Opis:
Mutations in CDKN2A have been found in sporadic cutaneous malignant (CMM), in familial CMM and in other syndromes associated with melanoma. In this study DNA was obtained from 207 individuals and five cell lines. There were 157 CMM patients and 50 healthy members of melanoma patients families. The CMM group included patients with one or two melanoma cases in the family, families with dysplastic nevus syndrom (DNS) and patients with a spectrum of other types of cancers in the family. PCR-SSCP analysis and sequencing identified: six substitutions in codon 58 CGA/TGA (Arg/Stop), 16 substitutions GAC/GAT in codon 84 (Asp/Asp), six substitutions CGA /TGA in codon 148 (Arg/Thr), 14 substitutions G/C in 3'UTR and 4 double changes (two in codon 84 and 3'UTR; two in codon 148 and 3'UTR). The mutation identified in codon 58 was found in tissue only. Other substitutions were polymorphisms found in DNA from tissue and blood samples. Most of them were identified in sporadic CMM (six in codon 148 Ala /Thr, 12 in codon 84 Asp/Asp and six in 3'UTR). The frequency of the polymorphisms was also high in DNS and CMM/DNS families (four in codon 84 Asp/Asp and six in 3'UTR). No mutations or polymorphisms were found in CMM patients with one or two melanoma cases and CMM patients, with other cancers in family history. The analysis of the CDKN2A gene mutations in the Polish population demonstrated: (i) no germline mutations; (ii) a relatively high number of genetic changes in sporadic melanoma; (iii) a high number of polymorphisms in DNS and CMM/DNS families.

Malignant melanoma is a highly invasive cancer which derives from neuroectodermic melanocytic cells. It is characterized by the highest frequency of cases in both sexes and all age groups. High metastatic potential of melanoma and its multidrug resistance might be a significant problem in the treatment. Permanent and complete remissions are rare, while the 5-year survival rate remains relatively low. In the last years there has been a breakthrough in the systemic therapy of malignant melanoma. Modern drugs have revolutionized previous therapeutic management of melanoma. As part of therapeutic programs, new medicines are available: dabrafenib, ipilimumab, pembrolizumab, trametinib and vemurafenib. The following paper presents current clinical management standards for melanoma diagnoses, based on the widely accepted scientific data and expert experience - with emphasis on modern methods of treatment of this cancer.

We report the synthesis, photochemical and photophysical properties and preliminary studies on biological effect of a new tritolylporphyrin dimer (T-D). Absorption and emission properties of T-D suggest its possible use in photodynamic therapy. T-D is capable of singlet oxygen production with 0.8 quantum yield. It also has a high photostability. The photodynamic properties of the dimer were examined following the growth of SKMEL 188 (human melanoma) cells irradiated with red light (cut off <630 nm). The surviving fraction of the cells decreased about 3-fold (vs. non-irradiated cells) for an 81 J/cm2 dose. Our results suggest that tritolylporphyrine dimer T-D may be an interesting hydrophobic sensitizer for photodynamic therapy.

In Poland, morbidity and mortality rates for melanoma are constantly increasing. In the case of inoperable disease or distant metastases, prognosis remains poor. For many years, dacarbazine has been the gold standard in systemic treatment. Recently, a significant progress in melanoma therapy has been observed. Introducing targeted therapy or immunotherapy significantly improved treatment outcomes. This review paper presents current knowledge on systemic treatment of advanced melanoma, including treatment availability in Poland.

Spectroscopic, photochemical and biological properties of indocyanine green (ICG) are presented. Light over 800 nm is effectively absorbed by ICG. This property as well as photochemical behaviour of ICG make it a very suitable dye for photodynamic treatment of melanoma cells. Cytotoxicity of ICG itself and the effect of photodynamic therapy (PDT) were evaluated by following the growth of human (SKMEL 188) and mouse (S91) melanoma cells. The surviving fraction of the cells irradiated (λex = 830 nm) vs non-irradiated, treated with the same dose of ICG, is significantly decreased (5- to 10-fold). These results show that ICG is a very promising dye for photodynamic therapy of melanomas.

Angioarrestin is a recently described anti-angiogenic protein whose expression is down-regulated in solid tumours of various origins. It has a sequence identical to angiopoietin related protein-1. In this study we investigated anti-tumour properties of angioarrestin in B16 (F10) melanoma tumour model. We constructed an expression vector encoding human angioarrestin under the control of EF-1α promoter. This vector was transferred to B16 (F10) cells and recombinant angioarrestin secreted from the transfected cells was tested for anti-angiogenic activity using endothelial cell proliferation assay. Finally, mice were injected subcutaneously with cells that had been transfected with either angioarrestin-encoding vector or empty vector and tumor growth was compared. The obtained recombinant angioarrestin inhibited proliferation of bovine aortic endothelial cells. Tumours derived from an angioarrestin-secreting B16 (F10) cell clone grew in vivo more slowly than tumours derived from a cell clone transfected with empty vector. These data show, to our knowledge for the first time, that angioarrestin can inhibit primary melanoma tumour growth.

U psów stosunkowo często spotykamy w gałce ocznej czerniaki. W przeciwieństwie do czerniaków występujących u kotów, u psów rzadko dają one przerzuty. Wczesne wykrycie tego typu zmian w oku zapobiega dalszym komplikacjom. Praca ta przedstawia przypadek czerniaka tęczówki u psa rasy labrador retriever. Początkowo zmianę tęczówki leczono steroidami i antybiotykami. Lecz leczenie to nie przyniosło oczekiwanych rezultatów. Dalsze diagnozowanie (ultrasonografia, gonioskopia) wykazało, że jest to czerniak. Wykonano resekcję części tęczówki. Siedem tygodni po resekcji zmiany, powrócił odruch źreniczny i widzenie. Badanie histopatologiczne wykazało, że usunięta zmiana to melanocytoma.

Expression of N-cadherin an adhesion molecule of the cadherin family, in tumor cells is associated with their increased invasive potential. Many studies suggested the role of N-linked oligosaccharides as important factors that contribute to metastasis by influencing tumor cell invasion and adhesion. N-cadherin is a heavily glycosylated protein. We have analysed the carbohydrate profile of this protein synthesized in human melanoma cell lines: WM35 from the primary tumor site and WM239, WM9, and A375 from different metastatic sites. N-cadherin was immunoprecipitated with anti-human N-cadherin polyclonal antibodies. Characterisation of its carbohydrate moieties was carried out by SDS/PAGE electrophoresis and blotting, followed by immunochemical identification of the N-cadherin polypeptides and analysis of their glycans using highly specific digoxigenin or biotin labelled lectins. The positive reaction of N-cadherin from the WM35 cell line with Galanthus nivalis agglutinin (GNA), Datura stramonium agglutinin (DSA) and Sambucus nigra agglutinin (SNA) indicated the presence of high-mannose type glycans and biantennary complex type oligosaccharides with α2-6 linked sialic acid. N-cadherin from WM239, WM9, and A375 cell lines gave a positive reaction with Phaseolus vulgaris leukoagglutinin (L-PHA) and lotus Tetragonolobus purpureas agglutinin (LTA). This indicated the presence of tri- or tetra-antennary complex type glycans with α-fucose. In addition, N-cadherin from WM9 (lymphomodus metastatic site) and A375 (solid tumor metastatic site) contained complex type chains with α2-3 sialic acid (positive reaction with Maackia amurensis agglutinin - MAA). The results demonstrated that N-glycans of N-cadherin are altered in metastatic melanomas in a way characteristic for invasive tumor cells.

Introduction and aim. This case report focuses on a 26-year-old female with metastatic melanoma. It highlights the diagnostic process, initial immunotherapy, disease progression, and successful response to second-line therapy. Emphasizing the importance of early detection, personalized treatment, and adaptive strategies, it provides valuable insights into managing this aggressive form of skin cancer. Description of the case. A 26-year-old Caucasian female presented with a suspicious pigmented lesion on her thigh in 2013. The lesion was confirmed as superficial skin melanoma. No lymph node biopsy was performed. In 2021, she had abdominal pain and imaging revealed melanoma metastasis in the peritoneum, lungs and brain. Genetic testing showed BRAF V600E mutation and PD-L1 expression in tumor cells. She received immunotherapy and radiation for a central nervous system metastases but developed a brain hematoma. Follow-up imaging showed disease progression. She started second-line therapy with iBRAF/iMEK, and her condition rapidly improved with regression of metastatic lesions. Follow-up imaging confirmed significant positive changes and almost complete regression of neoplastic lesions. She continues to receive the targeted therapy and shows a positive response. Conclusion. Early diagnosis improves outcomes in metastatic melanoma. Peritoneal metastases should be considered in patients with abdominal symptoms. The combination of gamma knife radiosurgery with immunotherapy or targeted therapy shows promise for managing brain metastases, but careful patient selection and monitoring are vital due to potential risks. Treatment responses in advanced melanoma vary, with this case highlighting a favorable response to BRAF/MEK inhibitor therapy in a patient with a BRAF gene mutation. Further research and clinical trials are needed to refine treatment approaches and improve outcomes in metastatic melanoma.

A new database containing 410 cases of nevi pigmentosi, in four categories: benign nevus, blue nevus, suspicious nevus and melanoma malignant, carefully verified by histopathology, is described. The database is entirely different from the base presented previously, and can be readily used for research based on the so-called constructive induction in machine learning. To achieve this, the database features a different set of thirteen descriptive attributes, with a fourteenth additional attribute computed by applying values of the remaining thirteen attributes. In addition, a new program environment for the validation of computer-assisted diagnosis of melanoma, is briefly discussed. Finally, results are presented on determining optimal coefficients for the well-known ABCD formula, useful for melanoma diagnosis.

Ipilimumab, an anticancer drug, is an anti-CTLA4 monoclonal antibody. It is used in treatment of disseminated melanoma. Therapy is associated with high risk of complications. One of the most serious, although one of the rarest is perforation of gastrointestinal tract. In this case report we describe a 52-year old male, with disseminated melanoma with unknown starting point, treated with anti- CTLA4 monoclonal antibody. After 3rd dose of drug administration, bloody diarrhea and acute abdominal pain occurred as a symptom of gastrointestinal perforation. A single perforation was sutured during laparotomy. Symptoms of acute abdomen returned after 10 days. Pus-faecalperitonitis, symptoms of necro-hemorrhagic colitis and multilocal perforation of the colon were found during relaparotomy. Pancolectomy with end ileostomy was performed. Few hours since relaparotomy pacient died due to multiple organ failure. The purpose of this case report is to draw attention to a risk of multilocal colon perforation in patient treated with ipilumumab.

Melanoma of unknown primary applies to 1-8% of all diagnosed melanomas, whereas primary melanoma of the small intestine is a extremely rare case. One of the melanoma characteristics is its capability of forming metastases in the small intestine which very often are diagnosed during autopsy.We present a case report of diagnosed melanoma of unknown primary, whose first symptom was intestinal obstruction. Before admission to the hospital cause of intestinal obstruction, the patient didn't present any signs and symptoms. All typical localizations of primary melanoma were excluded during diagnostic procedure. Palliative right hemicolectomy and segmental small intestine resection were performed. There were no complications in the postoperative course. On the ninth day the patient was discharged from hospital.Quick identification and radical resection of melanoma metastases in the alimentary tract may improve the survival rate in this group of patients. Resection, even if it is palliative by assumption, is not only the best method of elimination of persistent symptoms but it also gives hope for longer survival.

Opis:
Aberrant expression of sialic acids or altered linkage types is closely associated with malignant phenotype and metastatic potential, and can have prognostic significance in human cancer. The present study was undertaken to evaluate whether expression of sialylated derivatives on melanoma cell surface is associated with tumour progression. Four cell lines (WM1552C, WM115, IGR-39 and WM266-4) were used in the study. Cell surface expression of sialic acids was evaluated by flow cytometry with the use of Maackia amurensis and Sambucus nigra lectins. Moreover, adhesion and migration potential of melanoma cells and involvement of sialic acids in these processes were analysed. We have demonstrated that WM266-4 cells have a significantly higher level of α2,3-linked sialic acid residues than other cells, whereas IGR-39 cells had lower expression of α2,6-linked sialic acids. The adhesion efficiencies of WM1552C and WM115 cells were significantly lower than that of IGR-39 and WM266-4 cells. In contrast, WM266-4 cells repaired scratch wounds at least twice as fast as other cells. Melanoma cell adhesion to fibronectin in the presence of Sambucus nigra agglutinin (SNA) was reduced only in IGR-39 and WM266-4 cells, whereas the impact of Maackia amurensis agglutinin (MAA) on this process was much more important. Migration efficiency of melanoma cells was reduced more strongly in the presence of MAA than SNA. In conclusion, our results show that melanoma progression is associated with the increased expression of α2,3-linked sialic acids on the cell surface and these residues could promote melanoma cell interaction with fibronectin.

This article deals in the constantly developing branch of microelectronic devices used in various fields of medicine, i.e. diagnostics and treatment of previously incurable human diseases. A method for assessing and monitoring the vital functions of living cells by measuring cellular impedance in real-time using the ECIS® system and a commercial culture substrate is presented. The goal was to develop a substrate significantly less expensive than a commercial substrate that would be suitable for multiple uses and compatible with the ECIS® measurement station. Moreover, thanks to the use of a material with electrochemical properties other than the biocompatible material (gold or platinum) it is possible to observe the cells behavior with regard to the toxic agent. For this purpose, a culture substrate with nickel comb capacitors was used. To make the electrodes, a thin metal layer was sputtered on polycarbonate plates in the magnetron sputtering process. Prior to the next stages, technological masks were designed so as to fit in the ECIS® measuring station. Subsequently, the microelectronic processes of photolithography and etching the metal layer were performed. Finally, the wells were glued onto the culture medium with a biocompatible adhesive. The completed substrates were transferred to the Department of Human Physiology, Medical University of Lublin, for the culture test on A-375 human melanoma cells. The results of the experiment determined the usefulness of the device for monitoring cell culture vital functions by means of impedance measurement.

Opis:
Up to the ’70 one noticed mainly positive influence of solar radiation, and its component UV radiation. Only in next decade solar radiation was recognized as a main cause of many pathological skin changes, herein malicious tumor. At the same time there was a belief that using sun bathing beds is safe in relation to solar radiation. Nowadays in many countries it is recommended to popularize knowledge about the dangers of solariums, especially among youth. United States and Germany implemented restrictions forbidding under aged using sun beds without parental consent. Great Britain or Sweden are planning similar restrictions. Due to the latest scientific research published by the Lancet Oncology, starting using solarium sun bathing under 30 increases the skin cancer probability up to 70%. It was also proved that every solar radiation is carcinogenic, even though previously only UV-UVB was thought as carcinogenic.
Do lat 70-tych XX wieku dostrzegano głównie pozytywne aspekty oddziaływania promieniowania słonecznego, w tym i jego składowej - promieniowania ultrafi oletowego (UV) na zdrowie, propagowano właściwości przeciwkrzywicze światła słonecznego w kontekście roli jaką odgrywa w produkcji witaminy D3. Dopiero w następnym dziesięcioleciu promieniowanie słoneczne zostało uznane za czynnik sprawczy wielu zmian patologicznych w obrębie skóry, w tym nowotworów złośliwych. Jednocześnie uznawano, iż korzystanie z łóżek opalających jest bezpieczne w relacji do promieniowania słonecznego. Obecnie w wielu krajach zaleca się upowszechnianie wiedzy o potencjalnym niebezpieczeństwie opalania w solariach, zwłaszcza wśród młodzieży. W Stanach Zjednoczonych i Niemczech już wprowadzono restrykcyjne zakazy uniemożliwiające korzystanie z łóżek opalających przez młodzież do 18 roku życia, bez zgody opiekunów. Podobne obostrzenia są planowane w innych krajach np. Wielkiej Brytanii, Szwecji. W świetle najnowszych doniesień naukowych opublikowanych w Lancet Oncology szacuje się iż u osób, które zaczynają korzystać z solarium przed ukończeniem 30-go roku życia, ryzyko zachorowania na raka skóry wzrasta do 70%. Wykazano ponadto, że wszystkie typy promieniowania ultrafioletowego są rakotwórcze, podczas gdy dotąd sądzono, że niebezpieczny jest tylko jeden typ promieniowania UV – UVB.

The melanocytic skin lesion infobase, available at http://synthesis.melanoma.pl (also http://synteza.melanoma.pl, in Polish; referred to as INP) is currently undergoing a complete modification of the way in which (i) the internal synthesis algorithms and (ii) the classification of lesions are performed. We investigated 29 new real images of melanocytic skin lesions, focusing on how humans perform classification based on experience. In conclusion we suggest to add a new color – connected with the depth of a lesion – to the K term of Asymmetry (A),Border (B) and linear Combination of colors and structures (K) method (referred to as ABK).

Expression of p16 protein, intragenic mutations of CDKN2A and hypermethylation of CDKN2A promoter region in 41 sporadic primary uveal melanomas were studied. There were 2 cases of spindle cell B histological type, 11 of A + B and 28 of mixed type. All melanomas infiltrated sclera but in 28 cases infiltration was superficial while in 13 profound. In 7 cases the tumor infiltrated the optic nerve. Expression of p16 was studied by immunohistochemistry and recorded by assessment of the proportion of positive tumor cells and staining intensity. Results were expressed as staining index (IRS). Intragenic mutations were studied by PCR-SSCP followed by sequencing, while hypermethylation of the promoter region by CpG methylation assay. In 15% of cases less than 10% of melanoma cells were p16 positive, in 70% of cases less than 50% of cells, while in 7% more than 80% of cells stained for p16 (mean IRS for all cases was 4.87 ± 2.43). In B type the IRS was 8.5 ± 0.7, in A + B type 6.0 ± 2.1 and in the mixed type 4.17 ± 2.43 (differences statistically significant). In melanomas profoundly infiltrating sclera mean IRS was 4.16, while in those infiltrating optic nerve 3.71 (statistically not significant). Analysis of the intragenic mutations revealed in two patients a GAC/GAT substitution in codon 84 - a silent mutation. No hypermethylation of the CpG island of the p16 promoter region was found. In conclusion, we found that the degree of p16 expression is related to the histological type of tumor but not to the histological indicators of tumor invasiveness and that intragenic mutations and promoter hypermethylation are not major mechanisms of p16 inactivation in sporadic uveal melanoma.

In the last few years, a great attention was paid to the deep learning Techniques used for image analysis because of their ability to use machine learning techniques to transform input data into high level presentation. For the sake of accurate diagnosis, the medical field has a steadily growing interest in such technology especially in the diagnosis of melanoma. These deep learning networks work through making coarse segmentation, conventional filters and pooling layers. However, this segmentation of the skin lesions results in image of lower resolution than the original skin image. In this paper, we present deep learning based approaches to solve the problems in skin lesion analysis using a dermoscopic image containing skin tumor. The proposed models are trained and evaluated on standard benchmark datasets from the International Skin Imaging Collaboration (ISIC) 2018 Challenge. The proposed method achieves an accuracy of 96.67% for the validation set. The experimental tests carried out on a clinical dataset show that the classification performance using deep learning-based features performs better than the state-of-the-art techniques.

Ecto-5'-nucleotidase (eN, CD73) mediates extracellular adenosine production from 5'-AMP. Non-enzymatic functions of eN have also been reported. The aim of the study was to investigate the role of ecto-5'-nucleotidase in aggressive melanoma behaviour. Analysis of the involvement of eN in adhesion, migration and invasion revealed eN functions unknown to date. We found that following eN blockade by concanavalin A, the strength of adhesion to different ECM proteins was not altered, but at the same time the invasion ability of the cells was decreased. On the other hand, knocking down eN in melanoma cells did not influence cell invasion but abolished their migration on tenascin C (TnC). Ecto-5'-nucleotidase seems to fulfil a more distinct role as a receptor than as an enzyme in the cell interaction and mobility on TnC. Ecto-5'-nucleotidase activates also focal adhesion kinase and enhances the formation of complexes upon cell adhesion to TnC. All these observations prove that an eN-TnC complex is involved in cell migration and invasion and thus in the regulation of melanoma progression.

Malignant melanoma is highly aggressive type of malignant neoplasm. The skin of the head and neck region is common localisation for appearance of this tumour due to exposition to the sun light. However, some number of cases concern development of the malignant melanoma on the mucous membranes. Most common signs are epistaxis and nasal obstruction. Late diagnosis effects with poor clinical prognosis. In our case, despite complete surgical resection and adjuvant therapy, outcome of the treatment was unsatisfying, because of metastasis and local recurrence.

Standard ocular tumor treatment includes brachytherapy, as well as proton therapy, particularly for large melanoma tumors. However, the effects of different radiation types on the metastatic spread is not clear. We aimed at comparing ruthenium (106Ru, emitting β electrons) and iodine (125I, γ-radiation) brachytherapy and proton beam therapy of melanoma implanted into the hamster eye on development of spontaneous lung metastases. Tumors of Bomirski Hamster Melanoma (BHM) implanted into the anterior chamber of the hamster eye grew aggressively and completely filled the anterior chamber within 8-10 days. Metastases, mainly in the lung, were found in 100% of untreated animals 30 days after enucleation. Tumors were irradiated at a dose of 3-10 Gy with a 106Ru plaque and at a dose of 6-14 Gy using a 125I plaque. The protons were accelerated using the AIC-144 isochronous cyclotron operating at 60 MeV. BHM tumors located in the anterior chamber of the eye were irradiated with 10 Gy, for the depth of 3.88 mm. All radiation types caused inhibition of tumor growth by about 10 days. An increase in the number of metastases was observed for 3 Gy of β-irradiation, whereas at 10 Gy an inhibition of metastasis was found. γ-radiation reduced the metastatic mass at all applied doses, and proton beam therapy at 10 Gy also inhibited the metastastic spread. These results are discussed in the context of recent clinical and molecular data on radiation effects on metastasis.

Changes in CDKN2a gene are known to be linked with sporadic melanoma and hereditary predisposition to this cancer. In the Polish population mutations in the coding region of the CDKN2a gene are rather rare, therefore the attention has been focused on polymorphisms and alterations in uncoding regions such as 3' UTR. The aim of this study was to analyze two common polymorphisms, Ala148Thr and 500 C/G, and correlate them with the clinical course of melanoma. DNA from 285 patients was analyzed and found polymorphisms were correlated with the clinical parameters employing statistical methods. The obtained results allow us to conclude: (i) survival times of 500 C/G carriers vs. cumulating proportion surviving was not statistically significant; (ii) CDKN2a polymorphism 500 C/G correlated with Ala148Thr; (iii) no correlation was observed between the 500 C/G polymorphism and age of diagnosis, localization of primary melanoma and survival time; (iv) we did not find correlation between 500 C/G and type of cancer in the family; (v) changes in the CDKN2a gene were not found in patients with second cancer.

Malignant skin neoplasms are one of the most common human malignancies. The incidence of nonmelanoma skin cancers and malignant melanoma is constantly increasing. The current therapies, especially for malignant melanoma, have relatively low success rates. Therefore, there is an urgent need to develop new remedies that are both safe and effective. Natural substances have always been an important source for the discovery of new therapies. In turn, a number of studies have indicated that some phytocheicals could have an anti-tumour effect. In vitro and in vivo testing of malignant skin neoplasm models revealed different anti-tumour actions, including antioxidation, carcinogen inactivation, anti-proliferation, cell cycle arrest, induction of apoptosis, inhibition of angiogenesis, or a combination of them. The aim of this paper is to describe anti-tumour compounds derived from natural sources that might be used in the therapy of malignant skin neoplasm. The phytochemicals discussed below include carotenoids, terpenoids and flavonoids.

Juglone is an allelopathin secreted by black walnut tree of the Juglandaceae family and is used as an active ingredient in many herbal preparations and as a commercial dye. It is considered as an important phytochemical with wide therapeutic potential. Black walnut extract has long been used in folk medicine to treat various types of cancers. It demonstrates antiviral, antibacterial, antifungal, and antitumor activities. The present study aimed to analyze the effect of juglone on the viability and proliferation of melanoma cells of C-32 (amelanotic melanoma) and COLO 829 (melanotic melanoma) cell lines in vitro and on the mRNA expression of genes encoding the proapoptotic BAX protein and caspase 3 and the gene encoding antiapoptotic BCL2 protein. The results showed a dosedependent effect of juglone on the viability, proliferation, and death induction in C-32 and COLO 829 melanoma cells and in HFF-1 normal dermal fibroblasts in in vitro cultures, but melanoma cells were more sensitive to juglone. Our findings revealed different mRNA expression patterns for all the studied genes in melanoma and normal cells treated with juglone in in vitro cultures.

We reported a 57 year old woman treated for 2 years because of pustulosis plantaris (periodic exacerbations and remissions). In July 2013 she reported to the Outpatient Surgery because of a tumor in the left groin area. The biopsy revealed: metastases melanomatosis in lymphonodus. The patients was in October at the surgical removal of lymph nodes from left groin area (lymphadenectomy groin - ilio - obturatorii right). Full diagnostic was carried out and searching of of primary foci to no results. In the patient revealed the presence of BRAF and she was qualified to study of drug vemurafenib.

Interleukin 10 (IL-10) is a potent immunosuppressive cytokine with an antitumor activity. The effect of IL-10 on tumor growth was tested in murine melanoma cells manipulated by gene transfer to secrete IL-10. In mice bearing B16(F10) tumors expressing IL-10  tumor growth was decreased depending on the amount of secreted IL-10.

Malignant transformation is frequently associated with alternations in cell glycosylation, in particular those concerning branched complex-type N-glycans. These changes occur principally in β1,4-N-acetylglucosaminyltransferase III (GnT-III) that catalyzes the synthesis of glycans with bisected N-acetylglucosamine (GlcNAc) and β1,6-N-acetylglucosaminyltransferase V (GnT-V) that is involved in forming β1,6-branched antenna in complex-type glycans. We searched for the reasons of a different behavior of CM and UM cells in the expression of GnT-III and GnT-V and their oligosaccharide products. Our study showed that UM cells have more β1,6-branched glycans than CM cells, what results from a higher expression of MGAT5 gene encoding GnT-V. The higher β1,6-branching of glycans in UM may contribute to their higher potential to migrate on fibronectin and weaker binding to main extracellular matrix proteins, observed in our previous studies.

Human mitochondrial polynucleotide phosphorylase (hPNPase) is an exoribonuclease localized in mitochondria. The exact physiological function of this enzyme is unknown. Recent studies have revealed the existence of a relationship between induction of hPNPase mRNA and both cellular senescence and growth arrest of melanoma cells following β-interferon treatment. The aim of this study was to verify whether the augmented hPNPase mRNA level results in increase of the protein level. In several cell lines established from five metastatic melanoma patients we did not find any such correlation. However, an elevated level of hPNPase protein was observed in interferon-induced HeLa and Jurkat cells. This increase was correlated with a slight shortening of poly(A) tails of mitochondrial ND3 transcript.

Introduction: Cutaneous melanoma is estimated for 2% of malignant neoplasms occurring in humans. It is characterized by a high level of malignancy and low sensitivity to cytostatic drugs. The incidence of cutaneous melanoma is increasing in Poland. The lymphatic system is the most common route of dissemination of this neoplasm. The appearance of a sentinel node biopsy technique has made it possible to identify patients with a regionally advanced disease. It is a minimally invasive method with a small percentage of complications. Aim: Analysis of disease free survival (DFS) in cutaneous melanoma patients with sentinel lymph node biopsy. Material and methods: The analysis included 222 patients with cutaneous melanoma treated in the Department of Oncological Surgery in 2010–2015, who underwent a sentinel node biopsy. The study group consisted of 136 women and 86 men, the average age of patients was 59 years. Patients were qualified for sentinel node biopsy based on clinical evaluation and ultrasound of regional lymph nodes. The average follow-up was 25.1 months. About 2 hours before surgery, patients received a radioisotope, then lymphoscintigraphy SPECT was performed. Additionally, they were administered the Patent Blue dye in the operating room. Results: The sentinel node was identified in 217 patients (98%), and the average sentinel nodes were 2.25. Twenty-seven patients (12%) had a metastasis in sentinel nodes. In this group, the duration of symptom free survival was significantly shorter. Sentinel node status and age of the patient were independent factors affecting the prognosis of disease free survival. Conclusions: Sentinel node biopsy is a precise method to identify patients with cutaneous melanoma who have metastasis to regional lymph nodes, as well as the most important prognostic factor.

The paper presents special forms of an ensemble of classifiers for analysis of medical images based on application of deep learning. The study analyzes different structures of convolutional neural networks applied in the recognition of two types of medical images: dermoscopic images for melanoma and mammograms for breast cancer. Two approaches to ensemble creation are proposed. In the first approach, the images are processed by a convolutional neural network and the flattened vector of image descriptors is subjected to feature selection by applying different selection methods. As a result, different sets of a limited number of diagnostic features are generated. In the next stage, these sets of features represent input attributes for the classical classifiers: support vector machine, a random forest of decision trees, and softmax. By combining different selection methods with these classifiers an ensemble classification system is created and integrated by majority voting. In the second approach, different structures of convolutional neural networks are directly applied as the members of the ensemble. The efficiency of the proposed classification systems is investigated and compared to medical data representing dermoscopic images of melanoma and breast cancer mammogram images. Thanks to fusion of the results of many classifiers forming an ensemble, accuracy and all other quality measures have been significantly increased for both types of medical images.

Murine melanoma B16F0 cells were transfected with SA:DPPC:DOPE (2:1:1 molar ratio) liposomes associated with a plasmid encoding murine IL-12. Stearylamine, a cationic lipid, showed a greater transfection efficiency compared to DOTAP-containing liposomes. The lipid:DNA ratio was 2:1 (w/w). Control groups were mock transfected or transfected with an empty plasmid (pNeo). pNeo or IL-12 transfected cells and controls were inoculated intradermically into the dorsal region of the foot or the lateral flank of C57BL6 mice. Results showed that IL-12 expression had a marked effect on in vivo growth of B16 melanoma tumors developed in both anatomic sites, significantly retarding their growth and prolonging host survival.

Two water soluble porphyrins: meso-tetra-4-N-methylpyridyl-porphyrin iodide (P1) and 5,10-di-(4-acetamidophenyl)-15,20-di-(4-N-methylpyridyl) porphyrin (P2) were synthesised and evaluated in respect to their photochemical and photophysical properties as well as biological activity. Cytotoxic and phototoxic effects were evaluated in human malignant melanoma Me45 line using clonogenic assay, cytological study of micronuclei, apoptosis and necrosis frequency and inhibition of growth of megacolonies. Both porphyrins were characterised by high UV and low visible light absorptions. Dark toxicity measured on the basis of the clonogenic assay and inhibition of megacolony growth area indicated that P1 was non-toxic at concentrations up to 50 μg/ml (42.14 μM) and P2 at concentrations up to 20 μg/ml (16.86 μM). The photodynamic effect induced by red light above 630 nm indicated that both porphyrins were able to inhibit growth of melanoma megacolonies at non-toxic concentrations. Cytologic examination showed that the predominant mode of cell death was necrosis.

Epigenetics represents the mechanisms that influence the regulation and modification of the expression of genetic material not related to the alterations in DNA sequences. These mechanisms include both DNA methylation and histone modifications. In the present article, we review current views on the role of aberrations of DNA hyper- and hypomethylation processes and the acetylation of histones, associated with genes that control the cell cycle, cell differentiation, DNA repair, apoptosis, cell signaling, angiogenesis, metabolism of xenobiotics and invasion, in the pathogenesis of melanoma. In addition, new strategies for treatment of melanoma associated with epigenetics are presented.
Przez pojęcie epigenetyka należy rozumieć mechanizmy wpływające na regulację i modyfi kację ekspresji materiału genetycznego, jednocześnie niezmieniające sekwencji nukleotydów. Mechanizmy te obejmują zarówno metylację DNA, jak i modyfikacje histonów. W artykule dokonano przeglądu aktualnych poglądów dotyczących zaburzeń procesów hiperihipometylacji DNA oraz acetylacji histonów w patogenezie czerniaka, związanych z genami kontrolującymi cykl komórkowy, różnicowanie, naprawę DNA, apoptozę, sygnalizację komórkową, angiogenezę, metabolizm ksenobiotyków i powstawanie przerzutów. Ponadto przedstawiono nowe strategie leczenia czerniaka związane z epigenetyką.

It has been known that VEGF121 isoform can serve as a carrier of therapeutic agents targeting tumor endothelial cells. We designed and constructed synthetic cDNA that encodes a chimeric protein comprising abrin-a (ABRaA) toxin A-chain and human VEGF121. Expression of the ABRaA-VEGF121 chimeric protein was carried out in E. coli strain BL21(DE3). ABRaA-VEGF121 preparations were isolated from inclusion bodies, solubilized and purified by affinity and ion-exchanged chromatography (Ni-agarose and Q-Sepharose). Finaly, bacterial endotoxin was removed from the recombinant protein. Under non-reducing conditions, the recombinant protein migrates in polyacrylamide gel as two bands (about 84 kDa homodimer and about 42 kDa monomer). ABRaA-VEGF121 is strongly cytotoxic towards PAE cells expressing VEGFR-2, as opposed to VEGFR-1 expressing or parental PAE cells. The latter are about 400 times less sensitive to the action of this fusion protein. The biological activity of the ABRaA domain forming part of the chimeric protein was assessed in vitro: ABRaA-VEGF121 inhibited protein biosynthesis in a cell-free translation system. Preincubation of ABRaA-VEGF121 with antibody neutralizing the biological activity of human VEGF abolished the cytotoxic effect of the chimeric protein in PAE/KDR cells. Experiments in vivo demonstrated that ABRaA-VEGF121 inhibits growth of B16-F10 murine melanoma tumors.

Mucosal melanoma constitutes 1,3–1,4% of all melanomas. 25–50% of these cases occurs in head and neck region. Primary melanoma of the head and neck mucous membranes is most often diagnosed in the nasal cavity (50%), maxillofacial region (20%), oral cavity (17%), and then nasopharynx, casuistic in the larynx. Recurrent nasal bleeding and persistent nasal obstruction are the most frequently presented symptoms. In comparison to skin melanoma, mucosal melanoma is diagnosed 1 decade later and has different etiology. It is usually diagnosed at an advanced stage when metastases are already present. Approximately 20% of patients have metastases to the lymph nodes at diagnosis and 10% distant metastases. Histopathological diagnosis is based on the detection of malanocyte markers: S-100, Melan-A, HMB-4, MITF-1 and vimentin. Mucosal melanoma can be misdiagnosed as olfactory neuroblastoma. In ⅓ of the cases, melanoma of mucous membranes occurs in the amelanotic form. Treatment of choice is radical resection and adjuvant radiotherapy. In the head and neck region radical disscection may be a challenge for operating surgeon because of closeness of anatomical structures and preservation of function. In advanced stadium immunomodulatory treatment can be applied. Overall 5-year survival is estimated for 12–44%. Based on presented case authors note the necessity of inclusion additional factors in geriatric oncology and generally discuss the diagnosis and treatment of mucosal melanoma.
Czerniak błony śluzowej stanowi 1,3–1,4% wszystkich czerniaków. 25–50% z nich występuje w regionie głowy i szyi. Pierwotny czerniak błony śluzowej głowy i szyi najczęściej rozpoznawany jest w: jamie nosowej (50%), masywie szczękowo-twarzowym (20%), jamie ustnej (17%), następnie w nosogardle oraz kazuistycznie w krtani. Zazwyczaj prezentuje się następujące jego objawy: nawracające krwawienie z nosa i utrzymujące się upośledzenie drożności. W porównaniu do czerniaka skóry, czerniak błony śluzowej występuje o dekadę później; ma też inną etiologię. Zwykle rozpoznaje się go w zaawansowanym stadium, gdy obecne są przerzuty. U około 20% pacjentów w momencie rozpoznania stwierdza się przerzuty do węzłów chłonnych, a u 10% – odległe przerzuty. Rozpoznanie histopatologiczne opiera się na wykryciu markerów malanocytów: S-100, Melan- A, HMB- 45, MITF-1 i wimentyny. W ⅓ przypadków czerniak błony śluzowej występuje w formie amelanotycznej. Może być on mylony z nerwiakiem węchowym zarodkowym. Leczeniem z wyboru jest radykalna resekcja z następową radioterapią. Z uwagi na bliskość anatomiczną ważnych struktur, usunięcie guza zlokalizowanego w obrębie głowy i szyi w całości z zachowaniem funkcji czynnościowej stanowi wyzwanie dla operatora. W zaawansowanym stadium stosuje się leki immunomodulujące. Ogólne 5-letnie przeżycie kształtuje się na poziomie 12–44%. Na podstawie studium przypadku, autorki zwracają uwagę na konieczność uwzględnienia dodatkowych czynników w onkologii geriatrycznej oraz ogólnie omawiają diagnostykę i leczenie czerniaka błony śluzowej.

Recombinant retroviral vectors are still the most common gene delivery vehicles for gene therapy purposes, especially for construction of genetically modified tumor vaccines (GMTV). However, these vehicles are characterized by relatively low titre and in the case of many tumor cell lines, low transduction efficiency. We constructed bicistronic retroviral vector pseudotypes of amphotropic murine leukemia virus (A-MuLV) and gibbon ape leukemia virus (GaLV), encoding enhanced green fluorescent protein (EGFP) as a rapid and easily detectable reporter gene. Transduction of five different human melanoma and four renal carcinoma cell lines by these two virus pseudotypes revealed differences in transduction efficiency, which wase markedly lower for the renal carcinoma cell lines. Stimulation of retroviral receptor expression (PiT1 and PiT2) by phosphate depletion induced a limited increase of receptor mRNA levels, but did not improve the gene transfer efficiency. In contrast, simultaneous transduction with both vector pseudotypes markedly increased the transduction efficiency, compared to GaLV or A-MuLV alone. The same effect could be achieved by several repeated exposures of target cells to fresh vector preparation. Overexpression of GaLV receptor (PiT1) in target cells significantly increased the transduction rate and enabled retrovirus mediated gene transfer into the cells which normally are not transducible by GaLV pseudotypes. We demonstrated that, using different transduction strategies, the relatively inefficient, widely used retroviral vector systems could be significantly improved.

Introduction. Neoplastic diseases are a common cause of death, especially in developed countries. Various methods are used in cancer therapy including natural substances. Terpenes and terpenoids play a special role, including betulin and betulinic acid with the pentacyclic structure of lupane. They have a wide range of pharmacological activity, among which anti-cancer activity attracts the greatest attention. Abbreviated description of the state of knowledge. The review discusses the pharmacological effects of betulin and its derivatives on tumours, and focuses on the potential of its application in the treatment of melanoma. The review study also presents future prospects for the use of this natural substance. Betulin and its derivatives have the ability to influence the processes of apoptosis, angiogenesis and autophagy. Additionally, they have the ability to sensitize, increasing the sensitivity to commonly used methods of cancer treatment. To-date, numerous studies have been carried out on the synthesis of betulinic acid derivatives, which would show greater polarity and activity compared to the starting compound. Due to their greater bioavailability, the modified compounds showed a stronger anti-tumour effect. Conclusion. The results of the above-mentioned studies confirm the anti-cancer properties of betulin. Regarding melanoma, betulin shows significant therapeutic efficacy. This gives new perspectives for the treatment of the most dangerous skin cancer.

Melanoma skin cancer is one of the most dangerous and life-threatening cancer. Exposure to ultraviolet rays may damage the skin cell's DNA, which causes melanoma skin cancer. However, it is difficult to detect and classify melanoma and nevus mole at the immature stages. In this work, an automatic deep learning system is developed based on the intensity value estimation with a convolutional neural network model (CNN) to detect and classify melanoma and nevus mole more accurately. Since intensity levels are the most distinctive features for object or region of interest identification, the high-intensity pixel values are selected from the extracted lesion images. Incorporating those high-intensity features into the CNN improves the overall performance than the state-of-the-art methods for detecting melanoma skin cancer. To evaluate the system, we used 5-fold cross-validation. Experimental results show that a superior percentage of accuracy (92.58%), Sensitivity (93.76%), Specificity (91.56%), and Precision (90.68%) are achieved.

The article provides an overview of selected applications of deep neural networks in the diagnosis of skin lesions from human dermatoscopic images, including many dermatological diseases, including very dangerous malignant melanoma. The lesion segmentation process, features selectionand classification was described.Application examples of binary and multiclass classification are given.The described algorithms have been widely used in the diagnosis of skin lesions. The effectiveness, specificity, and accuracy of classifiers were compared and analyzed based on available datasets.
Artykuł zawiera przeglądwybranychzastosowań głębokich sieci neuronowych w diagnostyce zmian skórnych zobrazów dermatoskopowych człowieka z uwzględnieniem wielu choróbdermatologicznych, w tym bardzo niebezpiecznejz nich malignant melanoma. Został opisany processegmentacjizmiany, selekcji cech i klasyfikacji. Uwzględniono przykłady binarnej i wieloklasowej klasyfikacji. Opisane algorytmy znalazły szerokie zastosowanie w diagnostyce zmian skórnych.Porównano i przeanalizowanoskuteczność, specyficznośći dokładność klasyfikatorów w oparciu o dostępne zestawy danych.

Melanoma is one of the fastest spreading cancers.The aim of the article is to segment the skin lesionsfrom human skin dermatoscopic images covered by melanoma. Threshold segmentation was used, which allows a single skin lesionto be analyzed. Itshows the four areas of each based on their color. The created software monitors the border of skin lesion areas.Segmentation and analysis of the resulting images with different areas of skin change was carried out in the Matlab software.
Czerniak to jeden z najszybciej rozprzestrzeniających się nowotworów. Celem artykułu jest segmentacja zmiany skórnej z obrazów dermatoskopowych ludzkiej skóry objętych czerniakiem. Użyto segmentacj przez progowanie, która pozwala na analizę pojedyńczejzmiany skórnej. Ukazuje cztery obszary każdej z nich w oparciu o ich barwę. Stworzone oprogramowanie monitoruje granicę obszarów zmiany skórnej. Segmentacjai analiza powstałych obrazów z różnymi obszarami zmiany skórnej została przeprowadzona w środowisku Matlab.

Malignant melanomas are the most deadly type of skin cancer, yet detected early have high chances of successful treatment. In the last twenty years, the interest in automatic recognition and classification of melanoma dynamically increased, partly because of appearing public datasets with dermatoscopic images of skin lesions. Automated computer-aided skin cancer detection in dermatoscopic images is a very challenging task due to uneven sizes of datasets, huge intra-class variation with small interclass variation, and the existence of many artifacts in the images. One of the most recognized methods of melanoma diagnosis is the ABCD method. In the paper, we propose an extended version of this method and an intelligent decision support system based on neural networks that uses its results in the form of hand-crafted features. Automatic determination of the skin features with the ABCD method is difficult due to the large diversity of images of various quality, the existence of hair, different markers and other obstacles. Therefore, it was necessary to apply advanced methods of pre-processing the images. The proposed system is an ensemble of ten neural networks working in parallel, and one network using their results to generate a final decision. This system structure enables to increase the efficiency of its operation by several percentage points compared with asingle neural network. The proposed system is trained on over 5000 and tested afterwards on 200 skin moles. The presented system can be used as a decision support system for primary care physicians, as a system capable of self-examination of the skin with a dermatoscope and also as an important tool to improve biopsy decision making.

INTRODUCTION In the early stages of cancerogenesis, TGF-β strongly inhibits the proliferation of epithelial cells and is described as a tumor suppressor. However, in further stages of this process, TGF-β can even promote the formation of a neoplasm. It can influence the tumor invasiveness and the ability of cancer to infiltrate the area of surrounding normal tissue. Stimulating proangiogenic factors such as IL-8 and VEGF can also contribute to angiogenesis in the tumor periphery. An elevated level of TGF-β strongly modifies the immune response of the organism through inhibiting the proliferation of T-cells and limiting the possibility of antigen presentation by specialized cells. Aim of the study. The aim of the study was to evaluate the expression of genes encoding TGF-β1 and its receptors: type I, II and III in melanoma cell culture after photodynamic therapy with Photolon in vitro. MATERIAL AND METHODS The study was conducted in three melanoma cell lines: C-32, Colo-892 and SH-4 (ATCC). The expression of genes encoding TGF-β1 and its receptors was estimated at the transcript stage (mRNA). RESULTS All of the photodynamic therapy parameters applied in the study resulted in a significant reduction of TGF-β1 and its receptors mRNA copy numbers per μg of the total RNA. CONCLUSIONS According to the data connected with the role of TGF-β in expressing a more malignant character in cancer cells, decreasing the TGF-β1 mRNA copy numbers after photodynamic therapy may be advantageous for the therapeutic process.
WSTĘP W początkowym stadium kancerogenezy transformujący czynnik wzrostu β (transforming growth factor β – TGF-β) silnie hamuje proliferację komórek nabłonkowych i określany jest mianem supresora nowotworów. Jednak w późniejszych etapach tego procesu, czynnik ten może promować rozwój nowotworu: wpływać na jego inwazyjność oraz zdolność do naciekania prawidłowych tkanek. Przez stymulację czynników proangiogennych, takich jak IL-8 i VEGF, może także przyczyniać się do angiogenezy na obwodzie guza. Podwyższony poziom TGF-β silnie modyfikuje odpowiedź immunologiczną organizmu – hamuje proliferację limfocytów T oraz ogranicza możliwości prezentacji antygenów przez wyspecjalizowane komórki. Celem pracy była ocena ekspresji genów kodujących: TGF-β1 oraz jego receptory I, II i III w hodowlach komórek czerniaka, które poddano, w warunkach in vitro, terapii fotodynamicznej z Fotolonem. MATERIAŁ I METODY W badaniach wykorzystano trzy linie czerniaka: C-32, Colo-892 i SH-4 (ATCC). Ekspresję genów kodujących TGF-β1 i jego receptory oceniano na etapie transkryptów (mRNA). WYNIKI Zastosowanie przyjętych w pracy warunków terapii fotodynamicznej w przypadku wszystkich badanych linii czerniaka skutkuje znaczącą redukcją liczby kopii mRNA TGF-β1 i jego receptorów w przeliczeniu na μg RNA całkowitego. WNIOSKI W świetle informacji dotyczącej roli TGF-β w nabywaniu złośliwego charakteru komórek nowotworowych zmniejszenie liczby kopii mRNA TGF-β1 po przeprowadzeniu terapii fotodynamicznej może być korzystne dla przebiegu procesu terapeutycznego.

Segmentation of the border of the human pigmented lesions has a direct impact on the diagnosis of malignant melanoma. In this work, we examine performance of (i) morphological segmentation of a pigmented lesion by region growing with the adaptive threshold and density-based DBSCAN clustering algorithm, and (ii) morphological segmentation of the pigmented lesion border by region growing of the lesion and the background skin. Research tasks (i) and (ii) are evaluated by a human expert and tested on two data sets, A and B, of different origins, resolution, and image quality. The preprocessing step consists of removing the black frame around the lesion and reducing noise and artifacts. The halo is removed by cutting out the dark circular region and filling it with an average skin color. Noise is reduced by a family of Gaussian filters 3×3−7×7 to improve the contrast and smooth out possible distortions. Some other filters are also tested. Artifacts like dark thick hair or ruler/ink markers are removed from the images by using the DullRazor closing images for all RGB colors for a hair brightness threshold below a value of 25 or, alternatively, by the BTH transform. For the segmentation, JFIF luminance representation is used. In the analysis (i), out of each dermoscopy image, a lesion segmentation mask is produced. For the region growing we get a sensitivity of 0.92/0.85, a precision of 0.98/0.91, and a border error of 0.08/0.15 for data sets A/B, respectively. For the density-based DBSCAN algorithm, we get a sensitivity of 0.91/0.89, a precision of 0.95/0.93, and a border error of 0.09/0.12 for data sets A/B, respectively. In the analysis (ii), out of each dermoscopy image, a series of lesion, background, and border segmentation images are derived. We get a sensitivity of about 0.89, a specificity of 0.94 and an accuracy of 0.91 for data set A, and a sensitivity of about 0.85, specificity of 0.91 and an accuracy of 0.89 for data set B. Our analyses show that the improved methods of region growing and density-based clustering performed after proper preprocessing may be good tools for the computer-aided melanoma diagnosis.

Czerniak złośliwy jest jednym z najszybciej rozwijających się nowotworów skóry, a zachorowalność na niego stale wzrasta. Podstawowym badaniem nieinwazyjnym pozwalającym na jego rozpoznanie jest dermatoskopia. Celem badania dermatoskopowego jest diagnostyka różnicowa zmian barwnikowych z podziałem na zmiany melanocytowe i niemelanocytowe. Badanie pozwala odróżnić zmiany melanocytowe, wymagające wycięcia chirurgicznego oraz badania histopatologicznego, od zmian łagodnych. Szybki rozwój elektroniki i informatyki pozwolił na wyodrębnienie dwóch nowych dziedzin w dermatoskopii (foto- i wideodermatoskopii), które umożliwiają cyfrowy zapis zdjęć. Komputerowa analiza obrazów dermatoskopowych polega na ocenie poszczególnych zmian, określaniu ich zaawansowania i wyznaczaniu podstawowych parametrów diagnostycznych (określanie barwy, ilości barw, rozmiaru, symetrii oraz struktur różnicujących). Badania komputerowe wykazują dużą skuteczność, jednak nie są zalecane jako jedyny sposób oceny zmian. Obecne systemy nie ograniczają niepotrzebnych zabiegów chirurgicznych, co uznawane jest za ich podstawową wadę. Wymagany jest dalszy rozwój aplikacji oraz opracowanie nowych, nowatorskich rozwiązań, aby dermatoskopia wspomagana komputerowo stała się wiodącą metodą diagnostyczną.
Skin melanoma is one of the most malignant tumours and increasing melanoma incidence rate has been observed worldwide in the last several years. Due to high skin cancer incidence, dermatologic oncology has become a quickly developing branch of medicine. Dermoscopy is the most common and non-invasive method to diagnose skin cancer. The aim of dermoscopy is to dif erentiate malignant melanoma from other lesions of the skin (hemangiomas and nevi) as well as preliminary staging and malignancy assessment. It is possible to distinguish malignant tumours, requiring surgical removal followed by histopathological examination, from benign changes. The rapid development of electronics and information technologies enabled to create two new areas of dermoscopy (photo- and videodermoscopy) that use digital imaging for storing the data. The aim of computer systems in dermoscopy is to analyse each image and to evaluate each change, identifying them and determining the advancement of basic diagnostic parameters (determination of colour, border, size, asymmetry and dif erential structures). Recent studies of software systems show high ei ciency, but it is still not recommended that the software systems are the only one to evaluate the changes. Rapid development of medical equipment and computer systems for medical applications gives hope for better and faster diagnoses of malignant melanoma and that dermoscopy image processing will become a leading diagnostic method.

Skin cancers are one of the most common cancers in the Caucasian population. A constantly increasing number of nonmelanoma skin cancers and malignant melanomas is observed. The incidence of skin cancers is associated mainly with exposure to sunlight. Therefore, agricultural workers who work in open spaces are a particularly vulnerable group. Currently, studies on the pathogenesis of skin cancer focus on the molecular basis associated with ultraviolet radiation. This study is an attempt to summarize the current state of knowledge on this issue. There have been demonstrated mutations in different classes of genes associated with carcinogenesis, including protooncogenes, tumour suppressor genes, genes that control apoptosis, genes encoding transcription factors and DNA repair genes in patients with skin cancers. Mutations in the latter result in reducing the effectiveness of DNA repair and fixation of mutations. All changes at the gene level lead to structural changes, quantitative and dysfunction of proteins encoded by these genes. All these factors contribute to the process of carcinogenesis. Due to increasing number of skin cancers, it seems important to increase knowledge of the molecular basis of skin cancers. This knowledge could be crucial for predicting the course of the disease, and for the development of new therapeutic strategies.

The hydrazine derivatives of benzopyrones remain an unexplored group of chemical compounds. This preliminary study investigates the influence of A-5, CH-3 and K-2 derivatives at concentrations of 1, 10, 100 nM and 1 μM on selected biochemical factors of a melanoma cell line WM-115, with regard to their potential angiogenic properties. The studied compounds were found to influence cell proliferation, as well as total protein, bFGF and FGFR1 concentration.

Nowotwory skóry są najczęściej spotykanymi nowotworami na świecie. Czerniaki złośliwe stanowią od około 5 do 7% wszystkich nowotworów złośliwych skóry u człowieka. Ich wczesne zdiagnozowanie jest kluczowym czynnikiem w późniejszej pomyślnej terapii. Niniejsza praca zawiera propozycję rozwinięcia i zautomatyzowania najważniejszej metody diagnozowania czerniaków, metody ABCD Stoltza. W artykule przedstawiono koncepcję i implementację zautomatyzowanego systemu do diagnostyki znamion skórnych pod kątem wykrycia czerniaka zł ościowego. Zaproponowano nową, rozszerzoną wersję metody dermatoskopowej ABCD i zaimplementowano niezbędne algorytmy w środowisku Matlab. Główne cechy znamion skórnych o charakterze nowotworowym są wyszukiwane automatycznie przy pomocy metod przetwarzania obrazu oraz opracowanych algorytmów. Decyzja na temat rozpoznania lub nie czerniaka złośliwego podejmowana jest przez sztuczną sieć neuronową, wnioskującą na podstawie wskaźników wyznaczonych na etapie przetwarzania obrazów. Omawiany system wspomagania decyzji może służyć jako narzędzie usprawniające pracę lekarzy pierwszego kontaktu lub jako system umożliwiający szybkie samobadanie skóry przez pacjentów. Aplikację przetestowano na 126 znamionach skórnych. Uzyskano czułość równą 98% oraz swoistość równą 73%, co jest bardzo dobrym osiągnięciem.
Skin cancer is the most common cancer in the world. Malignant melanomas make up about 5-7% of all types of human skin cancer. The work describes the development process of an automated system purposed for the diagnosis of skin lesions in order to detect a malignant melanoma. The application should be used as a decision support system for primary care physicians or as a system capable of self-examination of the skin. When designing an application author developed and proposed a new, enhanced version of the ABCD dermatoscopic method of Stoltz. To describe main features of skin lesions for malignancy, image processing methods were used. In addition, application was trained by artificial neural network, which acts as a specialist doctor, who is responsible of making a diagnosis based on these features. The application has been tested on 126 the skin moles. It gets high final score with a sensitivity of 98% and specificity equal to 73.08%.

Promieniowanie ultrafioletowe (UV) jest fizycznym czynnikiem mutagennym i karcynogennym. Do powierzchni Ziemi dociera ok. 95% promieniowania ultrafioletowego A (UVA) (320–400 nm) i ok. 5% UVB (280–320 nm). Naturalnym czynnikiem ochronnym skóry przed UV jest melanina. Do raków skóry, których rozwój jest związany z długotrwałym narażeniem na promieniowanie UV, zalicza się niebarwnikowe raki skóry (non-melanoma skin cancers – NMSC) – raka podstawnokomórkowego (basal cell carcinoma – BCC) i raka kolczystokomórkowego (squamous cell carcinoma – SCC) oraz czerniaka złośliwego skóry (cutaneous malignant melanoma – CMM). Wysokie ryzyko rozwoju BCC związane jest z ostrą i wielokrotną ekspozycją na UV, powodującą oparzenia słoneczne. Badania molekularne BCC wykazały zaburzenia na szlaku regulacji sygnalizacji komórkowej sonic hedgehog (SHH), które były związane z mutacjami genu supresorowego PTCH1 (protein patched homolog 1). Ryzyko rozwoju BCC jest również związane z polimorfizmem genu receptora melanokortyny-1 (MC1R). Obserwowane w komórkach BCC mutacje genu P53 klasyfikowano jako wtórne zmiany genetyczne. W komórkach SCC mutacje indukowane UV najczęściej dotyczyły genu P53. W rozwoju SCC istotne znaczenie ma zwiększona ekspresja genu enzymu cyklooksygenazy-2 (COX-2). Innym czynnikiem patogenetycznym SCC jest nasilenie syntezy cytokin prozapalnych (czynnika martwicy nowotworu α (tumor necrosis factor α – TNF-α), interleukiny-1 α (IL-1α), IL-1β i IL-6). Obecnie w patogenezie CMM powszechnie uznawana jest rola UVB. W komórkach CMM notowano mutacje w genach: p16 INK4A (cyclin-dependent kinase inhibitor 2A INK4A), genie kodującym cyklinozależną kinazę 4 (cyclin-dependent kinase 4 – CDK4), Ras, PTEN (phosphatase and tensin homolog deleted on chromosome 10) i BRAF (proto-oncogene B-Raf). Mutacje genu BRAF stwierdzano w ok. 50% przypadków CMM. Mutacje genu P53 nie są charakterystyczne dla komórek CMM. Med. Pr. 2016;67(2):255–266

Skin cancers associated with long-term exposure to UV radiation are: basal cell carcinoma (BCC), squamous cell carcinoma (SCC) and cutaneous malignant melanoma (CMM). The high risk of BCC development is related to acute and repeated exposure to UV causing sunburn. Molecular studies of BBC demonstrated disorders in sonic hedgehog (SHH) cell signaling regulation pathway, associated with the suppressor protein patched homolog 1 gene (PTCH1) mutations. The risk of the BCC development is related to the polymorphism of melanokortin-1 receptor gene (MC1R). Tumor P53 gene mutations observed in BCC cells has been classified as secondary genetic changes. In SCC cells UV-induced mutations were mostly related to P53 gene. Increased expression of cyclooxigenase- 2 gene (COX-2) plays a significant role in the development of SCC. Other pathogenetic factors include intensification of the synthesis of pro-inflammatory cytokines (tumor necrosis factor α (TNF-α), interleukin-1 α (IL-1α), IL-1β and IL-6). Currently, the role of UVB has been recognized in the pathogenesis of CMM. In CMM cells the following gene mutations were noted: cyclindependent kinase inhibitor 2A INK4A (p16 INK4A), cyclin-dependent kinase 4 (CDK4), Ras, phosphatase and tensin homolog deleted on chromosome 10 (PTEN) and proto-oncogene B-Raf (BRAF). The BRAF gene mutations were observed in ~50% of CMM cases. Mutations of P53 gene are not characteristic of CMM cells. Med Pr 2016;67(2):255–266

Opis:
Sentinel lymph nodes are the first lymph nodes in the region that receive lymphatic drainage from a primary tumor. The detection or exclusion of sentinel lymph node micrometastases is critical in the staging of cancer, especially breast cancer and melanoma because it directly affects patient’s prognosis and surgical management. Currently, intraoperative sentinel lymph node biopsies using blue dye and radioisotopes are the method of choice for the detection of sentinel lymph node with high identification rate. In contrast, conventional ultrasound is not capable of detecting sentinel lymph nodes in most cases. Contrast enhanced ultrasound with contrast specific imaging modes has been used for the evaluation and diagnostic work-up of peripherally located suspected lymphadenopathy. The method allows for real-time analysis of all vascular phases and the visualization of intranodal focal “avascular” areas that represent necrosis or deposits of neoplastic cells. In recent years, a number of animal and human studies showed that contrast enhanced ultrasound can be also used for the detection of sentinel lymph node, and may become a potential application in clinical routine. Several contrast agents have been used in those studies, including albumin solution, hydroxyethylated starch, SonoVue®, Sonazoid® and Definity®. This review summarizes the current knowledge about the use of ultrasound techniques in detection and evaluation of sentinel lymph node.
Węzły wartownicze są pierwszymi węzłami chłonnymi na drodze spływu chłonki z okolicy guza pierwotnego. Wykluczenie lub potwierdzenie mikroprzerzutów w węźle wartowniczym odgrywa istotną rolę w ustalaniu stanu zaawansowania nowotworu, szczególnie nowotworu piersi i czerniaka, ponieważ bezpośrednio wpływa na rokowanie i wybór metody chirurgicznego leczenia. Obecnie śródoperacyjna biopsja węzłów wartowniczych z użyciem niebieskiego barwnika oraz radioizotopów są metodami z wyboru umożliwiającymi zidentyfikowanie węzłów wartowniczych. W przeciwieństwie do biopsji standardowe badanie ultrasonograficzne z reguły nie pozwala na ich uwidocznienie. Dzięki badaniu ultrasonograficznemu z użyciem środka kontrastującego oraz opcji obrazowania właściwych dla ultrasonografii ze środkami kontrastującymi można zdiagnozować położone obwodowo nieprawidłowe węzły chłonne. Metoda ta umożliwia ocenę węzłów w czasie rzeczywistym w każdej fazie naczyniowej, a także na uwidocznienie wewnątrzwęzłowych ogniskowych obszarów „nieunaczynionych”, które reprezentują obszary martwicy lub skupiska komórek nowotworowych. W ostatnich latach w wielu badaniach przeprowadzanych na zwierzętach i u ludzi wykazano, że badanie ultrasonograficzne z użyciem środka kontrastującego może uwidocznić węzeł wartowniczy i stać się rutynowym badaniem diagnostycznym. W badaniach tych zastosowano kilka środków kontrastujących, takich jak roztwór albuminy, skrobia hydroksyetylowana oraz SonoVue®, Sonazoid® i Definity®. Niniejsza praca stanowi podsumowanie obecnej wiedzy dotyczącej zastosowania ultrasonografii w diagnostyce węzłów chłonnych wartowniczych.
