Introduction and objective. Currently, most women with less advanced breast cancer are offered operations with breast conserving treatment; however, if it is necessary to completely remove the breast, the patients may benefit from free breast reconstruction. The aim of the study was to determine the motives for breast reconstruction after mastectomy or decisive reasons for rejecting this treatment. Material and methods. The study included 241 women hospitalized in Polish oncological hospitals who were divided into two groups: 55.19% (n=133) were women after mastectomy; 44.81% (n=108) after breast reconstruction. Quantitative approach with the use of survey method was utilised. Results. Women who wished to undergo breast reconstruction were generallyy younger, better educated and more often professionally active. For women who wished to undergo breast reconstruction, the most important aspect was the willingness to improve their body image, their mental well-being, the inconvenience connected with wearing the prosthesis, better sexual relations with husband/partner, and the possibility of exposing the neckline without discomfort. The strongest correlation was found between women’s young age and their functioning in a relationship. Women after mastectomy, who decided not to undergo a breast reconstruction, were afraid of another operation, suffering, and they believed that there were more important issues. Conclusions. The study revealed a relationship between the lack of knowledge, in which hospital breast reconstruction procedure can be performed and the lack of motivation for undergoing the procedure. Women who have had knowledge of the possibility of a free breast reconstruction were younger, better educated and more often lived in the city.

Introduction. Neoadiuvant therapy (NCT) in the treatment of breast cancer is employed for patients with early stage disease or with inoperable disease. NCT can decrease the tumor volume. It can facilitate breast conservation therapy. Response to NCT is a strong predictor of outcome breast cancer (BC). Direct target therapies has markedly improved the result of treatment BC. Aim. Therapy for breast cancer continues to improve. The importance of tumor burden on local control rates will be in the future. Material and methods. This analysis was performed using a systematic literature search. Results. The latest scientific reports give hope for greater safety and a better life for patients based on optimized and effective therapy. Conclusion. Currently, improving the effectiveness of breast cancer treatment is mainly related to the optimal use of classic therapeutic strategies. New classes of substances have been approved for treatment or are in advanced stages of clinical development.

Breast and cervical cancer represent a significant health and economic issue for Polish society, although if detected early, both can be cured successfully. For this reason, since 2006, according to the National Cancer Combat Programme, populationbased screening programmes have been implemented, aimed at reducing the mortality and morbidity for breast and cervical cancer. The aim of this study is to determine which of the selected four environmental factors affect attendance for screening mammography and cytology. Analysis included data from questionnaires filled in during mammography by 582,959 women aged 50–69 years, and 288,142 women during cytology, aged 25–59 years, in 2007–2012 in the Wielkopolska Province of Poland. It was found that the impact of medical staff on the attendance for cytological screening was the strongest statistically significant factor (p = 0,0001). Invitation by name (p=0,001) and other factors (p= 0,0001) also affected the attendance. In the cytological screening, medical staff was the factor that had the greatest impact on attendance. Other factors, such as self-reporting, increased participation in the next screening rounds, although the factors that affect attendance changed over time. Their constant analysis is essential for the efficient and effective evaluation of screening programsme.

Introduction. Breast cancer is one of the most frequently reported cases of cancer among women. Prevention and a healthy lifestyle offer a significant opportunity to avoid mastectomy which is the most radical form of treatment. The aim. The aim of the work is to present the review of literature on health promotion, prevention and the way women cope with everyday life after surgical removal of the breast. Overview. A number of factors contribute to the development of breast cancer. A crucial aspect of prevention is the enhancement of pro-health behaviors among women, which helps to reduce the risk of disease. Mastectomy is one of many methods of treatment for breast cancer. The quality of life of women after mastectomy is significantly affected. The most common problem is the lack of self-acceptance, a changed body image. Conclusions. Statistics show a substantial increase in the burden of cancer and the number of deaths from breast cancer. The results of the research among post-mastectomy women indicate huge changes in the mental and physical sphere of their functioning.
Wstęp. Rak piersi jest jednym z najczęściej odnotowywanych przypadków nowotworów wśród kobiet. Profilaktyka oraz zdrowy styl życia stwarzają szansę na unikanie radykalnej formy leczenia jaką jest mastektomia. Cel. Celem pracy jest przegląd piśmiennictwa obejmujący zakres promocji zdrowia, profilaktyki oraz sposobu radzenia sobie kobiet po chirurgicznym usunięciu piersi. Przegląd. Szereg czynników sprzyja rozwojowi raka piersi. Bardzo istotnym aspektem profilaktyki jest potęgowanie zachowań prozdrowotnych wśród kobiet, co sprzyja zmniejszeniu ryzyka wystąpienia choroby. Mastektomia jest jedną z wielu metod leczenia raka piersi. Jakość życia kobiet po mastektomii ulega znacznej zmianie. Najczęściej występującym problemem jest brak akceptacji własnej osoby, zmienionego obrazu ciała. Wnioski. Dane statystyczne wskazują na przyrost liczby zachorowań i zgonów z powodu raka piersi. Wyniki badań wśród kobiet po mastektomii wskazują na ogromne zmiany w sferze psychicznej oraz fizycznej ich funkcjonowania.

In order to present a reliable picture of hormone-dependent breast cancer treatment in Poland, an on-line survey has been conducted in 19 oncology centres. As a result, data on the treatment of 486 patients have been obtained (405 of them initially presenting with stage I–III of disease advancement, and 81 representing stage IV). It has been concluded that in the majority of cases the treatment in question involves combined therapy, including chemotherapy, hormonal therapy, targeted molecular therapy, radiotherapy, and surgical methods with reference to the group of patients subjected to radical treatment.

Objective. The assessment of knowledge about prevention and risk factors of women with diagnosed breast cancer. Material and methods. The study involved 100 women aged from 28 to 82 years (median 56.2 years) who were treated at the Oncology Center in Bydgoszcz in 2014 for breast cancer. Author’s original questionnaire containing questions on knowledge about breast cancer as well as environmental and demographic data was used in this research. Results. Self-examination of a breast was made by 83% of women, 85% of women benefited from invitations for mammography. 56% of women knew how often mammography should be repeated but only 28% knew at what age it should be started. Tumor in a breast (20.5%) and “pulling” nipples (14.4%) were the most frequently reported symptoms of a cancer. In terms of risk factors, genetics (32.2%) and unhealthy lifestyle (16.3%) were the factors pointed by women most often. Every third women indicated only one factor while every four indicated three factors. 69% of the respondents increased their knowledge after falling ill, from the Internet (20.5%) as well as from information leaflets and brochures (16.6%). Women’s knowledge about breast cancer was at the edge of low and average levels and the average value was 11.7 points. More knowledge had women with higher education 12.95 points, single 12.9 and young women 12.3. Conclusions. The knowledge about the disease among women with diagnosed breast cancer was unsatisfactory and did not depend on age, marital status, place of residence, type of work but on education level only. Older and uneducated women had less knowledge, particularly about factors increasing the risk of developing the disease. The Internet, information leaflets and brochures were the primary source of knowledge about the disease, not a doctor or a nurse. Women with breast cancer were aware of the importance of preventive tests. They declared that they can perform self-examination. The most frequently reported symptoms of cancer are tumor and “pulling” nipples.

Radiotherapy in breast cancer patients is an important component of multidisciplinary treatment. It reduces the risk of local recurrence and mortality from breast cancer. However, it can lead to secondary effects due to the presence of the heart within the irradiation field. Adjuvant radiation therapy for breast cancer increases the risk of coronary artery disease, myocardial infarction and cardiovascular death. It is important to determine the optimal treatment to minimize cardiotoxicity. Modern radiotherapy techniques may reduce radiation-induced cardiac toxicity, but it is necessary to determine the most sensitive structures within the heart, tolerance doses, and methods for early detection and monitoring of adverse effects.

The aim of the study was to analyze clinicopathological features in breast cancer patients with local recurrence (LR). Material and methods. A retrospective analysis of database of breast cancer patients operated on in the Department of Surgical Oncology in Łódź from 2 January 2009 to 30 June 2013, identified 1080 women with primary breast cancer and 11 patients with LR. Results. LR rate was 0.23% per year. True recurrence (TR) occurred more frequently in patients with luminal B molecular subtype, in HER-2 positive and in triple-negative subgroups. In one patient with luminal -A subtype new primary (triple negative) occurred. TR were noted predominantly in patients with axillary lymph nodes metastases and with luminal B subtype who did not receive adjuvant chemotherapy but were given only endocrine therapy. LR were observed more frequently in patients who did not receive adjuvant radiotherapy or this treatment was delayed. Minimal surgical margins in postoperative specimens measured by pathologist were 4-25 mm, mean 9.5 mm. Conclusions. The LR rate in patients operated on breast cancer in the Department of Surgical Oncology between 2009 and 2013 was low. TR was diagnosed in patients with non- luminal A breast cancer despite wide surgical margins, especially if the patients did not receive optimal adjuvant systemic treatment or radiotherapy was delayed or omitted. Complete cancer excision followed by an immediate implementation of optimal adjuvant treatment seems to be crucial especially in patients with poor tumor biology.

Breast cancer is the most common cancer among females worldwide. The mean age of breast cancer patients is > 60 yrs old, and it is seldom found in women < 40 yrs old (6.5%) and in very young women < 35 yrs old (0.6%). In young females, fertility and all its aspects are an additional therapeutic challenge. Before initiating treatment, the oncologist should offer effective contraception to be applied throughout the therapy, bearing in mind that fertility preservation is of utmost importance. When it comes to breast cancer in pregnancy, the attending physician should use a therapy which is safe for both the mother and the foetus. Chemotherapy, radiotherapy, hormonal therapy and immunotherapy can, to a lesser or greater degree, damage the ovarian function resulting in amenorrhea in women < 50 yrs (33–76%). However, owing to fertility preservation strategies, more and more pregnancies are successful, even in breast cancer survivors.

Introduction. According to statistics, breast cancer is the second leading cause of death in Poland. Progress in treatment and diagnosis gives an opportunity of a quick diagnosis, but women are reluctant to undergo prophylaxis screening. Aim. To identify women’s attitudes about breast cancer prophylaxis. Material and methods. The diagnostic survey was conducted in a group of 200 women. The research tool was the questionnaire developed by the authors. Results. Knowledge about breast cancer and prophylaxis was on an average level. Nearly half of the women surveyed (45.5%) declared that they do not ask for a breast examination while visiting a gynecologist. Only 26.8% of the surveyed women were systematically subjected to preventive examinations for breast cancer, while 18.7% of them performed breast self-examination. Every fifth respondent used invitation for a free mammogram. Conclusion. The women’s knowledge about breast cancer is average, but it does not translate into their attitudes towards the prevention of this cancer. Most women niether perform breast self-examination nor benefit from free prophylaxis programs. Younger respondents most often use the Internet. Education and place of residence do not affect the participation of women in preventive examinations for breast cancer.

Breast cancer is the most common malignancy in women in Poland and the entire world, but thanks to early diagnostics and efficacious therapeutic methods, its prognosis is good. In some patients with advanced breast cancer, long-term survival is observed, which is why it is essential to select the most appropriate therapy, and to follow up on the course of disease and possible adverse events related to the treatment applied. The slow development of hormone-receptor-positive cancer, and a multitude of therapeutic options constitute a real challenge in palliative treatment. The use of fulvestrant resulted in a good response in both of the presented cases, and did not in any way comprise the daily lives of the patients over a long period of time.

Liposomal doxorubicin is a newer form of chemotherapeutic agents that, due to its own special properties, preferably accumulates in cancer tissue. On the other hand, it shows lower affinity to cardiomyocytes and in this way is less cardiotoxic. As a result of that, there is the possibility to use liposomal form of doxorubicin until disease progression or chemotherapy intolerance in palliative setting, without treatment cessation after reaching the maximum cumulative dose of conventional doxorubicin. In this article we describe the case of a female patient diagnosed with breast cancer who was primary treated with adjuvant treatment, including chemotherapy and in whom a disease recurrence occurred after seven years of observation. As a primary palliative treatment the patient received chemotherapy based on liposomal doxorubicin and cyclofosphamide with a very good tolerance. The initial response was partial remission in lungs and in mediastinal lymph nodes. During the whole course of therapy there were no pathological changes in electrocardiogram, no signs and no symptoms of congestive heart failure, and the left ventricular ejection fraction was within normal limits.

Progression during endocrine therapy is unavoidable. Administration of mTOR inhibitor gives a chance of reversing the acquired resistance. The achieve pathologic complete response is proven to be the most important parameter of prognosis. Thereports evaluating the impact of obesity on the obtained pathologic response to chemotherapy are unequal. The aim of the study was to evaluate in locally advanced breast cancer patients, treated with AT(doxorubicin plus docetaxel) neoadjuvant chemotherapy: 1. The relationship of obesity with obtaining pathological response. 2. The relationship of obesity and free of disease recurrence survival (DFS) and overall survival (OS) associated with the tumour. Material and methods. A retrospective study was carried out in a group of 105 patients with locally advanced breast cancer, treated with AT neoadjuvant chemotherapy and then treated with radical surgery. Two variants of pathological response have been adopted: a pCR (T0N0) and pCR1 (TisN0, TxN1, T1N0, T1N1, T0N1). The relationship of obesity with pathological response and survival was investigated. Results. In univariate analysis the pCR1 was obtained with its arising from the borderline of statistical significance with lower incidence of obesity. In pCR1 multivariate analysis, negative pCR1 relationship with obesity was on the borderline of the statistical significance. The multivariate analysis showed a significant negative association OS with obesity (p=0.047) and positive with the occurrence of menopause (p = 0.029). Conclusions. In patients with locally advanced breast cancer treated with AT neoadjuvant chemotherapy. 1. Obesity seems to be an independent and unfavourable predictor of the lack of obtaining pCR1 pathological response 2. In the multivariate analysis, the obesity was a significant independent factor related to shorter OS.

Incidence of female breast cancer in our population remains very high and the incidence increase every year. Breast cancer and following demanding oncological treatment extensively influence life of female patients. It intervene biological, psychical and social life of patient. Objective of our research was to check how women are informed about preventive precautions and how they are following them. Our research sample were randomly selected women of all age groups. Our results showed that it is necessary to increase activity in the area of education, increase personal responsibility in area of breasts self-examination and better inform about the influence of obesity to breast cancer incidence. Considering our previous experience in health care we were surprised that the role of nurse in education process is very low. We think that general nurses have enough opportunities to use daily contacts with patients to inform and lecture them.

We discuss limitations of the known methods of IR imaging in diagnostics of breast cancer. In conclusion we show that the known methods, based on simple observation of external temperature distribution, are not fully effective. Even advanced pattern recognition could not help in analysis of static images. May active dynamic thermography, known in non-destructive testing of materials, be of any help in breast cancer diagnostics? Analysis of thermal transients forced by external thermal excitation shows, even on simple models, that one may expect a visible improvement in resolution after such excitation. Applied models allow analysis of both static and active thermograms. Basing on the models one may recognise elements of the internal structure of a breast not visible in static pictures. This method, new in clinical practice, seems to be promising, but requires further studies.

Introduction: Breast cancer is the most common malignant tumor in women in the Polish region. Surgery is a basic method of breast cancer treatment. Surgery often carries a lot of unwanted changes as follows: limitation of mobility in the shoulder joint on the operated side, secondary lymphoedema, post mastectomy pain syndrome (PMPS), reduction of muscle strength or disorders in body posture. Therefore, the implementation of physiotherapeutic activities that are designed to prevent and eliminate postoperative complications seems very important. The main aim of this work was to present physiotherapeutic management in women after mastectomy based on the analysis of available literature. The physiotherapeutic process can be divided into three periods: early hospital, early ambulatory and the late ambulatory period. In the first period, active slow exercises, self-support of the upper limb on the operated side and breathing exercises on the thoracic track are used to prevent circulatory disorders, pulmonary complications, and edema. The early ambulatory period includes corrective exercises, general improvement exercises, stretching and learning of automatic massage of the upper limb of the operated side. The last period should be enriched by recreational methods of physical activity such as swimming, cycling or Nordic walking to maintain physical fitness, proper mobility of the shoulder girdle and improve the patient's psychophysical state. It is very important the patient regular continues the rehabilitation program after curing of breast cancer as well. In the case of secondary lymphoedema of the upper limb, comprehensive rehabilitation physiotherapy is used, consisting of manual lymphatic drainage, healing exercises, compression therapy, and skin care. Conclusions: Physiotherapy in women after breast cancer surgery is a complex and long-term process. Physiotherapeutic methods are effective in treating complications after surgery of breast cancer surgery. It is necessary to constantly update the physio-therapy knowledge in women after breast cancer surgery.

Introduction. Breast cancer is the most frequent neoplasm among women. That is the reason why scientists all over the world are attempting to improve early detection methods of this particular malignancy. Aim. The most common and most widely used examination methods for screening for and detecting breast cancer is presented herein. Material and methods. This review was performed according to systematic literature search of three major bibliographic databases. Results. Available data suggest that incidence and mortality in high-resource countries has been declining whereas incidence and mortality in low-resource countries has been increasing. Conclusion. The role of a physician is to select the most suitable one for each patient in order to obtain the best result. No matter the method however, between 2005 and 2011, the 5-year relative survival was found to be 89%. This is thought to be due to both the increase in utilization of population-wide screening, as well as advances in treatment.

Breast cancer often requires combined oncologic treatments, the base of which is surgery. Quality of life (QoL) after each surgical procedure may influence the process of decision making among women, who qualify for multiple oncological strategies. Our knowledge about QoL in breast cancer patients is derived from comparative studies. Results may differ, depending on country, culture, and societal relations. The aim of the study was to investigate the quality of life of Polish patients treated with breastconserving therapy (BCT) or mastectomy with breast reconstruction. Material and methods. The study involved women who underwent surgery for breast cancer in the Department of Surgical Oncology of the Gdynia Oncology Center from September 2010 to November 2013. Eighty-two breast reconstructions (in 79 patients) and 226 BCT procedures were performed. QoL was measured with the use of EORTC QLQ-C30 and QLQ-BR23 questionnaires. Results. Global QoL was high in both groups and did not differ significantly. Body image was slightly better after BCT than after mastectomy with breast reconstruction, but sexual QoL was lower. Future perspective was quite low in both groups. Disease symptoms were not bothering. Conclusions. The global QoL among Polish breast cancer patients treated with BCT or mastectomy with breast reconstruction is high and does not differ between groups. There is a need for anxiety and disease-related fear prophylaxis and for the improvement of sex life of breast cancer survivors.

Coffee is an important dietary source of biologically active components, not to mention caffeine, phenolic acids, and diterpenes. It has been suggested that selected coffee secondary metabolites may beneficially modulate several mechanisms of anti-cancer protection. This literature review was intended to present current knowledge related to coffee and its components and hormone-dependent female cancers, such as breast, endometrial and ovarian cancer, and to identify gaps in research that may be exploited in the future. The search for studies was conducted through electronic databases. Publications on coffee composition, coffee preparation and brewing methods, in vitro and in vivo experiments with the use of substances naturally present in coffee, observational studies, and meta-analyses were collected. In population studies, the greatest attention has been paid to the anticancer effect of caffeinated coffee and/or caffeine. In general, most studies and meta-analyses indicated that there was no clear correlation between coffee and breast cancer or ovarian cancer. Some subgroups of women may benefit from coffee consumption. This is the case for post-menopausal women with regard to the risk of breast cancer and obese women with regard to the risk of endometrial cancer. This paper identifies a number of issues for future research, related to a better understanding of the anti-cancer mechanisms of coffee compounds and further research that would focus on specific target groups, taking into account both the different methods of coffee preparation and lifestyle factors that may influence the results.

Introduction. Breast cancer is the second most prevalent reason for cancer deaths after lung cancer. Incidence rates have increased worldwide. Aim. To present epidemiology of breast cancer in Podkapackie voivodship. Material and methods. This analysis was performed using a systematic literature search. Results. In the Podkarpackie voivodship up until 2014, the incidence rate of female breast cancer was rising and in 2015 it dropped by 68 cases. Even though the survival rate is increasing, it is still one of the leading causes of cancer deaths placing second after lung cancer. Conclusion. Mortality rate, unfortunately, rose gradually. This shows that even though there is a National Health Program aiming at breast cancer awareness and prevention, it is still not enough to reduce the number of breast cancer deaths.

Ultrasound is used for breast cancer detection as a technique complementary to mammography, the standard screening method. Current practice is based on reflectivity images obtained with conventional instruments by an operator who positions the ultrasonic transducer by hand over the patient’s body. It is a non-ionizing radiation, pain-free and not expensive technique that provides a higher contrast than mammography to discriminate among fluid-filled cysts and solid masses, especially for dense breast tissue. However, results are quite dependent on the operator’s skills, images are difficult to reproduce, and state-of-the-art instruments have a limited resolution and contrast to show micro-calcifications and to discriminate between lesions and the surrounding tissue. In spite of their advantages, these factors have precluded the use of ultrasound for screening. This work approaches the ultrasound-based early detection of breast cancer with a different concept. A ring array with many elements to cover 360. around a hanging breast allows obtaining repeatable and operator-independent coronal slice images. Such an arrangement is well suited for multi-modal imaging that includes reflectivity, compounded, tomography, and phase coherence images for increased specificity in breast cancer detection. Preliminary work carried out with a mechanical emulation of the ring array and a standard breast phantom shows a high resolution and contrast, with an artifact-free capability provided by phase coherence processing.

The aim of this study was to use a two-marker assay for the detection of breast cancer cells circulating in patients' blood. We have applied a PCR-based methodology to follow up the possibility of the development of metastatic disease in stage I and II patients who had undergone curative surgery. Since the number of circulating cancer cells in peripheral blood is very low, the technique for their detection needs to be not only highly sensitive, but also very specific. The reverse transcriptase-polymerase chain reaction (RT-PCR) technique may improve the sensitivity of breast cancer cell detection up to only a few cells per one million. The principle of the RT-PCR assay is to amplify a messenger RNA characteristic for breast epithelial cells in a blood sample. Since we do not expect such cells to be circulating in peripheral blood of healthy subjects, detection of the characteristic mRNA should indicate the presence of circulating breast cancer cells. We analyzed the usefulness of three mRNA markers: cytokeratin 19 (CK19), mammaglobin (hMAM) and β subunit of human chorionic gonadotropin (β-hCG) for this test. Blood samples (112) were obtained from 55 patients, in stages I and II, with or without metastasis to regional lymph nodes (N0 or N1). We found that a two-marker assay increases the sensitivity of detection of breast cancer cells in comparison with a single-marker one. Combination of two tumor-specific mRNA markers, hMAM/CK19 or β-hCG/CK19, allowed the detection of circulating breast cancer cells in 65% of N1 patients and 38% of N0 patients. By comparison, the combination hMAM/β-hCG allowed the detection of circulating breast cancer cells in the blood of 68% of N1 patients and 46% of N0 patients. Addition of the third marker did not significantly increase the detection sensitivity.

Introduction: Death anxiety, fear of abundance, isolation, stigma of the disease and medication side effects are among the most common sources of anxiety and depression in patients with breast cancer. Purpose: To examine the possible relationship between religiosity, psychological resilience and depression on breast cancer patients. Materials and methods: A cross-sectional design was employed in this study which 152 breast cancer patients participated. Data were collected with the following instruments: Patient Health Questionnaire-2-item scale, Connor-Davidson Resilience Scale 25 and Centrality of Religiosity Scale and a special designed sheet reporting social, demographic and clinical characteristics. Statistical analyses were conducted with the Statistical Package for the Social Science V25. Descriptive statistics such means, and frequencies were calculated and inferential statistics such correlation test, simple and multiple regression analysis were applied. Results: Approximately 1 in 3 patients suffered symptoms of depression while they were reporting moderate religiosity and resilience values. Based on the four-step mediation analysis religiosity was strongly associated with psychological resilience but it was not found to affect directly either depression, but psychological resilience can be a mediator between religiosity and depression. Conclusion: This study supports the notion thatreligiosity and spirituality can have a beneficial influence on health outcomes.

The aim of this paper is to present the phenomenon of social exclusion of persons whose body has been deformed due to disease process. Modern concepts of stigmatization and sociocultural aspects, which can be the background of social exclusion process, are going to be shown. Authors’ own research fo- cused on how the bodies of women after mastectomy and those suffering from anorexia are perceived in the society. In the lights of obtained results, stigmatization process is in the more advanced stage in women with anorexia than in those after mastectomy. In women after mastectomy it stops on social labeling, but in the context of anorexia it reaches the stage of social rejection according to Link and Phelan’s (2001) theory, which can be connected with social attribution of responsibility for having a stigma (somatic vs mental illness).

Although the widespread of early screening and advanced medical therapies, the breast cancer incidence rate continues to rise among Algerian women. This retrospective study investigated mammary lesions’ epidemiological profile and histopathological characteristics and evaluated primary invasive breast cancer prognostic factors. We found that the incidence of breast cancer increases in middle- aged women between 40 and 60 years. Scarff Bloom Richardson grade II predominates in invasive breast cancer samples. In this study, molecular profiling shows that 82.1% of invasive tumours are hormone receptor-positive. A significant correlation is observed between the age of the patient and the SBR grade (p = 0.001) and with the hormone receptor expression (p = 0.001). In addition, the tumour grade is significantly correlated to oestrogen and progesterone receptor expression (p = 0.000; p = 0.000, respectively). Twenty-two per cent of cases were human epidermal growth factor receptor 2-positive. The Ki-67 proliferation index is expressed in 91% of breast cancer patients and was significantly associated with Scarff Bloom Richardson grade (p = 0.030), the progesterone receptor expression (p = 0.029) and with human epidermal growth factor receptor 2-positivity (p = 0.023). Primary breast cancer with a high grade is more frequent (31%) in young women under 40 years old, presenting 17% of our population. In summary, breast cancer patients in Algeria develop an unfavourable profile. Immunohistochemistry assay has played a pivotal role in assessing breast cancer predictive biomarkers improving the tumour behaviour and response to treatment.

Objective: Assessment of the impact of life satisfaction in physically active women on their health promoting behaviours in terms of prevention and early detection of breast cancer. Materials and methods: The study, involving 100 women, was carried out in a fitness centre in Bydgoszcz in 2015. The research instruments used included the authors’ self-designed questionnaire and the SWLS life satisfaction score. Results: Women aged 25–34 constituted 57% of the study participants, with 82% of them domiciled in the city, and 74% of them holding secondary or higher education qualifications. 67% of them assessed their knowledge about breast cancer and breast cancer prevention programme as good, 65% of them believed it was impossible to protect oneself from cancer, 68% of them occasionally consumed alcohol. 89% of the respondents engaged in breast self-examination, and 68.4% of the subjects aged 25–34 considered excessive weight/obesity as a risk factor. 61.5% of women aged 45–60 were smokers. Respondents living in the city would dedicate one hour more for physical activity than those from the countryside. 93.2% of women with secondary/tertiary education carried out breast self-examination. Surveyed women received high scores on the scale of life satisfaction, averaging 25.69 points, they were less likely to smoke cigarettes, and more likely to engage in breast self examination (95.5%). Conclusions: Physically active women assess their knowledge on health-promoting behaviours well, lead a healthy lifestyle, and avoid breast cancer risk factors. Physically active women accomplish a high level of life satisfaction, which is especially true for married women with a higher education degree. On the other hand, life satisfaction does not correlate with age, place of residence or marital status. Women presenting a high level of life satisfaction are more involved in health-promoting behaviours, carry out regular breast self-examination, and undergo preventative medical check-ups.

Introduction. Socio-demographic factors may affect the decision making associated with the disease and the long-term results of treatment. Objective. Exploring the relationship between socio-demographic factors of women treated for breast cancer and decision- making associated with the disease and treatment. Material and methods. The study involved 100 women aged 30 to 72 (mean: 57 years) who were treated at the Oncology Center in Bydgoszcz in 2013–2014 due to breast cancer. A survey questionnaire on socio-demographic factors and data on knowledge about prevention and disease was used in the study. Results. The number of women performing breast self-examination decreased with age; 83% of patients over 50 years old and 76% (p = 0.0001) over 69 years old underwent mainly mammography (p = 0.03). Self-detection of breast tumor also decreased with age and was detected more frequently by the medical personnel (p = 0.0001). More educated women (85%) examined themselves more often than those with primary and vocational education (p = 0.001). According to our assessment, the number of women with knowledge about cancer decreased with age (p = 0.004). The same was true for women in a very good and good financial situation, with 94% of them declaring a higher knowledge level (p = 0.001). Conclusions. Women’s knowledge about breast cancer is not satisfactory, especially in older and less educated women. They obtain knowledge from the Internet mainly and from the medical staff in the smallest degree. Young women up to 49 years of age perform self-examination to detect breast cancer most often, while older women use mammography. Almost all women immediately report to the doctor and are admitted to clinics after detecting lesions with mammography or self-examination.

Introduction. Estrogen receptors (ER) are members of nuclear receptors that act in the ER signaling pathway regulating the pathophysiology of hormone-responsive target cells including breast tissue. Aim. This detailled review literature was written on the pathophysiology of ER signaling as well as the effect altered ERα and associated pathway derangement in the oncogenesis of breast cancer. Material and methods. This review was performed according to systematic literature search of three major bibliographic databases (Scopus, PubMed, and Cochran). Analysis of the literature. In this pathway, estrogen receptor alpha (ERα) is a key estradiol-17β (E2) induced transcription factor that has been implicated in the initiation and development of the major fraction of breast cancers. Hence understanding the ERα-mediated ER signaling that results in alterations from normal phenotypic features of breast tissue to the oncogenic features of breast cancer is important. The oncogenic effect of ERα in ER signaling is driven by combinations of molecular assets within the cancer cells. Normally, the transcriptional activity of ERα is controlled by tight regulation of its protein level inside the cells. Altered stability and activity of ERα due to its phosphorylation, ubiquitination, glycosylation, sumoylation, and acetylation events can trigger oncogenic ER signaling. Conclusion. The function and activity of ERα is also modulated by its interaction with coregulators as well as crosstalk with oncogenic factors from other oncogenic pathways. These all events increase the complexity of the progression of ER+ breast cancer and its response to endocrine therapy

Introduction. Breast cancer (BC) makes up nearly 26% of malignant tumours worldwide and is the leading cause of cancerrelated deaths in European women. With approximately 18,000 new cases of BC diagnosed in Polish women annually, breast cancer liver metastasis (BCLM) is respectively an increasing issue. Recent data found in literature indicates improved survival following liver resection with systemic therapy. Objective. The aim of study was to evaluate surgical treatment in patients with isolated BCLM. Materials and method. During 2009–2013, a retrospective study was undertaken and 30 cases analysed. From nearly 2,000 liver resections performed, 11 female patients at the mean age of 59.18 years with BCLM were qualified for surgery. Results. The median time between primary and secondary treatment was 3.5 years (1–7). One patient (9.1%) presented an extrahepatic lesion – bone metastasis. The left lobe, right lobe and both lobes of the liver were affected, respectively, in 3 (27.3%), 4 (36.4%) and 4 (36.4%) patients. 5 patients (45.5%) presented single hepatic lesion, in contrast to the maximum number of lesions which equalled 6 in the right lobe. Average hospitalisation period was 13.27 days and discharge on the 11.3 postoperative day. One-year survival was 72.7% (8 patients); therefore, three-year survival was 36.4% (4 patients). Conclusions. Oncological centres should assess BCLM patients more openly and qualify them for hepatic resection along with adjuvant systemic treatment in order to improve overall survival. This, however, needs to be studied in a multicentre randomized trial.

Introduction: Quality of life can be determined by a number of factors, including subjective perception of various spheres of health and health-unrelated factors. Purpose: To compare the quality of life of women who had breast cancer one month and one year after mastectomy, and to verify the usefulness of health-related quality of life (HRQoL) scales in early identification of patients having problems in various functional spheres. Materials and methods: The study included the group of 110 mastectomized women. Quality of life of the participants was estimated with EORTC QLQ-C30 and EORTC QLQ-BR23 questionnaires. Results: Global health status (QoL) determined with the aid of the EORTC QLQ-C30 questionnaire turned out to be significantly higher in women surveyed one year after mastectomy than in those examined one month after the surgery (74.23 vs. 58.33, p<0.001). Moreover, the two groups of patients differed significantly in terms of physical, cognitive, social and role functioning scores. Most of the symptoms assessed were resolved within a year after the breast cancer surgery. No significant intergroup differences were revealed with regard to emotional and socioeconomic functioning or future perspective scores. Conclusions: Quality of life of most mastectomized women improves considerably within one year after the surgery. The use of quality of life instruments can be useful in early postoperative identification of patients who score low on functional and symptom scales. Such patients require support and/or psycho-oncological treatment during the early postoperative period. Quality of life of breast cancer patients during the early postoperative period can be a predictor of this parameter in a longer-term perspective.

Introduction: General quality of life, health, and sensation of pain caused by cancer are connected with the physical, mental and social state of a human being. Aim of the study: The aim of the study is self-assessment of life quality, general health, and pain sensations in breast cancer patients after adjuvant therapy with chemo- and radiotherapy. Material and methods: The study was carried out in 2016 at the Professor Franciszek Łukaszczyk Oncology Clinic in Bydgoszcz. 56 women with breast cancer were qualified for the study, upon completion of treatment. Socio-demographic and clinical data was used, questions 29 and 30 from the QLQ C30 questionnaire, and the Memorial Pain Assessment Card. Results: Generally, the average self-assessment of health and life quality was 4.98 and 5.18 points, respectively. Age, education, marital status and the place of residence did not have any influence on the self-assessment of health and life quality (p > 0.05). 46 women (82.1%) did not take analgesics. Those patients who did not take analgesics assessed their health and life quality better, with the average scores of 5.3 and 5.63 points, respectively. The average for pain intensity was 2.05 on a 0–11 scale. Most women – 14 (25%) – assessed their mood as very good. The mood average was 2.91 points. Only the administration of analgesics influenced the mood score (p = 0.001). Conclusions: Women with breast cancer after radical treatment assess their health and life quality as good. Those not taking analgesics assess their health and life quality better and are decidedly in a better mood than those taking such medications. Socio-demographic factors do not influence self-assessment of health, life quality, or pain intensity.

Cancer is responsible for the death of millions of people all around the world. Among the large group of cancers, the prevalence of breast cancer is highest in women. Therapeutic interventions, including removal surgery, radiation therapy, hormone therapy, and chemotherapy which is largely used, may cause adverse effects on the quality of patients’ life. This fact has encouraged researchers to find natural substances such as honey to overcome harmful effects. Nowadays, honey is recommended for plenty of abnormalities because of its antioxidant, anti-inflammatory, and antimicrobial properties. Various studies have been conducted to explore the anticancer benefits of different types of honey from different origins. In this review, we are going to summarize in-vitro, animal, and human studies on the anti-breast cancer potential of honey.

A follow-up assessment plan after radical treatment is a part of a comprehensive approach to treating patients with breast cancer. Because breast cancer is the most frequent cancer both worldwide and in Poland, adequate follow-up is important not only for patients but also for economic reasons. Herein, we review current recommendations for follow-up assessments in patients with breast cancer. The main aim of such assessment is detection of early recurrence or tumor presence in the other breast, observation of long-term treatment complications, and creation of multidisciplinary infrastructure that will allow to reduce the risk of recurrence and alleviate physical, mental, and social consequences of treatment.

Introduction and objectives. Breast cancer (BC) is the most common malignancy and the leading cause of cancer death among women. About 15–20% of all BCs are HER2-positive. Proper assessment of HER2 status is crucial to choose appropriate treatment. The review summarizes data on anti-HER2 drugs used to treat HER2-positive BC and provides basic information on new agents under investigation. Brief description of the state of knowledge. Specific HER2-targeting drugs are available or are being evaluated in clinical trials. Anti-HER2 agents include: monoclonal antibodies, tyrosine kinase inhibitors, antibody-drug conjugates, bispecific antibodies, PI3K/AKT/mTOR inhibitors and heat shock protein 90 inhibitors, HER2-targeting vaccines and CDK4/6 inhibitors. The advent of anti-HER2 therapies increased the time of progression free survival and overall survival in BC patients. Results. Final analysis of the CLEOPATRA trial shows that the combination of trastuzumab, pertuzumab and taxane significantly improved outcomes in metastatic HER2-positive BC and it is currently preferred first-line treatment. The recommended second-line treatment is based on trastuzumab emtansine or on the combination of lapatinib and capecitabine. Some promising agents such as margetuximab or trastuzumab deruxtecan are still under investigation. Conclusions. Anti-HER2 directed treatment undoubtedly improves outcomes among patients with HER2-positive BC. Access to drugs such as trastuzumab, pertuzumab, lapatinib and T-DM1 improves prognosis even in patients with advanced disease. Further studies and clinical trials on novel anti-HER2 therapies are required. Nevertheless, BC treatment is becoming more effective and, hopefully, one day it may be possible to cure patients even with metastases.

It is widely accepted that a cancer diagnosis is a stressful and often traumatic experience for patients. Stress as a psychological side-effect of diagnosis has been well-researched. However, it is also possible that the life changing and potentially life-threatening aspects of disease may lead to symptoms of Post-Traumatic Stress Disorder (PTSD), including heightened anxiety, intrusive and distressing thoughts, avoidance of reminders of the trauma and sleeping disturbances. These symptoms may be of short duration but studies show that is possible for patients to live with PTSD for years after diagnosis and treatment. Even if not all the criteria for a full PTSD diagnosis are present, subthreshold or subsyndromal PTSD has been shown to cause significant distress and affect the quality of life of breast cancer patients. Given that with earlier diagnoses and ever more effective treatment interventions, the number of women who survive cancer and whose survival intervals are longer, identification, monitoring and treatment of patients living with PTSD becomes ever more important.

Mitochondria are cell energetic centers where ATP is produced. They also play a very important role in the PDT as intracellular sites of photosensitizer localization. Photosensitizers gathering in mitochondria (like porphyrin derivatives used in this work) are more effective in tumor cell destruction. Moreover, it was assumed that di-amino acid substituents attached to porphyrin ring will strengthen the effectivity of interaction with membrane receptors of examined cells. MTT assay was performed to investigate the influence of PP(Arg)2 and PP(Ala)2(Arg)2-based PDT on breast cancer cell viability for 24 h, 48 h and 120 h after cell irradiation. Then the influence of PP(Ala)2(Arg)2- and PP(Arg)2-mediated PDT on early mitochondrial apoptosis induction via measurements of the transmembrane mitochondrial potential changes was examined. Results showed that lower energy doses and maximal nontoxic photosensitizer doses of PP(Ala)2(Arg)2 and PP(Arg)2 applied in PDT can imply apoptotic cell death. It was confirmed that modification of the protoporphyrin IX by attaching two alanine substituents raised the efficiency of photodynamic therapy.

Introduction: Breast cancer is the most common malignant tumour among women in Poland, with a dynamically increasing incidence rate. At present, one-half of breast cancer cases are diagnosed at the stage of high locoregional advancement, which leads to common radical mastectomies. These procedures are the cause of the most serious complications, i.e. restricted mobility of the glenohumeral joint and lymphoedema of the upper limb, which usually require rehabilitation.Objective: An evaluation of the attendance and effectiveness of detecting suspicious breast lesions in 40-year-old female inhabitants of Kraków who were personally invited to take part in the Educational Programme of Early Breast Cancer Detection (EPEBCD). This study assumes that increasing the level of oncological education amongst women should result in high attendance in screening, which would lead to detection of breast cancer at earlier stages. In these cases, effective methods of breast-conserving therapy are used, which carry a minimal risk of complications that require rehabilitation.Material and methods: The subject of the analysis was a group of 19,773 female inhabitants of Kraków who took part in the EPEBCD between 2003 and 2008. The group consisted of 4723 women aged 40 years who were personally invited to take part in the Programme, and 15,010 other women who volunteered for the research. The percentage of suspicious lesions discovered in physical and mammographic examinations in both groups was analysed. In the statistical analysis, the stratum weight was examined, using a two-tailed test and the STATISTICA 6.0 program. The significance level α < 0.05 was assumed.Results: The attendance rate of 40-year old women who were sent personal invitations was similar in each year (2003 to 2008) and, on average, accounted for 24.4%, which was half of the assumed attendance. It demonstrates that the level of oncological education among young women is low. Breast cancer awareness increased with age and resulted in voluntary participation of older women in screening programmes. The percentage of suspicious changes detected by a physical examination among the 40-year-old women invited personally (Group 1; 0.49%) did not differ significantly from the “before screening” group (Group 2, aged 41 to 49; 0.47%) and even from the “screening” group (Group 3, aged 50 to 69; 0.45%). Only in the “after screening” group of the oldest women (Group 4, over 69), the percentage (1.57%) was significantly higher (p < 0.001). The percentage of suspicious changes detected by mammography did not differ significantly in the group of 40-year-old women (Group 1; 3.58%) and in the “before screening” group (Group 2; 4.11%; Groups 1 and 2, p > 0.05). Significant differences were only found in the “before screening” Group 2 and “after screening” Group 4 (p < 0.001). The percentage of suspicious changes identified by mammography did indeed increase with age, though this was only a marginal increase, at the level of tenths of a percent. Only in the oldest group (Group 4), this percentage (5.16%) was significantly higher than in the other research groups.Conclusions: The much higher percentage of suspicious breast lesions discovered by mammography in comparison with those discovered in a physical examination demonstrates a low level of breast cancer advancement in most cases, which can be treated by means of breast-conserving therapy. Dissemination of educational and screening programmes will help to increase the percentage of early detected breast cancer cases which, after breast-conserving therapy, will not constitute a risk of serious complications requiring rehabilitation.

INTRODUCTION Breast cancer is the most frequent malignancy in women in Poland. Overexpression of the HER2 receptor positively correlates with the aggressiveness of the disease. Trastuzumab, a humanized monoclonal antibody against HER2, radically improves the prognosis in such patients. However, its main side effect is cardiotoxicity. The aim of this study is to evaluate the cardiotoxicity in trastuzumab-treated patients. MATERIALS AND METHODS The cases of 19 women treated in the Department of Internal Medicine and Oncologic Chemotherapy Medical University of Silesia in Katowice in the period of 2010–2011 were retrospectively analyzed. RESULTS The median age at the time of diagnosis – 57 years, pathology: invasive ductal carcinoma in 17, invasive lobular carcinoma in 2 patients. Overexpression of HER2 was observed in all the cases. Trastuzumab was administered to 16 of them after previous therapy with antracyclines and/or taxan; the median total antracycline dose was 680 mg. Moreover, 13 patients were subjected to radiotherapy; the median total dose was 50 Gy. In 3 patients, trastuzumab and chemotherapy was used in the management of metastasized disease. The median time between the administration of trastuzumab and anthracyclines was 3 months. The median number of cycles was 10. Before administration of the drug, ECG and echocardiography were performed along with ejection fraction (EF) assessment. Before and after therapy the median EF was 65% and 61%, respectively. The therapy was withheld in 1 patient because of a decrease in EF to 40%. CONCLUSIONS The treatment with trastuzumab was well tolerated. In most patients, we did not observe a significant decrease in EF. However, the cases should be followed up to detect delayed cardiotoxicity.
WSTĘP Rak piersi to najczęstszy nowotwór złośliwy u kobiet w Polsce. W praktyce klinicznej ważne jest określenie ekspresji receptorów, w tym dla naskórkowego czynnika wzrostu typu 2 (HER2) na komórkach nowotworowych. Gdy komórki te wykazują wzmożoną ekspresję HER2, przebieg choroby jest agresywniejszy. Trastuzumab, humanizowane przeciwciało monoklonalne przeciw HER2, znacząco poprawia rokowanie. Jednak terapia taka nie jest pozbawiona działań niepożądanych, w tym kardiotoksyczności. Celem pracy była ocena kardiotoksyczności leczenia trastuzumabem u pacjentek z rakiem piersi. MATERIAŁ I METODY Dokonano retrospektywnej analizy historii chorób 19 kobiet leczonych w Klinice Chorób Wewnętrznych i Chemioterapii Onkologicznej Śląskiego Uniwersytetu Medycznego w Katowicach w latach 2010–2011. WYNIKI Mediana wieku w chwili rozpoznania – 57 lat, typ histologiczny: rak przewodowy naciekający u 17, rak zrazikowy naciekający u 2 pacjentek. Nadekspresję HER2 stwierdzono u wszystkich 19 kobiet, u 16 z nich zastosowano trastuzumab po wcześniejszym leczeniu upełniającym antracyklinami i/lub taksanami – mediana sumarycznej dawki antracyklin 680 mg. U 13 pacjentek dodatkowo przeprowadzono radioterapię – mediana dawki sumarycznej 50 Gy. U 3 pacjentek podano trastuzumab oraz chemioterapię w leczeniu rozsianej choroby nowotworowej. Mediana czasu włączenia trastuzumabu po antracyklinach wyniosła 3 miesiące. Liczba cykli: mediana 10. Przed włączeniem leku wykonano badanie EKG i czynnościowe serca z oceną frakcji wyrzutowej (EF). Mediana EF przed rozpoczęciem leczenia: 65%, po leczeniu: EF 61%. U jednej pacjentki przerwano leczenie z powodu obniżenia EF do 40%. WNIOSKI Leczenie trastuzumabem było dobrze tolerowane. U większości pacjentek nie obserwowano znamiennego obniżenia EF wymagającego odstawienia leku. Badana grupa wymaga jednak badań czynnościowych serca również po odstawienia trastuzumabu w celu wykrycia późnych powikłań kardiotoksycznych.

Breast cancer is the most frequently diagnosed female cancer in Poland (over 17,500 women). Anthracyclines have become one of the most important drugs in breast cancer systemic treatment. In the treatment of metastatic disease combination chemotherapy with doxorubicin provides the objective response rate of 60–85%, and the median time of progression-free survival is about 12 months. Non-pegylated liposomal doxorubicin (NPLD) in combination with cyclophosphamide is associated with a lower risk of cardiotoxicity, higher efficacy and more favourable toxicity profile as compared with conventional anthracycline regimes. Two cases of females patients treated with NPLD described in this article demonstrate the importance of the choice of chemotherapy, professional monitoring, early detection and treatment of adverse effects. Non-pegylated liposomal doxorubicin ordained in systemic treatment of stage IV breast cancer prolongs survival and enhances the quality of life. It is a reasonable option for palliative therapy.

Selection of the treatment method in breast cancer patients and its consequences may affect their quality of life through somatic, psychical, and social factors. The aim of the study was early evaluation of the quality of life of women after mastectomy vs. breast conserving surgery. Material and methods. The study included 100 women aged 31 to 79 years (mean: 57) who underwent surgery due to breast cancer (amputation: 52; breast conserving surgery: 48 women) at the Cancer Centre in Bydgoszcz in 2014. The QLQ C-30 and QLQ BR-23 questionnaires were used to evaluate the quality of life of the patients 3 months after surgery. Results. In the Global Health Status/QoL domain, the mean score for women after amputation and breast conserving surgery was 49 and 53, respectively; for Physical Functioning, the scores were 70 and 75, and for Role Functioning, 62 and 68, respectively. For Cognitive Functioning, the mean score was 74 and 73; for Emotional Functioning - 62 and 68, and for Social Functioning 64 and 60, respectively. The difference in the arm symptoms domain was significant at 46 and 33 points, respectively (p = 0.004). The patients treated with breast conserving surgery had a better body image than women after amputation - the mean score was 52 and 66, respectively (p = 0.01). Conclusions. With respect to Global Health Status/QoL and Physical Functioning, the quality of life of women in the early postoperative period was similar in women after breast amputation and those who underwent breast conserving surgery. Patients treated with breast conserving surgery had a better score for body image, while those who underwent amputation more often suffered from arm symptoms, such as pain, oedema, and problems with raising of the limb.

Breast cancer is the most common malignant tumor in women, so it is important to study the factors that can protect against it. One of them is physical activity, which has become the area of our interest, especially the practice of sports by women aged between 21 and 34. The aim of this retrospective study was to check how practice of sport in the age group of 21-34 in women from the research group diagnosed with breast cancer and in the control group (healthy women from families burdened with this cancer) influences the risk of developing breast cancer. The study showed that healthy women from families with a burden of breast cancer practiced sports in the age range 21-34 more frequently and intensively than women who had a history of breast cancer. (Sport 1-highest intensity P = 0.002 Sport 2-medium intensity P < 0.001 Sport 3-the lowest intensity P < 0.001.) It can be concluded that practicing sport in the age group of 21-34 is an important factor in the prevention of breast cancer, although the influence of other protective factors in women from the control group cannot be ruled out.

Aim. An evaluation of lifestyle changes (physical activity, diet) in patients after breast cancer surgery. Materials and method. 200 women after breast cancer surgery were surveyed. The respondents were asked whether the surgery caused a change in their diet and physical activity. An analysis was performed concerning the education, place of residence and age of the respondents. Results. Prior to being diagnosed with breast cancer, about one third of the respondents were concerned about their diet and physical activity. After the surgery more than a half of the respondents were concerned about a healthy lifestyle. Women below 50 years old with higher education, who live in a city, were concerned about their diet and physical activity both before and after surgery. Conclusions. As a result of the breast cancer surgery, lifestyle changes were most often found in women aged 50-69 years old with higher education who lived in a city. Statistical relevance of the results was noted.

Aim: The main goal of the article is to showcase how husbands performed the role of caregivers for their wives after breast cancer diagnosis. The researchers analysed this issue by comparing the perspectives of the male caregivers with their wives’ opinions. The wives were active members of Amazon clubs. Background: Partners of women with breast cancer are expected to assume responsibility for taking care of their wives. Simultaneously, however, they are not prepared to fulfil their new duties, nor are they being provided with any professional support in this regard. Method: Semi-structured interviews were conducted with eight married couples, separately with wives and husbands. Results: Husbands perceive their role in their wives’ struggles as primarily supportive and channel any negative emotions via undertaken actions. They support their wives in four ways: by focusing on their wives’ feelings and caring about her, taking part in the healing process, manag- ing the household during the period of their wives’ recovery, and by bringing back normalcy into their lives. Conclusions: Limited knowledge about oncological diseases, deficiencies in the assistance services offered to husbands of women with breast cancer and stereotypes about gender roles contribute to the difficulties men face when taking care of their sick wives. Even the wives active participation in the Amazon movement does not seem to mitigate their problems.

Oncoplastic and reconstructive techniques are essential tools in the armamentarium of contemporary breast surgeons. The aim of the study was to identify oncoplastic reconstructive patterns in breast cancer centers across Poland. A questionnaire of 18 questions was sent by email to the members of the Polish Society of Surgical Oncology and the Polish Society of Plastic, Reconstructive and Esthetic Surgery via their dedicated websites. The numbers of breast cancer patients operated on in each center ranged from 120 to 904 per year. Breast-conserving surgery (BCS) predominated in all but one center (range 50-70%). Immediate breast reconstructions (IBR) accounted for 6-42% of procedures, The most frequent type of IBR was either a two-stage expander followed by a permanent implant or one-stage implant- based with or without synthetic mesh. The most frequent type of delayed breast reconstruction (DBR) was a two-stage expander followed by implant-based reconstruction. None of the surveyed cancer centers performed free flap reconstruction. Deep inferior epigastric perforator (DIEP) flaps were performed in the plastic surgery department. Reconstructions based on pedicled flaps were performed in cancer centers. Acellular dermal matrices (ADM) and fat transfer were used in selected centers. In the clinical scenario of adjuvant radiotherapy, delayed breast reconstruction was favored. The full range of oncoplastic BCS was performed. Patient-reported outcome measures (PROM) and complications were assessed. Our findings can act as a platform for further improvement in skills, certification, data collection and audit, including patient reported expectation measures. There is also an urgent need to address pan-European inconsistencies in procedural reimbursement.

At present, sentinel lymph node biopsy is a standard procedure to assess the advancement of breast cancer and cutaneous melanoma. The aim of the study was to assess the role of the sentinel lymph node biopsy in the treatment of patients with breast cancer in our own material. Material and methods. Analyzed was medical documentation of 258 patients with initially operable breast cancer, qualified for operation with sentinel lymph node biopsy in 2004-2014 in the Department of Surgery of the 4th Military Teaching Hospital. A few hours prior to the planned surgery, radioisotope (technitium-99 sulfur colloid) was applied in the area of tumor or under the areola. 1-2 hours after administering the tracer, the lymphoscintigraphy with the labelling of the sentinel lymph node on the skin was performed. Results. On the basis of the gathered material, obtained were the following parameters: sensitivity – 100%, and specificity – 94.6%. Four cases were false negative (5.5%). Conclusions. 1. Marking the sentinel lymph node in breast cancer, based on the single visualisation method with the use of radioisotope, is a useful and effective technique. 2. The factor influencing the results of the sentinel lymph node biopsy (true positive and negative results and false negative result) was the number of the excised lymph nodes except for the sentinel lymph node. 3. Patients with estrogen receptor expression had often metastases to sentinel lymph node (145 cases – 56%). 4. The false negative rate, i.e. 5.5% in our material, is within the limits of acceptability given in the literature. 5. The sentinel lymph node biopsy performed by the experienced surgical team is a reliable diagnostic tool with a low complication rate.

This paper presents a case of a patient with invasive ductal breast cancer following breast augmentation. Following breast implants rupture in March 2013 the breast implants have been removed – histopathological examination revealed leaked silicone with inflammatory infiltration, without evidence of cancerous lesions. Diagnostic imaging revealed multiple encapsulated silicone particles and clusters of post-inflammatory macrocalcifications in both breasts. In January 2014 the patient presented with symptoms of massive inflammation of the left breast. Following surgical consultation the patient had undergone radical left-sided mastectomy with lymphadenectomy. Postoperative histopathological examination revealed a multifocal advanced invasive ductal cancer G3 pT3pN3a (vascular invasion, metastases in 11 of 12 examined axillary lymph nodes). Following surgery the patient was qualified for further treatment – chemotherapy, radiotherapy, hormone therapy. The discussion includes a review of literature on the risk evaluation of co-occurrence of breast cancers in women with silicone breast implants and presents diagnostic challenges of breast cancer in this patient group.

Opis:
Mammography based breast cancer screening is very popular because of its lower costing and readily availability. For automated classification of mammogram images as benign or malignant machine learning techniques are involved. In this paper, a novel image descriptor which is based on the idea of Radon and Wavelet transform is proposed. This method is quite efficient as it performs well without any clinical information. Performance of the method is evaluated using six different classifiers namely: Bayesian network (BN), Linear discriminant analysis (LDA), Logistic, Support vector machine (SVM), Multilayer perceptron (MLP) and Random Forest (RF) to choose the best performer. Considering the present experimental framework, we found, in terms of area under the ROC curve (AUC), the proposed image descriptor outperforms, upto some extent, previous reported experiments using histogram based hand‐crafted methods, namely Histogram of Oriented Gradient (HOG) and Histogram of Gradient Divergence (HGD) and also Convolution Neural Network (CNN). Our experimental results show the highest AUC value of 0.986, when using only the carniocaudal (CC) view compared to when using only the mediolateral oblique (MLO) (0.738) or combining both views (0.838). These results thus proves the effectiveness of CC view over MLO for better mammogram mass classification.

Breast cancer affects a large number of women worldwide. Surgical management has evolved towards mastectomies and breast-conserving surgeries. The complications following a mastectomy can be physical and/or psychological. The physical complications include pain, scarring, lymphedema, limitation in range of motion at the shoulder, muscle weakness, change in body posture, etc. Some of the psychological complications are negative boy image, anxiety, depression and depressive disorders, negative body image. Appropriate management requires a multi-disciplinary team of which the physiotherapist is a part of. Literature has shown that there is a better improvement in physical function if physiotherapy is commenced early. Therefore, physiotherapy should be incorporated pre and post-mastectomy. Physiotherapy management should focus on lymphatic drainage, soft tissue mobilization, range of motion exercises, strengthening exercises and postural correction. Increased physical activity and recommendation of support groups can help to improve psychological outcomes. It is the role of the physical therapist to deal with the physical and psychological complications of a mastectomy to improve the quality of life of the patients.

Psychological distress associated with body image disorders in patients with breast cancer is well described in the literature. Given the known insults to body image caused by both disease and oncological treatment, body image distress is commonplace within this patient population. Stress is also a frequent sequela of a diagnosis of and treatment for cancer and may manifest various forms. This study undertook to investigate the nature of stress and body image distress within a cohort of 80 female breast cancer patients drawn from three outpatient cancer treatment facilities in the Durban, South Africa region and to examine its correlation with psychological and behavioural indices reported by these patients as compared to a group of 80 female patients undergoing treatment for other types of cancer. The results indicated that although there were no significant differences between the two groups in terms of depressive symptomatology and measures of self-esteem, where scores largely fell within normal ranges, the breast cancer patients experienced higher levels of body image dissatisfaction as compared to the other group and that stress tended to be expressed in psychological and behavioural indices as opposed to physiological expression.

Introduction: Breast cancer is the most prevalent female malignancy in Poland. Oncological treatment and its adverse effects diminish quality of life of breast cancer patients, which is determined by a number of somatic, psychological and social factors. Purpose: To assess the quality of life and the level of knowledge and utilization of lymphedema prevention principles among women after surgical treatment of breast cancer. Materials and methods: The study included 145 breast cancer patients after radical mastectomy. The respondents were examined with the validated EORTC QLQ-BR23 questionnaire and a custom-designed survey. Results: Examination with the QLQ-BR23 questionnaire revealed that mastectomized women scored low on the body image scale. The most frequently reported ailments were arm and breast symptoms. While the respondents showed high level of knowledge with regards to lymphedema prevention, they poorly adhered to the prophylactic guidelines. The participants were well aware of the risk factors of lymphedema, and most of them declared avoidance of their harmful effects. Conclusions: Quality of life assessment should constitute an integral component of rehabilitation in every breast cancer patient, as mastectomy exerts significant effect on the outcome of perioperative period. Apart from specialist physiotherapy, also education of patients with regards to principles of lymphedema prevention and autotherapy constitutes an important component of complex management of lymphatic insufficiency.

The main role of melatonin is regulation of circadian and seasonal rhythm. Recently there were carried out a lot of researches on the role and potential application of melatonin in prevention and therapy of various dismelaeases. This work is related to researches about the melatonin influence on estrogen-dependent breast cancer and application in treating this neoplasm. Probable oncostatic mechanisms i.e. melatonin acting as SERM, SEEM and its influence on the hypothalamic – pituitary – reproductive axis, was analyzed. Moreover the relation between circadian disruptions and increase in breast cancer risk were observed. The aim of this work is to show mechanisms melatonin acting and how we can use this hormone in oncology.
Melatonina pełni przede wszystkim funkcję biochemicznego regulatora rytmów okołodobowych i sezonowych, ale szczególne zainteresowanie naukowców wzbudzają udowodnione już jej właściwości antyoksydacyjne i immunomodulujące oraz jej wpływ na regulację wydzielania innych hormonów. W ostatnich latach zostało przeprowadzonych wiele badań dotyczących roli oraz potencjalnego zastosowania melatoniny w profilaktyce i leczeniu różnych jednostek chorobowych. Praca jest przeglądem piśmiennictwa dotyczącego badań nad wpływem melatoniny na patogenezę raka sutka i jej terapeutycznego zastosowania w tym nowotworze. Badano prawdopodobne mechanizmy onkostatyczne dotyczące wpływu na oś podwzgórzowo-przysadkowo-gonadalną oraz działanie melatoniny jako selektywnego modulatora receptora estrogenowego (SERM) i enzymów biorących udział w biosyntezie estrogenów w tkankach obwodowych (SEEM). Ponadto zaobserwowano związek między zaburzeniami rytmu dobowego u kobiet a wzrostem ryzyka zachorowania na raka sutka. Celem niniejszej pracy jest przedstawienie mechanizmów działania melatoniny i jej zastosowania w onkologii.

Paraneoplastic neurological syndromes (PNS) are a group of rarely observed disorders, accompanying about 1% of cancer diseases. They have a typically aggressive course, leading to profound and essentially irreversible disability. The pathogenic cause behind PNS is damage of the nervous system structures as a consequence of the body’s immunological reaction induced by cancer. Onconeural antibodies generated as part of the reaction target the tumour tissues, but also the regular nervous tissue recognized as antigens. Presence of onconeural antibodies reveals a 50–60% sensitivity and 100% specificity in diagnosing PNS. Imaging and laboratory tests have a limited impact on diagnosis. What is crucial is the analysis of clinical systems following Graus’s criteria (elaborated in 2004). In the case presented here, the patient underwent treatment in many neurological departments due to the symptoms of progressive damage of the nervous system. She was eventually diagnosed with the stiff person syndrome related to breast cancer. In spite of the treatment, including an efficacious oncological therapy, the severe neurological deficit resulted in serious motor disability.

The breast cancer is the most common cancer in women, both in Poland and in the world. Consequences entail a disruption in the physical, psychological and social functioning. The aim of the study was to assess the acceptance of illness by patients treated for breast cancer in the early postoperative period. Material and methods. The research was conducted on the group of 100 consecutive patients aged 32‑80 years (median 56 years) who underwent surgery for breast cancer in the Centre of Oncology in Bydgoszcz w 2014 roku. 68 of women had mastectomy, 32 of women had conservative surgery. Polling was conducted in the early period after surgery. The original questionnaire containing closed questions the scale of acceptance of the disease (AIS) as well as mental adaptation to cancer (Mini-Mac) was used in the study. Results. 38% of patients had high acceptance of the disease, 48% averageand 14% had low acceptance. Patients after conservative surgery had a higher average values for the mental strategies to cope with the disease, for the fighting spirit (23.1), helplessness and hopelessness (13.5), positive revaluation (23), the patients had a lower average (16.5) in the strategy to absorb anxiety. Patients after conservative surgery had a higher average for constructive style (2.6) but lower for destructive style (1.5). High level of mental coping with the disease was observed in 53%of patients with constructive style and 4% of patients with destructive style. While, a low level of mental coping with the dosease was observed in 5% of patients with constructive style and 46% of patients with destructive style. Conclusions. Almost half of women after mastectomy or conservative surgery had an average acceptance of the disease. The disease was accepted best by educated women living in the cities, whitecollar workers with a good economic situation. The following factors were affected the better management of the disease, in order: age, education, current occupation and economic situation, while the type of surgery did not affect better management. More than half of women, regardless of the type of surgery reflected the high level of constructive style.

It is commonly thought that breast cancer, like many other cancers, is an illness equivalent to a death sentence. Though this may be true in some cases, the majority of women diagnosed with breast cancer do survive this illness. Breast cancer is a growing illness and is continuing to affect women worldwide, including developing countries like South Africa. Furthermore, this country’s medical system operates in terms of a duality. Here, hospitals and healthcare are mainly situated in either state operated institutions or in privately run practices. This duality emphasizes the inequality within the socio-economic classes, treatment regimens, and ethic-of-care. This article deals with how women from the higher socio-economic stratum of the deeply polarized South Africa deal with breast cancer. The aim is to understand how each participant renegotiates and transforms her self-perception, her identity, and issues around femininity. In addition, the authors also seek to understand if this medical encounter influences the participants’ sense of embodiment, as well as how the medical encounter impacts on their everyday lifeworld.

The healthcare industry is one of the many out there that could majorly benefit from advancement in the technology it utilizes. Artificial intelligence (AI) technologies are especially integral and specifically deep learning (DL); a highly useful data-driven technology. It is applied in a variety of different methods but it mainly depends on the structure of the available data. However, with varying applications, this technology produces data in different contexts with particular connotations. Reports which are the images of scans play a great role in identifying the existence of the disease in a patient. Further, the automation in processing these images using technology like CNN-based models makes it highly efficient in reducing human errors otherwise resulting in large data. Hence this study presents a hybrid deep learning architecture to classify the histopathology images to identify the presence of cancer in a patient. Further, the proposed models are parallelized using the TensorFlow-GPU framework to accelerate the training of these deep CNN (Convolution Neural Networks) archi-tectures. This study uses the transfer learning technique during training and early stopping criteria are used to avoid overfitting during the training phase. these models use LSTM parallel layer imposed in the model to experiment with four considered architectures such as MobileNet, VGG16, and ResNet with 101 and 152 layers. The experimental results produced by these hybrid models show that the capability of Hybrid ResNet101 and Hybrid ResNet152 architectures are highly suitable with an accuracy of 90% and 92%. Finally, this study concludes that the proposed Hybrid ResNet-152 architecture is highly efficient in classifying the histopathology images. The proposed study has conducted a well-focused and detailed experimental study which will further help researchers to understand the deep CNN architectures to be applied in application development.

The aim of the study was a comparison of two groups of women who suffered from breast cancer in the past, selected on the basis of the time elapsed since treatment completion (<7 and >7 years). The specific areas of interest were the quality of life, sense of control, and level of depression. Results from 37 women were analysed. The average age in the research sample was 57. The study was conducted with the use of EORTC QLQ-C30, MHLC, and Depression Assessment Questionnaire. Women in whose case the time elapsed since the completion of anticancer treatment is no more than 7 years have a more negative assessment of the quality of life aspects, that is physical and social functioning, in comparison to persons in whose case the adaptation time to changes, following cancer and treatment, is longer (more than 7 years). The studied persons with a shorter period elapsed since the completion of anticancer therapy hold stronger beliefs concerning their own influence on their health in comparison to women who have not undergone oncological treatment for at least 7 years. The author’s own research highlights the trend pertaining to the differences in the intensity of experiencing guilt and anxiety between the groups. However, it requires further research involving a larger group of studied patients. Despite the progress in the diagnostics and treatment of breast cancer, patients with this type of cancer still experience long-term subjective difficulties in areas making up their physical, mental, and social well-being. The research proves the differences in functioning of women depending on the time that has elapsed since the completion of anticancer treatment.

Metallothionein (MT) was first discovered over 50 years ago, as a cadmiumbinding protein in the kidney of a horse. Metallothioneins are a class of ubiquitously occurring cysteine and metal-rich proteins of low molecular weight, containing sulfur-based metal clusters formed with zinc (Zn), cadmium (Cd), and copper (Cu). In mammals, four distinct MT isoforms designated MT-1 – MT-4 are identified. The major physiological functions of metallothioneins include the homeostasis of essential metals Zn and Cu and the protection against oxidative stress and toxic effects of heavy metals such as Cd. Cadmium ,which has been classified as class I carcinogen, can stimulate cell proliferation, inhibit DNA repair, impair apoptosis and promote the development of cancer in a number of tissues. Significant quantities of Cd are introduced into the environment both by natural and anthropogenic activities, with anthropogenic activities contributing 3–10 times more Cd to the environment than natural activities. Cadmium accumulates in the human body with a long biological half-life ranging from 10 to 30 years. Cd accumulation and the aberrant expression of MTs have been found to be associated with the development of many types of cancers, including breast cancer.
Metalotioneina (MT) została po raz pierwszy odkryta ponad 50 lat temu, jako białko wiążące kadm w nerce konia. Metalotioneiny są grupą powszechnie występujących białek o niskiej masie cząsteczkowej, wysokiej zawartości reszt cysteiny, których grupy tiolowe wiążą jony takich metali jak kadm (Cd), cynk (Zn) i miedź (Cu). U ssaków wyróżnia się cztery izoformy metalotioneiny oznaczone jak MT-1 – MT-4. Do podstawowych funkcji metalotionein zalicza się utrzymanie homeostazy metali ciężkich, ochronę przed ich toksycznym działaniem, a także ochronę przed stresem oksydacyjnym. Kadm, który został zakwalifikowany do substancji karcynogennych I klasy, może między innymi stymulować proliferację komórek, hamować naprawę uszkodzeń DNA, zaburzać proces apoptozy oraz promować rozwój nowotworów w licznych tkankach. Kadm znajdujący się w środowisku naturalnym pochodzi ze źródeł naturalnych, jak i antropogenicznych, przy czym ilość kadmu dostająca się do środowiska w wyniku działalności człowieka od 3 do 10 razy przekracza ilości kadmu pochodzące ze źródeł naturalnych. Kadm akumuluje się w organizmie człowieka, a jego okres biologicznego półtrwania wynosi od 10 do 30 lat. Zarówno akumulacja Cd, jak i zaburzenia ekspresji metalotionein mogą mieć związek z rozwojem wielu typów nowotworów, w tym raka piersi.

Anti-cancer activities of some pyrano-pyridines have been previously reported. Herein, we investigated anti-proliferative and apoptotic effects of the novel pyrano [3, 2-c] pyridine (P.P, TPM.P, 4-CP.P and 3-NP.P) compounds against MCF-7 breast cancer cells. The MCF-7 cells were cultured in the presence of various concentrations (20-200 μM) of the tested compounds for 3 days and the cell viability was determined by MTT assay. Induction of apoptosis was qualitatively assayed by acridine orange/ethidium bromide (AO/EtBr) staining, DNA fragmentation assay, as well as quantitatively by Annexin V/PI double staining and cell cycle analysis. These compounds inhibited growth and proliferation of the MCF-7 cells in a dose- and time-dependent manner. The IC50 values of P.P, TPM.P, 4-CP.P and 3-NP.P after 24 h of exposure were calculated to be 100±5.0, 180±6.0, 60±4.0 and 140±5.0 μM, respectively. 4-CP.P was determined as the most potent compound and was chosen for further studies. The result of flow cytometric cell cycle analysis indicated an increase in sub-G1 population after 72 h treatment of the cells. Furthermore, this was accompanied by exposure of phosphatidylserine (PS) in the outer cell membrane after time course of treatment with the 4-CP.P. Based on these observations, the pyrano [3, 2-c] pyridines can be regarded as a valuable candidate for further pharmaceutical evaluations.

The purpose of this study was to evaluate the accuracy of statistical properties of the backscattered ultrasound in differential diagnosis of the breast lesions. The B-mode images, together with the appropriate RF echoes from the breast lesions and surrounding tissues were collected. The RF data was processed for the statistics of the backscattered echo signals, using K and Nakagami distributions characterized by the M and m parameters, respectively. Based on both, M and m parameters, a set of 18 parameters was derived. From the point of view of the sensitivity of detection of cancer, the best score was obtained using maximum value of M parameter, the best specificity was received using the differential Nakagami parameter (the differential values between lesions and surrounding tissues). In conclusion, quantitative sonography is a method which has potential to be a complementary tool for classification of the breast lesions.

Wstęp. Aktywność fiyczna ma ogromne znaczenie w przebiegu leczenia jak i w rokowaniu chorych na raka piersi, może zapobiegać powikłaniom jak i przyczyniać się do poprawy jakości życia. Badania zostały przeprowadzone w Mieleckim Klubie Amazonek. Materiał i metoda. Badaniem objęto 60 kobiet w wieku od 30 do 68 roku życia (średnia wieku 53,6 lat). Do przeprowadzenia badania użyto ankiety własnego autorstwa oraz Międzynarodowego Kwestionariusza Aktywności Fizycznej (IPAQ). Najliczniejszą grupę wśród badanych stanowiły kobiety po 50 roku życia (58,3%). Większość badanej grupy stanowiły kobiety zamężne 58,3%. 70% badanych kobiet mieszkało w mieście. Najmniejszą grupę stanowiły kobiety z wykształ- ceniem podstawowym 15%, zaś największą stanowiły kobiety z wykształceniem średnim (58,3%). 56,7% badanej grupy stanowiły kobiety niepracujące. Wyniki. Dzięki aktywności fiycznej 76,7% badanych lepiej się czuje, ponadto 55% kobiet uważa, że ma więcej energii oraz lepszą kondycję. Połowa badanych uważa, że aktywność fiyczna ma istotne znaczenie w zapobieganiu raka piersi i innych chorób nowotworowych. Według dużej liczby badanych, aktywność fiyczna bardzo pomogła im w powrocie do zdrowia po zabiegu operacyjnym. Wnioski. Z przeprowadzonych badań wynika, że zdecydowanie bardziej aktywne były panie młodsze (poniżej 50 lat). Nieznacznie częściej średnia ogólna aktywność badanych była większa wśród pań mieszkających na wsi. Średnia aktywność zawodowa badanych zwiększała się wraz ze wzrostem poziomu ich wykształcenia. Średnia aktywność badanych pod względem przemieszczania się największa była wśród kobiet z wykształceniem średnim, nieco mniejsza wśród kobiet w wykształceniem wyższym i najmniejsza wśród kobiet z wykształceniem podstawowym.
Introduction. Physical activity is very important in course of treatment as well as prognosis of patients with breast cancer and can prevent complications and help to improve the quality of life. The study was conducted in the Amazon Club in Mielec. Material and method. The study included 60 women aged 30 up to 68 (mean age 53.6 years). It was conducted by means of the author’s questionnaire and the International Physical Activity Questionnaire (IPAQ). The largest group among the respondents were women over 50 years of age (58.3%). The majority of the study group 58.3% were married women. 70% of the subjects were the residents of the city. The smallest group consisted of women with primary education - 15%, and most were women with secondary education (58.3%). 56.7% of the study group were women not professionally active. Results. Thanks to physical activity 76.7% of the respondents felt better, also 55% of the women noticed more energy and better wellbeing. A half of the respondents believed that physical activity is important in preventing breast cancer and other cancers. According to a large number of the respondents, physical activity really facilitated their regaining of health after surgery. Conclusions. The study showed that younger women (under 50 years) were more active. The women living in rural areas presented slightly higher mean general activity level. The average professional activity of the respondents increased with increasing level of their education. Mean activity of the subjects in terms of movement was the greatest among women with secondary education, slightly less among women with higher education and the smallest among women with primary education.

The aim of the study was to determine the role of self-efficacy, outcome expectancies, and risk perception (including consequences of mastectomy) in formulating the intention to undergo breast reconstruction in 178 women after total mastectomy. The social-cognitive variables were measured in the context of breast reconstruction, while depression was assessed using the Beck Depression Inventory. The structural equation modeling revealed that among the predictors there were only two that accounted for the intention to undergo breast reconstruction, i.e. self-efficacy and outcome expectancies (R2 = .67). Subsequent analyses of the related moderators, i.e. depression, age, and duration of the disease indicated a good fit to the data. Nevertheless, in subgroups with poorer resources (older age, depression, and longer duration of the disease) the direct effects of self-efficacy on intention were less noticeable or non-existent. The results suggest that self-efficacy may play the regulating role in making a breast reconstruction decision if individual resources are taken into account.

Breast cancer screening is based on X-ray mammography, while ultrasound is considered a complementary technique with improved detection in dense tissue. However, breast cancer screening requires a technique that provides repeatable results at the inspection interval which cannot be achieved with manual breast exploration. During the last years there have appeared several approaches to overcome this limitation by means of automated ultrasonic tomography performed with motorized probes or with a large set of array transducers. This work addresses these problems by considering a quite simple and low-cost arrangement, formed with a ring of conventional medical-grade array probes which are multiplexed to the electronics to build Full Angle Spatially Compounded (FASC) images. The work analyzes the performance of such arrangement in terms of resolution and isotropy, showing by numerical modelling and experimentally that it provides high resolution and homogeneity in the whole imaged region. The implementation of this technique would provide more than one circular FASC per second and a whole breast volume image in 1–2 minutes with conventional technology, a process fast enough to be clinically useful. Moreover, the automated technique is repeatable and can be used by the clinician to perform immediately the diagnosis without requiring additional data processing.

Breast cancer is the most common malignant cancer among women. Both drug resistance and metastasis are major problems in the treatment of breast cancer. Therefore, adjuvant therapy may improve patients’ survival and affect their quality of life. It is suggested that epigallocatechin gallate (EGCG) which is well known for its chemopreventive activity and acts on numerous molecular targets may inhibit the growth and metastasis of some cancers. Hence, discovering the metastatic molecular mechanisms for breast cancer may be useful for therapy.The aim of the study was to determine the effect of EGGC on the mRNA expression level of genes such as ZEB1, ABCB1, MDM2, TWIST1 and PTEN in MCF-7 breast cancer cells. MCF7/DOX were cultured in the presence of 0.2 μM DOX and EGCG (20-50 μM). The mRNA expression level was determined by real-time quantitative PCR using RealTime ready Custom Panel 96 kit. Our results showed an important increase (about 2-fold for 20 μM EGCG + 0.2 μM DOX and 2.5-fold for 50 μM EGCG + 0.2 μM DOX, p<0.05) in ZEB1 expression levels. In case of ABCB1 gene lack of influence on the mRNA level was observed (p>0.05). We also observed significant decrease of ZEB1 expression in MCF7 cells with 20 μM and 50 μM EGCG (p<0.05). In addition, EGCG (20 μM) caused an increase of MDM2 and PTEN mRNA levels in almost 100% (p<0.05) and 40% (p>0.05), respectively. Lack of the influence of EGCG was noted for the TWIST1 gene expression. In case of MCF7/DOX we showed an increase of mRNA level of PTEN gene about 50% (p<0.05). These results suggest that EGCG may be potentially used in adjuvant therapy in the breast cancer treatment.

Introduction. The results of breast cancer treatment depend mainly on better detection in mammography screening and, consequently, a higher proportion of women with early stage of the disease. They depend also on a better access to health care services and the effectiveness of oncological treatment. One of the methods of breast cancer control is a breastconserving surgery. With a proper patients’ classification for the treatment, the results of the breast-conserving surgery do not differ in relation to mastectomy. That’s why, the availability of breast-conserving surgical methods is particularly important, especially in a population in which a population screening is conducted. Objective. The analysis of the selected aspects of the breast cancer treatment’s standard in Podlaskie Voivodeship. Patients and methods. In years 2001–2002, 709 cases of women with breast cancer were reported to the Voivodeship Cancer Registry in Bialystok. 659 women were diagnosed with a primary invasive breast cancer. Based on a cohort of 499 women treated with radical methods the following indicators, recommended by WHO for the evaluation of the breastconserving surgery’s availability, were calculated: a percentage of patients with whom breast-conserving surgery was used, a proportion of breast cancer cases receiving post-operative breast radiotherapy after breast-conserving surgery and a proportion of breast-conserving surgery in pT1 cases. Results. The breast-conserving surgery has been used much less frequently in Podlaskie Voivodeship than in Western Europe, but more frequently than in Poland in general. Conclusion. It should be aimed to provide a surgical treatment with the use of breast-conserving surgical methods to the highest possible percentage of patients. The increase of the percentage of patients treated with breast-conserving surgery methods can be a result of an effective screening realisation.

Hereditary predisposition to breast cancer determined in large part by loss of function mutations in one of two genes BRCA1 and BRCA2. Besides BRCA1 and BRCA2 other genes are also likely to be involved in hereditary predisposition to breast cancer. TopBP1 protein is involved in DNA replication, DNA damage checkpoint response and transcriptional regulation. Expression of TopBP1 gene at the mRNA level was analyzed by semiquantitative reverse transcription-polymerase chain reaction (RT-PCR) in 94 samples of hereditary breast cancer. Analysis of TopBP1 mRNA level showed that expression of TopBP1 is significantly downregulated in poorly differentiated breast cancer (grade III according Bloom-Richardson system (P<0.05).

Breast cancer is the most common malignant tumour developing in Polish women. Each year, over 11,000 new cases are registered in our country. Breast cancer is the leading cause of mortality due to malignant neoplasms in the females. The aim of paper, apart of presenting extremely advanced clinical cases of breast cancer and management strategies implemented, is to highlight the complexity of this problem, due overlapping of several medical and social factors. Inoperable, locally advanced breast cancer usually presents clinically as a giant primary tumour, frequently with extensive metastases in the axillary fossa. Giant breast tumour, with a diameter exceeding 5 cm, frequently extensively invading skin and deeper tissues, is usually accompanied by lymphatic oedema. Occurrence of such clinically advanced and morphologically gigantic forms of breast cancer in the material of Palliative Care Unit is most often due to one of the following: dynamic progression or recurrence of the disease, refusal of cause-oriented treatment or its interruption and extreme neglect. The main treatment option in this group of patients is aggressive induction chemotherapy. If surgery is contraindicated, patients undergo radical radiotherapy. Principal goal of therapy at a palliative care unit is relief of symptoms and improvement of quality of life, realized by medical and non-medical means. Psychological and spiritual support aims at creating a positive attitude of the patient towards his or her condition. The key factor is partner-like relations with patients, preservation of their autonomy and respect for their decisions concerning continuation of further treatment.
Rak piersi jest najczęstszym nowotworem złośliwym występującym u kobiet w Polsce. Każdego roku w naszym kraju rejestruje się ponad 11 000 nowych zachorowań. Rak piersi jest także przyczyną największej liczby zgonów spowodowanych przez nowotwory złośliwe u kobiet. Celem pracy - oprócz przedstawienia krytycznie zaawansowanych obrazów klinicznych raka piersi i metod postępowania leczniczego - jest zwrócenie uwagi na złożoność problemu i nakładanie się różnorodnych czynników zdrowotnych i społecznych. Nieoperacyjny, miejscowo zaawansowany rak piersi obejmuje przypadki, dla których typowe są duży pierwotny guz oraz najczęściej rozlegle przerzuty w dole pachowym. Olbrzymi guz piersi - o średnicy przekraczającej 5 cm - często nacieka rozlegle skórę, tkanki położone głębiej, zazwyczaj towarzyszy mu obrzęk limfatyczny. Występowanie zaawansowanych klinicznie - często olbrzymich morfologicznie - postaci raka piersi w analizowanym materiale Oddziału Opieki Paliatywnej ma najczęściej trzy źródła: krytyczny rozwój lub nawrót choroby, odmowę leczenia przyczynowego lub jego zaniechanie oraz krytyczne zaniedbania. Podstawowym leczeniem systemowym w tej grupie chorych jest agresywna chemioterapia indukcyjna. W przypadku przeciwwskazań do zabiegu operacyjnego leczeniem z wyboru jest radykalna radioterapia. Zasadniczy cel leczenia na oddziale opieki paliatywnej to przede wszystkim łagodzenie objawów choroby oraz poprawa jakości życia, co uzyskuje się na drodze postępowania medycznego i niemedycznego. Poprzez wsparcie psychologiczne i duchowe dąży się do tego, aby pacjent osiągnął pozytywną ocenę swojego stanu. Istotne jest podmiotowe traktowanie chorego, dbałość o zachowanie jego autonomii i szanowanie jego decyzji co do dalszego postępowania terapeutycznego.

The breast cancer suppressor proteins BRCA1 and BRCA2 interact with RAD51, a protein essential for maintaining genomic stability by playing a central role in homology-dependent recombinational repair of the DNA double-strand breaks. Therefore, genetic variability in the RAD51 gene may contribute to the appearance and/or progression of breast cancer. A single nucleotide polymorphism in the 5'- untranslated region of RAD51 (a G to C substitution at position 135, the G/C polymorphism) is reported to modulate breast cancer risk. We investigated the distribution of genotypes and frequency of alleles of the G/C polymorphism in breast cancer. Tumor tissues were obtained from postmenopausal women with node-negative and node-positive breast carcinoma with uniform tumor size. Blood samples from age matched healthy women served as control. The G/C polymorphism was determined by PCR-based MvaI restriction fragment length polymorphism. The distribution of the genotypes of the G/C polymorphism did not differ significantly (P >0.05) from those predicted by the Hardy-Weinberg distribution. There were no differences in the genotype distribution and allele frequencies between node-positive and node-negative patients. There were no significant differences between distributions of the genotypes in subgroups assigned to histological grades according to Scarf-Bloom-Richardson criteria and the distribution predicted by Hardy-Weinberg equilibrium (P >0.05). Our study implies that the G/C polymorphism of the RAD51 gene may not be directly involved in the development and/or progression of breast cancer and so it may not be useful as an independent marker in this disease.

In this study, for the diagnosis and classification of breast cancer, we used and applied five classical pre-trained deep convolutional neural network models (DCNN) which have proven successful many times in different fields (ResNet-18, AlexNet, GoogleNet and SuffleNet). To make pre-trained DCNN models suitable for the purpose of our study, we updated some layers according to the new situation by using the transfer learning technique. We did not change the weights of all layers used in these five pre-trained DCNN models. Instead, we just gave new weights to the new layers so that new layers adapt faster to emerging new DCNN models. With these five pre-trained DCNN models, we have realized a quadruple classification as "cancer", "normal", "actionable" and "benign", and a binary classification as "actionable + cancer" and "normal + benign". With these two separate classification and diagnosis studies, we have carried out comparative experimental examination and analysis of pre-trained DCNN models for breast cancer diagnosis. In the study, it was concluded that successful results can be achieved with pre-trained DCNN models without extra time-consuming procedures such as feature extraction, and DCNN can perform quite successfully in cancer diagnosis and image comment.

A frequent dilemma faced by an oncologist about to take decision on a chemotherapeutic regime for patients with metastatic breast cancer is how to maintain balance between the expected treatment efficacy and predictable adverse events. In the case of anthracyclines what is problematic is their significant cardiotoxicity, in particular with reference to patients previously treated with them as part of adjuvant therapy. A relatively new method is replacement of conventional doxorubicin with its non-pegylated form, encapsulated in liposomes, which is capable of minimizing the side effects without compromising its therapeutic index. The present article discusses three cases of patients treated with non-pegylated liposomal doxorubicin (NPLD) as first-line chemotherapy administered for metastatic breast cancer. In all three cases considerable clinical improvement was observed, involving remission of pathological lesions and good quality of life.

Purpose: To determine the toxicity and efficacy profile of non-pegylated doxorubicin in combination with capecitabine administered according to LipAX regimen. Materials and methods: The analysis included 5 female patients undergoing first-line treatment for metastatic breast cancer. Patients received non-pegylated doxorubicin intravenously and oral capecitabine at usual doses used for monotherapy, until disease progression or unacceptable toxicity. Results: Patients received a total of 26 complete treatment cycles according to LipAX regimen. During treatment, 15 toxicities occurred, including 7 adverse events with grade 3 severity. Only two haematological toxicities were observed, and the other 13 were of a non-haematological nature. Only one patient experienced no adverse events. Apart from symptomatic treatment, the capecitabine dose was reduced twice and the non-pegylated doxorubicin once. Positive clinical outcomes were observed in 4 patients, and disease progression was reported in the case of 1 patient in the course of the treatment. The median time to disease progression was 10.4 months, and the median overall survival was 34.2 months. During the 54-month follow-up, 4 of the patients died. The surviving patient continues treatment. Conclusions: Therapy according to the LipAX regimen was relatively well tolerated, however, since the majority of patients discontinued treatment due to adverse events, and not disease progression, an adequate reduction in the cytostatic doses should be considered. The use of the LipAX regimen may contribute to the achievement of long-term remission in some patients, a fact that encourages further studies on this form of therapy.

We present a case of 68-year-old female with severe symptomatic aortic stenosis and locally advanced breast cancer disqualified from mastectomy due to heart failure and from aortic valve replacement due to malignant neoplasm. The patient received neoadjuvant chemotherapy without anthracyclines. The aortic valve replacement was performed and then mastectomy and lymphadenectomy were made without hemodynamic complications. Adjuvant hormonotherapy was started. During 42 months of follow-up the patient remained free of recurrent cancer disease as well as no progression of heart failure was observed.

Introduction: Endocrine and metabolic paraneoplastic syndromes in the course of malignant tumors result from ectopic production of hormones or hormone precursors in tumor cells. Production of hormones by endocrine tumors is relatively frequent, while such production by adenocarcinoma cells is definitely rare. The study presents a case of triple-negative invasive breast cancer, with the ectopic secretion of ACTH (adrenocorticotropic hormone), which provokes serious metabolic disorders. Materials and methods: The patient was admitted to hospital with symptoms of Cushing`s syndrome. Diagnostic tests revealed that the cause of metabolic disorders was breast cancer. After proper preparation, the patient was qualified for surgery. Results: After the mastectomy, the patient’s metabolism stabilized. The patient underwent adjuvant chemotherapy and radiotherapy. Four months after the last cycle of systemic treatment, cancer dissemination was found. The patient was treated with second-line chemotherapy, however, control CT revealed progression. The patient died 20 months after surgery and two months after the last cycle of chemotherapy. Conclusions: The case reported in this study – triple-negative invasive breast cancer, responsible for ectopic production of ACTH and causing Cushing’s syndrome – is a rare phenomenon. Treatment of patients with breast cancer showing hormonal activity should not differ from general rules applied for breast cancer. However, due to accompanying metabolic disturbances, the patients need individualized oncological approach, precise diagnostic tests, and adequate preoperative preparation.

Introduction: Time orientation can significantly improve health-related prevention behaviour and influence disease outcome through boosting health-oriented behaviour. This study aims to compare time perspective between diabetic and cancerous patients, and healthy people. Methods: A cross-sectional descriptive study was conducted on 300 patients (105 healthy, 195 patients: 108 type 2 diabetes and 87 breast and digestive system cancer) aged 20 to 70 years (average age of 45.4 years). The samples were separated into three groups based on a convenience sampling method and were matched in terms of gender, age, education and monthly income. Data was collected through the Zimbardo Time Perspective Inventory (ZTPI-56) questionnaire and then evaluated with analysis of covariance (F test) followed by Fisher’s Least Significant Difference (LSD) test. Results: The three groups showed different time perspectives (F = 4.213, p < 0.05) and different ranking in time orientation. Conclusion: Our findings show that the disease and its type can significantly impact the time orientation of patients. Therefore, to prevent potential subsequent outcomes, the patients’ time perspective towards disease should be improved, especially under disease conditions.

Wstęp. Obecnie rak piersi plasuje się na pierwszym miejscu pod względem zachorowalności u kobiet w Polsce. W celu leczenia raka piersi stosuje się mastektomię. Przygotowanie pacjentki do samoopieki po mastektomii obejmuje nie tylko sferę fizyczną, ale także psychiczną. Cel. Celem pracy jest analiza wybranych metod samoopieki dla pacjentki po zabiegu mastektomii. Materiał i metody. W pracy dokonano analizy literatury poświęconej zagadnieniom opieki pielęgniarskiej nad pacjentką po zabiegu mastektomii oraz właściwym przygotowaniu jej do samoopieki. Przegląd. Rozwojowi nowotworu piersi sprzyja szereg różnych czynników. Istotnym aspektem  leczenia jest określenie stadium zaawansowania i dobranie odpowiedniej terapii. Jedną z metod postępowania leczniczego jest mastektomia. Wówczas ważnym elementem procesu terapeutycznego jest wyjaśnienie pacjentce celu zabiegu i przygotowanie jej do dalszej samoopieki. Wnioski. Rak piersi istotnie modyfikuje jakość życia pacjentek. Po zabiegu mastektomii niezbędne jest zapewnienie prawidłowej opieki pielęgniarskiej pod względem psychicznym oraz fizycznym. Wpływa to na komfort pacjentki w trakcie pobytu, oraz po wyjściu ze szpitala.
Introduction. Currently, breast cancer ranks first in terms of incidence in women in Poland. A mastectomy is used to treat breast cancer. Preparing a patient for self-care after mastectomy covers not only the physical but also the mental sphere. Aim. The aim of the study is a methodological analysis of self-care for patient for mastectomy. Materials and methods. The study analyzes the literature devoted to the issues of nursing care for a patient after mastectomy and proper preparation for self-care. Overview. A number of factors contribute to the development of breast cancer. An important aspects of treatment is determining the stage advancement and selecting the appropriate therapy. One of therapeutic methods is mastectomy. Then, an important element of therapeutic process is explaining to the patient the purpose of the procedure and preparing her for further self-care. Conclusions. Breast cancer significantly modifies the quality of life of patients. After the mastectomy procedure, it is necessary to provide proper mental and physical care for nursing. This affects the patient's comfort during the stay and after leaving the hospital.

Introduction. Despite great progress in the diagnostics and treatment, breast cancer still remains the most frequently diagnosed malignant cancer in women. The outcomes of cancer treatment depend, to a considerable extent, on the degree of acceptance of the illness. The objective of the study was determination of the level of acceptance of the disease among the examined women with breast cancer. Materials and method. The study covered a group of 231 women who received treatment for breast cancer in the Rev. Bronislaw Markiewicz Subcarpathian Oncology Centre in Brzozow, south-eastern Poland, and conducted by the method of a diagnostic survey, using an author constructed questionnaire and the standard Acceptance of Illness Scale, adapted by Z. Juczyński. Statistical analyses were performed using the software IBM SPSS v. 21.0.0.1. The p values p≤0.05 were considered statistically significant. Results. The mean level of acceptance of the illness in the examined group of women was 26.53 (SD = 7.71; Me = 26). The largest number of respondents (50.6%) obtained scores within the range of 20–29, according to the AIS Scale. From among the adopted variables, the level of acceptance of the illness significantly varied according to age and occupational activity. However, no such differences were confirmed according to the place of residence, marital status, education level, or the type of occupation performed. Conclusions. In the examined group of women, a mediocre level of acceptance of cancer was observed. The respondents’ age and occupational situation exerted a significant effect on the level of acceptance of the illness. The results of the study should be considered as satisfactory because the number of women with a high level of acceptance of breast cancer was more than twice as high as those ones with a low acceptance of the disease.

Objective. The aim of the study was evaluation of satisfaction with life among women ill with breast cancer, with consideration of selected demographic and social factors. Materials and method. In the study participated 121 patients from the Sub-Carpathian Oncology Centre in Brzozów, Poland, who had undergone surgical treatment due to breast cancer. The method of a diagnostic survey was used. The research instruments were an author-constructed questionnaire and the Satisfaction with Life Scale. Results. Women who received surgical treatment due to breast cancer evaluated their satisfaction with life on a mediocre level. Younger patients had a lower satisfaction with life than those who were older. Women living in urban areas evaluated their satisfaction with life in more positive terms than rural women. In turn, education level, marital status and material standard had no effect on the level of satisfaction with life among the women in the study. Conclusions. According to the SWLS, women with the diagnosis of breast cancer obtained a mean result of 5.64 sten scores Younger women evaluated their satisfaction with life in more positive terms than those who were older. Women who lived in urban areas obtained a considerably higher result according to the SWLS, compared to rural inhabitants. Marital status, education level, material standard and occupational activity of the women were insignificant in the respondents’ evaluation of satisfaction with life.

Background: The aim of this study was to establish whether the gene expression of estrogen receptor alpha (encoded by ESR1) correlates with the expression of glutathione peroxidase 1 (encoded by GPX1) in the tumor and adjacent tumor-free breast tissue, and whether this correlation is affected by breast cancer. Such relationships may give further insights into breast cancer pathology with respect to the status of estrogen receptor. Methods: We used the quantitative real-time PCR technique to analyze differences in the expression levels of the ESR1 and GPX1 genes in paired malignant and non-malignant tissues from breast cancer patients. Results: ESR1 and GPX1 expression levels were found to be significantly down-regulated by 14.7% and 7.4% (respectively) in the tumorous breast tissue when compared to the non-malignant one. Down-regulation of these genes was independent of the tumor histopathology classification and clinicopathological factors, while the ESR1 mRNA level was reduced with increasing tumor grade (G1: 103% vs. G2: 85.8% vs. G3: 84.5%; p<0.05). In the non-malignant and malignant breast tissues, the expression levels of ESR1 and GPX1 were significantly correlated with each other (Rs=0.450 and Rs=0.360; respectively). Conclusion: Our data suggest that down-regulation of ESR1 and GPX1 was independent of clinicopathological factors. Down-regulation of ESR1 gene expression was enhanced by the development of the disease. Moreover, GPX1 and ESR1 gene expression was interdependent in the malignant breast tissue and further work is needed to determine the mechanism underlying this relationship.

S100A4 is a member of the S100 family of calcium-binding proteins that is directly involved in tumor metastasis. In the present study, we examined the potential role of S100A4 in metastasis in breast cancer and its relation with matrix metalloproteinase-13 (MMP-13). Analysis of 100 breast cancer specimens including 50 with and 50 without lymph node metastasis showed a significant upregulation of S100A4 and MMP-13 expression in metastatic breast cancer tissues. Positive immunoreactivity for S100A4 was associated with MMP-13 expression. Overexpression of S100A4 in the MDA-MB-231 breast cancer cell line upregulated MMP13 expression leading to increased cell migration and angiogenesis. SiRNA-mediated silencing of S100A4 downregulated MMP13 expression and suppressed cell migration and angiogenesis. Moreover, neutralization of MMP-13 activity with a specific antibody blocked cell migration and angiogenesis in MDA-MB-231/S100A4 cells. In vivo siRNA silencing of S100A4 significantly inhibited lung metastasis in transgenic mice. The present results suggest that the S100A4 gene may control the invasive potential of human breast cancer cells by modulating MMP-13 levels, thus regulating metastasis and angiogenesis in breast tumors. S100A4 could therefore be of value as a biomarker of breast cancer progression and a novel therapeutic target for human breast cancer treatment.

Female breasts have been admired for a variety of reasons throughout the ages. This can influence a preoccupation with breast size. As a result, augmentation mammoplasty (AM) has become a sought-after elective cosmetic procedure, especially in western culture. Research has confirmed the psychological benefits of AM, but it has also shown that some women with a self-perceived inadequate breast size who request AM can present with psychological problems. Likewise, authorities agree that the diagnosis of breast cancer which remains one of the most frequently diagnosed cancers among women worldwide, can have an adverse effect on women psychologically, and that the number of patients who live with the psychological sequelae of both the disease and its treatment continue to rise. Several factors relate to psychological distress in women coping with these issues. In particular, self-esteem and body image-related problems faced by women can add a burden to their psychological well-being. This article reviews some of these issues and psychological treatment options to enhance women’s adjustment in this regard.

Purpose. Topotecan, a semisynthetic water-soluble derivative of camptothecin exerts its cytotoxic effect by inhibiting topoisomerase I and causes double-strand DNA breaks which inhibit DNA function and ultimately lead to cell death. In previous studies it was shown that camptothecin causes ROS formation. The aim of this study was to investigate if Topotecan like camptotecin causes oxidative stress in MCF-7 human breast cancer cell line. Determining the oxidant effect of Topotecan may elucidate a possible alternative mechanism for its cytotoxicity. Experimental design. MCF-7 cells were cultured and exposed to Topotecan for 24 h at 37°C. The viability of the cells (% of control) was measured using the colorimetric 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) assay. Lipid peroxidation (TBARS), protein oxidation (carbonyl content), sulfhydryl, glutathione (GSH) levels, superoxide dismutase (SOD), catalase (CAT) and glutathione peroxidase (GPx) activities were determined in MCF-7 cells with and without Topotecan incubation. Results. We found the IC50 concentration of Topotecan as 0.218 µM in MCF-7 cells. This concentration of Topotecan was used in the incubations of the cells. Our data indicated increased oxidative status, as revealed by increased lipid peroxidation and protein oxidation, and decreased GSH and sulfhydryl levels in MCF-7 cells exposed to Topotecan compared to control cells. In contrast, there was a slight increase in SOD and a significant increase in GPx and catalase activity in MCF-7 cells incubated with Topotecan compared to the control. Conclusions. These results support our hypothesis that Topotecan increases oxidative stress in MCF-7 cells.

Background. Breast cancer is the most common malignant tumour in women in Poland. To reduce the risk of breast cancer, appropriate prevention is necessary. Numerous studies conducted over many years in Poland and throughout the world have demonstrated a significant effect of selenium on prevention of disease, including breast cancer in women. The following paper aims to present the literature on effects of selenium (Se) on prevention of breast cancer in women. Material and methods. Based on national and international literature, the paper presents information on the role of selenium (Se) in prevention of breast cancer in women. Results. Numerous studies conducted in research centres in Poland and abroad have shown that female patients with breast cancer and individuals with gastrointestinal cancer have significantly lower selenium concentration in their blood serum and whole blood, as well as significantly lower GSH - Px activity in plasma and red blood cells, compared to healthy women. Low selenium concentration may indicate an increased risk of breast cancer. Selenium is an essential co-factor in the production of antioxidant enzymes and may affect the development of cancer. Clinical trials which assessed selenium content in food showed that its supplementation reduced cancer mortality. Conclusions. Results of numerous national and prospective international studies indicate that low selenium intake and/or concentration in serum/plasma/nails is a high-risk marker of the majority of cancers, including breast cancer in women.
Wprowadzenie. Nowotwór gruczołu piersiowego jest najczęstszym nowotworem złośliwym u kobiet w Polsce. Aby zmniejszyć ryzyko zachorowania na raka piersi niezbędna jest odpowiednia profilaktyka. Liczne badania prowadzone od wielu lat w Polsce i na świcie pokazały znaczące działanie selenu w profilaktyce chorób nowotworowych – w tym nowotworów piersi u kobiet. Celem niniejszej pracy jest przedstawienie w świetle literatury przedmiotu wpływu selenu (Se) na zapobieganie nowotworom gruczołu piersiowego u kobiet. Materiał i metody. W pracy zaprezentowano na przykładzie dostępnej literatury przedmiotu (krajowej jak i zagranicznej) informacje dotyczące roli selenu (Se) w profilaktyce nowotworu gruczołu piersiowego u kobiet. Wyniki. Liczne badania prowadzone w ośrodkach zarówno w Polsce jak i zagranicą wykazały, że u pacjentek z rakiem gruczołu piersiowego oraz u osób z nowotworami przewodu pokarmowego stwierdza się znamiennie niższą koncentrację selenu w surowicy, w pełnej krwi oraz znamiennie niższą aktywność GSH - Px w osoczu i krwinkach czerwonych w porównaniu do kobiet zdrowych. Niska koncentracja selenu może być wskaźnikiem zwiększonego ryzyka raka gruczołu piersiowego. Selen jest ważnym kofaktorem w produkcji enzymów antyoksydacyjnych i może wpływać na przebieg procesu nowotworowego. W próbach klinicznych związanych z oceną zawartości Se w pożywieniu wykazano, że suplementacja Se zmniejsza śmiertelność z powodu raka. Wnioski. Wyniki licznych badań prospektywnych krajowych i zagranicznych wskazują, iż niskie spożycie selenu i/lub stężenia tego pierwiastka w surowicy/osoczu/paznokciach jest markerem wysokiego ryzyka zachorowania na większość nowotworów, w tym nowotworu gruczołu piersiowego u kobiet.

The aim of this study was to expand our knowledge about anticancer activity of some polyamine derivatives with quinoline or chromane as terminal moieties. Tested compounds were evaluated in vitro towards metastatic human prostate adenocarcinoma (PC3), human carcinoma (DU145) and mammary gland adenocarcinoma (MCF7) cell lines. Cell viability was estimated on the basis of mitochondrial metabolic activity using water-soluble tetrazolium WST1 to establish effective concentrations of the tested compounds under experimental conditions. Cytotoxic potential of polyamine derivatives was determined by the measurement of lactate dehydrogenase activity released from damaged cells, changes in mitochondrial membrane potential, the cell cycle distribution analysis and apoptosis assay. It was revealed that the tested polyamine derivatives differed markedly in their antiproliferative activity. Bischromane derivative 5a exhibited a rather cytostatic than cytotoxic effect on the tested cells, whereas quinoline derivative 3a caused changes in cell membrane integrity, inhibited cell cycle progression, as well as induced apoptosis of prostate and breast cancer cells which suggest its potential application in cancer therapy.

Significant, unexplained disparities in the incidence of lung cancer in men and breast cancer among some neighbouring populations of the Pomorskie voivodeship poviats were revealed. They were not found for lung cancer in women. Examinations were conducted on data from the Pomeranian Cancer Registry. A hypothesis was put forward that the observed accumulation of certain surnames in certain poviats argues for the repetitive occurrence of certain features of the genotype in populations inhabiting these areas, which may have an influence on the emergence of certain cancers and play a role in the emergence of the above mentioned disparities.

Mesodermal mixed malignant tumours (MMMT) are the rare neoplasms of postmenopausal women, with extremely poor prognosis. They constitute about 1-2% of all gynaecological malignancies and about 2-5% of uterine corpus malignancies. Due to rare frequency, the exact knowledge about their characteristics is difficult. Therefore, the treatment is based on the treatment methods used in endometrial cancer. For most authors, because of their clinical course, they are treated as more aggressive endometrial carcinomas. In the literature, there were described a lot of cases of endometrial cancer and MMMT development after the chronic therapy with tamoxifen, due to breast cancer, but rare data suggest the possibility of coexistence of the two above neoplasms, without the previous therapy with tamoxifen. In this paper, we present the case of woman, aged 78, with tumour mixtus mesodermalis malignus corporis uteri, coexistent with breast cancer, and unilateral lymph nodes metastases, coming from the uterine tumour, with no previous therapy. In the literature we did not meet any case of breast cancer, coexistent with MMMT with the axillary lymph node metastases of MMMT component.
Złośliwe mieszane guzy mezodermalne (MMMT) macicy to rzadkie nowotwory o bardzo agresywnym przebiegu, występujące u kobiet w wieku pomenopauzalnym. Stanowią 1-2% wszystkich nowotworów narządów płciowych kobiecych i 2-5% nowotworów trzonu macicy. Ze względu na niezbyt częste występowanie dokładne poznanie charakterystyki tych nowotworów jest utrudnione. Wobec powyższego, sposób postępowania jest oparty na schematach postępowania w raku trzonu macicy. Przez większość autorów, z uwagi na przebieg kliniczny, są one traktowane jako raki endometrium o agresywniejszym przebiegu. W piśmiennictwie opisano wiele przypadków występowania raka endometrium, jak również MMMT po przewlekłej terapii tamoksyfenem z powodu raka piersi, natomiast nieliczne dane wskazują na możliwość współwystępowania obu nowotworów bez uprzedniej terapii. W pracy przedstawiono przypadek 78-letniej kobiety, u której stwierdzono guza złośliwego mieszanego mezodermalnego trzonu macicy oraz raka piersi z jednoczesnym występowaniem przerzutów do węzłów chłonnych po stronie zdiagnozowanego raka piersi, pochodzących z guza macicy. Nie spotkano natomiast opisu przypadku współwystępowania MMMT i raka piersi z przerzutami MMMT do pachowych węzłów chłonnych.

Wstęp: Nowotwory to druga po chorobach układu krążenia przyczyna zgonów w Polsce. Najczęściej diagnozowanymi chorobami nowotworowymi u kobiet są rak piersi i szyjki macicy. Metody prewencji są tanie i ogólnodostępne. Najpopularniejsze badania profilaktyczne w kierunku raka piersi to samobadanie oraz mammografia. W profilaktyce raka szyjki macicy podstawowymi narzędziami diagnostycznymi są: badanie ginekologiczne i skryning cytologiczny. Cel pracy: Określenie wiedzy kobiet na temat profilaktyki nowotworów piersi i szyjki macicy oraz częstości korzystania z badań profilaktycznych. Materiał i metody: Badania zostały przeprowadzone metodą sondażu diagnostycznego, techniką ankiety w grupie 80 kobiet. Wyniki: 80% (64) kobiet przyznaje, że lekarz pierwszego kontaktu nie informuje ich o możliwości wykonania badań profilaktycznych. 96,25% (77) kobiet deklaruje, iż potrafi wykonać samobadanie piersi, natomiast zaledwie 21,25% (17) kobiet wykonuje je regularnie. Wnioski: Należy nasilić działania edukacyjne na poziomie podstawowej opieki zdrowotnej, ukierunkowane na udział pacjentek w badaniach profilaktycznych i samoopiekę.
Background: Cancer is the second, after cardiovascular diseases, cause of deaths in Poland. The breast cancer and the cervical cancer are the most frequently diagnosed types in women. The prevention methods are cheap and widely available. Mammography and breast self-examination are the most common screening tests used to diagnose breast cancer. The diagnostic tools for cervical cancer include basic gynaecological examinations and cytological screening. Aim of the study: The aim of the study is to assess the knowledge of women about the prevention methods used to diagnose breast and cervical cancer, and to check the frequency of prophylactic examinations use. Material and methods: The studies were conducted using a diagnostic survey with a questionnaire for a group of 80 women. The results were as following: 80% (64) of women reported that their primary care doctor did not inform them about the possibility of preventive examinations. 96.25% (77) of them declared that they can do the breast self-examination while, unfortunately, only a little over 21.25% (17) of the respondents do it regularly. Conclusions: Higher educational efforts should be made at the level of primary health service and they should be targeted at female patients’ wider participation in the preventive diagnostics and self-care.

In only the past century, the landscape of breast cancer treatment has completely changed. The Halstedian hypothesis of the “contiguous spread” of breast cancer has been replaced by a consideration of its systemic nature. Today, patients with early-stage breast cancer are managed with breast-conserving therapy, which is as effective as mastectomy. Sentinel lymph node biopsy has largely replaced axillary lymph node dissection. Post-operative radiotherapy, chemotherapy and endocrine therapy have increased survival. Pre-operative cytotoxic therapy allows for less extensive surgery and for a curative resection even in more advanced stages. Rapid progress in molecular oncology revealed a large heterogeneity of breast cancer, resulting in a more personalized approach. Targeted therapies directed against epidermal growth factor receptor type 2 (HER2) have improved survival in HER2-positive breast cancer, which was once a poor-prognosis entity. Multi-gene prognostic signatures better predict prognosis and allow many patients to avoid chemotherapy. Personalized treatment has resulted in decreased toxicity and an improved quality of life. Within the past decades, breast cancer has become a good-prognosis malignancy with a five-year survival in the range of 80-85%. Future development of personalized medicine may further refine treatment based on the tumor’s molecular features.

Recent years have witnessed a dynamic development of mammary gland imaging techniques, particularly ultrasonography and magnetic resonance imaging. A challenge related to these studies is the increase in the precision of the anatomical assessment of breast, particularly for early detection of subclinical lesions, performance of ultrasound- guided biopsy procedures, and accurate preoperative location of pathological lesions so as to optimize the surgical treatment. Ultrasound imaging is a primary and baseline diagnostic procedure the patient with suspected pathological lesions within breast is referred to by the surgeon. Lesions visualized in ultrasound scans are classified according to the BI-RADS US assessment categories. The successive categories (2 through 6) encompass individual pathological lesions, estimating the risk of malignancy and provide guidelines for further diagnostic and therapeutic management. This article described the important aspects of ultrasonographic imaging of focal lesions within the breasts as significant from the standpoint of surgical treatment of patients falling within BI-RADS US categories 3, 4, 5, and 6. Attention is drawn to the importance of ultrasound scans in the assessment of axillary fossa lymph nodes before the decision regarding the surgical treatment.
W ostatnich latach obserwujemy dynamiczny rozwój metod obrazowania gruczołów piersiowych, zwłaszcza ultrasonografii i badania techniką rezonansu magnetycznego. Wyzwaniem dla tych badań pozostaje jeszcze bardziej precyzyjna ocena anatomii piersi, zwłaszcza pod kątem wczesnego wykrywania zmian subklinicznych, monitorowanie zabiegów biopsyjnych pod kontrolą obrazu ultrasonograficznego, dokładna lokalizacja przedoperacyjna nieprawidłowych zmian w celu optymalizacji leczenia chirurgicznego. Badanie ultrasonograficzne jest podstawowym i wyjściowym badaniem diagnostycznym, na które chirurg kieruje pacjentkę z podejrzeniem zmian patologicznych w piersiach. Uwidocznione w tym badaniu zmiany są na podstawie cech morfologicznych przydzielane do konkretnej kategorii wg klasyfikacji BIRADS-usg. Kolejne stopnie tej klasyfikacji (od 2 do 6) obejmują poszczególne nieprawidłowe zmiany (patologie piersi), szacują ryzyko ich złośliwości i zawierają wskazówki dotyczące dalszego postępowania diagnostyczno-terapeutycznego. W artykule omówiono ważne elementy obrazowania ultrasonograficznego zmian ogniskowych w piersiach, istotne z punktu widzenia leczenia chirurgicznego w poszczególnych kategoriach BIRADS-usg 3, 4, 5 i 6. Zwrócono uwagę na znaczenie badania ultrasonograficznego w ocenie węzłów dołów pachowych przed decyzją dotyczącą leczenia chirurgicznego.

Cancer-related microangiopathic hemolytic anemia is rarely recognized as a paraneoplastic syndrome with a very poor prognosis in cancer patients. The treatment and prognosis are significantly different from that in other thrombotic microangiopathies, such as thrombotic thrombocytopenic purpura and hemolytic uremic syndrome. The case of described patient demonstrates the diagnostic difficulties in diagnosing the causes of hemolytic anemia in patient with breast cancer and appropriate treatment.

The paper provides a preview of some work in progress on the computer system to support breast cancer diagnosis. Diagnosis approach is based on microscope images of the FNB (Fine Needle Biopsy) and assumes distinguishing malignant from benign cases. Studies conducted focus on two different problems, the first concern the extraction of morphometric parameters of nuclei present in cytological images and the other concentrate on breast cancer nature classification using selected features. Studies in both areas are conducted in parallel. This work is devoted to the problem of feature selection from the set of determined features in order to maximize the accuracy of classification. Morphometric features are derived directly from a digital scans of breast fine needle biopsy slides and are computed for segmented nuclei. The quality of feature space is measured with four different classification methods. In order to illustrate the effectiveness of the approach, the automatic system of malignancy classification was applied on a set of medical images with promising results.

Rak piersi jest najczęstszym nowotworem złośliwym u kobiet zarówno w Polsce jak i na świecie. Nowotwór spośród wszystkich chorób, odbierany jest przez kobiety jako bardzo silne przeżycie traumatyczne. Celem niniejszej pracy była ocena stopnia akceptacji choroby nowotworowej przez kobiety po mastektomii. Badaniu poddano 54 pacjentki po mastektomii, średnia wieku 57,2 lat. W badaniach posłużono się ankietą składającą się z pytań dotyczących danych demograficznych i skali AIS. W momencie rozpoznania 92,59% kobiet nie zaakceptowało choroby. W chwili obecnej u 75,93% stwierdzono akceptację raka piersi. Badane czują się zarówno potrzebne (42,59%) jak i niezależne od innych (31,48%). Ponad połowa pacjentek (53,70%) nie odczuwa, że jest ciężarem dla innych, a 44,44% uważa się za pełnowartościowe kobiety. Za samowystarczalne uważa się 27,78% ogółu badanych kobiet. Niski poziom akceptacji miało 10/54 kobiet. Wysoki poziom akceptacji zaobserwowano u co drugiej ankietowanej. Na poziom akceptacji nie wpływał wiek, miejsce zamieszkania i stopień wykształcenia badanych. Upływający czas od zabiegu operacyjnego miał wyraźny wpływ na akceptację choroby (p < 0,001). Wysoki poziom akceptacji miały kobiety, które pogodziły się z chorobą do 2 lat po operacji.
Aim. Breast cancer is the most common malignancy in women, both in Poland and worldwide. Unlike other diseases, cancer is perceived by women as a very severe trauma. The aim of this study was to assess the degree of cancer acceptance among women after mastectomy. Material and method. The study involved 54 patients after mastectomy, the mean age of 57.2 years. The study used a questionnaire consisting of questions and AIS scale. Results. At diagnosis, 92.59% of women did not accept the disease and at the moment of the study 75.93% did. Tested women felt both needed (42.59%) and independent from others (31.48%). More than a half of patients (53.70%) did not feel a burden to others and 44.44% considered themselves high-value women. 27.78% of the whole group stated they were self-sufficient. The low level of acceptance was observed in 10 out of 54 women. A half of women declared high level of acceptance. Age, place of residence and education of respondents did not influence the level of acceptance. The time from the surgery had a clear impact on the acceptance of the disease (p < 0.001). Conclusion. Very good level of acceptance was observed in women who got used to the fact they had cancer up to 2 years after surgery.

Lobular carcinoma in situ of the breast is classified as non-invasive malignant tumor of the breast. Its diagnosis is a marker for an increased risk of developing other histological types of breast cancer. Lobular carcinoma in situ is usually asymptomatic, with no characteristic radiological features. The diagnosis is often accidental while diagnosing lesions found on screening mammography. The most important clinical issue associated with the diagnosis of lobular carcinoma in situ is the possible risk of concurrent breast cancer of another histological type. According to the current standards of managing patients with lobular carcinoma in situ of the breast, surgical biopsy of the mass is most commonly recommended. Specific principles of therapy depend on the circumstances surrounding the diagnostic process. The complete diagnosis requires histopathological study of tissue specimens from paraffin blocks. It is necessary to determine the histological subtype of the identified tumor as different subtypes present significant differences as to the course of the disease (classic lobular carcinoma in situ, pleomorphic, florid or comedo with necrosis ones). Contrary to the classic lobular carcinoma in situ, other subtypes are characterized by significantly higher risk of coexisting infiltrating breast cancer. In such cases, it is necessary to perform surgical excision of the tumor. Diagnosis of LCIS does not require surgical treatment (possible active surveillance of the patient).
Rak zrazikowy gruczołu piersiowego in situ należy do postaci nieinwazyjnych nowotworów złośliwych o tej lokalizacji narządowej. Jego zdiagnozowanie jest markerem zwiększonego ryzyka rozwoju w przyszłości innych typów histologicznych raka piersi. Zrazikowy rak piersi in situ to najczęściej guz bezobjawowy, nieposiadający charakterystycznych cech radiologicznych. Do jego wykrycia dochodzi zwykle przypadkowo, w trakcie diagnostyki zmian uwidocznionych podczas skriningu mammograficznego. Najbardziej istotnym problemem klinicznym związanym ze zdiagnozowaniem tego nowotworu jest ryzyko współistnienia zmiany z innym rodzajem histologicznym raka piersi. Zgodnie z aktualnymi standardami leczenia chorych ze zrazikowym rakiem piersi in situ zalecana jest najczęściej biopsja chirurgiczna zmiany. Szczegółowe zasady postępowania terapeutycznego zależą od okoliczności zdiagnozowania guza. Rozpoznanie choroby wymaga oceny histopatologicznej preparatów tkankowych, pochodzących z bloków parafinowych. Niezbędne jest określenie podtypu histologicznego wykrytej zmiany, które wykazują znaczne różnice dotyczące przebiegu choroby (postać klasyczna, podtyp pleomorficzny, podtyp w stadium rozkwitu bądź podtyp comedo z martwicą). W odróżnieniu do typu klasycznego zrazikowego raka piersi in situ, pozostałe postaci cechuje istotnie wyższe ryzyko współistnienia form raka naciekającego piersi. W przypadku ich stwierdzenia konieczne jest operacyjne wycięcie zmiany. Wykrycie postaci klasycznej raka zrazikowego gruczołu piersiowego in situ nie wiąże się z taką koniecznością (możliwość aktywnego nadzoru chorych).
