Obesity, which occurs both in rich as well as developing countries, is a chronic disease that can cause many serious illnesses. It has become a global health problem, which affects onethird of adult population in the world. To induce weight loss, profound lifestyle modification is required. In the case, when lifestyle changes alone are insufficient, pharmacotherapy can be helpful. Most of anti-obesity drugs (e. g. fenfluramine, sibutramine, rimonabant), which have been approved for treatment of obesity, were subsequently withdrawn from the market due to their adverse effects. In this manuscript, a short overview of currently available anti-obesity drugs like orlistat, lorcaserin, liragutide as well as phentermine/topiramate and naltrexone/ bupropion combination, was presented. Herein we focus on efficiency, mechanism of action and possible side effects of anti-obesity drugs. Finally, it was emphasized that better understanding of mechanisms involved in appetite regulation could be useful for developing novel, more effective and better tolerated anti-obesity drugs.

Introduction. Obesity in children and adolescents is a growing problem in the 21st century. The epidemic of chronic non-communicable diseases resulting from obesity is currently one of the biggest problems of modern medicine. Excessive body weight is the result of a long-lasting imbalance between the amount of energy supplied and its expenditure. Energy regulation of the body is subject to both genetic and environmental factors. Among other things, due to this, the problem of excessive body weight is most severe in societies with a high degree of socio-economic development. The aim of this paper is to determine the influence of selected environmental and social factors on the occurrence of overweight and obesity in pre-school children. Material and methods. The study included pre-school children from south-eastern Poland. After obtaining the consent from parents, 200 children (87 boys, 113 girls) aged 3 to 6 years were examined. A questionnaire used for the research was derived from the program: European Pilot Study Evaluating the Influence of Local Promotional Activities on Prevention of Obesity in Pre-school Children. In the subjects, body weight was measured on an electronic scale three times and the body height was measured three times using a stadiometer. Obesity was determined according to the criteria developed by the International Obesity Task Force (IOTF). Results. The prevalence of overweight and obesity among children amounted to 6.4% in 3-year-olds, 11.3% in 4-year-olds, 17.7% in 5-year-olds, and 20.7% in 6-year-olds. A factor significantly increasing the risk of obesity among the examined girls and boys was the mother’s BMI index. Conclusion. Although knowledge about the factors that promote overweight and obesity is common, it is still a common health problem. Particular attention should be paid to the prevention of obesity in children of parents with a BMI above 30 kg m2. Early maternal education can change the lifestyle of the whole family.

Introduction. Obesity is today one of the most dangerous and the fastest growing civilization diseases in the world. The number of overweight or obese people is continually increasing. Obesity is defined as abnormal fat accumulation in an organism that may cause health impairment. Obesity may be conducive to an increased risk increase for occurrence of cardiovascular diseases as well as stroke, some types of cancer, endocrinal disorders, osteoarthritis and other bone disorders. Some studies have demonstrated that high body mass index (BMI) is protective against the development of osteoporosis and osteoporotic fractures in men and women. In slim people with a lower BMI than normal, weight loss is associated with low bone mineral density (BMD). On the other hand, obesity in childhood may lead to fragility fractures and may lead to early development of osteoporosis in adulthood. Currently, we have numerous methods for measurement of obesity such as dual-energy X-ray absorptiometry (DXA), bioelectrical impedance analysis (BIA), total body electrical conductivity (TOBEC) as well as magnetic resonance imaging (MRI) and computed tomography (CT). These methods are useful for diagnosing obesity and bone tissue disorders such as osteopenia with sarcopenia or osteoporosis, in particular in perimenopausal women and men after andropause. Aim of the study. The purpose of the study was review the literature on obesity and bone tissue disorders and their interrelations. Material and method. Analysis of literature.

Background: In 2020, Polish experts recommended the use of the guidelines of the American Association of Clini cal Endocrinologist and the American College of Endocrinology (AACE/ACE) in the diagnosis of obesity instead of the 1998 WHO criteria. Thus, obesity can be also diagnosed in patients with BMI 25.0-29.9 if there are complicaitons related to the excess body weight including infertility and hypogonadism, asthma, sleep apnea, gastroesophageal reflux disease, stress urinary incontinence, osteoarthritis and depression. Material and methods: The aim of the study was to assess the prevalence of obesity diagnosed according to the new criteria in the group of Polish social media users. Results: Among the respondents, 9.7% had a BMI below 18.5 kg/m2, 63.9% declared normal BMI, and 18.8% had BMI 25.0-29.9 kg/m2. The value of BMI ≥ 30 kg/m2, equivalent to the diagnosis of obesity according to WHO, was found in 7.6% of the respondents. For comparison, the prevalence of obesity according to the AACE/ACE criteria was more than twice as high (17.2%, p < 0.05). Conclusions: Concluding, according to the new criteria, more respondents were diagnosed with obesity compared to WHO criteria. The results of the study indicate that the application of the new diagnostic criteria may enable earlier diagnosis and thus earlier treatment of obesity. Samek G, Szulc A, Dardzińska JA. The aim of the publication is to present the problem of overweight and obesity in children and adolescents, along with general health consequences resulting from the problem, based on a review of literature. Obesity and overweight in childhood and adolescence, result in problems with maintaining the right body weight in adulthood. The problem of obesity is quite complex and not only medical aspect but also cultural, economic and social aspects can be easily identified within it. Preventive and therapeutic activities undertaken in childhood could help prevent many health problems in adulthood. An effective therapy should be conducted by parents and a group of professionals. It is necessary to disseminate health promotion programs and achieve high levels of effectiveness in preventive activities among young people, which will improve the health state of obese children and reduce a growing number of children with excessive body weight .
Celem artykułu jest przedstawienie problemu nadwagi i otyłości w populacji wieku rozwojowego wraz z jego konsekwencjami ogólnozdrowotnymi w oparciu o przegląd literatury. Otyłość i nadwaga w wieku dziecięcym i młodzieńczym, skutkuje problemami z utrzymaniem prawidłowej masy ciała w wieku dojrzałym. Problem otyłości jest dość złożony, bez trudu można wyodrębnić w nim aspekt nie tylko medyczny, ale także kulturowy, ekonomiczny oraz socjalny. Działania profilaktyczne i lecznicze podejmowane w dzieciństwie mogłyby pozwolić na uniknięcie wielu problemów zdrowotnych w wieku dojrzałym. Skuteczna terapia powinna być prowadzona przez rodziców oraz grono profesjonalistów. Trzeba dążyć do upowszechniania programów promocji i osiągać wysokie wskaźniki skuteczności działań profilaktycznych wśród młodzieży, które poprawią stan zdrowia dzieci otyłych oraz zmniejszą rosnącą liczbę dzieci z nadmierną masą ciała.

Background. Smart device usage has become favorable among children worldwide. Objectives. The study aimed to identify the relation between usage of electronic devices with obesity and speech delay. Material and methods. A cross-sectional study was conducted among 452 healthy children (18 months to 14 years old) from pediatric clinics of the Ministry of Health (MOH), Jeddah, Saudi Arabia. Data was collected from June to July 2018. Analysis included linear regression, logistic regression, chi-square, t-test and ANOVA. Results and discussion. Male participants totalled 57.7% and females 42.3% (mean age 82.27 months) (SD = 40.18). Mean duration of usage: 3.1 hours (SD = 2.58) per day. Among toddlers only, 31.1% had speech delay. The results showed no relation between the duration of smart device usage and obesity (p-value = 0.904) or speech delay (p-value = 0.538). Duration of usage was not influenced by gender or parents’ marital status; however, children who live with both parents spent less time on smart devices than others. The smart device most used was a tablet (47%), and the main usage was primarily entertainment (60.8%) and games (47.6%). Only 57.8% reported having parental supervision. Among children, 59.3% eat chips and 48.9% eat candy as snacks during usage. Among parents, 71.5% believe devices reduce children’s physical activities, 64.8% believe that the smart device is a problem, 62.5% of parents should control time of use, and 60.5% believe that their children are attached to the devices. Conclusions. There is no association between the duration of using smart devices and obesity, nor speech delay. Future directions and recommendations should be discussed.

Introduction and objective. A decreased concentration of iron, and consecutively haemoglobin, ferritin and decreased level of saturated transferrin, were observed in obese individuals more often than in healthy subjects. The purpose of this study was to determine whether iron, ferritin, transferrin saturation are significantly diminished in obese female patients compared to non-obese counterparts, and whether excess adiposity and inflammation were associated with depleted iron. Materials and methods. Female patients (n=48) diagnosed with obesity (BMI > 30 kg/m2), aged 18–40 were accepted for the study. A control group (n=30) encompassed normal weight women, aged 18–30. All obese women obtained an individually adjusted dietary plan with an energy content of 1,500 kcal. Blood glucose, insulin, lipids, ferritin, TIBC and iron concentrations were assayed in serum twice, initially and after 8 weeks of dieting. Results. The obese women at the initial evaluation, in comparison to non-obese control women, were characterized by a significantly lower mean red blood cell volume (MCV; 84.2±12.4 vs. 91.3±9.3 fL; p<0,0001), serum iron level (92.6±42.4 vs. 119.8±44.0 μg/dL; p<0.01), and transferrin saturation (TSAT; 25.9±12.7 vs. 38.8±15.7%; p<0.01), but by higher plasma level of the C-reactive protein (CRP; 7.0±6.7 vs. 1.2±1.3 mg/L; p<0.01). The obese women after 8 weeks of diet decreased their mean total body weight from 104.1±21.3 to 99.2±20.7 kg (p<0,0001). CRP level decreased slightly but significantly from 6.9±7.1 to 6.2±7.5 (p<0.05). Conclusions. Obese women exhibit an increased level of CRP which may affect iron homeostasis. Weight loss leads to decrease in the CRP level, but it does not change haematologic parameters in the period of 8 weeks.

Purpose: The purpose of the study is to investigate the course of acute pancreatitis in obese patients, the development of local and systemic complications and mortality rates. Materials and methods: We took and analyzed 482 histories of acute pancreatitis treated at Kyiv Regional Clinical Hospital from January 1, 2011 to February 2, 2019. The data were statistically processed in the Excel 2010 program using a descriptive method applying relative, absolute numbers, mean square deviations and their errors. A correlation between variables was studied using the Pearson’s test (R2). The significance of the difference between the two independent groups was tested with Student’s t-test. Results: We included 482 patients in our study, i.e. 260 patients (54%) with obesity (the study group), and for comparison, 222 (46%) patients with normal body mass, constituting a control group. Obese patients had a higher mean age (55.4 ± 9.4 years, P = 0.01); also, they showed a statistically higher incidence of severe course of acute pancreatitis [85 (32.7%) vs. 16 (7.2%); P = 0.01*]. We noted an increase in the rate of acute pancreatitis with severe course in obese patients with mass gain (from 10.20% to 53.93%, P = 0.03*). Hospitalization time of obese patients was longer than in case of patients with normal body mass. In addition, we observed a two-fold longer hospitalization of obese patients at intensive care units (5.8 ± 0.8 vs. 2.7 ± 0.5 days, P = 0.01*). When investigating the mortality rate, we found out that the main cause of death was the progression of organ failure – 30 cases (6.3%), pulmonary embolism (TB) – 15 (3.1%) and DIC – 18 (3.7%). C onclusions: The presence of obesity in patients involves a high risk of severe acute pancreatitis. This risk increases with body mass increase. In addition, in obese patients the hospitalization and in-patient care takes longer, which increases the total cost of treatment and requires a cost-effective algorithm in the future. A high mortality rate in obese patients requires an improved treatment algorithm.

The contemporary trend towards elevated consumption of processed food and a sedentary lifestyle has induced noticeable alterations in body composition observed in the population. These unprecedented changes have given rise to the emergence of novel pathological conditions, including sarcopenic obesity. This condition is characterized by an excessive accumulation of body fat in conjunction with skeletal muscle depletion and functional impairments. Diagnosis hinges upon a comprehensive evaluation of medical history and the performance of functional and imaging studies. The therapeutic strategy relies on the appropriate dietary regimen and increased physical activity. Current scientific research in the domain of sarcopenic obesity is advancing significantly, with potential pharmacotherapy involving novel weight-loss medications and other strategies addressing this health issue.
Współczesny trend związany ze wzmożonym spożyciem wysoko przetworzonej żywności oraz prowadzeniem siedzącego trybu życia spowodował zauważalne zmiany w składzie ciała obserwowane w populacji. Te bezprecedensowe zmiany skutkują pojawieniem się nowych jednostek chorobowych, takich jak otyłość sarkopeniczna. Stan ten charakteryzuje się nadmiernym gromadzeniem tkanki tłuszczowej w połączeniu z utratą masy mięśniowej oraz ograniczeniami funkcjonalnymi. Postępowanie diagnostyczne opiera się na dokładnym wywiadzie lekarskim oraz przeprowadzeniu badań czynnościowych i obrazowych pozwalających na rozpoznanie tej patologii oraz wdrożenie odpowiedniego postępowania terapeutycznego opartego na zbilansowanej diecie i zwiększeniu aktywności fizycznej. Problematyka otyłości sarkopenicznej jest tematem wielu badań naukowych dotyczących możliwości farmakoterapii przy użyciu nowych leków redukujących masę ciała oraz innych strategii terapeutycznych.

Introduction. Cardiovascular diseases are responsible for about 50% of deaths in Europe. In Poland, mortality from stroke and heart attack is from approximately 1.5 to 3 times higher than in the old European Union. Alarming is the fact that the higher mortality compared to the countries of the EU regards largely the young and middle-aged. The most effective method to reduce morbidity and decrease mortality is primary prevention. Aim. The aim of the study was to assess the risk of a cardiovascular event according to SCORE algorithm with the population of Primary Health Care (PHC) patients participating in the Program for the Prevention of Cardiovascular Diseases in Silesian Province. Material and methods. The preventive program was aimed at people with risk factors included in the list PHC medical clinic in the age group with the highestrisk of cardiovascular disease, ranging from35 to 55 years of age. The research was carried out with the use of the Charter of Prophylactic Examination from January to December 2014. The study included 142 patients (86 women - 60.56% and 56 men 39.44%). Results. The factors affecting the increased incidence of cardiovascular disease among the studied population was obesity, which appeared in 21.13% of the population surveyed, very low physical activity 83.1%, diabetes 4.23%, hypertension 21.83%, increased values of cholesterol (63.38%), LDL cholesterol (60.56%) TG (19.72%) and smoking (61.11%). Conclusions. In the study population as an example of SCORE there were mostly people with moderate risk (56.34%) of a cardiovascular event within the next ten years, 26.06% of low risk and 14.79% with an increased risk. Only in 2.82% of respondents a significant risk of the disease was identified. In the result of preventive examinations 5.63% of patients were diagnosed with cardiovascular disease and referred for further evaluation or treatment.
Wstęp. Choroby sercowo-naczyniowe odpowiadają za około 50% zgonów w Europie. W Polsce umieralność z powodu udaru mózgu i zawału serca jest około 1,5 do 3 razy wyższa, niż w krajach starej Unii Europejskiej. Niepokojący jest fakt, iż wyższa umieralność w stosunku do krajów UE dotyczy w znacznym stopniu ludzi młodych i w średnim wieku. Najskuteczniejszą metodą ograniczenia zachorowalności oraz zmniejszenia umieralności jest profilaktyka pierwotna. Cel badań. Celem badań była ocena ryzyka wystąpienia incydentu sercowo-naczyniowego według algorytmu SCORE wśród populacji pacjentów Podstawowej Opieki Zdrowotnej (POZ) biorących udział w Programie Profilaktyki Chorób Układu Krążenia w województwie śląskim. Materiał i metoda. Program profilaktyczny skierowany był do osób obciążonych czynnikami ryzyka znajdujących się na liście lekarza poradni POZ w grupie wiekowej najbardziej zagrożonych chorobami układu krążenia, tj. od 35 do 55 roku życia. Badania przeprowadzono przy zastosowaniu Karty Badania Profilaktycznego od stycznia do grudnia 2014 roku. Badaniami objęto 142 pacjentów (86 kobiet - 60,56% i 56 mężczyzn 39,44%). Wyniki. Czynnikami wpływającymi na zwiększoną zachorowalność na choroby sercowo-naczyniowych wśród badanej populacji była: otyłość, która występowała u 21,13% badanej populacji, zbyt niska aktywność fizyczna 83,1%, cukrzyca 4,23%, nadciśnienie tętnicze 21,83%, podwyższone wartości cholesterolu (63,38%), LDL (60,56%), trójglicerydów (19,72%) czy palenie papierosów (61,11%). Wnioski. W badanej zbiorowości na przykładzie SCORE przeważały osoby o umiarkowanym ryzyku (56,34%) wystąpienia incydentu sercowo-naczyniowego w ciągu najbliższych dziesięciu lat, 26,06% z małym ryzykiem oraz 14,79% ze zwiększonym ryzykiem. Tylko u 2,82% respondentów stwierdzono znaczne ryzyko wystąpienia choroby. W wyniku badania profilaktycznego u 5,63% pacjentów rozpoznano chorobę układu krążenia, którzy zostali skierowani na dalszą diagnostykę lub leczenie.

Introduction: One of the major nutritional mistakes committed by children and adolescents is snacking between meals. Sweet or salty snacks are rich in simple sugars, fats, preservatives and dyes, and poor in minerals and vitamins. Their excessive consumption can lead to the development of diet-related diseases in the future. Purpose: To evaluate the nutrition of children and adolescents with a focus on snacking between meals. Materials and methods: The study included 162 students from Bialystok schools, of which primary school students accounted for 30.2%, junior high school 38.3%, high school 31.5%. The questionnaire used by the authors contained 27 questions. Results: Regular meals were consumed by only 55.1% of primary school students, 35.5% of junior high school, and 37.3% of high school students. Two and fewer meals a day were consumed by 1.6% to 5.8% of the students studied. First breakfast was omitted by 18.4% of the students in elementary school, 16.1% of junior high school, and 15.7% of high school. Second breakfast was omitted by 12.9% to 17.6% of the respondents.Sweet products instead of a second breakfast were consumed by 44.9% of primary school students, 61.3% of junior high school, and 64.7% of high school students. Snacking most often occurred in the respondents' home (69.4% of elementary school students, 43.5% of secondary level students, and 52.9% of high school students). Water in school was drunk by 85.7% of primary level students, 56.5% of junior high school, and 51.0% of high school. The main source of knowledge about nutrition for 61.2% of primary level students was the family, while television, radio, and the Internet were the main sources of knowledge for 62.7% of high school students. Conclusions: The regularity of eating in the study group decreased with the studied students’ age. The most popular products in the group of younger students were salty snacks and sweets. Healthy snacking was observed more frequently in the group of high school students. The main place of snacking was the home. The primary source of knowledge about nutrition for primary school students was the family and for high school students the mass media.

Introduction. One of the reasons of obesity in university students might be mindful eating. Aim. This study was performed to evaluate the relationship between mindful eating, body mass index (BMI) and physical activity in nursing students. Material and methods. This cross-sectional study was conducted with 718 nursing students in a university in Turkey. “Personal Information Form” and “Mindful Eating Questionnaire (MEQ)” were used as data collection tools. Results. Statistically significant differences were found between some mean subscale and total scores of MEQ based on sex of the students, their state of exercising regularly, number of their meals, their state of having snacks, the type of bread they often consumed and the frequency of eating fast foods (p<0.05). A significant relationship was found between age and BMI of the students and their mean subscale and total scores of MEQ (p<0.05). Conclusion. The older the nursing students grew, the higher their level of mindful eating became. Sociodemographic characteristics such as sex and presence of a person with obesity within the family affected subscales of their mindful eating. Mindful eating decreased as body mass index increased; and besides, mindful eating increased as physical activity increased.

This study is to analyze the effectiveness of an incentive-based obesity management program (the Midas Project aimed to improve good health habits) at an electronics company in 2005. A total of 95 company participants with a high body mass index (BMI) were recruited for a health promotion program for 3 months that awarded gold medals as an incentive for body fat loss. BMI decreased from 28.8 to 27.8 kg/m² (p = .000), body weight decreased from 87.2 to 83.5 kg (p = .000), and body fat weight decreased from 25.4 to 23.3 kg (p = .000). Systolic and diastolic blood pressure decreased from 130.5 to 125.1 mmHg (p = .002), from 86.4 to 81.7 mmHg (p = .009). The percentage of participants exercising more than 3 times per week increased from 27.3 to 52.3% (p = .000). The percentage who avoided overeating at parties and midnight eating increased from 65.9 to 72.7% (p = .767) and 70.5 to 84.1% (p = .172), respectively. This incentive-based obesity management program was effective in improving not only BMI but also health status.

Introduction: The prevalence of obesity in Poland and worldwide is constantly rising. High effectiveness of bariatric surgery has been proven in literature. It is recommended that bariatric procedures should be done by highly qualified surgeons with the appropriate, up-to-date medical equipment. Aim: The purpose of the study is to establish Polish recommendations and standards for the use of medical equipment for bariatric surgery centers. Materials and methods: The review of the present recommendations of the worldwide organizations and societies (including EAES, IFSO, SAGES) and guidelines was made. On the basis of current literature and authors’s clinical experience we proposed standardized protocol for bariatric surgical equipment. Conclusions: Relevant equipping of bariatric surgery centers and implementation of standardized perioperative and surgery protocols will result in significant improvements in bariatric treatment. This will ensure patients safety, a shorter length of hospital stay and considerably reduce the risk of morbidity. Moreover, it will contribute to the efficacy of the bariatric and metabolic surgery procedures, in accordance with the highest globally accepted standards.
