Opis:
The lap joint welding of Al 3003 alloy by stationary shoulder friction stir welding (SSFSW) was performed under the conditions of tool rotation and welding speed, and it was confirmed that the welding was performed under all conditions. The tunnel defects and pores were formed in the weld zone at the lowest tool rotation and welding speed, and it is increased, the weld surface has been improved. At the same tool rotation speed at the welding speed is increased, the grain size was refined in the stir zone (SZ) and thus the hardness increased by about 14% compared to the base metal. The tensile shear strength is measured to be 10 kN or more under most conditions, and in the 4000 rpm with high heat input, the shear tensile strength was measured relatively lower than other conditions due to excessive heat input of the material.

Opis:
Crossminton is a sport that combines elements of squash, badminton and tennis, creating a new sport, which is currently being played all over the world. The aim of our study was to identify and compare the bilateral differences in the internal and external rotation of the glenohumeral joint of crossminton players. The subject of our study were 13 crossminton players from Slovak Republic (25.1±9.3 years; 172.8±9.1 cm; 71.8±16.1 kg). The range of motion of internal (IR-ROM) and external rotation (ER-ROM) of the glenohumeral joint was measured bilaterally using a standard two-arm goniometer. Bilateral comparison, decimal and sport age pairwise comparisons were carried using the T-test for independent samples. Results point to the occurrence of glenohumeral joint adaptation. Given the bilateral differences, the dominant shoulder has lower IR-ROM (p < 0.05) and higher ER-ROM (p < 0.05) than the non-dominant shoulder. GIRD (IR difference > 13 °) was observed in 4 players. The decimal age of the players did not have a significant impact on the rotation values of the glenohumeral joint. Only IR-ROM of the dominant upper limb was lower with the growing sport age of the players (p < 0.05). Given the fact that adaptations in ROM of glenohumeral joint promote an increased injury risk, the present study reveals the relevance of monitoring and finding differences between upper limbs in crossminton players.

Opis:
Ubytki kostne głowy kości ramiennej i panewki, predysponują do kolejnych zwichnięć i do niepowodzenia leczenia chirurgicznego. Materiał i metody. Do badania włączono pacjentów z urazową, jednokierunkową niestabilnością przednią stawu ramiennego, leczonych od 2009 do 2016 roku w dwóch ośrodkach artroskopową metodą Latarjet. Oceniono wyniki kliniczne, w tym zakres ruchu, Subiektywną Ocenę Stawu Ramiennego i punktację w skali Walch-Duplay. Odnotowano powikłania pooperacyjne. Wyniki. Średnia punktacja Walch-Duplay wzrosła z 30±19 przed operacją do 83±16 (p<0,05).
Background. Various arthroscopic stabilization procedures are associated with recurrence rates ranging from 10.8% to 21.1%. Recurrences occur especially in young male patients participating in contact sport activities. Bony defects of the humeral head and the glenoid predispose not only to subsequent dislocations but also to failure of surgical treatment. This is the group where "bony" procedures such as arthroscopic Latarjet are recommended to provide better stability as the primary treatment. Material and methods. Patients with traumatic unidirectional anterior shoulder instability treated from 2009 to 2016 with an arthroscopic Latarjet procedure operated on in two centres. Clinical results, including range of motion, Subjective Shoulder Value and Walch-Duplay score, and postpoperative complications were evaluated. Results. 158 patients were available for follow-up at a minimum of 2 years after surgery. The mean follow-up was 43±18 months. Mean age at the time of surgery was 27.9 (16-53) years. At final follow-up, 8 cases of recurrent instability were identified, including 6 cases of recurrent dislocation and two cases of recurrent subluxation. Mean Walch-Duplay score increased from 30±19 preoperatively to 83±16 (p<0.05) at the last follow-up. An average loss of external rotation of 11.8⁰ (0-70) (p<0.05) when compared with the contralateral shoulder was observed at the last follow-up. Mean Subjective Shoulder Value score was 92.8±9.4%. 8 (5%) patients presented with loss of shoulder stability. 25 (15.8%) patients reported subjective return to sport anxiety. Eleven (7%) patients complained of anterior compartment pain. The total number of revision surgeries was 14 (8.9%). Conclusions. 1. The arthroscopic Latarjet procedure can achieve satisfactory clinical outcomes for the treatment of anterior shoulder instability 2. The rate of complications and recurrence does not increase with time and is comparable at a minimum of 2 years’ follow-up to early results described in literature.

Opis:
Wstęp. Starzejąca się struktura społeczeństwa powoduje rosnąca potrzebę leczenia choroby zwyrodnieniowej z wykorzystaniem technik endoprotezoplastyki stawu ramiennego. Endoprotezoplastyka odwrócona powstała by usprawnić motorykę i siłę stawu ramiennego u chorych bez jednoczesnego zwiększenia ryzyka zwichnięć. Materiał i metody. Wniosek. Każdy z operowanych chorych uzyskał istotne statystycznie zmniejszenie dolegliwości bólowych, zwiększenie funkcjonalności stawu oraz zwiększenie zakresu ruchomości stawu.
Background. The aging structure of society results in a growing need for treatment of shoulder osteoarthritis. Reverse shoulder arthroplasty (RSA) has been developed to improve the motor function and strength of the shoulder joint without increasing the risk of dislocation. The aim of the study was to assess the change in quality of life and functional assessment of the shoulder joint after RSA in patients with osteoarthritis, based on the Oxford Shoulder Score (OSS) and an assessment of joint mobility parameters. Material and methods. A total of 10 patients were admitted to our centre for shoulder arthroplasty between August 2020 and October 2021. The assessment of the degenerative changes was based on the Walch classification and the Samelson & Prieto classification. The OSS questionnaire was used to assess function of the affected shoulder joint and pain in each patient. The range of motion in the shoulder joint was assessed in the pre- and postoperative period. A follow-up examination was performed approximately 6 months after surgery. Results. Analysis of the OSS scores revealed a mean value of 46 points preoperatively and 25 points postoperatively. The OSS scores six months after the procedure were an average of 20.5 points lower. A statistically significant improvement was noted in shoulder flexion (mean 37˚), abduction (42˚), external rotation (34˚), and internal rotation (5˚)(p<0.05). Conclusion. Each patient reported reduced pain and demonstrated an increased range of motion in the affected shoulder joint and functional improvement.

Opis:
Traffic safety is influenced, among other factors, by characteristics of the roads, which include the width of the shoulder. Shoulder width was noted to have a large effect on crash frequency, as well as on traffic speed. In this paper, we focused on paved shoulders. Previous studies confirmed that increasing the width of the paved shoulder is associated with a decrease in crash frequency. However, wider shoulders may encourage higher driving speed, which is related to an increase of impact speed and crash severity – this issue was hypothesized, but not statistically investigated. Thus, conclusions based on crashes and speeds contradict each other, and there is no simple answer to the question of the safety impact of wide shoulders. To address this gap, we analyzed a sample of two most typical categories of Czech secondary roads, which differ only in the paved shoulder width (S9.5 roads with 0.75m-wide shoulder, and S11.5 roads with 1.75m-wide shoulder) and thus present a suitable example for studying the safety impact of paved shoulder width. We used generalized linear models of crash frequency, and multinomial logistic models of crash severity (separately for single-vehicle and multi-vehicle crashes), as well as a statistical test of differences in speed for the two road categories. The results showed that: Firstly, there were fewer crashes on S11.5 roads compared to S9.5 roads; this was true for both single-vehicle and multi-vehicle crashes. Secondly, single-vehicle crashes on S11.5 roads were more severe compared to S9.5 roads; the change of severity in multi-vehicle crashes was not statistically significant. Thirdly, driving speeds on S11.5 roads were approx. by 7 km/h higher compared to S9.5 roads. These findings support the hypothesis of an association between wider shoulders, higher speeds, and increased crash severity, especially in the case of single-vehicle crashes. As a practical solution, various speed management measures, including widening to a 2+1 road, may be recommended.

ObjectivesThis study aimed to assess the surgical outcomes of patients with work-related upper extremity musculoskeletal disorders (UE-MSDs) who failed conservative treatment.Material and MethodsThis was a retrospective study of 17 patients who had work-related UE-MSDs and underwent the following surgeries and follow-up evaluations: decompression, external and internal neurolysis of the upper trunk of the brachial plexus and the long thoracic nerve (LTN), and a partial resection of the anterior and middle scalene muscle. A detailed history of clinical presentation including pain, physical and clinical examinations of the extent of scapular winging (ESW), and upper extremity anatomical postures, such as active forward arm flexion and shoulder abduction, were recorded before and after 3 months of the surgery. Nerve conduction velocity and electromyography examination reports were obtained to assess the sensory or motor loss of the nerve injury before their operation.ResultsAll 17 patients included in this report showed some improvement anatomically in the scapula appearance and functionally in their shoulder movements. More specifically, 9 (53%) patients got a restored to near healthy appearance of the scapula, and 11 (65%) patients recovered a full range of motion, 180° post-surgically. Overall, the mean shoulder flexion and abduction improved to 157±37.5° and 155±40.2° after the surgery from 106±30.2° and 111±34.8°, respectively (p < 0.0001). The ESW score also significantly improved from a mean of 1.5±0.51 to 3.5±0.71 (p < 0.0001). The post-surgical improvements were statistically highly significant (p < 0.0001).ConclusionsDecompression and neurolysis of the upper brachial plexus and the LTN, and the partial release of the scalene muscle contracture procedures have fostered improvements in the shoulder anatomical appearance and movements in all 17 patients.

Opis:
Introduction. The Western Ontario Shoulder Instability Index (WOSI) is a patient reported outcome measure to assess the impact of unstable shoulder on various spheres of a patient’s life. The aim of this paper is to present the stages of linguistic adaptation of the English version of the WOSI questionnaire into the Polish version (WOSI-PL). Material and methods. The research used the procedure of translation of research tools recommended by Mapi Research Institute, which allows to minimize errors resulting from the translation. Results. Two versions of the translation were created: A1V and A2V, and based on them, a common version BV was agreed. An English native speaker made a back translation version – BTV. Next, the BTV was compared with the source version SV, corrections were made and the CV version was created. Based on the analysis of experts’ assessments, a DV version was agreed, then evaluated by a group of 6 patients. The patients’ responses was analyzed, and the final version – WOSI-PL was created. Conclusion. The WOSI-PL constitutes a valuable tool to evaluate health-related quality of life in patients with shoulder instability. The questionnaire before being introduced to scientific research and clinical practice will be subjected to a validation process in order to evaluate psychometric properties.

Opis:
Objectives The objective of this study was to assess the postures that were commonly used in automobile chassis repair operations, and to evaluate shoulder girdle muscle fatigue for different combinations of the weight of hand-tools. Material and Methods Two right muscles, including upper trapezius (UT) and middle deltoid (MD), were selected. Surface electromyography (SEMG) and a perceived level of discomfort (PLD) were used to assess the degree of shoulder girdle fatigue. Fifteen healthy young male subjects from the Northwestern Polytechnical University participated in the test. The test consisted of assuming 4 different postures and maintaining each of them for 60 s. The 4 postures varied in terms of dumbbell weights, standing for the hand-tools weight: W1 was 0.48 kg and W2 was 0.75 kg; the 4 shoulder postures were shoulder flexions of 150°, 120°, 90°, and 60°, combined with an included elbow angle of 180°, 150°, 120° and 90°, respectively. The experimental sequences were randomly selected. The signals of SEMG and the values of PLD in the shoulder girdle were recorded in 60 s. All subjects completed the whole test. The repeated measure analysis of variance (ANOVA) was performed to ascertain differences between dumbbell weight (0.48 kg and 0.75 kg) and shoulder postures (150°/180°, 120°/150°, 90°/120° and 60°/90°). The Friedman test was utilized to determine the significant differences for UT(PLD) and MD(PLD) on shoulder postures. Spearman’s correlation was used to analyze the relationship between the subjective and objective measurements. Results Significant correlational relationships existed between the UT percentage of the maximal voluntary electrical activation (%MVE) and UT(PLD) (r = 0.459, p < 0.01), between MD(%MVE) and MD(PLD) (r = 0.821, p < 0.01). The results showed that SEMG and PLD of the 4 postures under analysis differed significantly (p < 0.05). Conclusions It was indicated that posture T4 (shoulder forward flexion 60° and included elbow angle 90°) resulted in the lowest fatigue, both in terms of the objective measure and the subjective perception, which meant that this posture was more ergonomic. Opis:
Objectives Shoulder pain associated with upper limb musculoskeletal disorders is an important health problem in clinical practice due to its high prevalence, frequent consultations in primary healthcare and occupational health. Preventive strategies, including information disseminated among workers, can be useful. The aim of this study is to analyze the associations between non-traumatic osteomuscular diseases of the shoulder and the workers’ knowledge of the risks at the workplace and preventive measures developed there, as well as the association with ergonomic requirements. Material and Methods An observational case-control study was carried out on an occupationally active population assisted during 1 year in 1 healthcare center, involving 690 participants. Data were collected through a questionnaire including sociodemographic variables, the workplace, the knowledge of prevention and ergonomic requirements. The independent effect of the variables associated with non-traumatic shoulder pathology was explored through logistic regression. Results In total, 66.7% of the participants stated that they had been informed of the occupational hazards related to their jobs. The following variables were associated with a lower probability of shoulder injuries: male gender, working hours > 9 h/day or > 40 h/week, as well as having information on the risks associated with the workplace, using personal protective equipment, the existence of an occupational risks prevention service and/or risk assessment, the knowledge of the prevention plan, periodic medical examinations, and using one arm or physical force at work. A multivariate analysis revealed that the risk increased with age and lower educational levels, forced postures, repeated gestures, monotony and temporary absences from work. Furthermore, being informed of workplace risks, and using a single arm as well as physical force were shown as independent protective factors. Conclusions Information on both the ergonomic requirements and how to prevent occupational risks is a useful tool for the prevention of non-traumatic shoulder diseases. Preventive policies including health education interventions in the workplace could benefit other developed preventive activities. Int J Occup Med Environ Health. 2019;32(6):825–40

Opis:
The identification of activation, synchronization and work of individual muscles in the subsequent stages of lifting weights is interesting for researchers and trainers. Unfortunately, the existing methods of research do not provide such possibilities. Such information could be gathered from infrared measurements as they are non-invasive and can be carried out without the direct involvement of the weightlifter. The purpose of the study was to analyse temperature changes in the shoulder girdle in the first phase of the snatch in weightlifting. Methods: The study involved 11 weightlifters who competed in two weight categories, 94 and 105 kg, during the World University Championships in 2018. The performance of the snatch was recorded using a thermographic camera in three consecutive attempts. We analysed the temperature changes in the left and right shoulder girdles in the two initial stages of the snatch. Statistical analysis of empirical data was performed using linear mixed effects models. Results: Statistically significant temperature increases were found from the moment of gripping the barbell to the moment it was pulled. These effects were different in individual weightlifters, but did not depend on the attempt or the side of the body. Conclusions: Temperature increases in the initial phase of the snatch are most likely the result of activating successive motor units in order to perform the effort needed to pull the barbell and cause it to accelerate. The results obtained confirm that thermography is an effective method of monitoring muscle activity in weightlifting, which may be useful for coaches and athletes.

Opis:
Upper limb impairment in the early phase of brain stroke is one of the key problems in rehabilitation. An estimation of muscle force can be a helpful factor for functional improvement after a stroke. The primary goals of this study were to determine the muscles with the lowest force in the affected (A) and non-affected (non-A) upper limb. Moreover, the differences between men and women were compared and these values were correlated with age. Methods: One hundred (n = 35 female, n = 65 male) post-stroke patients met the inclusion criteria. The mean age of the study participants was 66.1 years. Muscle force for external and internal rotators of shoulder was measured with handheld dynamometer. Moreover, the correlation coefficients for differences in muscular force with the patient’s age were estimated. Results: Our study reports that the force of the (A) side in relation to the (non-A) was by 37% weaker. We observed about a 40% decrease in the force of the shoulder’s external rotation (female – 42%; male – 41%) and shoulder’s flexion (by 38% – female; 40% – male). Significant correlations between the muscle force and the age of post-stroke patients were also found. It was concluded that about 4 weeks after the first stroke in the patient’s life, the external rotators are the most affected group of shoulder muscles. Conclusions: Neither sex nor the side of the ischemic brain injury influence the muscle force, whereas age determines both muscle force and muscle force deficits. Older post-stroke patients demonstrate fewer deficits in muscle strength than younger ones.
