Hypercholesterolemia is a common disorder in adult population, but total cholesterol concentrations beyond 1000 mg/dl occur rarely, and are found in patients with homozygous familial hypercholesterolemia and familial lecithin-cholesterol acyltransferase deficiency, in chronic graft-versus-host disease of the liver, after intravenous infusion of fat emulsion (intralipid), in newborn infants with immature liver function, and in obstructive biliary cholestasis. Cholestasis induces a dramatic increase in plasma cholesterol and the appearance of an abnormal lipoprotein, lipoprotein X (LpX), in the plasma. We report a case of severe hypercholesterolemia mediated by LpX in a patient transplanted for primary biliary cirrhosis (PBC), who was qualified for liver re-transplantation (re-LTx) due to chronic cholestasis. Four months after re-LTx, the cholesterol concentration was normal. The problems in diagnosis and treatment are discussed.

In this paper a novel kinetic analysis is used for evaluation of the low density lipoprotein (LDL) apoB fractional catabolic rate (FCR) after LDL apheresis which creates a nonsteady state. The formulae for calculation of FCR, and LDL rate of appearance (Ra) as functions of time have been derived. Calculations for one normal and one familial hypercholesterolemic (FH) subjects showed that FCR as well as Ra exhibited variations which can be attributed to the circadian rhythm in LDL metabolism. LDL apheresis had a little impact on both Ra and FCR in normal as well as in FH subjects. However, Ra in FH subject was found to be much higher than - that in the normal subject.

Objectives: The roles of cardiorespiratory fitness (CF) in reducing cardiovascular disease (CVD) and allcause mortality risks are well established; however, little is known about the role of CF in reducing risk of metabolic syndrome (MetS), a cluster of CVD risk factors, particularly in Asian countries. This research examined associations between CF and MetS diagnosis and its five components in Indonesian middle-aged and older adults. Methods: This cross-sectional study included 161 participants (aged 63±8 years; 70% female). CF was assessed with a 6-minute walk test. MetS diagnosis and its components were assessed with the Adult Treatment Panel III. Logistic regression modelling was conducted to examine the relationships between CF and MetS diagnosis and its components, after adjustment for BMI and other confounders. BMI was categorized using cut-points for Asians. Results: In adjusted models, participants with low CF had a greater likelihood of being diagnosed with MetS than those with high CF (OR=4.79, 95%CI:2.17-10.62). They were also more likely to have low highdensity lipoprotein (OR=2.07, 95%CI:1.02-4.18) or hypertriglyceridemia (OR=2.37, 95%CI:1.15-4.86). There was also borderline significant findings that suggested that participants with low CF had greater likelihood of having abdominal obesity (OR=2.34, 95%CI:0.97-5.65, p=0.06) or hyperglycaemia (OR=2.07 95%CI:0.98-4.41, p=0.06). Conclusions: Low CF is associated with increased likelihood of being diagnosed with MetS. The adverse effects of low CF are mainly characterized by dyslipidaemia. Public health messages should emphasise the importance of improving CF for preventing MetS. Assessment of CF could be useful for targeting individuals most likely to benefit from intervention to prevent MetS.

Opis:
Myeloperoxidase (MPO), an abundant enzyme in phagocytes, has been implicated in the pathogenesis of various inflammatory diseases including atherosclerosis. The major oxidant produced by MPO, hypochlorous acid (HOCl), is able to modify a great variety of biomolecules by chlorination and/or oxidation. In this paper the reactions of lipids (preferentially unsaturated fatty acids and cholesterol) with either reagent HOCl or HOCl generated by the MPO-hydrogen peroxide-chloride system are reviewed. One of the major issues has been whether the reaction of HOCl with lipids of low density lipoprotein (LDL) yields predominantly chlorohydrins or lipid hydroperoxides. Electrospray mass spectrometry provided direct evidence that chlorohydrins rather than peroxides are the major products of HOCl- or MPO-treated LDL phosphatidylcholines. Nevertheless lipid peroxidation is a possible alternative reaction of HOCl with polyunsaturated fatty acids if an additional radical source such as pre-formed lipid hydroperoxides is available. In phospholipids carrying a primary amino group such as phosphatidylethanolamine chloramines are the preferred products compared to chlorohydrins. Cholesterol can be converted by HOCl to great variety of oxysterols besides three isomers of chlorohydrins. For the situation in vivo it appears that the type of reaction occurring between HOCl and lipids would very much depend on the circumstances, e.g. the pH and the presence of radical initiators. The biological effects of lipid chlorohydrins are not yet well understood. It has been shown that chlorohydrins of both unsaturated fatty acids as well as of cholesterol may cause lysis of target cells, possibly by disruption of membrane structures.

Impairments in mitochondrial function have been proposed to play an important role in the pathogenesis of diabetes. Atherosclerotic coronary artery disease (CAD) is the leading cause of mortality in diabetic patients. Mitochondrial dysfunction and increased production of reactive oxygen species (ROS) are associated with diabetes and CAD. Elevated levels of glycated low density lipoproteins (glyLDL) and oxidized LDL (oxLDL) were detected in patients with diabetes. Our previous studies demonstrated that oxLDL and glyLDL increased the generation of ROS and altered the activities of antioxidant enzymes in vascular endothelial cells (EC). The present study examined the effects of glyLDL and oxLDL on mitochondrial respiration, membrane potential and the activities and proteins of key enzymes in mitochondrial electron transport chain (mETC) in cultured porcine aortic EC (PAEC). The results demonstrated that glyLDL or oxLDL significantly reduced oxygen consumption in Complex I, II/III and IV of mETC in PAEC compared to LDL or vehicle control using oxygraphy. Incubation with glyLDL or oxLDL significantly reduced mitochondrial membrane potential, the activities of mitochondrial ETC enzymes - NADH dehydrogenase (Complex I), succinate cytochrome c reductase (Complex II + III), ubiquinol cytochrome c reductase (Complex III), and cytochrome c oxidase (Complex IV) in PAEC compared to LDL or control. Treatment with oxLDL or glyLDL reduced the abundance of subunits of Complex I, ND1 and ND6 in PAEC. However, the effects of oxLDL on mitochondrial activity and proteins were not significantly different from glyLDL. The findings suggest that the glyLDL or oxLDL impairs mitochondrial respiration, as a result from the reduction of the abundance of several key enzymes in mitochondria of vascular EC, which potentially may lead to oxidative stress in vascular EC, and the development of diabetic vascular complications.

The purpose of the present study was to check the infl uence of rural and urban environments on some of the parameters of lipids metabolism in the cord blood serum in healthy newborns, as well as the evaluation of the concentration of lipids, lipoproteins and apolipoproteins in the cord blood serum with relation to risk factors of atherosclerosis in the family of the studied newborns. The study included 75 newborns (37 from rural areas and 38 from urban areas). Newborns weight and length were recorded at birth. On the basis of the family history taken from the mothers, the atherosclerosis risk factors were established in the families of the studied newborns. In all of the studied newborns, concentration of triglycerides, total cholesterol and LDL, VLDL and HDL cholesterol as well as of apolipoproteins (apo-AI, apo-B) in the cord blood serum sampled soon after birth were performed. No statistically signifi cant differences between the mean levels of triglycerides, total cholesterol and cholesterol of fractions: LDL, VLDL, HDL and apolipoproteins (apo-AI, apo-B) in the cord blood serum in the newborns from rural and urban areas were found. However, when analysing the concentration of parameters of lipid metabolism in cord blood serum in newborns with regard to gender, higher concentrations of total cholesterol, LDL cholesterol and apo-AI in female newborns from rural areas, and higher HDL cholesterol and apo-AI in female newborns from urban regions were confi rmed. When analysing the concentration of lipids and lipoproteins in cord blood serum in newborns from families with risk factors confi rmed as compared to the families without that risk, both in the rural and urban regions no signifi cant differences were confi rmed. The studies have not proved any signifi cant differences between the levels of lipids, lipoproteins and apolipoproteins in the cord blood serum in newborns from rural and urban areas.

Familial hypercholesterolemia (FH) is most commonly caused by mutations in the LDL receptor (LDLR), which is responsible for hepatic clearance of LDL from the blood circulation. We described a severely affected FH proband and their first-degree blood relatives; the proband was resistant to statin therapy and was managed on an LDL apheresis program. In order to find the causative genetic variant in this family, direct exon sequencing of the LDLR, APOB and PCSK9 genes was performed. We identified a compound heterozygous mutation in the proband with missense p.(W577C) and frameshift p.(G676Afs33) variants at exons 12 and 14 of the LDLR gene respectively. DNA sequencing of LDLR gene from the parents demonstrated that the missense variant was inherited from the mother and frameshift variant was inherited from the father. The frameshift variant resulted in a stop signal 33 codons downstream of the deletion, which most likely led to a truncated protein that lacks important functional domains, including the trans-membrane domain and the cytoplasmic tail domain. The missense variant is also predicted to be likely pathogenic and affect EGF-precursor homology domain of the LDLR protein. The segregation pattern of the variants was consistent with the lipid profile, suggesting a more severe FH phenotype when the variants are in the compound heterozygous state. The finding of a compound heterozygous mutation causing severe FH phenotype is important for the genotype-phenotype correlation and also enlarges the spectrum of FH-causative LDLR variants in the Arab population, including the Saudi population.

Wprowadzenie. Obecnie prowadzone badania wskazują na wzrost zgonów u osób z niskim stężeniem cholesterolu w surowicy, które spowodowane jest zmniejszaniem frakcji lipoprotein o wysokiej gęstości (HDL-C) i lipoprotein o niskiej gęstości (LDL-C). Cel. Celem pracy była ocena związku pomiędzy polimorfizmem TaqIB genu CETP, a stężeniami wybranych parametrów gospodarki lipidowej u pacjentów z nadwagą i otyłością. Materiał i metoda. Badaniami objęto 73 osoby w wieku 24-68 lat. Otyłość androidalną stwierdzono u 90,4% kobiet, w tym u 66,7% ze stężeniem HDL-C powyżej 50 mg/dL. Wyniki. Najwięcej z otyłością brzuszną było heterozygot B1B2 (38,1%). Otyłość brzuszna występowała u 48,4% mężczyzn, w tym u 25,8% heterozygot B1B2 oraz u 67,7% z HDL>40mg/dl. Stwierdzono, większe nieistotne statystycznie stężenie TG, TC, LDL-C, procentowej zawartości masy tkanki tłuszczowej (%FM) u pacjentów z genotypem B2B2 w porównaniu do osób z genotypem B1B1 i B1B2. U kobiet wyższe stężenie TC, LDL-C oraz HDL-C stwierdzono w przypadku obecności genotypu B1B2, natomiast większe stężenie wszystkich badanych frakcji lipidowych wykazali mężczyźni z genotypem B2B2, u których stwierdzono także największą zawartość %FM. Wnioski. Uzyskane wyniki wskazują, że u otyłych mężczyzn polimorfizm TagIB może być niezależnym czynnikiem wpływającym na stężenie HDL-cholesterolu.
Background. Ongoing research shows an increase in mortality in patients with low levels of serum cholesterol, which is caused by decreasing the fraction of high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein (LDL-C). Objective. The aim of our study was to assess the relationship between the CETP gene TaqI polymorphism and concentrations of selected parameters of lipid metabolism in patients with overweight and obesity. Materials and method. The study involved 73 people aged 24-68 years. Android obesity was found in 90.4% of women, including 66.7% in the HDL-C levels above 50 mg / dL. Results. Most of abdominal obesity was the B1B2 heterozygotes (38.1%). Abdominal obesity was present in 48.4% of men, including 25.8% in B1B2 heterozygotes and in 67.7% of HDL> 40mg/dl. It was found statistically insignificant higher levels of TG, TC, LDL-C, the percentage of fat mass (% FM)in patients with genotype B2B2 compared to those with genotype B1B1 and B1B2. In women, higher levels of TC, LDL-C and HDL-C were found in the presence of B1B2 genotype, whereas a higher concentration of all lipid fractions studied showed the men with the B2B2 genotype, who also had the highest % FM. Conclusions. The results indicate that in obese men TagIB polymorphism may be an independent factor influencing HDLcholesterol.
