There is no doubt that vitamin D plays a crucial role in the maintenance of musculoskeletal system. But the function of this ancient molecule presumably ranges far beyond hormone-like regulation, as it could be generated by simple unicellular organisms. First, we are going to discuss the role of vitamin D as a global regulator of homeostasis from a historical perspective, but later we will focus on current views and its relevance to human physiology and pathology. Three milestones are defining the impact of vitamin D on science and humanity. Firstly, discovery that vitamin D is the cure for rickets, brought us supplementation programs and rapid irradiation of this devastating disease. Secondly, detail description of photoproduction of vitamin D, its subsequent metabolism and interaction with vitamin D receptor VDR, provided mechanistic background for future discoveries. Finally, recent large epidemiological studies provided indirect, but strong evidence that optimal level of vitamin D in serum has beneficial effects on our health and protects us from multiple diseases, including cancer. Furthermore, existence of alternative pathways of vitamin D metabolism and multiple intracellular targets broadens our understanding of its physiological activities and offers new and very promising tools for prophylactics and treatment of many diseases of civilization. Although vitamin D (and its derivatives) should not be regarded as a cure-all for every human disease, its beneficial effects on the human health have to be taken under consideration.

Introduction: Vitamin D deficiency is a worldwide public health problem. The objective of this survey was to assess the undergraduate students’ vitamin D status and knowledge about this vitamin. Materials and methods: An online multi-choice survey was designed and launched in Northern Poland (Gdańsk region). The first part of the survey assessed diet, supplementation, UV radiation exposure (UVE) and general health of respondents. The second part was a vitamin D knowledge test (vitamin D optimal level, deficiency-related diseases). 1766 student volunteers responded to the survey: 369 male and 1397 female. Data was divided according to the respondents’ sex and university affiliation. Appropriate parametric or non-parametric statistical tests were used with statistical significance set at p<0.05. Results: Regular consumption of vitamin D-rich food was high, except for fish (only 18; 22%). High number of participants did not declare any type of supplementation (43; 44%) and only occasionally were exposed to UV (77%; 80%). The most frequently recognized disease linked to vitamin D was osteoporosis. Medical University students obtained higher test scores (4,55), however this did not correlate with healthy vitamin D habits. Conclusion: Undergraduate medical and non-medical students have unsatisfactory vitamin D status and poor understanding of its function and impact on health, which implies the need for changes in the educational program.

Vitamin D plays important, pleiotropic role in the maintenance of global homeostasis. Its influence goes far beyond the regulation of calcium and phosphorus balance, as diverse activities of vitamin D and its natural metabolites assure proper functioning of major human organs, including skin. Recently, we reviewed the current understanding of vitamin D impact on human health from historical perspective (Wierzbicka et al. (2014) The renaissance of vitamin D. Acta Biochim Pol 61: 679-686). This article focuses on its functions in the skin. The skin and its appendages, creates a platform connecting and protecting internal organs against, usually harmful, external environment. It uppermost layer - epidermis in order to maintain a protective barrier undergoes a constant exchange of cornified keratinocytes layer. Its disturbance leads to development of serious skin disorders including psoriasis, vitiligo, atopic dermatitis and skin cancer. All of those dermatopathologies have a huge impact on modern societies, affecting not only the physical, but also mental state of patients as well as their social status. Furthermore, multiple human systemic diseases (autoimmune, blood and digestive diseases) have skin manifestation, thus "condition of the skin" often reflects the condition and pathological changes within the internal organs. In humans, the skin is the natural source of vitamin D, which is produced locally from 7-dehydrocholesterol in photoreaction induced by ultraviolet B (UVB) radiation from the sun. It is also well established, that the process of proliferation and differentiation of keratinocytes is tightly regulated by calcium and the active form of vitamin D (1,25(OH)2D3). Thus, the skin physiology is inseparably connected with vitamin D production and activity. Unfortunately, UVB, which is required for vitamin D production, is also known as the main cause of a skin cancer, including melanoma. Here, we are going to review benefits of vitamin D and its analogues in the maintenance of epidermal barrier and its potential use in the treatment of common skin diseases.

In recent years, a significant increase in the incidence of cancer and neurodegenerative diseases has been observed. This is mainly because of the higher life expectancy and thus prolonged contact with risk factors, inadequate diet and lack of physical activity. Along with aging processes, the prevalence and incidence of many diseases rise dramatically. New drugs and solutions are constantly being sought that would help in particular preventing. One way could be the use of vitamin D or its analogues, which through their proven pleiotropic action, have become the focus of the interest of many scientists in the last decade. One might think that the problem of vitamin D deficiency should affect a small part of the population, due to health campaigns carried out and the availability of many OTC preparations with vitamin D in various forms. Research, however, confirms that the need for vitamin D supplementation is overlooked by healthcare professionals, and its deficiencies are widespread worldwide and affect all ages. It is very meaningful problem. It is important to check the concentration of this vitamin in the blood, which in the case of low levels can be correlated with potential health problems. The influence of vitamin D on physiological processes, confirmed by numerous studies and scientific publications, and thus also the prevention of numerous diseases, is an important argument indicating the need for patient education on how to take it regularly. For this reason, preventive actions carried out by professional medical staff, including pharmacists in community pharmacies, regarding awareness of the important role of vitamin D and its impact on our health are important. Patient counseling may have a significant effect on improving quality of life. This article discusses some mechanisms of antiproliferative, neuroprotective and immunomodulatory activity of active vitamin D derivatives on the example of selected cancers, Alzheimer's disease, and multiple sclerosis.
W ostatnich latach obserwuje się znaczny wzrost zachorowań na nowotwory i choroby neurodegeneracyjne. Wciąż poszukuje się nowych leków i rozwiązań, które pomogłyby zwłaszcza w zapobieganiu tym chorobom. Jednym ze sposobów może być stosowanie witaminy D lub jej analogów, które poprzez swoje udowodnione wielokierunkowe działanie, stały się w ostatniej dekadzie przedmiotem zainteresowania wielu naukowców. Można by pomyśleć, że problem niedoboru witaminy D powinien dotyczyć niewielkiej części populacji, ze względu na przeprowadzane kampanie zdrowotne i dostępność wielu preparatów OTC z witaminą D w różnej postaci. Badania potwierdzają jednak, że potrzeba suplementacji witaminy D jest pomijana przez pracowników służby zdrowia, a jej niedobory są powszechne na całym świecie i dotykają wszystkich grup wiekowych. Duże znaczenie ma sprawdzanie stężenia tej witaminy we krwi, co w przypadku niskiego poziomu można skorelować z potencjalnymi problemami zdrowotnymi. Potwierdzony w licznych badaniach i publikacjach naukowych wpływ witaminy D na procesy fizjologiczne, a przez to także na zapobieganie licznym chorobom, jest istotnym argumentem wskazującym na potrzebę edukacji pacjentów na temat jej regularnego przyjmowania. Z tego powodu istotne są akcje profilaktyczne prowadzone przez fachowy personel medyczny, w tym także przez farmaceutów w aptekach ogólnodostępnych, dotyczące uświadamiania pacjentom ważnej roli witamy D i jej wpływu na nasze zdrowie. W niniejszym artykule omówiono niektóre mechanizmy działania antyproliferacyjnego, neuroprotekcyjnego i immunomodulującego aktywnych pochodnych witaminy D na przykładzie wybranych chorób nowotworowych, choroby Alzheimera oraz stwardnienia rozsianego.

Objectives Aim of this project is determination of the correlation between the level of vitamin D in blood serum and duration of pregnancy in population in central Poland. Material and Methods 25-Hydroxyvitamin D (25(OH)D) level was determined in blood serum, using enzyme-linked immunosorbent assay (ELISA). Standardized history of each patient was recorded. The history included: general medical history, data regarding the course of pregnancy and information about health-related behavior that could influence vitamin D concentration. Two hundred-and-one Caucasian women at childbirth were qualified into the study. The study group was divided into 2 parts: 100 patients who had a spontaneous premature birth and 101 patients who had birth at full term. Results Vitamin D deficiency (< 30 ng/ml) was very common for both groups (69.6% of patients in the premature group and 72% – in the control group). Patients who had a premature birth had severe vitamin D deficiency (less than 10 ng/ml) more often than in the control group (34% vs. 14.2%, p = 0.001). Severe vitamin D deficiency increased the risk of premature birth but the association was not statistically significant in the multivariate regression model (odds ratio (OR) = 2.47, 95% confidence interval (CI): 0.86–7.15, p = 0.094). Conclusions Severe vitamin D deficiency (< 10 ng/ml) may be the factor increasing the risk of preterm birth. Int J Occup Med Environ Health 2017;30(6):933–941

ObjectivesContrary to popular opinion on the preventive properties of vitamin D, results of previous studies have been inconclusive. The aim of this research was to evaluate the associations between the intake of vitamin D and metabolic abnormalities in a representative sample of Polish adults.Material and MethodsWithin the framework of the Multi-Center National Population Health Examination Survey (referred to as WOBASZ), a random sample of 2381 adult residents of Poland (53.8% of whom were women) was examined. All the study subjects were extensively reviewed, including 24-h dietary recall. The intake of vitamin D was assessed on the basis of dietary and supplements reviews. Metabolic abnormalities were evaluated using measurements of waist circumference (WC), blood pressure (BP), serum triglycerides (TG), high-density lipoprotein cholesterol (HDL-C) and fasting glycemia. Metabolic syndrome was defined according to the International Diabetes Federation.ResultsOf all the study participants, about 4.4 % of women and 2.6% of men declared a regular supplementation of vitamin D. Among women, a significant inverse correlation between vitamin D supplementation and the mean systolic BP was found (p < 0.01). A more substantial relationship was noted after dividing the study subjects according to their body mass. Among obese men, there was a significant inverse relationship between vitamin D intake and the mean systolic BP (p < 0.01) and diastolic BP (p < 0.05), as well as a positive correlation with HDL-C (p < 0.05). Among obese women, a negative correlation was found between vitamin D supplementation and the mean systolic BP (p < 0.01) and diastolic BP (p < 0.05), and a positive correlation with TG (p < 0.05). Among non-obese male subjects, a negative correlation between vitamin D intake and WC was observed.ConclusionsThe obtained findings suggest that the correlation between vitamin D intake and metabolic abnormalities may depend on the obesity status. A higher vitamin D intake may reduce BP and increase HDL-C in obese subjects. The positive relationship between vitamin D intake and TG concentration in women needs further investigation.

Introduction. Reports continue to show that a significant association exists between serum vitamin D level and metabolic syndrome (MS)-associated inflammation. However, information on the serum levels of vitamin D and alterations in inflammation in different vitamin D status is presently lacking. Aim. To determine the serum levels of vitamin D and TNF-α, and assess their possible relationship with gender in individuals with MS. Material and methods. Sixty adults with MS and 40 controls were enrolled into this case-control study. Serum vitamin D and TNF-α levels were measured and participants stratified into different vitamin D status. Results. None of the participants had vitamin D deficiency and the mean vitamin D level was similar in MS compared with the controls. However, TNF-α level was significantly higher in MS compared with the controls. Serum vitamin D level had significant inverse correlation with serum TNF-α level in MS. Also vitamin D level was significantly lower while TNF-α level was significantly higher in female-MS compared with the male-MS. Conclusion. Adults with MS have elevated TNF-α level which appears to be associated with the serum level of vitamin D. Also, females with MS have low vitamin D level and this may exacerbate the MS-associated inflammation in them.

Objectives To assess the association between night work and 25-hydroxy vitamin D (25OHD) levels, and to evaluate effect of potential risk factors on 25OHD levels. Material and Methods A total of 140 adult Jordanian employees were recruited. Demographic, lifestyle and working patterns data were documented through a well-structured questionnaire. Vitamin D status was assessed by measuring circulating concentrations of 25OHD. Results Mean 25OHD level was 23.8 ng/ml. No significant difference was found in 25OHD levels between the summer and winter (p = 0.46), or between males and females (p = 0.35). The female night workers had significantly lower serum 25OHD levels compared to the female day workers (p = 0.01). No significant difference in serum 25OHD levels was found between the night and day male workers (p = 0.25). The number of night shifts/month was negatively correlated with 25OHD levels in both the males and females (p = 0.01 and p = 0.007, respectively). Age was positively correlated with 25OHD levels in both the males and females (p = 0.02 and p = 0.001, respectively). Body mass index was negatively associated with 25OHD levels in the whole sample (p = 0.03), but not within each gender group (p = 0.21 for the males and p = 0.09 for the females). Smoking had no significant association with 25OHD levels (p = 0.99 for the males and p = 0.22 for the females). Conclusions Our results suggest that women working night shifts are at higher risk of 25OHD deficiency, and, consequently, of other health problems linked to 25OHD deficiency. In recent years, attention has been focused on pleiotropic directions of effects exerted by vitamin D. Epidemiological data indicate that deficiency of vitamin D in various population groups represents an increasingly widespread phenomenon, while a decreased serum concentration of calcitriol correlates with manifestation of civilization-linked diseases, including visceral obesity. This study aims at a review and synthesis of data linked to relationships between lowered vitamin D concentrations in blood and manifestation of obesity, and potential mechanisms which affect the concentration of the vitamin in conditions of an excessive accumulation of adipose tissue. Several variables are distinguished which can affect the status of vitamin D in obesity, but the key role in this respect is ascribed to the metabolic activity of visceral adipose tissue. Among others, the activity favours sequestration and modulation of calcitriol turnover. On the other hand, the effects of vitamin D on the process of adipogenesis and its involvement in remodelling of adipose tissue are pointed out. Also, several factors of an environmental nature (e.g. time of year/day, dietetic supply of vitamin D), genetic nature (e.g. genetic polymorphisms) and other conditioning (e.g. coexisting diseases, age, content of melanin in skin) cannot be bypassed as they may affect the concentration of vitamin D. Nevertheless, it still remains unresolved to what extent hypovitaminosis D represents the cause and to which it is the effect of obesity.

Background: Nutrition influence on cystic fibrosis (CF) patients survival is well documented and dietary therapy is one of basic elements of their treatment. Prolonged survival of CF patients might yet emerge comorbidities, which include bone mineral disease. The aim: The assessment of the dietary pattern and its relationship between bone density in boys and girls with cystic fibrosis. Material and methods: 89 patients aged 10-18 years from 3 Polish CF Centres were included into the study. To obtain a knowledge about quality of diet, a 3-day food record was assessed and percent of recommended for CF patients intake of energy, protein, carbohydrates, fat, vitamin D3, calcium, phosphorus was counted. Bone mineral density (BMD) in lumbar spine (L1-L4) was measured and expressed as a Z-score. To assess nutritional status, anthropometric measurements was evaluated (body weight, height and BMI). Descriptive methods, Mann-Whitney test, T-Student test, Spearman correlation and one-way ANOVA were used for statistical analyses. Results: The patients with cystic fibrosis did not meet specific for CF nutritional guidelines. A deficiency in recommended intake was observed in energy (88%), protein (82%), calcium (78%) and vitamin D3 (71%). The intake of phosphorus was higher than recommended (142%). A nutritional status was significantly reduced, as compared with the reference group (p<0.001). Boys characterized significantly lower body weight (p=0.019) and height (p=0.036) than girls as well as worse caloric (p=0.023) and carbohydrates intake (p=0.005). However, girls had reduced vitamin D3 content in their diet (p<0.001). The bone mineral density in the whole group was reduced and Z-score amounted to -0.95±1.17. Tendency to decreasing of BMD with age was observed. BMI showed important correlation with bone mineral density both in girls (p<0.001) and in boys (p=0.020). Conclusion: CF patients do not follow specific for them dietary recommendations and essential differences were observed between girls and boys. Nutritional status (BMI) showed correlation with bone mineral density in CF patients. Therefore intensive nutritional therapy according to recommendations is needed.
Wprowadzenie: Wpływ żywienia na długość życia chorych z mukowiscydozą (ang. Cystic Fibrosis − CF) jest dobrze udokumentowany, a postępowanie dietetyczne jest jednym z podstawowych elementów leczenia pacjentów z CF. Systematyczne wydłużanie się wieku przeżycia chorych może powodować ujawienie się niedoborów żywieniowych prowadzących do chorób towarzyszących, jakimi są m.in. zaburzenia w gospodarce mineralnej kości. Cel: Celem pracy była ocena sposobu żywienia dziewcząt i chłopców z mukowiscydozą i jego związku z gęstością mineralną kości. Materiał imetody: Do badania włączono 89 pacjentów wwieku 10-18 lat. Wcelu ustalenia jakości diety, oceniono sposób żywienia za pomocą zapisu 3-dniowych jadłospisów, obliczając procent realizacji zapotrzebowania zgodnie z rekomendacjami opracowanymi dla pacjentów z mukowiscydozą. Dotyczyło to pokrycia zapotrzebowania kalorycznego, białka, tłuszczu, węglowodanów, witaminy D3, wapnia i fosforu. Oceniono gęstość mineralną kości (Bone Mineral Density − BMD) w odcinku lędźwiowym (L1-L4). Wynik wyrażono, jako wartość standaryzowaną Z-score. W celu określenia stanu odżywienia dokonano pomiarów antropometrycznych (masa i wysokość ciała oraz BMI). Wykonano następujące obliczenia statystyczne: analizy opisowe, test Manna-Whitneya, test t-Studenta, korelację Spearmana, oraz jednoczynnikową analizę wariancji ANOVA. Wyniki: Badani pacjenci nie spełniali opracowanych dla chorych na mukowiscydozę zaleceń żywieniowych. W diecie zaobserwowano niedobory energii (88% normy), białka (82%), wapnia (78%) oraz witaminy D3 (71%). Wykazano nadmierne spożycie fosforu (142%). Stan odżywienia (parametry antropometryczne) badanej grupy był obniżony i w sposób statystycznie istotny różnił się od grupy referencyjnej (p<0,001). Chłopcy charakteryzowali się znacznie niższą masą ciała (p=0,019) iwysokością (p=0,036) niż dziewczęta, a także niższą kalorycznością diety (p=0,023) oraz zawartością węglowodanów (p=0,005). Dietę dziewcząt cechowała mniejsza zawartość witaminy D3 w diecie (p<0,001) w porównaniu do chłopców. Średnia wartość gęstości mineralnej kości w całej grupie była obniżona i wynosiła -0,95±1,17. Zaobserwowano tendencję do obniżania się BMD wraz z wiekiem. Stan odżywienia wyrażony jako BMI wykazał statystycznie istotną korelację z gęstością mineralną kości, zarówno u dziewcząt (p<0,001), jak i u chłopców (p=0,020). Wnioski: Pacjenci z CF nie przestrzegali opracowanych dla nich zaleceń dietetycznych. Zaobserwowano istotne różnice między dziewczętami i chłopcami. Stan odżywienia (BMI), wykazał korelację z gęstością mineralną kości (BMD), co wymaga intensywnej terapii żywieniowej, zgodnie z opracowanymi rekomendacjami.

RESEARCH OBJECTIVE: Perform a review of protein and vitamin D supplementation in Sarcopenia. THE RESEARCH PROBLEM AND METHODS: Demographic change implies new demands for public health, with health promotion and prevention activities seeking to delay the appearance of common diseases in elderly population. We performed a review of protein and vitamin D supplementation in Sarcopenia. THE PROCESS OF ARGUMENTATION: Changes in body composition occur during the aging process, resulting in increased body fat and reduced muscle mass and also bone mass. Sarcopenia is defined by a syndrome characterized by generalized and progressive loss of muscle mass and strength. After 50 years, there is a reduction between approximately 1% and 2% of muscle mass per year. RESEARCH RESULTS: Currently progressive resistance training is the most well-established intervention, and dietary interventions such as protein and vitamin D also mentioned in literature. Leucine is a branched chain amino acid knows as key role in muscle protein synthesis. The consumption of leucine-enriched amino acids showed beneficial effects in the elderly contributing to the conservation of skeletal muscle mass. Vitamin D has pre-hormone functions, vitamin D deficiency is related with diffuse musculoskeletal pain, myopathy, sarcopenia and falls. Vitamin D supplementation of 800 to 1000 IU/day is associated with increased muscle strength in the elderly. CONCLUSIONS, INNOVATIONS AND RECOMMENDATIONS: The protein and vitamin D supplementation are strategies that are proving to be important for the prevention and treatment of sarcopenia, especially when associated with resistance training.

The most common public health problems include, among others, overweight, obesity, and cardiovascular diseases. In addition, the number of people with thyroid disturbances is still growing. Thyroid abnormalities can lead to many metabolic dysfunctions, including secondary osteoporosis, alterations in body mass, lipid profile, and insulin resistance. Recently, the studies have been focused on the connections between thyroid gland function, obesity, metabolic syndrome and cardiovascular diseases, as well as bone turnover. Fatty tissue plays an important role in whole body homeostasis. Adipose tissue hormones, such as leptin, resistin and adiponectin are proteins having immunomodulatory lproperties, and their balance is needed to control immune response, as well as inflammation processes. The following article constitutes a review of literature concerning thyroid function with regard to adipocytokines and vitamin D, as well as the influence of this gland on the skeletal system. For this purpose, Medline Pub Med base and Google Scholar were used. All the citied studies in this review article underline how much should be done to achieve more efficient treatment of thyroid disorders, specifically, how to prevent its complications, for instance, osteoporosis, over-weight, obesity or cardiovascular diseases.

INTRODUCTION: Vitamin D, which is a fat-soluble vitamin, plays a key role in enhancing the intestinal absorption of calcium, magnesium and phosphate. In severely ill patients, vitamin D can adversely affect immune and metabolic functions, contributing to poorer outcomes. The aim of this study was to correlate vitamin D with mortality in critically ill patients. MATERIALS AND METHODS: prospective observational study was conducted, involving 162 patients in an intensive care unit (ICU). 162 patients were divided into two groups according to vitamin D Deficiency Group levels ≤ 20 ng/ml and Non vitamin D deficiency group levels <20 ng / ml and non-vitamin D deficiency group B levels > 20ng/ml. Data collected during the study included the APACHE II (acute physiology and chronic health evaluation) score at ICU admission, SOFA (sequential organ failure assessment) scores throughout the ICU stay, the need for mechanical ventilation, inotropic support, length of stay in ICU, and ICU outcomes, which were classified as either discharge or mortality. RESULTS: Of the 162 patient admitted to ICU, the prevalence of vitamin D deficiency in this study was 140 (86.4%) and nondeficient 22 (13.6%). The mortality rate in the vitamin D deficient group was 40% compared to 18.18% in the nondeficient group. The difference in mortality in both groups for mortality was statistically significant (p-value < 0.05). Vitamin D deficiency was not associated as an independent risk factor for ICU mortality [Odds ratio (OR) 1.220, 95% CI (0.825- 1.805) (p-value -0.320)]. CONCLUSIONS: The vitamin D-deficient group had a significantly higher mortality rate compared to the patient in the nondeficient group. But vitamin D deficiency was not found to be an independent risk factor for mortality.

Introduction and objective. Since the role of vitamin D is essential in numerous biological processes its deficiency was suggested to be a risk factor for e.g. osteoporosis, musculoskeletal pain and spine pain. The purpose of the study was to analyse whether serum vitamin D concentration is related to pain involving the motor system in Polish postmenopausal women working in agriculture. Material and methods. The study group consisted of 1,751 post-menopausal women, aged 45–65, at least 12 months from the last menstrual period, living in rural areas and working in agriculture. The research method was self-assessment of pain involving the motor system using VAS, laboratory test of serum vitamin D concentration and a medical interview. Statistical methods included generalized linear models, analysis of variance, t test for two means in two independents, χ2 test of stochastic independence. Results. Postmenopausal women working in agriculture and suffering from pain in at least one part of the motor system were younger and lower educated, they also had higher abdominal obesity and lower serum vitamin D, compared to those without pain in any part of the motor system. Decreased serum vitamin D concentration in postmenopausal women working in agriculture is important from the aspect of a higher prevalence of pain in the thoracic spine and more severe pain in the neck spine, but not for severity of pain in the lumbar spine; higher occurrence of pain in both hands or wrists; higher prevalence and more severe pain in at least one knee; and no prevalence or severity of pain in the shoulders and elbows. Conclusion. Serum vitamin D concentration is important for the prevalence and severity of pain in the neck and thoracic spine, knees and hands or wrists, but not for the lumbar spine, shoulders and elbows.

Vitamin D is a compound responsible for maintaining mineral homeostasis. It protects against calcium and phosphate deficiency through the effects on the intestine, kidney, parathyroid gland and bone. All mechanisms that help maintain mineral homeostasis of the body are regulated by the vitamin D hormonal form - calcitriol. Synthesis of vitamin D starts in the skin as a non-enzymatic process, which begins during exposure to sunlight, when the absorption of ultraviolet B (UVB) radiation results in convertion of 7-dehydrocholesterol, a metabolite of cholesterol that is stored in the skin, to precholecalciferol (previtamin-D3) that is immediately converted into cholecalciferol (vitamin D3). After the skin synthesis cholecalciferol is transported to the liver where it undergoes hydroxylation, what results in formation of calcidiol (25(OH) D3). The second metabolic process takes place in the kidney, where calcidiol undergoes hydroxylation at the C-1 position to the hormonal, the most active metabolite - 1,25-dihydroxyvitamin D (calcitriol). Vitamin D deficiency may result in bone diseases, such as rickets in children and osteomalacia and osteoporosis in adults. Symptoms of osteomalacia affect mainly the skeletal system and are similar to that observed in rickets. It concerns thoracic kyphosis, pelvis deformities and also the varus knee. Osteoporosis is another condition that is related to abnormalities of mineral homeostasis. It is characterized by the progressive loss of bone mass, impaired bone microarchitecture, and consequently increased fragility and susceptibility to fracture. For the last several years other, non-classic actions of vitamin D3 have been discussed. It was engendered by the discovery of vitamin D3 receptor (VDR) in the most of body tissues and cells. Hence, there are many hypotheses which suggest the inverse relationship between vitamin D status and various diseases, such as cancer, autoimmune diseases, diabetes mellitus and others.
Witamina D jest związkiem chemicznym odpowiedzialnym za utrzymanie homeostazy mineralnej organizmu. Poprzez wpływ na jelita, nerki, przytarczyce i kości zapobiega niedoborowi wapnia i fosforanów. Aktywną formą witaminy D3 o właściwościach hormonalnych jest kalcytriol, który odpowiada za utrzymanie homeostazy mineralnej organizmu. Pierwszy etap syntezy witaminy D zachodzi w skórze, pod wpływem ekspozycji na światło słoneczne (UVB). Polega on na nieenzymatycznej przemianie 7-dehydrocholesterolu do prowitaminy D (pre-D3), która natychmiast ulega przekształceniu do cholekalcyferolu (witaminy D3). Wyprodukowana w skórze witamina D3 jest następnie transportowana do wątroby, gdzie ulega hydroksylacji, w wyniku której powstaje kalcydiol (25(OH)D3). Drugi proces metaboliczny zachodzi w nerkach, gdzie kalcydiol ulega hydroksylacji w pozycji C-1 do hormonalnie najbardziej aktywnego metabolitu witaminy D - 1,25-dihydroksywitaminy D (kalcytriol). Niedobór witaminy D może prowadzić do chorób kości, takich jak krzywica u dzieci oraz osteomalacja i osteoporoza u osób dorosłych. Objawy osteomalacji dotyczą głównie układu kostnego i są zbliżone do tych obserwowanych w krzywicy. Są to m.in.: kifoza piersiowa, deformacje miednicy i szpotawość kolan. Osteoporoza to choroba, która także związane jest z zaburzeniem homeostazy mineralnej. Charakteryzuje się postępującą utratą masy kostnej, uszkodzeniami mikroarchitektury kości, i w konsekwencji zwiększoną ich kruchością i podatnością na złamania. Na przestrzeni ostatnich lat pojawiły się doniesienia dotyczące innych, nieklasycznych działań witaminy D3. Stwierdzono obecność receptora witaminy D (VDR) w wielu tkankach organizmu, które nie biorą udziału w utrzymaniu homeostazy wapniowo-fosforanowej. Świadczy to o wielokierunkowym działaniu tego związku. Pojawiło się wiele hipotez sugerujących związek między niedoborem witaminy D a występowaniem wielu różnych chorób, takich jak: nowotwory, choroby autoimmunologiczne, cukrzyca i inne.

Witamina D jest hormonem plejotropowym ważnym dla prawidłowego funkcjonowania układów wielonarządowych. Niedobór witaminy D jest bardzo powszechny oraz wiąże się z różnorodnymi przewlekłymi schorzeniami w populacji ogólnej. Do tej pory najlepiej udokumentowanym związkiem z niedoborem witaminy D jest występowanie chorób układu kostnego np. krzywicy u dzieci oraz niemowląt. Jednak istnieją dane sugerujące, że niedobór witaminy D wpływa również na zwiększenie ryzyka wielu innych chorób np. insulinooporności, cukrzycy typu 2, chorób układu krążenia oraz różnych typów nowotworów. Niniejszy przegląd ma na celu podsumowanie aktualnej bazy wiedzy na temat niedoboru witaminy D w różnych jednostkach chorobowych.
Vitamin D is a pleiotropic hormone important for the proper functioning of multi-organ systems. Vitamin D deficiency is very common and is associated with a variety of chronic conditions in the general population. So far, the best documented association with vitamin D deficiency is the occurrence of skeletal diseases, e.g. rickets in children and infants. However, there are data suggesting that vitamin D deficiency also increases the risk of many other diseases, such as insulin resistance, type 2 diabetes, cardiovascular disease and various types of cancer. This review aims to summarize the current knowledge base on vitamin D deficiency in various diseases.

This study was conducted for investigating the effects of chronically administered vitamin D3 and resveratrol, which has antioxidant properties, on memory and learning impairment caused by streptozotocin-induced diabetes. Saline was administered to the mice in the first group. Streptozotocin (60 mg/kg, i.p.) was given to the mice in the second group for 3 consecutive days and no treatment was performed. Vitamin D3 (800 IU/kg, i.p.) was given to the streptozotocin-induced hyperglycemic mice in the third group for 4 weeks. Resveratrol (40 mg/kg, i.p.) was given to the streptozotocin-induced hyperglycemic mice in the fourth group for 4 weeks. Then, passive avoidance test was applied to the mice. A significant difference was found between DM+Vit D3 group (21.6±1.7, n=7) and diabetic control group (28±2.6, n=7) in the acquisition test (p<0.001). A similar result was observed between diabetic-resveratrol group (23.7±2.5, n=7) and the untreated diabetic mice group (28±2.6, n=7). There was a significant difference. 4-week vitamin D3 (400 IU/day, i.p.) and 4-week resveratrol (40 mg/kg, i.p.) treatments given to streptozotocin-induced diabetic mice were observed to significantly improved the passive avoidance acquisition and retention times. These results show the positive effects of vitamin D3 on memory.

Vitamin D plays a role not only in the remodeling of bone, but also in functions involving other organs. Its tissue availability depends on dermal synthesis and/or ingestion from diet, as well as on the amount of adipose tissue binding vitamin D into its cells. This study evaluates vitamin D concentrations in relation to body-mass index (BMI) in healthy people aged over 60 and living in the Subcarpathian Province of southeastern Poland. The study involved 246 participants – 172 women (69.9%), 74 men (31.1%), with an average age of 6.9 (±11.9). The mean concentrations of vitamin D, assessed with ECLIA method, was 18 ng/ml, which indicates a marked deficiency in vitamin D. Defined vitamin D deficiency (<20ng/ml) was noted in 62% of participants without any difference between males and females. There was no correlation between BMI and vitamin D deficiency in the studied group. The results suggest the need for vitamin supplementation in healthy people aged over 60 according to the guidelines.

Vitamin D Binding Protein (DBP) is a multifunctional protein which main role is to carry vitamin D and its metabolites, but it also acts as an actin scavenger and is the precursor of the macrophage activating factor molecule (GcMAF), which has reported highly promising results against cancer, HIV, and neurological disorders including autism, Alzheimer disease, Chronic Fatigue Syndrome (CFS), among others. DBP leads to the formation of GcMAF due to the loss of the O-glycosylated oligosaccharide moiety of the peptide by glycohydrolysis mediated by T and B cells. Some of the current noticed diseases have got increased levels of α-N-acetylgalactosaminidase (Nagalase), a molecule that deglycosylates DBP so it cannot drive to GcMAF, leading to immunosuppression. In this review we take a close look at the state of art strategies and trials using GcMAF as well as the controversies that have emerged during the last decade with this ‘polemic’ molecule.

Introduction: Growing population of elderly requires effective medical diagnostics and help. Criteria developed by Fried et al. are most often used for the diagnosis of the frailty syndrome. An inherent element of frailty syndrome is malnu-trition. Malnutrition results from inadequate food supply, coexistence of acute and chronic diseases. Effective nutritional interventions conducted on frail older persons can prevent them from developing the frailty syndrome. Review: The prevalence of frailty in elders is 17% moreover 42.3% are prefrail. There are many scales designed to identify frailty syndrome, but the most common is the classification of Fried et al. Malnutrition is a common state in frail elderly. Basic questionnaire, which is used to diagnose malnutrition, is MNA (Mini-Nutritional Assessment). Mini-Nutritional Assessment Short-Form (MNA-SF) and Malnutrition Universal Screening Tool (MUST) also can be used. Sarcopenia, which is defined as reduced muscle mass and strength and impaired muscle performance, significantly contribute to the development of frailty. Many studies have shown that an effective method in the preventing of sarcopenia is protein supplementation. Other beneficial lifestyle and diet changes, which can help prevent the development of frailty syndrome, are adherence to the Mediterranean diet, appro-priate intake of carotenoids, vitamin E, selenium and zinc. Another important protective factor is vitamin D levels. Low serum 25(OH)D is strongly associated with frailty. Conclusions: From a nutritional point of view adherence to a Mediterranean-style diet, sufficient intake of protein, micronutrients and vitamin D, as well as regular moderate physical activity, can be crucial in the preventing of the frailty syndrome.

Children with thalassemia major generally experience growth retardation. One of the growth parameters is serum vitamin D levels. The main source of vitamin D comes from endogenous synthesis with sun exposure and dietary sources. This cross sectional study involved 84 children with thalassemia major aged 4-14 years old taken through concecutive sampling. Nutritional intake was obtained through a semi quantitative food frequency questionnaire, while the duration of sun exposure was calculated at peak UVB intensities at 11.00-15.00. The enzyme-linked immunosorbent assay (ELISA) method was used to examine vitamin D levels. The data was analized by Spearman Rank corelation dan multiple linier regresion. The results were 4.8% of subjects had vitamin D insufficiency and 95.2% had deficiency. Daily average intake of nutrients compared to their requirements were energy intake 70.5% (1538.7 Kcals), protein 55,4 % (45.5 grams), fat 79,2% (57.7 grams), carbohydrates 68,6% (204.9 grams) and vitamin D 15,8% (2.4 µg). The mean (SD) of sun exposure was 158.3 (91.2) minutes. Energy, protein, and fat intake were significantly associated with vitamin D levels (r = 0.27; 0.31; 0.38 and p <0.025). Sun exposure was strongly associated with vitamin D levels (r = 0.80; p <0.025), while vitamin D intake was associated with vitamin D levels (p> 0.025). Fat intake and sun exposure have a strong effect on vitamin D levels, with a regression equation of vitamin D levels = 5.62 + 0.22 * fat intake + 0.12 * sun exposure.

Introduction: Postoperative hypocalcemia is a narrow but significant problem for patients undergoing thyroid and parathyroid surgery. It is the most common complication after thyroidectomy. It is associated with transient or permanent hypoparathyroidism. It could potentially be life-threatening for patients and increases the costs of hospitalization. The aim of the study was to evaluate the results of studies that routinely administrated calcium and/or vitamin D during the postoperative period. Materials and Methods: In this article, a literature review – 15 studies that used routine perioperative calcium (7 studies), vitamin D (2 studies) and calcium with vitamin D (11 studies) supplementation was performed. Supplementation effectiveness in prevention of postoperative hypocalcemia was compared to no prophylaxis in 10 studies. Five studies compared the effect of combined administration (calcium and vitamin D) to calcium alone. The number of papers dealing with this problem is not particularly high. Results: Supplementation significantly decreased the rate of laboratory and symptomatic hypocalcemia. It was also effective in reducing the severity of symptoms. The combination of calcium with vitamin D was the most effective strategy. No hypercalcemia or parathyroid hormone inhibition was observed in the supplemented groups. Routine supplementation was less expensive than performing laboratory tests in the course of treatment of hypocalcemia. Conclusions: The results of analyzed studies showed the clinical and economic advantage of routine perioperative prophylactic supplementation of vitamin D and/or calcium as compared to no prophylaxis. However, the majority of studies showed a significant range of variability in patients’ characteristics. Numerous studies did not evaluate the preoperative 25-hydroxycholecalciferol level – a risk factor for postoperative hypocalcemia. Discussion: The use of routine prophylactic supplementation of calcium and vitamin D in the perioperative period can be useful in everyday clinical practice. Further research is needed to draw clear guidelines regarding prophylactic calcium and vitamin D therapy for patients after thyroidectomy.

In the twenty-first century, we are constantly facing allergic diseases, autoimmune diseases and cancers. Despite the genetic predisposition to the above-mentioned diseases, there is a possibility of supporting the organism with substances with immunomodulatory properties, such as vitamin D. Although it has been known so far mainly for the regulation of calcium and phosphate metabolism, its role in supporting the immune system is being discovered. The evidence for this is the demonstration that genes regulated by vitamin Dencode proteins involved in the processes of cell differentiation, proliferation and apoptosis and the detection of vitamin D receptors in cells of the immune system – thymus, B and T lymphocytes and bone marrow. Vitamin D occurs in foods of animal origin in the form of cholecalciferol, and in plant products in the ergocalciferol form. Cholecalciferol can also be obtained through the synthesis by human skin under the influence of UVB radiation. In Poland, there is a common deficiency of vitamin D – during summer, on average, 50% of the population is deficient, while in the winter, this percentage increases to 80%. The deficiency may predispose to many diseases. The objective of this paper was to examine the potential impact of vitamin D in terms of preventing and/or improving selected autoimmune and allergic diseases. Clinical studies in the scientific literature have been analyzed to determine the influence of vitamin D supplementation on the prevention and/or course of cancer in adults, autoimmune diseases in adults, and allergic diseases in children and adults. The conclusions are based on a meta-analysis of 8 clinical studies and 1 in vitro study on cell culture. The results of the study show that the compensation of 25(OH)D deficiency results in a milder course of systemic lupus erythematosus and Hashimoto’s disease. The supplementation of at least 1,200 IU of vitamin D reduces the incidence of cancer by an average of 35% among adults at risk compared to people without supplementation. The recommended intake of vitamin D by pregnant women also reduces the risk of allergic diseases in children from age 1 to 10. Moreover, vitamin D (2000 IU) supplementation in adults suffering from asthma supports a milder course of asthma and reduces the incidence of upper respiratory tract infections compared to the group not using supplementation.
