Introduction and aim. Any hospital’s outpatient department (OPD) is regarded as the storefront of the facility, and patient satisfaction is an important measure of healthcare quality. Hence, this cross-sectional study was conducted in different OPDs at a tertiary care center. Material and methods. Two hundred patients were recruited, and structured personal interviews were conducted with questions based on the Patient Satisfaction Questionnaire Short Form (PSQ-18). T-tests and analysis of variance (ANOVA) were used to compare satisfaction scores between variables. Results. Upon analysis, 86% of the patients rated their overall experience as either ‘very good’ or ‘good’. Interpersonal manners (mean score±SD; 4.60±0.55), communication (4.39±0.66), general satisfaction (4.03±0.79), and technical qualities (3.86±0.57) were the domains in which the patients were most satisfied, while time spent with the doctor (3.77±0.89), accessibility and convenience (3.77±0.67), and financial aspects (3.37±0.83) were the areas that lagged. Conclusion. Satisfaction scores were found to vary significantly with gender, age, waiting times, and the number of visits per day. Regular patient satisfaction surveys should be conducted in all hospitals for devising interventions to provide patients with the best possible care.

Opis:
Quality in primary health care is gaining a predominant role. Both public and private providers of primary health care services increasingly focus on quality issues, which allows them to gain a competitive advantage. In the article, fifty attributes determining the quality of services identified in literature are considered and evaluated in the context of primary health care services. Both technical attributes relating to the medical process itself and functional attributes resulting from patient experience as a client are studied. An analysis aiming to rank the importance of attributes was made on the basis of surveys carried out among 98 primary health care PHC managers. The research allowed for the identification of the most and the least important primary health care service attributes determining quality from the perspective of PHC managers. Functional attributes relating to patients` experience of care are being noticed as important and desired by patients. The findings resulting from this study can be practically applied at primary health care entities in order to maximise the satisfaction of patients through proper adjustment of business models.

In 2006 the Netherlands commenced a major reform of its health care system. The main elements of the reform were: 1) replacement of the existing system of social health insurance for people with below average income and private health insurance for people with above average income by a universal private health insurance with the identical entitlements and contributions for all 2) the gradual introduction of elements of managed competition in hospital markets. The main aims of the reforms were to improve the so-called “public interests” in health care which were defined as quality, access, efficiency and cost containment in health care. This paper describes the reforms that have been enacted in the Dutch health care system and evaluates the impact of these reforms on the “public interests” in health care. The health care reforms have had positive effects on most of “public interests”, though still much needs to be done e.g. development of quality standards, curbing the rapid rising costs of health care. Nevertheless, the reforms are still a work in progress, and there is still a great deal of  room for further improvement in ‘public interest’ in the Dutch health care system.

Quality measurement is the first step in improving overall quality. Servqual has been identified as one of the most important methods frequently used in assessing the quality of healthcare services. This study aims to systematically review and conduct meta- analysis of conducted studies in this regard in which Servqual tools were used to assess and measure the quality of services. In this systematic review and meta-analysis, the required data were collected using several keywords (and their Persian equivalents): services quality, Servqual, quality, gap, primary health cares, health services, Iran. The databases searched were as follows: PubMed, Scopus, Google Scholar, SID, Magiran, and Iranmedex. The comprehensive meta-analysis (CMA) software, Version 2 was used for data analysis. Based on the random effect model, the total mean score of Service Consumers’ perception, Service Consumers’ expectation and the gap between them were estimated as being 3.81 (95% CI: 3.56–4.06), 4.49 (95% CI: 4.31–4.67) and -0.83 (95% CI: -065 – -1.01), respectively. The lowest score of consumers’ perception of services quality, was related to the empathy dimension (3.57) and the highest score of consumers’ expectation of services quality was related to the reliability dimension (4.60). Furthermore, there was a quality gap in all dimensions. In order to achieve desired quality and meet service receivers’ needs, attention should be always given to their views, so that proper planning can be done to address existing problems through assessing views. Should such be undertaken, we would be able to witness services quality improvement.

Introduction. An adverse event is an incident induced while providing health care services or resulting from it, not related to the natural course of a given disease or health condition, which causes or is likely to cause negative consequences for the patient, including their death, a threat to life, the necessity of hospitalisation or its prolongation, permanent or considerable health detriment; or is a foetal disease, congenital defect or the result of foetal damage. Objective. The aim of this analysis is to explore the problem of the occurrence of adverse events from the perspective of doctors and ward nurses who manage wards. Materials and method. The research on the occurrence of adverse events among doctors and nurses (the management staff) was conducted with the use of a postal survey. Results. It was ascertained that 86.5% of the medical personnel had taken part in an adverse event, of which 20.2% took part in an occurrence associated with pharmacotherapy, 16.2% – in an event related to diagnostics and diagnosis, or an infection – 15.7%. 14.2% of respondents were involved in an occurrence linked to a medical device malfunction, and 14.1% – in an adverse event related to an operation. Conclusions. The adverse events most often identified in the nursing professional group are occurrences associated with pharmacotherapy, and in the doctors’ professional group – occurrences related to diagnostics and diagnosis. The research established that the most frequent reason for not informing patients about the occurrence of an adverse event is fear of their filing a complaint. Medical management staff show high acceptance of an adverse event reporting system as a tool for improving patient safety.

Responsibilities in the field of health care have been imposed on public authorities in the Polish Constitution. The most important of these is the need to ensure citizens, regardless of their financial situation, equal access to healthcare services financed from public funds. Other responsibilities of public authorities is to provide special healthcare to children, pregnant women, disabled people and elderly people, as well as to combat epidemic illnesses and prevent the negative health effects of environmental degradation. In addition, the State should promote the development of physical culture, particularly among children and young people. The considerations undertaken in the article apply primarily to implementing of the above-mentioned liability of providing high-quality health care for children, pregnant women, disabled people and elderly people. Regrettably, the analysis of binding legal acts, current health care programs and the Commissioner of Patients’ Rights reports leads to the conclusion that the State does not fulfil the obligation in a satisfactory manner.
Obowiązująca Konstytucja RP nakłada na władze publiczne obowiązki w zakresie ochrony zdrowia. Najważniejszym z nich jest konieczność zapewnienia obywatelom, niezależnie od ich sytuacji materialnej, równego dostępu do świadczeń opieki zdrowotnej finansowanej ze środków publicznych. Innym obowiązkiem władz publicznych w zakresie ochrony zdrowia jest zapewnienie szczególnej opieki zdrowotnej dzieciom, kobietom w ciąży, osobom niepełnosprawnym oraz osobom starszym, a także zwalczanie chorób epidemicznych i zapobieganie negatywnym dla zdrowia skutkom degradacji środowiska. Ponadto państwo powinno wspierać rozwój kultury fizycznej, zwłaszcza wśród dzieci i młodzieży. Rozważania podjęte w artykule dotyczą głównie realizacji obowiązku zapewnienia szczególnej opieki zdrowotnej dzieciom, kobietom w ciąży, osobom niepełnosprawnym i osobom starszym. Niestety, analiza obowiązujących aktów normatywnych, aktualnych programów zdrowotnych oraz sprawozdań Rzecznika Praw Pacjenta prowadzi do wniosku, że państwo nie wywiązuje się z niego w sposób zadowalający.

This article presents a summary of the scientific view of the quality of health care facilities in dimensions reflecting the quality of certain device management. The main objective is to construct an alternative model that will reflect the quality of the health care device using the minimum inputs. The authors have selected inputs from four areas to evaluate the quality. The study has selected the area of marketing-management to the recommendations of the patient, from the field of crisis management in the dimension of preventive activity with the output of the number of extraordinary events. The third area captures the quality of the services provided as a prerequisite for minimizing sanctions and of course, the area of financial management has not been omitted, and so the model also includes the variable ability to generate own resources. The study can be characterized as secondary since the data were collected from the INEKO databases and the Health Care Surveillance Office with a sample of 65 health care facilities in the territory of Slovakia during the period of 2009-2016. The before mentioned dependent variables at the outputs of the correlation analysis significantly approximated all independent variables. If dependent variables were to be considered separately, the “recommending the hospital” variable correlated with more than 50% of the independent variables. Variable of sanctions is more than 30% and the variable of the ability to generate own resources is more than 20% of independent variables. Significant correlation with the variable of extraordinary events has occurred in almost 20% of cases. Many other health care quality-evaluating models have a common feature, which is cumbersome and complex, on contrary, the proposed model is easy to apply, and it is useful when “fast” evaluation is needed.
W artykule przedstawiono podsumowanie naukowego poglądu na jakość placówek służby zdrowia w wymiarach odzwierciedlających jakość zarządzania. Głównym celem było stworzenie alternatywnego modelu, który odzwierciedlałby jakość urządzeń medycznych przy użyciu jak najmniejszej ilości danych wejściowych, w celu oceny jakości wybraliśmy dane wejściowe z czterech obszarów. Z obszaru zarządzania marketingowego do obszaru zaleceń pacjenta, z obszaru zarządzania kryzysowego w wymiarze działania profilaktycznego z wynikiem zdarzeń nadzwyczajnych. Trzeci obszar określa jakość świadczonych usług. Model obejmuje również zmienną zdolność do generowania własnych zasobów. Przeprowadzone badania scharakteryzować można jako wtórne, zebrane zostały z baz danych INEKO i Urzędu Nadzoru Opieki Zdrowotnej w próbie 65 zakładów opieki zdrowotnej na terenie Słowacji w latach 2009-2016. Wspomniane wcześniej zmienne zależne na wyjściu analizy korelacji znacznie przybliżyły wszystkie zmienne niezależne. Jeżeli zmienne zależne rozpatrywano by osobno, zmienna "polecanie szpitala" korelowałaby z ponad 50% zmiennych niezależnych, zmienna sankcji korelowałaby z ponad 30%, a zmienna zdolność do generowania własnych zasobów z ponad 20% zmiennych niezależnych. Istotna korelacja ze zmienną zdarzeń nadzwyczajnych wystąpiła w prawie 20% przypadków. Wiele innych modeli oceny jakości opieki zdrowotnej ma wspólną cechę, są skomplikowane, a przez to trudne do praktycznego wykorzystania, nasz model w przeciwieństwie do pozostałych jest łatwy do zastosowania i przydatny, gdy potrzebna jest "szybka" ocena.

Introduction and objective. Measuring the level of patient satisfaction is a useful tool in delivering quality care that is responsive to consumer preferences. Various socio-demographic factors might be considered as potential predictors of patient satisfaction. The aim of the study was to assess whether place of residence (rural/urban) affects patient satisfaction with hospital health care. Materials and method. Data were obtained using face-to-face questionnaire, administered in five large, urban hospitals in Podlaskie Province, north-eastern Poland, during 2014–2018. The study sample comprised of 1,624 participants (585 rural, 1039 urban) who assessed satisfaction with 28 hospital health care items. The means and standard deviations were presented to compare 28 satisfaction items between the rural and the urban samples. Regression analysis was used to determine whether location difference (rural vs urban) affected patient satisfaction with various domains of hospital health care. Results. The mean results of 28 satisfaction items on the 1–5 scale were similar among the rural and the urban samples, and generally skewed towards positive experiences. In the univariate analysis, significant associations between place of residence and patient satisfaction were identified with regard to three components of inpatient care: 1) hospital settings and staff care, 2) doctors’ professional skills, and 3) hospitalization outcomes. After adjusting for socio-demographic variables, the association remained significant only with respect to satisfaction with hospitalization outcomes (b = 0.121; SE = 0.055; p = 0.028). Conclusions. Some evidence for differences in patient satisfaction by place of residence was found. Study findings may be helpful in implementing care quality improvement programmes.

This article presents the quality, safety and assessment system as the important tools to supervise the health care organization. Quality is the sum of the properties and characteristics of a product, process or service that is appropriate to meet the complex requirements. In the case of health care is difficult to valuate quality, the universal indicators become useful tool. The development of quality indicators requires collecting of data and their proper processing. The high quality of medical services requires continuous improvement and adaptation to patients’ needs. The quality indicators, for example, refer directly to the effects of therapy and are used to measure the success or failure of the applied therapeutic methods. Measuring the results of activities indicates only the level of quality of the services provided. The data helps to analyze information and improve the quality and safety of health care services.

This study is divided in three parts which intend to meet the following targets: Primarily to provide theoretical approaches toward the concept of equality in the healthcare. In the theoretical treatment of the concept of equality in health care the authors are cautious while making the difference between equality and justice; between equality in levels of health or health care and equity in financing etc. Secondly target it to assess the concept implemented in our legislation and make an overall assessment of whether this legal framework is comprehensive and convenient for the realization of the right to health care enshrined in the Constitution. The Albanian legislation stipulates that health care in the Republic of Albania is guided by very important principles. These principles embody the Constitution predictions from the formal point of view, primarily when declaring the right to health care as a key right of the individual, as well as when guaranteeing the equal rights based on non-discrimination. Thirdly to determine some of the specific areas of the legislation, which have to be improved and completed. After the analysis make in this paper, we found the need for some theoretical and practical improvements, which are reflected in the conclusions.

The safety of the patient during the providing the health services is the main defiance for health policy and for the proper functioning of the units of the health care. The conditions of the health care system determinate the quality of the health services, and consequently cause the safety of the patient incidents. As a result, the right of patient to the safety is violated, frequently in very serious way. In this acrticle there are described two main questions. First of all, some statistical data has been cited, which illustrate the scale of the medical incidents, related to the health services in Poland and the other countries. Then there are briefly described european and polish legal acts, which regulate the procedures, areas and methods of the activities for improving the safety of the patients.
Bezpieczeństwo pacjenta w trakcie udzielania świadczeń zdrowotnych stanowi kluczowe wyzwanie dla polityki zdrowotnej i należytego funkcjonowania podmiotów leczniczych. Uwarunkowania systemowe powodują, że nienależyta jakość opieki zdrowotnej prowadzi w następstwie do zdarzeń niepożądanych i stawia pod znakiem zapytania prawo pacjenta do bezpieczeństwa. W artykule poruszono głównie dwie kwestie. Najpierw przywołano niektóre nowsze dane statystyczne ilustrujące skalę zdarzeń niepożądanych związanych z udzielaniem świadczeń zdrowotnych w Polsce i w innych krajach. Następnie zwięźle omówiono kluczowe dokumenty europejskie i polskie, które określiły konieczne procedury, zakresy i metody działania dla poprawy bezpieczeństwa pacjenta.

Introduction: The role of quality in achieving, improving, maintaining repeatable processes, service level guarantee patient satisfaction, determinants of hospitals.Objective:Identification of the mechanisms consistent quality in the provision of services in the public hospitals.Materials and methods:The study was conducted on a random sample of 104 public hospitals in the provinces of A, B, C. Author's questionnaire was distributed among 8975 participants of the medical staff. An analysis of the operating environment and documents, query literature. Was conducted individual in-depth interview with 540 medical experts from January 2007 to December 2011.Results: Diagnosed public hospitals network problems in the implementation phase of quality management system for medical services: interpretation of the requirements of the standards, development of implementation documentation, knowledge of procedures, standards. The work confirmed the theory that managers/Medical is responsible for the good/bad its functioning.Conclusions: Building on the paradigms of science organization and management expanded the scope of the study on the analysis of the factors determining the quality management of medical services based on a family of ISO standards. Factors focused on human capital and structural describing the quality of intellectual capital, supplemented by a layer of organizational and functional entities. This made it possible to get an answer in terms of phenomena, which in the area of quality in the network of public hospitals can be observed. And suggests practical solutions. Indicated tools and capabilities to implement the principles of quality in shaping the satisfaction of stakeholders.

The article addresses the topic of alternative legal models for pursuing out-of-court claims for medical damages, taking as its starting thesis the inefficiency of the Polish model stemming from its faulty legal construction. The purpose of the article is to conduct a legal analysis of the Polish alternative model of medical damage compensation, with particular attention to de lege ferenda postulates presented in the legal doctrine. In addition, in order to show the possible directions of legislative changes, the article presents the characteristics of selected foreign alternative models of medical damage compensation, including the model operating in New Zealand, which is an example of a model that can be classified as ‘no-fault’, the German model and the French model. The article culminates with an analysis of the draft of the Law on Quality in Health Care and Patient Safety to the extent that the draft provides for a new legal regulation of the alternative model for the out-of-court settlement of medical damage claims. The purpose of this analysis was to see whether the legislator has recognized the demands for change articulated in the legal doctrine, and whether the draft law on quality in health care and patient safety is inspired by solutions adopted in foreign legislation in terms of the legal construction of the alternative model of compensation for medical damages. The analysis here confirms the initial thesis of the work and leads to the conclusion that the draft law on quality in health care and patient safety takes into account many proposals constructed in the legal doctrine in connection with regulating an alternative model for the compensation of medical damages. It is also clear that the proposed model is inspired by some solutions that have been successful in other countries. Nevertheless, the article distinguishes proposed regulations that may still raise doubts and reservations.
