Opis:
Przedstawiono stosowane powszechnie modele wypadków. Przeanalizowano wypadki związane z operacjami przeładunkowymi w porcie szczecińskim za okres 1999 - 2000. Dokonano oceny ryzyka zawodowego w sferze przeładunków. Zaproponowano modelową kartę oceny ryzyka zawodowego na stanowiskach przeładunkowych.
Commonly used models of accidents at work have been presented. A quantitative analysis of accidents at the port of Szczecin (frequency, seriousness, accident rate) has been carried out for the years 1996 - 2000. A detailed quantitative and qualitative analysis has been carried out for accidents during cargo-handling operations. On the basis of the estimated occupational hazard, employees' safety has been assessed. A model card of occupational hazard assessment for cargo-handling places has been suggested.

The ship technical failures contribution in overall number of navigational accidents are significantly smaller than those caused by human factor but in safety analysis they cannot be neglected. The paper presents methodology of modeling the technical failures of ships with respect of most important ship systems such as main engine, power generators and steering gear. The repair time is also taken into account. The data for simulation was obtained from analysis of ships statistical data of polish owners. The model could be used mostly in assessment with projecting phase of ship appliances, simulating owner’s economical analysis or generating random events in marine simulators.

a statistical study is carried out on the basis of information of marine traffic accidents occurred from 1996 to 2005 on the Bohai Sea. The time distribution and geographic distribution of the marine traffic accidents, as well as the relationships between the accidents and weather condition, type of accidents area, accident category, size of ships are analyzed, which provide references for adopting safety decision-making and take relative precautionary measures to avoid and decrease occurrence of accidents.

Simulators are useful in very wide field of current life. Very important is to know their advantages and disadvantages. One of the most important of navigational simulator using is deck officers training. Possibilities of using real collision situations for officers training can seriously improve knowledge about deck officers behavior in dangerous situations. Analysis of the possibility of using the NMS-90 simulator for ship's collision reconstruction was the aim of experiment. Some problems of recorded data accuracy and simulator possibilities influence on accident reconstruction process are described in this paper.
Symulatory są obecnie szeroko wykorzystywane w wielu dziedzinach życia. W związku z tym bardzo istotną kwestią jest znajomość zarówno ich wad, jak i zalet. Jedną z dziedzin, w których wykorzystuje się symulatory nawigacyjne, jest szkolenie oficerów pokładowych. Wykorzystanie w trakcie symulacji sytuacji nawigacyjnych, w których doszło do kolizji statków, powinno znacząco wpłynąć na poznanie sposobu zachowania się nawigatorów, jak i przebiegu procesu podejmowania przez nich decyzji w trudnych sytuacjach nawigacyjnych. Istotą przeprowadzonego w symulatorze eksperymentu było sprawdzenie możliwości wykorzystania symulatora NMS-90 do rekonstrukcji wypadków morskich. W artykule przedstawione zostały wybrane problemy związane z wpływem dokładności zarejestrowanych danych źródłowych oraz możliwości samego symulatora, związanych z dokładnością rekonstrukcji sytuacji kolizyjnych.

Opis:
The highest priority for safe ship navigation is the avoidance of collisions and groundings. For this purpose, the concept of ship domain has been introduced to describe the surrounding effective, waters which should be kept clear of other ships and obstacles. In the last decades, a large variety of ship domains have been developed differing in the applied method of their determination as well as in the modelled shape, size, and safety areas. However, a ship domain should be adjusted in real time to enable a reliable evaluation of collision risks by the officers of the watch. Until today in discussions about modelling and utilization of ship domains, it has been mostly unnoticed that the performance of vessel’s position (P), navigation (N), and timing data (T) ultimately determines the accuracy and integrity of indicated ship domain. This paper addresses this question, and starts with a comprehensive analysis of AIS data to prove the violation of ship domains in the maritime practice. A simulation system has been developed to enable, for the first time, investigation into the extent inaccuracies in PNT data result to a faulty evaluation of collision risks. The simulation results have shown that there is a non-negligible risk of not detecting a collision, if inaccuracies of sensor data remain unnoticed.
Zapobieganiu wypadkom na morzu przyznaje się najwyższy priorytet w ramach bezpiecznego prowadzenia statku. Dla poprawy bezpieczeństwa żeglugi stworzono koncepcję domeny statku. Definiuje ona taki obszar wokół jednostki pływającej, w którym nie powinno być ani żadnych innych uczestników ruchu morskiego, ani przeszkód. Na przestrzeni minionych dziesięcioleci powstały rozmaite domeny statku różniące się między sobą sposobem ich definiowania, modelowaniem kształtu, rozmiarem czy strefami bezpieczeństwa. Istotnym warunkiem do należytej oceny ryzyka kolizji jest umożliwienie oficerowi wachtowemu dopasowania w czasie rzeczywistym domeny statku do panujących warunków. W dotychczasowych dyskusjach na temat modelowania i stosowania domeny statku przeważnie pomijano istotną zależność pomiędzy dokładnością pozycji statku (P), wektora ruchu (N) oraz znacznika czasu (T) a dokładnością i spójnością domeny statku. W niniejszym artykule poruszono kwestię powyższego związku. Na wstępie przeprowadzono dogłębną analizę danych AIS i dowiedziono się, że naruszanie domeny statku ma miejsce w praktyce żeglugowej. Ponadto opracowano system symulacyjny, który po raz pierwszy umożliwia badanie wpływu niedokładności danych PNT na błędność oceny ryzyka kolizji. Wyniki symulacji potwierdziły, że istnieje poważna możliwość niewykrycia groźnej sytuacji zbliżeniowej, jeżeli pozostanie niedostateczna dokładność czujników pokładowych niezauważona.

This paper presents an overview of ship traffic volume and accidents in the Baltic Sea with a special focus on the Gulf of Finland. The most common accidents are groundings and collisions, usually reported to be caused by human error. The annual number of Baltic Sea accidents reported to HELCOM varied from 34–54 for collisions and 30–60 for groundings. The number of yearly port calls varied from 468–505 thousand with a peak in 2008. Exact port call data could not be found for all ports and hence had to be estimated. The number of line crossingings in HELCOM AIS data was found to be a good, rough surrogate measure for the total number of port calls and could be used if more precise port call data was not available. By analyzing two separate accident databases, an estimate for accident underreporting was calculated. Different statistical methods yielded an underreporting rate in the range of 40–50%. Lastly, the true number of accidents was estimated, based on the estimated underreporting percentage for the Baltic Sea. Based on these results, the true number of true accidents should be first estimated if accident statistics are used in building or validating maritime risk models. When using such models or accidents statistics in decision-making, the underlying uncertainty in the accident statistics should be taken into account as the underreporting frequency estimates are only approximations of the real number of accidents.

Wreck of a ship is an incident that must be avoided. Ship accidents are generally caused by a several cases, such as human error, natural disaster, technical errors, missed communication, poor condition of the ship, and many more. Ship wreckage have huge impact for ship navigation, environment, economics, and others. Those impact have many disadvantages for the shipowners, and also for environment. For examples the fuel spills that pollute the environment, make disturbance to sailing ship because the track for those navigation is blocked by the ship wreck and their cargo especially on shallow location (<50 m). These research will discuss the effect the container when it is floats on the sea and its interference other ships. The main objective of this study is to present a risk assessment on the environmental impact of the wreck and container cargo. Wrecks on the seabed is likely to pose a risk to passing ships. container and its contents as well as the possibility of refloat, and also their environmental risks emanating from the wreck and container cargo, such as fuels, lubricants, and chemical cargo. Variations scenario is a collision between ships that pass by floating containers. The frequency of refloating container, and the consequences of the passing ship depends on several factors, which will be the subject of research. However, because of the frequency of refloating containers is unlikely, then the risk is low and does not pose a danger to navigation. These risk assessment using risk matrix 5x5 which is the combined value of the frequency and consequences of the incident. The results of this study indicate the level of risk, whether the risk is accepted, not accepted or received by considering the costs and benefits (ALARP). To consequence, there are two parameters which energy is absorbed and the penetration occurs. The absorbed energy is divided into two, namely the energy absorbed by ship and the energy absorbed by containers. In this study were taken 5 groups based on the size of the vessel. In this cases any 5 size group of vessels is based on the size of the ships that pass in the shipping lanes at the site of the sinking. Assumed these vessels have speed 10 knots at the location. As well as speed drifting containers having 0 to 3 knots.

It presents its specificity for this type of operation, describes selected problems that may occur during its operation and identifies the main factors initiating extraordinary situations. The sailing safety was a very important issue that had to be solved in Indonesia. There were accidents causing victims and material losses caused by ship operator’s ignorance and less awareness on safety. The ship operator was responsible for technical matters and to guide human resources. The aim was to map the correlation between ship accident causes and particular routes. The research used the Pearson Product Moment correlation trial to map correlation between accident events to particular routes. The data used was decision report of shipping court published in 2015–2016. Then, the causes were grouped and the result showed that human factor is the main factor of accident. The accident factor had the high correlation with the location where the accident happened.

A paradigm shift is presently underway in the shipping industry promising safer, greener and more efficient ship traffic. In this article, we will look at some of the accidents from conventional shipping and see if they could have been avoided with autonomous ship technology. A hypothesis of increased safety is often brought forward, and we know from various studies that the number of maritime accidents that involves what is called “human error” ranges from some 60‐90 percent. If we replace the human with automation, can we then reduce the number of accidents? On the other hand, is there a possibility for new types of accidents to appear? What about the accidents that are today averted by the crew? This paper will present a method to assess these different aspects of the risk scenarios in light of the specific capabilities and constraints of autonomous ships.

The statistics invariably show that most onboard fires originate in the engine room. In hazardous conditions, fires can spread to other rooms of the ship and cause the loss of human life, and can cause the ship to be out of service or lost completely. To prevent these serious consequences, the engine room crew should be aware of hazards and ways to prevent them. It is also advisable to support their routine activities and actions in critical situations with an appropriate management system. For this reason, a survey was conducted at the beginning of 2019 of engine room crew members employed by a European shipowner, as a contribution to an analysis of fire safety management. Based on the results of the survey, some of the elements of the fire safety management system of the ship engine room are described. A properly constructed system that is understandable and accepted by the crew is one of the most important factors in increasing fire safety on a ship. Familiarisation with adequate procedures can significantly contribute to the successful prevention of accidents. This paper also proposes a checklist based on suggestions by the crew, which may be helpful in onboard fire prevention.

The identification of navigational risks of accidents in fairways is a basic principle for the construction or modernization of waterways in restricted waters and when the conditions of safe operation change. The estimation of navigational risk requires the evaluation of the consequences of accidents occurring in fairways: grounding, impact against a vertical shore, port/offshore structure or moored ship. When determining the consequences of these accidents, it is necessary to know the emergency maneuvering area and the speed of the ship at the time of the accident. The method of ship movement simulation in real time was used to determine the parameters of emergency maneuvering areas and the speed of vessels during the maneuvers. The results of the simulation experiment include full form vessels (bulk carriers, tankers) with a capacity ranging from 5,000 DWT to 100,000 DWT. The presented results of simulation tests enabled the evaluation of probability and consequences of ships’ accidents in fairways resulting from technical failures of ships’ steering equipment, allowing to assess the navigational risk for ships sailing through different types of fairways (approach channels, port entrances and inner port fairways).

Work shortly analyses statistics of maritime casualties registered by European Maritime Safety Agency (EMSA) for years 2014-2019 and gives short description of chosen collisions that took place in last ten years. Article points out main causes of navigational accidents and the need to keep improving navigators skills and operation of systems available on board to minimalize risk of collision or grounding due to human error. In this work very small sample is presented and author mainly focuses on European waters and incidents investigated by only one maritime authority – Danish Maritime Accidents Investigation Board (DMAIB).
