Background. Health education is one of the main cores of primary health care (PHC). However, there is limited evidence on the difficulties of implementing health education programs. This study explored the barriers of implementing health education programs in Iranian rural communities. Objectives. A qualitative study with conventional content analysis approach was conducted. Applying purposeful sampling, 34 rural folks and health care providers were employed to participate in the study. Data were collected through individually, semi-structured interviews. Data analysis continued until data saturation, when no new theme or idea emerged. Material and methods. A qualitative study with conventional content analysis approach was conducted. Four themes, including “Ineffective teaching and learning processes”, “Lack of health educators’ motivation”, “Communication gaps”, and “Lack of resources and facilities for teaching and learning” emerged as the barriers of implementing health education programs in rural communities. Conclusions. Several executive and communicational problems were identified as the local-level obstacles of implementing health education programs in rural areas. Better understanding on the extensive range of health education barriers in rural areas may be helpful for rural health workers and stakeholders in designing and/or revisiting health education programs in rural communities.

The paper presents the issue of the sustainable tourism which corresponds with the concept of the sustainable development. The work concentrates on the theoretical analysis of the sustainable tourism, often treated as a tool of accomplishment of the sustainable development or the tool of the development of tourism itself. The article is an attempt to state the basic rules of the sustainable tourism on the so-called rural health resort areas. The work shows what kind of tourism can be developed on the analysed rural areas, at the same time being in harmony with the concept of the sustainable tourism. In the paper the analysis of the national and foreign literature from the domain of the concept of the sustainable tourism, including rural areas, and the observation of the chosen indicators of the sustainable development of health resorts villages are presented. The concept of the sustainable development arose in the 1970s and it was an expression of the critical reaction to the growing interference in natural and social and cultural resources. The concept of the sustainable development is not opposite to the possibilities of economic growth, on condition that the pace of the reproduction of the environmental resources is not slower than the pace of the usage of these resources. The theoretical basis of the usage of the rules of the sustainable development in the world literature started to be seen at the turn of the 1980s and 1990s. The document „Agenda 21 for Travelling and Tourism Economy” turned out to be fundamental for the development of the concept of the sustainable tourism. The practical use of the concept of the sustainable development in the rural areas economy needs devising indicators and indexes characterizing this development. The indicators are divided into: indicators of environmental pressure (danger, stress), indicators of condition (quality) of the environment and indicators of preventive activities. The economic significance of the tourism lies in its positive influence on the different departments of the economy, including agriculture. One of the more important impacts is stopping the migration of people from the rural areas.

A quiet revolution underway is leading to changes in healthcare for many countries of the world. The sentinels of health have always been physicians; the medical model, representing the research, education, and experience of medical doctors, has worked well for treating acute conditions of injury and physical illness and been very successful in reducing disease-producing morbidity and mortality. But pressing contemporary issues, such as spiraling healthcare costs, population aging and the need to manage chronic conditions, and recognition that mental health care is necessary for societies to be physically healthy, are forcing a reevaluation of existing conceptualizations of care. In response, physicians are increasingly working in integrated practices with other specialists to broaden care coverage to include social and behavioral conditions empirically demonstrated to influence medical care outcomes. Aiding the development of these new, more inclusive models of healthcare are advances in communication technologies. Practitioners are now using synchronous and asynchronous forms of communication to deliver physical and mental health services beyond the boundaries of traditional brick-and-mortar practices, into communities where clients live and work. This brief review of the potential of telehealth to address disparities in healthcare has two purposes: 1) examine an innovative model of comprehensive, integrated biopsychosocial services useful in single- and multiple-location practices; and, 2) identify challenges of using synchronous, virtual technologies for client-centered, mental health care service provision in rural, underserved areas.

A Practice-Based Research Network (PBRN) consists of a group of clinicians, practices or institutions that are devoted primarily to the delivery of patient care and are associated with one another in order to answer community- based health care questions and translate research findings into practice. The main goal of PBRNs is to involve busy community-based clinicians in studies conducted by investigators experienced in clinical and health service research. Doctors are drawn to take part in PBRNs in order to provide answers relevant to their practice, with the goal of improving the quality of practice and the health of their community. PBRNs provide access to phenomena often neglected by researchers, but which are of great importance to those directly affected by the issues being studied. Practice-based research in family medicine is an important way to acquire new knowledge by the means and outcomes of family medicine practice. Although Practice-Based Research Networks (PBRNs) are useful tools for conducting practice-relevant research in the busy primary care setting, their existence is threatened by a range of challenges, e.g. the limited financial support that rural areas have been receiving over the last few years, especially during this period of austerity in many countries within the European zone. Recruitment difficulties are a major impediment, fuelled by general practitioners’ time constraints, lack of remuneration, non-recognition and workforce shortages. In conclusion, despite the difficulties and challenges that PBRNs are facing, clinicians as individuals and organizations like EG PRN and EURI PA are trying to establish such types of networks, especially in low-income countries, in order to enhance the improvement and delivery of rural health care.
Sieci badawcze oparte na praktyce (PBRN) składają się z grupy lekarzy, praktyk oraz instytucji, które są zaangażowane przede wszystkim w zapewnienie opieki pacjentom i powiązane ze sobą tak, aby wyjść naprzeciw potrzebom opieki zdrowotnej społeczności oraz przekształcić wyniki badań w praktykę kliniczną. Głównym celem sieci badawczych opartych na praktyce jest zaangażowanie klinicystów pochłoniętych pracą na rzecz społeczności lokalnych w badania prowadzone przez badaczy, którzy mają doświadczenie w dziedzinie badań klinicznych i usług zdrowotnych. Lekarze są zachęcani do wzięcia udziału w działaniach sieci badawczych opartych na praktyce po to, aby mogli udzielić odpowiedzi na pytania ważne dla swojej praktyki, a tym samym poprawić jakość praktyki i stan zdrowia społeczności. Sieci badawcze oparte na praktyce zapewniają dostęp do zjawisk często pomijanych przez badaczy, ale bardzo istotnych dla osób bezpośrednio związanych z badanymi kwestiami. Badania naukowe oparte na praktyce prowadzone w dziedzinie medycyny rodzinnej są ważnym sposobem zdobywania nowej wiedzy zarówno na podstawie stosowanych procedur, jak i rezultatów uzyskiwanych w praktyce medycyny rodzinnej. Chociaż sieci badawcze oparte na praktyce są użytecznymi narzędziami w prowadzeniu badań istotnych dla praktyki w dziedzinie podstawowej opieki zdrowotnej, ich istnienie jest zagrożone przez szereg problemów, takich jak ograniczenie wsparcia finansowego na obszarach wiejskich w ostatnich kilku latach, szczególnie w trwającym w wielu krajach Unii Europejskiej okresie zaciskania pasa. Trudności z rekrutacją są główną przeszkodą powodowaną ograniczeniami czasowymi, jakim podlegają lekarze rodzinni, brakiem wynagrodzenia, uznania oraz niedoborami kadrowymi. Podsumowując, pomimo niedogodności i wyzwań, które stoją przed sieciami badawczymi opartymi na praktyce, sami klinicyści oraz organizacje, takie jak EG PRN (Europejska Sieć Naukowa Medycyny Rodzinnej) i EURI PA (Europejskie Stowarzyszenie Lekarzy z Terenów Wiejskich i Izolowanych) próbują tworzyć ten rodzaj sieci, zwłaszcza w krajach o niskim dochodzie, aby poprawić jakość i zapewnić jak najlepszy dostęp do opieki zdrowotnej na obszarach wiejskich.

Introduction. The issue of inequalities in the use of health services and health inequalities between urban and rural dwellers is a subject of numerous scientific reports, but similar publications on dental health are scarce. Objective. The aim of this study is to assess the correlations between the use or non-use of dental services and the classical socio-economic factors (SES) in urban and rural groups in a representative population of Poland. Materials and method. The analysis consisted in desk research based on a questionnaire survey conducted by the Central Statistical Office of Poland on 12,532 individuals (urban areas – 6411, rural areas – 6121) in 2013. The chi-squared test, multivariate and univariate logistic regression models were applied. Results. There were major differences in the use of dental services depending on income level, education and source of income among rural and urban populations, as well as differences between these populations. Urban dwellers had 1.34 times greater odds of using dental services than rural ones (95% CI: 1.20–1.51). In the case of the highest income group, the odds were 3.26 (95% CI 2.21–4.83) times greater in cities and 2.07 (95% CI 1.51–2.85) times greater in villages than the odds in the lowest income group. In the highest education group, the odds were 1.58 (95% CI 1.17–2.13) times greater in urban areas and 2.08 (95% CI 1.48–2.91) times greater in rural areas than the odds in the lowest education group. Conclusions. There are considerable inequalities in the use of dental services, yet the differences in non-use, despite such a need, are less evident. This could imply that health disparities arise not only from economic constraints and unavailability of health care, but also from health attitudes and behaviours.

Unfavorable living conditions of the population, particularly in rural areas, and the relatively low level of health culture, expressed by bad nutrition, insufficient physical activity, alcohol abuse, and smoking, are not conducive to maintaining the health of the population. The health status of the population, including children and young people, is assessed as highly unsatisfactory. The aim of this study was to evaluate the research on health behavior, serving as a basis for formulation of health education programs in rural areas. To develop the action strategy for health education, addressed to a group of children and young people of school age in rural areas, it is important to know the perception of health in this environment. Literature reports seem to indicate that this sphere of life and education is a bit neglected, probably highly diverse and determined by the influence of various factors, both environmental and individual. Therefore, identification of hazards, understanding their life situation, diagnosing the situation, especially in a group of school children, is particularly desirable and expected. The development of health promotion and prevention programs among adolescents must be based on a fully reliable diagnosis of social situation, documented and monitored. Circulating information from newspaper reports and the school environmentsuggests the need forparticular actions in the field of school health education, conducted by prepared, authoritative staff of educators. Such are the expectations and needs of young people and those who consider the health issues of children significant. The problem of great importance is the issue of research on the health behavior of young people, including rural areas, and the development of compatible research tools. The lack of such tools makes the comparison of the results obtained by different authors difficult. The acquired theoretical knowledge and implementation of various health programs have often failed to produce practical results. What is needed are the actual steps to promote health in schools and homes, and the skills to use the existing knowledge to make the analysis and search for the determinants of health behavior of young people. The skillful linking of thinking, action and knowledge of the determinants of health behavior, will prevent from one-sided trends in education, and will bring more focus on the skills and versatility in the harmonious development of young people.

Authors define and explain the meaning of two concepts, namely human and social capital and their influence on the socio-economic progress in agriculture and rural areas. The presented studies and analyses point to the link between the growing role of these two factors and the general socio-economic progress. A major part of the paper is devoted to analysing the past and current state of human and social capital in Poland, focusing on their deficiencies and the need to improve their quality. In conclusion the authors note that the significant inflow of financial means after the the Polish accession to the EU as well as structural and administrative changes will not bring in full the possible benefits in the future unless there is an improvement in quality and an increase of creative participation of the human and social capital. The health aspect is also explored in the paper as a crucial element influencing human capital in the Polish rural areas.

Introduction and objective. Measuring the level of patient satisfaction is a useful tool in delivering quality care that is responsive to consumer preferences. Various socio-demographic factors might be considered as potential predictors of patient satisfaction. The aim of the study was to assess whether place of residence (rural/urban) affects patient satisfaction with hospital health care. Materials and method. Data were obtained using face-to-face questionnaire, administered in five large, urban hospitals in Podlaskie Province, north-eastern Poland, during 2014–2018. The study sample comprised of 1,624 participants (585 rural, 1039 urban) who assessed satisfaction with 28 hospital health care items. The means and standard deviations were presented to compare 28 satisfaction items between the rural and the urban samples. Regression analysis was used to determine whether location difference (rural vs urban) affected patient satisfaction with various domains of hospital health care. Results. The mean results of 28 satisfaction items on the 1–5 scale were similar among the rural and the urban samples, and generally skewed towards positive experiences. In the univariate analysis, significant associations between place of residence and patient satisfaction were identified with regard to three components of inpatient care: 1) hospital settings and staff care, 2) doctors’ professional skills, and 3) hospitalization outcomes. After adjusting for socio-demographic variables, the association remained significant only with respect to satisfaction with hospitalization outcomes (b = 0.121; SE = 0.055; p = 0.028). Conclusions. Some evidence for differences in patient satisfaction by place of residence was found. Study findings may be helpful in implementing care quality improvement programmes.

Introduction. Quality of life (QOL) is an important aspect of life, reflecting health and conditioning the well-being of older adults. Maintaining the QOL is essential in times of demographic changes which resulting longer life spans, and consequently an increased proportion of older adults in society. Objective. The aim of the study was to assess the quality of life and its basic determining factors in the elderly living in rural areas of south-eastern Poland. Material and method. The study was conducted on 973 respondents aged 60–80 years living in rural areas of south-eastern Poland (Podkarpackie Region). The research tool used in the study was the WHOQOL-Bref and a questionnaire on personal characteristics and health. Statistical analysis was performed using Statistica version 10. Results. All quality of life domains assessed were above the median values of the scale. The highest values were found in the social domain (67.35 ± 17.31) and the lowest values in the physical domain (58.74 ± 14.80). All the quality of life values decreased with an increase in age and increase in number of chronic diseases in a given person. A higher quality of life was found in subjects who were physically and socially active. Conclusion. The study stresses the impact of modifiable determiners of QOL. Interventions aimed at improving the quality of life of older adults should involve interdisciplinary monitoring of health, early treatment of diagnosed problems, and promotion of physical activity and daily life activities.

Health care of children and young people is part of a system of care of the so called “developmental age population.” In this system, prevention plays a key role. The aim of the study was, basing on the obtained results, to develop guidelines for the regional pro-family policy in care of a rural child. The study included children and young people of school age, from 14 to 19 years of age. Overall the analysis included 6,971 children and adolescents and 6,971 parents. Organization of health care of children and school adolescents depended on the solutions approved by individual founding bodies of Health Care Units and Regional Patients’ Funds. The withdrawal of nurses from school was observed. The tasks of prevention character performed previously by nurses employed at schools began to be implemented within the framework of primary health care in the child’s place of residence. For proper implementation of the tasks of school nurses and the proper development of school hygiene, it is important to establish a uniform and maximum number of pupils per one nurse, taking into account all circumstances, and to develop standards of care for the student, taking into account the age of the student, school type, and the region.
