Opis:
Antibiotic resistance by pathogenic microorganisms has become a major clinical and public health problem in the 21st century. Countries across the world are faced with restricted healthcare resources, high demand for modern drugs and increasing antimicrobial resistance. This escalation in antibiotic resistance is attributed to the misuse and over use of antibiotics, poor isolation practices and long hospital stay of patients who then become susceptible to nosocomial infection. The misuse and overuse of antibiotics is now generating resistance to almost all classes. However, it is pertinent to emphasize that even with the pervasive nature of antibiotic resistance worldwide; infections acquired by patients in the hospital must be treated. Thus, this review article proposes some control options and recommendations to protect us against resistant bacterial strains.

Dysfunction of T-helper 1 mediated immune responses is a hallmark of the progression of visceral leishmaniosis (VL). Several factors such as altered antigen presentation, and abnormalities in MHC/HLA, antigen processing, and T cell receptor recognition regulate the onset of immunosuppression. Recent investigations on VL patients suggest that susceptibility to visceral leishmaniosis is genetically determined and varies between populations in different geographical locations. Emerging evidence also indicates the importance of the role played by myeloid derived suppressor cells in progressive VL. This study provides a mechanistic view of means to target the signaling mechanisms of immunosuppression to determine potential therapeutic interventions.

A group of 10-day-old chickens was experimentally infected with a dose of 500 infective eggs of Ascaridia galli (Schrank, 1788) (Nematoda). Forty-nine days post infection the chickens were necropsied and the infection intensily and prevalence were determined. The group maintained on the feed containing 11% of protein showed higher infection intensity and prevalence compared to the chickens fed a diet containing 19% of protein. The addition of vitamin B₂ to the feed containing 11% protein resulted in lower intensity and prevalence of the infection. In both series of experiments no effect of Zn-bacitracin or vitamin B₂ on infection intensity or prevalence was observed for chickens fed feed containing 19% of protein.

Surgical site infection is a common complication in surgery, which increases treatment cost, extends hospitalization time and can lead to septic complications. The aim of the study was analysis of postoperative infections in own material and finding significant risk factors with preserving the obligatory procedures in the clinic. Material and methods. Prospective analysis of 270 consecutively operated patients aged from 18 to 101 was performed with observation of early infection until 7th day postoperatively. Factors judged included: age, sex, BMI, operation type: elective or urgent, physical preparation for surgery, antibiotic prophylaxis, length and type of surgery. Wound observation card was used. Data were analysed statistically (t-Student’s test, chi2 test, U Mann Whitney test and logistic regression analysis). Results. Wound infection was observed in 33 patients (12.22% of the entire group). In 24 (8.88%) it was a superficial infection and in 9 (3.33%) deep infection. Statistically significant risk factors were age, presence of diabetes, operation time and operations on large bowel. The average age of patients with present infection was 61.2. In the group without infection there were 6,3% patients with diabetes and 20.8% in the group with infection. In our study diabetes increased the risk of infection four times. The longer the operation time the higher was the risk of deep infection (without complications 76.2 minutes, superficial 94.9 minutes, deep 148.9 minutes). Operations on large bowel were performed in 11.9%of all study patients. In the group of 33 patients with surgical wound infection, 39.4% had colon surgery, 39.4% of all deep infections and 29.2% of all superficial infections. Conclusions. In own study material significant risk factors of surgical wound infection were: age, presence of diabetes, length of operation, large bowel surgery. In preoperative course risk factors should be identified to perform certain prophylactic procedures to lower the risk of infectious complications.

Viral hepatitis is caused by viruses, the most important of which are hepatitis A virus (HAV), hepatitis B virus (HBV) and hepatitis C virus (HCV). New discoveries and new therapeutic options result in better and better results of treatment. However, the public's awareness of the occurrence of these diseases and the detection of infections as soon as possible is still important, what is associated with better outcomes. In order to assess the basic knowledge about viral hepatitis among Polish society, an online survey spread throughout Poland and covering mainly young people was created. 2205 responses were collected and analyzed. On the basis of the results obtained, it can be concluded that Polish youth have little knowledge about viral hepatitis, what is associated with less awareness of the possibility of infection.

Objectives: To evaluate the level of knowledge of, to investigate the attitudes toward, and to determine the emphasis given to the national prevalence of HBV/HCV infections among healthcare professionals. Materials and Methods: A total of 206 healthcare professionals (mean (SD) age: 37.0 (6.3) years; 86.9% – females) including medical laboratory technicians (N = 54) and nurses (N = 152) employed in the Antalya Training and Research Hospital, Antalya, Turkey. Laboratory (N = 53), operating room (N = 41) and in-patient clinic (N = 112) staff were included in this descriptive study. A 33-questionnaire composed of questions related to their level of knowledge and attitudes toward HBV/HCV infections, the sources of their knowledge of HBV/HCV infections and the emphasis given to the national and global importance of the diseases was administered via a face–to-face interview method with each subject; participation was volunteer based. Results: The participants working in the in-patient clinic (18.0 (3.2)) had the highest mean (SD) knowledge level compared to the laboratory (16.4 (3.1), p < 0.05) and operating room (17.0 (2.8), p < 0.05) staff. The participants from the in-patient clinic (44.6%) had a more advanced level of knowledge compared to the participants working in the laboratory (27.8%, p < 0.05) and the operating room (30.0%, p < 0.05). Most of the subjects (60.7%) had education concerning HBV/HCV infections in the past. There was no signifi cant difference between the hospital units in terms of the attitudes of healthcare workers (HCWs) toward HBV/HCV infections and the level of education concerning them. Conclusions: Our fi ndings revealed a moderate level of knowledge in most HCWs, regardless of their exposure to risk. While the highest knowledge scores and vaccination rates were noted among the in-patient clinic staff, there was no signifi cant difference between the hospital units in terms of the attitudes of HCWs towards a patient or a colleague with an HBV/HCV infection.

Invasive fungal infections (IFI) are devastating and life-threatening infections affecting especially immunocompromised patients. We report on a case of a 14-year-old boy with myelodysplastic syndrome (MDS), after haploidentical hematopoietic stem cell transplantation, in whom invasive pulmonary aspergillosis (IA) was diagnosed and successfully treated with subsequent (despite the secondary antifungal prophylaxis) IA or breakthrough infection development. Thanks to intensive and broad spectrum antifungal treatment (voriconazole upfront therapy, followed by a combination of voriconazole and micafungin, and eventually by the amphotericin B lipid complex), significant improvement was accomplished.

Ruminants often live in environments where the natural balance has been disturbed by humans. As a result, there is a transfer of parasitosis to domestic animals and sometimes humans. The aim of the study was to determine the annual species composition and level of gastrointestinal nematode infection of mouflon (Ovis aries musimon) from the half-open breeding colonizing areas of the Goleniowska Forest, and to assess the effectiveness of deworming. The course of the parasitic infection of mouflon depends largely on geoclimatic and breeding factors. These diseases are caused by more than 17 species of nematodes with varying degrees of pathogenicity. Due to the high degree of parasitic infections in deer, both in Poland and abroad, it is necessary to develop specific preventive programs using preparations with a broad spectrum of action.

Purpose: Respiratory tract infections and severe allergy reactions are a leading cause of hospitalization and morbidity in infants and children. The protection of breastfeeding against infectious diseases as well as allergy development has often been suggested. The aim of the study was to assess the relationship between the various models of infant nutrition, its immunity influence and children response after 3-6 years of age. Materials and methods: The research was based on a voluntarily questionnaire, filled in by parents of pre-school children. Information on breast and milk formula feeding duration (never; 1-6 months; 6-12 months; 12-24 months; 24>months) were collected. The frequency of infections, chronic diseases, and allergies occurrence was analysed. Results: A statistically significant differences between infants breastfeeding and milk formula feeding in case of reduced infection (p=0.003) and infection recurrences (p=0.001) were found. This relationship was not found among children at further stages of development. There was no correlation between the consumption of only mothers’ milk, and the reduced occurrence of asthma, allergies, and eczema. Conclusions: Maternal milk has an influence on the reduced risk of infection in the first year of the child’s life compared to children fed only with artificial mixtures

GV can be used to treat uncomplicated bacterial and/or yeast infections, support antibiotic therapy of more severe infections, but also to protect medical equipment against colonization by microorganisms. In the light of recent studies, there are many new possibilities for GV application. It has been shown to be effective in the treatment of viral infections, some chronic skin diseases and oncology. GV can induce apoptosis of tumor cells among others by elevating caspase 8, inhibiting NADPH oxidases, decreasing mitochondrial thioredoxin 2 or inhibiting STAT3/SOX2 axis. Preclinical and in vitro studies have also demonstrated GV efficacy in the treatment of breast cancer, melanoma tumors and cutaneous T-cell lymphoma. There is no unambiguous evidence indicating the toxicity of GV, whereas its safety has been proven by its long history of use, its inclusion in numerous guidelines and its legal trade and distribution with no specific approval requested in many countries around the world. The article gathers the available knowledge about GV and its potential use in the future.

Inflammation is a non-specific immune response to infection, irritation or other injury, the key features being redness, warmth, swelling and pain. A number of mediators are released which alter the resistance of mucosa to injury induced by noxious substances. Oxidative stress is a unifying mechanism of injury in many types of disease processes, including gastrointestinal diseases. It has been defined as an imbalance in the activity of pro and antioxidants. Pro-oxidants favour free radical formation while antioxidants inhibit or retard the same. A number of markers of oxidative stress have been identified. This review provides an overview of various mediators of inflammation and oxidative stress, and diverse approaches for prevention and treatment of gastrointestinal inflammation.

Opis:
The Epstein-Barr virus (EBV) was discovered in 1964 by Michael Anthony Epstein and Yvonne Barr, who discovered a herpesvirus-like infectious agent in a biopsy specimen from a patient with Burkitt’s lymphoma. This virus belongs to the Herpesviridae family (subfamily Gammaherpesvirinae, genus Lymphocryptovirus). EBV is a ubiquitous herpesvirus that is causally associated with various malignant tumours. According to the current nomenclature, it was named human herpesvirus type 4 (human herpesvirus 4 – HHV-4). Primary infection usually occurs in childhood. In developing countries, the infection rate among young children is higher than in developed countries. It was the first human tumour virus and it is currently categorized as a group-1 carcinogen due to its association with various cancers. It is estimated that over 90% of the adult population has been infected with this pathogen, but only a minority will develop the disease. EBV establishes latent infection characterized by the expression of a limited number of viral genes called latent genes. Moreover, during its life cycle, EBV periodically reactivates and can be transmitted to other susceptible hosts. The oral cavity is the main site of EBV occurrence and the most common source of infection. This study discusses EBV frequency and its association with the occurrence of malignant tumours and the pathways of tumour progression.

The objective of the study was the evaluation of the frequency of occurrence of acute respiratory infections among rural population from the Lublin Region, and verifi cation of the appropriateness of the use of antibiotics in empirical fi rst-line therapy of these diseases. The analysis covered medical records concerning 1,839 patients selected from 27 rural health centres within the period of one year (1 September 2005–31 August 2006). As many as 656 patient visits were recorded, the reasons for which were qualifi ed by physicians as infections of the upper and lower airways. These disorders were diagnosed in 450 patients, which constituted 24.46% of the population examined. The following diagnoses were most frequently made: acute pharyngitis and tonsillitis (31.25% of the total number of diagnoses), acute upper respiratory infections of multiple and unspecifi ed sites (22.25%), and common cold (21.64%). Antibiotics were used in empirical fi rst-line therapy in as many as 82% of the total number of patients. The analysis showed that in the great majority of cases the use of antibiotics was inappropriate. If this situation persists, in the near future it will lead to an increase in the phenomenon of antibioticoresistance in Poland. In order to prevent this situation, it is necessary to carry out an extensive educational action concerning the hazardous effect of excessive use of antibiotics, both among physicians and the whole society.

99mTc-HIG can be used for early detection of inflammation and infection foci especially in immunodeficiency patients. The present study involves in-house preparation and evaluation of human immunoglobulin-hydrazinonicotinic acid (HIG-HYNIC) kit to be labeled with 99mTc. In this indirect labeling method, the structure and biological activity of the protein can be maintained. The kit consists of two vials A and B, the vial A contains 2 mg of HIG-HYNIC, and the vial B contains 10 mg of tricine and 1 mg of stannous chloride dihydrate (SnCl2.2H2O). The results revealed that the lyophilized kits were stable for at least 10 months, and showed a high quality from the points of physical, chemical, radiochemical and biological purities, and its validity for human injection. The labeling yield of HIG-HYNIC compound exceeded 98%. The application of the prepared kit was experimentally illustrated in animal models with artificially induced inflammatory lesions after E. coli injection into rat leg muscle. The biodistribution and imaging studies in rats have shown good localization in infected muscle (T) in comparison to the normal muscle (M) 4 and 24 h after injection. Ratios of 17 and 18 were achieved 4 and 24 h post injection, respectively. The results of biodistribution of 99mTc-HIG in human and initial clinical evaluation show that this agent is a good radiopharmaceutical for infection imaging.

A number of factors are known to be involved in Candida albicans virulence, although biofilm development on the surfaces of indwelling medical devices is considered to promote superficial or systemic disease. Based on previously reported up-regulation of saccharopine and acetyllysine in biofilm cells and activation of the lysine biosynthesis/degradation pathway, we investigated the consequences of Candida albicans lysine auxotrophy on adhesion to host tissues and biofilm formation. Our data indicate that mutant strains lysΔ21/lysΔ22, defective in homocitrate synthase, and lysΔ4, defective in homoaconitase activity (the first two α-aminoadipate pathway enzymes), are able to adhere to mouse embryonic fibroblast cells (cell line NIH/3T3) to the same extent as a control strain SC5314. On the other hand, the auxotrophic mutant strains' development on mouse fibroblast monolayers was significantly reduced up to 5 h post infection. Although invasion into human-derived oral epithelial cells was unaltered, both mutant strains formed a significantly different biofilm architecture and demonstrated diminished viability during long term biofilm propagation.

During reaction to stress caused by viral infection, RNA granules are formed in order to protect mRNA. Stress granules (SG) and processing bodies (PB) provide cell homeostasis and mRNA stability. They are formed, for example, during polio virus and MRV (mammalian orthoreovirus) infections. Some viruses, such as influenza virus and HTLV-1 (Human T-lymphotropic virus 1), block the formation of granules. In addition, there are viruses like West Nile Virus, Hepatitis C Virus (HCV) or human Herpes viruses, which influence the functioning of the granules.

Introduction. The occurrence of the severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) has emerged as a global pandemic with huge death tolls. Coronavirus disease 2019 (COVID-19) may progress from minimal infection to serious respiratory failure mandating treatment for a continuum of developed disease condition. Aim. The purpose of this review is to summarize the findings related to epidemiology, clinical manifestations, modes of transmission, diagnosis and the treatment modalities (both experimental and repurposed) for COVID-19. Material and methods. Literature were searched using various search engines like PubMed, SCOPUS, EMBASE, J-Gate, Google Scholar to look for review articles, randomized controlled trial results, prospective studies and, retrospective studies done on COVID-19 for the purpose of this comprehensive review. Analysis of the literature. The transmission seems to be occurring through droplet, fomite and aerosols (rarely). Currently there is no specific/targeted vaccine available. Priority is highly placed to identify possible treatment approaches to circumvention this disease. Conclusion. Till we find a vaccine, we have to strategize to optimally use the existing evidence of the indirect effects of these various available drugs for therapy and maintain a strict protocol for prevention and we must use triage system to admit only those critically ill or having severe disease.

The COVID-19 pandemic caused changes in the operation of children’s hospitals around the world. The prevention of the spread of infections is one of the most pressing issues faced by everyone involved in medical care. The following article reminds what the Karol and Maria Hospital for Children in Warsaw opened in 1913 did to prevent nosocomial infections. At the time, this hospital was one of the most modern in Europe, and the solutions it offered were welcomed with admiration and amazement. Even if not widely used, many of them seem relevant today.
Karola i Marii w Warszawie, otwartym w 1913 roku.

Dłuższy czas pracy w więzieniu i ośrodkach penitencjarnych sprzyja zakażeniu latentnemu wśród personelu więziennego. Dotychczas opublikowano niewiele prac dotyczących zagadnienia latentnego zakażenia prątkami gruźlicy wśród pracowników zakładów karnych.Materiał i metodyPrzebadano łącznie 84 pracowników zakładów karnych zlokalizowanych w województwie lubelskim. Materiał do badań stanowiły próbki krwi pobrane metodą testu QFT-GIT.WynikiLatentne zakażenie gruźlicą stwierdzono u 16,6% funkcjonariuszy. Tuberculosis is a chronic infectious disease caused by acid-fast mycobacteria from the Mycobacterium tuberculosis complex group. The disease spreads when people with pulmonary tuberculosis excrete bacteria when they cough, sneeze, laugh or speak. Prisons are often called reservoirs of patients with tuberculosis, posing a great threat to their staff. Longer working hours in prison and penitentiary facilities aid latent contamination among the prison staff. So far, there have been few works discussing latent tuberculosis infection issues among the prisoners and prison staff.Material and MethodsA total of 84 employees of penitentiary facilities located in the Lublin Voivodship were examined using the QuantiFERON-TB Gold In-Tube (QFT-GIT) test which was approved for use in 2005 by the U.S. Food and Drug Administration as an aid in the diagnosis of Mycobacterium tuberculosis. The material for the research included blood samples collected using the QFT-GIT.ResultsPositive test results of latent tuberculosis infections were found in 16.6% of officers. The highest percentage with positive results of tuberculosis infections was found among the surveyed single women aged 36–57, working in penitentiary facilities. There have been no statistically significant differences between the position of officers and the risk factors increasing the chance of the latent tuberculosis infection. Work experience and contact with inmates were found to have an impact on tuberculosis infection.ConclusionsThe obtained results partially confirm other authors’ observations concerning the influence of the risk factors of latent tuberculosis infections in prisons. Work experience and contact with inmates have an impact on tuberculosis infections among the prison staff in the Lublin Voivodeship. Med Pr. 2021;72(4):415–22

Zoonoses are frequently associated with wild animals. Research on reptiles either living in their natural habitat or kept as pet animals has shown that these animals frequently serve as the asymptomatic hosts of bacterial zoonotic agents, including Salmonella spp. and Escherichia coli. Studies have shown the potential of reptiles to transmit these pathogens to humans and other animals. Epidemiological research on the herpetofauna of various regions has demonstrated the high potential of reptiles as a reservoir of Salmonella spp. In the present study, Salmonella spp. were not isolated or identified from the snake-eyed lizard. Out of 150 cloacal swab samples of snake-eyed lizard 25 (16.7%) E. coli were isolated and out of these 4 (2.7%) were identified to be E. coli O157:H7 by PCR. The results suggest that Ophisops elegans could be involved in the transmission of E. coli, rather than Salmonella spp. This study demonstrates for the first time that the snake-eyed lizard acts as a cloacal carrier of E. coli O157:H7 and presents data that may aid in preventing the transmission of this strain to humans.

In comparison to the control group we found a lower percentage of monocytes with costimulatory molecules (CD80 before and CD86 after lipopolysaccharide stimulation) at the time of diagnosis and of monocytes with HLA-DR molecules after remission induction. We also noted a lower percentage of monocytes with HLA-DR expression in the group with severe or therapy resistant infections. The results of our investigation suggest some defect in costimulation and antigen presentation in lymphoproliferative diseases in children.

The study presents the results of a research on the frequency of infection with Enterobius vermicularis in different urban and rural human populations, conducted by Zoology Department of Podlaska Academy in Siedlce between 1998 and 2006. Since 2001, samples from one examinee were collected seven times. On the basis of the seven tests, a table of the frequency of infection and the efficiency of its detection was presented. The study indicates the populations in which all the infected were detected earlier than after the seventh test. It also presents the results of family infection examination (there were two groups of the examined: A- only children, B - children as well as parents): out of 125 families 47 (37.6%) were infected. The author concluded that the method of taking pinworm samples should be standardized to the Graham method with the samples collected on seven consecutive days, and that whole families should be examined wherever one person is detected to be infected.

The present paper refers to the cytometric analysis of lymphocytes T (with receptor CD5+), Th (with receptor CD4+), Tc/Ts (with receptor CD8+), lymphocytes CD25+ and lymphocytes B with receptor CD19+ in rabbits experimentally infected with strains of RHD virus - Rainham, Frankfurt and Asturias, not having haemagglutinogenic capacities, which makes them unique, as haemagglutinogenic capacity is a classic and typical property of most strains of this virus. The study was performed in the dynamic system, drawing blood samples from animals at hour 0, namely before the administration of the viral antigen, and then at 4, 8, 12, 24 and 36 h after the infection. The study indicated that Rainham and Asturias strains of RHD virus cause a similar amount of changes as the most immunogenic haemagglutinogenic strains CAMP V-561 and CAMP V-562 of the RHD virus do. In contrast, the Frankfurt strain of the RHD virus is characterised with 5-6-fold lower reactivity in this respect and is most similar to the least immunogenic haemagglutinogenic strain CAMP V-558 of the RHD virus.

Bipolaris sorokiniana is a dangerous pathogen of plants from Poaceae family. A secondary metabolite with carcinogenic properties produced by this fungus is sterigmatocystin. A field experiment with inoculation of the grain of 9 spring barley genotypes with B. sorokiniana No. 36 was carried out in the years 2012–2014 in the Zamość region. Field observations revealed leaf spot caused by B. sorokiniana in all studied genotypes. In 2012, values of the leaf infection index varied from 21.88 (Oberek) to 48.12 (Hajduczek), in 2013 from 25.31 (Skald, Oberek) to 50.00 (STH 7910) and in 2014 from 21.88 (Oberek) to 50.00 (Hajduczek). In the experimental combination with B. sorokiniana, colonies of this fungus in the years 2012–2014 accounted for: 81.22%, 93.11% and 71.78%, respectively, and in control combination: 40.06%, 32.26% and 33.72%, respectively. The chemical analysis of grain of 9 barley genotypes obtained from plants in an experimental combination with B. sorokiniana in 2014, revealed the presence of sterigmatocystin in the genotypes: Hajduczek, Kormoran, Stratus and STH 7910. The sterigmatocystin concentration ranged from 5.39 ng·g–1 (STH 7910) to 67.05 ng·g–1 (Hajduczek). A statistically significant correlation was found between the value of the leaf infection index and the concentration of sterigmatocystin in the grain.

Opis:
Vibrio cholerae, the causative agent of Asiatic cholera, is a gram-negative motile bacterial species acquired via oral ingestion of contaminated food or water sources. Cholera has spread from the Indian subcontinent where it is endemic to involve nearly the whole world seven times during the past 185 years. V. cholerae serogroup O1, biotype El Tor, has moved from Asia to cause pandemic disease in Africa and South America during the past 35 years. A new serogroup, O139, appeared in south Asia in 1992, has become endemic there, and threatens to start the next pandemic. The facultative human pathogen V. cholerae represents a paradigm that evolved from environmental non-pathogenic strains by acquisition of virulence genes. The major virulence factors of V. cholerae, cholera toxin (CTX) encoded by the ctxAB genes residing in the genome of filamentous lysogenic bacteriophage (CTXɸ) and toxin coregulated pilus (TCP) encoded by vibrio pathogenicity island (VPI). CTX, a potent stimulator of adenylate cyclase, causes the intestine to secrete watery fluid rich in sodium, bicarbonate, and potassium, in volumes far exceeding the intestinal absorptive capacity, by ADP-ribosylation of the alpha subunit of the GTP-binding protein. Thus intestinal infection with V. cholerae results in the loss of large volumes of watery stool, leading to severe and rapidly progressing dehydration and shock. Without adequate and appropriate rehydration therapy, severe cholera kills about half of affected individuals. Today, cholera still remains a burden mainly for underdeveloped countries, which cannot afford to establish or to maintain necessary hygienic and medical facilities. During the last three decades, intensive research has been undertaken to unravel the virulence properties and to study the epidemiology of this significant human pathogen. This review provides an overview of the role of CTX in the occurrence of this disease in humans.

In this paper the authors would like to present a correct procedure in both surgical and hyperbaric treatment of patients with gas gangrene admitted to a surgical department during ER. Gas gangrene is not very common these days, but when it comes to dealing with gangrenous infection in the emergency it is quite likely to make errors in both diagnostic and therapeutic manners. When there is a gas gangrene in a patient at the emergency time plays crucial role and the proper application of procedures is vital for the patient’s survival. 10 cases made the study group here, all of them were patients diagnosed and treated surgically due to gas gangrene. As shown here, It is important to perform a revision of surgical wounds after few hours since primary surgery and to begin hyperbaric treatment as quickly as possible. The findings and suggestions included in this study are supported by own experience of The 2nd Department of General and Gastrointestinal Surgery of Medical University in Białystok, Poland.

This study aims to present results regarding the presence and identification of bacterial strains found in bile and gallstones located in the gallbladder and bile ducts in patients operated on due to cholelithiasis. Materials and Methods. Bacterial culture was evaluated in 92 patients. There were 54 women (59%) and 38 men (41%) who underwent surgery on account of cholelithiasis and /or gallstones in bile ducts between 2013 and 2014. Bile and gallstone samples were cultured intraoperatively for bacteria; bacterial strains were identified, and their sensitivity to antibiotics was determined. Molecular methods (NGS and Sanger method) were used to separate bacterial strains in one of the gallbladder stones and the results were compared with bacterial strains grown from the bile. Results. Bile cultures were positive in 46 patients that is, 50% of the study group. The following bacteria strains were grown: Enterococcus spp. (44%), Escherichia coli (37%) and Klebsiella spp. (35%). Candidiasis accompanied by bacterial infection was detected in 7 patients (15%). Molecular testing of gallstones revealed DNA of Enterococcus spp., Escherichia spp., Streptococcus spp. and Clostridium spp. In the bile culture of the same patient Enterococcus spp. (avium and faecalis) was detected. Conclusion 1. More than one pathogen was grown on samples obtained from 31 patients (70%) with bile infection. 2. The most common pathogens include Enterococcus spp., Escherichia coli and Klebsiella spp. 3. Bacterial infections are often accompanied by a fungal infection (Candida albicans) 4. Bacterial strains grown from a gallstone sample partially corresponded with strains identified in the bile of the same patient.

Despite development of combustiology, infections continue to be the most important cause of death among patients with burns. Sepsis is the most severe clinical presentation of infection in patients after thermal injuries who require immediate treatment. Early diagnosis and proper treatment of sepsis are important in the clinical management that is often hampered for multiple reasons, e.g. impaired patient immunity, problems with microorganisms with multi-antibacterial drug resistance. The aim of the study was to assess effect of type of a microorganism isolated from blood and wound on results of treatment of sepsis in patients with burns.Material and methods. Effect of type of microorganisms isolated from blood and wound on the result of treatment of sepsis was studied in 338 patients hospitalized immediately after an injury in Centre for Burn Treatment in Siemianowice Śląskie in years 2003 - 2004 (at the age of 18 - 96 years, 66 women and 272 men). Clinical symptoms of generalized infection were found in all study subjects. The study group was divided into two subgroups: cured patients and patients who died of sepsis. The following parameters were assessed in both subgroups: type of microorganism isolated from blood, type of microorganism isolated from wound as well as occurrence of the same and different infections of blood and burn wound.Results. positive blood cultures were found in 165 patients (48.8%), 106 (64.2%) were cured, 59 (35.8%) died. The most commonly isolated microorganisms in cured patients were Gram(+) Staphylococcus epidermidis MRSE (19.81%) and Staphylococcus aureus MRSA (18.87%). Gram(-) intestinal rods were least commonly isolated from this group. The most commonly isolated microorganisms from blood of patients who were to die, included non-fermenting Gram(-) rods Acinetobacter baumannii (35.59%) and Pseudomonas aeruginosa (22.03%). Mixed bacterial flora was found in the blood of 22.03% patients. Among patients who were to die, the same microorganisms were found in the blood and in the wound in 32.2% of patients, while this rate was 17.92 in cured patients. The most commonly found bacteria in the blood and burn wound in the cured patients included Staphylococcus aureus MRSA (31.58%) and Staphylococcus aureus (21.05%). In the group of patients who were to die, the most common bacteria isolated simultaneously from the blood and burn wound included Acinetobacter baumannii (47.37%) and Pseudomonas aeruginosa (36.84%).Conclusions. 1. The patients with thermal injuries are at higher risk of death in the event of sepsis caused by Gram(-) bacteria versus Gram(+) bacteria. 2. Infection of blood and burn wound caused by the same bacteria Pseudomonas aeruginosa and Acinetobacter baumanni increases the risk of death due to sepsis in patients with burns following thermal injuries.

Opis:
Towards the end of June of 1997 a massive outbreak of blue mold caused by Peronospora tabacina on the experiment plots of the Institute of Soil Science and Plant Cultivation gave an opportunity to observe and record the response to that disease of 192 different N. tabacum genotypes held at the institute’s tobacco germplasm collection. Both the date of the onset of the first symptoms and the intensity of the symptoms varied. Symptoms were from very severe (e. g. Virgin Mutant A, Burley White) to severe, moderate, mild and none. Less than 10% of the accessions scored for blue mold were symptom-free. They belonged to the flue-cured type (15 out of 71) and dark air-cured (5 out of 49). All the remaining entries including orientals and semi-orientals, burleys and cigar tobaccos were affected to some extent. Except the very susceptible Virgin A Mutant systemic symptoms were rare probably because the fungus attacked relatively late in the season when the majority of accessions had already grown out of the most vulnerable stage and were at either budding or flowering.

Maize is the host for a large number of pathogens, which invade all of its organs from the germination until being harvest, ear and grain infection often persistent even during storage. Diseases, through their symptoms reduce significantly the quantity and the quality of yield, estimated between 7-17% but, in the favorable years for this disease, they can be much greater. Fusarium diseases reduce yield value and quality by massive accumulation of Fusarium mycelium biomass (about 85%) on grain and ears and by mycotoxin contamination such as deoxynivalenol (DON), zearalenone (ZEA) and fumonisins (FUM). In this paper are presented aspects regarding the reaction of some maize hybrids under Fusarium  spp. natural and artificial infections; the effect of Fusarium  ear infection on yield, grain chemical composition, and mycotoxin content; the correlation between ear rot disease degree and yield ability, starch, protein and fat content. ANOVA evidenced the significant influence of experimental factors: infection conditions with Fusarium spp., maize genotypes, and their interaction on expression of the disease degree, yield capacity, protein, starch, fat and DON content. Average yield losses ranged between 7,0-9,3% during the experimental period. The hybrids Turda Star and Turda Favorit were more resistant to Fusarium ear rot, and Turda 165 was the most susceptible one. The artificial infection of ear with Fusarium spp. determined significantly decrease of starch and fat content and increases the protein and DON content for the most part of maize hybrids. Between rot diseased kernels and DON content a positive correlation was determined.

Objectives Health care workers, including dentists, are at the front line for acquiring blood-borne virus infections. This study aimed to investigate the prevalence of hepatitis B, hepatitis C and human immunodeficiency viruses among Iranian dentists. Material and Methods The survey included 1628 dental health care workers who attended the 51st annual Congress of the Iranian Dental Association. Data on the risk of blood-borne virus transmission during health care and infection control practices were gathered from self-administered questionnaires. Sera were screened serologically by enzyme-linked immunosorbent assay (ELISA). The positive samples were examined by polymerase chain reaction (PRC) followed by direct sequencing. Results Six (0.36%) and 81 (5.0%) were positive for hepatitis B surface antigen (HBsAg) and (anti-hepatitis B virus core antigen (anti-HBc), respectively. Only 1 (0.061%) was positive for anti-hepatitis C virus (anti-HCV). No case was positive for anti-human immunodeficiency virus (anti-HIV). One case was diagnosed as being occult hepatitis B virus (HBV) infection. One thousand five hundred thirty-five (94.3%) of participants had received at least 1 dose of HBV vaccine. One thousand three hundred fifty-nine (88.5%) contained hepatitis B surface antibody (anti-HBs) > 10 IU/ml, of whom 55 (4.0%) were anti-HBc positive, suggesting that they had been infected with HBV in the past. Anti-HBc positive cases had past histories of hepatitis, either their own or their spouses’. Individuals with inadequate anti-HBs levels (< 10 IU/ml) were significantly more prevalent among anti-HBc positive cases (p < 0.001). Conclusions The prevalence of blood-borne viruses among dental HCWs found in this study was lower than past reports from Iranian dentists and general population. The implementation of HBV vaccination together with improvement in infection control procedures has reduced the potential for risk infection among Iranian dentists.

Opis:
Helicobacter pylori is a Gram-negative, spiral bacterium capable of colonizing the gastric mucosa. Infections caused by this microorganism often lead to the development of various gastrointestinal complaints. Simultaneous human colonization by H. pylori and intestinal parasites is a common phenomenon. Moreover, the two groups of pathogens share the similar predisposing factors. The presence of parasites together with H. pylori can significantly influence the modulation of the host immune response. During H. pylori infection, strong polarization of Th1 cells is observed. The presence of protozoa, also contributing to the recruitment of Th1 cells, may well aggravate this response and exacerbate gastric mucosal damage. In contrast, intestinal helminth infection is associated with the polarization of lymphocytes towards Th2; their presence enhances the regenerative processes within the digestive tract and lowers the host overresponse. A literature review suggests that co-infection with intestinal helminths may serve as a buffering mechanism against the effects of H. pylori and/or protozoan infection, alleviating the Th1-dependent response and protecting against inflammations within the gastrointestinal tract.

Purpose: The role of Helicobacter pylori (Hp) in the development of gastroesophageal reflux disease (GERD) remains disputable. This study was planned to determine clinical-epidemic correlations between reflux esophagitis (RE) and Hp in older children. Materials and methods: 308 children with RE and 418 patients with chronic gastritis (CG) without RE between the ages of 12 and 18 were examined. Hp was diagnosed by histological (Giemsa and hematoxylin-eosin stain, with evaluation of dissemination grade) and rapid urease test. Subjective symptoms (heartburn, abdominal pain, other dyspeptic complaints) were analyzed by questionnaire. Results: Hp infection was found in 44.5% of children with RE (no difference in patients without RE), and it does not increase the erosive esophageal defects risk. Clinical symptoms are not connected to this infection in examined patients. The frequency of Hp infection decreases with the disease duration increase. Conclusion: The development of RE is not connected with Hp, but the disease course has certain peculiarities in the conditions of Hp infection.

Purpose: Central line care bundle comprises a few evidence-based interventions for improving patients' outcomes and recovery process. This semi-experimental study aimed to determine the effect of pediatric central line care bundle implementation on central line-associated bloodstream infections (CLABSI) rates. Materials and methods: A central line care bundle was implemented for pediatric surgery patients (n=70). Baseline observations were made to determine the central line care bundle compliance of healthcare professionals for 435 catheter days. Subsequently, physicians and nurses were educated about the central line care bundle. After the implementation period, 722 catheter days were observed to determine post-implementation compliance. Baseline CLABSI rates were compared with post-implementation CLABSI rates. Results: It was found that the entire central line care bundle compliance was 32.4% pre-implementation and 86.3% post-implementation. After education, the physicians' and nurses' central line care bundle compliance showed statistically significant improvement (p= 0.0001). There were five CLABSI events in the pre-implementation period and three CLABSI events in the post-implementation period. It was determined that the number of CLABSI decreased in the post-implementation period compared to the pre-implementation period, but this difference was not statistically significant (p= 0.207). Conclusions: Central line care bundle implementation decreased the CLABSI rates. It is recommended the Implementation of a central line care bundle on the care of pediatric surgery patients with the central venous catheter.

INTRODUCTION. Clostridium difficile associated colitis became over last years a worldwide medical issue. It involves patients of the Polish hospitals too. The aim of the study was the analysis of CDAD incidence and the course of infection in Gastroenterology Ward of Regional Specialist Hospital of Zgierz, 2012-2015. MATERIAL AND METHODS. Retrospective analysis of the medical documentation of 79 patients with CDAD was performed. Demographic and epidemiological data and the clinical course of infection were analyzed. RESULTS. The study group comprised of women in 59,5% and men in 40,5%. The patients’ average age was 70,5 years. The average hospitalization period was 10,3 days. CDAD infection seasonality was proved, with statistically significant peak in springtime. 73% of patients were previously hospitalized and 85% - had co-morbidities. 76% of patients underwent antibiotherapy, whilst 29% - used PPI prior to CDAD diagnosis. 50,6% of patients had severe CDAD diagnosed. The recurrence reached 14%. In 19% of patients CDAD resulted in death. DISCUSSION. The results of the study confirm increase of the incidence of the patients with CDAD in Gastroenterology Ward over 4-years’ observation. The prevalence was higher among 65+ patients, after prior hospitalization and antibiotherapy. The co-morbidities was a significant risk factor, especially common in severe cases. CONCLUSION. The results we obtained confirm substantial importance of Clostridium difficile infection leading to antibiotherapy-associated diarrhea among adults, causing prolonged hospitalization, increased prevalence and mortality of patients. Getting to know and minimizing the risk factors will prevent the future outbreak of the disease.

Respiratory infections are a major health and economic problems in every country. Most infections are viral, bacterial in-fection but also are not rare. It should be emphasized that the respiratory tract infections are the cause of 20% of all med-ical consultations and 75% antibiotic treatment. To reduce the need for antibiotics, more and more attention is paid to the preparation increases the body’s resistance to bacterial infection. Non-specific multivalent vaccines containing killed at high temperature, bacterial cells or lysates. These preparations are non-specifically stimulate the overall immunity. They operate on the non-specific defense mechanisms, leading to increased antibody type IgA in secretions of the mu-cous membranes, phagocytic activity and IFN-γ. They may also stimulate the production of specific antibodies against bacterial antigens included in the formulation. Available on the Polish market oral preparations immunomodulatory of the best documented clinical efficacy are Luivac, Ribomunyl, Broncho-Vaxom and Ismigen. All lysates of bacterial strains are most often causing respiratory tract infections. Many clinical studies have shown that oral bacterial lysates reduce the risk of recurrent respiratory tract infections in adults and children and reduce the need for antibiotics.

Out of 305 tick samples, 45 were (14.8%) DFM positive. The following polymerase chain reaction (PCR) then revealed 37 (12.1%) samples positive for the presence of Borrelia burgdorferi sensu lato DNA. All 37 samples were further analysed by restriction fragment length polymorphism (RFLP) method. PCR-RFLP analysis revealed 14 strains of B. afzelii (37.8%), 15 strains of B. garinii (40.5%) and 2 strains of B. burgdorferi sensu stricto (5.4%). Four samples (10.8%) showed a mixed population of these genospecies. Two samples produced atypical RFLP pattern which were detected by sequence analysis as B. valaisiana (5.4%). Isolation attempts resulted in 21 spirochaetal strains (including two stains of B. valaisiana). The results show the diversity of B. burgdorferi s.l. in tick population and refer the risk of infection by pathogenic borreliae in Brno.

Opis:
Zinnia elegans is an annual plant recommended for cultivation for cut flowers as well as flowerbeds. This ornamental plant can be infected by: viruses, bacteria, phytoplasms and by a number of fungi species. Presented studies were conducted in 2006 in three localities of the Lublin region: in the vicinity of Zamość, near Krasnystaw and near Opole Lubelskie. Those studies included three cultivars of Zinnia elegans Jacq.: Golden Dawn, Lawa, Scarlet Flame as well as the mixed material of the cultivars belonging to the dahlia group. Six weeks after the sowing, the seedling’s healthiness was assessed. The proportion of seedlings with disease symptoms ranged from 7% to 44%. The main cause of root infection at the seedling stage proved to be the species of F. equiseti and S. sclerotiorum. Considerable amounts of B. cinerea and F. culmorum and A. alternata were also detected from diseased seedlings. Studies on the susceptibility of analyzed cultivars and the mixed material of elegans zinnia to infection by B. cinerea and F. avenaceum F. culmorum and F. equiseti were conducted in a growth chamber. Results obtained from this experiment confirmed considerable harmfulness of the species B. cinera and F. equiseti towards the seedlings of elegans zinnia, which can be reduced introducing less susceptible varieties to the cultivation. However, none of the studied varieties of elegans zinnia, only its mixed material, showed such properties.
Prezentowane badania przeprowadzono w 2006 r. w trzech lokalizacjach woj. lubelskiego: w okolicach Zamościa, w okolicach Krasnegostawu i w okolicach Opola Lubelskiego.

Human gamma globulin can be labeled by a direct or indirect method of radiotracer incorporated in a protein molecule. In this indirect method hydrazinonicotinic acid (HYNIC) is used which saves the structure and biological activity of the protein. Our goal was the efficient labeling of the human gamma globulin and evaluation of its biodistribution in different organs which can be used on experimentally induced infection causing inflammation. Immune globulin is mixed with s-hynic and IgG-hynic is developed using sidle A-lyzer and stored at –20°C which can be used at least for six months and then Sn-tricine kit is prepared which is used for 99mTc labeling. Efficiency of 99mTc-IgG-hynic labeling at pH 6.4 was very much dependent on ligand (hynic) and coligand (tricine) presence in the reaction mixture. Radiochemical purity was more than 90% in the kits prepared. Serum stability study showed no decomposition of 99mTc from the complex. The biodistribution studies showed the highest percentage ID/organ in the blood, liver and kidney, respectively. A human gamma globulin was successfully labeled through hynic to 99mTc by an indirect method with high radiochemical purity.

The aim of the study was to evaluate serum beta 2-microglobulin (beta-2m) and neopterin (NPT) in in patients with giardiasis. Twenty-two patients with giardiasis were examined and compared with twelve healthy subjects as a control group. Serum beta-2m and NPT concentration were determined twice: at the moment of diagnosis of giardiasis and six months after antiparasitic treatment with metronidazole. It was shown that serum beta-2m concentration in patients with giardiasis was remarkably elevated. It decreased significantly, but six months after treatment it was still higher as compared to the control group. However, serum NPT before antiparasitic treatment was slightly lower than in the control group, but after elimination of Giardia an increase of NPT concentration above control values was observed. It is concluded that Giardia infection leads to long-lasting disturbances in the immunological status of the host and may influence macrophage function and downregulate their parasiticidal effects.

SARS-CoV-2 infection
Opis:
Introduction. On 11 March 2020, the Director-General of the World Health Organization (WHO) announced COVID-19 (Coronavirus Disease 2019) as a global pandemic Currently, no vaccines are available and there is little evidence of the efficacy of potential therapeutic agents. Furthermore, there is presumably no pre-existing immunity in the population to the new coronavirus, and it is as-sumed that everyone in the population is susceptible. Objective. The aim of the procedures described in the article is to minimize the risk of human-to-human transmission of the SARS-CoV-2 (Severe acute respiratory syndrome – coronavirus 2) virus during procedures carried out in endoscopic laboratories. Brief description of the state of the art. SARS-CoV-2 infection can be asymptomatic, cause severe pneumonia, or lead to death. Symptoms of COVID-19 range from none (asymptomatic) to severe pneumonia and it can be fatal. Case studies to-date indicate that this infection causes a mild illness (i.e. pneumonia or mild pneumonia) in approximately 80% of cases, and most cases recove; 14% have a more severe illness, 6% experience a critical illness. The vast majority of the most serious illnesses and deaths have occurred among the elderly and people with other chronic underlying diseases. Average progression times include: • in mild cases: from the onset of symptoms to recovery in almost 2 weeks; • in severe cases: from the onset of symptoms to recovery in 3–6 weeks, and from symptoms to death in 2–8 weeks. Conclusions. Special precautions should be taken and procedures followed when performing invasive medical procedures in endoscopic laboratories in patients with specific or clinically probable SARS-CoV-2 infection. This article contains up-to-date information as at 04/04/2020.

Opis:
Human papillomavirus (HPV) infection is the most frequent sexually transmitted disease. According to current estimates, there are about 5 500 000 new infections diagnosed annually. Among about 100 known types of HPV, 40 are designed as genital due to their preferential affinity to vulvar, vaginal, cervical, penile and anogenital epithelium. Chronic infection with types 16, 18, 31, 33, 35, 39, 45, 51, 52, 56, 58 and 59 is associated with an elevated risk of malignant transformation, whereby the type 16 is detected in 50-55% of cases of invasive cervical cancer. Certain diagnosis of HPV infection at every stage of viral life cycle is provided by molecular virological techniques, while detection of viral DNA gains a novel significance as an adjunct to cytological study. Promising diagnostic potential in the setting of chronic HPV infections is associated with the test based on analysis of mRNA transcripts, enabling detection of active oncogenes E6 and E7 of the virus. This enables not only to confirm the presence of virus as such, but also to determine its activity gauged by initiation of transcription, i.e. the earliest phase of carcinogenesis. This is most useful the management of women infected by non-highly oncogenic HPV types. Most vulvar, vaginal and anal cancers are associated with HPV infection. Recently, the incidence of anogenital malignancies is increasing significantly. In view of continuously increasing number of HPV infections, it is reasonable to expect an increased number of anogenital malignancies and precursor lesions thereof, being diagnosed in the near future. Therefore, current epidemiological studied focus on inclusion of HPV testing (HPV DNA, HPV mRNA and early HPV genes) into large-scale screening programs for cervical cancer.
Pewne rozpoznanie zakażenia HPV w każdym stadium cyklu życiowego wirusa zapewniają molekularne metody wirusologiczne, a oznaczenie DNA HPV nabiera znaczenia jako procedura uzupełniająca badanie cytologiczne.

Malignancies were one of the earliest diagnosed manifestations that resulted in the description of acquired immunodeficiency syndrome (AIDS). Most cancers in AIDS patients are associated with coinfection with oncogenic viruses. Knowledge and public awareness about HIV infectious materials are important elements in preventing the spread of HIV infections. The aim of the study was to assess the knowledge about HIV infectious materials among three populations in Poland. A Poland-wide on-line survey was conducted and 2205 anonymous answers were analysed. Almost half of the answers came from students, of which 40% were medical students.

Most indigenous people have acquired expertise in the use of medicinal plants, but information on the utilization of the medicinal plants are poorly documented. Thus, this study deals with the ethnobotanical survey of medicinal plants used in treating viral infections in Saki West Local Government Area, Oyo State. Data was collected using structured questionnaires and oral interviews to acquire information from selected members of Saki West Local Government Area. A total of sixty (60) structured questionnaires were given randomly to respondents in the selected villages in the study area. Data were analyzed using descriptive statistics. The finding shows that the majority of the respondents were females (61.7%). Moreover, 50 % of the respondents were from the age group of 31 to 40 (also the largest group). In addition, a majority (88.3%) of the respondents were married, and only a few (16.7%) had no formal education. Furthermore, the major occupation of the respondents was herb selling (35.0%). The findings on the various plant parts utilized for herbal medicine indicated that the leaves, stem bark and whole root were the most commonly used parts. Most of the respondents also prepared herbal medicine by decoction. What is more, oral ingestion (drinking) was found to be the most (70.0%) preferred mode of administration in the study area. Finally, the major problem encountered while acquiring medicinal plants was wild animal threat. This work indicated that there is a need for an ethnobotanical survey in every state of the nation on the medicinal plants used in the treatment of viral infection.

The evaluation of Toxocara canis infection in stray dogs from two shelters and private owners dogs in the Warszawa district was the aim of this study. In 1998 five hundred faecal samples were examined. The homeless dogs were found more infected than those kept as pets. T. canis was recorded in 3.4% and 8.8% of stray dogs from the shelters and in 0.4% of animais from tlats. The higher prevalence of infection in homeless dogs was due to high density of dogs population, worse environmental condition and irregular anthelmintic treatment in the shelters when compare with housed dogs.

Opis:
Since one of the main mode of the COVID-19 transmission is through close contact with the infected person (<1.5 m distance) and body fluids (saliva, blood, aerosol particles), the dentists have become one of the groups being at most risk of being infected. The aim of this article is to present, based on the research search (publications from the last year 2020 and from the beginning of the current year 2021), the general overview of the situation during the COVID-19 pandemic with regard to patients and dentist as well as supporting personal protection. Authors identified 7 questions related to this topic. They address among others such issues as identification of the COVID-19 patients coming to the dental practice; protection of both the dentist and the patient from infection and a need for guidelines on the scope of emergency dental services during the pandemic. Opinions on the subject, expressed by the experts in the field, including general guidelines were also taken into consideration. In general, based on the literature overview, the COVID-19 pandemic has completely transformed the healthcare system, including dentistry. The identification of the COVID-19 patients coming to the dental practice has become a necessity (including taking epidemiological survey, temperature measurement). Currently, since “the group zero” in significant part has completed the full vaccination process, the health personnel (including dentists) might feel safer. The efficacy of the vaccines is high and provides a certain sense of security.

Opis:
Bacterial lysates stimulate the general immunity of the body in a non-specific way. They act on non-specific defense mechanisms, leading to an increase in type A antibody in mucous membranes, phagocytic activity and INF-ƴ production. They can also stimulate the production of specific antibodies against the bacterial antigens that make up the preparation. The oral immunomodulatory preparations with the best documented clinical efficacy available on the Polish market are Ismigen, Broncho-Vaxom, Ribomunyl and Luivac. They are all lysates of bacterial strains that most often cause respiratory tract infections. In many clinical trials, oral bacterial lysates have been shown to minimize the risk of recurrent respiratory infections in children and adults and reduce the need for antibiotics.
