Objective: The purpose of this study was to examine the influence of gender and existing, recurrent low back pain (rLBP) on lower extremity and trunk mechanics, as well as neuromuscular control, during a lift task. Design: A multivariate design was used to examine the effects of gender and group on biomechanical and neuromuscular control variables in randomized symmetric and asymmetric lifting. Methods: 68 Males and females with rLBP and healthy performed symmetric and asymmetric weighted box lifting trials to a 1 meter height table. Results: Lifting style was different between gender and between the rLBP versus healthy groups during a 1m box lifting. A significant two-way interaction effect between gender and group was observed for multifidus muscle activity and knee rotation in asymmetric lifting. Several gender and group main effects were observed in pelvis obliquity, trunk flexion and side flexion, knee abduction angles in symmetric lifting; and in pelvis obliquity and rotation, trunk flexion and side flexion, hip abduction, knee abduction angles, external oblique and internal oblique muscles activity in asymmetric lifting. Conclusions: Females and individuals with rLBP appear to use different lifting styles that emphasize movement at the pelvis accompanied by poor kinematic control features at the hip, trunk and knee. Clinicians should be mindful of these changes when developing prevention and rehabilitation programs aimed at improving trunk control in preparation for lifting tasks during domestic and occupational activities.

Musculoskeletal disorders lead to pain and suffering and result in high costs to industry. There is evidence to suggest that whereas conventional ergonomics training programs result in knowledge gains, they may not necessarily translate to changes in behavior. There were 11 participants in an ergonomics training program, and a subsample of participants received a motivational intervention in the form of incentives for correct workstation setup. Training did not yield any changes in ergonomics measures for any participant. Incentives resulted in marked and durable changes in targeted workstation measures. The data suggest that improving worker knowledge about ergonomically correct workstation setup does not necessarily lead to correct workstation setup, and that motivational interventions may be needed to achieve lasting behavior change.

Introduction. The high requirements in terms of physical fitness of hockey players may be a factor predisposing to injuries. The purpose of the study was to determine the functional limitations of the locomotor system of children practicing ice hockey. Materials and met hods. 104 children took part in the study, including 16 girls and 88 boys, divided into two groups. The first group consisted of children practicing hockey (n=38). The second group consisted of children who do not practice hockey (n=66). The research tool was the FMS test consisting of seven movement activities graded on a 0 - 3 scale. The Mann - Whitney U test was used to evaluate the differences between particular groups, and the Wilcoxon’s test was used to evaluate the differences between the sides. The rela tionships between the variables were established based on the rho Spearmann correlation. The minimal statistical significance was set at p ≤ 0.05. Results. The boys practicing hockey obtained a significantly higher total result than the boys who are not hock ey players (p=0.008). The girls practicing hockey obtained a result close to their peers who do not play hockey. A significant positive correlation was observed between the age of the players and the result obtained in the FMS test in the group of hockey p layers (r=0.77; p<0.001), and between the training experience and the result of the test (r=0.49; p<0.01). Conclusion. The players obtained a significantly statistically higher result in the FMS test, which may indicate a higher level of functional fitness , resulting from a rational training and the acceleration of motor development.

Introduction. The purpose of this study was to describe factors of possible importance for the occurrence of hand injury from powered wood splitters. Patients. Patients were identified by a computerized patient registry. Information was obtained from hospital records, a written questionnaire and a structured telephone interview. Results. Very few splitters were constructed according to European standards. Twenty-one percent of patients injured with wedge splitters thought that having more than one person at the machine was one cause of the accident. Seventy-nine percent of patients injured with screw splitters stated that glove use was one cause of the accident. Conclusions. The level of safety in wood splitters that cause hand injury is often poor. Having more than one person at the machine during work may contribute to wedge splitter injury. Glove use commonly contributes to screw splitter injury. Prevention should be directed towards unsafe machines and dangerous patterns of use.

Opis:
Bike-fitting methods based on knee kinematics have been proposed to determine optimal saddle height. The Holmes method recommends that knee angle be between 25° and 35° when the pedal is at bottom dead centre in static. Other authors advocate knee angle of 30–40° during maximum knee extension while pedalling. Although knee angle would be 5–10° greater at bottom dead centre during pedalling, no study has reported reference values in this condition. The purpose of this study was to compare these three methodologies on knee, hip, and ankle angles and to develop new dynamic reference range at bottom dead centre. Methods: Twenty-six cyclists volunteered for this experiment and performed a pedalling test on their personal road or mountain bike. Knee, hip, and ankle angles were assessed by two-dimensional video analysis. Results: Dynamic knee angle was 8° significantly greater than static knee angle when the pedal was at bottom dead centre. Moreover, dynamic knee angle with the pedal at bottom dead centre was 3° significantly greater than dynamic knee angle during maximum knee extension. The chosen methodology also significantly impacted hip and ankle angles under most conditions. Conclusions: The results allow us to suggest a new range of 33–43° when the pedal is at bottom dead centre during pedalling. Thus, this study defines clearly the different ranges to determine optimal saddle height in cycling according to the condition of measurement. These findings are important for researchers and bike-fitting professionals to avoid saddle height adjustment errors that can affect cyclists’ health and performance.

Opis:
Injury is the leading cause of death and disability in children and adolescents in Poland. In 2009, 1220 children age 0-19 years died as a result of injury. If the rate of injury deaths in Poland could be reduced to the level of the Netherlands, it is estimated that 703 (58%) of these lives could have been saved. The aim of this study was to assess child and adolescent unintentional injury prevention in Poland through national law and policy. The study was based on an examination of law and policies existing and implemented or enforced in Poland which support child safety, including specific injury areas such as road safety, water safety, fall prevention, poisoning prevention, burn and scalds prevention, choking and strangulation prevention. The review of law and policy documents, as well as interviews by phone or e-mail with representatives of government departments was conducted. The questionnaire developed in the frame of the European Project TACTICS (Tools to Address Childhood Trauma, Injury and Children’s Safety) was applied. Each policy and law as a marker for specific injury areas was assessed on 3-points scale. The following scores were obtained for each of the areas of child and adolescent injury prevention in Poland (out of possible 100%): 89% in moped and motor scooter safety, 86% in poisoning prevention, 75% in pedestrian safety, 75% in cycling safety, 75% in water safety and drowning prevention, 64% in burn and scalds prevention, 59% in passenger and driver safety, 59% in choking and strangulation prevention, 50% in fall safety. Child home safety related to prevention of falls, burns and scalds, choking and strangulation has not received the adequate attention. The greatest gains in injury prevention have been made in road safety and poisoning prevention. It is important to both continue these efforts and give equal attention to injuries occurring in and around the home, sport and leisure environments. There is a need to support and fund good practices injury prevention measures based on community level targeting families with low socio-economic status.

ObjectivesThis paper describes some operational tactical procedures (OTP) and discusses the results of a 14-year-long study, spanning the period 2003–2016, conducted by the Municipal Police of Cádiz, Spain, which comprised 3 time periods: 2003–2006, when the officers were trained in traditional policing procedures; 2007–2013, when the officers were taught an innovative set of OTP in the form of a basic set of self-defense and arrest mechanisms, different from the traditional policing procedures that rely on martial arts and combat sports; and finally 2014–2016, when the OTP training was discontinued. The aim of this study was to improve policing and reduce officer injuries resulting from interventions in controversial or violent situations, such as problematic arrests.Material and MethodsThe study involved 162 police officers and commanders of the Municipal Police of Cádiz, who were in street duty for their first time. There were 8 females and 154 males aged 24–55 years. Three OTP stages are shown as examples.ResultsBased on the analysis of “training hours” and “physical interventions in problematic arrests,” the results were: 1) the number of sick leaves in the police was identical according to the number of arrests, and 2) data on sick leaves show remarkable differences among the 3 periods under analysis.ConclusionsThe OTP-based training substantially reduced officer sick leaves. The overall reduction in sick leaves in the period 2007–2013 was observed that cannot be ascribed to a decrease in criminal acts, and hence in police physical interventions.

Aim. The main objective of this article is to contribute to the research in the area of accident incident rates among school children. Methods. A non-standardized, 12-item questionnaire based on independent compilation was used for data collection. The questionnaire contained closed questions which monitor the nature of school-related injuries, gender and age of respondents. Results. 100 students with diagnosed ADHD and 100 children without ADHD diagnosis participated in the research. The accident and incident data were collected between September 2016 and June 2017. Out of 100 given questionnaires filled by ADHD children, the result shows that 49% of them experienced an injury in school and children without ADHD diagnosis reported injury in only 26% of cases, which means that roughly every second child with a Attention Deficit Hyperactivity Disorder experienced an injury some time during the school year during class, whereas among neurotypical children this ratio would refer to every fourth child. Conclusions. The results of the analysts pointed at the possible need of informing all staff involved in the education system of ADHD children about the risks of injury in school environment and thus support the prevention of injuries, support the prevention programs which would be focused on children with attention disorders.

Occupational injuries requiring admission to a trauma unit were examined to outline the events surrounding the injury and to examine the costs. Sixty-nine patients were admitted over a 12-month period, representing 4.30% of all work-related injuries attending the emergency department and 4.25% of all admissions to the trauma unit. Most were male (91%), working in skilled trade occupations (65%), with a mean age of 38.8 years. Personal protective equipment was used only by 46% of injured workers who should have been using it. Sixty-one percent of patients believed that their injury was preventable. Half of the injuries were to the upper limb, fall was the most frequent mechanism (25%) and the median duration of admission was 2 days. The direct hospital costs were estimated at over 300 000 GBP. Failure to use protective equipment and to follow health and safety guidelines suggests that opportunities exist for injury prevention.

Objective. The aim of the study is to present four cases of lawn mowers injuries as a cause of serious visual acuity impairment. Materials and method. A retrospective study of four patients admitted in 2013–2015 to the Department of Vitreoretinal Surgery in Lublin with severe open or closed globe injury, one with an intraocular foreign body (IOFB). The presence of eye protective equipment was assessed, as well as visual acuity, eye tissue condition before and after treatment, and applied therapy. Results. In all cases an improvement was achieved in local conditions. The intraocular foreign body was removed, wounds sutured and damaged tissues placed in position. All eyeballs were saved. In three cases, visual acuity was improved to a usable level. Three patients underwent pars plana vitrectomy, one with IOFB removal from the vitreous cavity. Conclusions. Lawn mower induced eye injuries are a significant cause of serious visual acuity impairment or blindness. The presented study shows that lawn mower eye injuries are still a therapeutic, social and economic problem, yet are very preventable with proper eye protection and patients’ education. Current prevention strategies are inadequate, and therefore should be updated.

Objective. This research examines the emerging role of FMS™ in the context of predicting lower extremity injury in females University athletes. The secondary purpose of this study is to investigate the differences between FMS performance comparisons between three martial arts sports to establish baseline comparisons. Method. Forty-seven university female athletes were recruited for this study, The data collected was separated into three groups based on their sports discipline (judo N=17 age: 19}4, wrestling N=15 age: 18}5, karate N=15 age: 19}3), Independent t-tests were performed on each group with significance being set at P<0.05 to determine difference in FMS™ scores between injured and non injured athletes during the successive competitive seasons. One-way analyses of variances were used to determine if there was a significant difference between sports, ‘body parts injured’ groups, and ‘mechanisms of injury’ groups. Results. One-way analysis of variance revealed no statistically significant difference between the two (ankle, knee) injury groups, and non-injury group (F2,54= 2.34; p=0.106). There was no statistical difference between the pre-season FMS™ scores of the injured and non-injured groups (t47 = -1.68; P=.100; d=0.52; 95%CI: -0.11, 1.15). Finally, strong evidence of FMS score was found when comparing the three sports with one-way ANOVAs (F=5.83, df= 2, 54, p=0.005). Conclusion. One of the more significant findings to emerge from this study is that FMS™ has emerged as a powerful tool for identifying lower extremity injury in female athletes. Further investigation and experimentation into FMS™ are strongly recommended before implementing them into a pre-participation physical examination (PPE) for combat sports. What is now needed is a cross-national study involving other sports.
Cel.

Do przeprowadzenia badań wykorzystano oryginalną ankietę. Ankieta była standaryzowana, anonimowa, zawierała 29 pytań. Wyniki: W wyniku przeprowadzonych badań wykazano, iż wśród 301 przebadanych uczniów ponad połowa – 158 (52,5%) doznała urazu na lekcjach wychowania fizycznego. Analizując rodzaj doznanego urazu wykazano, że najczęściej dochodziło do stłuczeń (53,16%), które dotyczyły głównie kończyn dolnych (55,06%). Stwierdzono wysoce istotną statystycznie zależność pomiędzy częstotliwością urazów a wiekiem badanych (p<0,001), środowiskiem zamieszkania (p<0,001), a także rodzajem aktywności ruchowej pozalekcyjnej (p<0,05).
Introduction: The classes of Physical Education include a range of activities aimed at comprehensive physical development of human body. However, there is no doubt that exercises at Physical Education (PE) are associated with the risk of injury or damage to the body. The threat of loss of health in school environment is currently becoming a significant social issue. Therefore, particular attention should be paid to the safety of young people both in course of planning of PE classes and the choice of methods and forms of activity. Aim: The aim of this study was to evaluate the incidence of injuries in primary and secondary schools students during PE classes in the Podkarpackie Province and the assessment of correlation between the incidence of sports injuries among children and adolescents and selected factors. Material and methods: 301 primary and secondary school students from the Podkarpackie Province were included in the study. The study group comprised of 151 boys and 150 girls. Age of the participants ranged from 9 to 16 years of age (mean age 12.58 years). The author’s questionnaire was used to carry out the research. The survey was standardized, anonymous and consisted of 29 questions. Results: The results of the study showed that out of the total of 301 students surveyed, more than a half – 158 (52.5%) suffered occasionally from an injury at PE lessons. The analysis in terms of injury type revealed that contusions were predominant (53.16%), the lower limbs were the most frequently affected (55,06%). There was a highly significant relationship between the frequency of injuries and the age of students (p <0.001), living environment (p <0.001) as well as a kind of after-school physical activity (p <0.05). Conclusions: Injuries at PE classes are common among students in the Podkarpackie Province. There is a significant relationship between the age, living environment, afterschool physical activity and the incidence of injury during physical education classes.

Background: The most common risk factors for running-related injuries are mistakes, such as insufficient warm-up and stretching exercises, during training. Good preparation and proper training reduces the risk of sport-related injuries. Aim of the study: To examine fundamental movement patterns and likelihood of injury in amateur runners. Material and methods: Twenty-four amateur long-distance runners from Opole region (Poland) were divided into two groups. The first group comprised 12 runners from the club “Kotwica Brzeg”, who did a proper warmup before training and stretching exercises after training. The second group comprised 12 runners from other clubs who did not undertake any warm-up or stretching exercises (control group). Fundamental movement patterns were tested by the Functional Movement Screen test (FMS). Results: The mean FMS test score was higher in “Kotwica Brzeg” runners (17.08 points) than in the control group (15.50 points), but this was not statistically significant. The “Kotwica Brzeg” runners performed better in five of the FMS tests, but this was only significant for the rotational stability test. Conclusions: Runners who did a proper warm-up and stretching exercises achieved better results in the FMS test, which may reduce the risk of running-related injuries.

Opis:
Contrast-induced nephropathy (CIN) is a well known side-effect of the administration of radiocontrast media. It could be defined as occurrence of signs of acute kidney injury de novo, or worsening of renal function in patient with chronic kidney disease, after administration of IV contrast. Its clinical manifestation is often poor, usually reversible, but it can lead to serious adverse outcomes. In general population CIN occurs with 1% frequency. In case of patients with risk factors its frequency rise up to 50%. It is the third leading cause of acute kidney injury in hospitalised patients and substantially increases mortality, morbidity and length of hospitalisation. Direct pathophysiological mechanisms of CIN are not well known. In most cases of contrast administration we use iodinated agents. In MR imaging gadolinium based agents are used. There is no specific treatment of CIN. The best mode of proceeding is prevention. In patients with normal renal function we should simply avoid volume depletion. We present here a variety of preventive measures in patients with CIN risk factors.

Opis:
Transfusion-related acute lung injury is defined as acute respiratory failure which develops during or within 6 hours after transfusion of a blood component in a patient with no risk factors for respiratory insufficiency. Transfusion-related acute lung injury is diagnosed based on clinical manifestation and by excluding other causes of acute lung injury. Unambiguous diagnosis is difficult. Looking for anti-HLA and/or anti-HNA antibodies in donors and sometimes in recipients plays an important role in lab tests. Negative antibody findings, either in a donor or in a recipient, do not exclude transfusion-related acute lung injury, which, however, does not exempt from performing leukocyte antibody tests since they are extremely important for transfusion-related acute lung injury prophylaxis. The ways to prevent this reaction include: disqualifying donors with anti-HLA/HNA antibodies, screening for antibodies in multiparous women and in individuals after transfusion, modifying the way blood components are prepared and limiting blood transfusion in clinical practice. The paper presents a case of a 38-year-old woman with acute myeloid leukaemia, hospitalised at the Department of Internal Diseases and Haematology of the Military Institute of Medicine for subsequent courses of chemotherapy. During treatment, the patient had red cells and platelets concentrates transfused several times with no transfusion-related reactions. Eight days after the last chemotherapy infusion, the patient developed high temperature and her platelet count was 14 × 103/mL. Therefore, the patient received a platelet concentrate again. About 1 hour after transfusion, the patient complained about chest pain and dyspnoea. She needed oxygen therapy. Chest X-ray revealed lung oedema with no signs of left ventricular failure. Once other causes of acute lung injury were excluded, transfusion-related acute lung injury was diagnosed.
W artykule przedstawiono przypadek 38-letniej kobiety hospitalizowanej w Klinice Chorób Wewnętrznych i Hematologii Wojskowego Instytutu Medycznego z powodu ostrej białaczki szpikowej w celu wykonania kolejnych kursów chemioterapii. W czasie leczenia chorej kilkakrotnie przetaczano koncentraty krwinek czerwonych i koncentraty krwinek płytkowych bez reakcji poprzetoczeniowych.
