The most important global problem being solved by the whole world community nowadays is to provide sustainable mankind development. Recent research in the field of sustainable development states that civilization safety is impossible without transfer sustainable development. At the same time, sustainable development (i.e. preservation of human culture and biosphere) is impossible as a system that serves to meet economical, cultural, scientific, recreational and other human needs without safety. Safety plays an important role in sustainable development goals achievement. An essential condition of effective navigation functioning is to provide its safety. The “prescriptive” approach to the navigation safety, which is currently used in the world maritime field, is based on long-term experience and ship accidents investigation results. Thus this approach acted as an the great fact in reduction of number of accidents at sea. Having adopted the International Safety Management Code all the activities connected with navigation safety problems solution were transferred to the higher qualitative level. Search and development of new approaches and methods of ship accidents prevention during their operation have obtained greater importance. However, the maritime safety concept (i.e. the different points on ways, means and methods that should be used to achieve this goal) hasn't been formed and described yet. The article contains a brief review of the main provisions of Navigation Safety Conceptions, which contribute to the number of accidents and incidents at sea reduction.

The maritime industry plays a crucial role in the global economy, with roughly 90% of world trade being conducted through the use of merchant ships and more than a million seafarers. Despite recent efforts to improve reliability and ship structure, the heavy dependence on human performance has led to a high number of casualties in the industry. Decision errors are the primary cause of maritime accidents, with factors such as lack of situational awareness and attention deficit contributing to these errors. To address this issue, the study proposes an Ant Colony Optimization (ACO) based algorithm to design and validate a verified set of instructions for performing each daily operational task in a standardised manner. This AI-based approach can optimise the path for complex tasks, provide clear and sequential instructions, improve efficiency, and reduce the likelihood of human error by minimising personal preference and false assumptions. The proposed solution can be transformed into a globally accessible, standardised instructions manual, which can significantly contribute to minimising human error during daily operational tasks on ships.

The priority of the maritime transport operation is a high level of security, which must be constantly monitored by shipowners with a security management system. The origin of the International Code for the Management of Safe Ship Operations and Pollution Prevention and the revised guidelines for the administration of the ISM Code by administrations, the first formal mandatory standard for safety management and pollution prevention goes back to the early 1980s. Arrangement of safe ship management and operation and consequent minimization of errors human being by far the most common cause of marine disasters is its main. The shipowner's and ship's certification procedure for the Safety Management System (SMS) is in line with Regulation (EC) No 336/2006 [1] of the European Parliament and the Council of Europe of 15 February 2006 on the implementation of the ISM Code in the Community, together with the Commission's amendment European Union of June 16, 2008. The documentation contains a developed Safety Management System adapted to the conditions of a given shipowner. All elements of the system must be documented, and, above all, the duties must be described in the organizational structures. Documentation must indicate how management supervises activities at various levels, employee participation and individual management representatives in individual processes, and how the safety management system is constantly improved. The shipowner maintains a security management system in line with the requirements of the Code and complies with the requirements of the certification system. The amount of information and the speed of changes taking place in the environment makes it necessary to introduce a system of information management and channels of their flow not only within the organization, but also in communication with third parties. Proper management of information resources increases the efficiency and effectiveness of the system. In the publication, the possibility of a formalized description of information flow in a well-functioning shipowner's safety management system.

Over the past generation, the ISM code has brought forth tremendous opportunities to investigate and enhance the human factor in shipping through the implementation of Safety Management Systems. One of the critical factors to this implementation has been mandatory compliance and a requirement for obtaining a Document of Compliance (DOC) for vessels operating globally or at least internationally. A primary objective of these systems is to maintain them as “living” or “dynamic” systems that are always evolving. As the ISM code has evolved, there have been instances where large organizations have opted to maintain a voluntary DOC from their respective class society. This has been accomplished with a large human factor element as typically an organizational culture does not always accept change readily especially if there is not a legal requirement to do so. In other words, when considering maritime training is it possible that organizations may represent cultural challenges? The intent of this paper will be to research large maritime operations that have opted for a document of compliance voluntarily and compare them to similar organizations that have been mandated by international law to do the same. The result should be to gain insight into the human factors that must contribute to a culture change in the organization for the purposes of a legal requirement versus the human factors that contribute to a voluntary establishment of a safety management system. This analysis will include both the executive decision making that designs a system implementation and the operational sector that must execute its implementation. All success and failures of education and training can be determined by the outcome. Did the training achieve its goal? Or has the education prepared the students to embrace a new idea in conjunction with a company goal or a new regulatory scheme? In qualifying the goal of a successful ISM integration by examining both mandatory and voluntary ISM implementation in large maritime operations, specifically ferry systems, hopefully we can learn from the various factors that have gone into each.

A comprehensive assessment of watchkeeping officers should be considered as a preventive measure in terms of safety of navigation. Watchkeeping officers are assigned after passing exams of state authorities and various evaluation procedures of companies. However, they are not subjected to comprehensive evaluations with practical implementations. Therefore, this study aims to develop effective use of simulators in terms of comprehensive evaluation of watchkeeping officers. In this study, simulation was designed with Goal-Directed Task Analysis (GDTA) technique to indicate important points on the situations watchkeeping officers face in navigation. 3 parameters were determined for difficulty adjustment; visibility, traffic density, geography. At the last phase of this study, performance evaluation method was prepared for performance evaluation of watchkeeping officer and scenario was evaluated for comprehensiveness by oceangoing masters. Hereby, the developed performance evaluation method for navigation can be used to generate more reliable method to evaluate the officers’ competence of technical skills.

Improvement of maritime safety has previously been based on a reactive regulatory approach, where regulatory improvements have been imposed to prevent recurrence of a specific type of accidental event or accidental scenario, after such an accidental event has happened. The ISM Code requires that hazardous situations are to be reported to the company, investigated and analyzed in order to prevent future happenings. Near-miss reporting is positively evaluated in this respect, because, near-misses are represented experiences and mistakes that should be shared to learn from in order to prevent the occurrence of accidents. The expression “that was too close” on ships’ bridges between the master and the officers is rarely transferred to a near miss report form, preserving the probability of reoccurrence. Near misses occurred and near misses reported might present a big difference in number. Officers easily forget the near miss situation when the safety of the ship is restored. Hazards identification will be based on documented management system (SMS- TMSA- ISO). The analysis of the documented safety and quality management will address the gap in order to improve the implemented systems. The objective of this research is to find out the best practices about near-miss reporting from the companies considered to have high level of commitment to safety within their organization. The study is based on interviews with a total of 35 seafarers who are joining on Greek ocean going vessels, and 4 representatives from safety departments of Greek maritime companies. The research also aims to address the seafarers’ perspective of reporting all near misses which have been experienced while they were in charge of specific duties (bridge watch, engine room watch) or any other operation (mooring, maintenance, drill) carried out on board. The majority of both the seafarers and the companies’ representatives believe that prior to the near miss reporting issue a safety culture environment on board is the real gain pursuit. In their perspective this is first priority to improve the general safety on board. It seems that near miss reporting is carried out on board as a compulsory compliance to the regulatory framework (ISM implementation). Further, it seems that the companies are not yet utilizing the reported data to improve feedback and the follow-up within the organization. The authors would like to thank companies’ representatives and seafarers who have participated in this study.

Bezpośrednią przyczyną opracowania Międzynarodowego kodeksu zarządzania bezpieczną eksploatacją statków i zapobieganiem zanieczyszczaniu (Kodeks ISM) była seria tragicznych wypadków morskich u schyłku XX wieku spowodowanych błędami człowieka. Kodeks ten wprowadził do zbioru międzynarodowych mechanizmów prawnych, mających zapewnić bezpieczeństwo podczas eksploatacji statku, zupełnie nowy instrument w postaci systemu zarządzania bezpieczeństwem (SMS — safety management system). Właściwie zastosowany i wdrożony przez armatora SMS, w jego przedsiębiorstwie żeglugowym, może być walorem, który nie tylko wpłynie na wymierne podniesienie poziomu bezpieczeństwa, lecz także będzie procentował optymalizacją kosztów i wzrostem renomy przedsiębiorstwa. Jednak, jeśli SMS zostanie stworzony bez zaangażowania i przekonania ze strony kadry zarządzającej, będzie jedynie pustą i fasadową, biurokratyczną procedurą, która nie tylko nie spełni swojego celu, ale będzie też jedynie dodatkowym obciążeniem dla całego personelu. W niniejszym artykule przedstawiono specyfikę SMS i jego wpływ na szeroko rozumiane bezpieczeństwo w przedsiębiorstwie prowadzonym przez armatora, a w tym bezpieczeństwo statków w jego flocie, oraz podkreślony został warunek skuteczności procedur SMS określonych w każdym shipboard manual (manualu statkowym) dla statku, czyli konieczność ukształtowania nowej kultury bezpieczeństwa.
The main reason behind the development of the International Safety Management Code was a series of tragic maritime accidents at the end of the twentieth century caused by human error. The ISM Code has introduced a brand new instrument, the safety management system (SMS), to the set of already existing legal devices, established to ensure safety during ship operation. Properly applied and implemented within the shipping company, SMS can be an advantage that will not only result in a measurable increase in the level of safety, but will also result in cost optimization and an increase in the company’s reputation. However, if an SMS is conceived without commitment and conviction on the part of the management, it will be only an empty and façade, bureaucratic procedure that will not only fail its purpose, but will also only be an additional burden for all staff. The following article will present the characteristics of SMS and its impact both on the broadly understood safety in the shipping company, and including vessels in its fleet, moreover the requirement of the efficiency of SMS procedures specified in shipboard manual for each vessel will be emphasized, which is the need to create a new safety culture.

The aviation safety issue has been one of the most urgent challenges since the first day of flight operations. Aircrafts, avionics, engines, and navigation, communication and surveillance aids have changed and developed, but the problem has never lost its relevance. The main purpose of ICAO is to ensure the safe and orderly development of all aspects of international civil aviation. ICAO develops Standards and Recommended Practices. They are set out in 19 Annexes to the Convention on International Civil Aviation. The new ICAO strategies are the basis for the further sustainable development of global civil aviation. The paper paid special attention to the civil aviation hazards monitoring and risks management in the context of global aviation development trends.The paper deals with challenges of international education and introduces practical experience of the National Aviation University according to the new technologies in transport and aviation education, and trends of international cooperation with the International University of Logistics and Transport in Wroclaw in this sphere.

Zarządzanie bezpieczeństwem w lotnictwie cywilnym stało się przedmiotem regulacji prawnych stosunkowo niedawno. Samo bezpieczeństwo zaś stanowi priorytet w działalności podmiotów lotniczych już od czasu II wojny światowej. Zaczęto wówczas zwracać uwagę na czynniki obniżające poziom bezpieczeństwa w trakcie wykonywania operacji lotniczych. Wtedy rozpoczęły się także ery poszczególnych czynników wpływających negatywnie na bezpieczeństwo lotnicze. W pierwszej erze – czynnika technicznego – szczególną uwagę poświęcano wadom konstrukcyjnym statków powietrznych. Po wyeliminowaniu większości problemów natury technicznej, co było ściśle związane z rozwojem konstrukcji płatowców, nastąpiła era czynnika ludzkiego. Zaczęto skupiać się na błędach ludzkich, uchybieniach i słabościach człowieka. Era czynnika ludzkiego tak naprawdę nigdy się nie zakończyła – człowiek nadal pozostaje przyczyną występowania niektórych zdarzeń lotniczych. Jednakże poczyniono ogromne inwestycje w środki neutralizujące skutki ludzkich błędów. Po erze czynnika ludzkiego nastąpiła era systemowa (organizacyjna). Zwrócono uwagę, że lotnictwo jest systemem złożonym z wielu elementów, które uzupełniają się wzajemnie, a niewydolność choćby jednego z nich zaburza działanie całego systemu. Zauważono także, iż ludzie nie funkcjonują w próżni, ale w określonych sytuacjach operacyjnych. Ponadto lotnictwo funkcjonuje w zmiennym otoczeniu (środowisko naturalne, wzrost ruchu lotniczego, pasażerowie, konkurencja, koncepcje użytkowania przestrzeni powietrznej, technologie) – aby sprostać jego wymaganiom, niezbędne jest prowadzenie efektywnego procesu zarządzania bezpieczeństwem. Międzynarodowe organizacje lotnicze kooperują w zakresie opracowywania regulacji prawnych oraz wytycznych, również tych dotyczących bezpieczeństwa. Muszą one uwzględniać pojawiające się, coraz to nowe wyzwania dla lotnictwa cywilnego i wspierać poszczególne państwa członkowskie w zarządzaniu bezpieczeństwem w sposób jednolity.
Civil aviation safety management has become a relatively recent subject of regulation, while safety itself has been a priority in the activities of aviation entities since World War II. It was back then when people started to pay attention to the factors lowering the level of air operations safety. Since that moment the era of individual factors negatively influencing aviation safety has begun. The first era – the technical factor – concerned aircraft construction defects. After eliminating most of technical problems, which was closely related to the development of airframe structures, the era of the human factor began. The focus was set on human errors, human breaches and weaknesses. The era of the human factor has never really ended, people continue to cause of some aviation incidents. However, huge investments have been made in measures to counteract the effects of human error. The era of the human factor was followed by the systemic (organizational) era. It was pointed out that aviation is a system composed of many elements that complement one another, and the failure of even one of them disrupts the operation of the entire system. It has also been noticed that people do not function in a vacuum, but in certain operational situations. Moreover, aviation functions in a changing environment (natural environment, air traffic growth, passengers, competition, airspace use concepts, technologies), in order to meet its requirements it is necessary to conduct an effective safety management process. International aviation organizations cooperate in the development of regulations and guidelines, including those related to safety. They must take into account the emerging, new challenges faced by civil aviation as well as support individual member states in managing safety in a consistent manner.
