Demographic changes associated with the aging population mean that surgeons increasingly have contact and make decisions about treating patients from the oldest age groups. The aim of the study was to review the literature concerning the treatment of acute appendicitis in patients over the age of 60 years old. Material and methods. A review of the literature published in the years 2000-2015 has been carried out using the PubMed database. The initial number of results corresponding to the query in English, „appendicitis (MeSH) AND elderly (MeSH)” was 260. Selection based on the titles, abstracts, and eventually whole articles, ultimately resulted in 11 papers concerning the treatment of appendicitis in patients above 60 years of age. Results. Nine papers were retrospective and 2 were prospective. In total, the studies included 82,852 patients. Laparoscopic appendectomy was associated with a lower mortality rate, a smaller number of postoperative complications and a shorter length of hospital stay, which led to it being recommended by most authors. Four of the ten papers demonstrated that the patients who were qualified for laparoscopic surgery had less comorbidity and were in a lower ASA (American Society of Anaesthesiology) category. Antibiotic therapy as an independent method was assessed in one study in a group of elderly people, on a selected group of 26 patients, and its effectiveness was shown to be 70%. Most studies, however, are highly heterogeneous which significantly hindered comparisons. Conclusions. Currently, laparoscopic appendectomy seems to be the treatment of choice in the elderly with acute appendicitis. Antibiotic therapy, as an independent method of treatment of acute appendicitis, cannot currently be recommended. However, further, prospective, and better-designed studies are needed, involving a larger number of patients, and primarily dedicated to the elderly.

Introduction. The 2019-novel-coronavirus (2019-nCoV) outbreak has become a common problem for the whole world. Aim. To investigate the impact of the 2019-nCoV pandemic period in our country on emergency room admissions. Materials and methods. The study was designed as a retrospective cohort. The first case of pneumonia infected with nCoV in our country was diagnosed on March 11, 2020. Considering the date of March 11, 2020, emergency room admissions for two periods of 7 days were investigated, pre-COVID-19 and COVID19. Demographic data, admission diagnosis, vital findings, stay times in emergency room, terminations and emergency department mortality examined. A group of ‘geriatric’ patient populations were created to examine the admission characteristics of patients aged-65 and over. Results. The 3466 patients included in the study. The average number of daily admissions was significantly higher in the pre-COVID-19 period (350.4 ± 54.5), compared to the COVID-19 period (144.7 ± 20.2, p=0001). While the proportion of cardiac-caused admissions increased during the COVID period, the proportion of gastrointestinal-induced admissions decreased (p<0001). Hospitalization rates for both adult and geriatric patients increased during the COVID period (p<0001). Conclusion. Despite all the negativity caused by the outbreak, this period has been one in which the public is aware of unnecessary emergency room which has been subsequently lessened.

Involution changes are normative consequences of ageing of the organism. They are observed through transformations in the structure of tissues and organs. As age increases, the systemic reorganisation affects the functions (changes in hormonal activity and the number of neurotransmitters) and the structure (gradual brain shrinking) of the central nervous system (CNS). Neurobiological changes are visible in one’s cognitive capabilities, which are nowadays seen as the index of a brain’s potential and plasticity. As a theoretical basis, the model of selective optimisation with compensation and the adaptive-regulatory theory are used. According to them, changes in the elderly age result from natural processes and can be considered as the measures of individual adaptive capability. Taking into consideration the resources possessed in the late adulthood, this article presents a particular point of view on such a topic. Practically speaking, it implies the need to conduct educational and rationalising actions that will improve the living standard of elderly people.
Zmiany o charakterze inwolucyjnym są normatywnym skutkiem starzenia się organizmu. Przejawiają się przekształceniami w budowie tkanek i narządów. Zachodząca wraz z wiekiem reorganizacja ogólnoustrojowa przekłada się na funkcje (zmianę aktywności hormonalnej i ilości neurotransmiterów) i strukturę (stopniowy ubytek masy mózgowia) ośrodkowego układu nerwowego. Wykładnikami zmian neurobiologicznych jednostki są jej możliwości kognitywne, które współcześnie rozpatruje się jako wskaźnik potencjału i plastyczności mózgu. Jako teoretyczną podstawę opisu przyjęto model selektywnej optymalizacji i kompensacji oraz koncepcję regulacyjno-adaptacyjną, zgodnie z którymi zmiany zachodzące w okresie senioralnym wynikają z naturalnych przeobrażeń i można je uznać za miarę możliwości przystosowawczych jednostki. Artykuł stanowi propozycję spojrzenia na okres późnej dorosłości z perspektywy posiadanych zasobów, co w wymiarze praktycznym implikuje do prowadzenia działań edukacyjnych i usprawniających, podnoszących jakość życia seniorów.

Advanced age is often burdened with many deficits that are a consequence of the aging process, unfavorable lifestyle and multi-morbidity. They contribute to increased morbidity and disability of the older people. Laryngological problems often concern seniors, particularly progressing with age hearing impairment, dizziness, balance disorders, epistaxis, nasopharyngeal diseases, larynx, sinuses, and salivary glands disorders. They are not only a health threat, such as head and neck cancer, but they contribute to the deterioration of mobility, falls and injuries, depression, cognitive functions impairment and, consequently, functional disability, loss of independence and a sense of isolation. In this context, taking as an example the most common ailment, which is hearing impairment, laryngological procedures in the form of the use of hearing aids or cochlear implant improves cognitive functions, mood, self-esteem, opportunities for social interaction and everyday functioning, and the quality of life of older people. In turn, in view of the typical for older age polypathology and of significant reduction of the reserves of the organism, effective treatment aimed at improving health, preventing complications of diagnostics and therapy as well as disability with the desire to maintain independence and good quality of life requires the cooperation of different specialists. The knowledge and experience of geriatricians and the comprehensive geriatric assessment used as the diagnostic tool, aimed at identifying deficits typical of seniors’ age may significantly contribute to a more adequate risk and benefit assessment, selection of optimal treatment for a given health situation and identification of high-risk individuals requiring during the treatment period special supervision to reduce the risk of complications and the risk of development of geriatric syndromes such as delirium, cognitive disorders, malnutrition, falls and injuries, functional disability.

In the light of the Constitution of the Republic of Poland the citizen has the right to health care. This also applies to elderly people in a nursing home (DPS). The deepening of indigestion, the coexistence of many diseases, their chronicity and the level of complications trigger the increase in demand for DPS for systematic, 24-hour geriatric medical care (medical care, nursing and rehabilitation services). Realizing the health needs of elderly people in DPS by health care providers is a source of controversy, hence the need for legislative changes to shift the burden of health financing in DPS to the Polish National Health Fund (NFZ).

GUS demographic forecasts show that by 2035 subpopulation of people with 65 years or more in Poland will grow by 62% compared to 2010. While the subpopulation of people with 80 years or more will increase as much as 96% over the same period. The aim of this article is a diagnosis of selected geriatric care resources in a situation of aging population of rural and urban areas in Poland. To achieve this aim, the study includes demographic forecasting for Poland until 2035 with differentiation on voivodeships and rural and urban areas. Additionally, an analysis of selected geriatric care resources (geriatric beds and specialists in geriatrics) in the context of an aging population was considered. The obtained results conclude that in Poland there is lack of geriatric care resources such as geriatric beds and specialists in geriatrics. Although the projected percentage of people aged 65+ in rural areas will be lower in 2035 than in the urban areas, the access to these resources in rural areas is more limited, due to the concentration of geriatric care resources mainly in big cities.
Prognozy demograficzne GUS pokazują, że do 2035 r. subpopulacja ludzi mających 65 lat i więcej w Polsce wzrośnie o 62% w porównaniu do 2010 r., natomiast subpopulacja ludzi mających 80 lat i więcej wzrośnie w tym samym okresie aż o 96%. Celem artykułu jest diagnoza wybranych zasobów opieki geriatrycznej w sytuacji postępującego starzenia się społeczeństwa wsi i miast w Polsce. Aby zrealizować cel, przedstawiono prognozy demograficzne dla Polski do 2035 roku z podziałem na poszczególne województwa oraz wieś i miasto, jak również dokonano analizy wybranych zasobów opieki geriatrycznej (łóżek geriatrycznych oraz lekarzy geriatrów) w kontekście starzejącego się społeczeństwa. Uzyskane wyniki badań pozwalają stwierdzić, że w Polsce występuje niedostatek zasobów geriatrycznych, tak łóżek szpitalnych, jak i lekarzy specjalistów. Mimo że prognozowany odsetek osób w wieku 65+ na wsi będzie w 2035 r. niższy niż w miastach, to na terenach wiejskich dostęp do tych zasobów jest bardziej ograniczony ze względu na koncentrację zasobów geriatrycznych głównie w dużych miastach.

Observed demographic changes mean that we are becoming a ‘greying society.’ This makes it necessary to introduce new curriculum content in the vocational training of speech therapists and creates a need for specialists in the area of therapy and functional assistance of seniors. At the same time, this indicates the need for conducting extensive studies in the field of gerontological linguistics, concerning the particularities of speech related to age as well as the specific features of communication with the elderly. In this speech the authors would like to discuss one of the directions of research on language phenomena related to old age, i.e. research on the particular manner of communication of those in the senior’s close environment (family, doctors, therapists, among others) with the senior. The style of communication with a person of senior age, being a result of social stereotypes of elderly people, may increase their discomfort and indicate paternalistic and condescending treatment of those in the highest age group. This may even be a sign of ageism, i.e. social discrimination based on people’s age.

While population is aging we are facing raising number of geriatric problems. It is obvious nowadays that if we do not start creating certain plans considering medical and social care of elderly people, the situation may get out of control. The natural course of diseases in elderly is different to middle aged and young people. These individuals show different symptoms and social background, different prognosis and complications of the disease and of diagnostic and therapeutic medical procedures themselves. The major feature that distinguishes geriatric medicine from others is comprehensive geriatric assessment which is holistic and considers not only medical condition but also social, psychological background together with identifying aspects of functional status, nutrition, transportation and others. It seems that the need for professionals with experience in geriatrics is growing. The most important in this area are general practitioners and geriatricians. There is also a constant need for educational effort to create society whose members are responsible for their own health.

The Vulnerable Elders Survey (VES-13) is a simple function based frailty screening tool that can be also administered by the nonclinical personnel within 5 minutes and has been validated in the out- and in patient clinic and acute medical care settings. The aim of the study was to validate theaccuracy of the VES-13 screening method for predicting the frailty syndrome based on a CGA in polish surgical patients. Material and methods. We included prospectively 106 consecutive patients ≥65, that qualify for abdominal surgery (both due to oncological and benign reasons), at the tertiary referral hospital.We evaluated the diagnostic performance of VES-13 score comparing to the results from the CGA, accepted as the gold standard for identifying at risk frail elderly patients. Results. The prevalence of frailty as diagnosed by CGA was 59.4%. There was significantly higher number of frail patients in the oncological group (78% vs. 31%; p<0.01). According to the frailty screening methods, the frailty prevalence was 45.3%. The VES-13 score had a 60% sensitivity and 78% specificity in detecting frailty syndrome. The positive and negative predictive value was 81% and 57%, respectively. The overall predictive capacity was intermediate (AUC=0.69) Conclusions. At present, the VES-13 screening tool for older patients cannot replace the comprehensive geriatric assessment; this is due to the insufficient discriminative power to select patients for further assessment. It might be helpful in a busy clinical practice and in facilities that do not have trained personal for geriatric assessment.
