Opis:
Peculiarities of formation, distribution and fertility of typical chernozems on the territory of Europe as a whole and within the agricultural lands of Ukraine were given. It is noted that the existing agro-industrial characteristics of agricultural products least reflect the quality of raw materials and therefore are not analyzed as a factor influencing public health. The relevance of biogeochemical research of food (agricultural) raw materials and the introduction of new criteria for its quality were outlined. The purpose of the presented research was to expand the quality indicators of agricultural products on the basis of topical structure of agricultural landscapes and biogeochemical criteria for evaluating "typical chernozems – cereals and vegetables" systems. A system of biogeochemical parameters and quality criteria of plant products containing two components has been developed. The first component is the spatial differentiation of natural and agro-technological features of agrolandscapes (systems of rocks-typical chernozems-cultures), which highlight the prevalence of nutrients in bedrock, particle size distribution of typical chernozems and the ratio of their prevalence within the administrative regions of Ukraine. There are 12 groups of agricultural landscapes that differ in these features. A set of biogeochemical parameters for the distribution of essential elements in the components of the agricultural landscape, criteria for their ecological balance and impact on the quality of agricultural raw materials.Implementation of the proposed criteria for the quality of agricultural products was carried out on the territories of two agrolandscapes of typical chernozems in the Kyiv and Poltava regions.It was established that the physico-chemical balance of Zn, Cu and excess Mo in the rocks and soils of the agro-landscape of the Right Bank Forest-Steppe is not consistent with biogeochemical deficiency of copper and, in part, zinc in cereals (oats). The physico-chemical balance of Zn, Cu in the soils of the agro-landscape of the Left Bank Forest-Steppe does not agree with the biogeochemical deficiency of zinc in vegetable crops (potatoes, carrots, and beets). On the basis of the biogeochemical criteria for the distribution of Zn, Cu, Mo in agricultural landscapes, the proposed territorial measures to improve the quality of grain and vegetable products to prevent copper and zinc "hidden hunger" in the population.

In the current national curriculum (the Framework Educational Programme for Basic Education – FEP) the concept of child /pupil/student has become the basis for conceptual thinking about the whole educational process. If we want to respect the pupil´s personality, then it is necessary realize in practice the relevant didactic concept. It includes an approach to student assessment, too. Such assessment fully engages pupils in targeted judging process. In this contribution we describe a model of a comprehensive and developing assessment and conditions for its implementation. In the proposed model student self-assessment is fully connected to the systematic assessment of the teacher and gains a significant importance for self-directed learning. Both assessing processes (teacher assessment and pupil self-assessment) support the student´s motivation and efforts to achieve the set goals and encourage teachers to select appropriate strategies developing the personality of the pupil.

While population is aging we are facing raising number of geriatric problems. It is obvious nowadays that if we do not start creating certain plans considering medical and social care of elderly people, the situation may get out of control. The natural course of diseases in elderly is different to middle aged and young people. These individuals show different symptoms and social background, different prognosis and complications of the disease and of diagnostic and therapeutic medical procedures themselves. The major feature that distinguishes geriatric medicine from others is comprehensive geriatric assessment which is holistic and considers not only medical condition but also social, psychological background together with identifying aspects of functional status, nutrition, transportation and others. It seems that the need for professionals with experience in geriatrics is growing. The most important in this area are general practitioners and geriatricians. There is also a constant need for educational effort to create society whose members are responsible for their own health.

The Vulnerable Elders Survey (VES-13) is a simple function based frailty screening tool that can be also administered by the nonclinical personnel within 5 minutes and has been validated in the out- and in patient clinic and acute medical care settings. The aim of the study was to validate theaccuracy of the VES-13 screening method for predicting the frailty syndrome based on a CGA in polish surgical patients. Material and methods. We included prospectively 106 consecutive patients ≥65, that qualify for abdominal surgery (both due to oncological and benign reasons), at the tertiary referral hospital.We evaluated the diagnostic performance of VES-13 score comparing to the results from the CGA, accepted as the gold standard for identifying at risk frail elderly patients. Results. The prevalence of frailty as diagnosed by CGA was 59.4%. There was significantly higher number of frail patients in the oncological group (78% vs. 31%; p<0.01). According to the frailty screening methods, the frailty prevalence was 45.3%. The VES-13 score had a 60% sensitivity and 78% specificity in detecting frailty syndrome. The positive and negative predictive value was 81% and 57%, respectively. The overall predictive capacity was intermediate (AUC=0.69) Conclusions. At present, the VES-13 screening tool for older patients cannot replace the comprehensive geriatric assessment; this is due to the insufficient discriminative power to select patients for further assessment. It might be helpful in a busy clinical practice and in facilities that do not have trained personal for geriatric assessment.

Prognozy demograficzne wskazują, że najszybciej rosnącą grupą wiekową pacjentów na świecie są osoby powyżej 65. roku życia. Szacuje się, że ich odsetek w 2050 r. osiągnie ok. 31,3% populacji Polski. Oznacza to, że ludzie w wieku podeszłym będą dominować wśród pacjentów zgłaszających się do lekarzy wszystkich specjalności. Opieka medyczna w okresie starości powinna być ukierunkowana na indywidualne potrzeby pacjenta. Prawidłowe zdiagnozowanie objawów, określenie potrzeb zdrowotnych seniora we wszystkich wymiarach determinuje dalsze postępowanie medyczne. Narzędziem ułatwiającym to zadanie jest całościowa ocena geriatryczna (COG). To wielodyscyplinarne narzędzie, a zarazem złożony proces diagnostyczny, składający się z szeregu skal, którego celem jest określenie zakresu zaburzeń zdrowia seniora, jego zdolności do samodzielnego funkcjonowania, potrzeb i problemów na płaszczyźnie fizycznej, psychicznej, socjoekonomicznej oraz środowiskowej. Główne skale znajdujące zastosowanie w COG to: ADL, IADL, Barthel i inne służące do oceny stanu funkcjonalnego seniora. W ramach COG ocenia się także stan psychiczny chorego, stosując skalę MMSE , skrócony test sprawności umysłowej Hodgkinsona, test rysowania zegara, skalę Yesavage’a i inne. Z kolei ocena socjalno-środowiskowa ma na celu zdefiniowanie potrzeb osoby w wieku podeszłym w zakresie opieki doraźnej oraz długoterminowej.
Demographic projections show that the fastest growing age group of patients in the world are people over 65 years of age. For instance, it is estimated that in 2050 their share will reach approximately 31.3% of the Polish population. This means that the elderly will certainly dominate among patients visiting doctors of all specializations. Health care at old age should be focused on individual needs of the patient. A right diagnosis of symptoms, identifying the health needs of seniors in all dimensions, determines application of further medical procedures. A tool that facilitates this task is Comprehensive Geriatric Assessment (CGAGA). CGAGA is a multidisciplinary tool and also a complex diagnostic process, consisting of a series of scales, the aim of which is to determine the scope of seniors’ health disorders, their ability to operate independently, their needs and problems encountered in the physical, psychological, socio-economic and environmental spheres. The main scales which apply in the CGAGA are: ADL, IADL, Barthel and others which serve to assess the functional capacity of the senior. As part of the CGAGA one can also evaluate the psychological state, using MMSE , Hodgkinson test, CDT, scale of Yesavage and others. In turn, the social-environmental assessment is meant to define the needs of elderly patients, which are connected with emergency interventions and long-term care.
