Safety in aviation has various connotations. According to the International Civil Aviation Organization (ICAO), it is a state in which the possibility of harm to people and property is minimised and maintained within a continuous process of identifying threats and managing safety risks at an acceptable level or below an acceptable level. Actions related to threat identification can be reactive, proactive, or predictive. Reactive actions aim to verify what happened, why, and how to prevent it from recurring. For this purpose, incidents that occurred in the past are analysed. By Polish law, the Commission for Investigating Aviation Accidents investigates accidents and serious incidents, while incidents mostly fall under the responsibility of the aviation organisation in which the incident occurred. Therefore, this article aims to identify threats causing incidents. Three thousand two hundred aviation incidents reported between 2017 and 2022 to the Civil Aviation Office as part of the mandatory and voluntary event reporting system were analysed. The identified causes, due to the diversity of their description, were divided into four groups. The first group consists of human factors, representing inadequate actions by pilots, crews, or individuals who caused a situation of danger. The second group comprises errors in the operation of technical objects, including aircraft. This group also includes situations where foreign objects violate airspace or minimum conditions are breached. The next group includes environmental causes such as wild animals, birds, and weather conditions. The last group consists of procedures related to flight phases.

Systemy Zarządzania Bezpieczeństwem (SMS) są usystematyzowanym zbiorem zasad, procedur i reguł postępowania przy wykonywaniu działalności przedsiębiorstwa kolejowego, mającym zapewnić wysoki poziom bezpieczeństwa świadczonych usług. Można tu m.in. wyróżnić genezę zagrożeń, diagnozę stanu i systemu bezpieczeństwa, programowanie działań zapobiegawczych, środki poprawy, monitoring podejmowanych działań oraz ocenę ich efektywności. W zakresie szacowania ryzyka wystąpienia działań niepożądanych istnieje wiele metod mających swoje odzwierciedlenie w literaturze przedmiotu. Celem niniejszego artykułu jest zaprezentowanie faktu, że pomimo wykorzystywania szeregu zbieżnych metod szacowania ryzyka w działalności polskich przewoźników kolejowych, występują istotne różnice w szacowaniu ryzyka mające odzwierciedlenie w dokumentacji związanej z zarządzaniem bezpieczeństwem (SMS).
Ensuring a sense of security is a value not only for transport users, including rail transport, but also for the environment in which the transport operates. Thus, important aspects related to an efficient safety management system are a sense of no threat and a sense of security certainty for both transport customers and its environment. Safety Management Systems (SMS) are a systematic set of rules, procedures and rules of conduct in the performance of a railway undertaking’s activities, designed to ensure a high level of security of services rendered, where we can distinguish the genesis of threats, diagnosis of the state and security system, programming preventive measures, improvement measures, monitoring of actions taken and assessment of their effectiveness. In accordance with European Union standards, Polish rail carriers were required to implement Safety Management Systems (SMS) in their operations. The implementation of railway safety management systems in Poland had a positive effect on the decrease in the number of events (sum of serious accidents, accidents and incidents). This is evident in the statistics included in the annual reports published by the State Commission for the Investigation of Railway Accidents. However, in the last few years there has been a noticeable increase in the number of events in relation to transport performance. The analysis indicates that one of the reasons may be the fact that despite the use of a number of convergent methods for estimating the risk of transport activities in these documents (SMS), there are significant differences in the estimation of risk, which are reflected in the documentation related to safety management (SMS), which means that carriers achieve very different risk results for the same risks under the same conditions. One of the conclusions is the proposal to create a list of permanent threats for nomenclature uniform carriers that should be included in safety management systems, which will also facilitate the assessment of the efficiency of these systems by the authorities responsible for this area in Poland.

W artykule przedstawiono zagadnienia związane z oceną obciążenia zadaniowego operatora w aspekcie doskonalenia układu człowiek–technika–otoczenie (C–T–O). Organizacja, która chce bezpiecznie wykonywać zadania operacyjne, potrzebuje efektywnych narzędzi służących do monitorowania i identyfikowania zagrożeń w systemie bezpieczeństwa. Okazuje się na przykładzie lotnictwa, że poziom monitorowania zagrożeń związanych z techniką lub otoczeniem jest dobry. Z badań wynika jednak, że to szeroko rozumiany czynnik ludzki powoduje największą liczbę zdarzeń niepożądanych. Niestety, poziom monitorowania zagrożeń związanych z tym czynnikiem jest gorszy; opiera się ono głównie na danych subiektywnych (obserwacje i audyty oraz raporty załóg). Celem głównym artykułu jest rozwiązanie tego problemu przez przedstawienie koncepcji oceny obciążenia zadaniowego operatora, opartej na rozwiązaniach stosowanych już od pewnego czasu w lotnictwie. Koncepcja ta opiera się na trzech rodzajach monitoringu operatora z naciskiem na uzyskiwanie danych obiektywnych przez rejestrację w czasie rzeczywistym wybranych parametrów psychofizycznych operatora na pokładowych rejestratorach parametrów lotu. Ponadto w artykule znajdują się informacje na temat rodzajów strategii bezpieczeństwa, koncepcji poziomu niezawodności człowieka oraz teoretycznej relacji pomiędzy wymaganiami dotyczącymi zadania, wydajnością oraz wysiłkiem operatora.
This article presents basic issues related to operator workload assessment in the context of man–machine–mediasystem (3M) improvement. Organizations that want to operate safely need effective tools to monitor and identify hazards in a safety system. Some proven solutions to that have been used for some time in aviation. Two elements of the 3M system are monitored at a good level (machine and media), but the human element is monitored using mostly subjective methods. According to research, the human factor is the most common cause of adverse events and is the most important, but its monitoring is not sufficient. The main aim of the paper is finding a solution to this problem by introducing the concept of operator workload assessment used in aviation. It consists of three types of monitoring, which emphasizes objective data acquisition, with a system based on objective methods of measuring the response of the human body to workload changes. It is measured according to the adopted scheme for methods of data analysis from flight data recorders or from ground-based recording devices. This group includes a number of methods that record changes in one or several physiological parameters. In addition, there is information about the types of safety strategies, the idea of levels of human reliability and the relationship between workload and performance.

W artykule opisano ogólną strukturę funkcjonalną systemu SMS oraz wyspecyfikowano elementy niezbędne do jego prawidłowego funkcjonowania. Przytoczono podstawowe europejskie i krajowe akty prawne, na podstawie których działa system SMS. Dokonano specyfikacji podmiotów operujących na obszarze dworców kolejowych oraz określono zakres współdziałania tych podmiotów przy likwidacji zagrożeń. Opisano rodzaje zagrożeń, które występują lub mogą wystąpić podczas eksploatacji dworców kolejowych oraz przytoczono orientacyjne dane dotyczące liczby przestępstw, wypadków i zdarzeń na dworcach kolejowych.
The paper describes general functional structure of the SMS system, as well as specifies the elements necessary for its proper functioning. Essential European and domestic legal documents being a basis for the functioning of the SMS system are quoted. Specification of the subjects active on the premises of railway stations is given, as well as the range of cooperation between these subjects in elimination of these risks is defined. Types of risks occurring or likely to occur during operation of railway stations are described, as well as approximate information related to number of offences, accidents and occurrences in railway stations is given.

Przedstawiono także opracowane działania, procedury i instrukcje, dzięki którym wprowadzono system zarządzania BHP w przedsiębiorstwie.
The presented article is an attempt of the optimization of the HSW management system in the industrial enterprise, with the aim of reducing the number of events potentially causing occupational diseases, accidents at work and, hence, of raising the job security on every position. Action, procedures and instructions were drawn up thanks to which, a system of HSW managing in the enterprise was implemented.

The aim of this paper is to assess whether audit may be recognised as the evaluation tool for management systems in terms of their functioning and ensuring constant improvement. The paper content refers to the role of both internal and external audit. It also includes a characteristic of an internal auditor as a person directly responsible for efficient and effective audit. A correlation between the management system and the management process based on the guidelines is a subject of the analysis, too. The paper includes also the presentation of the safety management system and ship energy efficiency management systems the implementation of which is obligatory at ships. However, they differ in the methodology of the efficiency assessment. The author analysed the data previously collected and the information obtained during interviews with individuals to whom were delegated the duties in the scope of the both management schemes. The possibility of the using audit as a tool to improve the energy efficiency of ships is also considered hereunder. A proposed solution that may be applied to develop detailed standards in terms of the ship energy efficiency management process is also included.

The aim of the article is to present the concept of safety culture management systems in an enterprise and to indicate the directions of their improvement. In the course of two-stage empirical research, elements of the management system that highly determine the development of occupational safety culture were identified and described. This applies in particular to: technical and managerial competences of the management staff, modernity of IT technology, work organization techniques, cooperation, values professed in the company. These factors were found to be important criteria for assessing the effectiveness of systems for managing work safety culture. The concept of work safety culture and proposals of the authors of management systems related to work safety culture were characterized. The article concludes with general recommendations and methodological guidelines for enterprises interested in the effectiveness of occupational safety culture management systems in the context of company performance.
Celem artykułu jest przedstawienie koncepcji systemów zarządzania kulturą bezpieczeństwa pracy w przedsiębiorstwie oraz wskazanie kierunków ich doskonalenia. W trakcie dwustopniowych badań empirycznych zidentyfikowano i opisano elementy systemu zarządzania, które w wysokim stopniu determinują rozwój kultury bezpieczeństwa pracy. Dotyczy to w szczególności: kompetencji technicznych i menedżerskich kadry kierowniczej, nowoczesności technologii IT, technik organizacji pracy, kooperacji, wartości wyznawanych w przedsiębiorstwie. Czynniki te uznano za istotne kryteria oceny sprawności systemów zarządzania kulturą bezpieczeństwa pracy. Scharakteryzowano pojęcie kultury bezpieczeństwa pracy oraz propozycje autorów systemów zarządzania odnoszących się do kultury bezpieczeństwa pracy. W zakończeniu artykułu podano ogólne rekomendacje merytoryczne oraz wskazania metodyczne dla przedsiębiorstw zainteresowanych sprawnością systemów zarządzania kulturą bezpieczeństwa pracy rozpatrywaną w kontekście wyników przedsiębiorstwa.

Safety culture is one of the most important elements of the organization's culture. Its role is particularly important in production plants, where employees are more often exposed to threats related to the working environment. The safety culture is strongly associated with the organizational level of the enterprise, which is largely influenced by the implemented management systems. The article presents the results of the evaluation of work safety culture in production plants with different functioning management systems, including the so-called turquoise organization. A relationship between the organizational level of the plant and the culture of occupational safety was stated.

This paper addresses the improvements to the reliability of the safety systems of nuclear reactors using redundancy allocation technique. The study has been carried out using the Probabilistic Safety Assessment (PSA). PSA involves, among others, the use of fault and event tree tools in the evaluation of the safety system failure probabilities and the quantification of annual occurrence probability of the accidental conditions postulated in the design of the nuclear reactors. The PSA has been presented and discussed. The Egypt Second Research Reactor, ETRR-2, has been used as a case study. The failure probability of the already existing safety systems has been reviewed. The effect of the allocation of more redundant components to the existing safety systems on the failure probability of the systems has been evaluated. The event trees for two selected initiating events, from those events postulated in the ETRR-2 design, have been studied considering the allocation of more redundant components to the safety systems. The result of the study showed that further improvement could be introduced to the reliability of the Confinement Ventilation System (CVS).

The aim of this study is to study compounds, that quality, as part of standardised management systems, affects safety, which is one of the primary elements for the functioning and development of an organisation. In the presented paper many attempts were made to indicate that quality is closely connected to safety, and as part of standardised management systems, significantly affects the improvement of the effectiveness of actions undertaken, consistent with the adopted strategy and the vision of an organisation. The conclusions from the conducted research state that standardised management systems are aimed at improving and maintaining quality as well as the fact that maintained safety allows to achieve it. The research results are part of a multi-aspect analysis of the Central Pomeranian entrepreneurship used to build models of quality shaping in organisations within the qualitative foresight framework. Obtained results, from more than 1,600 entities, confirm the hypothesis that safety is an important element of quality development under standardised management systems.
Celem opracowania jest badanie związków, że jakość, w ramach znormalizowanych systemów zarządzania, oddziałuje na bezpieczeństwo, które stanowi jeden z głównych elementów funkcjonowania i rozwoju. W opracowaniu starano się wykazać, korelację jakości i bezpieczeństwa, która bezpośrednio wpływa na poprawę efektywności podejmowanych działań, zgodnych z przyjętą strategią oraz wizją organizacji. W ramach przeprowadzonych badań stwierdza się, że znormalizowane systemy zarządzania mają na celu doskonalenie i utrzymanie jakości, a zachowane bezpieczeństwo pozwala je osiągnąć. Szczególnie uwidacznia się zjawisko to w procesie planistycznym oraz realizacji. Uzyskane dane w ramach przeprowadzonej ankietyzacji z ponad 1600 podmiotów potwierdzają hipotezę, że bezpieczeństwo stanowi ważny kształtowania jakości, w ramach znormalizowanych systemów zarządzania.

In the article the ProSIL software for computer aided functional safety management is presented. The software consists of three modules for the determination of the required SIL level (ProSILen) and verification of the SIL level (ProSILver). In the ProSIL the calibrated knowledge-based risk graph method for determining the required safety integrity level (SIL) of the safety functions identified in hazard analysis is implemented. The SILs are then verified for safety-related control and protection systems implementing relevant functions with regard to random failures and potential systematic failures. The assessment methods are compatible with international standards IEC 61508 and IEC 61511. Some current challenges and methodological issues concerning knowledge-based functional safety management in life cycle are also discussed.

Opis:
The construction industry in India is the second most important contributor to its gross domestic product. However, high rate of accidents and fatalities have tarnished the image of industry in India. Although the industry contributes significantly to the Indian economy, safety management is the primary concern alongside with frequent workplace accidents. The role of safety management is vital to improve safety performance of an organization. The functions of safety management include planning, organizing, staffing, directing, controlling and coordinating safety activities with an aim to minimize accidents/injuries. Safety trainings, employee participation, compliance of safety procedures and motivational schemes are part of safety management which influences the overall safety performance. Several metrics were developed to measure the safety performance of an organization but not a single measure will reflect the overall performance. The present study considered parameters pertaining to the safety management which have influence on the safety performance of a construction organization in India. The parameters are analyzed by formulating a goal programming model. The results of the study suggests that much improvement is needed in the area of safety trainings and the revised targets were established.

Bezpieczeństwo żywności obejmuje zagadnienia związane m.in. z procesami wytwarzania, przetwarzania czy też dystrybucją żywności. Ujednolicenie europejskiego prawa żywnościowego poprzez wprowadzenie odpowiednich ustaw i rozporządzeń pozwala na kontrolowanie jakości i bezpieczeństwa żywności produkowanej, przetwarzanej oraz konsumowanej nie tylko we własnym kraju, ale również sprowadzanej z całego świata. System zarządzania bezpieczeństwem żywności ma na celu szeroko pojętą ochronę konsumentów z jednoczesnym zagwarantowaniem zrównoważonego i niezakłóconego działania rynku. Współpraca pomiędzy producentami, konsumentami i rządami poszczególnych państw warunkuje odpowiednią jakość i zapewnia bezpieczeństwo produkowanej i dystrybuowanej żywności. Niniejsza praca przedstawia podstawowe pojęcia dotyczące bezpieczeństwa żywności, przegląd obowiązujących w Polsce praw dostosowanych do ustawodawstwa Unii Europejskiej oraz podstawowe systemy zarządzania jakością i bezpieczeństwem żywności pozwalające na uzyskanie żywności bezpiecznej, ekonomicznie dostępnej oraz zaspokajającej potrzeby konsumenta.
Food safety covers a very wide range of issues concerning all its aspects starting from scratch production through the entire process of production, finally distribution. Unification of European food law by the introduction of relevant laws and regulations, allows us to control the quality and most of all the safety of food produced, processed and consumed in the European Union as well as imported from around the world. Food safety management system has aiming at the broader consumer protection along with ensuring balanced and smooth functioning on the field. Cooperation between producers, consumers and governments of each country determines the quality and ensures the safety of food produced and distributed. This paper presents the basic concepts of food safety, a review of laws applicable in Poland adapted to EU legislation and basic systems of quality management and food safety can achieve safe food, economically availability and serves the needs of the consumers.

Opis:
The implementation of the CSM regulation by the European Commission in 2009 which harmonizes the risk assessment process and introduces a rather new concept of judging changes within the European railway industry. This circumstance has risen the question how other technology sectors handle the aspect of modifications and alterations. The paper discusses the approaches for judging the significance of modifications within the three transport sectors of European railways, aviation and maritime transportation and the procedure which is used in the area of nuclear safety. We will outline the similarities and differences between these four methods and discuss the underlying reasons. Finally, we will take into account the role of the European legislator and the fundamental idea of a harmonization of the different approaches.
