The research results presented in this article originate from the research and educational project conducted from January to June 2017, titled Prevention of cervical carcinoma, or how it is done in Tychy. Analysis of knowledge sources and the views on HPV vaccines, which was funded with a grant by the Polish Cancer League Foundation. The research material allowed for formulating the guidelines for the educational and preventive treatment programs addressed to young people and their parents. Despite free vaccinations against the HPV virus from some local governments, the percentage of people covered by vaccinations is decreasing. What is needed in order to reverse the negative tendencies is a set of new, innovative solutions, addressed at the needs and expectations of the beneficiaries. Therefore, people should be educated on the risk of falling ill, while HPV vaccinations constitute an important element of primary prevention.

Based on NFZ published aggregate data, the public spending on cancer care in Poland was 6.3 billion PLN in 2011 (or approximately 10% of total public health spending). Poland is one of the few large countries in the world that has two centralised and public data sources for cancer, namely the National Cancer Registry (NCR, pol. Krajowy Rejestr Nowotworów – KRN) and activity expenditure database run by the National Health Fund (NFZ, pol. Narodowy Fundusz Zdrowia). We show in our article that having a population-based registry and a complete treatment/clinical care dataset is a necessary condition to have a useful cancer strategy data set that can in turn lead to evidence based health policies in the area of cancer. Lack of audited and publicly available cancer data means that a coherent cancer strategy, assessing service provision and cost effectiveness of treatments and monitoring outcomes is, in our opinion, currently not possible in Poland. We postulate that Poland should create a task force to create cancer data strategy based on NCIN (National Cancer Intelligence Network) in the UK or Cancer Australia.

Introduction: Colorectal cancer (CRC) is the third most common malignancy in men and the second most common in women. The disease constitutes a significant civilization and social problem. The aim: The aim of the study is to assess the sudy group’s awareness and knowledge about CRC, as well as about its diagnostics and treatment. Material and methods: An online questionaire form was distributed in the study group regarding issues related to CRC, and followed by statistical analysis and interpretation of the obtained survey results. Results: After analysis, we found that a significant percentage of the surveyed sample group had basic knowledge and awareness in the area of CRC, whereas about half of the respondents did not consider themselves sufficiently informed about the disease. Conclusions: Considering the scale of the problem posed by CRC, it is necessary to undertake broader action to promote knowledge about this disease and to carry out this type of research on a larger and more socioeconomically diverse population.

Overdiagnosis of thyroid cancers contributes to increased incidence of thyroid cancers worldwide, which is already a serious public health problem. A great number of medical tests, helping to detect thyroid cancer, may result in an epidemic of diagnosis. A dramatic increase in the number of detected cases of thyroid cancer may be associated with a great number of neck ultrasounds, fine- needle aspirations and incidental findings during examination. Unfortunately, in response to overdiagnosis, more and more surgeries are being performed. It is important to differentiate stationary cancers from potentially aggressive diseases. Detection of cancer contributes to saving lives. However, in some instances it can be harmful, particularly if the disease is overdiagnosed. The aim of this review is to give a balanced view of thyroid cancer epidemic and controversies arising out of overdiagnosis.

Background. It is recognised that both nutritional status and an improper diet have significant effects on weakening the outcomes of treatment in cancer patients. As a result, a lowered response to therapy and an increase in untoward side effects is often observed leading to a deteriorating quality of life. The role of an adequately balanced diet is thus regarded as being vital in supporting recovery. Objective. To assess the dietary consumption of calories, macro-elements and selected vitamins and minerals for subjects diagnosed with cancers of the breast, lungs and bones or soft tissue. Material and Methods. A survey was performed on 100 subjects diagnosed with various tumours between the September and December months of 2011 consisting of 34 with breast cancer, 33 lung cancer and 33 with bone or soft tissue cancer. The questionnaire was devised in-house, which included a three day dietary record. Results. The average daily calorific intake was found to be inadequate at 1608 kcal. In addition, abnormal proportions of energy derived from macro-elements was seen, where the contributions made by fats and proteins were somewhat high at respectively 35.1% and 16.5%, but too low in the case of carbohydrates at 52.1%. Up to 78% subjects had insufficient protein intakes, 88% showed deficiencies in consuming carbohydrates, as were 89% for fibre, 85% vitamin C, 99% calcium, 98% magnesium and 81% for iron. Conclusions. Many dietary shortcomings were observed in the studied subjects. There is therefore a need to educate persons suffering from cancer to adopt an adequate and balanced diet as means of providing vital support for treatment to be more effective.
Wprowadzenie. Nieprawidłowy sposób żywienia oraz stan odżywienia osób z chorobą nowotworową wpływa na słabszą odpowiedź na leczenie przeciwnowotworowe, może nasilać jego objawy uboczne oraz wpływać na jakość życia. Prawidłowo zbilansowana dieta pełni wspomagającą rolę w powrocie do zdrowia. Cel pracy. Celem badań była ocena spożycia energii, makroskładników oraz wybranych witamin i składników mineralnych u osób z diagnozowaną chorobą nowotworową piersi, płuc oraz kości i tkanek miękkich. Materiał i metody. Badanie przeprowadzono w okresie od września do grudnia 2011 roku. Badaną grupę stanowiło 100 osób ze zdiagnozowaną chorobą nowotworową (34 osoby z nowotworem piersi, 33 osoby z nowotworem płuc i 33 osoby z nowotworem kości i tkanek miękkich). Badania przeprowadzono przy użyciu autorskiej ankiety oraz metody 3-dniowego bieżącego notowania. Wyniki. Spożycie energii przez osoby z chorobą nowotworową było niewystarczające i średnio wynosiło 1608 kcal/dzień. Procentowy udział poszczególnych makroskładników w dostarczaniu energii był nieprawidłowy, w przypadku tłuszczu i białka był zbyt wysoki (odpowiednio 35,1% oraz 16,5%), a w przypadku węglowodanów zbyt niski (52,1%). Aż 78% badanych nie realizowało normy na białko, w przypadku węglowodanów wartość ta wynosiła 88%, a dla błonnika 89%. Spożycie witaminy C było zbyt niskie u 85% badanych, wapnia u 99%, magnezu u 89%, a żelaza u 81% badanych. Wnioski. W ocenianych racjach pokarmowych pacjentów obserwowano liczne nieprawidłowości. Istnieje potrzeba edukacji osób z chorobą nowotworową w zakresie zasad prawidłowego żywienia i znaczenia bilansowania diety.

Background: With the development of genetic research in oncology, it has become possible to track and identify early and preclinical forms of hereditary oncological diseases, which allows timely and effective preventive and therapeutic measures in relation to relatives at risk. Aim of the study: Assessment of genetically determined neoplasms in the region and the development of organizational forms and methods for early diagnosis. Material and methods: 10,727 residents of the Belarus-Poland border region were examined. Clinical and medical history data of 2,054 patients with tumors of the breast (1406), ovaries (239), and colon (409) were analyzed. As a result of the questionnaire, three main observation groups were formed: “high risk of hereditary cancer”, “hereditary cancer suspected”, and “no risk of hereditary cancer”. Results: Register and hospital screenings were the most informative types of screening. Of the 149 HBC patients who underwent molecular genetic testing, BRCA1 gene mutations were found in 5.37%, 5382insC in all cases. Seven mutations were detected in 77 individuals with a diagnosis of HOC and in 6 cases 5382insC and in 2 – 4145delA. Signs of hereditary ovarian cancer and suspicion of it were found in 1.12%, including people who were found to have a high risk of hereditary ovarian cancer. By their effectiveness, register and hospital screenings significantly exceeded the population, p<0.01. 1.67% of women suffering from this disease met the high clinical risk criteria for hereditary ovarian cancer. A high clinical risk of hereditary tumor genesis was established in 0.73% of cases among patients with a diagnosis of colon cancer. Conclusions: The results of assessing the clinical risk of hereditary cancer according to population screening indicates that approximately 1.2% of the population has an increased clinical risk of developing hereditary breast, ovarian, and colon cancer.

Breast cancer is the most common cancer among females worldwide. The mean age of breast cancer patients is > 60 yrs old, and it is seldom found in women < 40 yrs old (6.5%) and in very young women < 35 yrs old (0.6%). In young females, fertility and all its aspects are an additional therapeutic challenge. Before initiating treatment, the oncologist should offer effective contraception to be applied throughout the therapy, bearing in mind that fertility preservation is of utmost importance. When it comes to breast cancer in pregnancy, the attending physician should use a therapy which is safe for both the mother and the foetus. Chemotherapy, radiotherapy, hormonal therapy and immunotherapy can, to a lesser or greater degree, damage the ovarian function resulting in amenorrhea in women < 50 yrs (33–76%). However, owing to fertility preservation strategies, more and more pregnancies are successful, even in breast cancer survivors.

The study presented a patient with asymptomatic gastric cancer, in whom the first symptom was metastasis to the brain. The patient was initially diagnosed by a neurologist and subject to surgical intervention in the area of residence, where he underwent craniotomy with the excision of the metastatic lesions located in the occipital lobe. The histopathological examination revealed the presence of adenocarcinoma metastases. Following complex diagnostics the patient was diagnosed with cardial carcinoma, being subject to cerebral radiotherapy and chemotherapy. The patient was then referred to surgery at the Wielkopolska Cancer Center in Poznań. After final exclusion of disease dissemination (by means of PET-CT) the patient underwent total gastrectomy with D2 lymphadenectomy, and gastrointestinal tract reconstruction by means of the Roux-en-Y method. The histopathological examination result was as follows: tubular-papillary G2 adenocarcinoma (intestinal type), pT2 pN0 (23 evaluated lymph nodes without cancer metastasis), vascular neoplastic emboli, and positive HER2 protein expression. After surgery the patient was subject to adjuvant chemotherapy. Control brain CT examinations revealed the presence of 4 recurrent metastatic lesions-the patient was disqualified from stereotactic radiation therapy and was subject to palliative chemotherapy. The discussion presented the problem of treating patients with stage IV gastric cancer, including current management guidelines, as well as literature review concerning the treatment of patients with diagnosed gastric cancer and brain metastases.

The ability of various cytokines to hamper tumor growth or to induce anti-tumor immune response has resulted in their study as antitumor agents in gene therapy approaches. In this review we will concentrate on the costimulation of antitumor immune responses using modification of various cell types by cytokine genes. Several strategies have emerged such as (i) modification of tumor cells with cytokine genes ex vivo (whole tumor cell vaccines), (ii) ex vivo modification of other cell types for cytokine gene delivery, (iii) delivery of cytokine genes into tumor microenvironment in vivo, (iv) modification of dendritic cells with cytokine genes ex vivo. Originally single cytokine genes were used. Subsequently, multiple cytokine genes were applied simultaneously, or in combination with other factors such as chemokines, membrane bound co-stimulatory molecules, or tumor associated antigens. In this review we discuss these strategies and their use in cancer treatment as well as the promises and limitations of cytokine based cancer gene therapy. Clinical trials, including our own experience, employing the above strategies are discussed.

Lung cancer is associated with one of the highest mortality rates among malignant tumors. It is the main death cause of men and women in Poland where over 15 000 men and 7 000 women die with this diagnosis annually. 1.7 million people die every year due to lung cancer in the world. Two main types of lung cancer are small cell lung cancer (SCLC) and non-small cell lung cancer (NSCLC). The history of lung cancer treatment begins with surgical approach followed by addition of chemotherapy and radiotherapy, which were used either separately or in combination depending on the stage of the lung cancer. Many somatic mutations were identified and molecularly targeted therapies could have been established. One of the oldest and the best known group of molecularly targeted drugs used in lung cancer treatment are tyrosine kinase inhibitors (TKIs) of epidermal growth factor receptor (EGFR). First EGFR-TKI was gefitinib, which has been examined in clinical trials before erlotinib, afatinib, dacomitinib and osimertinib. EGFR-TKIs increased overall survival (OS) with significantly less side effects when compared to standard chemotherapy. Another group of molecularly targeted drugs are anaplastic lymphoma (ALK) kinase inhibitors such as crizotinib, alectinib and ceritinib. Another innovation which was introduced in NSCLC treatment was immunotherapy. Its effect is based on modification of immune system leading to activation of cytotoxic T lymphocytes (CTL). Currently nivolumab and pembrolizumab (anti-PD1 antibodies) as well as atezoliumab (anti-PD-L1 antibody) are being used in NSCLC patients. The discovery of innovative therapies for NSCLC patients resulted in significant extension of patients' life expectancy while minimizing the side effects of such therapy. Moreover, the quality of patients’ life was significantly improved. However, important problems still remain to be solved: overcoming the resistance in the course of molecularly targeted therapies and the lack of predictive factors that determine the effectiveness of immunotherapy.

Introduction and objective. Adapting to gynaecological cancer is the problem of accepting and coping with the disease and its consequences. The aim of this study is to analyze the relationship between stress intensity and strategies for coping with stress and psychological adaptation to disease in women diagnosed with gynaecological cancer. Materials and method. The study was performed on a group of 102 women diagnosed with gynaecologic malignancy. The mean age of patients was 56.1 (SD ±10.75) years. Three self- designed questionnaires were used to carry out the study: Scale of Perceived Stress (PSS-10), Multidimensional Inventory for Measuring Coping with Stress (Mini-COPE) and the Scale of Mental Adaptation to Cancer (Mini-MAC). Statistical significance was set at p <0.05. Results. In more than half of the women surveyed, the severity of stress experienced during the last month was high. Women with a malignant illness significantly differed (p<0.001) in the styles of coping with the disease, preferring a constructive style (M=43.5; SD±5.76) rather than a destructive style (M=21.7; SD±5.28). The highest median values were obtained by positive revalidation (M=21.9; SD±3.01) and fighting spirit (M=21.6; SD±3.47) strategies. The most prominent indicator of predicting a constructive style of coping with cancer was the coping strategy described as Positive Revalidation (ßeta=0.38; R²=0.41); whereas an Acceptance strategy was a predictor of a destructive style (ßeta=-0.30; R²=0.31). Conclusions. Most women facing gynaecological cancer experience a high level of stress, and try to use active strategies to deal with the disease. The results indicate that there is a need to offer special psychological care to oncologically diagnosed women.

Introduction and objective. Matrix metalloproteinases (MMPs) are proteolytic enzymes responsible for the decomposition of extracellular matrix elements. They play an important role during embryogenesis, wound healing, endometrial epithelial exfoliation, the formation of new blood vessels, and also during cancer development. Throat and larynx tumours are included in a large group of head and neck cancers. These tumours are characterized by a poor prognosis. Despite advances in medical science, cancer treatment is difficult and often ineffective. The aim of the study was to evaluate the level of MMP-3 and MMP-9 in the serum and saliva of patients with pharynx and larynx tumours. Materials and method. Samples of saliva and serum were collected from 60 patients with larynx or throat cancer. Twenty patients without cancer comprised the control group. MMPs in saliva and serum were determined by the ELISA method. Results. In the study group, concentrations of MMP-3 in saliva were from 0.2 – 77.6 ng/ml. Patients with malignant tumours had higher saliva MMP-3 levels than healthy subjects. The concentration of MMP-3 in the serum of the study group ranged from 10.9 – 200.00 ng/ml, which was also higher than in the control group. There were no statistically significant difference in the MMP-9 level between the study and control groups (both in serum and saliva). Conclusions. This study is another element that shows the phenomena taking place at the cellular level during oncological disease. In serum and saliva samples, higher values of MMP3 were found in patients with cancer. The increase in the concentration of this enzyme in the risk group may be used for early detection of tumour transformation and evaluation of treatment.

Recently, the diagnostic use of saliva in medicine has become a major goal for many researchers. There are many advantages of using saliva diagnostics. Its drawing is non-invasive, painless, quickly repeated, and straightforward. Its diagnostic potential has been demonstrated, in particular for infectious diseases and diseases of the oral cavity. Because of the unabated frequency of occurrence of malignant neoplastic diseases, especially in seniors, this paper aims at presenting an overview of chosen studies that have used saliva in the diagnostic and monitoring of selected malignant diseases. The potential use of saliva in the diagnosis of early-stage lung cancer, by detecting genetic material in saliva and analysis of characteristic mutations of the EGFR gene, has been described. Additionally, prostate cancer can be detected by positive identification of microRNA markers in saliva. Evaluation of PSA concentrations in saliva has also been carried out and obtained results were correlated with blood concentrations. There are also reports concerning the Ca15-3 marker in the saliva in women with breast cancer. The evaluation of lung cancer resistance protein in the saliva of these patients has also been investigated. The results of the cited studies are promising and give hope for the potential use for salivary diagnosis of these cancers.
W ostatnich latach diagnostyczne wykorzystanie śliny w medycynie stało się celem naukowym dla wielu badaczy. Ślina jako biologiczny materiał diagnostyczny ma wiele zalet: jej pobranie jest małoinwazyjne, niebolesne, szybko powtarzalne i łatwe. Udowodniono jej potencjał diagnostyczny przede wszystkim w chorobach infekcyjnych i jamy ustnej. Ze względu na niesłabnącą częstość występowania nowotworów złośliwych wśród populacji osób w starszym wieku, niniejsza praca ma na celu przedstawić przegląd wybranych badań naukowych wykorzystujących ślinę w diagnostyce oraz monitorowaniu wybranych chorób nowotworowych. Opisano potencjalne wykorzystanie śliny w rozpoznaniu wczesnego stadium raka płuca dzięki detekcji w ślinie materiału genetycznego i analizie charakterystycznych mutacji genu kodującego białko EGFR. Realną perspektywą w raku prostaty jest wykrywanie w ślinie wybranych mikroRNA. Dokonuje się także oceny stężenia antygenu PSA w ślinie, a otrzymane wyniki analizuje się względem jego stężenia we krwi. Istnieje wiele doniesień na temat oznaczeń antygenu Ca15-3 w ślinie wśród kobiet z rakiem piersi. Dokonuje się także oceny białka oporności raka płuc w ślinie u tych chorych. Wyniki przytoczonych badań są obiecujące oraz dają nadzieję na możliwość zastosowania wczesnej diagnostyki tych nowotworów.

Biomineralogical investigations of cancer in various tissues -- breast, connective tissue, skin, lungs, thyroid, pancreas, bone, cartilage, etc. -- have been conducted in our Laboratory with the use of mineralogical methods (SEM-EDS, EMP, IR, X-RD, ASA, etc.) for the last 40 years. Obtained results allowed us to discover elevated levels of various elements at tumor sites of mentioned cancers. Obtained data suggest local biomineralization og liquids may lead to creation of cancer.
Biomineralogiczne badania nowotworów różnych tkanek, piersi, tkanki łącznej, skóry, płuc tarczycy, trzustki, kości, chrząstki prowadzone były w naszym laboratorium z wykorzystaniem mineralogicznych metod (SEM-EDS, EMP, IR, X-RD, ASA, etc.) w ciągu ostatnich 40 lat. Otrzymane wyniki pozwoliły odkryć w tkankach nowotworowych podwyższone zawartości różnych pierwiastków e badanych tkankach nowotworowych. Może to oznaczać, że lokalna biomineralizacja płynów ustrojowych różnymi pierwiastkami i substancjami sprzyja powstawaniu nowotworów.

Significant, unexplained disparities in the incidence of lung cancer in men and breast cancer among some neighbouring populations of the Pomorskie voivodeship poviats were revealed. They were not found for lung cancer in women. Examinations were conducted on data from the Pomeranian Cancer Registry. A hypothesis was put forward that the observed accumulation of certain surnames in certain poviats argues for the repetitive occurrence of certain features of the genotype in populations inhabiting these areas, which may have an influence on the emergence of certain cancers and play a role in the emergence of the above mentioned disparities.

Introduction: Management of advanced laryngeal cancer has shown fluctuating trends during the last few decades. Though many extensive reports are available in the literature regarding survival outcomes for advanced laryngeal cancer, there is a paucity of elaborate systematic reviews giving a complete picture of facts and figures. The present analysis brings to attention the most relevant data in a focused and up to date format, for simpler interpretation of evidence-based inference. Objective: To present the trends in the treatment of T4 laryngeal cancer over the past few decades and analyze survival outcomes for different treatment modalities in the management of T4 laryngeal cancer by way of systematic review. Methods : An electronic search was conducted using the terms “total laryngectomy”, “T4 laryngeal cancer”, “survival outcomes” in combination with the following search strategy : Search block Laryngeal cancer - "Laryngeal Neoplasms"[Mesh] OR ((Laryngeal[tiab] OR larynx[tiab] OR "Larynx"[Mesh]) AND (“Neoplasms”[Mesh] OR neoplasm* [tiab] OR tumor* [tiab] OR tumour* [tiab] OR cancer* [tiab] OR malignancy* [tiab] OR carcinoma* [tiab] OR neoplasm* [tiab] OR oncology* [tiab])); Search block Total laryngectomy - "Laryngectomy"[Mesh] OR total laryngectomy*[tiab] OR total laryngopharyngectomy*[tiab] OR total pharyngolaryngectomy*[tiab]; Search block T4 - t4[tiab]. Clinical studies were retrieved from the electronic databases of PubMed, EMBASE, SCOPUS and Cochrane Library. 304 articles had been published till June 2017, which included prospective studies, randomized controlled trials, retrospective studies, and smaller descriptive studies. References of the selected studies were further searched for relevant articles. Apart from this, a search was conducted on Google Scholar to obtain related articles. Results: Numerous studies, as mentioned in this review, provide authentic evidence in relation to the efficacy and outcome of surgical treatment for T4 laryngeal cancer. To address the problem of heterogeneity with regards to patient selection, numerous reports pertaining to T4 patients exclusively have been included. Conclusion: Total laryngectomy remains the gold standard for management of T4a laryngeal cancer. After the unparalleled oncological outcomes of more than a century, the technique has stood the test of time. An exhaustive review of the literature has been presented, discussing the trends in the treatment of advanced laryngeal cancer across different continents. However, it must be specified that the purpose of the study is not to prove one treatment protocol to be superior to the other but to bring out patterns of adherence to protocols and guidelines as suggested by multidisciplinary consensus reports and the consequent outcomes.

High-dose chemotherapy currently used in the treatment of children with cancer may induce late effects on psychosocial and cognitive functioning in some of them. The aim of the study was to evaluate psychosocial functioning of childhood cancer survivors living in rural regions. In total, 29 children entered the study. The children were diagnosed as having cancer between 1993-1995 in a single centre. Each patient was examined 5 years after the completion of cancer therapy. The children had been diagnosed with leukemia and lymphoma (72.4%), and solid tumours. Self-report questionnaires, as well as standardized psychological test (WISC-R, WAIS-R-PL) were performed for the evaluation of psychosocial and cognitive functioning of the patients. Most of the studied cancer survivors living in rural areas did not reveal diffi culties with cognitive and psychosocial functioning. In some patients, however, we observed more diffi culties in verbal tasks and existing major learning diffi culties. Cancer survivors living in rural areas in our region may be more unprivileged due to poor additional supporting services.

Introduction. The incidence of head and neck cancers in Poland demonstrated a general tendency to stabilize in the last two decades. However, a global phenomenon in the change of morbidity structure in terms of the specific anatomical location is observed, which will probably increasingly apply also to Poland. Aim. The aim of the study was to present the changes that have occurred in the structure of the incidence of oropharyngeal cancer in comparison to laryngeal cancer in the period from 1980 to 2013, in the region of south-eastern Poland and the whole country. Material and methods. A retrospective analysis of the incidence due to the head and neck organ cancer in 1980-2013 in the region of south-eastern Poland and the whole country was performed based on demographic data from the Provincial Statistical Office in Rzeszow and Podkarpackie Cancer Register as well as the Department of Epidemiology, Oncology Centre in Warsaw. Results and conclusion. In the last three decades, the percentage of laryngeal cancer incidence in women (13% vs 18%) as well as oropharyngeal cancer (18.2 % vs 21.6%) was lower in Podkarpacie than in Poland overall. The incidence of oropharyngeal cancer in men in Podkarpacie was lower in the analyzed period than in Poland overall, and only in the last 3 years of observation has it reached a value close to the average for the country. The incidence of laryngeal cancer in men showed a dramatic downward trend both in Poland and in the Podkarpacie province, whereby both in terms of incidence rates and standardized rates and percentages – it is far more pronounced in the analyzed province.

Bacterial cancer therapy is a concept more than 100 years old - yet, all things considered, it is still in early development. While the use of many passive therapeutics is hindered by the complexity of tumor biology, bacteria offer unique features that can overcome these limitations. Microbial metabolism, motility and sensitivity can lead to site-specific treatment, highly focused on the tumor and safe to other tissues. Activation of tumor-specific immunity is another important mechanism of such therapies. Several bacterial strains have been evaluated as cancer therapeutics so far, Salmonella Typhimurium being one of the most promising. S. Typhimurium and its derivatives have been used both as direct tumoricidal agents and as cancer vaccine vectors. VNP20009, an attenuated mutant of S. Typhimurium, shows significant native toxicity against murine tumors and was studied in a first-in-man phase I clinical trial for toxicity and anticancer activity. While proved to be safe in cancer patients, insufficient tumor colonization of VNP20009 was identified as a major limitation for further clinical development. Antibody-fragment-based targeting of cancer cells is one of the few approaches proposed to overcome this drawback.

The most common cancer among all skin cancers is basal cell carcinoma. It occurs five times more likely than squamous cell carcinoma. In total, the incidence of skin cancer in Poland was in 2005, more than 4,300 new cases in women and nearly 4,000 men. Skin cancer is ranked in terms of the incidence of cancer, respectively, the 3 and 5 position. The incidence of skin cancer is associated with exposure to sunlight. Skin cancer most often locates on the exposed parts of the body (hands, face, neck, scalp - particularly in those balding). Most often affects people in 6 and 7 decade of life.The aim of the study was to present the carcinogenic factors responsible for the development of skin cancers and to briefly discuss individual skin cancers. We analysed Polish and foreign bibliography on the prevention of skin cancers based on the most recent reports.

Background: Breast cancer is the first in the structure of the incidence of neoplastic diseases in women, with the number of affected individuals becoming higher every year. The risk of breast cancer is influenced not only by genetic factors, but also by the lifestyle. Proper dietary habits, a high level of physical activity and normal body weight not only reduce the risk of developing a primary neoplastic lesion, but also a recurrence. In 2007 the World Cancer Research Fund (WCRF) and American Institute for Cancer Research (AICR) published their recommendations concerning lifestyle in the prophylaxis of neoplasms. Objective. The aim of the study was to assess whether the adherence to WCRF/AICR recommendations influenced the risk of developing breast cancer in women. Materials and methods: A case control study included 108 women aged over 50 with a history of breast cancer. The study group was divided into two subgroups: women who completed oncological treatment and experienced no recurrences for at least 5 years (group I, n=82) and women who had a recurrence (group II, n=26). The control group included women with no history of breast cancer (n=74). The adherence of lifestyle was assessed by assigning points for 8 WCRF/AICR recommendations. The results were compared in the study and control groups, both in all participants and separately in those who declared no changes in dietary habits after being diagnosed with breast cancer. Results. The adherence of lifestyle to WCRF/AICR recommendations was significantly lower in the group of women with a history of cancer compared to the control group. It was reported both in the study group as a whole (5.5 ± 1.34 vs 6.4 ±1.48 points) and in those who declared no changes in dietary habits after being diagnosed with breast cancer (5.3 ± 1.24 vs 6.6 ± 1.38 points). The differences in the lifestyles of the participants with breast cancer and those in the control group were associated predominantly with the adherence to recommendations concerning appropriate physical activity, avoiding the consumption of sweetened drinks and limiting the consumption of processed and red meat. Conclusions. The results of the study confirmed the benefits of complying with WCRF/AICR recommendations in the prevention of breast cancer.
Wprowadzenie. Nowotwór piersi zajmuje pierwsze miejsce w strukturze zachorowań na nowotwory u kobiet, a z roku na rok liczba chorujących kobiet rośnie. Na ryzyko raka piersi wpływ mają nie tylko czynniki genetyczne, ale także styl życia. Prawidłowy sposób odżywiania, wysoki poziom aktywności fizycznej i utrzymywanie prawidłowej masy ciała zmniejszają ryzyko nie tylko pojawienia się pierwotnej zmiany nowotworowej, ale także wznowy. W roku 2007 Światowy Fundusz Badań nad Rakiem (WCRF) oraz Amerykański Instytut Badań nad Rakiem (AICR) wydały zalecenia dotyczące stylu życia w profilaktyce nowotworów. Cel. Celem badań była ocena czy stosowanie zaleceń WCRF/AICR ma wpływ na ryzyko powstania raka piersi u kobiet. Materiał i metody. Badanie kliniczno-kontrolne objęło 108 kobiet po 50. roku życia z rakiem sutka w wywiadzie. Badane podzielono na dwie grupy: kobiety, które zakończyły leczenie onkologiczne oraz pozostawały bez wznowy przez minimum 5 lat (grupa I, n=82) i kobiety, u których wystąpiła wznowa choroby (grupa II, n=26). Grupę kontrolną stanowiły kobiety, u których nigdy nie rozpoznano raka piersi (n=74). Metodą punktową oceniono zgodność stylu życia z 8 zaleceniami WCRF/AICR. Porównano uzyskaną punktację w grupie badanej i kontrolnej, zarówno u wszystkich uczestniczek, jak i osobno u tych, które deklarowały brak zmiany sposobu żywienia po zachorowaniu na raka piersi. Wyniki. Kobiety z historią nowotworową cechowały się istotnie mniejszą zgodnością stylu życia z zaleceniami WCRF/AICR w porównaniu z grupą kontrolną. Dotyczyło to zarówno całej grupy badanych kobiet (5,5 ± 1,34 vs 6,4 ± 1,48 punktów), jak i tych które deklarowały brak zmian sposobu żywienia po zachorowaniu na raka piersi (5,3 ± 1,24 vs 6,6 ± 1,38 punktów). Różnice w stylu życia między uczestniczkami z rakiem piersi w wywiadzie w porównaniu z grupą kontrolną dotyczyły przede wszystkim przestrzegania zalecenia dotyczącego odpowiedniej aktywności fizycznej, unikania spożycia słodzonych napojów, a także ograniczenia spożycia przetworzonego i czerwonego mięsa. Wnioski. Wyniki badania potwierdziły korzyści płynące z przestrzegania zaleceń WCRF/AICR w zapobieganiu rakowi piersi.

Paraneoplastic syndromes (PS) involve signs and symptoms from various organs, which accompany tumors at various stages of their development: they may precede the development of cancer by a few months or even years, may be detected during the diagnosis or treatment of a malignant tumor, or they may precede its relapse following treatment. The most frequent mechanism of their development is thought to involve disturbances in the endocrine or autoimmune system, due to the hormones secreted by the tumors, their precursors or cytokines (secretion of ADH, PTHrP, PTH, ACTH, IL-1, IL-6, TNF). Immune mechanisms result in the development of antibodies specific for antigens of own healthy tissues, e.g. in neurologic PS antibodies are detected which are specific for Yo, CV2 and Ri. Among many types of PS in tumors of female genital organs, the most frequent ones involve endocrine syndromes leading to hormonal dysregulation, neurologic ones which result in occasionally severe disturbances of the central nervous system, hematologic PS, the most common one being thromboembolic lesions, and dermatological PS. The other PS found in early and, more frequently, in more advanced cancer include the syndrome of neoplastic cachexia, sometimes encountered in ovarian and cervical cancer.
Zespoły paranowotworowe (ZP) są to objawy ze strony różnych tkanek i narządów, towarzyszące nowotworom na różnych etapach ich rozwoju. Mogą wyprzedzać o kilka miesięcy lub nawet lat rozwój raka, mogą być stwierdzane w czasie diagnostyki lub leczenia nowotworu złośliwego, a także wyprzedzać jego nawrót po leczeniu. Za najczęstszy mechanizm ich powstawania uważa się zaburzenia w układzie endokrynnym lub autoimmunologicznym. Mechanizm endokrynny polega na wydzielaniu przez guz hormonów, ich prekursorów lub cytokin (ADH, PTHrP, PTH, ACTH, IL-1, IL-6, TNF). W wyniku mechanizmów immunologicznych dochodzi do powstania przeciwciał przeciwko antygenom własnych zdrowych tkanek, na przykład w ZP neurologicznych stwierdza się przeciwciała anty-Yo, anty-CV2 i anty-Ri. Wśród wielu zespołów paranowotworowych występujących w przebiegu nowotworów narządów płciowych kobiet najczęściej obserwuje się zespoły endokrynologiczne, prowadzące do dysregulacji hormonalnej, neurologiczne, w wyniku których stwierdza się niekiedy ciężkie zaburzenia ze strony ośrodkowego układu nerwowego, hematologiczne, spośród których najczęstsze są zaburzenia zatorowo-zakrzepowe, oraz ZP dermatologiczne. Do innych ZP związanych między innymi z odżywianiem i metabolizmem zalicza się zespół wyniszczenia nowotworowego, stwierdzany w przebiegu zaawansowanego raka jajnika i szyjki macicy.

microRNAs (miRNAs) are small, non-coding RNA molecules that regulate gene expression by targeting specific mRNAs. microRNAs play a role in several physiological processes in the cell, including migration, proliferation, differentiation and apoptosis. Apart from their role in regular metabolism, abnormal profiles of miRNA expression accompany cancer transformation, including colorectal cancer (CRC) metastasis. microRNAs may play a role in each phase of CRC metastasis including angiogenesis, invasion, intravasation, circulation, extravasation and metastatic colonization. microRNA levels may serve as a predictive CRC marker, which was confirmed by the serum level of miR-29a targeting KLF4, a marker of cell stemness, and the plasma level of miR-221 down-regulating c-Kit, Stat5A and ETS1, which are signal transducers and transcription factor, respectively. In turn, the level of miR-143 in CRC cells decreasing the amount of MACC1 (metastasis-associated in colon cancer-1) and oncogenic KRAS protein, may be utilized as a prognostic marker. Also, single nucleotide polymorphisms of genes encoding miRNAs, including miR-423 and miR-608, which correlate with tumor recurrence, may be useful as diagnostic, prognostic and predictive indicators in CRC metastasis. Pre-miR-34a and pre-miR-199a decreased the level of Axl, a tyrosine-protein kinase receptor, so they can be considered as drugs in antimetastatic therapy. On the other hand, miR-222 targeting ADAM-17, a disintegrin and metalloproteinase, and miR-328 interacting with ABCG2, an ABC transporter, may overcome drug resistance of cancer cells. microRNAs may be considered in wide-range application to facilitate CRC metastasis diagnosis, prognosis, prediction and therapy, however, further clinical, epidemiological and in vitro studies should be conducted to verify their utility.

Most patients diagnosed with gastrointestinal carcinomas are older people. The above-mentioned fact may lead to an erroneous finding that the problem does not concern patients aged between 20 and 30 years. Unfortunately, this assumption is often the reason for late diagnosis and delayed treatment of these malignancies. The study presented an example of three patients subject to surgical management of gastrointestinal carcinomas at the II Department of General and Gastroenterological Surgery, Medical University in Białystok.

According to the age-period-cohort projection of lung cancer incidence (ICD-10 codes C33-C34), smoking prevention will be crucial for both men and women longevity. Lung cancer remains the main cancer cause of death among males in Poland. In 2014 the number of deaths in Poland caused by lung cancer among men was more than twice as high as for women. The study is focused on identification of attitudes influencing the propensity to smoke, which is performed by structural equation modelling (SEM) based on weighted least squares means and variance adjusted (WLSMV) estimation. Good fit of the presented models is demonstrated by RMSEA, NNFI and CFI values. Reliability of scales is assessed by ordinal alpha and ordinal theta coefficients. Commitment to learning in childhood exhibits the highest standardized effect on propensity to smoke, which is an argument for youth smoking prevention. Structural equation model was estimated for men and women separately because of gender differences in smoking determinants. A strong character, individualism and self-reliance are the factors, which increase smoking propensity among males. These can be treated as features of masculinity and are not statistically important determinants of being a smoker for females. Depressive disorders, being a victim of violence as adults are statistically significant determinants of smoking among women.

The incidence of peritoneal carcinomatosis of colorectal cancer amounts to 5%-15% for synchronous metastases and as much as 40% in cases of local recurrence. Despite significant advances in pharmacological treatment consisting in the introduction of targeted medications, prognoses for patients with peritoneal metastases are still unfavorable. Mean survival of these patients undergoing palliative treatment is in the range of 6-9 months; overall survival rarely exceeds 2 years upon extensive chemotherapy. Markedly better results are obtained for cytoreductive surgery combined with hyperthermic intraperitoneal chemotherapy (HIPEC). This treatment offers much better outcomes, leading to 5-year survival rates of as much as 30%-50%. The possibility to achieve such outcomes depends on the staging of peritoneal metastases as well as the completeness of macroscopic cytoreduction. The procedures require significant experience in abdominal surgery, are time-consuming (mean duration of the procedure ranging from 6 to 8 hours) and are burdened by complications that are due not only to the procedure itself but also to the intraperitoneal administration of the cytostatic drug at elevated temperature (41.5 °C). However, in patients with metastases limited to the peritoneum, the procedure is justified by its outcomes. At the same time, a combination of surgical cytoreduction with HIPEC followed by systemic chemotherapy offers a radical change in prognosis for patients hitherto qualified for palliative treatment only.

Introduction. Neoadiuvant therapy (NCT) in the treatment of breast cancer is employed for patients with early stage disease or with inoperable disease. NCT can decrease the tumor volume. It can facilitate breast conservation therapy. Response to NCT is a strong predictor of outcome breast cancer (BC). Direct target therapies has markedly improved the result of treatment BC. Aim. Therapy for breast cancer continues to improve. The importance of tumor burden on local control rates will be in the future. Material and methods. This analysis was performed using a systematic literature search. Results. The latest scientific reports give hope for greater safety and a better life for patients based on optimized and effective therapy. Conclusion. Currently, improving the effectiveness of breast cancer treatment is mainly related to the optimal use of classic therapeutic strategies. New classes of substances have been approved for treatment or are in advanced stages of clinical development.

On 2–4 May 2013, in Brussels took place the 5th IMPAKT conference on translational research in breast cancer. The most important issue raised during the conference was the phenomenon of breast cancer heterogeneity, both inter- and intratumour. Heterogeneity of breast cancer was confirmed at the molecular level, showing the differences in the frequency of genetic aberrations (typically PIK3CA, TP53 and ARID1A) in patients with hormone-dependent advanced breast cancer (BOLERO-2 trial). The results of genetic tests were also presented, which showed that in order to reduce the impact of heterogeneity within the tumor, 3 biopsies should be performed from each breast cancer. The most common disorders of PI3K/mTOR pathway were analyzed depending on the molecular type of breast cancer. However, an attempt to determine by immunohistochemistry biomarkers predicting response to combination therapy with everolimus and trastuzumab failed. The results of two studies, which analyzed the value of molecular tests in predicting late recurrence of breast cancer (in 5–10 years after diagnosis), were presented – the best results were achieved with PAM50 and Breast Cancer Index. The latest discoveries in molecular imaging were also presented – it allows for early assessment of treatment efficacy.
W dniach 2–4 maja 2013 r. w Brukseli odbyła się 5. konferencja IMPAKT poświęcona tematyce badań podstawowych nad rakiem piersi. Najważniejszym zagadnieniem poruszanym podczas konferencji było zjawisko heterogenności raka piersi, występujące zarówno między guzami, jak i w obrębie jednego guza. Potwierdzono heterogenność raka piersi na poziomie molekularnym, wykazując różnice w częstości aberracji genetycznych (najczęściej w obrębie PIK3CA, TP53 i ARID1A) w guzach u chorych na hormonozależnego, zaawansowanego raka piersi (badanie BOLERO-2). Przedstawiono również wyniki badań genetycznych, które wykazały, że aby ograniczyć wpływ heterogenności w obrębie guza, należy wykonywać 3 biopsje z każdego raka piersi. Zbadano najczęstsze zaburzenia szlaku PI3K/mTOR w zależności od typu molekularnego raka piersi. Natomiast próba określenia za pomocą immunohistochemii biomarkerów odpowiedzi na terapię skojarzoną ewerolimusem z trastuzumabem nie powiodła się. Zaprezentowano wyniki dwóch prac naukowych, w których analizowano wartość testów molekularnych w predykcji późnego nawrotu raka piersi (5–10 lat od rozpoznania) – najlepsze wyniki osiągnięto w przypadku PAM50 i Breast Cancer Index. Poza tym przedstawiono najnowsze doniesienia w zakresie obrazowania molekularnego pozwalającego na wczesną ocenę skuteczności stosowanej terapii.

Palliative chemotherapy in patients with thrombophilia is a challenge for clinical oncologists. On one hand the optimal treatment should be very safe for patient, on the other hand – as effective as possible. That therapy is also connected with proper thromboprophylaxis. The incidence of multiple cancers is a serious problem of modern oncology. In patients whose medical history includes two or more cancers, it is essential to obtain histopathological diagnosis before the administration of treatment of disseminated disease. It allows to avoid improper therapy which is often toxic. The article presents the case of a patient with antithrombin deficiency and two cancers in medical history: colon cancer and endometrial cancer. The treatment of the patient because of the metastatic cancer was started after the histopathological diagnosis. In metastatic lymph node the endometrial cancer was recognized. The carboplatin and paclitaxel were used in the therapy. The treatment was conducted in four course chemotherapy. It revealed considerable polyneuropathy and hematologic toxicity limiting from further therapy. Treatment allowed to obtain biochemical response, decrease of metastases and the condition of a patient improved. During the systemic therapy thromboprophylaxis with acenocumarol was used. The incidence of venous thromboembolism or bleeding complications were not noticed.

Coffee is an important dietary source of biologically active components, not to mention caffeine, phenolic acids, and diterpenes. It has been suggested that selected coffee secondary metabolites may beneficially modulate several mechanisms of anti-cancer protection. This literature review was intended to present current knowledge related to coffee and its components and hormone-dependent female cancers, such as breast, endometrial and ovarian cancer, and to identify gaps in research that may be exploited in the future. The search for studies was conducted through electronic databases. Publications on coffee composition, coffee preparation and brewing methods, in vitro and in vivo experiments with the use of substances naturally present in coffee, observational studies, and meta-analyses were collected. In population studies, the greatest attention has been paid to the anticancer effect of caffeinated coffee and/or caffeine. In general, most studies and meta-analyses indicated that there was no clear correlation between coffee and breast cancer or ovarian cancer. Some subgroups of women may benefit from coffee consumption. This is the case for post-menopausal women with regard to the risk of breast cancer and obese women with regard to the risk of endometrial cancer. This paper identifies a number of issues for future research, related to a better understanding of the anti-cancer mechanisms of coffee compounds and further research that would focus on specific target groups, taking into account both the different methods of coffee preparation and lifestyle factors that may influence the results.

Age-related increase of risk of developing a genital malignancy in the females, increasing level of health awareness in general population, improved oncologic prevention, better diagnostic techniques and therapies, all of them contribute to increased number of patients at menopausal age completing oncological treatment. The problem concerns also young women, who require radical surgical consisting in oophorectomy, radiotherapy and chemotherapy. An increasingly frequent problem in the gynecologic practice is the use of hormonal replacement therapy in patients with a history of malignancy. Oophorectomy before menopause results in development of atrophic lesions in estrogen-dependent tissues, osteoporosis, cardiovascular and urogenital diseases, sexual dysfunction, vasomotor disorders and compromised quality of life, resulting in disorders of lipid metabolism and affects mental condition. An absolute contraindication for hormonal replacement therapy during menopause is active estrogen-dependent malignancy. Other clinical situations and controversies associated therewith concerning safety, particularly in view of hormone-dependent tumors, result in several attempts at development of uniform and generally accepted guidelines concerning the use of hormonal replacement therapy in menopausal women. This paper discusses hormonal replacement therapy in the context of its role in the carcinogenesis process and its use in patients with current or past history of a malignancy. The aim of this paper is to present current state-of-the-art in the aspect of safety of hormonal replacement therapy in patients with a malignancy: endometrial cancer, breast cancer, ovarian cancer, cervical cancer, vulvar cancer and also benign tumors, e.g. uterine myoma and endometrial cyst.
Zwiększające się z wiekiem ryzyko zachorowania na nowotwory żeńskich narządów płciowych, wzrost poziomu świadomości społecznej, profilaktyki onkologicznej, lepsze możliwości diagnostyczne oraz terapeutyczne schorzeń nowotworowych przyczyniają się do wzrostu liczby pacjentek w wieku menopauzalnym po leczeniu onkologicznym. Problem dotyczy również kobiet w młodym wieku, wymagających radykalnego leczenia chirurgicznego obejmującego usunięcie jajników, radioterapię i chemioterapię. Coraz częstszy problem w praktyce ginekologicznej stanowi aspekt stosowania hormonalnej terapii zastępczej u pacjentek z chorobą nowotworową w wywiadzie. Usunięcie jajników przed menopauzą skutkuje rozwojem zmian zanikowych w tkankach estrogenozależnych, osteoporozy, schorzeń układu sercowo-naczyniowego i układu moczowo-płciowego, dysfunkcji seksualnych, objawów wazomotorycznych oraz pogorszeniem jakości życia, stanowi przyczynę rozwoju zaburzeń gospodarki lipidowej oraz wpływa na kondycję psychiczną. Bezwzględnym przeciwwskazaniem do stosowania hormonalnej terapii wieku menopauzalnego jest czynna estrogenozależna choroba nowotworowa. Pozostałe sytuacje kliniczne i związane z nimi kontrowersje dotyczące bezpieczeństwa stosowania terapii, szczególnie w aspekcie nowotworów hormonozależnych, skłaniają do licznych prób wypracowania ujednoliconych, obowiązujących wytycznych stosowania leczenia hormonalnego wieku menopauzalnego w tej grupie pacjentek. W opracowaniu omówiono hormonalną terapię zastępczą w kontekście udziału w procesie karcynogenezy oraz stosowania leczenia u pacjentek z rozpoznaną aktualnie lub w przeszłości chorobą nowotworową. Celem opracowania jest przedstawienie aktualnego stanu wiedzy na temat bezpieczeństwa stosowania hormonalnej terapii zastępczej w przypadku nowotworów złośliwych: raka endometrium, piersi, jajnika, szyjki macicy, sromu oraz guzów łagodnych: mięśniaków macicy, torbieli endometrialnych.

Mitochondria play the central role in supplying cells with ATP and are also the major source of reactive oxygen species (ROS) - molecules of both regulatory and destructive nature. Dysfunction of mitochondrial metabolism and/or morphology have been frequently reported in human cancers. This dysfunction can be associated with mitochondrial DNA (mtDNA) damage, which may be changed into mutations in mtDNA coding sequences, or the displacement-loop region, changes in the mtDNA copy number or mtDNA microsatellite instability. All these features are frequently associated with human cancers. Mutations in mtDNA can disturb the functioning of the ROS-producing organelle and further affect the entire cell which may contribute to genomic instability typical for cancer cells. Although the association between some mtDNA mutations and cancer is well established, the causative relationship between these two features is largely unknown. A hint suggesting the driving role of mtDNA mutations in carcinogenesis comes from the observation of tumor promotion after mtDNA depletion. Mitochondria with damaged DNA may alter signaling of the mitochondrial apoptosis pathway promoting cancer cell survival and conferring resistance to anticancer drugs. This resistance may be underlined by mtDNA copy number depletion. Therefore, mitochondria are considered a promising target in anticancer therapy and several mitochondria-targeting drugs are in preclinical and clinical trials. Some other aspects of mitochondrial structure and functions, including morphology and redox potential, can also be associated with cancer transformation and constitute new anticancer targets. Recently, several studies have disclosed new mechanisms underlying the association between mitochondria and cancer, including the protection of mtDNA by telomerase, suggesting new approaches in mitochondria-oriented anti-cancer therapy.

Background: Cancer, next to cardiovascular disease, obesity, diabetes, accidents, and mental illness, is one of the most common diseases of the 21st century. In the female population, cervical cancer is most often diagnosed at late stages. Aim of the study: The goal of the study is to determine the opinions of women who reside in Bialski poviat about midwife participation in cervical cancer screening and prevention. Material and methods: The study group was recruited from randomly selected patients from two physiotherapy offices in the Bialski poviat, as well as students and employees of the State University Pope John Paul II in Biala Podlaska. The research tool consisted of the author’s questionnaire concerning the role of midwives in cervical cancer prevention and consisted of 25 questions. Results: Almost half of the respondents or 44.2% (76) believe that the role of a midwife in cervical cancer prevention is based on health education given to women about cervical cancer screening and prevention. 27.3% (47) believe that the role of a midwife in prevention is based on the availability of cytological pap smears. Most or 56% (14) respondents from the age group over 55 and 29.2% (7) surveyed in the 18–25 age group knew about the important role of midwives performing cytological pap smear as part of a prevention strategy against cervical cancer. Conclusions: 1. The knowledge base of the women examined, regarding the role of a midwife in the prevention of cervical cancer, is lacking or insufficient. 2. The role of the midwife in the prevention of cervical cancer is unknown to young women in the 18–25 age group.

Endometriosis is a benign proliferative process, where tissues of the uterine mucosa are present ectopically, generally manifesting by infertility and pain. Awareness of similarity between endometriosis and neoplastic process, as well as of oncological risk associated therewith is low. Since 1925, when Sampson was the first to highlight the fact that endometriosis may transform into a malignant lesion, several papers appeared in the medical literature extensively documenting the correlation between endometriosis and tumors, in particular with ovarian cancer. The aim of this paper was to examine correlations linking endometriosis with the neoplastic process by reviewing medical literature published in English in the aspect of epidemiological evidence of endometriosis – cancer correlation, comparison of clinical-pathological features of both entities and comparison of molecular and genetic features of endometriosis with the “cancer phenotype” as defined by Hanahan and Weinberg in their paper The hallmarks of cancer. A large body of evidence has been collected documenting similarity of endometriosis to the neoplastic process, its association with malignant transformation and increased risk of cancer and other malignant tumors. It is not clear, whether endometriotic lesions may undergo malignant transformation directly, indirectly with an intermediate phase, the so-called atypical endometriosis, or endometriosis and cancer possess common initial antecedent mechanisms and/or predisposing factors (e.g. genetic susceptibility, sensitivity to exposure to environmental toxins) with subsequent divergence on the molecular level. Incidence of endometriosis undoubtedly creates an oncological risk, necessitating elucidation by further genetic and molecular studies.
Endometrioza to łagodny proces rozrostowy, w którym tkanki błony śluzowej macicy występują ekotopowo. Schorzenie kojarzone jest powszechnie z niepłodnością oraz z bólem. Mała jest świadomość podobieństw endometriozy do procesu nowotworowego oraz zagrożeń onkologicznych związanych z jej występowaniem. Od 1925 roku, kiedy Sampson jako pierwszy zwrócił uwagę, że endometrioza może przekształcić się w zmianę złośliwą, w literaturze medycznej pojawiły się liczne publikacje obszernie dokumentujące związek gruczolistości z nowotworami, a w szczególności z rakiem jajnika. Celem pracy było zbadanie powiązań endometriozy z procesem nowotworowym poprzez przeszukanie anglojęzycznego piśmiennictwa medycznego pod kątem dowodów epidemiologicznych na związek endometriozy z rakiem, porównania cech kliniczno-patologicznych endometriozy i raka oraz porównanie cech molekularnych i genetycznych endometriozy z cechami „fenotypu raka” zdefiniowanymi przez Hanahana i Weinberga w publikacji zatytułowanej The hallmarks of cancer. Zebrano liczne dowody dużego podobieństwa endometriozy do procesu nowotworowego, związku z transformacją złośliwą oraz ze zwiększonym ryzykiem raka i innych nowotworów złośliwych. Nie jest jasne, czy zmiany endometriotyczne mogą bezpośrednio ulegać przemianie złośliwej, z etapem pośrednim, tzw. atypowej endometriozy, czy też endometrioza oraz rak posiadają wyłącznie wspólne początkowe, antecendentne mechanizmy i/lub czynniki predysponujące (np. genetyczną podatność, wrażliwość na ekspozycję toksynami środowiskowymi) z oczywistą dywergencją w przebiegu molekularnym. Występowanie endometriozy niewątpliwie stwarza zagrożenie onkologiczne, którego źródło powinno być wyjaśnione poprzez dalsze badania genetyczne i molekularne.

Malignant neoplasms are in Poland a major social problem not only in the older age groups, but they are the main cause of premature mortality (before age 65). This phenomenon is observed particularly in women - for several years cancer have been leading cause of death before age 65 (about 30% of deaths among young women and nearly 50% of deaths among middle-aged women). The projected increase in the number of cancer cases and deaths is attributable to demographic changes of the Polish population. Central Statistical Office in Poland estimates that the number of people over 65 years will increase by almost 75% by 2025. These changes translate into an increase in the number of new cases of the most common cancers in elderly (prostate, colon, lung among men and lung, breast, endometrial among women). The health phenomena taking place in the Polish population requires building a strong tool to monitor and analyze the processes involved. In case of cancer such tool is the cancer registry. In response to this need the National Cancer Registry launched a project aimed at creating a central database of cancers, establishing social baseline of conducting modern epidemiological research using information society technologies.

WSTĘP: Ze względu na znaczenie dla zdrowia publicznego problemu, jakim jest poznanie czynników psychologicznych mogących mieć znaczenie dla przebiegu choroby nowotworowej, zrealizowano badanie, którego celem była ocena czę-stości i uwarunkowań akceptacji choroby u pacjentów z rakiem jelita grubego w okresie bezpośrednio poprzedzającym interwencję chirurgiczną. MATERIAŁ I METODY: W badaniu kwestionariuszowym wzięło udział 200 pacjentów z rozpoznanym rakiem jelita grubego. W celu zidentyfikowania ostatecznych predyktorów mających wpływ na akceptację choroby nowotworowej wykonano analizę regresji logistycznej wraz z weryfikującą parametryzacją za pomocą automatycznej selekcji wstecznej. WYNIKI: Nieco ponad połowa badanych (56,5%) z rakiem jelita grubego w okresie bezpośrednio poprzedzającym interwencję chirurgiczną nie akceptuje swojej choroby. Wpływ na akceptację choroby miały zmienne: występowanie raka jelita grubego w rodzinie oraz zadowolenie z opieki medycznej. WNIOSKI: Częstsza akceptacja choroby nowotworowej jelita grubego występuje u pacjentów deklarujących zadowolenie z opieki medycznej oraz u pacjentów, u których rak jelita grubego występował w rodzinie.
INTRODUCTION: Because of the importance of the problem for public health, which is the knowledge of psychological factors that may affect the course of cancer, a study was carried out to assess the frequency and determinants of disease acceptance in patients with colorectal cancer in the period immediately preceding the surgical intervention. MATERIAL AND METHODS: 200 patients with colorectal cancer participated in the questionnaire study. In order to identify the final predictors influencing the acceptance of cancer, logistic regression analysis was performed along with the verification of parametrization using the automatic backward selection method. RESULTS: Slightly more than half of the respondents (56.5%) with colorectal cancer do not accept their disease in the period immediately preceding the surgical intervention. Among the determinants influencing the acceptance of the disease were the following variables: family history of colorectal cancer and satisfaction with medical care. CONCLUSIONS: More frequent acceptance of colorectal cancer occurs in patients who declare satisfaction with medical care and among patients with a family history of colorectal cancer.

Introduction and objective. The integrity, stability and composition of sperm chromatin are of great importance in the fertilizing potential of male gametes and their capacity to support normal embryonic development. In this study, the author presents the current state of knowledge about the sperm epigenetic profile and risk of cancer. Abbreviated description of the state of knowledge. The obtaining of pregnancy and the state of health of the baby depends on the quality of the genetic material of both the female and the male. Health behaviours and environmental factors directly affect the quality of sperm, as well as the human egg cell and, consequently, on the reproductive capabilities, the course of pregnancy and the state of the newborn. There exist two thoroughly investigated epigenetic modifications: DNA methylation and histone modifications. The process of DNA methylation can be also a fundamental factor contributing to the development of cancer, where epigenotype undergoes significant modifications. When considering numerous DNA aberrations in the male gamete, the most commonly encountered is DNA fragmentation, particularly in infertile subjects. Surprisingly, an intracytoplasmatic sperm injection study of mice oocytes, using spermatozoa with a high DNA Fragmentation Index (DFI), revealed that a considerable percentage of adults born as a result of this method, showed a significant increase in the incidence of abnormal behavioural tests, malformations, cancer and signs of premature aging. Summary. The issue of assisted procreation raises the need to look for an appropriate treatment for males with sperm chromatin abnormalities. As a result, the fight against smoking addiction becomes the obvious necessity. Moreover, the reasonable solution nowadays seems to be supplementation with micronutrients and folic acid. It has been proved that the process of DNA fragmentation is a phenomenon that intensifies over time. Therefore, there should be a pursuance for, as close as possible, to the moment of ejaculation, application of semen to reproductive techniques. Finally, epigenetic changes are suspected of being one of the factors responsible for the deterioration of male sperm parameters observed in recent decades.

Incidence of female breast cancer in our population remains very high and the incidence increase every year. Breast cancer and following demanding oncological treatment extensively influence life of female patients. It intervene biological, psychical and social life of patient. Objective of our research was to check how women are informed about preventive precautions and how they are following them. Our research sample were randomly selected women of all age groups. Our results showed that it is necessary to increase activity in the area of education, increase personal responsibility in area of breasts self-examination and better inform about the influence of obesity to breast cancer incidence. Considering our previous experience in health care we were surprised that the role of nurse in education process is very low. We think that general nurses have enough opportunities to use daily contacts with patients to inform and lecture them.

New diagnostic methods for thyroid diseases are still being searched for. Immunohistochemical diagnosis is expanded by the introduction of new biomarkers including ß-catenin (B-Cat). Associations are indicated between the cellular expression of this biomarker and tumor stage, nodal metastases and the degree of tumor cell differentiation. Reports are scarce regarding the plasma level of this biomarker in malignant neoplastic diseases. The aim of the study was to analyze the plasma B-Cat concentration and the possibility of it use in the diagnostics of patients with nodular goiter and papillary thyroid carcinoma. Material and methods. Plasma B-Cat concentration was determined in 64 patients with goiter and 15 healthy volunteers. The final histopathological examination revealed 41 cases of papillary thyroid carcinoma (PTC) and 13 cases of nodular goiter (NG). Results. A significant increase in B-Cat (p <0.05) in both groups compared to the control group. No differences in the concentrations of biomarker was demonstrated between the PTC and NG groups. After determining the AUC for the tested biomarker, the B-Cat ratio of the area value 0.721 was the strong diagnostic test. Conclusions. Changes in the plasma B-Cat concentration can be the biomarker of thyroid cancer but it cannot be used for the detection of papillary thyroid carcinoma becouse of concomitant tumor-like lesions in the thyroid gland.

The global spread of cancer makes every preventive action aiming at stopping the increase in number of cases and deaths related to cancer extremely important. Preventive actions may be especially effective in diseases etiologically related to lifestyles, including above all eating habits and level of physical activity. It is worth implementing them in compliance with the participatory approach, which constitutes a key component of health promotion and health education. The aim of this article is to present the reasons for and the need to implement prevention based on the idea of participation. This is justified by the results of the research conducted as part of a grant sponsored by Polska Liga Walki z Rakiem [Polish Cancer League] (“Onkogranty II” contest), entitled Promotion of healthy eating habits and physical activity in primary schools throughout the city of Bytom – a diagnosis of educational needs in the context of planning effective actions in primary cancer prevention. It seems that involving students in the process of identifying needs, determining priorities, planning and evaluating these actions is currently one of the most effective ways of developing awareness of cancer prevention.

Opis:
Increase in the occurrence of multiple primary malignant tumours is associated with significant extention of survival rate among patients treated for malignant neoplasm. Treatment of multiple primary malignant tumours demands precise clinical evaluation, imaging, laboratory diagnostics and histopathological evaluation of the lesion. In the presented case immunohistochemical staining of lymphatic metastasis from nasopharyngeal carcinoma was mimicking distant metastasis from poorly differentiated ovarian cancer. It impeded histopathological diagnosis and required additional imaging enriched by PET-CT, which allowed the visualization of the second tumor.

Objective: The aim of the study was comparison of cancer incidence risk of lungs, stomach, colon, bladder and kidneys from ex-miners of black coal mines and the general male population of the Czech Republic. Materials and methods: The analysis was conducted in two cohorts of ex-miners according to the presence of coalworkers’ pneumoconiosis (CWP). The first cohort included the miners without CWP (N = 6705), and the second cohort included the miners who were compensated for CWP (N = 2158). Personal and occupational data was merged with the data in the National Population Register and the National Oncological Register for the period from 1992 to 2006. Cancer risk in miners in comparison to the general male population of the Czech Republic was evaluated by SIR (Standardized Incidence Ratio) and 95% confidence interval (CI). Results: About twice as high risk of lung cancer was found in miners with CWP (SIR = 2.21; 95% CI: 1.75–2.76). Lung cancer risk correlated with the severity of CWP (simple CWP SIR=1.96; 95% CI: 1.48–2.56, progressive massive fibrosis SIR = 4.29; 95% CI: 2.09–7.87). No increased risk of lung cancer was found in ex-miners without CWP. The risk of malignant neoplasm at the other selected sites was comparable with the risk in the general male population of the Czech Republic. Conclusions: This study found increased lung cancer risk in coal miners with CWP, but not in those without CWP, comparing with the general population. These results served as the basis for the inclusion of lung cancer in association with CWP into a new Czech list of occupational diseases.

Gastro-esophageal cancer is a very common malignant tumours arising from the digestive tract. Hypoalbuminemia and weight loss often result from malnourishment. Malnourished patients have a higher incidence of mortality and morbidity causing prolonged hospital stay. To discuss the incidence and effects of Gastro-esophageal cancer related malnutrition and demonstrate different methods to solve this problem. The current review revealed that malnutrition increases the risk of complications and duration of stay in hospital and ICU in GIT cancer patients, as such, accounts for burden on ICU budget.

Introduction: The two major recognized risk factors for laryngeal cancer are tobacco and alcohol consumption. However, according to WHO and IARC, occupational factors are probably underestimated. Confirmed/highly probable occupational carcinogens related to laryngeal cancer include asbestos and strong organic acid mists. Probable carcinogens are cement, polycyclic aromatic hydrocarbons (PAH) and wood dust. Suspected carcinogens are organic solvents, formaldehyde, coal/silica dust, leather dust and cotton dust. An important problem of epidemiological studies appears to be the lack of differentiation between laryngeal and hypopharyngeal (squamous cell) carcinomas. Material and Methods: For the purposes of this study, a database of 164 well-documented cases of laryngeal and hypopharyngeal cancers was created by collecting data from 4 centres in Belgium, Italy and France. In order to identify confirmed, probable and suspected carcinogenic risks, jobs and exposures over the whole career of the patients were exhaustively identified and mapped. Confounding factors were taken in account. Results: In general, occupational exposure to known or suspected carcinogens is significantly higher in laryngeal than in hypopharyngeal carcinomas. As regards the exposed subjects, the global occupational exposure profiles differ significantly (10 categories). Asbestos and acid mist exposures are significantly more important in laryngeal carcinoma. Coal/silica dust exposure is significantly more important in pharyngeal carcinoma..
