It was presented short analysis of connection ISM Code on vessels on the number of incidents threats. Introduced into force ISM Code in 1998 and 2002 (contained in chapter 9 of SOLAS Convention) as obligatory on vessels have had the aim of decreasing the number of injuries, serious injuries, fatalities, other incidents threats and total loses during vessel operation in maritime shipping. The next aim vicariously was improving the maritime safety and prevention of marine pollution from ships. As a result of ship-owner and crew requirements, it should eliminate from shipping market the bad and poor management systems of charterers or ship operators and improper qualified crewmembers. The company and the ship shall comply with the requirements of ISM Code and the company shall have the Document of Compliance (referred to in regulation 4 of ISM Code). After about twenty years when ISM Code was in force some comparisons, conclusions and remarks were presented. It is observed the decreasing number of vessel total losses but other comparisons is not so clear due to different definitions of incident being obligatory in states of flag and different databases in the different Memoranda of Understanding. The influence of introducing ISM Code on maritime shipping is serious in good or bad matters (e.g. the increasing of bureaucracy).

This paper compares accuracy of indoor positioning systems using one of three selected ISM bands: 433, 868 or 2400 MHz. Positioning is based on Received Signal Strength Indication (RSSI), received by majority of ISM RF modules, including lowcost ones. Investigated environment is single, indoor space (e.g. office, hall) and personal use, thus 2-dimensional (2D) coordinate system is used. Obtained results, i.a. average positioning error, are compared with similar measurements taken at outdoor, open space environment. The system is local, i.e. its operational area is limited by range of used RF modules – typical a few tens of meters. The main focus is research of how much accuracy (and usefulness) can be expected from standard RF modules working at typical ISM frequencies.

The article analyses eleven selected barriers to the implementation of the process approach in Polish organizations. Based on a survey, analysis has been carried out and the ISM model was created. The results of the research show that the underlying barriers are: the employee's resistance to change and the lack of financial resources. Studies show that effective implementation of the system based on the process approach requires change in the mentality of Polish employees. Adequate training should be introduced and implemented, and the involvement of management should be increased. Of less importance is the search for new funds.

Po tragedii statku Herald of Free Enterprise pod koniec lat 80-tych Międzynarodowa Organizacja Morska (International Maritime Organization – IMO) postanowiła uregulować i ujednolicić kwestie zarządzania bezpieczeństwem na statkach i wprowadziła Międzynarodowy Kodeks Zarządzania Bezpieczeństwem (International Safety Management Code - ISM). Narzuca on na armatorów posiadających statki powyżej 500 GT obowiązek prowadzenia Systemu Zarządzania Bezpieczeństwem (ang. Safety Management System – SMS), który reguluje praktyczne zastosowania Kodeksu ISM u armatora. Stanowi on zbiór wytycznych, procedur i zasad, które należy stosować w celu zachowania bezpieczeństwa ludzi, mienia i środowiska, które weryfikowane są następnie poprzez regularne raportowanie i zgłaszania wykonanych podczas pracy zadań do firmy. Największy obowiązek stosowania się do Systemu Zarządzania Bezpieczeństwem spoczywa na załogach. Nie tylko na osobach pracujących na poziomie zarządzania, ale obowiązek jakim jest „raportowanie każdego zdarzenia, każdego pojawiającego się niebezpieczeństwa czy sytuacji niezgodnej z prowadzoną przez Firmę polityką Systemu Zarządzania jakością i Bezpieczeństwem” spoczywa również na najniższych rangą, natomiast kadra zarządzająca zobowiązana jest następnie do przedłożenia informacji Firmie. W artykule przeprowadzono analizę raportów uszkodzeń technicznych zbieranych przez okres 5 lat na sześciu statkach w celu weryfikacji jakości prowadzonych w związku z Systemem Zarządzania Bezpieczeństwem zapisów. W ciągu pięciu lat zebrano ich 217, co na rok daje liczbę 44, co odpowiada dokonywaniu zgłoszenia przez statek średnio raz na dwa miesiące. Przeanalizowane raporty pozyskano ze statków eksploatowanych kilkanaście lat, co może prowadzić do wniosku, że zgłaszanie uszkodzeń występowało bardzo rzadko. W artykule podjęto próbę wyjaśnienia takiego stanu rzeczy oraz znalezienia odpowiedzi na pytanie jak wpływa on na bezpieczną eksploatację statku biorąc pod uwagę także pozostałe procedury i dokumenty wymagane przez Kodeks ISM oraz wynikające z nich obowiązki załóg.

Improvement of maritime safety has previously been based on a reactive regulatory approach, where regulatory improvements have been imposed to prevent recurrence of a specific type of accidental event or accidental scenario, after such an accidental event has happened. The ISM Code requires that hazardous situations are to be reported to the company, investigated and analyzed in order to prevent future happenings. Near-miss reporting is positively evaluated in this respect, because, near-misses are represented experiences and mistakes that should be shared to learn from in order to prevent the occurrence of accidents. The expression “that was too close” on ships’ bridges between the master and the officers is rarely transferred to a near miss report form, preserving the probability of reoccurrence. Near misses occurred and near misses reported might present a big difference in number. Officers easily forget the near miss situation when the safety of the ship is restored. Hazards identification will be based on documented management system (SMS- TMSA- ISO). The analysis of the documented safety and quality management will address the gap in order to improve the implemented systems. The objective of this research is to find out the best practices about near-miss reporting from the companies considered to have high level of commitment to safety within their organization. The study is based on interviews with a total of 35 seafarers who are joining on Greek ocean going vessels, and 4 representatives from safety departments of Greek maritime companies. The research also aims to address the seafarers’ perspective of reporting all near misses which have been experienced while they were in charge of specific duties (bridge watch, engine room watch) or any other operation (mooring, maintenance, drill) carried out on board. The majority of both the seafarers and the companies’ representatives believe that prior to the near miss reporting issue a safety culture environment on board is the real gain pursuit. In their perspective this is first priority to improve the general safety on board. It seems that near miss reporting is carried out on board as a compulsory compliance to the regulatory framework (ISM implementation). Further, it seems that the companies are not yet utilizing the reported data to improve feedback and the follow-up within the organization. The authors would like to thank companies’ representatives and seafarers who have participated in this study.

This paper proposes a transmit/receive (T/R) nanoswitch in 130 nm CMOS technology for 2.4 GHz ISM band transceivers. It exhibits 1.03-dB insertion loss, 27.57-dB isolation and a power handling capacity (P1 dB) of 36.2-dBm. It dissipates only 6.87 μW power for 1.8/0 V control voltages and is capable of switching in 416.61 ps. Besides insertion loss and isolation of the nanoswitch is found to vary by 0.1 dB and 0.9 dB, respectively for a temperature change of 125°C. Only the transistor W/L optimization and resistive body floating technique is used for such lucrative performances. Besides absence of bulky inductors and capacitors in the schematic circuit help to attain the smallest chip area of 0.0071 mm2 which is the lowest ever reported in this frequency band. Therefore, simplicity and low chip area of the circuit trim down the cost of fabrication without compromising the performance issue.

Over the past generation, the ISM code has brought forth tremendous opportunities to investigate and enhance the human factor in shipping through the implementation of Safety Management Systems. One of the critical factors to this implementation has been mandatory compliance and a requirement for obtaining a Document of Compliance (DOC) for vessels operating globally or at least internationally. A primary objective of these systems is to maintain them as “living” or “dynamic” systems that are always evolving. As the ISM code has evolved, there have been instances where large organizations have opted to maintain a voluntary DOC from their respective class society. This has been accomplished with a large human factor element as typically an organizational culture does not always accept change readily especially if there is not a legal requirement to do so. In other words, when considering maritime training is it possible that organizations may represent cultural challenges? The intent of this paper will be to research large maritime operations that have opted for a document of compliance voluntarily and compare them to similar organizations that have been mandated by international law to do the same. The result should be to gain insight into the human factors that must contribute to a culture change in the organization for the purposes of a legal requirement versus the human factors that contribute to a voluntary establishment of a safety management system. This analysis will include both the executive decision making that designs a system implementation and the operational sector that must execute its implementation. All success and failures of education and training can be determined by the outcome. Did the training achieve its goal? Or has the education prepared the students to embrace a new idea in conjunction with a company goal or a new regulatory scheme? In qualifying the goal of a successful ISM integration by examining both mandatory and voluntary ISM implementation in large maritime operations, specifically ferry systems, hopefully we can learn from the various factors that have gone into each.

The aim of this paper is to assess whether audit may be recognised as the evaluation tool for management systems in terms of their functioning and ensuring constant improvement. The paper content refers to the role of both internal and external audit. It also includes a characteristic of an internal auditor as a person directly responsible for efficient and effective audit. A correlation between the management system and the management process based on the guidelines is a subject of the analysis, too. The paper includes also the presentation of the safety management system and ship energy efficiency management systems the implementation of which is obligatory at ships. However, they differ in the methodology of the efficiency assessment. The author analysed the data previously collected and the information obtained during interviews with individuals to whom were delegated the duties in the scope of the both management schemes. The possibility of the using audit as a tool to improve the energy efficiency of ships is also considered hereunder. A proposed solution that may be applied to develop detailed standards in terms of the ship energy efficiency management process is also included.

Celem tego kodeksu jest zapewnienie na jak najwyższym poziomie bezpieczeństwa na morzu, zapobieganie nieszczęśliwym wypadkom, utracie życia i mienia, degradacji środowiska w tym środowiska morskiego, a tym samym dążenie do zminimalizowania błędów ludzkich i organizacyjnych (Human and Organisational Errors), które stanowią kilkadziesiąt procent wszystkich wypadków morskich.
The paper deals with the issue of analysis of Safety Management System supported on International Management Code for the Safe operation of ships and for Pollution Prevention and on Revised guidelines on implementation of The International Safety Management (ISM) Code by Administrations Res.A.1022(26), mandatory standard of the Safety Management System on the example of "Baltic Beta" IMO Nr 8756588 and "Petrobaltic" IMO Nr 8753940 Mobile Offshore Drilling Units. The objectives of the Code is to ensure safety at sea, prevention of human accidents, loss of life and investment goods, and avoidance of deterioration of the natural environment, in particular, to the marine environment, it means mostly in practice to minimization of Human and Organizational Errors - (HOE), which are the reason of much percentage of marine accidents.

The article tackles the issue of amendments to Polish maritime safety law which has been amended a number of times since it came to life several years ago. The last amendments of 2015 have been the most profound and complex. Today, maritime safety is given much more importance than several dozen years ago. The public and private aspects of maritime safety became the focus of attention rather recently but are now a branch of law that is developing the most dynamically. As has been shown in the paper, although private maritime law was already known in the Ancient Times, public-private law regulations were adopted only after the sinking of the Titanic in 1912. Still, it is not the private but the public aspects of maritime safety law which constitutes the major challenge. The abundance of international and European legislation ought to prompt the Polish legislator to consider a possible adoption of one comprehensive maritime safety act which would approach the whole matter in question in a systematic manner. The deliberations presented in the paper sum up the amendments to maritime safety law made so far. They are also an attempt to analyse the new legal order more deeply, and propose some more general de lege ferenda conclusions. The analysis is set in a wider context, taking into account the amendments introduced to international and EU regulations.
