The literature points that attachment is key mechanism in the development of people. According to the attachment theory, the experience of neglect, abandonment and other early traumas have influence on forming the dysfunctions of emotions and personality pathology. The aim of this article is the verification of the thesis about relation between attachment style and personality disorders. The analysis of results of 155 participants has been conducted (persons in the similar age, similar educational level, similar number of females and males). The diagnosis of personality disorder was based on SCID-II (Structured Clinical Interview for Axis – II), and additionally confirmed by PDQ-4 Hyler Inventory. To examine the attachment style the Plopa’s Attachment Styles Questionnaire and the Revised Experiences in Close Relationships Inventory (ECR-R designed by Fraley et al., 2000), were used. The multiple regression model was used to test whether attachment styles maybe the predictors for personality disorders. The current results partly support the Lyddon and Sherry’ thesis. The main predictor for the relations between attachment and personality disorders is the anxious attachment style.

Opis:
Preventing school dropout and promoting successful graduation is a national concern in Bulgaria that poses a significant challenge to schools and educational communities working with youth at risk of school failure. Few studies have examined predictors of dropping out of school before students reach high school and their data have been analysed. Research shows dropping out is strongly tied to family psychopathology, individual characteristics and childhood development disorders. Finally, yet importantly, there are some socioeconomic and cultural factors associated with dropping out of school. A multifactor approach is needed in order to understand the risk factors for school dropout. Identifying the predictors of school failure is crucial for understanding the causes and processes of dropping out, which, in turn, can help guide the creation of effective programs for the prevention of this problem. Implications for prevention of early high school dropout are discussed. The authors place an emphasis on applying a trans-cultural approach to maximize the potential benefit of dropout prevention programs.

The question ‘Do psychopathology dimensions correlate with musical preferences in healthy individuals?’ still remains poorly investigated. Additionally, verbal working memory, psychopathology and music preferences have not been examined together. Participants consisted of ninety-three young women without a previous or current psychiatric diagnosis. All participants were examined with the Forward Digit Span Task, and completed a psychopathology symptom instrument along with a musical preferences question. Results revealed that the Global Score Index, Somatization, Hostility and Depression correlated in a statistically significant way with musical preferences in female adults. Hostility and Depression predicted classical music preferences. Depression, Obsessive/Compulsive, Somatization and Hostility predicted a pop preference, while Somatization and Psychoticism predicted a rock preference. No significant correlations were found between the above variables and verbal working memory, except for a significant correlation between age and the span of the working memory. Certain psychopathology dimensions do not influence cognition in the form of the verbal working memory in women, but can predict a specific choice of music genres. Findings suggest that music listening preferences may represent a field of inner experiences that could reveal easy-to-obtain information about the mental health of women who have not an official psychiatric diagnosis, but may be at risk of developing psychological problems due to high self-reported symptoms of psychopathology.

After offering a brief outline of Cassirer’s fundamental ideas on symbolization, the article looks at its application to psychopathology, e.g. psychosis, a theme not introduced by Cassirer himself. Psychosis is conceived of as a distortion of a fundamental symbolization, a radical metaphor, thus elaborating a version of Cassirer’s own line of thought. Cassirer’s concept of basis phenomena appears to provide a fruitful conceptual scheme in this regard. At the same time, a case is made for the reappraisal of an anthropological brand of psychopathology. It constitutes a qualification of the currently dominant biological psychiatry, offering a legitimate perspective on psychic disorder, albeit a relative one in both cases – a vision in line with Cassirer’s basic assumptions of symbolization.

Motivational internalism is a view according to which moral judgments are necessarily motivating. Rationalist internalism (RI) is the most popular version of this view; it limits internalism to people who are practically rational. Motivational internalism, including RI, has been criticized as being incompatible with research into certain personality disorder; in particular psychopathic personality and pathological personality which is the result of damage to the ventromedial prefrontal cortex (shortened to VM damage). In this paper, I argue that many of the features of psychopathic personality and of VM damage, which some philosophers interpreted as direct proof against internalism, should be understood as having an effect on the practical rationality of the patients. This means that these personality disorders cannot be used as counter examples to RI and can, in fact, be seen as supporting RI to some extent . I begin by describing RI. I then turn to I describing the phenomenon of psychopathic personality and VM damage and their philosophically relevant features. Finally I discuss whether the features characterizing psychopathy and VM damage influence the degree to which these disorders can serve as counterexamples to internalism of the rationalist variety.

Emotion Regulation (ER) has been identified as a factor that may be related to psychopathological symptoms. However, evidence about the relationship between ER and psychopathological symptoms is still unspecific. Moreover, although the ability of distress tolerance (DT) has gained increasing attention, it has not yet been sufficiently explored in relation to specific psychopathological symptoms. The aim of the study was to analyze the role of different specific ER mechanisms on various psychopathological symptoms, with particular emphasis on the role of DT. To do so, a correlational study was carried out. A total of 128 university students between 18 and 44 years old (mean age = 26.7, SD = 6.14) answered the Distress Tolerance Scale, the Difficulties in Emotion Regulation Scale and the Symptom Check List 90-Revised. For each psychopathological symptom (and for general distress), linear regression were applied. All models were statistically significant with differences in the amount of explained variance and in the predictors. DT predicted symptoms of depression, anxiety, obsessions and compulsions and general distress. The study highlights the importance of the different mechanisms of ER in each specific psychopathological symptom and their implications for mental health.

We investigated the relationship between borderline symptoms in women and their partner’s desire to continue the relationship and relationship commitment in an aversive living environment using an experimental design. We aimed to highlight the adaptive mechanisms of borderline symptoms, which may materialize in reproductive advantages for women in aversive contexts. We conducted two experiments to test the same hypotheses. In Study 1, online participants were assigned to the experimental group (N = 123). The experimental scenario hypothesized having a relationship with a woman high in borderline characteristics, during an aversive life environment. The control group (N = 118) hypothesized having a relationship with a woman high in borderline characteristics, during normal living conditions. We repeated the same experimental manipulation using a within-subjects design in Study 2 (N = 171). Environmental conditions influenced the desire to end the relationship with women characterized by borderline symptoms; men had a lower desire to end the relationship with their partner in aversive life events than in normal living conditions. Thus, women’s borderline behavioral characteristics may bring mating benefits in aversive living environments.

Parentification happens when children take on parental duties, which leads to role reversal between children and their parents. Parentification has multiple causes and can have a short- and long-term negative impact on parentified children. This study examined prevalence and correlates of parentification in Polish adolescents. The study included 557 16-year-old students in four types of Polish high schools. Results indicate that parentification is present to a considerable degree and may negatively affect students’ school functioning and affective well-being.

Autor przedstawia swoje refleksje dotyczące kwestii winy w kontekście nauk psychiatrycznych i psychoterapeutycznych. Zdaniem autora, w praktyce duszpasterskiej oraz w psychoterapii od wielu lat istnieje silna tendencja do psychiatrycznego lub teologicznego redukcjonizmu, jak również rywalizacji pomiędzy duszpasterzami, psychiatrami i psychoterapeutami . Na przełomie XIX i XX wieku istniała tendencja do wzajemnego uznawania ich kompetencji i chęci do współpracy. Możliwość opisywania problemów pacjenta/penitenta z psychologicznego punktu widzenia z jednej strony, a z punktu widzenia etycznego z drugiej strony pozwala nam uniknąć konfliktu kompetencji na rzecz osób poszukujących pomocy psychiatrycznej - psychoterapii lub wsparcia duchowego.
The author presents his reflections on the issue of guilt in psychiatric and psychotherapeutic practice. According to the author, in pastoral practice and in psychotherapy for many years there has been a strong tendency to psychiatric or theological reductionism as well as rivalry and mutual invalidation between pastors and psychiatrists and psychotherapists. At the turn of the century there was a tendency for the mutual recognition of their competence and willingness to cooperate. The possibility of describing the problems of the patient / penitent from a psychological perspective onthe one hand and from the ethical perspective on the other hand lets us to avoid conflict of competence for the benefit of people seeking psychiatric-psychotherapeutic and / or spiritual help.

The aim of the article is to discuss the specifics of human actions and decision-making processes from the psychopathological perspective. The concepts of action and decision making are reported in the context of human experience and the experiential structure of self-determination. The starting point is provided by considerations related to the notion of mental illness as described by Lennart Nordenfelt, as well as Antoni Kępiński’s concept of informational metabolism. The consequences of and changes to decision-making processes in mental disorders will be presented on the example of depressive experience. Decision-making process can be understood in terms of three-dimensional pre-reflexive experience that undergoes changes in depressive patients: space of possibility, sense of agency and time experience. The analysis will allow us to better understand the structure of experience attributable to people suffering from depression.

The article deals with relational aspects of mental disorders. The author takes into account the influence of mental illness on intersubjectivity and interpersonal relations in three aspects: (1) “attitude to the illness,” that is, changes in the functioning of the subject and difficulties in dealing with the experience of mental illness; (2) “dialogical relationship” in the form of difficulties in maintaining social cognition and entering into relationships with others; (3) “social consensus,” that is, difficulties in adapting to the social world and a common sense deficit. The analysis is made with reference to phenomenological research in contemporary philosophy of psychiatry. The result of this study is a reconstruction of a “different existence,” in other words, presentation of the transformation of the life-world and the specific way of inhabiting the world of a person in a mental crisis. Symptoms and experience of the illness are presented in the light of human possibilities of transformation. The analysis emphasizes the role of patients’ subjectivity and their efforts to find the meaning of a painful situation. The study of subjective aspects of disorders aims to reveal their consequences for understanding how to deal with mental crisis and to understand the positive aspects of the recovery process.

Introduction: Risk of suicide in patients with schizophrenia is 20- to 50-fold higher than in the general population. The impact of suicidal behaviour on quality of life rarely was a subject of in-depth analysis. The issue is particularly important in patients after first psychiatric hospitalisation, since the risk of suicide is very high during the first postdischarge months. The aim of this study was to analyse correlations between presence of suicidal ideations prior to first hospitalisation and quality of life of patients with schizophrenia during the first post-hospitalisation year. Material and method: Overall, 86 patients were enrolled in the study. Suicidal ideations and behaviours were assessed based on interviews with patients and their relatives, as well as on medical records. The patients’ quality of life was evaluated one month (1st exam) and after one year (2nd exam) after discharge. Quality of life was assessed using the WHOQOL questionnaire and the SFS scale. Psychopathological symptoms were evaluated using the PANSS inventory. Results: Prior to first hospitalisation, suicidal ideations were present in 39.5% of our patients. Persons with suicidal thoughts presented more severe dipsychopathological symptoms (1st exam; p=0.05). Both examinations revealed differences in the patients’ quality of life. Lack of suicidal ideations was associated with a better quality of life as assessed by the WHOQOL questionnaire in the following domains: general well-being (1st and 2nd exam), state of health (1st exam), Mental (1st and 2nd exam), Physical (1st exam), Environmental domains (1st exam) and Work/Employment (SFS scale, 1st and 2nd exam). A correlation was noticed between suicidal ideation and change in quality of life (WHOQOL) in domains Social relationships and Environmental. An improvement of quality of life in this domain was noticed in persons with suicidal ideations. Conclusions: Suicidal ideations prior to first psychiatric hospitalisation constitute an important predictive factor for a worse quality of life in patients with schizophrenia one year after discharge from hospital.
Wprowadzenie: Ryzyko samobójstwa u chorych na schizofrenię jest wyższe 20-50 razy niż w ogólnej populacji. Wpływ zachowań samobójczych na jakość życia rzadko był przedmiotem analiz. Problem nabiera szczególnego znaczenia u chorych po pierwszej hospitalizacji psychiatrycznej, gdyż większe ryzyko samobójstwa występuje w pierwszych miesiącach po opuszczeniu szpitala. Celem pracy była analiza zależności pomiędzy obecnością myśli samobójczych przed pierwszą hospitalizacją a jakością życia chorych na schizofrenię w okresie roku po hospitalizacji. Materiał i metodyka: Badaniami objęto 86 chorych. Ocenę myśli i zachowań samobójczych przeprowadzono na podstawie wywiadów z chorymi i ich rodzinami oraz dokumentacji medycznej. Ocenę jakości życia wykonano po miesiącu od wypisu (badanie I) i po roku (badanie II). Do oceny jakości życia zastosowano kwestionariusz WHOQOL i skalę SFS. Objawy psychopatologiczne badano przy użyciu PANSS. Wyniki: Przed pierwszą hospitalizacją u 39,5% osób występowały myśli samobójcze. Stwierdzono większe nasilenie objawów u chorych z myślami samobójczymi (badanie I – p=0,05). W obu badaniach ujawniono różnice jakości życia. Osoby bez myśli samobójczych cechowały się lepszą jakością życia według WHOQOL: w ocenie ogólnej (badanie I i II), stanu zdrowia (badanie I), w dziedzinach: Psychologicznej (badanie I i II), Fizycznej (badanie I), Środowisko (badanie I) oraz według SFS: w dziedzinie Praca/Zatrudnienie (badanie I i II). Stwierdzono zależność pomiędzy myślami samobójczymi a zmianą jakości życia (WHOQOL) w dziedzinach Relacje społeczne i Środowisko. U osób z myślami samobójczymi stwierdzono poprawę jakości życia w tym zakresie. Wnioski: Myśli samobójcze przed pierwszą hospitalizacją psychiatryczną stanowiły istotny czynnik predykcyjny gorszej jakości życia chorych na schizofrenię przez rok od hospitalizacji.

Relationship between psychopathological symptoms and quality of life in patients with first episode of schizophrenia. A follow-up study

Aim of paper: Analysis of correlation between psychopathological symptoms and quality of life of patients with schizophrenia during 4-6 years after the first psychiatric hospitalisation. Material and method: Study population included 74 patients (28 females, 46 males). They were assessed at fixed time-points: 1 month and 13 months after the first hospitalisation and 4-6 years after the first examination (T1, T2 and T3, respectively). The following instruments have been used to assess severity of symptoms and quality of life: PANSS scale (5-factor schizophrenia model), SFS (Social Functioning Scale) and the WHOQOL-BREF questionnaire. Results: At T1, the mean PANSS score was 62.4. Significant deterioration has been noticed in all groups of symptoms, with the exception of cognitive functions. The greatest deterioration was noticed in the domains of negative symptoms and depression/anxiety. At T1, the mean SFS score was 103.4, while mean subjective quality of life and general health status scored 3.3 and 3.0, respectively. During the followup period, significant improvement of SFS score was noticed, with no meaningful change in WHOQOL-BREF score and concomitant exacerbation of symptoms as assessed by the PANSS scale. The patients’ quality of life was particularly compromised by negative symptoms and depression/anxiety. Correlation coefficients between SFS score and negative symptoms varied from -0.61 to -0.76, while those between SFS and depression/anxiety – from -0.40 to -0.66. Correlation coefficients between subjective quality of life and negative symptoms ranged from -0.31 (domain Environmental) to -0.64 (domain Physical). To a large extent, symptoms were responsible for variance in both SFS and WHOQOL-BREF scores. Conclusions: 1) Clinical course of schizophrenia varied, depending on adopted criteria of evaluation. 2) Negative symptoms and depression/anxiety had a particularly deleterious influence on quality of life. The role these symptoms varied, depending on time elapsed since the first psychiatric hospitalisation.
Cel: Analiza zależności między objawami psychopatologicznymi a jakością życia chorych na schizofrenię w ocenie prospektywnej w okresie 4-6 lat po pierwszej hospitalizacji psychiatrycznej. Materiał i metody: Zbadano 74 pacjentów (K=28, M=46) po upływie 1 miesiąca od pierwszej hospitalizacji (badanie I), po 13 miesiącach (badanie II) oraz po 4-6 latach od badania I (badanie III). W badaniu zastosowano skalę PANSS (5-czynnikowy model schizofrenii), Skalę Funkcjonowania Społecznego (SFS) oraz kwestionariusz WHOQOL-BREF. Wyniki: W badaniu I punktacja PANSS wynosiła 62,4 pkt. Stwierdzono istotne nasilenie wszystkich grup objawów oprócz zaburzeń poznawczych. Największe nasilenie dotyczyło objawów negatywnych oraz depresji/lęku. W badaniu I średni wynik SFS wynosił 103,4 pkt, a średnie oceny subiektywnej jakości życia: dla ogólnej oceny jakości życia – 3,3 pkt, dla ogólnej oceny zdrowia – 3,0 pkt. W okresie obserwacji stwierdzono istotną poprawę SFS, brak istotnych zmian WHOQOL-BREF i jednoczesny wzrost nasilenia objawów (PANSS). Odnotowano szczególnie niekorzystny wpływ objawów negatywnych oraz depresji/lęku na jakość życia. Współczynniki korelacji SFS z objawami negatywnymi wynosiły od -0,61 do -0,76, a z depresją/lękiem od -0,40 do -0,66. Współczynniki korelacji subiektywnej jakości życia z objawami negatywnymi wynosiły w zależności od dziedziny od -0,31 (dziedzina Środowisko) do -0,64 (Dziedzina Fizyczna). Objawy wyjaśniały znaczną część wariancji SFS i WHOQOL-BREF. Wnioski: 1) Przebieg schizofrenii był zróżnicowany w zależności od przyjętego kryterium oceny. 2) Szczególnie niekorzystny wpływ na jakość życia wywierały objawy negatywne oraz depresja/lęk. Rola tych objawów była zróżnicowana w zależności od upływu czasu po pierwszej hospitalizacji.
