This article concerns the situations experienced by the researcher in one of the sensitive research groups—hospice patients. The article is based on the author’s experiences in three studies in Poland—94 in-depth interviews and observations in inpatient and home care hospices. Through the seven presented categories the author faced during the interviews, she analyzes the dilemmas of conducting qualitative research from a practical perspective. During studies, we learn about our preferences, sometimes defining ourselves on one of the sides—becoming a quantitative or qualitative researcher, thus deciding further scientific paths. Conducting qualitative research requires specific activities, including knowledge of the literature, selection of the proper method, and analysis of the research group (Silverman 2012). These principles turn out to be only the beginning of the process in which we intuitively, through trial and error, pave the way to deal with demanding situations, previously inexperienced emotions, coordination, and technical and ethical problems. Some studies require special preparation, particularly considering the specificity of certain research groups, such as hospice patients, who will face the dying process soon.

Celem niniejszego artykułu jest wstępne zbadanie oraz porównanie specjalistycznego dyskursu medycznego oraz dyskursu zapośredniczonego komputerowo dotyczących COVID-19 na podstawie polskojęzycznych publikacji naukowych oraz wpisów na forum internetowym. Są to dyskursy tworzone odpowiednio przez profesjonalistów jak i przez laików, którzy opisują konkretne przypadki choroby. Badanie to skupia się na językowej konstrukcji takich elementów jak choroba czy pacjent oraz konsekwencji konkretnych wyborów językowych ukazujących doświadczenie choroby autorstwa różnych osób, w konkretnych kontekstach oraz różnymi kanałami, przeznaczonych dla różnych czytelników. O ile istnieje szereg badań dotyczących nowych jednostek leksykalnych oraz tych, które zyskały nowe znaczenie/nową rzeczywistość do opisu w języku angielskim, wydaje się, iż porównanie dwóch dyskursów dotyczących COVID-19 tworzonych przez profesjonalistów jak i laików w języku polskim nie zostało dotychczas przeprowadzone. Dane w niniejszym badaniu stanowią polskojęzyczne artykuły naukowe dotyczące koronawirusa oraz wpisy na forum internetowym o tej samej tematyce. Zastosowana metodologia to analiza z perspektywy jakościowej, uzupełniona o elementy badania korpusu wspartego komputerowo i odnosi się gramatyki dyskursu o podstawach kognitywnych oraz funkcjonalnych. Zwraca się w niej uwagę na tekstowe wyznaczniki pacjenta i choroby, oraz ich tekstowe uwarunkowania, które mogą się różnić w zależności od kontekstów, w których tworzone są teksty. Jak pokazują wyniki badania, obie grupy tekstów charakteryzują się innymi punktami ciężkości oraz użytymi środkami językowymi (np. słownictwo, frazy czy struktury dyskursywne), które odpowiadają za różny efekt końcowy językowej reprezentacji COVID-19.
The aim of the article is to determine preliminarily the character of the specialised medical discourse as well as of computer-mediated discourse about COVID-19 on the basis of Polish language professional publications and entries on an Internet forum. The intention here is to examine and then juxtapose the two discourses as used by medical professionals and lay people respectively, about the same disease event, described in particular cases. The proposed research will focus on the linguistic construction of the concept of the disease as well as of the patient and the consequences of linguistic choices selected as mediating illness experience with respect to different contexts, channels, authorship, and readership. While novel language items or metaphors related to COVID-19 in English have received researchers’ attention, the present author knows of no study in which the discourse about COVID-19 in Poland would be analysed and compared in both professional and lay contexts. The data for the current study are constituted by scientific articles related to COVID-19 derived from Polish medical journals for professionals, as well as the entries from an Internet forum on which the issues connected with COVID-19 are discussed. The study was conducted following the principles of qualitative discourse analysis combined with some insights from a computer-aided examination of the corpus, and refers to cognitively and functionally based grammar of discourse, paying attention to such elements as patient and disease references and their co-texts, which may vary in the contexts from which the texts analysed were derived. The results demonstrate that the two groups of texts use different communicative accents, as well as linguistic means (e.g., vocabulary, phrases, or discourse structures, etc.), which, consequently, produce different effects.

The aim of the article is to present unmet expectations and needs of patients concerning the scope and quality of medical care. The complaints submitted with the Patients Ombudsman at the Ministry of Health have been deemed as exemplary, since this is a central authority of governmental administration relevant in cases regarding the protection of patients specified in the act, which encompasses the entire country in the scope of its powers. This is also the authority which has most often appeared in the mass media, hence it is the most probable thatthe patients will address exactly this institution with any problem they may have (very often without knowing any other way or procedure). The aim of the article is also to present selected strategies assumed by thepatients to allow them to solve the encountered difficulties. Recipients of benefits more and more effectively manage and undertake effective actions aimed at obtaining assistance to the extent they expect. They adopt attitudes proving the assumption of responsibility for their own health (empowered), which is proved even by their Internet activity to the extent of which they search for information on e.g. disease prevention, treatment, application of medicines or good nutrition (however, the conclusions in this regard one cannot generalize interms of the whole population of people enjoying medical benefits). In addition, the article presents also a process of changes with respect to the attitudes towards the value of health and changes occurring in the profile of the modern patient. Presentation of the selected problem area was made on the basis of the results of qualitative analysis of the scope and nature of the complaints submitted with the Patients’ Ombudsman (2009–2013) as well as a selective review and survey of the literature and its analysis.

The present paper constitutes a qualitative discursive investigation of the nineteenth century case reports from ophthalmology derived from a specialist American medical journal, and focuses on citation as well as on authorial and patient’s presence. Regarding the first aspect, the present study generally confirms the results of the previous research, i.e. significant subjectivity, directness and informal character of the texts at hand. However, it also provides another insight into the scholarly communication in the nineteenth century, focusing on how patients are positioned therein, which has not received significant attention in the literature of the subject. Additionally, the paper offers an overview of the studies on specific aspects of scientific prose of the nineteenth century, including the medical context, as well as attempts to show the relation between the texts analysed and the context of their production and functioning, following the tradition of Genre Rhetorical Studies.

This article discusses the patient-doctor relationship in terms of Rita Charon’s concept of narrative medicine. The discussion is based on an interpretation of Intoxicated by My Illness and Other Writings on Life and Death, a collection of essays written by American writer and literary critic Anatole Broyard after he was diagnosed with prostate cancer. Writing about the ordeals of life and death, the author devoted a lot of attention to his meetings with doctors. Particularly in the essay The Patient Examines the Doctor, he points to a certain power struggle that takes place between the person who is ill and their physician. While physicians are generally believed to be in a privileged position, Broyard argues that they are also subjected to a similar, often equally painful, diagnosis process. His reflections on an ideal doctor (embodied by Oliver Sacks) turn out not to be completely utopian. Confronted with Rita Charon’s theses on narrative medicine, they seem to gain scientific confirmation.

Ewolucja hierarchicznej zależności pacjent-opiekun między głównymi bohaterkami filmu Wygrać z Losem, May-Alice Culhane and Chantelle, którą dodatkowo komplikują różnice rasowe widoczne w relacjach białej pani domu a jej czarną służącą, w dużej mierze dokonuje się poprzez łączące kobiety czynności związane z jedzeniem – jego przygotowywaniem, podawaniem, z funkcjonowaniem potraw zarówno w życiu codziennym, jak i w momentach uroczystych. Celem artykułu jest prześledzenie i zanalizowanie roli posiłków jako czynnika stymulującego przebieg tego procesu. Szkic przedstawia, w jaki sposób jedzenie pomaga May-Alice i Chantelle zbudować relację interpersonalną opartą na czułości oraz porozumieniu, pozwala im, dzięki odrzuceniu uprzedzeń rasowych, wyjść poza paradygmatyczne role białej pani domu i jej czarnej służącej. Definiując swoje relacje między zależną pacjentką i odpowiedzialną opiekunką, dwie kobiety muszą skonfrontować swoje uprzedzenia, stanąć twarzą w twarz ze sobą, przyjąć postawę empatycznego współdziałania.
A transformation of hierarchical patient-caregiver relationship between May-Alice and Chantelle, complicated by racism inscribed in the relationship between a white mistress and a black helper, is negotiated through the use of food – its preparation, serving, and daily as well as festive meals. It is my intention to trace and analyze the role of food in the evolution of their relationship. I will demonstrate that food helps these women to build affectionate coexistence and meaningful communication across the color line, rather than re-inscribes racial asymmetry into their relationship through references to the served and the servant dichotomy.

This paper explores a range of health care seeking actions adopted by Polish migrants living in London. It is based on the in-depth interviews with 62 Polish migrants who resided in London in 2007 and 2008. The study reflects experiences of a diverse group of participants encompassing individuals of different ages, family circumstances and employment statuses. It uncovered a number of actions such as avoiding contact with any health services, self-medication, utilising Polish private doctors in London, and accessing public health services in Poland and London. These services and strategies were often used successively, concurrently or interchangeably depending on individual’s assessment of their situation and circumstances at the time of experiencing a particular medical need. The research uncovered a prominence of transnational health care seeking practices as most participants continued to access at least some form of health care in Poland. It also noted the impact of the length of stay, knowledge of English and labour market position on patterns of health care use. Engagement with the British National Health Service (NHS) was characterised initially by lack of knowledge of the system resulting in meandering between different institutions. There was also a sense of cultural unease and mismatch of expectations characterising doctor-patient encounters. However, the pathways of access and attitudes towards the NHS began to change as migrants learned their way through the English health care system. This underlines an importance of taking into account a longitudinal dimension in studies of migrant health care seeking behaviour.

Twenty-three patients with a putative diagnosis of Prader-Willi syndrome (PWS) were reassessed clinically and then examined by cytogenetic and molecular techniques. Nineteen patients fulfilled the diagnostic criteria for PWS and the remaining four were judged to have atypical PWS. Definite molecular defects were detected in all clinically typical PWS patients but one. A deletion of part of chromosome 15q could be identified molecularly in 14 patients (74%) and maternal disomy for chromosome 15 in four (21%). In all, except one, PWS patients with molecularly detected deletions, the deletion was also identified by cytogenetic studies. Cytogenetic deletions were suspected in two of the atypical PWS patients. However, based on the results of scoring the diagnostic criteria for PWS and on the PW71B methylation test, we were able to rule out PWS in all of our atypical patients. Our study confirms observations that both clinical and cytogenetic investigations can provide misdiagnoses of PWS in some patients, and the first, simple and fast investigation, which can confirm the PWS in most, if not all PWS patients, is molecular analysis by the methylation test.

Purpose: To establish the relation between the reasons one registers to a primary care physician and a patient's expectations of and satisfaction from the medical services provided. Material and methods: A total of 422 patients from public and non-public health care centres participated in the research. The research was carried out in 20 institutions of public and non-public centres for primary care in the Swiętokrzyskie Voivodeship. A Patient Request Form was used as the research tool. The Polish version of the Patient Request Form (PRF) is composed of 18 statements concerning different reasons for the present contacting of a general practitioner. Comparative analysis of interval or ratio scale type variables was performed using variance analysis, which in the case of significance of the main effects or interactions was supplemented by post-hoc analysisResults: After a repeated measures analysis of variance was done, a significant variation was found in terms of the type of institution: a public and non-public health care centre (p=0.01955). The effect of the type of expectation also proved significant (p=0.00000). A significant interaction was also found between the type of institution and the kind of PRF (Patient Request Form) (p=0.00985). A significant effect (p=0.00805) was obtained using single-classification analysis of variance upon analysing the assessment of the treatment conditions at a primary care clinic (JUM2) in terms of the type of institution.Conclusions. Patients participating in the study indicated expectations related with an explanation of the illness and obtaining information about test results and further treatment as the reasons for the present registration to a primary care physician. As regards assessment of the treatment conditions (JUM2) at primary care clinics in view of the type of institution and area of services, the patients participating in the study assessed non-public health care centres more highly.

The economic importance of therapy compliance has grown steadily in recent years, not only because of the efficacy of newer therapeutic methods but also because of the increased costs of treating the consequences of poor compliance. Improved compliance can lead to significant savings by preserving or restoring a patient's health, improving quality of life, by reducing the number of medical services required when therapy fails or appears ineffective, and by helping limit the rise in national health care costs. Within the framework of the TEN-Telecom European Commission Programme, C-Monitor project developed an integrated health telematics platform to enhance chronic patient compliance to therapy and interactive communication with their attending physicians. The overall aim of the project was to study the potential benefits, both in clinical and financial aspects, of such innovative systems and services along cost-efficiency of care provision. The platform developed has been validated in controlled small-scale trials in a number of European countries. The Greek pilot involved installation of the system in a private hospital in Athens and the validation scenario dealt with morbid obesity patients that have undergone surgical operation. A number of 30 patients were recruited for the trial. Results of the trial indicated the technological robustness of the proposed system and the potential clinical and economic benefits of running such services. Further trials are required to better address cost-efficiency issues with respects to the service goals.

This study contains an analysis of legal regulations on the access to patient’s medical records and the compliance with these regulations in Poland. Based on the extensive case-law of the administrative courts, the following were examined, i.a., the forms of making medical records available, with particular respect to the making available of the original of these records, the possibility of charging fees for the provision of medical records, the form of the request for access to medical records and the time within which the provider of health services is required to make that documentation available. The list of entities authorised to get access to patient’s medical records has been discussed and attention has been drawn to problems in gaining access to the medical records of a deceased patient.
Przedmiotem niniejszego opracowania jest analiza regulacji prawnych dotyczących dostępu do dokumentacji medycznej pacjenta oraz ich przestrzegania. W oparciu o bogate orzecznictwo sądów administracyjnych poddano rozważaniom m.in. formy udostępnienia dokumentacji medycznej ze szczególnym uwzględnieniem udostępniania oryginału tej dokumentacji, możliwość pobierania opłat za udostępnienie dokumentacji medycznej, formę wniosku o udostępnienie dokumentacji medycznej oraz czas, w którym podmiot udzielający świadczeń zdrowotnych zobowiązany jest udostępnić tę dokumentację. Omówiono również katalog podmiotów uprawnionych do uzyskania dostępu do dokumentacji medycznej pacjenta oraz zwrócono uwagę na problemy z uzyskaniem dostępu do dokumentacji medycznej zmarłego pacjenta.
