The aim of the study was to present early outcomes of liver resection using laparoscopic technique. Material and methods. Retrospective analysis of patients who underwent liver resection using laparoscopic method was conducted. The analyzed group included 23 patients (11 women and 12 men). An average patient age was 61.3 years (37 – 83 years). Metastases of the colorectal cancer to the liver were the cause for qualification to the procedure of 15 patients, metastasis of breast cancer in 1 patient and primary liver malignancy in 5 patients. The other 2 patients were qualified to the liver resection to widen the surgical margins due to gall-bladder cancer diagnosed in the pathological assessment of the specimen resected during laparoscopic cholecystectomy, initially performed for other than oncology indications. Results. Hemihepatectomy was performed in 11 patients (9 right and 2 left), while the other 12 patients underwent minor resection procedures (5 metastasectomies, 4 nonanatomical liver resections, 1 bisegmentectomy, 2 resections of the gall-bladder fossa). An average duration of the surgical procedure was 275 minutes 65 – 600). An average size of the resected tumors was 28 mm (7 – 55 mm). In three cases conversion to laparotomy occurred, caused by excessive bleeding from the liver parenchyma. Postoperative complications were found in 4 patients (17.4%). Median hospitalization duration was 6 days (2 – 130 days). One patient (4.3%) was rehospitalized due to subhepatic abscess and required reoperation. Histopathology assessment confirmed radical resection (R0) in all patients in our group. Conclusion. Laparoscopic liver resections seem to be an interesting alternative in the treatment of focal lesions in the liver.

Experimental research on liver transplantation was introduced in Poland by Waldemar Olszewski and his team in 1972. Not until 15 years later, in 1987, did Stanisław Zieliński in Szczecin and Marian Pardela in Katowice make an attempt of transplanting liver in humans. In 1989 another attempt was made by Jacek Pawlak and Marek Krawczyk in Warsaw. The first successful liver transplantation in Poland was performed in 1990 by Piotr Kaliciński at the Children’s Memorial Health Institute, Warsaw. Also, in early 1990s the attempts were made by Jerzy Polański in Warsaw and Piotr Szyber in Wrocław. In the next years, liver transplantations were connected with three centers in Szczecin and were associated with the following persons: Roman Kostryka, Maciej Wójcicki and Samir Zeaira. In Warsaw, 1994, Jacek Pawlak, Bogdan Michałowicz and Krzysztof Zieniewicz performed another successful liver transplantation. The program started to develop rapidly and is still up and running. In 2000, Wojciech Rowiński and Marek 
Pacholczyk created another liver transplant center in Warsaw, while in 2005 Lech Cierpka and Robert Król did the same in Katowice. In the following years, liver transplantation was initiated by Maciej Słupski in Bydgoszcz (2017) and Zbigniew Śledziński in Gdańsk (2018). In the developing liver transplant centers, an exceptional contribution was made by Paweł Nyckowski, Jacek Pawlak, Krzysztof Zieniewicz, Waldemar Patkowski, Tadeusz Wróblewski, Rafał Paluszkiewicz, Marek Pacholczyk, Andrzej Chmura, Maciej Kosieradzki and Marek Krawczyk – all employed by the Medical University of Warsaw. In Wrocław, Dariusz Patrzałek and Paweł Chudoba were very active in the field of liver transplantations. In 1996, the Organizing-Coordinating Center for Transplantation POLTRANSPLANT was brought to life and at first was directed by Janusz Wałaszewski, then by Roman Danielewicz and Artur Kamiński. In 1999, Piotr Kaliciński and Marek Krawczyk started the program for liver fragment harvesting and transplantation from living donors. Until the end of 2016, 4186 liver transplantations including 314 liver transplants from living donors were performed in Poland. Currently there are three active centres in Warsaw, namely Pediatric Surgery and Transplantation Surgery, Children’s Memorial Health Institute; Department of General, Transplant and Liver Surgery, Medical University of Warsaw; 
Department of General and Transplant Surgery, Medical University of Warsaw. Other active centres in Poland are Department of General and Transplant Surgery, Provincial Hospital in Szczecin, Department of General, Vascular and Transplant Surgery, Medical University of Silesia, Katowice, Department of Vascular, General and Transplant Surgery in Wrocław. Liver transplant programs have also been initiated at the Department of Liver and General Surgery, Bydgoszcz, and Department of General, Endocrine and Transplant Surgery, Medical University of Gdańsk.

Introduction. Breast cancer (BC) makes up nearly 26% of malignant tumours worldwide and is the leading cause of cancerrelated deaths in European women. With approximately 18,000 new cases of BC diagnosed in Polish women annually, breast cancer liver metastasis (BCLM) is respectively an increasing issue. Recent data found in literature indicates improved survival following liver resection with systemic therapy. Objective. The aim of study was to evaluate surgical treatment in patients with isolated BCLM. Materials and method. During 2009–2013, a retrospective study was undertaken and 30 cases analysed. From nearly 2,000 liver resections performed, 11 female patients at the mean age of 59.18 years with BCLM were qualified for surgery. Results. The median time between primary and secondary treatment was 3.5 years (1–7). One patient (9.1%) presented an extrahepatic lesion – bone metastasis. The left lobe, right lobe and both lobes of the liver were affected, respectively, in 3 (27.3%), 4 (36.4%) and 4 (36.4%) patients. 5 patients (45.5%) presented single hepatic lesion, in contrast to the maximum number of lesions which equalled 6 in the right lobe. Average hospitalisation period was 13.27 days and discharge on the 11.3 postoperative day. One-year survival was 72.7% (8 patients); therefore, three-year survival was 36.4% (4 patients). Conclusions. Oncological centres should assess BCLM patients more openly and qualify them for hepatic resection along with adjuvant systemic treatment in order to improve overall survival. This, however, needs to be studied in a multicentre randomized trial.

Liver is the most common location of the colorectal cancer metastases occurrence. Liver resection is the only curative method of treatment. Unfortunately it is feasible only in 25% of patients with colorectal liver metastases, often because of the extensiveness of the disease. The aim of the study was to evaluate the predictive value of total tumor volume, size and number of colorectal liver metastases in patients treated with right hemihepatectomy. Material and methods. A retrospective analysis was performed in a group of 135 patients with colorectal liver metastases, who were treated with right hemihepatectomy. Total tumor volume was estimated based on the formula (4/3)πr3. Moreover, the study included an analysis of data on the number and size of tumors, radicality of the resection, time between primary tumor resection and liver resection, pre-operative blood serum concentration of carcinoembryonal antigen (CEA) and carcinoma antigen Ca19-9. The predictive value of the factors was evaluated by applying a Cox proportional hazards model and the area under the ROC curve. Results. The univariate analysis has shown the predictive value of size of the largest tumor (p=0.033; HR=1.065 per each cm) on the overall survival, however no predictive value of number of tumors (p=0.997; HR=1.000) and total tumor volume (p=0.212; HR=1.002) was observed. The multivariate analysis did not confirm the predictive value of the size of the largest tumor (p=0.141; HR=1.056). In the analysis of ROC curves, AUROC for the total tumor volume, the size of the largest tumor and the number of tumors were 0.629, 0.608, 0.520, respectively. Conclusions. Total tumor volume, size and number of liver metastases are not independent risk factors for the worse overall survival of patients with colorectal liver metastases treated with liver resection, therefore increased values of these factors should not be a contraindication for surgical treatment

Introduction. Non-alcoholic fatty liver disease (NAFLD) is excessive fat build-up in the liver due to causes other than alcohol use. Aim. To evaluate the clinical efficacy and safety of Liv.52 DS tablets in the management of NAFLD. Material and methods. Prospective, interventional clinical study conducted on 60 patients of both sex, aged between 18-65 years, confirmed with NAFLD from clinical examination, laboratory test, ultrasound findings and those willing to give informed consent. All patients received Liv.52 DS at a dose of 2 tablets twice daily for 2 months. All patients were evaluated at baseline, end of 1st month, and end of 2nd month for liver function tests, hepatomegaly by ultrasound, NAFLD Fibrosis Score, lipid profile, hematology and biochemical investigations. Results. Study data was analyzed with GraphPad Prism Software Version 6.07. Data of those patients who completed the study was considered for analysis. Significant improvement in hepatomegaly, liver enzymes was observed. NAFLD fibrosis score revealed no progression of liver fibrosis due to NAFLD during the study period. No abnormal lab values were recorded and there were no adverse events reported during the study. Conclusion. Study concludes that Liv.52 DS is safe and beneficial in individuals suffering from NAFLD.

Objective. Evaluation of the neutrophil-lymphocyte ratio and mean platelet volume as predictive factors for liver fibrosis and steatosis in HBV patients qualified for antiviral treatment. Materials and method. The study comprised 38 CHB patients who had commenced antiviral treatment, and 20 healthy volunteers who constituted the clinical control group. All patients had their blood count taken and underwent hepatic assessment using transient elastography with CAP (controlled attenuation parameter). Results. It was found that the mean hepatic fibrosis was 8.7 kPa (±8.8) and the mean liver steatosis – 286 db/m (±64). Mean NLR – 2.78(±1.1), whereas in the control group the mean NLR value was 1.64(±0.98). A negative linear correlation (r= -0.34; p=0.035) was found between liver fibrosis and the NLR value in the study group. No correlation was observed between hepatic steatosis and the NLR. Mean MPV – 12.6fl (±3.1), which was considerably higher in the CBH patients than in the control group. A positive correlation (r= 0.79, p= 0.001) was found between MPV and disease severity evaluated with transient elastography. Conclusions. The NLR and the MPV were significantly higher in the CHB patients than in the healthy volunteers. Both the NLR and the MPV can be treated as predictive factors for liver fibrosis in this group of patients.

Liver transplant provides a definitive therapeutic measure for patients with chronic and acute liver diseases. Apart from the improvement of overall health, an organ transplant entails several metabolic complications. They are multi-agent and depend, among others, on the function of organ being transplanted, adverse effects of immunosuppression being applied, organ complications induced by failure of the organ being transplanted, current treatment, concomitant diseases and consequences of the acute and chronic rejection processes. Improvements in surgical techniques, peritransplant intensive care, and immunosuppressive regimens have resulted in significant improvements in short-term survival. Focus has now shifted to address long-term outcomes of liver transplantation. Therefore, this paper presents the current review of literature referring to specificity of the prevalence of metabolic syndrome and its complications in patients after liver transplantation.

Oleic acid (OA) is widely used in pathology studies of hepatocellular lipid deposition. Identifying the effects of different solvents on OA-induced liver lipid deposition would be beneficial for studies on hepatocytes. We treated BRL 3A cells with OA dissolved in different solvents. After 12 h incubation, cell viability was assessed using MTT assays. Reactive oxygen species (ROS), triglyceride (TG) and total cholesterol (TC) counts, and the expression level of glucose regulated protein (GRP78), sterol regulatory element binding protein (SREBP-1C) and fatty acid synthase (FAS) were analyzed. Water, PBS and DMSO were disadvantageous to the dissolution of OA and did not cause an OA-induced response in hepatocytes. In the alcohol+OA-treated cells, the severe ER stress, oxidative stress and cellular fat deposition were significantly increased. BSA promoted cell growth and the cells treated with 1.2% BSA+OA showed a lower grade TG and endoplasmic reticulum stress compared with KOH+OA and alcohol+OA treatments. KOH had no significant influence on BRL 3A cells viability. When treated with OA dissolved in KOH, BRL 3A cells showed a typical hepatocyte damage. KOH was considered the suitable choice for an OA solvent for BRL 3A cells in hepatic lipidosis research.

Hemangiomas are the most common benign primary hepatic neoplasms, often being incidentally discovered. In most of the cases they are small and asymptomatic. It is widely accepted that clinical intervention is indicated only for symptomatic hemangiomas. We present a case of an asymptomatic giant hemangioma managed by enucleation due to its atypical localization. The hemangioma, originally located in segment 5, was now described in Computer Tomography (CT) Imaging as separating the gallbladder from the liver parenchyma. A careful evaluation of images revealed proximity to the portal vein (PV), right hepatic artery (RHA), right hepatic duct (RHD) and right branch of the portal vein (RBPV). Thus, in the case of an emergent operation, surgical maneuvers in the area of the altered hepatic anatomy and proximity to the hemangioma itself, would in fact increase the risk endangering the patient’s life. After patient’s consent, a surgical enucleation en block with the gall-bladder was performed. It is of great importance that specifically selected, asymptomatic patients diagnosed with a giant hemangioma, with the above mentioned or similar localization should be considered for surgical treatment.

Background: Gallstone disease (GSD) and nonalcoholic fatty liver disease (NAFLD) are connected with a high prevalence in the general population and they share common risk factors for their occurrence. Limited literature with inconsistent results is available, suggesting a potential association between these lifestyle-induced diseases. Liver biopsy is the gold standard for diagnosing NAFLD. The aim of this study was (1) to identify the prevalence of asymptomatic NAFLD or NASH in liver biopsy; (2) to identify the association of hypercholesterolemia with NAFLD in patients undergoing laparoscopic cholecystectomy (LC). Methods: This is a prospective observational study conducted on patients who underwent LC for symptomatic gallstones in the Department of General Surgery, Post Graduate Institute of Medical Education and Research, Chandigarh, India, from 1st July 2013 to 31st December 2014. All included patients had ultrasonography (USG) and the following parameters tested: serum triglycerides (TG), cholesterol, low density lipoprotein cholesterol (LDL-C), and high-density lipoprotein cholesterol (HDL-C). A wedge liver biopsy was obtained from free edge of the right liver lobe during LC and all biopsy specimens were analyzed by a single pathologist. Results: Among 101 patients included in the study, dyslipidemia was present in 49.50%. There was no association between NAFLD and serum cholesterol, TG or LDL-C (P, 0.428, 0.848, 0.371 respectively). NAFLD was confirmed in liver biopsy in 21.8% of patients but none had fibrosis or cirrhosis on biopsy. No complications were observed following liver biopsy. Conclusions: Liver biopsy during LC gives an opportunity to diagnose the disease at an early and reversible stage. It is feasible, safe and cost effective.

Liver resection is essential part of colorectal cancer liver metastases (CLM) treatment. Mean 5-year overall survival after resection achieves 30-45%. There are many factors influencing long-term outcomes, and among them the inflammatory response to tumor plays an important role. The aim of the study was evaluation of outcomes and treatment safety of patients with metastatic colorectal cancer to the liver with estimation of prognostic factors. Material and methods. 130 consecutive patients (70 men and 60 women) operated in MSC Institute and Cancer Center in Gliwice from 2001 to 2009 due to colorectal liver metastases were analysed. Age of the patients ranged from 33 to 82 years (median 60 years). 96 (74%) patients underwent potentially radical resection, and in remaining 34 (26%) was performed radiofrequency ablation (RFA) alone or combined with the resection. In the resection group 37 right hepatectomies, 11 left hepatectomies, 28 segmentectomies and 20 metastasectomies were performed. Disease-free survival (DFS) and overall survival (OS) were statistically analysed using the Kaplan-Meier method. Factors determining DFS and OS were analysed using Cox regression model. Results. In the resection group the 3- and 5-years OS was 64,5% and 46,6% respectively, and the 3- and 5-years DFS was 32% and 30,5% respectively. In the RFA group the 3- and 5-years OS was 33% and 9,5%. Statistically significant prognostic factors in the resection group in uni- and multivariate analysis were: grade and nodal involvement of the primary tumor, diameter of metastatic focus, positive and narrow (<1 mm) resection margins, preoperative fibrinogen level, preoperative neutrophil to lymphocyte ratio and leukocyte amount of the peripheral blood. The perioperative mortality rate was 3%. Conclusions. Liver resection due to colorectal liver metastases is a safe and effective method resulting in high survival rates. We confirmed some generally accepted prognostic factors influencing longterm outcomes and shown the impact of inflammatory response. We also confirmed the hypothesis that preoperative plasma fibrinogen level influences outcomes after liver resection due to CLM.

The aim of the study was to analyze indications and results of the first one thousand liver transplantations at Chair and Clinic of General, Transplantation and Liver Surgery, Medical University of Warsaw.Material and methods. Data from 1000 transplantations (944 patients) performed at Chair and Clinic of General, Transplantation and Liver Surgery between 1994 and 2011 were analyzed retrospectively. These included 943 first transplantations and 55 retransplantations and 2 re-retransplantations. Frequency of particular indications for first transplantation and retransplantations was established. Perioperative mortality was defined as death within 30 days after the transplantation. Kaplan-Meier survival analysis was used to estimate 5-year patient and graft survival.Results. The most common indications for first transplantation included: liver failure caused by hepatitis C infection (27.8%) and hepatitis B infection (18%) and alcoholic liver disease (17.7%). Early (< 6 months) and late (> 6 months) retransplantations were dominated by hepatic artery thrombosis (54.3%) and recurrence of the underlying disease (45%). Perioperative mortality rate was 8.9% for first transplantations and 34.5% for retransplantations. Five-year patient and graft survival rate was 74.3% and 71%, respectively, after first transplantations and 54.7% and 52.9%, respectively, after retransplantations.Conclusions. Development of liver transplantation program provided more than 1000 transplantations and excellent long-term results. Liver failure caused by hepatitis C and B infections remains the most common cause of liver transplantation and structure of other indications is consistent with European data.

The quality of liver assessment in an oncological patient plays an important role in the selection of a proper type of medical intervention. Diagnostic techniques commonly used in liver imaging are still far from perfect. Intraoperative liver evaluation using an intraabdominal ultrasound probe remains an important tool for proper assessment of this organ. The aim of the study was to evaluate suitability of this intraoperative diagnostic method for detection of primary and secondary neoplastic pathologies of the liver. Material and methods. Between March 2010 and the end of December 2011, we performed intraoperative ultrasound examinations of the liver during 220 of 461 laparotomies carried out for oncological reasons. Results. In 72 patients (33%), intraoperative ultrasonography using an intraabdominal probe revealed neoplastic pathologies in the liver. In 16 patients (7%), the pathologies had not been observed in the preoperative imaging examinations. In 7 cases (3%), the detected tumors were impalpable and invisible in macroscopic examination routinely performed during laparotomy. The time of performing preoperative liver examinations did not affect the detection of previously unrecognized liver tumors (p > 0.05). We found progression in the number of liver tumors in 28 patients (39%). In 20 patients (9%), the primary surgical plans were changed intraoperatively. Conclusions. Liver examination using an intraabdominal ultrasound probe is a useful tool for assessment of neoplastic disease progression. The procedure allows proper choice of an optimal treatment regime and decreases the risk of performing an unnecessary oncological invasive procedure.

Ultrasonography is a noninvasive diagnostic tool used to image size, shape, parenchyma and vascularization of various body organs. Unfortunately, the ultrasonographic image is characterized by a low contrast due to similar acoustic properties of the soft tissue. The Doppler mode provides information about blood flow, but is incapable of imaging small vessels and capillaries because of their low blood flow velocity (1 mm/s). However, a possibility to increase the effectiveness of ultrasonographic diagnostics exists, thanks to intravenous ultrasound contrast agents (UCAs) consisted of gas microbubbles. The purpose of this review paper is to characterize specific imaging techniques necessary to conduct a contrast-enhanced liver examination and indications for CEUS as an alternative diagnostic method.

Introduction: The musculocartilaginous complex is a structure composed of cartilaginous, osseous and muscular elements, which is located at the thoracoabdominal junction, at the level of the right costal arch. Aim: To determine the ultrasonographic characteristics of this complex under normal conditions as well as to demonstrate its effects on the liver depending on the constitutional body built, respiratory phase and patient’s body position. Materials and methods: All abdominal ultrasound scans were performed between 2006 and 2015. A total of 1000 patients (566 females and 434 males aged between 35 and 82 years, mean age 52 years), who had no significant upper abdominal pathologies identified based on clinical and imaging data, were enrolled for the analysis. In addition to standard internal organ assessment, we also attempted to identify the symptoms of hepatic compression by the musculocartilaginous complex. We used 3–6 MHz convex and 7–12 MHz linear transducers. The degree of musculocartilaginous compression of the liver was assessed during breathing in supine and sitting position, as well as with trunk inclined forward. Results: The study showed that musculocartilaginous compression of the liver mostly affects females (96%) with leptosomatic body build. The complex compressing the liver shows a heterogeneous echostructure. Increased hepatic compression was observed during exhalation, in a sitting position and with trunk inclined forward. Conclusions: Cartilaginous compression of the liver depends on body built and patient’s body position. The musculocartilaginous complex may cause focal or segmental compression of the hepatic parenchyma, causing pain in the right upper abdomen in some patients.
Wprowadzenie: Kompleks chrząstkowo-mięśniowy to struktura składająca się z elementów chrzęstnych, kostnych i mięśniowych, zlokalizowana na pograniczu piersiowo-brzusznym, na poziomie prawego łuku żebrowego. Cel: Określenie ultrasonograficznych cech tego kompleksu w warunkach prawidłowych oraz wykazanie jego wpływu na wątrobę w zależności od budowy konstytucjonalnej, fazy oddechowej oraz pozycji przyjmowanej przez pacjenta. Materiał i metoda: Wszystkie badania ultrasonograficzne jamy brzusznej przeprowadzono w okresie od 2006 do 2015. Do analizy zakwalifikowano 1000 osób (566 kobiet i 434 mężczyzn, w przedziale wiekowym 35–82 lata, średnio 52 lata), u których na podstawie danych klinicznych i obrazowych nie stwierdzono istotnych zmian w narządach nadbrzusza. W badaniach ultrasonograficznych jamy brzusznej, oprócz standardowo ocenianych narządów wewnętrznych, skupiono się na poszukiwaniu objawów ucisku wątroby przez kompleks chrząstkowo-mięśniowy. Do badań wykorzystywano głowice konweksowe o częstotliwości 3–6 MHz oraz liniowe o częstotliwości 7–12 MHz. Stopień ucisku wątroby przez kompleks chrząstkowo-mięśniowy oceniano w trakcie oddychania, w ułożeniu pacjenta na plecach, w pozycji siedzącej i w przodopochyleniu tułowia. Wyniki: Wykazano, że ucisk chrząstkowopochodny wątroby dotyczy głównie kobiet (96%) o budowie leptosomicznej. Uciskający wątrobę kompleks najczęściej wykazuje heterogeniczną echostrukturę. Większy stopień ucisku wątroby występował w trakcie wydechu, w pozycji siedzącej i w przodopochyleniu tułowia. Wnioski: Ucisk chrząstkowopochodny wątroby jest uzależniony od budowy ciała oraz przyjmowanej przez pacjenta pozycji. Kompleks chrząstkowo-mięśniowy może wywierać ucisk ogniskowy lub odcinkowy na miąższ wątroby, powodując u części osób ból w prawym nadbrzuszu. Cirrhosis leads to an inability of the liver to perform its biochemical functions. It can also lead to hepatocellular carcinoma in which, as we showed lately, arginase isoenzyme pattern changes. The present work presents our results on arginase isoenzymes and their possible role in liver cirrhosis. The study was performed on tissues obtained during liver transplantation from 60 patients with liver cirrhosis, and on samples of histologically normal liver (control) from 40 patients with benign or colorectal cancer liver metastases removed during surgery, 6-7 cm from the tumor border. Arginase isoenzymes AI (so-called liver-type arginase) and AII (called extrahepatic arginase) were identified by Western blotting and isolated by ion-exchange chromatography. Their expression on mRNA level was studied by RT-PCR. A significant decrease in arginase activity, dependent of the liver clinical stage, was observed in cirrhotic tissue. Arginase AI activity and its mRNA level were significantly decreased in cirrhotic liver, whereas the activity and expression of arginase AII were concurrently raised, as compared to normal liver. Since arginase AI is a key enzyme of the urea cycle, whereas arginase AII most probably takes part in the biosynthesis of ornithine and polyamines, the defective ammonia inactivation and increased collagen biosynthesis observed in cirrhotic liver may be related to the changes in arginase AI and AII levels, respectively.

Galactosamine (GalN), a well-known hepatotoxin that depletes the cellular pool of uracil nucleotides, was previously shown to have greater impact on the inhibition of protein synthesis in hepatocytes of old rats as compared with young animals (Kmieć 1994, Ann. N.Y. Ac. Sci. 717, 216-225). In the present study we compared the effects of GalN on the nucleotide content (measured by ion-exchange HPLC) in the livers of young (4 months), adult (12 months), and old (24-26 months old) rats two hours after its intraperitoneal administration. UTP content of the livers of old control rats was significantly lower (by 28%) than that of young animals. GalN administration decreased the UTP content in the livers of young, adult and old rats by, respectively, 55%, 65% and 89%, and increased the content of UDP-sugars by 189%, 175% and 305%. The hepatic content of ATP, ADP, AMP, NAD, GTP except CTP did not differ significantly among the age groups of rats studied, and was not changed by GalN treatment. The content of CTP was significantly higher in old rats (P < 0.03) upon GalN treatment. The lower hepatic content of UTP may partially explain the increased sensitivity of hepatocytes and livers of old rats to the action of galactosamine, and possibly to other hepatotoxic compounds that decrease transcription in the liver.

Background: Feature of modern existing hazards both environmental and occupational is cumulative exposure often leading to unexpected response of the organism resulting, among other things, in interactions with cytochrome P450 system involved in biotransformation of trichloroethylene and paracetamol. Hepatotoxity of paracetamol is closely connected with hepatic glutathione level. „In therapy of acute paracetamol poisoning application of N-acetylcysteine as a factor, which protects GSH level in cells, is recommended.” Materials and method: Tests were performed on rats which were treated with trichloroethylene, paracetamol and/or N-acetylcysteine. In rat liver total level of glutathione was determined i.e. reduced and oxidized form. Results: Paracetamol just after completion of the exposure affected the glutathione level. Trichloroethylene throughout the period of observation stimulated growth of glutathione level in liver. N-acetylcysteine didn’t have any influence on the level of investigated tripeptyde. Conclusions: N-acetylcysteine removes negative effect of paracetamol especially when it’s applied with 2-hour delay. After exposure for trichloethylene immediate application of N-acetylcysteine caused noticeable lowering of glutathione level. Cumulative exposure for three xenobiotics had positive influence for glutathione level in rat liver.
Wstęp: Cechą współcześnie występujących zagrożeń, zarówno środowiskowych jak i zawodowych jest narażenie łączne, wielokrotnie prowadzące do nieprzewidzianej odpowiedzi biologicznej organizmu, wynikającej między innymi z oddziaływań na układ cytochromu P450 biorący udział w biotransformacji trichloroetylenu i paracetamolu. Hepatotoksyczność paracetamolu jest między innymi ściśle związana z wątrobowym poziomem glutationu. W terapii zatruć ostrych paracetamolem zalecane jest podawanie N-acetylocysteiny jako czynnika ochraniającego poziom GSH w komórkach. Materiał i metody: Badania wykonano na szczurach, które traktowano trichloroetylenem, paracetamolem i/lub N-acetylocysteiną. W wątrobie szczura oznaczano poziom całkowity glutationu, tj. formę zredukowaną i utlenioną. Wyniki: Paracetamol tuż po zakończeniu ekspozycji negatywnie wpływał na poziom glutationu. Trichloroetylen przez cały czas obserwacji stymulował wzrost poziomu glutationu w wątrobie. N-acetylocysteina nie miała żadnego wpływu na poziom badanego tripeptydu. Wnioski: N-acetylocysteina usuwała negatywny wpływ paracetamolu, szczególnie wtedy, kiedy podano ją z 2-godzinnym opóźnieniem. Po narażeniu na trichloroetylen natychmiastowe podanie N-acetylocysteiny niosło za sobą wyraźnie obniżenie poziomu glutationu. Narażenie łączne na trzy oceniane ksenobiotyki.

Wątroba jest największym, nieparzystym narządem jamy brzusznej i jednocześnie największym gruczołem ludzkiego organizmu. Ciąża jest stanem, który wprowadza w organizmie matki wiele zmian adaptacyjnych, umożliwiających prawidłowy rozwój płodu. Celem niniejszej pracy było zebranie i przedstawianie, na podstawie dostępnej literatury informacji na temat wątroby matki, podczas fizjologicznej ciąży. Wymiary i budowa histologiczna wątroby w czasie ciąży nie ulegają zmianie, natomiast ciąża może wywoływać zmiany obserwowane w schorzeniach wątroby. Ciąża może wpływać na produkcję i wydalanie kwasów żółciowych, a z uwagi na wysoki poziom estrogenów, może dochodzić do niewielkiego zastoju żółci. W czasie ciąży wzrasta także poziom białek produkowanych przez wątrobę.
The liver is the largest unpaired organ in the abdominal cavity and at the same time the largest gland in the human body. Pregnancy is a condition related with the implementation of numerous adaptive changes in the mother's organism which enable proper development of the foetus. The objective of the study was to collect and present on the basis of available literature, information on changes in the mother's liver in the course of physiological pregnancy. The liver's dimensions and histological structure remain unchanged during pregnancy, yet pregnancy may induce changes similar to those observed in various liver conditions. Pregnancy may influence the production and extraction of bile acids, and, due to high oestrogen levels, lead to mild cholestasis. Also, during the course of pregnancy, there is an observable increase in the level of proteins produced by the liver.

Purpose: To investigate the involvement of oxidative stress in Cadmium (Cd) induced alteration in the functional status of the liver. And to assess the efficacy of folic acid and vitamin B12 in preventing Cd-induced damage in the same. Materials and methods: The experiment was carried out for four weeks. For the experiment, 25 healthy male adult Wistar albino rats were randomly selected and were divided into five equal groups and treated as control, treated with Cd, supplemented with vitamin B12 and folic acid and in the combination of these two. After 28 days the liver function enzymes and oxidative stress parameters were measured. Results: Cd is the silent killer of the hepatic system through the induction of oxidative stress in male rats. From this investigation, it is evident that the folic acid+vitamin B12 possess significant hepatoprotective and antioxidant activity against Cd-induced hepatotoxicity in the rat model. In addition, results revealed that the folic acid alone and or in combination with vitamin B12 blunted the hepatotoxic effect significantly. Conclusions: Based on results obtained, it can be concluded that folic acid and vitamin B12 offer a protective effect in Cd-induced oxidative stress associated with hepatocellular injury. Folic acid and vitamin B12 can be considered as a potent natural antioxidant which has the capacity to provide protection against Cd-induced oxidative stress in the liver in rats. However, to elucidate the exact mechanism of this modulatory effect and to examine its potential therapeutic effects further studies are essential.

Introduction: Resection is an optimal way of treatment of hepatic tumors and metastasis from another organs. Operational injury may influence on patients body composition examined by bioelectrical impedance (BIA). Analysis of parameters may be helpful in identifying early changes indicating of deterioration in nutritional status. Aim of the study: was to assess changes in body composition of patients before and after resection of liver tumors and potential radiofrequency ablation of lesions. Material and methods: The study included a group of 50 patients of the Department of General, Gastroenterological and Oncological Surgery, Medical University of Warsaw, who were qualified for radical surgical treatment of tumors within the liver. Data on water content, fat, muscle and cell mass were analyzed. Results: Comparing data obtained from patients before and after intervention in the liver, statistically significant (p < 0.05) loss of intracellular water, muscle mass, cell mass as well as adipose tissue was demonstrated. The phase angle value in these patients also significantly changed, decreasing by an average of 0.61°. On the other hand, the increase in content was noted in the case of extracellular water. Conclusions: Surgical intervention within the liver causes noticeable, unfavorable changes in the body composition, as evidenced by the reduction in the value of muscle mass, as well as cellular mass, resulting in a decrease in the phase angle. Bioelectric impedance is a suitable method for assessing changes in body composition of patients undergoing liver resection and is useful in clinical practice. It is advisable to conduct further research in the group of patients undergoing invasive treatment of the liver due to: an increasing number of such operations and centers where this type of surgical intervention is performe.

Intraabdominal hemorrhage remains one of the most frequent surgical complications after liver transplantation. The aim of the study was to evaluate risk factors for intraabdominal bleeding requiring reoperation and to assess the relevance of the reoperations with respect to short- and long-term outcomes following liver transplantation. Material and methods. Data of 603 liver transplantations performed in the Department of General, Transplant and Liver Surgery in the period between January 2011 and September 2014 were analyzed retrospectively. Study end-points comprised: reoperation due to bleeding and death during the first 90 postoperative days and between 90 postoperative day and third post-transplant year. Results. Reoperations for intraabdominal bleeding were performed after 45 out of 603 (7.5%) transplantations. Low pre-transplant hemoglobin was the only independent predictor of reoperation (p=0.002) with the cut-off of 11.3 g/dl. Postoperative 90-day mortality was significantly higher in patients undergoing reoperation as compared to the remaining patients (15.6% vs 5.6%, p=0.008). Post-transplant survival from 90 days to 3 years was non-significantly lower in patients after reoperation for bleeding (83.3%) as compared to the remaining patients (92.2%, p=0.096). Nevertheless, multivariable analyses did not reveal any significant negative impact of reoperations for bleeding on short-term mortality (p=0.589) and 3-year survival (p=0.079). Conclusions. Surgical interventions due to postoperative intraabdominal hemorrhage do not appear to affect short- and long-term outcomes following liver transplantation. Preoperative hemoglobin concentration over 11.3 g/dl is associated with decreased risk of this complication, yet the clinical relevance of this phenomenon is doubtful.

Fat cow syndrome (fatty liver disease) is a common metabolic problem in dairy cow production during the transition period, from the dry period to lactation. This syndrome occurs when due to liver uptake of non-esterified fatty acids (NEFAs), they are stored in excess as triacylglycerols (TAGs) in the liver. Currently, diagnosis of this disorder must be confirmed by a liver biopsy to determine the fat content of liver cells, as well as blood tests for increased levels of liver enzymes (GGT, SDH, AST, and bilirubin). Fat cow syndrome is associated with the occurrence of clinical diseases, decreased immunity, and problems with reproduction. It is a major problem in the herd health due to the high cost of veterinary services, a longer calving interval, and reduced longevity of cows, causing enormous economic losses. The best means of prevention is good nutrition and routine blood tests in the herd.

Introduction. In Poland, an increasing number of patients are hospitalized due to liver diseases. One of the common liver diseases is cirrhosis, which can be caused by alcohol, viral hepatitis, autoimmune processes and metabolic diseases. Materials and method. The study included 99 patients with alcoholic cirrhosis from the Lublin region of Eastern Poland. The control group consisted of 20 healthy individuals without liver disease who did not abuse alcohol. The concentrations of serum kallistatin and chemerin were determined using ELISA kits. Objective. The aim of the study is to evaluate serum levels of kallistatin and chemerin in patients with different stages of alcoholic liver cirrhosis. Results. The highest chemerin level was found in the control group – 182.6±80.4 ng/ml. In other stages of liver cirrhosis, the following levels were observed: 175.7±62.7 ng/ml in Child-Pugh stage A (Ch-P A), 150.2±59.7 ng/ml in Ch-P B and 110.3±73.6 ng/ml in Ch-P C. Significant differences in chemerin levels between controls and Ch-P C patients (p=0.01), as well as between the Ch-P A patients and Ch-P C patients (p=0.02), were demonstrated. The highest kallistatin level was demonstrated in the control group – 8.2±3.5 μg/ml. In other stages of liver cirrhosis, the following concentrations were found: 7.2±27 μg/ml in Ch-P A, 4.4±2.2 μg/ml in Ch-P B and 3.5±1.9 μg/ml in Ch-P C (Tab. 3). Statistically significant differences were observed between controls and Ch-P B patients (p<0.001), controls and Ch-P C patients (p<0.001), Ch-P A and Ch-P B patients (p=0.01), as well as Ch-P A and Ch-P C patients (<0.001). Conclusions. The levels of chemerin and kallistatin decrease with progression of liver damage during alcoholic liver cirrhosis. The impairment of its synthetic function leads to reductions in levels of the adipokines studied.

The aim of this work is to study the effect of drug mebikar on the clinical manifestations of hepatic encephalopathy in patients with liver cirrhosis. The study involved 34 patients with cirrhosis of different etiologies. The average age of the examined patients was (48.5±0.9) years, that prevailed patients of working age, indicating the medical and social significance of the problem of early diagnosis and adequate treatment of cirrhosis. One of the most frequent complications of cirrhosis is hepatic encephalopathy. In addition to conventional clinical and laboratory findings in patients with liver cirrhosis, severities of hepatic encephalopathy were determined according to West-Haven criteria before and after treatment. In patients with liver cirrhosis was established the presence of latent or clinically expressed hepatic encephalopathy. The treatment of the control group of patients consisted of the following drugs: essential phospholipids, mixture of sorbitol and major ions, arginine glutamate, furosemide, verospiron, lactulose, amoxicillin trihydrate and lansoprazole. In the complex treatment of the main group of patients medicine mebikar was administered additionally. Analysis of the clinical manifestations of hepatic encephalopathy showed a marked improvement in patients who received additional treatment with mebikar. Specifically, the incidences of mood changes as well as anxiety decreased in this group on average of 38% compared with those patients without an additional treatment with mebikar. Also, sleep disturbances in the main group was observed to be lower by 7.2% compared to those in the control group. Inclusion in the treatment of patients with liver cirrhosis, the drug mebikar – a daytime tranquilizer with anxiolytic properties reduces neurotic disorders, improves emotional state which may indicate a regression in the manifestations of hepatic encephalopathy thereby improving the quality of life of patients and thus substantiating an expedient inclusion of mebikar an anxiolytic drug to the complex therapy of patients with liver cirrhosis.

Dysregulated lipid metabolism in liver is an important hallmark of non-alcoholic fatty liver disease (NAFLD), which may be modulated by dietary polyphenols or microRNAs (miRNAs). However, the underlying epigenetic regulatory mechanism of polyphenols remain unclear. The current study aimed to address how miRNA mediates hepatic lipid metabolic control of curcumin, a polyphenolic food supplement. The results showed that 24 h treatment with 5 - 20 μM curcumin prevented free fatty acid-induced lipid accumulation by around 10 ~ 50% in HepG2 cells, which was attenuated by pre-transfection with 40 nM miR-22-3p mimic for 48 h. In consequence, transfection with 40 nM miR-22-3p inhibitor for 48 h significantly reduced lipid accumulation by around 10%. And 48 h overexpression of miR-22-3p targeting cardiolipin synthase 1 (CRLS1) gene, which encodes a mitochondrial phospholipid synthase, showed a similar regulatory effect. Thus, miR-22-3p and CRLS1 showed opposite effects in modulating lipid metabolism, which probably involved mitochondrial control. In summary, this study demonstrated that curcumin improved hepatic lipid metabolism via targeting the miR-22-3p/CRLS1 pathway. Identification of the epigenetic regulatory mechanism underlying lipid metabolism may thereby facilitate alleviation of metabolic disorders by natural polyphenols.

Aims. The aim of the study was to assess the usefulness of TNF-α and CK-18 as diagnostic markers of alcoholic liver cirrhosis. Additionally, the effects of the stage of liver cirrhosis on concentrations of TNF-α and CK-18, as well as their correlation, were evaluated. Materials and method. Sixty-two patients with alcoholic liver cirrhosis treated in various hospitals were randomly enrolled. The stage of cirrhosis was assessed according to the Child-Turcotte-Pugh scoring system. The control group consisted of 31 healthy people without liver disease. Concentrations of TNF-α and cytokeratin-18 in blood plasma of patients and controls were measured using the sandwich enzyme immunoassay technique, with commercially available quantitative ELISA test kits. Results. The concentration of CK-18 was statistically higher in patients with alcoholic liver cirrhosis, compared to the control group. The concentration of TNF-α was significantly higher in patients with alcoholic liver cirrhosis, compared to the control group. Higher concentrations of TNF-α were found only in patients with stage C and B alcoholic liver cirrhosis, compared to healthy persons. Conclusions. The levels of TNF-α and total CK-18 were higher in patients with alcoholic liver cirrhosis than in healthy individuals. No correlation was found between the level of CK-18 and stage of liver cirrhosis.

Wstęp: HCV stanowi poważny problem w praktyce medycznej. Większość powikłań w przebiegu chorób wątroby jest konsekwencją zaawansowanego włóknienia prowadzącego nawet do marskości wątroby. Cel pracy: Celem niniejszych badań jest analiza wybranych czynników ryzyka rozwoju włóknienia wątroby u osób z zakażeniem hcv. Materiał i metody: Materiał badawczy stanowiła grupa pacjentów mieszkających na terenie województwa śląskiego. Badaną grupę stanowiły 102 osoby w wieku 13-70 lat. chorzy diagnozowani byli na oddziałach chorób zakaźnych i chorób wewnętrznych z podejrzeniem hcv. Ostateczne rozpoznanie ustalono na podstawie wywiadu z pacjentami oraz oceny histopatologicznej biopsji wątroby. Wyniki: W niniejszych badaniach udział wzięło 66 mężczyzn i 36 kobiet. Przedział wiekowy 13-23 lat stanowi 7,8%, 13, 7% to populacja w wieku 24-34 lat, 32, 4% to badani w wieku 35-45 lat, 27, 5% - pacjenci w przedziale wiekowym 46-56 lat, 16, 7% to populacja w wieku 57-67 lat, 1,9% badanych stanowiła grupa powyżej 67 roku życia. nadwaga występowała u 55,9% badanych, 76, 5% populacji to osoby palące. Wnioski: Na podstawie analizy badań stwierdza się, że wirus hcv z postępującym włóknieniem najczęściej występuje w przedziale wiekowym 35-45 lat, częściej wśród mężczyzn niż wśród kobiet. większość badanych to osoby palące papierosy oraz osoby z nadwagą.
Introduction: Hcv constitutes a serious problem in medical practice. Most complications in the course of the disease derive from advanced fibrosis leading even to the liver cirrhosis. Aim of the research: The aim of the research is the analysis of some selected risk factors of the development of fibrosis in patients with hcv and the frequency of the virus infections in The Silesia Region. Material and methods: The material was collected from the patients living in The Region of Silesia. The population for the research consisted of 102 patients at the age of 13 – 70. The patients were diagnosed as sufering from hcv by The Infectious and Internal Diseases Departments. The final diagnosis was made after interviewing the patients and the examinations of histopathological liver biopsy. The results: There were 66 males and 36 females participating in the research. Patients aged 13-23 ( 13) constituted 7,8 per cent, 24-34 (32) 7 per cent, 35-45 (27) 4 per cent, 46-56 (16) 5 per cent, 57-67 7 per cent and 2 per cent of patients over 67. 55,9 per cent of the patients were overweight and 76,5 per cent smokers. Conclusions: Taking into account the research analysis, it might be concluded that the hcv virus accompanied by liver fibrosis is found most often in the group of patients aged 35-45 and more often in males than in females. Most research patients are overweight and smoke cigarettes.

The aim of the present study was to determine whether ultrasonographic evaluation of the hepatic parenchyma could be used as a diagnostic and prognostic approach in cows and buffaloes with hepatic lipidosis. For this purpose, cows (n=16) and buffaloes (n=10) with fatty infiltration of the liver were examined by ultrasonography. Treated cows and buffaloes were monitored for hepatic changes ultrasonographically, biochemically and histologically. Clinical findings were non-specific and included anorexia, recumbency, muzzle necrosis, and icteric mucosal membranes. Laboratory data revealed neutrophilia, hyper γ-globulinemia, elevated activities of aspartate aminotransferase, γ-glutamyl transpeptidase, creatine kinase and lactate dehydrogenase, and high concentrations of insulin, total bilirubin, non-esterified fatty acids and β-hydroxyl butyric acid. Laboratory results 7, and 21 days after treatment showed progressive improvement in the chemistry profile. On admission, ultrasonographic examination of the hepatic parenchyma in cows and buffaloes revealed either increased or decreased hepatic echogenicity; histologic examination revealed marked fatty infiltration of the hepatocytes. One week after treatment, the hepatic parenchyma was visualized easily, liver boundaries were clearly imaged, and histologic examination of hepatic specimen showed a moderate degree of fatty infiltration. Three weeks after treatment, the hepatic parenchyma was almost similar to normal, the hepatic and portal blood vessels could be easily imaged, and the histologic picture had greatly improved where the liver resembled the normal organ. Six cows and seven buffaloes made a full recovery while the remaining ten cows and three buffaloes were slaughtered and thoroughly examined postmortem. Ultrasonography showed a good correlation with histologic and laboratory findings.

Currently, liver disease is widespread and the awareness of these diseases is low. Early symptoms of liver disease do not necessarily indicate problems with this organ and patients are usually informed of their problems when the stage of the disease is already advanced. Invasive biopsies are the clinical diagnostic method most commonly used in the evaluation of liver disease. A biopsy is associated with a high risk of false results and additional complications. Finding new non-invasive imaging methods has led to the discovery of a new method called Magnetic Resonance Elastography (MRE). This technique allows one to evaluate the mechanical properties of tissues and to distinguish between pathological states. Testing using this technique can be performed on a conventional magnetic resonance system by using few additional components and properly prepared software. Studies have shown that there is a strong correlation between MRE-measured liver stiffness and the degree of fibrosis. MRE is also useful in characterizing liver tumors. Studies show that this technique is highly credible in both health volunteers and patients with liver fibrosis. MRE has tremendous diagnostic potential. The described technique is not currently widely used and has the 127-132potential to serve as a safe and accurate alternative in clinical diagnostics in the future.

Introduction. Wilson's disease is a real threat to patients' quality of life. It leads to significant neurological deficits, psychiatric disorders and mainly severe liver dysfunctions. Aim. The aim of the study is to analyse selected aspects of nursing care for a patient in the course of Wilson's disease, using terms derived from ICNP®. Case study. The case report concerns a patient at a young age ,hospitalised due to exacerbation of Wilson's disease symptoms. Discussion. Wilson's disease mainly affects young people. Therefore, it is necessary to design such nursing care, which, apart from solving the somatic problems of the patient, does not affect their autonomy and minimizes deficits in the scope of social roles performed so far. Conclusions. The use of ICNP® in the planning of nursing care for a patient in the course of Wilson's disease may significantly support the implementation of systematic actions aimed at improving the quality of her life.
Wstęp. Choroba Wilsona stanowi realne zagrożenie dla jakości życia pacjentów. Prowadzi bowiem do istotnych deficytów neurologicznych, zaburzeń o podłożu psychiatrycznym a głównie poważnych dysfunkcji wątroby. Cel pracy. Celem opracowania analiza wybranych aspektów opieki pielęgniarskiej wobec pacjentki w przebiegu choroby Wilsona, z wykorzystaniem terminów zaczerpniętych z ICNP®. Prezentacja przypadku. Opis przypadku dotyczy pacjentki w młodym wieku hospitalizowanej w związku z zaostrzeniem objawów choroby Wilsona. Dyskusja. Choroba Wilsona dotyczy głównie osób młodych. Niezbędne jest zatem projektowanie takiej opieki pielęgniarskiej, która poza rozwiązywaniem somatycznych problemów chorego, nie narusza ich autonomii i minimalizuje deficyty w zakresie dotychczas pełnionych ról społecznych. Wnioski. Zastosowanie ICNP® w planowaniu opieki pielęgniarskiej wobec pacjentki w przebiegu choroby Wilsona może istotnie sprzyjać wdrażaniu systematycznych działań służących poprawie jakości jej życia.

Liver transplantation has become one of the most effective treatments for end-stage renal disease. For patients, however, the decision to have orthotopic liver transplantation (OLT) is often made in an effort to improve their quality of life and to reduce their risk of mortality and morbidity. Quality of life is an important aspect of therapy for transplant patients because this category reflects the subjective evaluation of one's own life in the physical, psychological and social dimensions. One of the means to achieve a better quality of life is not only good health, but also physical activity. Physical activity has been demonstrated to be of significance not only in the assessment of fitness levels but also could be of importance in long term recovery process after major surgical operations. The aim of this study was a review of literature showing the improved quality of life in patients after liver transplantation as well as the influence of physical activity on their physical health, mental health and quality of life after transplantation. Longitudinal data showed remarkable improvement of common domains of QOL comparing pre- and post-transplant items. Gender, occupation and regular physical activity have an influence on the quality of life after liver transplantation.

In this work, we applied two polarized light based approaches to visualize histological patterns of liver pathologies. The first one involves acquisition of two images through a polarizing microscope, one image (Ppar) acquired with the analyzer oriented parallel to the polarization of illumination and the other (Pper) acquired with the analyzer oriented perpendicular to the illumination. The final image is based on the polarization ratio, Preconstructed = (Ppar – Pper)/(Ppar + Pper). Using the second technique, the histological specimens were illuminated with a polarized laser beam with wavelength of 635 nm. Polarimetric parameters as azimuth, angle of ellipticity, degree of polarization and reflected power have been measured to quantify the change in the polarization state of the incident light after interaction with the sample of the healthy tissue and of the tissue with abnormal morphological changes.

The aim of the study was to evaluate the possibility to use live anesthetized pigs as a model for laparoscopic liver resection. During two days laparoscopy course two trainees were operating on two live animals performing exposure of the liver, Pringle manoeuver, division of liver ligaments, dissecting of the structures inside the hepatoduodenal ligament, dissection of the hepatic veins and left lateral liver sectionectomy. Exposure of the liver and Pringle manoeuver were performed correctly within 50 and 35 minutes. Left lateral sectionectomy has been performed correctly within 2 hours. The full dissection of the hepatoduodenal ligament and exposure of the hepatic veins were judged as insufficient by experienced laparoscopic tutors. There was one injury to the suprahepatic vena cava that was managed laparoscopically. The porcine model can be used as an advanced training for laparoscopic liver surgery.

We report here the cloning and characterization of a novel human short-chain dehydrogenases/reductase gene SCDR9, isolated from a human liver cDNA library, and mapped to 4q22.1 by browsing the UCSC genomic database. SCDR9 containing an ORF with a length of 900 bp, encoding a protein with a signal peptide sequence and an adh_short domain. GFP localization shows SCDR9 protein concentrated in some site of the cytoplasm, but not in the ER. Expression pattern in eighteen tissues revealed that SCDR9 is expressed highly in liver. Soluble recombinant protein was successfully purified from Escherichia coli using pET28A(+) expression vector. Our data provides important information for further study of the function of the SCDR9 gene and its products.

Opis:
Background: Liver metastases of differentiated thyroid cancers (DTC) are uncommon. Surgery has proven to be effective in patients with 131I-negative hepatic lesions. Here, we present two patients who underwent liver resection for metastatic DTC. Case presentation: The first patient is a 36-year-old woman who reported with 70-mm hepatic metastases of papillary thyroid cancer. After primary treatment of cancer, she was disease-free for 8 years when the elevation of TSH levels resulted for the need to search for metastasis. Notably, the 131I SPECT did not show any lesions. The CT scan revealed an 80mm diameter mass in the liver. Histology confirmed metastasis of thyroid cancer. Lack of iodine uptake and the size of lesion excluded treatment with radioactive iodine. Radical resection of the metastasis was performed with good short- and long-term postoperative result. The second patient is a 65-year-old man previously treated for follicular thyroid cancer. When a iodine-negative 70mm diameter metastasis was detected within the liver, he was referred for surgery. Extended right hepatectomy was performed. In a 12-months follow-up, he remained stable, with no signs of recurrence. Conclusions: These two cases show that resection of hepatic metastases of DTC is an option even in the case of large lesions. Given the effectiveness and safety of liver surgery, we reckon that it should be the treatment of choice when possible. The decision to perform surgical treatment should be based on analysis of the ability to perform radical and safe resection.

This study presented a septic shock occurring with striking speed during the percutaneous drainage of a large abscess of the liver in an 18-year-old female patient after an appendectomy performed 3 months earlier using the laparoscopic method because of its phlegmonous and gangraenosus inflammation. The post-operative course was complicated by increased suppuration in places after removal of the trocars. After including the intravenous antibiotic therapy and under the USG control, the percutaneous drainage of the abscess was performed. While withdrawing the guide of the drainage catheter set, the symptoms of septic shock occurred. The pharmacological treatment was applied with continuation of the antibiotic therapy till a negative result of microbiological tests was obtained and the normalization of life parameters. The patient was not qualified for treatment in the ICU. She was discharged in a generally good condition after 24 days of hospitalization. In the case of this patient, the drain covered with the content of the abscess may have contact with the adjacent tiny blood vessels, which may have contributed to the systemic inflammatory response syndrome that was linked with fast-developing sceptic shock.

The research aim was to evaluate the levels of contamination with fluorine compounds in selected tissues of laying hens kept in a free range system in Central Pomerania (Poland) in relation to the distance from contaminant emission sources. The study covered 60, randomly selected, two-year-old laying hens on 6 farms. Fluorine levels in tissues of laying hens were determined with the potentiometric method using an ion-selective electrode from ORION. The results for the tissues and organs, such as pectoral muscles, livers and breastbones, were investigated with an analysis of variance, the Tukey’s test and linear correlation coefficients. The analyses revealed a higher mean fluorine compound content in the pectoral muscles, livers and breastbones of laying hens in zone 1, located 50 km to the west of the contaminant emission sources, than in zone 2, situated between 80 and 100 km to the west of the pollution sources. Fluorine compound levels in the pectoral muscles of zone 1 birds were almost twice as high in hens from experimental zone 2 (respectively: 1.835 mg kg-1 of DM and 0.978 mg kg-1 of DM). A statistically significantly higher average content of fluorine compounds (4.92 mg kg-1 of DM) was found in the liver of laying hens in zone 1, nearer to the sources of fluorine compounds emission compared to zone 2 (3.34 mg kg-1 of DM). The mean fluorine compounds content in the breastbones of hens in zone 1 was more than 4 times as high as the level of this element identified in the bones of hens kept in zone 2 (respectively: 765 mg kg-1 of DM and 183 mg kg-1 of DM). A significant positive correlation was confirmed between fluorine compound levels in the breastbones and pectoral muscles. The lower level of fluorine compounds in the examined tissues of laying hens in zone 2 could have been influenced by the dominant west and south wind, which limited the spread of pollution. This may also be due to the lower level of fluorine compounds in soil and feed for laying hens in this zone determined in previous studies. The study showed that the closer the sources of emissions of fluorine compounds, the more fluoride in hard tissues of hens’ bodies.

Background: There is a number of factors which potentially affect occurrence of toxic change in liver after overdosing of paracetamol. Hepatic metabolism of trichloroethylene has primary impact on hepatotoxic effect of this solvent. This means that the combined exposure to these xenobiotics can be particularly harmful for human. The influence of N-acetylcysteine (NAC) as a protective factor after paracetamol intoxication was studies. Materials and method: Tests were carried out on rats which were treated with trichloroethylene, paracetamol and/or N-acetylcysteine. In the hepatic microsomal fraction activity of the components of cytochrome P450- dependent monooxygenases was determined Results: Paracetamol slightly stimulated cytochrome P450 having no effect on reductase activity cooperating with it. Cytochrome b5 and its reductase were inhibited by this compound. Trichloroethylene was the inhibitor of compounds of II microsomal electron transport chain. N-acetylcysteine inhibited activity of reductase of NADH-cytochrome b5. Conclusions: Tested doses of the xenobiotics influenced on II microsomal electron transport chain. Protective influence of N-acetylcysteine was better if this compound was applied 2 hours after exposure on xenobiotics.
Wstęp: Istnieje szereg czynników, które potencjalnie wpływają na ryzyko wystąpienia zmian toksycznych w wątrobie po przedawkowaniu paracetamolu. Wątrobowy metabolizm trichloroetylenu ma pierwszorzędny wpływ na hepatotoksyczny efekt tego rozpuszczalnika. Oznacza to, że narażenie łączne na te ksenobiotyki może być szczególnie szkodliwe dla człowieka. Oceniono wpływ N-acetylocysteiny (NAC) jako czynnika osłaniającego po zatruciu paracetamolem. Materiał i metody: Badania wykonano na szczurach, które traktowano trichloroetylenem, paracetamolem i/lub N-acetylocysteiną. We frakcji mikrosomalnej wątroby oznaczano aktywność składników monooksygenaz zależnych od cytochromu P450. Wyniki: Paracetamol lekko stymulował cytochrom P450 nie mając wpływu na aktywność reduktazy współpracującej z nim. Cytochrom b5 i jego reduktaza były hamowane przez ten związek. Trichloroetylen był inhibitorem składników II mikrosomalnego łańcucha transportu elektronów. N-acetylocysteina hamowała aktywność reduktazy NADH-cytochrom b5. Wnioski: Badane dawki ocenianych ksenobiotyków swój wpływ ujawniały raczej na składniki II mikrosomalnego łańcucha transportu elektronów. Ochronny wpływ N-acetylocysteiny był wyraźniejszy, jeśli podano ten związek 2 godziny po zakończeniu ekspozycji na badane ksenobiotyki.

Background: Non-alcoholic fatty liver disease (NAFLD) is one of the most common liver diseases, influenced by various risk factors associated with metabolic disorders. Currently there is no specific effective treatment for NAFLD. Probiotics have been extensively researched for their health benefit Probiotic Supplementation for NAFLD. Methods: A simple literature review was performed based on searches via PubMed, ScienceDirect and Google Scholar, using the keywords "probiotic," "microbiota," "non-alcoholic fatty liver disease," "metabolic disorder," and "therapy." Results: Research on the use of probiotics for NAFLD demonstrated improvement in liver function and histology. However, the literature is inconsistent regarding the probiotics’ influence on the NAFLD risk factors. Probiotics can be an alternative therapy for NAFLD through the ability to modulate the microbiota of the gastrointestinal tract. Conclusion: Probiotics can be an alternative therapy in NAFLD patients, however more extensive studies are needed to ensure the safety of this approach.

Introduction: Bleeding from esophageal varices is a serious clinical condition in hemophilia patients due to congenital deficiency or lack of clotting factors VIII (in hemophilia A) and IX (in hemophilia B), decreased clotting factor II, VII, IX, X synthesis in the course of chronic liver disease and hipersplenic thrombocytopenia. The aim of this study was to assess the efficacy and safety of endoscopic sclerotherapy in acute esophageal variceal bleeding and in secondary prophylaxis of hemorrhage. The aim was also to investigate the optimal activity of deficiency factors VIII or IX and duration of replacement therapy required to ensure proper hemostasis after sclerotherapy procedures. Material and methods: 22 hemophilia patients (A-19, B-4) with coexistent liver cirrhosis and active esophageal variceal bleeding treated with endoscopic sclerotherapy were subjected to prospective analysis. The patients who survived were qualified to repeated sclerotherapy procedures every 3 weeks within secondary prophylaxis of bleeding (investigated group). A 3-day substitution therapy enhanced the infusion of the deficient or lacking factor in doses allowing to reach 80-100% of normal value activity of factor VIII on the 1st day and 60-80% in the next two days. The desired activity of factor IX was 60-80% and 40-60% respectively. The control group consisted of 20 non-hemophiliac patients with liver cirrhosis comparable in terms of age, sex, stage of advancement of liver cirrhosis, who underwent the same medical proceedings as the investigated group. Results: Active esophageal bleeding was stopped in 21 of 22 (95%) hemophilia patients. Complications were observed in 3 patients; 2 patients died. The rate of hemostasis, complications and deaths in the control group were comparable and no statistical differences were found. In hemophilia patients subjected to secondary prophylaxis of hemorrhage, in 18 of 20 (80%), complete eradication of esophageal varices was achieved after 4 to 7 sclerotherapy procedures in 1 patient (average 5.4). Recurrent bleeding was observed in 15% of patients, complication in 20%; 1 patient died. Time lapse from bleeding to eradication was 12-21 weeks (average 15.2). In the control group the rate of variceal eradication, complication and deaths was comparable and no statistical differences were found. The usage of factor VIII concentrates was as follows: in hemophilia A, in a severe form - 80.9 U/kg b.w./day, in hemophilia A in a severe form with an inhibitor <5 BU – 95.2 U/kg b.w./day, in mild form – 64.2 U/kg b.w./day and in severe hemophilia B – 91.6 U/kg b.w./day. Conclusions: Sclerotherapy is an effective method in the management of esophageal variceal bleeding in hemophilia patients. It is also effective for total eradication of varices when applied as a secondary prophylaxis of hemorrhage. In our opinion, a 3-day replacement therapy at the applied doses is sufficient to ensure hemostasis and avoid bleeding complications.

Objectives The aim of this study has been to investigate serum activities of liver enzymes in workers exposed to sub-TLV levels of dimethylformamide (DMF). Material and Methods Seventy-two workers and 72 healthy controls participated in the study. All subjects underwent complete physical examinations and abdominal ultrasound examination. Serum aspartate aminotransferase (AST), alanine aminotransferase (ALT), and c-glutamyl transpeptidase (c-GT) were determined by an auto-chemistry analyzer. The data of airborne concentrations of DMF was obtained from the local Center of Disease Control and Prevention. The level of urine N-acetyl-S-(N-methylcarbamoyl)cysteine (AMCC) was measured by means of high-performance liquid chromatography. Results Time weighted average (TWA) concentration of the DMF in workplace was 18.6 (range: 9.8–36.2) mg/m³. The concentration of the AMCC in workers’ urine was 28.32 (range: 1.8–58.6) mg/l and 9 workers’ AMCC exceeded the biological exposure index (40 mg/l). Thirty-one workers reported gastrointestinal symptoms (abdominal pain, nausea, anorexia) and 10 workers reported headache, dizziness and/or palpitation in the exposed group. Serum analysis revealed that both the mean of serum activities of liver enzymes (ALT, AST and c-GT) and the percentage of workers with abnormal liver function were significantly higher in the exposed group as compared to the controls. Conclusions Dimethylformamide can cause liver damage even if air concentration is in the sub-threshold limit value (sub-TLV) level. The protection of skin contact against the exposure to the DMF might be a critical issue as far as the occupational health is concerned.

Opis:
Richter’s syndrome (RS) is a rare complication in which chronic lymphocytic leukaemia (CLL) or small lymphocytic lymphoma (SLL) transforms into a more aggressive type of lymphoma – diffuse large B cell lymphoma (DLBCL), or Hodgkin’s lymphoma (HL). The review describes the clinical case of a patient with CLL and RS diagnosis. A computed tomography (CT) scan of the abdominal cavity detected numerous normodense areas in the liver. Simultaneously, ultrasound examination (USG) of the thyroid revealed the presence of a solid hypoechogenic lump. The material sampled from closed biopsies of liver and thyroid in both cases allowed the diagnosis of diffuse large B cell lymphoma (DLBCL). The liver and the thyroid are particularly rare locations of RS. However, those cases allowed the conclusion that RS may occur even in a very unexpected and less probable location.

Introduction and aim. We aimed to evaluate the usefulness of dynamic contrast-enhanced (DCE) MRI semiquantitative analysis values in focal liver lesions (FLL) to provide additional qualities that can be used in daily practice in the differential diagnosis of lesions. Material and methods. This retrospective study included 91 patients with liver masses on DCE-MRI. The sensitivity and specificity of time intensity curves (TIC) and semiquantitative analysis values were evaluated to differentiate benign and malignant lesions. Results. The study included 91 patients (376 lesions), aged between 28-81 years. Of the lesions, 303 were malignant and 73 were benign. In TIC semiquantitative analysis, it was found that “Tpeak” and “wash-out” rate values showed differences, especially in the differentiation of HCC, metastasis, and hemangioma. Area under curve, maximum relative enhancement, and “wash-in” and “wash-out” values of metastases and hemangiomas were different. Brevity of enhancement values of HSK, hemangiomas, and metastases were found to be different. The risk of malignancy was found to be high when the “wash-out” ratio was above 0.08 (sensitivity: 64.3%, specificity: 70.4%). Conclusion. We think that the 0.08 threshold value we found for the washout ratio with DCE-MRI semiquantitative analysis data will be useful in daily practice in the differentiation of malignant and benign FLL.

Abstract. An elevation in plasma cardiac troponins is an indicator of increased perioperative risk in orthopaedic and vascular surgery, however, data on liver transplantation (LTx) are scarce. The aim of the study was to evaluate the prevalence of cardiac troponin I (cTnI) elevation in the perioperative period of LTx, and its potential relationship with 1-year mortality. Material and methods. Analysis included 79 patients with liver cirrhosis. During LTx all patients underwent hemodynamic measurements. cTnI level was determined before the operation, 24, 48 and 72 hours afterwards. One-year mortality was assessed. Results. 12.7% patients died, all during in-hospital period. cTnI level on day 1. was identified as the most promising marker of increased death risk with optimal cut-off value of 0.215 ng/mL (the sensitivity of 60.0%, specificity of 87.0%, positive predictive value of 40.0%, negative predictive value of 93.8%). The most important predictor of cTnI increase was the duration of the LTx procedure followed by amount of packed red blood cells transfused, basic stroke volume index, and cardiac output index. In conclusion: value of cTnI level assessed 24 hours post-surgery was a reliable predictor of death following LTx with optimal cut-off value of 0.215 ng/mL. The surgery time was the most important predictor of cTnI elevation.

The authors present the case of a female patient with a tumor of segment VII of the liver, which was postoperatively identified as a tuberculous granuloma. The patient was admitted for elective surgery for a liver tumor, which had been diagnosed a few months before. Computed tomography and nuclear magnetic resonance were performed, based on which focal nodular hyperplasia was suspected. Thirteen years prior to admission the patient had undergone a transurethral resection of superficial bladder carcinoma, followed by adjuvant intravesical Bacillus Calmette-Guérin (BCG-therapy). Upon surgery, segment VII of the liver was resected; postoperative course was uneventful. After the identification of granuloma, the patient was referred to a phthysiatric clinic for further diagnostics and treatment. The authors have deemed this case worthy of reporting primarily due to the exceptionally long period between the completion of BCG therapy and the onset of hepatic tumor.

Objective: This study evaluated the effects of combined ethanol extract of Funtumia africana and Abutilon mauritianum leaves (FAAM) on the liver function indices of benign prostatic hyperplasia (BPH) induced rats. Materials and Methods: The study used 30 rats divided into 5 groups, comprising normal control, BPH control, standard control, and BPH induced rats treated with 200 and 600 mg/kg/day of FAAM respectively. Results: The BPH induction caused significant (p<0.05) increases in aspartate transaminase (AST) and alkaline phosphatase (ALP) activities of the BPH control when compared with the normal control. The BPH control also had significantly (p<0.05) reductions in the total protein, albumin and globulin concentrations and significant (p<0.05) elevated total bilirubin and direct bilirubin concentrations relative to the normal control. The FAAM treated BPH-induced rats had non-significantly (p>0.05) reduced AST, and alanine transaminase (ALT) activities relative to the BPH control. The BPH-induced rats treated with 600 mg/kg/day of FAAM had significantly (p<0.05) reduced ALP activities relative to the BPH control. Treatment with FAAM caused significant (p<0.05) increases in the total protein, albumin, globulin concentrations and significant (p<0.05) reductions in the total bilirubin and direct bilirubin concentrations relative to the BPH control. BPH had no observable adverse effects on the liver histomorphology of the rats. Conclusion: The findings of this study indicated that BPH impairs liver functions and treatment of BPH with combined ethanol extract of F. africana and A. mauritianum leaves restore normal liver functions in rats with BPH.

Opis:
Effective drug therapy promoting liver regeneration is a challenging goal in pharmacotherapy of liver diseases. Several plant phytochemicals recommended in traditional medicine from over hundred plants have been investigated for its use in various liver disorders. Regeneration of injured liver depend on a proliferative potential of mature hepatocytes as well as different subsets of intrahepatic and extrahepatic stem/progenitor cells. In clinical trials a stem cell therapy resulted in a limited improvement of liver functions. Animal studies have demonstrated the involvement of bone marrow-derived stem/progenitor cells in liver regeneration. For this reason, the pharmacological activation of endogenous stem cells and pharmacological control of macrophage phenotypic polarization could be an effective method of mobilizing progenitor cells to injured liver.
Wyzwaniem dla współczesnej farmakoterapii są skuteczne leki wspomagające regenerację wątroby. Wiele substancji z ponad stu roślin zalecanych w tradycyjnej medycynie przebadano w celu ich zastosowania w chorobach wątroby. W procesie regeneracji wątroby biorą udział subpopulacje wewnątrz-wątrobowych i poza-wątrobowych komórek macierzystych/progenitorowych oraz dochodzi do proliferacji dojrzałych hepatocytów. Dane kliniczne wskazują na ograniczoną skuteczność terapii komórek macierzystych w polepszeniu funkcji wątroby. W badaniach modeli zwierzęcych regeneracji wątroby wykazano udział komórek macierzystych/ progenitorowych, pochodzących ze szpiku kostnego. Z tego powodu, rozważa się możliwość wprowadzenia farmakologicznej aktywacji endogennych komórek macierzystych oraz farmakologiczną kontrolę fenotypowej polaryzacji makrofagów, jako skutecznej metody mobilizacji komórek progenitoworych dla uszkodzonej wątroby.

We investigated the oxidative phosphorylation efficiency of liver and gastrocnemius muscle mitochondria in thermoneutral and cold-acclimated ducklings. The yield of oxidative phosphorylation was lower in muscle than in liver mitochondria, a difference that was associated with a higher proton conductance in muscle mitochondria. Cold exposure did not affect oxidative phosphorylation efficiency or basal proton leak in mitochondria. We conclude that the basal proton conductance of mitochondria may regulate mitochondrial oxidative phosphorylation efficiency, but is not an important contributor to thermogenic processes in cold-acclimated ducklings.

Introduction. Recently, more attention is being paid to the drug-induced liver injury (DILI) as a consequence of the tuberculos is treatment and the need for new medicine is emphasized. The use of isoniazid and rifampicin has a potentiating effect, which increases the risk of substancial liver damage. In turn, systemic accumulation of toxic metabolites leads to negative changes in various organs, including the brain. It causes an imbalance in biochemical and neurophysiological processes in the brain, ultimately giving the onset to the development of hepatic encephalopathy. Aim. The effects of rifampicin and isoniazid on the central nervous system have not been studied before and we aimed to evaluate the impact these two substances have on the neuronal cell adhesion molecules (NCAM) distribution and animal behavior in the rat model of DILI. Material and methods. The 24 male Wistar rats, weighing 180-220 g were used for the experiment and divided to the groups (n=6): 1 – control; 2 – rats with experimental DILI; 3 – rats with DILI plus the intravenous infusion of S-adenosyl-L-methionine at a dose of 35 mg/kg; 4 – rats with DILI plus a fixed combination of ipidacrine hydrochloride at a dose 1 mg/kg body weight and phenibut at a dose 60 mg/kg body weight daily for the last 14 days of the experiment. All experimental procedures were carried out in the accordance with the principles outlined in the current Guide to the Care and Use of Experimental Animals. The locomotor and research activities were studied in the open field test. The activity of aspartate aminotransferase (AST, ЕС 2.6.1.1) and alanine aminotransferase (ALT, ЕС 2.6.1.2) in the serum of rats were tested to confirm the liver damage. The quantitative analyses of soluble and membrane forms of NCAM were performed with ELISA. The ANOVA followed by a Tukey post-hoc test was used to assess statistical differences between groupsResults. Our investigation in the open field test revealed a significant decrease in the locomotor and research activity of rats after 28 days of rifampicin and isoniazid administration. The recovery of investigated parameters was observed in groups of animals treated with ademetionine (AD group) or combination of ipidacrine and phenibut (IP/PB group). We also observed that changes in rats’ behavior were consistent with alterations of the NCAM levels in the thalamus and hippocampus. Thus, the level of membrane NCAM was significantly decreased under DILI in both investigated brain regions (thalamus and hippocampus), while both AD and IP/PB treatments restored membrane NCAM levels towards those observed in the control group at least in the hippocampus. Conclusion. Obtained data suggests that both ademetionine and combinated drug containing ipidacrine and phenibut possesses neuroprotective properties and could prevent the decline in synaptic plasticity under antitubercular therapy.

Introduction; p to 300 million people have the body mass index (BMI) greater than 30 kg/m2. Obesity is the cause of many serious diseases, such as type 2 diabetes, hypertension, and non-alcoholic fatty liver disease (NAFLD). Bariatric surgery is the only effective method of achieving weight loss in patients with morbid obesity. Objectives: The aim of the study was to assess the impact of bariatric surgery on non-alcoholic fatty liver disease in patients operated on due to morbid obesity. Material and Methods: We included 20 patients who were qualified for bariatric procedures based on BMI > 40 kg/ m2 or BMI > 35kg/m2 with the presence of comorbidities. The average body weight in the group was 143.85kg, with an average BMI of 49.16kg/m2. Before the procedure, we evaluated the severity of non-alcoholic fatty liver disease in each patient using the Sheriff-Saadeh ultrasound scale. We also evaluated the levels of liver enzymes. Follow-up evaluation was performed twelve months after surgery. Results: Twelve months after surgery, the average weight was 102.34 kg. The mean %WL was 33.01%, %EWL was 58.8%, and %EBMIL was 61.37%. All patients showed remission of fatty liver disease. Liver damage, evaluated with ultrasound imaging, decreased from an average of 1.85 on the Sheriff-Saadeh scale, before surgery, to 0.15 twelve months after surgery (p < 0.001). As regards liver enzymes, the level of alanine aminotransferase decreased from 64.5 (U/l) to 27.95 (U/l) (p < 0.001), and the level of aspartate aminotransferase decreased from 54.4 (U/l) to 27.2 (U/l). Conclusions: Bariatric procedures not only lead to a significant and lasting weight loss, but they also contribute to the reduction of fatty liver disease and improve liver function.

Opis:
Celem niniejszego opracowania jest przedstawienie roli argininy w organizmie człowieka. Arginina jest aminokwasem względnie egzogennym, co oznacza, że pomimo tego, iż jest produkowana w ludzkim ciele, to dostarczanie jej wraz z pożywieniem jest równie ważne dla zachowania homeostazy organizmu. Główne źródło argininy w diecie stanowią mięso i owoce morza. Aminokwas ten jest syntezowany w czasie cyklu mocznikowego w wątrobie, a także w nerkach. Bierze udział między innymi w wytwarzaniu tlenku azotu, który jest czynnikiem rozszerzającym naczynia. Liczne prace wskazują również na skuteczność suplementacji argininą w zaburzeniach erekcji. Udział argininy w tej reakcji przyczynił się do zainteresowania wielu badaczy. Dowiedziono, że oprócz powszechnie znanego zastosowania argininy jako leku wspomagającego wątrobę w usuwaniu z organizmu amoniaku, odpowiednia suplementacja tego aminokwasu ma pozytywny wpływ na poprawę wartości ciśnienia tętniczego indukowanego ciążą, nadciśnienia płucnego oraz wyrównanie niewydolności nerek, cukrzycy, jaskry, gojenia się ran i profilaktykę udarów mózgu. Uważa się, że stężenie argininy wpływa na rozwój chorób neurodegeneracyjnych postępujących z wiekiem. Aminokwas ten jest też środkiem chętnie stosowanym przez sportowców w celu poprawienia wydolności organizmu. Jednak przeprowadzone do tej pory badania nie potwierdzają jego pozytywnego wpływu w tym zakresie.

A case is presented of mucormycosis in a patient with acute myeloblastic leukemia following liver transplantation for Wilson’s disease. A 58-year-old female was admitted to the Department of Haematology with deterioration of her general condition, loss of appetite, tiredness and difficulty with mental contact for a few days. Blood and urine cultures for bacteria and fungus, galactomannan antigen were negative. Whole body computed tomography demonstrated bilateral hilar lymphadenopathy with necrotic lesions: splenomegaly with a hypodensive lesion 13 × 20 × 19 mm and lower pulmonary infiltrates suggested fungal etiology. Magnetic resonance imaging of the brain showed thickened meninges. Finally, mucormycosis was diagnosed. Treatment with amphotericin B lipid complex was started, resulting in an partial improvement of the general condition and decreased level of inflammatory markers. However, the patient’s condition continued to deteriorate, with sepsis etiology Escherichia coli, and despite the intensive managements she eventually died.

Introduction and aim. Hydrogen sulfide (H2S) has attracted the attention of researchers as a novel signaling molecule that affects vascular metabolism, immune function, stress and inflammation. It plays an important role in pathophysiological disorders under the conditions of the development of obesity, diabetes, non-alcoholic fatty liver disease and cardiovascular diseases. The purpose of this work is to establish correlation ratios of H2S concentration with markers of oxidative-nitrosative stress and extracellular matrix metabolism of the liver during chronic alcoholic hepatitis modeling and AMPK modulation by phenformin and doxorubicin. Material and methods. The experiments were performed on 36 white, sexually mature male Wistar rats, weighing 180-220 g. Alcoholic hepatitis was modelled by alcohol administration, on the background of alcoholic hepatitis animals received phenformin orally at a dose of 10 mg/kg or doxorubicin at a dose of 1.25 mg/kg intraperitoneally. Statistical processing of the results of biochemical studies was carried out using the non-parametric method of Spearman to determine correlations. Results. H2S during alcoholic hepatitis inversely proportionally strongly correlates with the concentration of nitrites, oxyproline and arginase activity. Phenformin administration during alcoholic hepatitis leads to formation of inversely proportionally strongly correlation of H2S with the production of superoxide anion radical, the concentration of malondialdehyde, activities of constitutive NO-synthases, nitrite reductases, nitrate reductases, and arginase. Doxorubicin administration during alcoholic hepatitis leads to formation of directly proportional strongly correlation of H2S with the activity of constitutive NO-synthases, nitrite reductases, nitrate reductases. Conclusion. Administration of phenformin or doxorubicin expands correlations between H2S and indicators of oxidative-nitrosative stress.

The aim of the study was to determine serum concentrations of afamin and adropin in patients with alcoholic liver cirrhosis and to define their correlation with the stage of disease. The study included 99 patients with alcoholic cirrhosis from the region of Lublin, (Eastern Poland). Liver cirrhosis was diagnosed based on clinical features, history of heavy alcohol consumption, laboratory tests and abdominal ultrasonography. The control group consisted of 20 healthy individuals without liver disease who did not abuse alcohol. The serum afamin and adropin concentrations were determined using ELISA kits. The concentration of afamin was found to be significantly lower in patients with compensated alcoholic liver cirrhosis, i.e. P-Ch B (85.1±40.6 μg/ml) and P-Ch C (56.4±32.3 μg/ml) individuals, compared to the control group (135.9±43.6 μg/ml); p-value was <0.01 and <0.001, respectively. As far as adropin is concerned, a reverse relationship was demonstrated: the highest concentration was found in patients with P-Ch C (11.7±5.7 ng/ml) cirrhosis. Furthermore, the above concentration was significantly higher compared to patients with P-Ch A cirrhosis (7.2±2.8 ng/ml; p<0.05) and controls (7.5±2.6 ng/ml; p<0.05). The concentration of afamin decreases with the severity of alcoholic liver cirrhosis, which most likely results from impaired hepatic synthesis. Otherwise, the higher the stage of disease according to the Child-Pugh score, the higher the concentration of adropin.

The aim of the study was to evaluate the effect of acetaminophen (APAP) and/or trichloroethylene (TRI) on the liver cytochrome P450-dependent monooxygenase system, CYP2E1 and CYP1A2 (two important P450 isoforms), and liver glutathione (GSH) content in rats. Rats were given three different doses of APAP (250, 500 and 1000 mg/kg b...) and then the above-mentioned parameters were measured for 48 h. The lowest APAP dose produced small changes in the cytochrome P450 content of liver. At 500 mg/kg APAP increased the cytochrome P450 content to 230% of the control. The inductive effect was seen at 1000 mg/kg dose but at 24 h and later. NADPH-cytochrome P450 reductase activity was the highest after the lowest dose of APAP, while after the highest dose it was equal to the control value. TRI increased both the cytochrome P450 content and the NADPH-cytochrome P450 reductase activity. When TRI was combined with APAP, both these parameters increased in the first hours of observation, but they returned to the control values at 24 h. When APAP was given at 250 mg/kg, GSH levels decreased to 55% of the control at 8 h and returned to the control values at 24 h. The higher doses of APAP decreased GSH levels more than the lowest dose, but after 24 h GSH levels did not differ from those of the control. When TRI was given at 250 mg/kg, the GSH levels decreased to 68% of the control at 2 h and then they increased gradually and tended to exceed the control values at 48 h. The effect of TRI combined with APAP on the level of GSH was virtually the same as that of APAP alone given at 500 mg/kg.

Neospora caninum is an important cause of abortion in dairy cattle. The general health of affected cows has not been investigated before. Therefore, the main objective of this study was to identify possible relationships between certain metabolic diseases and selected serum biochemical parameters in seropositive dairy cows against N. caninum antibodies in different stages of lactation. The study was carried out using 72 N. caninum seropositive cows and 61 seronegative dairy cows (control). Serum from all cows was tested to determine their N. caninum status (seropositive vs seronegative) using commercially available indirect enzyme-linked immunosorbent assay test kit (iELISA). In addition, serum biochemical parameters including beta-hydroxybutyrate (BHB), glucose, creatinine, blood urea nitrogen, total protein, albumin, alkaline phosphatase (ALP), aspartate aminotransferase (AST), alanine transaminase (ALT), lactate dehydrogenase (LDH) and gamma-glutamyltranspeptidase (GGT) were determined using routine laboratory methods. The stage of lactation was obtained at the time of sampling from farm records. Student independent t-test showed that there was a significant difference in the serum concentrations of BHB, AST, ALT, and LDH between seropositive and seronegative cows. There was no significant association between seropositivity and the stage of lactation. However, multivariable logistic regression analysis showed that there was a strong association between seropositivity and BHB concentrations. Results of this study indicate a possible relationship between N. caninum seropositivity and certain metabolic diseases such as ketosis and fatty liver syndrome in dairy cows.

Gynaecomastia is uni- or bilateral breast enlargement in males associated with benign hyperplasia of the glandular, fibrous and adipose tissue resulting from oestrogen-androgen imbalance. Asymptomatic gynaecomastia is a common finding in healthy male adults and does not have to be treated, while symptomatic gynaecomastia might be the symptoma of many pathological conditions and requires meticulous diagnosis and therapeutic management. The commonest causes of gynaecomastia in the Polish population include liver cirrhosis and drugs used to treat its complications. The current study presents the case of severe painless gynaecomastia in a patient with decompensated alcoholic liver cirrhosis, treated with spironolactone because of ascites. Breast enlargement assessed a IIb according to the Simon’s Scale or III according to the Cordova-Moschella classification, developed slowly over the two-year period of low-dose spironolactone therapy The course and dynamics of disease are described and the main mechanisms leading to its development discussed. The importance of effective treatment of patients with severe gynecomastia is emphasized as the disease may result in significant psychosocial problems.

Liver metastases are diagnosed synchronously with the primary tumour in 25% of patients with colorectal cancer. A half of the remaining patients develop liver metastases within 3 years following colectomy. At present, the only radical treatment of metastases is liver resection. Only 2.6% of patients survive 3 years if such treatment is not implemented. The aim of the study was to assess predictive factors of long-term survival in the group of patients with unresectable colorectal liver metastases carcinoma. Material and methods. Of 1029 patients with colorectal liver metastases, who were treated in the Department of General, Transplant and Liver Surgery of the Medical University of Warsaw in the years 2006-2012, cases of liver metastases assessed intraoperatively as unresectable were selected. The retrospective analysis included 85 patients. Based on the medical documentation, information concerning age, sex, characteristics of primary and secondary tumours, reasons for unresectability, neoadjuvant chemotherapy as well as local treatment of liver tumours was collected. Preoperative serum concentrations of CEA and CA 19-9 markers were considered. The Cox regression model, Kaplan- Meier estimator and log-rank test were applied in the statistical analyses. Results. The most common reason for unresectability were: number of metastases in 31 patients (36.5%) and extrahepatic metastases in 19 cases (22.4%). Overall survival in the entire group was 56.1% and 15.5% after 1 and 3 years respectively. A single-factor analysis showed that CEA serum levels (p=0.032; HR=1.002 per increase by 1 ng/ml) and the presence of extrahepatic metastases (p=0.037; HR=2.06) were predictors of worse survival. In a multivariate analysis, CEA concentration (p=0.017; HR=1.002 per increase by 1 ng/ml) was an independent predictor of death whereas the presence of extrahepatic metastases were not statistically significant (p=0.059; HR=2.09). Conclusions. Serum concentration of CEA marker is an independent predictor of worse survival, but the presence of extrahepatic metastases shows a similar tendency

Wstęp. Życie pacjenta z niewydolnością wątroby w zaawansowanym stadium można uratować jedynie dzięki transplantacji. Istnieje jednak wiele przyczyn, z powodu których pacjenci mogą zostać do niej niezakwalifikowani. Opieka nad pacjentem, niezakwalifikowanym do transplantacji, wymaga szczególnego zaangażowania pielęgniarki we wszystkie biopsychospołeczne sfery życia pacjenta. Cel pracy. Celem pracy była identyfikacja problemów pielęgnacyjnych, określenie deficytu samoopieki oraz zaplanowanie opieki pielęgniarskiej nad pacjentem z niewydolnością wątroby przeszczepionej, niezakwalifikowanego do retransplantacji, przebywającego w szpitalu na oddziale transplantologii. Opis przypadku. Zaplanowano opiekę nad chorym z niewydolnością wątroby przeszczepionej w przebiegu przewlekłego odrzucania, chorującym także na przewlekłą chorobą nerek o niejasnej etiologii, u którego występują również zaburzenia świadomości, agresja, obniżony nastrój i strach przed śmiercią. W planowaniu opieki zastosowano Międzynarodową Klasyfikację Praktyki Pielęgniarskiej ICNP®. Dyskusja. Plan opieki został opracowany według teorii Virginii Henderson. Pacjent wymagał pomocy w czynnościach dnia codziennego oraz wsparcia psychicznego, szczególnie pomocy w zrozumieniu jego obecnej sytuacji i radzeniu sobie z uczuciem samotności. Wnioski. Zastosowanie Międzynarodowej Klasyfikacji Praktyki Pielęgniarskiej ICNP® w planowaniu opieki pielęgniarskiej nad pacjentem z niewydolnością wątroby przeszczepionej pozwala na polepszenie jakości opieki na pacjentem i umożliwiona analizę wiedzy o opiece nad pacjentem w kontekście międzynarodowym.
Introduction. Life of patient suffering from an advanced-stage liver failure can be saved only by transplantation surgery. Many patients, however, do not qualify for this type of treatment for a variety of reasons. Caring for a patient who was disqualified from the surgery is challenging and requires focusing on all biopsychosocial aspects of patient’s life. Aim of the study. The aims of this study were to identify nursing problems, to estimate the self-care deficits, and to plan nursing care for a patient with a liver transplant failure currently hospitalized in a transplantology unit. Case study. Nursing care of patient with a liver graft failure in the course of chronic graft rejection was carefully planned. The patient was also diagnosed with a chronic kidney disease of unknown aetiology. Moreover, the patient suffered from a fluctuating consciousness and mood depression, manifest aggressive behaviours and the fear of death. To plan patient’s care the International Classification for Nursing Practice ICNP® was used. Discussion. Nursing care was planned in accordance with Virginia Henderson’s theory which emphasizes the Maslow hierarchy of needs Patient required help with daily activities and needed psychological support, especially in coping with his current situation and dealing with the feeling of loneliness. Conclusions. Applying the International Classification for Nursing Practice ICNP® in the planning of nursing care for a patient with liver graft failure enables the improvement of the patient's quality of life and the awareness of nursing care in the international area.

Opis:
Non-invasive methods for liver stiffness assessment have been introduced over recent years. Of these, two main methods for estimating liver fibrosis using ultrasound elastography have become established in clinical practice: shear wave elastography and quasi-static or strain elastography. Shear waves are waves with a motion perpendicular (lateral) to the direction of the generating force. Shear waves travel relatively slowly (between 1 and 10 m/s). The stiffness of the liver tissue can be assessed based on shear wave velocity (the stiffness increases with the speed). The European Federation of Societies for Ultrasound in Medicine and Biology has published Guidelines and Recommendations that describe these technologies and provide recommendations for their clinical use. Most of the data available to date has been published using the Fibroscan (Echosens, France), point shear wave speed measurement using an acoustic radiation force impulse (Siemens, Germany) and 2D shear wave elastography using the Aixplorer (SuperSonic Imagine, France). More recently, also other manufacturers have introduced shear wave elastography technology into the market. A comparison of data obtained using different techniques for shear wave propagation and velocity measurement is of key interest for future studies, recommendations and guidelines. Here, we present a recently introduced shear wave elastography technology from Hitachi and discuss its reproducibility and comparability to the already established technologies.
W ostatnich latach pojawiły się nowe nieinwazyjne metody oceny sztywności wątroby. Dwie główne spośród tych metod znalazły zastosowanie w praktyce klinicznej do elastosonograficznej oceny włóknienia wątroby: elastografia fali poprzecznej oraz elastografia quasi-statyczna/ odkształceń względnych. Fale poprzeczne to fale o ruchu prostopadłym (lateralnym) do kierunku siły wzbudzającej. Fale poprzeczne przemieszczają się stosunkowo wolno (od 1 do 10 m/s). Sztywność tkanki wątroby można oceniać na podstawie szybkości fali poprzecznej (sztywność wzrasta proporcjonalnie do szybkości). Europejska Federacja Towarzystw Ultrasonograficznych w Medycynie i Biologii (The European Federation of Societies for Ultrasound in Medicine and Biology) wydała wytyczne zawierające opis tych technologii oraz zalecenia dotyczące ich klinicznego stosowania. Dotychczas większość dostępnych danych uzyskiwano z wykorzystaniem aparatury FibroScan (Echosens, Francja), punktowego pomiaru szybkości fali poprzecznej przy użyciu silnego impulsu akustycznego (Siemens, Niemcy) oraz elastografii fali poprzecznej z zastosowaniem aparatu Aixplorer (SuperSonic Imagine, Francja). W ostatnim czasie również inni producenci wprowadzili na rynek technologię elastografii fali poprzecznej. Porównanie danych uzyskanych z zastosowaniem różnych nietechnik propagacji fali poprzecznej oraz pomiaru jej szybkości ma kluczowe znaczenie dla przyszłych badań, zaleceń oraz wytycznych. W niniejszej pracy przedstawiono niedawno wprowadzoną technologię elastografii fali poprzecznej (Hitachi) oraz omówiono jej powtarzalność i porównywalność względem dotychczas stosowanych technologii.

The enzymes aldehyde oxidase and xanthine oxidase catalyze the oxidation of a wide range of N-heterocycles and aldehydes. These enzymes are widely known for their role in the metabolism of N-heterocyclic xenobiotics where they provide a protective barrier by aiding in the detoxification of ingested nitrogen-containing heterocycles. Isovanillin has been shown to inhibit the metabolism of aromatic aldehydes by aldehyde oxidase, but its inhibition towards the heterocyclic compounds has not been studied. The present investigation examines the oxidation of phthalazine in the absence and in the presence of the inhibitor isovanillin by partially purified aldehyde oxidase from guinea pig liver. In addition, the interaction of phthalazine with freshly prepared guinea pig liver slices, both in the absence and presence of specific inhibitors of several liver oxidizing enzymes, was investigated. Aldehyde oxidase rapidly converted phthalazine into 1-phthalazinone, which was completely inhibited in the presence of isovanillin (a specific inhibitor of aldehyde oxidase). In freshly prepared liver slices, phthalazine was also rapidly converted to 1-phthalazinone. The formation of 1-phthalazinone was completely inhibited by isovanillin, whereas disulfiram (a specific inhibitor of aldehyde dehydrogenase) only inhibited 1-phthalazinone formation by 24% and allopurinol (a specific inhibitor of xanthine oxidase) had little effect. Therefore, isovanillin has been proved as an inhibitor of the metabolism of heterocyclic substrates, such as phthalazine, by guinea pig liver aldehyde oxidase, since it had not been tested before. Thus it would appear from the inhibitor results that aldehyde oxidase is the predominant enzyme in the oxidation of phthalazine to 1-phthalazinone in freshly prepared guinea pig liver slices, whereas xanthine oxidase only contributes to a small extent and aldehyde dehydrogenase does not take any part.

Elevated plasma homocysteine is a risk factor for cardiovascular disease and Alzheimer's disease. To understand the factors that determine the plasma homocysteine level it is necessary to appreciate the processes that produce homocysteine and those that remove it. Homocysteine is produced as a result of methylation reactions. Of the many methyltransferases, two are, normally, of the greatest quantitative importance. These are guanidinoacetate methyltransferase (that produces creatine) and phosphatidylethanolamine N-methyltransferase (that produces phosphatidylcholine). In addition, methylation of DOPA in patients with Parkinson's disease leads to increased homocysteine production. Homocysteine is removed either by its irreversible conversion to cysteine (transsulfuration) or by remethylation to methionine. There are two separate remethylation reactions, catalyzed by betaine:homocysteine methyltransferase and methionine synthase, respectively. The reactions that remove homocysteine are very sensitive to B vitamin status as both the transsulfuration enzymes contain pyridoxal phosphate, while methionine synthase contains cobalamin and receives its methyl group from the folic acid one-carbon pool. There are also important genetic influences on homocysteine metabolism.

The cellular localization of the 35 kDa, low molecular mass acid metallophosphatase (LMW AcPase) from the frog (Rana esculenta) liver and its activity towards P-Ser and P-Tyr phosphorylated peptides were studied. This enzyme was localized to the cytoplasm of hepatocytes but did not appear in other cells of liver tissue (endothelium, macrophages, blood cells). This LMW AcPase does not display activity towards 32P-phosphorylase a under conditions standard for the enzymes of PPP family. Proteins containing P-Ser: rabbit 32P-phosphorylase a and phosvitin are hydrolysed only at acidic pH and are poor substrates for this enzyme. The frog AcPase is not inhibited by okadaic acid and F- ions, the Ser/Thr protein phosphatase inhibitors. Moreover, the frog enzyme does not cross-react with specific antisera directed against N-terminal fragment of human PP2A and C-terminal conserved fragment of the eukaryotic PP2A catalytic subunits. These results exclude LMW AcPase from belonging to Ser/Thr protein phosphatases: PP1c or PP2Ac. In addition to P-Tyr, this enzyme hydrolyses efficiently at acidic pH P-Tyr phosphorylated peptides (hirudin and gastrin fragments). Km value for the hirudin fragment (7.55 ± 1.59 × 10-6 M) is 2-3 orders of magnitude lower in comparison with other substrates tested. The enzyme is inhibited competitively by typical inhibitors of protein tyrosine phosphatases (PTPases): sodium orthovanadate, molybdate and tungstate. These results may suggest that the LMW AcPase of frog liver can act as PTPase in vivo. A different cellular localization and different response to inhibition by tetrahedral oxyanions (molybdate, vanadate and tungstate) provide further evidence that LMW AcPase of frog liver is distinct from the mammalian tartrate-resistant acid phosphatases.

My surgical education began at a time when Poland formed part of the communist bloc and was isolated from the world, or in today’s Terms – it remained behind the Iron Curtain. This was true of all areas of life, including medicine. When after 40 years of work, I look back at my professional career; I wonder whether I owe my proficiency in surgery to my experience and dexterity or, like many others, to technological progress. Two of the great Polish surgeons were my mentors and teachers. Professor Zdzisław Łapiński was the one I met first. He was a manual genius and an unusual operational strategist. Granted, he had one character defect, but nobody’s perfect after all. In 1975, I defended my dissertation. I was convinced that I should continue my education at a center abroad, preferably within a postdoctoral scholarship. Professor Łapiński wanted me to learn everything about surgery from him. I decided otherwise, and in 1978 with his tacit agreement, I obtained a Humboldt Fellowship and went to Heidelberg, to the department headed by none other than Professor Fritz Linder.1 I started my research for the habilitation thesis at the Experimentelle Chirurgie Abteilung of his Department.

The association of focal nodular hyperplasia (FNH) and various neoplasms was described, but coincidence of multiple FNH and hepatic perivascular epithelioid cells tumor (PEComa) has not been reported. The clinical debate of oral contraceptive (OC) influence on FNH growth is ongoing, but no evidence exists about association of hepatic PEComa with OC use. Herein, we report a case of two FNH lesions and huge (150x100x80 mm) left hepatic lobe PEComa that occurred simultaneously in 18-year-old female with previous two year history of OC use, who underwent left hemihepatectomy and right hepatic FNH enucleation. Up to date, the patient has been followed-up for 65 months and remained disease-free. FNH and PEComa have a common vascular cytogenetic denominator. Our case raising a question of a causal relationship of FNH and PEComa with OC use that might be attributed to vascular changes. Future researches of larger sample sizes should further address this issue.

Arsenic is a prevalent and pervasive environmental contaminant with varied amounts in drinking water. Arsenic exposure causes cancer, cardiovascular, liver, nerve, and ophthalmic diseases. The current study aimed to find the best conditions for eliminating arsenic from simulated wastewater and their effect on biomarkers of hepatic in mice. Adsorption tests including pH, contact duration, Al-kheriat dosage, and arsenic concentrations were evaluated. Seventy-two healthy albino mice (male) were accidentally allocated into nine groups (n = 8), the first group was considered as healthy control, the second group (AL-Kheriat), and other groups received AL-Kheriat and arsenic 25, 50, 75, 100, 125, 150 and 175 mg/kg, respectively. Next 10 days, the following were examined: LD50 level, ALP (alkaline phosphatase), ALT (alanine aminotransferase), and AST (aspartate aminotransferase), besides the histological condition of the liver. The results showed that the best time for arsenic removal was 4 hours, pH 8, Alkheriat dose 1 gram, and 50 ppm of pollutants. The level of alkaline phosphatase ALP, alanine transaminase ALT, and aspartate transaminase AST was increased to 150.96 (U/L), 143.1(U/L), and 32.8(U/L), respectively, in Al-Khriet and arsenic exposed population than the healthy control group, When the appropriate dose of Al-Khriet and arsenic mixture is used, it can aid in the selection of a safe way of disposing of the adsorbed residue. Additionally, it can serve as a low-cost rodent pesticide, increasing the commercial viability of this removal strategy.

Celem niniejszej pracy było utworzenie algorytmu służącego do segmentacji wątroby w serii obrazów pochodzących z badania wykorzystującego rezonans magnetyczny. Rozwiązanie zostało zawarte w graficznym interfejsie użytkownika tworząc czytelny program. W algorytmie wykorzystano metodę aktywnego konturu, morfologię matematyczną oraz podobne przekształcenia. Wynikiem jest maska 3D oraz liczba wokseli w niej zawartych. Program umożliwia konfrontację wyników.
As a main aim of this project was made a liver segmentation algorithm from Magnetic Resonance Images. Whole solution was made in graphical user interface to make program simple and intuitive in use. In algorithm were used methods like: active contour (snake), mathematical morphology and similar transformations. As result is 3D mask of liver and sum of the voxels containing the mask. Application includes confrontation results.

During the early manifestations of traumatic disease as a result of cranioskeletal injury (CSI) the content of total bile acids in bile is significantly reduced, biliary excretion is slowed, lithogenic properties of bile are increased. Intraperitoneal injection of fetal nerve cells in 12 hours after causing an injury accompanied by improvement of biligenic and biliary excretion liver functions, reduces the lithogenic properties of bile.

Mutation of the SERPINA1 gene is present in about 2% of patients with cystic fibrosis but is more common and accounts for about 5% in patients with cystic fibrosis and co-existing liver lesions. The SERPINA1 gene is responsible for the synthesis of a serine protease inhibitor. The protein related with this gene is accumulated within the endoplasmic reticulum of hepatocytes causing their damage, inflammation and cirrhosis. The aim was to assess the presumable effect of the SERPINA1 mutation gene in patients with diagnosed cystic fibrosis on damage to the liver and/or cholestasis. Material and method: The analysis included 30 children, 13 girls (43.3%) and 17 boys (56.6%), aged from 6 months to 18 years (the average age was 5.5 years) with diagnosed cystic fibrosis. All the patients have undergone a genetic test of the mutation of the SERPINA1 gene. The analysis included age, sex, clinical symptoms, type of mutation of the CFTR protein, abnormalities in laboratory tests (the activity of aminotransferases, GGTP, alkaline phosphatase , protein, the indicator of acid steatocrit, the rate of APRI) and abdominal ultrasonography. Results: Symptoms of damaged liver were concluded in 9/30 patients (30%) with diagnosed cystic fibrosis. Most commonly observed were increased activities of aminotransferases in 9/30 patients (30%) and of gamma glutamyl transferase in 6/30 (20%) of the assessed patients. In 4/30 patients the abdominal ultrasonography revealed an enlarged liver and increased echogenicity. Mutation within the SERPINA1 gene was observed only in 1/30 patients (3.3%) with diagnosed cystic fibrosis. As far as the patient is concerned, currently the activities of aminotransferases, GGTP and AF are normal, but there has been a considerable increase in the intensity of symptoms from the respiratory system. No corelation between the mutation of the SERPINA1 gene and clinical symptoms, type of mutation of the CFTR protein, laboratory results of the functions and damage to the liver and the abdominal ultrasonography was observed. Conclusions: We did not find a more frequent occurrence of the SERPINA1 gene mutation in children with cystic fibrosis and coexisting features of damaged liver and cholestasis. The obtained results suggest the contribution of other than SERPINA1 gene mutation factors responsible for the development of changes in the liver in patients diagnosed with cystic fibrosis. The studies on the subject should be extended and performed on a larger group of patients.
Mutacja genu SERPINA1 występuje u około 2% pacjentów z mukowiscydozą , natomiast u około 5% pacjentów z mukowiscydozą i współistniejącymi zmianami wątrobowymi. Gen SERPINA1 odpowiedzialny jest za syntezę inhibitora proteazy serynowej, a białko związane z tym genem jest gromadzone w obrębie retikulum endoplazmatycznego hepatocytów prowadząc do ich uszkodzenia, zapalenia i marskości. Celem pracy było poszukiwanie wpływu mutacji genu SERPINA1 na wystąpienie uszkodzenia wątroby i/lub cholestazy u pacjentów z rozpoznaną mukowiscydozą. Materiał i metoda: Analizą objęto 30 dzieci, 13 dziewczynek (43,3%) oraz 17 chłopców (56,6%), w wieku od 6 miesięcy do 18 lat (średni wiek 5,5 lat) z rozpoznaną mukowiscydozą. U wszystkich pacjentów wykonano badanie genetyczne w kierunku mutacji genu SERPINA1. W analizie uwzględniono wiek, płeć, objawy kliniczne, rodzaj mutacji genu białka CFTR, nieprawidłowości w wynikach badania laboratoryjnych (aktywności aminotransferaz, GGTP, FA, białka, wskaźnik kwaśnego steatokrytu, współczynnik APRI) oraz badanie usg jamy brzusznej. Wyniki: Cechy uszkodzenia wątroby stwierdzono u 9/30 pacjentów (30%) z rozpoznaną mukowiscydozą. Najczęściej obserwowano podwyższone aktywności aminotransferaz – u 9/30 (30%), natomiast gammaglutamylotransferazy u 6/30 (20%) badanych pacjentów. U 4/30 pacjentów stwierdzono powiększoną wątrobę o podwyższonej echogeniczności w badaniu usg. Mutację w obrębie genu SERPINA1 wykazano jedynie u 1/30 (3,3%) pacjenta z rozpoznaną mukowiscydozą. U pacjentki tej aktualnie stwierdza się prawidłowe aktywności aminotransferaz, GGTP, FA, natomiast obserwuje się znaczne nasilenie objawów ze strony układu oddechowego. Nie wykazano korelacji pomiędzy występowaniem mutacji genu SERPINA1 a objawami klinicznymi, rodzajem mutacji białka CFTR, wynikami badań laboratoryjnych funkcji i uszkodzenia komórki wątrobowej oraz badaniem ultrasonograficznym jamy brzusznej. Wnioski: Nie wykazano częstszego występowania mutacji genu SERPINA1 u dzieci z rozpoznaną mukowiscydozą i współistniejącymi cechami uszkodzenia wątroby i cholestazy. Uzyskane dane sugerują udział innych, niż mutacje genu SERPINA1 czynników odpowiedzialnych za rozwój zmian obrębie wątroby u pacjentów z rozpoznaną mukowiscydozą. Wskazane byłoby przeprowadzenie badań na większej grupie pacjentów.

Homocysteine (Hcy), a product of methionine metabolism, is elevated by the consumption of a high-methionine diet that can cause fatty liver disease. Paraoxonase 1 (Pon1), a hydrolase expressed mainly in the liver and carried in the circulation on high-density lipoprotein, participates in Hcy metabolism. Low Pon1 activity is linked to fatty liver disease. We hypothesize that hyperhomocysteinemia and low Pon1 induce changes in gene expression that could impair liver homeostasis. To test this hypothesis, we analyzed the liver proteome of Pon1-/- and Pon1+/+ mice fed a high methionine diet (1% methionine in the drinking water) for 8 weeks using 2D IEF/SDS-PAGE gel electrophoresis and MALDI-TOF mass spectrometry. We identified seven liver proteins whose expression was significantly altered in Pon1-/- mice. In animals fed with a control diet, the expression of three liver proteins involved in lipoprotein metabolism (ApoE), iron metabolism (Ftl), and regulation of nitric oxide generation (Ddah1) was up-regulated by the Pon1-/- genotype. In mice fed with a high-methionine diet, expression of four liver proteins was up-regulated and of three proteins was down-regulated by the Pon1-/- genotype. The up-regulated proteins are involved in lipoprotein metabolism (ApoE), energy metabolism (Atp5h), oxidative stress response (Prdx2), and nitric oxide regulation (Ddah1). The down-regulated proteins are involved in energy metabolism (Gamt), iron metabolism (Ftl), and catechol metabolism (Comt). Expression of one protein (Ftl) was up-regulated both by the Pon1-/- genotype and a high-methionine diet. Our findings suggest that Pon1 interacts with diverse cellular processes - from lipoprotein metabolism, nitric oxide regulation, and energy metabolism to iron transport and antioxidant defenses - that are essential for normal liver homeostasis and modulation of these interactions by a high-methionine diet may contribute to fatty liver disease.

Liver is a unique mammalian organ with a great capacity of regeneration related to its function. After surgical resection or injury, hepatic cells, especially hepatocytes, can proliferate rapidly to repair the damage and to regenerate the structure without affecting the function of the liver. Loss of catalase activity during regeneration indicates that oxidative stress is present in the liver not only in pathological conditions but also as a 'physiological' factor during regeneration. As we have shown in our previous work, liver stem cell-like cells treated with 4-hydroxynonenal (HNE), a cytotoxic and growth regulating lipid peroxidation product, recover in the presence of spleen cells. In the current study we characterized this novel cell line as liver-derived progenitor/oval-like cells, (LDP/OCs), i.e. functional liver stem-like cells. We showed that LDP/OC were OV6 positive, with abundant glycogen content in the cytoplasm and expressed α-fetoprotein, albumin, biliverdin reductase and γ-glutamyl transferase. Also, we compared their growth in vitro with the growth of cultured primary hepatocytes stressed with HNE and co-cultured with autologous spleen cells. The influence of spleen cells on HNE-treated primary hepatocytes and on LDP/OCs showed that spleen cells support in a similar manner the recovery of both types of liver cells indicating their important role in regeneration. Hence, LDP/OC cells may provide a valuable tool to study cell interactions and the role on HNE in liver regeneration.

Opis:
Disturbances in the antioxidant system could play a role in pathogenesis of chronic liver disease. The aim of our study was to evaluate the levels/activities of antioxidants in blood of patients with chronic liver disease. We estimated selenium and glutathione concentrations and glutathione peroxidase activities in blood of 59 patients with chronic hepatitis B or C virus infection (group 1) and 64 patients with alcoholic, autoimmune or cryptogenic chronic liver disease (group 2). The results were compared with 50 healthy controls. Whole blood and plasma selenium and red cell glutathione concentrations were significantly lower in the patients compared with the controls. Red cell glutathione peroxidase activity was slightly reduced in both subgroups of group 1 and in group 2 with normal alanine aminotransferase values. Plasma glutathione peroxidase activity was slightly but significantly higher in patients with elevated aminotransferase values. The findings suggest that disturbances in antioxidant parameters in blood of patients with chronic liver disease may be the cause of the peroxidative damage of cells.

The basic fish-breeding and biotechnological and biological indicators of yearlings of scaly carp grown in recirculation installations using forage mixture of natural food objects were researched. It was established that the introduction of natural fodder for feeding carp in controlled conditions positively affect the growth of fish. Using histological studies it was determined that in terms of growing fish in closed water plants, feeding fish with natural feed ingredients have positive effect on the metabolism in the body of the fish.

Non-alcoholic fatty disease (NAFLD) is a liver disorder that affects up to 30% of the population, mainly in Western countries. It is estimated that up to 75% of NAFLD patients will develop a more aggressive form of the disease, non-alcoholic steatohepatitis (NASH). NAFLD can lead to fibrosis and liver failure; however, it is difficult to diagnose NAFLD due to its non-specific symptoms. Unfortunately, there is no treatment available for this disease. The risk factors of NAFLD are obesity and insulin resistance (IR). The molecular factors that seem to play an important role in the pathogenesis of NAFLD are oxidative stress as well as impaired DNA damage repair processes; a great body of evidence confirms an association with the base excision repair (BER) pathway. The activity of BER is decreased in patients with NAFLD and in animal models of this disease. In order to better understand the underlying basis of the disease, knowledge should be broadened in the area of DNA repair in NAFLD.

Introduction and objective. Lithium and selenium have been the subjects of extensive research for many years. Concern about the physiological action of these elements results from the application of lithium in medicine, as well as the beneficial influence of selenium supplementation in different pathological states. However, little data are available concerning their effect on the homeostasis of essential microelements in organs. Therefore, the aim of the current study was to evaluate the influence of lithium and/or selenium administration on homeostasis of iron, zinc, copper and manganese in the liver of rats. Materials and method. The experiment was carried on 24 male Wistar rats (4 groups of 6 animals) subjected to the treatment with: group I (control) – saline; group II – lithium carbonate at a dose of 2.7 mg Li/kg b.w.; group III – sodium selenite at a dose of 0.5 mg Se/kg b.w.; group IV – both lithium carbonate and sodium selenite at the doses mentioned above. Administration was performed for 6 weeks, once a day by stomach tube, in the form of water solutions. Results. The tissue content of zinc, iron and manganese showed no statistical differences among the studied groups. Zn was slightly decreased in group III vs. control and group II. Fe was insignificantly enhanced in groups II and IV vs. control. Mn in group IV was slightly increased vs. groups II and III. Cu in group III was significantly decreased vs. group IV and slightly depleted vs. groups I and II. Conclusion. With the exception of Cu in the Se-treated group, lithium and/or selenium did not affect the studied microelements’ liver homeostasis in a significant way. These results may contribute to the knowledge of essential microelements homeostasis in human organisms undergoing lithium therapy, and/or selenium supplementation.

Background and Aim: Fibrosis, which develops during the progression of liver damage, is the previous stage of cirrhosis. Carbon tetrachloride is one of the most commonly used hepatic toxins in experimental animal models of liver fibrosis. It was investigated the hepatoprotective effects of Tribulus terrestris, Ashwagandha and N-acetylcysteine in an experimental model of liver fibrosis induced by carbon tetrachloride in this study. Methods: Fifty Wistar rats were divided into five groups of 10 each, as follows: 1) control, 2) carbon tetrachloride, 3) carbon tetrachloride plus N-acetylcysteine, 4) carbon tetrachloride plus T. terrestris, and 5) carbon tetrachloride plus Ashwagandha group. At the end of 6 weeks, the rats were sacrificed, and serum and tissue samples were collected. Aspartate aminotransferase, alanine aminotransferase, gamma-glutamyl transpeptidase, malondialdehyde, NF-κB, collagen 1, nuclear factor erythroid-2-related factor 2, tumor necrosis factor-α were analyzed, and histopathological evaluation was performed. Results: There were no significant differences in serum aspartate aminotransferase and alanine aminotransferase levels of the N-acetylcysteine-treated group versus those in the T. terrestris- and Ashwagandha-treated groups (p>0.05). Liver malondialdehyde levels were lower in the N-acetylcysteine-, T. terrestris- and Ashwagandha-treated groups than in the carbon tetrachloride-administered group (p<0.001). There were differences between groups in NF-κB, collagen 1, nuclear factor erythroid-2-related factor 2 and tumor necrosis factor-α levels (p<0.05). Conclusions: In conclusion, T. terrestris, Ashwagandha and N-acetylcysteine had protective effects on the liver in this experimental fibrosis model. T.Terrestris was a little more effective than Ashwagandha in combating liver fibrosis.

Introduction: Haemangiomas are the most common benign tumours of the liver. Most of them are asymptomatic. Giant hepatic haemangioma is defined as size greater than 5 cm in diameter. The surgical treatment for giant hepatic haemangioma is not very well defined and reported. Here we analysed the treatment outcome of giant hepatic haemangiomas and redefined the nomenclature according to the size and proposing an algorithm for management of all hepatic haemangiomas. Material and Methods: Retrospective analyses of 6 giant hepatic haemangioma more than 10 cm in size treated by a single surgeon were included. The clinical characteristics, diagnosis, managements and outcomes were recorded. Review of literature was done for definitions, management strategies and outcome of giant haemangiomas and an algorithm was proposed. Results: Five patients were female and the mean age was 36.6 years (range 32 to 45). Pain abdomen was the most common symptoms (100%). Ultrasound abdomen was the initial diagnostic modality followed by contrast enhanced computed tomography. The mean size of haemangioma was 17.4cm (range 12cm to 32cm). Four patients had haemangioma in the left lobe and two had in the right lobe of liver. Formal hepatectomy was done in three patients and enucleation was done in three patients. The mean operating time was 3.66 hours (range 2.5 hours to 5 hours). The mean blood loss was 840ml (range 300ml to 1500ml). There was no surgical morbidity or mortality. On follow-up no haemangioma related complications are reported. Conclusions: Giant hepatic haemangioma is mostly symptomatic. Hepatectomy and enucleation can be done without significant morbidity and mortality. There is a need for redefining the nomenclature of giant hepatic haemangiomas according to the size.

The most serious complication after pancreatic surgical procedures is still a postoperative pancreatic fistula. In clinical practice there are various methods to prevent the formation of pancreatic fistula, but none of them is fully efficient. Recently, the role of grafting the round ligament of the liver on the pancreas is emphasized as a promising procedure which reduces the severity and shortens the healing time of postoperative pancreatic fistula. The aim of the study was to assess the impact of grafting a round ligament patch on the pancreatic stump or the area of the pancreatic anastomosis on the severity and healing of pancreatic fistula after surgical treatment of the pancreas (alternatively on prevention of pancreatic fistula formation). Material and methods. The retrospective study covered patients operated due to pancreatic tumors in the Department of General, Gastrointestinal and Oncologic Surgery of the WUM. Pancreatic fistula was diagnosed according to the definition developed by the ISGPS (International Study Group of Pancreatic Surgery). Results. 10 patients with pancreatic tumors of different location were operated. The round ligament was grafted on the pancreatic stump, the area of the pancreatic anastomosis or on the site of the local tumor removal. Pancreatic fistula developed in 9 patients, including grade A pancreatic fistula in 5 patients, grade B fistula in 3 patients, and grade C fistula in 1 patient. Distant complications occurred in one patient. None of the patients required a reoperation and no deaths were reported. The average hospital stay was 22.4 days. The hospital stay of patients with grade A fistula was shorter than in case of patients with grade B and C fistula. Conclusions. Grafting of the round ligament of the liver on the pancreatic stump did not prevent the development pancreatic fistula. Grade A pancreatic fistula developed most often. Grade C fistula developed in 1 patient and was complicated by intraabdominal abscesses and sepsis. Although the patient did not require a repeated surgery, but only a continuation of conservative treatment on an outpatient basis. Patients with grade B fistula required prolonged drainage and in the end were supervised by the surgical polyclinic.

Liver transplantation is a well-established treatment of patients with end-stage liver disease and selected liver tumors. Remarkable progress has been made over the last years concerning nearly all of its aspects. The aim of this study was to evaluate the evolution of long-term outcomes after liver transplantations performed in the Department of General, Transplant and Liver Surgery (Medical University of Warsaw). Material and methods. Data of 1500 liver transplantations performed between 1989 and 2014 were retrospectively analyzed. Transplantations were divided into 3 groups: group 1 including first 500 operations, group 2 including subsequent 500, and group 3 comprising the most recent 500. Five year overall and graft survival were set as outcome measures. Results. Increased number of transplantations performed at the site was associated with increased age of the recipients (p<0.001) and donors (p<0.001), increased rate of male recipients (p<0.001), and increased rate of piggyback operations (p<0.001), and decreased MELD (p<0.001), as well as decreased blood (p=0.006) and plasma (p<0.001) transfusions. Overall survival was 71.6% at 5 years in group 1, 74.5% at 5 years in group 2, and 85% at 2.9 years in group 3 (p=0.008). Improvement of overall survival was particularly observed for primary transplantations (p=0.004). Increased graft survival rates did not reach the level of significance (p=0.136). Conclusions. Long-term outcomes after liver transplantations performed in the Department of General, Transplant and Liver Surgery are comparable to those achieved in the largest transplant centers worldwide and are continuously improving despite increasing recipient age and wider utilization of organs procured from older donors.

Parvovirus of Aleutian disease causes mainly damage to kidneys, but immune complexes deposition and damage may occur also in other organs. In mink farms of Latvia the liver dystrophy or hepatic lipidosis of mink is widely distributed. The goal of this study was to examine probability of liver damage and regeneration of mink infected with Aleutian disease virus. Liver injury was assessed histologically. The mink liver demonstrated inflammation of liver parenchyma and foci of fatty liver. In immunohistochemistry, during liver regeneration the matrix metalloproteinases MMP-9, vascular endothelial growth factor and β-defensin 2 expressions were lower, but MMP-2 and nerve growth factor receptor p75 expression was increased.

Background. Apoptosis is involved in the pathogenesis of alcoholic liver cirrhosis. Its development can be triggered by an inflammatory process. In the present study, levels of apoptotic factors – survivin human protein p53 (HP 53) and IL-6, IL-18 were determined according to the stage of liver cirrhosis. Material and methods. Seventy patients with alcoholic liver cirrhosis, treated in various hospitals of the Lublin region, Poland were included in the study. Serum levels of IL-6, IL-18, HP53 and survivin were determined by the enzyme-linked immunosorbent assay (ELISA) technique. Results. The serum level of survivin in patients with alcoholic liver cirrhosis was not statistically different from that found in the control group. The level of HP53 was significantly higher in the group of patients with alcoholic liver cirrhosis compared to the control group (16.53±22.69 vs. 0.39±1.31 U/ml; p<0.001). Likewise, the level of IL-6 was significantly higher in the group of patients with alcoholic liver cirrhosis compared to the control group (33.83±41.78 vs. 0.88 ± 0.56 pg/ml; p<0.001). Moreover, the level of IL-18 was significantly higher in the group of patients with liver cirrhosis compared to the control group (23.96±31.07 vs. 5.3±8.6 pg/ml; p<0.001). Conclusion. In conclusion, increased serum levels of IL-6 and IL-18 were demonstrated in patients with alcoholic liver cirrhosis. Moreover, the liver cirrhosis patients had elevated levels of HP53, which is a marker of apoptosis. Our results did not demonstrate the correlation between the levels of apoptosis markers (survivin, HP53) and the levels of cytokines (IL-6, IL-18) in the blood serum.

The effect of different ripening stages of durian Durio zibethinus fruit on zinc content in liver of rats loaded with cholesterol. Durian Durio zibethinus is an exotic, climacteric fruit, rich in dietary fiber, polyphenols, saccharides, vitamins and minerals. Postharvest changes in durian fruit influence its physical and chemical quality, which determined its health protective effect – antioxidant and hypocholesterolemic. Durian is a good source of zinc. The aim of this study was to investigate the zinc content in lyophilisate of durian fruit and in the diets supplemented with durian fruits at different stages of maturity. The in vivo experiment was designed to determine the influence of these diets on zinc content in the liver of rats loaded with cholesterol. The zinc content in durian fruit, in the diets, and in rats livers was examined using a flame atomic absorption spectrophotometry (Perkin-Elemer 1100B). The zinc content in durian fruits ranged from 8.1 to 12.3 mg/kg. The zinc content amounted 45.0, 44.2, 47.1, 47.3 and 48.0 mg/kg in C, ch, chDM, ch DR and chDOR diet, respectively. The content of zinc in the liver in all animal groups was within the normal limits and amounted from 25.0 to 33.0 mg/kg fresh weight. Diet supplemented with durian fruits at different stages of ripening increases accumulation of zinc in the liver of rats loaded with cholesterol. Durian fruits, especially ripe ones, could be used as a natural supplementation of zinc in the diet and might be helpful in prevention of diseases or disorders related to zinc deficiency.
Wpływ różnych faz dojrzałości owocu durianu Durio zibethinus na zawartość cynku w wątrobie szczurów obciążonych cholesterolem. Durian Durio zibethinus to egzotyczny, klimakteryczny owoc, bogaty we włókno pokarmowe, polifenole, oligosacharydy, witaminy i minerały. Zmiany następujące po zbiorze owoców durianu wpływają na ich fizyczne i chemiczne parametry determinujące ich właściwości zdrowotne – przeciwutleniające i hypocholesterolemiczne. Durian jest cennym źródłem cynku. Celem badania było określenie zawartości cynku w owocach durianu, w dietach z dodatkiem durianu w różnych fazach jego dojrzałości. Badanie in vivo miało na celu określenie wpływu tych diet na zawartość cynku w wątrobie szczurów obciążonych cholesterolem. Zawartość cynku w owocach durianu, w dietach i wątrobach oznaczono metodą płomieniowej absorpcji atomowej (Perkin-Elmer 1100B). Zawartość cynku w liofilizacie owoców durianu mieściła się w zakresie 8,1–12,3 mg/kg. Zawartość cynku w dietach: C, ch, chDM, ch DR and chDOR wynosiła odpowiednio 45,0, 44,2, 47,1, 47,3 i 48,0 mg/kg. W wątrobie szczurów wszystkich grup zawartość cynku mieściła się w zakresie wartości referencyjnych i wynosiła od 25,0 do 33,0 mg/kg świeżej masy. Diety z dodatkiem owoców durianu w różnej fazie dojrzałości zwiększają akumulację cynku w wątrobie szczurów obciążonych cholesterolem. Owoce durianu, w szczególności dojrzałe, mogą być stosowane jako naturalny suplement cynku w diecie i być pomocne w prewencji chorób lub stanach związanych z niedoborem cynku.

Opis:
Pork liver is a by-product generated daily in large quantities due to the high number of pigs slaughtered and its relatively high weight in relation to the carcass weight. However, it is not widely accepted by consumers for direct consumption due to its organoleptic properties. The present work aims to provide information of interest that may be useful for obtaining protein extracts from pork liver with potential as techno-functional ingredients. Specifically, the obtained results indicate that pork liver has a high protein content, with collagen representing only a small protein fraction and being poor in non-protein nitrogenous compounds. In addition, from the solubility profile obtained in the pH range from 4.8 to 8.5, high extraction yields can be expected at alkaline pH. However, and despite the high microbiological quality of pork livers properly collected and processed, their pH, water content and nutritional richness make them highly susceptible to microbiological spoilage.
