Opis:
Potters and sculptors perform their work in very awkward postures. The purpose of this study was to analyse these postures. The modified Nordic questionnaire was used to analyse musculoskeletal discomfort. Rapid entire body assessment (REBA) and rapid upper limb assessment (RULA) were used to evaluate the subjects' postures. There were no significant differences between times of discomfort and the group of subjects. However, there were significant differences in discomfort in different body parts. The analysis indicated that various body postures were harmful to the subjects and that there were profound deviations from natural curvature of various body parts due to awkward body postures. Ergonomics intervention was required to improve the quality of life.

The present study aimed at investigating the control of upright quiet standing in pregnant women throughout pregnancy, and whether low-back pain exerts influence on this motor task. Methods: Myoelectric signals from postural muscles and stabilometric data were collected from 15 non-pregnant and 15 pregnant women during upright quiet standing. Electromyogram envelopes and center of pressure metrics were evaluated in the control group, as well as in pregnant women in their first and third trimester of pregnancy. A correlation analysis was performed between the measured variables and a low-back pain disability index. Results: Pregnant women exhibited a decreased maximum voluntary isometric activity for all postural muscles evaluated. Additionally, the activity of lumbar muscles during the postural task was significantly higher in the pregnant women in comparison to the non-pregnant controls. The soleus muscle maintained its activity at the same level as the gestation progressed. Higher postural oscillations were observed in the anteroposterior direction while mediolateral sway was reduced in the third trimester of pregnancy. No correlation was detected between the lowback pain disability index and neuromechanical variables. Conclusion: This study provides additional data regarding the functioning and adaptations of the postural control system during pregnancy. Also, we provide further evidence that postural control during quiet standing cannot be used to predict the occurrence of low-back pain. We hypothesize that the modifications in the neural drive to the muscles, as well as in postural sway may be related to changes in the biomechanics and hormonal levels experienced by the pregnant women.

Zespoły bólowe dolnego odcinka kręgosłupa są częstą przyczyną ograniczeń aktywności zawodowej i fizycznej. Współczesna medycyna wciąż poszukuje najlepszych sposobów rehabilitacji pacjentów ze schorzeniami kręgosłupa. W pracy zaprezentowano szczegółowy opis pacjenta cierpiącego na przewlekły zespół bólowy lędźwiowo-krzyżowej części kręgosłupa oraz dokładnie omówiono przebieg jego rehabilitacji za pomocą pochodzącej z Finlandii metody Documentation Based Care (DBC). Jest to nowoczesny system aktywnej terapii kręgosłupa. Metoda oparta jest na zasadach EBM (Evidence Based Medicine), a wykorzystuje się w niej specjalistyczne urządzenia do badania i ćwiczenia kręgosłupa z progresywnie wzrastającym oporem. W metodzie tej stosuje się również kinezyterapię funkcjonalną oraz edukację pacjenta w zakresie ergonomii pracy zawodowej i życia codziennego. Przytoczone piśmiennictwo wykazuje, że korzystne, długotrwałe efekty rehabilitacji można osiągnąć dzięki połączeniu opartych na faktach naukowych metod leczniczych oraz mulitidyscyplinarnego podejścia do pacjenta.
Low back pain syndromes are frequently responsible for physical and occupational activity limitations. The medicine of nowadays still seeks the best rehabilitation approaches for patients with spinal disorders. The paper presents a detailed case report, including description of the patient with chronic lumbosacral pain syndrome and the precise description of the course of patient rehabilitation procedure using Documentation Based Care (DBC) approach, developed in Finland. This is a modern system of active spine care. The approach is based on Evidence Based Medicine principles and uses specialist devices for examining and exercising the spine with progressively increasing resistance. It also includes functional kinesitherapy and patient education within the range of occupational activities and everyday life. The cited references allow obtaining long-term rehabilitation outcome by combining research-based therapeutic approaches and a multidisciplinary approach to the patient.

The objective of this study was to prevent occupational musculoskeletal injuries. Our workers stacked boxes of ceramics weighing 10–27 kg, making low back pain common in our enterprise. In all the stacking stations, recommended weight limits (RWL) were separately calculated using the revised National Institute for Occupational Health lifting equation. Since the boxes weighed significantly more than the RWL, we developed a new ergonomic design that completely changed the stacking process. The load put on the workers’ waist vertebrae in the new and the old stacking methods was compared to evaluate the success of the new ergonomic design, using Newton’s third law of motion. Thanks to the new ergonomic design, the load on the workers’ vertebrae decreased by 80%. Due to its simple technology and its very low cost compared to robots, the new ergonomic design can be commonly used in enterprises with repeated and constraining stacking.

Objectives Increasing bilateral gluteus medius co-activation has been identified as one of the most important factors in developing low back pain due to prolonged standing in healthy people. This study aims to investigate the impact of an anti-fatigue mat on the bilateral gluteus medius co-activation pattern and to report the low back pain subjectively in 2 different standing positions on the normal rigid surface and on the anti-fatigue mat. Material and Methods While carrying out an easy simulated profession, 16 participants who had no low back pain background were requested to stand for 2 h in each position, with and without using the anti-fatigue floor mat, respectively. At the beginning of standing process and at every 15 min until the time of 120 min lapses, electric activities for the bilateral gluteus medius co-activation and subjective pain level in low back area were collected by the surface electromyogeraphy (EMG) and the visual analogue scale (VAS), respectively in each position. Results The obtained findings revealed that the anti-fatigue mat significantly decreased subjective pain level in low back area among 15 participants (p < 0.05). However, there was objectively no significant difference in the bilateral gluteus medius co-activation pattern among the participants between the position 1 and the position 2 (p > 0.05). The findings obtained under this study related to the impact of the anti-fatigue mat upon the low back pain based on the increase of > 10 mm on the VAS threshold, which showed that this intervention had no significant impact upon decreasing the number of patients suffering from the low back pain and also minimizing the bilateral gluteus medius co-activation in both pain developer groups (p > 0.05). However, 73% of the participants preferred to apply it. Conclusions It seems that the anti-fatigue mat may be useful in reducing the low back pain although it objectively didn’t significantly change the gluteus medius co-activation pattern related to the low back pain.

Introduction. According to the European Occupational Diseases Statistics (EODS-2005) diseases related with musculoskeletal disorders occupy the first position on the obligatory list of occupational diseases. These disorders most frequently occur in the agriculture-hunting-forestry sector in such countries as: Finland, Holland, Germany, France and Spain (data: EU-OSHA). Materials and method. The study included a selected group of males – 414 forestry workers, employees of the State Forests, aged 25–65, mean age 48. The control group were 119 office workers aged 23–64, mean age 45.8. The basic research instrument was a questionnaire designed by specialists concerning pain complaints occurring in 7 areas of the motor system: neck, arms, upper and lower back, hips, knees and feet. Results. The results of the survey showed that in the selected group of forestry workers, among 7 investigated areas of the motor system, the most frequently reported pain complaints involved the lower part of the spine (272 persons, 65.7% of the total number of respondents). In the control group such pain complaints occurred in 55.5% (66) of office workers (p = 0.04). Forestry workers most often described this pain as permanent, radiating to the leg, or as an acute pain. According to the frequency of reporting pain, the area of the knees was placed on the second position (214 foresters; 51.7%). Conclusions. The work of a forester which consist, among other things, in relocation on foot or by vehicle over long distances along an uneven terrain and in various weather conditions, is a risk factor of the occurrence of musculosceletal disorders concerning the low back and knees.

Objectives: This study aims to assess muscle fatigue for kitchen workers during various tasks in a kitchen. Material and Methods: This study consists of 2 phases: a) surface electromyography (EMG) to determine muscle fatigue of trapezius and erector spinae muscles bilaterally for 12 healthy male volunteers during 30 min of Indian flatbread (chapati) preparation, b) self-administered questionnaire study after 30 min preparation to determine the impact on performance. Results: Surface EMG results show that the right trapezius muscle had significantly higher fatigue during kneading, rolling and roasting tasks. Similarly, right erector spinae muscle showed significantly higher fatigue during rolling and roasting tasks. Conclusions: Both phases of the study conclude that, even though the dynamic postural adjustment may reduce fatigue during the work schedule; the combination of risk factors like the repetitive task, forceful exertion and prolonged standing during working results in a high risk of developing muscle fatigue. Background: Low back pain is one of the most common musculoskeletal disorders with which patients come to a physiotherapist and it affects younger and younger people. Aim of the study: The aim of this paper was to assess the effect of mobilisation techniques for low spine on the range of motion and pain level in patients complaining of strong pain in that area. Material and methods: The study population consisted of 20 patients, aged between 28 and 67 years old (x = 47.55±12.04) with low back pain. Physical examination was performed to determine the movements which increased the pain. To assess the severity of pain, an analogue VAS scale was used. During treatment, “closing” techniques for the spinous processes were used. The treatment was carried out once and its duration was half an hour. After the therapy, the measurements were repeated. Results: A significant reduction of pain was observed after the treatment in the following movements: extension (p ≤ 0.001), lateral flexion to the left side (p < 0.000), lateral flexion – right side (p < 0.000), rotation to the left (p ≤ 0.014), rotation to the right (p ≤ 0.016). An analysis of the Student’s t-test results showed statistically significant (p < 0.001) difference in the range of motion during extension before and after treatment, with greater range after treatment. Conclusions: The techniques of manual mobilization of the lumbar spine are effective in decreasing pain and increasing the range of motion during lumbar spine extension.
Wstęp: Ból odcinka lędźwiowego kręgosłupa jest jedną z najczęstszych dysfunkcji narządu ruchu, z jaką pacjenci zgłaszają się do fizjoterapeuty, i dotyczy coraz młodszych osób. Cel pracy: Celem pracy była ocena wpływu technik mobilizacji dolnego odcinka kręgosłupa na jego zakres ruchu i poziom bólu u pacjentów z silnymi dolegliwościami wymienionej okolicy. Materiał i metody: Do badań zakwalifikowano 20 pacjentów w wieku 28–67 lat (x = 47,55±12,04) z dolegliwościami bólowymi w dolnym odcinku kręgosłupa. W celu określenia ruchów nasilających dolegliwości bólowe wykonano badanie fizykalne, a do oceny nasilenia bólu posługiwano się skalą analogową VAS. W terapii wykorzystano technikę „zamykania” wyrostków kolczystych. Terapia przeprowadzona została jednorazowo, a czas jej trwania wyniósł pół godziny. Po terapii pomiary powtórzono. Wyniki: Zaobserwowano istotne zmniejszenie bólu po terapii w ruchach: wyprostu (p ≤ 0,001), skłonu bocznego w lewo (p < 0,000), skłonu bocznego w prawo (p < 0,000), rotacji w lewo (p ≤ 0,014), rotacji w prawo (p ≤ 0,016). Analizując wyniki w teście T-Studenta, zaobserwowano istotne statystycznie zwiększenie zakresu ruchu wyprostu po zastosowanej terapii (p < 0.001). Wnioski: Techniki manualnej mobilizacji odcinka lędźwiowego kręgosłupa przy ograniczeniu ruchu w kierunku wyprostu wpływają na zmniejszenie dolegliwości bólowych oraz zwiększają zakres jego ruchu.

Introduction: In this study, strength and endurance of the trunk muscles were assessed in patients with spondyloarthrosis of the lower spine. Material and methods: 52 women and 49 men with low back pain syndrome (LBPS) were enrolled into the study. The muscle strength of the flexors and extensors of the trunk was determined by calculation of the maximum (MT) and relative torque (RT) during an isometric contraction. Evaluation of endurance of the extensors used calculation of the regression coefficient of the strength reduction during the testing of maximum strength capability. The obtained results were compared to results obtained in healthy persons. Moreover, the relation of those parameters to age, body-mass index – BMI (describing somatic habitus) and the Oswestry Disability Index – ODI (describing pain intensity during activities of daily living) was assessed. Separate analyses were conducted for the groups of males and females.Results: Based on the conducted study and the performed analyses, it can be stated that in patients with chronic low back pain syndrome there is a marked reduction in trunk muscle strength as compared to healthy persons. For females, the reduction ranged from 45% (flexors) to 48% (extensors) for absolute maximum torque values, while for relative values (RT) – from 51% (flexors) to 56% (extensors). In men, the strength reduction of abdominal and back muscles was smaller and was approximately 40% (for MT and RT). Because of such differences in the strength of the back muscles and methodological reasons, endurance variables were not compared. Reduction in trunk muscle strength is primarily due to the pain syndrome and does not depend on patients’ age or habitus. Lower muscle strength can affect the intensity of spinal pain during every-day activities.Conclusions: Precise determination of strength and endurance of the trunk muscles and the appropriate selection of methods of their reinforcement should constitute one of the principal aims of rehabilitation treatment in patients with low back pain.

Background Spinal overload among blue-collar workers occurs most frequently in the lumbar spine. Long-lasting spinal overload results in pain syndromes leading to limitations in everyday and professional life. Material and Methods The research included 106 adult males working as farmers for at least 10 years. The control group included 50 male white-collar workers. Four research tools were used: an interview questionnaire, a questionnaire assessing pain intensity and limitations in everyday functioning of study participants, a Numeric Rating Scale (NRS) measuring pain intensity and Oswestry Disability Index. Results The research revealed that as many as 86% of the farmers (91 individuals) experienced low back pain. In the control group only 64% of all the white-collar workers complained of pain in this part of the spine. The farmers defined their low back pain as constant pain or stiffness significantly more often. In addition, they reported hip pain radiating to one or 2 legs and experienced chronic and acute back pain more frequently. The total time in which they experienced this pain was usually 11–20 years. The correlation between pain intensity measured on a numeric scale and the level of disability of farmers was revealed. Conclusions The research revealed that individual farmers complained of low back pain significantly more often. The pain was usually defined as constant pain, chronic pain, acute pain or hip pain radiating to legs. Med Pr 2018;69(4):355–364

Objectives: To elucidate the association between metabolic syndrome (MetS) and disability due to low back pain (LBP) among care workers. Material and Methods: This cross-sectional study enrolled 656 care workers having experienced LBP in the year prior to the year of this study. The Roland-Morris Disability questionnaire (RDQ) and self-reported questionnaires regarding LBP, fear of movement, depressive symptom, psychosocial factors, intensity of pain, and duration of pain were administered, and a medical examination was performed. Metabolic syndrome was defined according to the international definition agreed in 2009. Out of the 656 care workers, we included 316 care workers (response rate: 48.2%) who had fully completed the questionnaires as the study sample (males: 13.6%, median age = 51 years old, range: 35–74 years old). To examine the association between MetS and the level of disability due to LBP, we used the Poisson regression analysis and estimated crude and adjusted prevalence ratios (PR). Results: Out of the 316 care workers, 52 (16.5%) were diagnosed as having MetS. Metabolic syndrome was significantly associated with the RDQ score (adjusted PR: 1.57, 95% confidence interval (CI): 1.17–2.11) after adjusting for covariates, such as age, sex, fear of movement, job demands, social support, intensity of pain, and duration of pain. Conclusions: This study showed that MetS was independently associated with disability due to LBP among care workers. A multidisciplinary intervention taking MetS into consideration may be an effective way to reduce disability due to LBP in people with both LBP and MetS. Background Back pain represents one of the most common diseases across various populations of workers worldwide. This study analyzes the prevalence and severity of back pain, based on selected demographic variables, and the relationship with physical activity among school teachers. Material and Methods The study included 998 professionally active teachers (840 females and 158 males) from the southern part of Poland. Validated psychometric tools, namely: 1) for evaluation of disability due to back pain – a Polish version of the Oswestry Disability Index (ODI) and Neck Disability Index (NDI), 2) for physical activity assessment – the Subjective Experience of Work Load (SEWL) as well as the authors’ supplementary questionnaire, addressing demographic and anthropometric variables were used. Results There was no correlation between age and the NDI scores in females (r = 0.027), in contrast to males (r = 0.202; p ≤ 0.05). Statistically significant correlations (p ≤ 0.05) were reported between: age and the ODI, in both females (r = 0.219) and males (r = 0.180). No associations between the body mass index (BMI)-ODI, and BMI-NDI were found. In the case of women, disability related to low back pain (LBP) had a negative impact on the indicators of their activity during work, sports, and leisure time. In the case of men, the NDI and ODI scores did not differ, based on activity indicators. Conclusions Our findings confirm that back pain represents a serious concern among teachers. Age appears to be a prognostic factor, while no association between the BMI and LBP has been revealed. The limitation of physical activity leads to more frequent back pain. Med Pr 2015;66(6):771–778
Wstęp Ból pleców to na całym świecie jedna z najczęściej spotykanych dolegliwości zdrowotnych w różnych populacjach pracowników. Celem badania była ocena występowania i nasilenia dolegliwości bólowych pleców wśród nauczycieli polskich szkół w powiązaniu z ich aktywnością fizyczną i zebranymi zmiennymi demograficznymi. Materiał i metody W badaniu uczestniczyło 998 czynnych zawodowo nauczycieli (840 kobiet, 158 mężczyzn) z południowej Polski. Wykorzystano następujące narzędzia psychometryczne: 1) do oceny niepełnosprawności z powodu bólu pleców – polską wersję wskaźnika niepełnosprawności Oswestry (Oswestry Disability Index – ODI) i szyjny indeks niepełnosprawności (Neck Disability Index – NDI), 2) do oceny aktywności fizycznej – subiektywną ocenę wykonywanej pracy (Subjective Experience of Work Load – SEWL) oraz autorski kwestionariusz z pytaniami dotyczącymi zmiennych demograficznych i antropometrycznych. Wyniki Nie wykazano zależności między wiekiem a NDI w grupie kobiet (r = 0,027), w przeciwieństwie do mężczyzn (r = 0,202; p ≤ 0,05). Różnice istotne statystycznie (p ≤ 0,05) uzyskano między zmiennymi: wiek i ODI (u kobiet: r = 0,219, u mężczyzn: r = 0,180). Nie wykazano istotności statystycznej między wskaźnikiem masy ciała (body mass index – BMI) a ODI ani między BMI a NDI. U kobiet z dolegliwościami bólowymi kręgosłupa stwierdzono niski poziom aktywności fizycznej w czasie pracy, w trakcie uprawiania sportu i spędzania wolnego czasu, natomiast u mężczyzn nie odnotowano takiej zależności. Wnioski Uzyskane wyniki potwierdzają, że ból pleców jest poważnym problemem wśród nauczycieli. Wiek jest czynnikiem prognostycznym, natomiast nie odnotowano związku między BMI a bólami kręgosłupa. Ograniczenie aktywności ruchowej wiąże się z częstszymi dolegliwościami bólowymi kręgosłupa. Istnieje potrzeba wdrażania wśród nauczycieli programu promocji zdrowego stylu życia, ze szczególnym uwzględnieniem aktywności ruchowej. Med. Pr. 2015;66(6):771–778

Objectives Work-related musculoskeletal disorders (MSD) are quite frequent in healthcare workers (HCWs), but data about MSD in home-based healthcare workers (HHWs) are lacking. In this study we describe the prevalence of MSD among Italian HHWs. Material and Methods A case-control study was carried out among 300 random-selected female HCWs, the sample comprising 100 HHWs, 100 HCWs with a low exposure to patient handling (MAPO – Movimentazione e Assistenza Pazienti Ospedalizzati – Movement and Assistance of Hospital Patients index 0–5) and 100 HCWs with high exposure to patient handling (MAPO index ≥ 5.01). As a negative control group, 200 visual display unit workers were also randomly selected. Musculoskeletal disorder cases were collected using a standardized case definition. A multivariate logistic regression analysis was performed comparing the MSD prevalence in the 4 groups. Results The overall prevalence of MSD was 17% in the reference group and 28.3% for HCWs. HHWs and HCWs with MAPO index ≥ 5.01 had similar prevalence of neck pain (9% and 11%, respectively), whereas lumbosacral pain prevalence was higher in the HHWs group (31%), with similar results in residential HCWs groups (21% in MAPO index 0–5 group and 25% in MAPO index ≥ 5.01 group). HCWs of group MAPO index ≥ 5.01 and HHWs showed the higher prevalence of upper limb complaints, with a prevalence of 20% and 10%, respectively. In multivariate regression analysis, prevalence of MSD complaints was quite similar in HHWs (adjusted odds ratio ($\text{OR}_\text{adj}$) = 2.335, 95% confidence interval (CI): 1.318–4.138) and in HCWs of the group MAPO ≥ 5.01 ($\text{OR}_\text{adj}$ = 2.729, 95% CI: 1.552–4.797). Conclusions The prevalence of MSD in the examined HCWs was relatively high, with HHWs appearing as a particularly high-risk group for lumbosacral back pain. In higher exposed HCWs, upper-limb symptoms were particularly frequent, probably reflecting the different tasks required to manage residential and homebased patients. In conclusion, this study reaffirms the high prevalence of MSD in HHWs, seemly similar to high-risk residential groups. Int J Occup Med Environ Health 2017;30(2):291–304

The aim of the study was to analyze the risk factors of back pain in M.A. students. The study involved 150 physical therapy students from the University School of Physical Education (AWF) in Krakow and 100 applied information technology (IT) students from the Cracow University of Technology. The study group was determined on the basis of dean’s lists and quota sampling. The study was conducted using a proprietary questionnaire. The results showed that the level of knowledge on prevention of back pain in physical therapy students is greater than in science students. Physiotherapy students experience back pain more often than IT students. The physical activity level of physical therapy students is higher than the IT students. The type of activity may signifi cantly affect the occurrence of back pain in students. The presented results may be indicative of the significant effect of stress on the occurrence of pain among students. Cite this article as: Pasiut S., Osińska A. Celem badań była analiza czynników ryzyka bólów kręgosłupa u studentów studiów magisterskich. W badaniu wzięło udział 150 studentów fizjoterapii AWF w Krakowie i 100 studentów informatyki stosowanej Politechniki Krakowskiej. Badaną grupę określono na podstawie list dziekańskich i doboru kwotowego. Badanie przeprowadzono za pomocą autorskiego kwestionariusza ankietowego. Wyniki wykazały, iż poziom wiedzy profilaktyki bólów kręgosłupa u studentów fizjoterapii jest większy niż u studentów informatyki. Studenci fizjoterapii częściej doznają bólów kręgosłupa niż studenci informatyki. Aktywność fizyczna studentów fizjoterapii jest większa niż studentów informatyki. Charakter zajęć, może w istotny sposób wpływać na występowanie dolegliwości bólowych kręgosłupa u studentów. Przedstawione wyniki mogą wskazywać, iż stres może w istotny sposób wpływać na występowanie dolegliwości bólowych wśród studentów.
