Toxoplasmosis is an opportunistic infection caused by the parasite Toxoplasma gondii. The infection is severe and difficult to diagnose in patients receiving allogeneic haematopoietic stem cell transplantation (HSCT). It frequently involves the central nervous system. The case is presented of cerebral toxoplasmosis in a 17-year-old youth with Fanconi anaemia treated with haematopoietic stem cell transplantation (HSCT).

The aim of this study was to analyze pharmacotherapy cost of chronic myeloid leukemia from the society’s, the payer’s (National Health Fund), and the patient’s perspective. The study included 55 patients with a diagnosed and treated chronic myeloid leukemia at the selected hematology clinic in the city of Poznan. Retrospective study involved time horizon of one calendar year – 2013. The data required for economic evaluation were obtained from the patients’ case histories and the Department of Organization and Accounting of the selected health care facilities. The total cost of chronic myeloid leukemia pharmacotherapy for 55 patients from the society’s perspective in 2013 amounted to 1,483,416.88 EUR. Average annual cost of medication per patient in 2013 amounted to 26,971.22 EUR (Median – 32,854.22 EUR). Average cost of chronic myeloid leukemia pharmacotherapy for a patient without transplantation was 32,167.34 EUR (Median – 30,623.00 EUR), and for a patient after transplantation amounted to 413.13 EUR (Median – 378.40 EUR). The cost from the payer’s perspective is 99.93% of total costs from the society’s perspective. The cost from the patient’s perspective represents 0.07% of the total cost of chronic myeloid leukemia pharmacotherapy from the society’s perspective. Costs of chronic myeloid leukemia pharmacotherapy are very high and represent a significant burden to society. The highest costs associated with treatment of chronic myeloid leukemia are incurred by the society, and, subsequently, the public payer (NHF), and the patient.

Around 25,000 people annually worldwide are subject to the procedure of allogeneic hematopoietic stem cell transplantation (HSCT). As the number of procedures has been on the rise, post-transplant care is becoming an essential part of work at transplant centres and general practitioners’ offices. Advances in technology and supportive therapies extend long-term survival of patients after HSCT, which increases the risk of complications resulting from exposure before, during and after transplant procedures. The complications may lead to significant morbidity, deteriorated quality of life, and late mortality in patients who have received hematopoietic stem cells. Statistical analyses have shown that the survival rate of post-HSCT patients is ca. 30% lower than expected, and that infections, organ failure and secondary cancers lead to mortality in this population of patients.

Infections in immunocompromised patients after hematopoietic stem cell transplantation can have a severe and atypical course. Some opportunistic pathogens are difficult to detect in microbiological tests, and that is why treatment success depends on an accurate clinical diagnosis. This article presents a case of a 7-year-old girl with severe aplastic anemia treated with bone marrow transplantation with post-transplantation period complicated by persistent, hectic fever, with peak episodes of 39–40°C, lasting several weeks. Repeated microbiological tests failed to reveal the etiological agent, and empirical anti-infective treatment was ineffective. In the fourth week of fever, imaging showed multiple foci resembling abscesses in the patient’s internal organs and, subsequently, in soft tissues. The characteristics of these changes and data concerning environmental exposure led to the clinical diagnosis of cat scratch disease (bartonellosis) with multi-organ involvement and enabled the targeted treatment to be implemented. Fever subsided and organ lesions regressed. In this case, repeated ultrasound imaging was the basic diagnostic tool that helped arrive at a correct diagnosis and implement effective treatment of this life-threatening complication after hematopoietic stem cell transplantation.
Zakażenia w warunkach upośledzonej odporności po transplantacji komórek krwiotwórczych mogą mieć ciężki, nietypowy przebieg. Niektóre patogeny oportunistyczne są trudne w detekcji mikrobiologicznej i powodzenie leczenia zależy od właściwego rozpoznania klinicznego. W niniejszej pracy przedstawiono przypadek 7-letniej dziewczynki z ciężką anemią aplastyczną leczoną przeszczepieniem szpiku kostnego, u której w przebiegu potransplantacyjnym przez kilka tygodni występowały uporczywe, hektyczne stany gorączkowe z epizodami szczytowymi do 39–40°C. Powtarzane badania mikrobiologiczne nie wskazały czynnika etiologicznego, empiryczne leczenie przeciwzakaźne było nieskuteczne. W czwartym tygodniu trwania gorączki w badaniach obrazowych wykazano obecność mnogich ognisk o obrazie ropni w narządach wewnętrznych, następnie w tkankach miękkich. Charakterystyka zmian oraz informacja o ekspozycji środowiskowej dały podstawę do klinicznego rozpoznania narządowej postaci choroby kociego pazura (bartonellozy) i wdrożenia ukierunkowanego leczenia. Uzyskano ustąpienie gorączki i ograniczenie zmian narządowych. W omówionym przypadku powtarzane badania ultrasonograficzne były podstawowym narzędziem diagnostycznym w celu właściwego rozpoznania i zastosowania skutecznego leczenia w zagrażającym życiu powikłaniu po transplantacji krwiotwórczej.

Invasive fungal infections (IFI) are devastating and life-threatening infections affecting especially immunocompromised patients. We report on a case of a 14-year-old boy with myelodysplastic syndrome (MDS), after haploidentical hematopoietic stem cell transplantation, in whom invasive pulmonary aspergillosis (IA) was diagnosed and successfully treated with subsequent (despite the secondary antifungal prophylaxis) IA or breakthrough infection development. Thanks to intensive and broad spectrum antifungal treatment (voriconazole upfront therapy, followed by a combination of voriconazole and micafungin, and eventually by the amphotericin B lipid complex), significant improvement was accomplished.

This paper reports on diagnostic and therapeutic management of pulmonary invasive fungal disease (IFD) in a child with relapsed acute myeloid leukaemia, undergoing chemotherapy followed by haematopoietic stem cell transplantation. Surgical management with resection of the involved lung tissue was based on the location of fungal infiltrates close to large circulatory vessels. After examination of resected pulmonary tissue, a diagnosis of proven IFD was done. This case report is an example that aspergillosis is usually the cause for pulmonary IFD. Pharmacotherapy of pulmonary IFD should be based on compounds with good penetration to lung tissue: amphotericin B lipid form or voriconazole.

Pneumonia is one of the most frequent cause of death after hematopoietic stem cell transplantation (HSCT). The objective of this review is to present various aspects of pneumonia in this group of patients, with focus on invasive pulmonary aspergillosis and cytomegalovirus disease, being the most frequent etiological causes of pneumonia after HSCT. The review is aimed at practical approach to diagnostic and therapeutic management of pneumonia after HSCT with special attention to: definitions of infections and level of diagnosis of upper and lower respiratory tract infections, including issues specific for invasive fungal disease, pneumocystosis, cytomegalovirus disease, community acquired respiratory viral infections and bacterial pneumonia. Another topics analyzed in the review are: epidemiology and risk factors for development of infection and risk of death due to pneumonia; invasive and non-invasive diagnostics, including imaging and laboratory biomarkers; methods of pharmacological and environmental prophylaxis and specific targeted therapy of pneumonia after HSCT.

This is an application paper of applying standard methods of computational intelligence to identify diagnostic gene targets and to use them for a successful diagnosis of a medical problem - acute graft-versus-host disease (aGVHD). This is the major complication after allogeneic haematopoietic stem cell transplantation (HSCT) in which functional immune cells of donor, recognize the recipient as ”foreign” and mount an immunologic attack. In this paper we analyzed gene-expression profiles of 47 genes associated with allo-reactivity in 59 patients submitted to HSCT. We have applied different dimensionality reduction techniques of the variable space, combined with different classifiers to detect the aGVHD at onset of clinical signs. This is a preliminary study which utilises both computational and biological evidence for the involvement of a limited number of genes for the diagnosis of aGVHD. Directions for further studies are also outlined in this paper.

Skeletal muscles are composed of multinucleated fibers that cannot divide. They retain the ability to regenerate due to the presence of mononucleated cells, called satellite cells. Mitotically quiescent satellite cells are located between the sarcolemma and the basal lamina of the muscle fiber. They can be activated in response to muscle injury. Then they proliferate, differentiate and eventually fuse to damaged myofibers or fuse together to form new myofibers. A part of the activated cells escape differentiation and restore the pool of quiescent satellite cell under the basal lamina of the newly formed muscle fibers. A decline in the number and activity of satellite cells results in an impaired regeneration potential of aged muscle. However regeneration can be improved by modification of the microenvironment of the skeletal muscle. Satellite cells have been considered as a source for cell-based therapies in the treatment of diseases such as muscular dystrophies, heart failure, insufficient function of external urethral sphincter. They can be derived easily from skeletal muscle biopsies and cultured in vitro before cell transplantation. After injection to the host muscle they undergo a myogenic differentiation program. In the case of muscle dystrophy, clinical trials have demonstrated a lack of health improvement after transplantation. This was the result of the poor survival and limited migratory capacity of the injected cells. More promising results were obtained when satellite cells were transferred to the locally damaged muscles. This article demonstrates the role of satellite cells in skeletal muscle regeneration, and the possibilities of their use in cell-based therapies in the case of diseases where muscle fibers are impaired.
