ISSN 2300-0422. DOI 10.14746/kse.2018.14.13. This study discusses the ethical issue of the development of hearing technology in the form of cochlear implants that are used by an ever-growing number of deaf and hard of hearing people. The development of this kind of technology has progressed so quickly all over the world, including Poland, that the deafness paradigms have drastically changed. This may affect, to a greater or lesser degree, not only the implementation of early support programmes, but also the image of the Deaf communities using their respective sign languages on a daily basis. It is therefore necessary to create an eclectic model of education for deaf and hard of hearing people which would allow for the ethical option to integrate the conflicting approaches on the proper procedure for deciding on the language (spoken or signed) and the culture (the hearing or the Deaf one) for the development of deaf and hard of hearing students with cochlear implants or hearing aids.

The main issue addressed in this paper is a practical application of sign language in early development support for deaf and hard of hearing children in Poland. This discussion is set in a broad cultural, linguistic, psychological and rehabilitation context. This paper presents the concept of Mary Pat Moeller which includes four approaches to the use of sign language in the programming of language in DHH. The discussion on the possibility of Polish rehabilitation centres adopting these solutions required an explanation of the differences between auditory deprivation and language deprivation, which is another important issue addressed in this paper. The main aim of this publication is to propose a flexible approach to the creation of a model of education for deaf and hard of hearing children, one that will completely fulfill the needs of a specific child and to substantially argue about the conviction that sign language is a factor potentially detrimental to the development of these children.

Celem artykułu jest rozważenie problemu rehabilitacji dzieci z implantami ślimakowymi na obecnym etapie rozwoju systemu edukacji specjalnej w Ukrainie. Autor porusza problematykę wszczepiania implantów ślimakowych, rehabilitacji i treningu prowadzonego z dziećmi z implantami ślimakowymi w nowoczesnych warunkach, a także przedstawia dane statystyczne Państwowej Służby Statystycznej Ukrainy, główne aspekty wykrywania ubytków słuchu w różnym stopniu, wczesną pielęgnację (badanie przesiewowe) słuchu oraz wykorzystanie nowoczesnej technologii w rehabilitacji osób z wadą słuchu (implantacja ślimakowa). W artykule opisano główne wskaźniki rozwoju słuchu u dziecka po wszczepieniu implantu ślimakowego oraz znaczenie kompleksowego podejścia do rehabilitacji, której ważnym elementem jest przeszkolony specjalista. Autor zwraca uwagę na fakt, że w Ukrainie z roku na rok wzrasta liczba użytkowników implantów ślimakowych, ale szkolenie specjalistów do pracy z tą kategorią dzieci prawie nie istnieje, ponieważ w procesie kształcenia surdopedagogów ten obszar jest traktowany powierzchownie. Dlatego przy wsparciu organizacji pozarządowej powstał projekt „Rehabilitacja osób z implantami ślimakowymi w Ukrainie”, którego celem jest szkolenie specjalistów do pracy z takimi dziećmi. Dziś wiedzę w tym zakresie otrzymuje około 500 specjalistów. Autor opracowania podkreśla konieczność wdrożenia programów korekcyjnych rehabilitacji dzieci po wszczepieniu implantu ślimakowego, opracowanie zaleceń metodycznych dla specjalistów i rodziców oraz literatury dotyczącej kwestii wychowawczych i rehabilitacyjnych.
The aim of the article is to consider the problem of rehabilitation of children with cochlear implants at the current stage of development of the special education system in Ukraine. The author discusses the issues of cochlear implantation, rehabilitation and training to work with children with cochlear implants in modern conditions, and presents statistical data of the State Statistics Service of Ukraine, main aspects of the detection of hearing loss at various stages, the occasional development and the modern method of hearing aids (slime implantation). The article describes the main indicators of hearing development in a child after grafting an implant and the significance of a complex approach to rehabilitation, an important element of which is the training of a specialist. The author draws attention to the fact that in Ukraine the number of users of cochlear implants is increasing year by year. However, the training of specialists to work with this category of children is not true, but in the process of teaching deaf educators, it is still treated confidentially. Therefore, with the support of a non-governmental organization, the project “Rehabilitation of persons with cochlear implants in Ukraine” was created, the aim of which is to train specialists to work with such children. Today, about 500 specialists receive knowledge in this district. The importance and finality of the implementation of correctional programs in the process of slime implantation, treatment of methodical treatments for specialists and relatives, and literature on the direction of educators and educators were also emphasized.

The publication presents the latest trends in indications for cochlear implantation in children. The principles of early detection of hearing loss in children as well as the detailed methodology and tools for diagnosis of a child before cochlear implant were discussed. Based on a review of publications from recent years, reference was made to the latest trends in implantation, taking into account: electroacoustic stimulation, electronatural stimulation, unilateral deafness, asymmetric sensorineural hearing loss, early implantation in children under 12 months of age and indications for brainstem implants in children with inner ear malformations . The significance of residual hearing in patients with cochlear implants as well as the use of appropriate electrodes for a specific clinical situation were also discussed. These factors may influence future development of speech and hearing.

Early implantation and hearing and speech rehabilitation provide much wider opportunities to develop linguistic and communication skills for children with hearing impairment. The article presents own research, the aim of which was to determine the level of language competence of children with prelingual hearing deafness, which were implanted with a cochlear implant until the second year of life. The study involved a group of 169 children with prelingual hearing deafness. The tests used 6 Ling’s sounds, the MAIS scale, the MUSS scale and the TAPS test.

Severe hearing damage in the prenatal period or when the baby is born is a disability that significantly impairs the correct functioning in the society of hearing people. A particularly perceptible consequence of severe hearing impairment is the lack or significant delay in the development of speech and language acquisition. Thanks to the use of cochlear implants, the Program Chirurgicznego Leczenia Głuchoty Metodą Wszczepów Ślimakowych [Program of Surgical Treatment of Deafness With the Method of Cochlear Implantation] allows children with perceptive deafness for the access to speech sounds through the auditory pathway. It gives them an opportunity to develop speech and language and in the future – the ability to satisfactorily communicate with other people. The study covered 54 born-deaf children, bilaterally implanted in the sequential mode. The implantation was performed at the Department of Otolaryngology and Laryngological Oncology of the Poznan University of Medical Sciences. The results indicate a constant, dynamic increase of auditory and linguistic-communication skills in the examined group.

Artykuł zawiera teoretyczne wyjaśnienie oraz syntetyczne podsumowanie doświadczeń praktycznych i badawczych zgromadzonych w toku wieloletniej działalności logopedycznej i pedagogicznej oraz eksperymentów naukowych i analiz badawczych związanych ze stosowaniem w wychowaniu językowym dzieci z uszkodzeniami słuchu Metody Fonogestów (Cued Speech). Metoda ta jest obecnie używana w wielu krajach, najczęściej komplementarnie ze stosowaniem protez słuchowych (zwłaszcza implantów ślimakowych) jako sposób ułatwiania słuchania i wspierania naturalnego procesu przyswajania języka fonicznego. Stosując ją konsekwentnie, można zaobserwować pełną zgodność percepcji słuchowo-wzrokowej dźwięków mowy z wykorzystywaniem gestów pomocniczych, które wspomagają czynności odróżniania oraz identyfikacji głosek w sylabach w sposób ściśle odpowiadający ich wzorcom fonemowym. Fonogesty to gesty, które wzbogacają liczbę cech dystynktywnych poszczególnych głosek dodatkowymi oznaczeniami gestowymi, dostępnymi dla wzroku i ułatwiającymi odróżnianie i rozpoznawanie głosek w sylabach pojawiających się w szybko płynącym strumieniu mowy. Jest to możliwe na drodze uważnego słuchania i jednoczesnej obserwacji widocznych ruchów artykulacyjnych oraz specjalnych – prostych, celowych i dyskretnych – ruchów ręki wykonywanych przez rozmówcę w pobliżu ust. Ruchy ręki towarzyszą wymawianym sylabom w sposób eurytmiczny.
The study contains a theoretical explanation and a synthetic summary of practical and research experiences accumulated during many years of logopedic and pedagogical activities and scientific experiments involving the use of the Cued Speech method in linguistic upbringing of hearingimpaired children. This method is currently used in many countries, most often as complementary with the use of auditory prostheses (particularly cochlear implants) as a way to facilitate listening and to support the natural way of phonic language acquisition. Through its consistent application, one can observe a full conformity of auditory-visual perception of speech sounds with the use of auxiliary cues (gestures) that support the functions of differentiation and identification of sounds in syllables in the way that exactly corresponds with their phonemic patterns. Cued speech are cues (gestures) that enrich the number of distinctive features of individual sounds with additional gestural signs accessible to sight, which facilitate differentiation and recognition of sounds in syllables that appear in rapid speech flow. This is possible by listening attentively and by simultaneously observing visible articulatory movements and special – simple, deliberate and discreet – hand movements made by the speaker near the mouth. Hand movements accompany the pronounced syllables in a eurhythmic way.

The objective of the study is to assess the hearing performance of cochlear implant users in three device microphone configurations: omni-directional, directional, and beamformer (BEAMformer two-adaptive noise reduction system), in localization and speech perception tasks in dynamically changing listening environments. Seven cochlear implant users aided with Cochlear CM-24 devices with Freedom speech processor participated in the study. For the localization test in quiet and in background noise, subjects demonstrated significant differences between different microphone settings. Confusion matrices showed that in about 70% cases cochlear implant subjects correctly localized sounds within a horizontal angle of 30–40◦ (±1◦ loudspeaker apart from signal source). However localization in noise was less accurate as shown by a large number of considerable errors in localization in the confusion matrices. Average results indicated no significant difference between three microphone configurations. For speech presented from the front 3 dB SNR improvements in speech intelligibility in three subjects can be observed for beamforming system compared to directional and omni-directional microphone settings. The benefits of using different microphone settings in cochlear implant devices in dynamically changing listening conditions depend on the particular sound environment.

Over thirty years of progress in development of surgical techniques and cochlear implant technology led to a change in approach and extending of indications for qualifying patients to treatment of sensorineural hearing loss. Basic indications for cochlear implants include bilateral deafness, bilateral residual hearing or profound bilat-eral sensorineural hearing impairment as well as lack of acoustic gain from classic air conduction hearing aids af-ter a minimum of 5-6 months of use. Methods of subjective and objective hearing assessment are applied. Psycho-logical evaluation is aimed at assessing expectations of the patient/child’s parents, motivation, and intellect with regard to the presence of deficits that might pose difficulty in the course of further rehabilitation as well as accep-tance of this new type of stimulation. Speech and language evaluation takes into consideration the current level of speech development, articulation disorders related to hearing impairment, and the way to communicate with the environment. Final qualification requires imaging diagnostics of the head/ears (CT and/or MRI). Our surgi-cal experience allowed for implanting children with various congenital anomalies of the inner ear. Inflammation within the tympanic cavity and air-filled spaces of the mastoid process might require removal of inflammatory foci prior to surgery or selection of different surgical approach.

The model of “crisis” is a culturally shared and widespread idea of human reactions to misfortunes such as accidents, diagnosis of disease, divorces, becoming a parent of a disabled child, and so on. The crisis model conveys the idea of coming to terms with unwanted experiences while advancing through various phases, for example, of denial, processing, and acceptance. The language of crises is integrated into Western emotional culture, particularly in the language used by caring professionals (e.g., social workers, psychologists, counselors, and health staff). Crisis talk is also frequent in the media, popular science books, and in everyday conversations when individual experiences are reported, debated, or discussed. Investigating the specific local culture of the Swedish world of the Deaf, to which families whose children have been diagnosed deaf belong, we aimed to extend the current understanding of crisis. How do parents and professionals make use of the crisis model when speaking about their own experiences, as well as the experiences of others? We observed that the crisis model served as a prop in such talk; it was used to compare, defend, criticize, and explain the behavior of others, but also to account for one’s own emotions and behavior. In the process, locally relevant identities and categorizations of others were constructed. The crisis model was originally a way of “diagnosing” parents’ emotional experiences when they learned about their children being deaf, but it has proven useful for other purposes in a context with abundant ideological differences.

The article focuses on hearing assistive technologies for hearing impaired people, and analyzes them in terms of the development of Deaf Culture. The culture of this community manifests itself through their attachment to the use of sign language and in various artistic activities which strengthen the identity of the members and supporters. With various assistive technologies available (particularly hearing aids and cochlear implants), a cultural dilemma arises related on the one hand to the cyborgisation and on the other hand to the neo-Luddism of the deaf and hard of hearing. Most of them avail of technology, but at the price of a certain level of cyborgization. However, there is a minority in Deaf Culture which, wishing to preserve its Deaf identity, rejects technology, thus subscribing to the technophobic process of opposing technological devices and machines, initiated by the Luddites in the 19th century.

Introduction: The use of cochlear implants (CI) has been a remarkable success in reducing disabilities in patients with impaired hearing. The definition of success for those patients means improvement of hearing ability for adults, possibility to develop speech and language for children, quality of life improvement and satisfaction with the whole CI implantation procedure. The aim: To examine patient satisfaction, determined by their place of residence, with all activities in progress during CI implantation including care in subsequent years. Material and methods: The study was conducted prospectively. Online surveys were sent to 1,906 CI patients, with the response rate reaching 33%; thus 630 surveys were analyzed. Demographic data of the respondents were collected: gender, age of implantation, one- or two-sided implantation, place of residence and implanting clinic. A detailed statistical analysis of the obtained data was performed. Results: One of the problems observed was the aspect of waiting time for qualification, implantation and replacement of the speech processor. The second problem was the travel time to the CI center and that was strictly connected with the place of the residence of a patient. Patients’ satisfaction with the control visit was high for majority of them. Conclusions: This research highlighted the limitations of the current CI service delivery. Significant differences concerning the CI pathway were found for particular regions of Poland. One of the future goals for healthcare providers should be to level out the differences in access to CI services between different regions of Poland.

Introduction: Although it is recommended to perform cochlear implantation in both ears at the same time for management of profound hearing loss in children, many centers prefer to perform sequential implantation. There are many reasons as to why a simultaneous bilateral implantation is not commonly accepted and performed. The major risk is the possibility of bilateral vestibular organ impairment. However, it is beyond doubt that children who received the first implant should be given a chance for binaural hearing and associated benefits. In the literature, there are no homogenous criteria for bilateral implantation, and it is hard to find uniform and convincing algorithms for second cochlear implantation. The aim of this study is an attempt to identify a safe way of qualifying for second cochlear implantation in children. Material and methods: Forty children with one cochlear implant were qualified for the second implantation. During qualification, the following were taken into account: time of the first implantation, audiometry results, use of the hearing aid in the ear without an implant and benefit of the device, speech and hearing development, and vestibular organ function. R esults: Fifteen out of forty children (38%) were qualified for the second implantation. In 35% of children, the decision was delayed with possible second implantation in the future. Eleven children (27%) were disqualified from the second surgery. Discussion: During evaluation according to the protocol presented in our study, 38% of children with a single cochlear implant were qualified for the second implantation with a chance for an optimal development and effective use of the second cochlear implant. We are convinced that sequential implantation with a short interval between surgeries and with an examination of the vestibular organ, hearing and speech development as well as an assessment of potential benefits from the second implant (bimodal stimulation) before the second implantation is the safest and most beneficial solution for children with severe hearing loss.
