also negatively affects the quality of life. Fat cells – adipocytes – are responsible for the biosynthesis and release of adipokines, among others, leptin, visfatin, chemerin and omentin-1. They are active biological substances, including mediators of inflammation, which may lead to the development of metabolic disorders in the body. The above-mentioned properties of the described adipokines make it possible to potentially use them as diagnostic and therapeutic factors in the course of obesity and its accompanying disorders such as insulin resistance, type 2 diabetes mellitus, and cardiometabolic complications. Despite significant progress in understanding the role played by the discussed adipokines, further research is necessary to precisely describe the mechanisms of action and to determine the precise relationship between their plasma concentrations and state of diseases.
Otyłość to przewlekła choroba, stanowiąca globalny problem i poważne wyzwanie dla zdrowia publicznego. W jej przebiegu dochodzi do nadmiernego nagromadzenia się tkanki tłuszczowej w organizmie, co prowadzi nie tylko do wzrostu ryzyka wystąpienia powikłań zdrowotnych, lecz także negatywnie wpływa na jakość życia. Komórki tłuszczowe – adipocyty – są odpowiedzialne za biosyntezę i uwalnianie adipokin, do których należą m.in. leptyna, wisfatyna, chemeryna oraz omentyna-1. Są to aktywne substancje biologiczne, mogące wykazywać właściwości prozapalne lub przeciwzapalne. Zaburzenie równowagi pomiędzy adipokinami prozapalnymi i adipokinami przeciwzapalnymi może prowadzić do rozwoju zaburzeń metabolicznych. Wspomniane właściwości adipokin sprawiają, że możliwe jest ich potencjalne wykorzystanie jako czynników diagnostycznych oraz terapeutycznych w przebiegu otyłości, a także towarzyszących jej zaburzeń, takich jak insulinooporność, cukrzyca typu 2 czy powikłania kardiometaboliczne. Pomimo znacznego postępu w poznaniu roli, jaką odgrywają omówione adipokiny, konieczne są dalsze badania, których wyniki umożliwią precyzyjne omówienie mechanizmów ich działania oraz określą ścisłe zależności ich stężeń w osoczu od stanu choroby.

W artykule autorzy podjęli próbę analizy argumentów przemawiających za włączeniem cukrzycy do grupy chorób pośrednio związanych z pracą. Przeprowadzono przegląd literatury spośród artykułów opublikowanych w latach 1980–2016 w języku angielskim, w którym wykorzystano kombinacje słów kluczowych związanych z pracą zawodową i cukrzycą. Źródłem danych była baza PubMed. Autorzy wskazują, że za uznaniem cukrzycy za chorobę pośrednio związaną z pracą przemawia spełnienie warunków jej definicji: cukrzyca nie należy do chorób zawodowych, nie jest bezpośrednio związana ze środowiskiem pracy lub sposobem jej wykonywania, ale obserwowany jest niekorzystny wpływ na jej rozwój lub przebieg określonych uciążliwości zawodowych (w tym przypadku zmianowej pracy nocnej, wydłużonego czasu pracy, stresu zawodowego czy długotrwałej pracy siedzącej). Liczba publikacji poświęconych cukrzycy, w których uwzględniono problem pracy zawodowej, jest porównywalna z liczbą analogicznych publikacji dotyczących chorób uznanych za związane z pracą, jak nadciśnienie tętnicze czy choroba niedokrwienna serca. Aspekty aktywności zawodowej zostały uwzględnione ponadto w zaleceniach klinicznych dotyczących postępowania wobec chorych na cukrzycę, co jest niespotykane w innych tego typu standardach, nawet w przypadku chorób powszechnie uznanych za związane z pracą. Argumentem przemawiającym za włączeniem cukrzycy do grupy chorób pośrednio związanych z pracą, potwierdzającym jednocześnie potrzebę dalszych badań i praktycznych działań ukierunkowanych na prewencję cukrzycy w miejscu pracy, są także dane z piśmiennictwa, które wskazują na skuteczność interwencji w miejscu pracy ukierunkowanej na zapobieganie cukrzycy lub pogorszenia jej przebiegu. In this article the authors attempted to analyze the arguments for considering diabetes as a work-related disease. An overview of literature has been done out of articles published in the years 1980–2016, with the use of combination of key words referring to employment, workplace, and diabetes. The PubMed database was the source of data. The authors indicate that the following arguments are in favor of diabetes being recognized as a work-related disease: diabetes is not an occupational disease, it is not directly related to the work environment or the way the work is performed, but there are observed adverse effects of occupational work-related nuisances, such as night shift work, long work hours, job strain or workplace standing time, which influence its development and course. The number of publications on diabetes, including the problem of occupational work is comparable to the number of analogous publications concerning work-related diseases such as hypertension or ischemic heart disease. Moreover, some aspects of professional activity and diabetes were also included in clinical recommendations for the workplace, which is uncommon in the case of other diseases, even those generally recognized as work-related. Data from medical references, indicating the effectiveness of workplace interventions aimed at preventing diabetes development and/or worsening of its course should be considered as an argument for the inclusion of diabetes into the group of work-related diseases. This should also support the need for further research and practical actions aimed at preventing diabetes at the workplace. Med Pr 2017;68(5):667–675

Opis:
Diabetic neuropathy is one of the most common and serious complications of diabetes both type I and type II. In type I diabetes mellitus neuropathy usually assumes form of a distal symmetric polyneuropathy (DPN) and/or a diabetic autonomic neuropathy (DAN). Other forms as acute sensory neuropathy, cranial neuropathy, truncal radiculoneuropathy or proximal motor neuropathy are present sporadically in patients with type I diabetes. Damage to nerves in the abnormal environment of diabetes mellitus leads to diabetic neuropathy. There are three hypotheses that explain the pathogenetic mechanism of polyneuropathy: metabolic, vascular and immunological. Many diabetic patients have demonstrable abnormalities of autonomic function without any evidence of clinical disease. Tests of autonomic function and tests of conduction veloautocity in peripheral nerves are assumed to be a measure of neurological state and may be important methods of assessing therapy of diabetic complications. Control of hyperglycaemia is the basis of the adequate management. α-liponic acid is used due to some evidences suggesting the role of free radicals in pathogenesis diabetic neuropathy. Antiepileptics and antidepressants inhibiting selective norepinephrine and serotonin reuptake are administered for pain control in symptomatic management.
Neuropatia cukrzycowa jest jednym z najczęściej występujących poważnych powikłań w cukrzycy zarówno typu I, jak i typu II. W cukrzycy typu I zwykle występują obwodowa symetryczna polineuropatia (DPN) i/lub cukrzycowa neuropatia autonomiczna (DAN). Inne formy, takie jak ostra czuciowa neuropatia, neuropatia czaszkowa, radikulopatia tułowia, neuropatia ruchowa proksymalna, u pacjentów z cukrzycą typu I występują rzadziej. Uszkodzenie włókien w przebiegu cukrzycy prowadzi do wystąpienia neuropatii. Wysuwane są trzy hipotezy dotyczące mechanizmów prowadzących do neuropatii cukrzycowej: metaboliczna, naczyniowa i immunologiczna. U wielu chorych występują nieprawidłowości w funkcjonowaniu układu autonomicznego bez wyraźnych klinicznych objawów choroby. Wykonywanie testów czynności układu autonomicznego i testów oceniających przewodnictwo w nerwach obwodowych jest ważne dla oceny stanu neurologicznego i dla decyzji terapeutycznych w przypadku powikłań cukrzycowych. Kontrola hiperglikemii jest podstawą adekwatnego leczenia neuropatii cukrzycowej. Kwas α-liponowy jest stosowany w związku z uzasadnionymi sugestiami udziału wolnych rodników tlenowych w patogenezie neuropatii cukrzycowej. Leki przeciw-drgawkowe i antydepresyjne hamujące zwrotny wychwyt noradrenaliny lub serotoniny są stosowane dla łagodzenia dolegliwości bólowych.
