The paper presents design and manufacturing process of an individualized ankle orthosis using additive manufacturing technologies and reverse engineering. Conventional processes of manufacturing of orthoses are expensive and time consuming Thus, an alternative method was proposed. The patient’s leg was 3D scanned and the orthosis was designed using a CAD system. It was then manufactured using the Fused Deposition Modelling technology, assembled and fully tested. Positive results were obtained.

BACKGROUND During half time or breaks in play cryotherapy is often applied for analgesia for minor musculoskeletal sport injury, however the effect of cryotherapy on dynamic stability is debated. A risk factor for further lower limb injury may be heightened due to a reduction in dynamic postural stability. OBJECTIVES The purpose of the current study was to investigate the effects of wetted-ice applied for 20-minutes at the ankle on dynamic stability using the star excursion balance test, immediately-post exposure and over a rewarming period of 30-minutes. MATERIALS AND METHODS Twenty-two healthy male athletes that regular took part in land-based sport were assessed on reach directions of Anterior (Ant), Posteromedial (PM), and Posterolateral (PL) using the modified star excursion balance test (mSEBT) on the non-dominant limb. Thermal imaging quantified skin surface temperature (Tsk) over lateral and medial regions. Participants were tested pre-intervention, exposed to 15-minutes wetted-ice cryotherapy application, immediately-post and up to 30-minutes post intervention at 10-minute intervals. RESULTS Significant decreases in Tsk over the medial and lateral regions of the ankle (p < 0.05) not returning to pre-cooling temperatures at 30-minutes post. Significant decrease in reach -distance scores (ANT, PL and PM) pre-immediately post and at 10, 20 and 30-minutes post cryotherapy exposure. CONCLUSION Following wetted ice application to the non-dominant ankle, dynamic postural stability was adversely affected for up to 30-minutes post exposure demonstrated through a decrease in reach scores for ANT, PL and PM directions. Functional performance which requires stabilising mechanisms may be negatively affected and contribute to a heightened risk of injury or further injury in consideration of the findings.

Lateral ankle sprains are one of the most common injuries in athletics. Injury to lateral ankle ligaments can result in persistent instability of the ankle joint, known as functional ankle instability (FAI). Two methods of treating FAI are ankle taping and ankle bracing. The purpose of this study was to compare the effects of ankle taping and ankle bracing on ankle joint kinematics and kinetics during a landing task. Methods: Seven individuals with FAI and seven healthy controls performed three landing trials in each of three conditions: control, taped and braced. Ground reaction forces and three-dimensional kinematics were collected simultaneously while participants performed single-leg step-off landing trials from a box with a height of 0.6m. Peak ankle joint angles and moments were calculated using customized software. Results: Individuals with FAI produced significantly smaller inversion moments compared to healthy controls (p = 0.006). Ankle stabilization did not significantly alter ankle joint angles, ranges of motion or moments. Discussion: The present findings suggest that individuals with FAI exhibit unique ankle biomechanics independent of ankle stabilization modality. Future research may seek to investigate the multi-joint biomechanical adaptations associated with ankle stabilization in individuals with FAI compared to healthy controls.

Purpose: The objective of the study is to evaluate the degree of balance disorders in patients with surgical treatment of ankle fractures with the use of stabilometric examinations.Methods: The subjects in the study were 21 patients with ankle fractures treated surgically, within one year of the procedure. The control group were 20 healthy subjects. The balance was evaluated with the use of force platform in standing posture in both single and double limb stance. The analysed parameters were the transition area of the centre of feet pressure (COP), the length of the COP path and the COP velocity. The range of movement in the ankle joint and the intensification of pain were also measured.Results: In the balance evaluation in double limb stance, there were no statistically significant differences. A significant difference was found in the attempts of single limb-stance An average value of COP transition area in the study group was 261.2 mm2 in single stance (on the right operated limb) and in the control group – 93.2 mm2, so the difference was statistically significant (p=0,0096). The presence of pain, the presence or the removal of anastomosis had no significant influence on the balance of the studied subjects. Also a significant correlation between the balance of the subjects and their age was found.Conclusions: The balance in single limb stance after an instable ankle fracture within one year of the surgical procedure is significantly poorer in comparison with healthy subjects. Elderly persons have significantly poorer balance control.

Opis:
Proposition of compliant foot for bipedal robot is introduced and its properties are investigated. The foot consists of four compliant elements (spring-damper) mounted to four vertices of a rectangular frame. The results of robot gait analysis using Zero Moment Point method are shown. ZMP trajectories for rigid and compliant foot are compared and conclusions are formulated. Foot compliance reduces needed for postural stabilization compensatory movements of the upper part of the body by those simplifying control methods and construction. Obtained results will be applied in real prototype of small humanoidal robot. Robot construction in which proposed foot will be applied is shortly introduced.

Introduction and objective. Common to the talocrural and subtalar joints, the calcaneofibular ligament forms with the anterior bundle a mechanical coupling factor for ankle stabilization. Adhering to the deep surface of the fibular sheath, doubled in depth by the lateral talocalcaneal ligament, it is solicited in isolated varus and its lesion is involved in subtalar instability. The therapeutic strategy for chronic ankle instability is currently based on the anatomical reconstruction of the ligaments, particularly by arthroscopy, hence the objective of this study, which aims to highlight the great variability of the calcaneofibular ligament. Materials and method. Our work is based on the dissection of 24 ankles. The number of beams, the dimensions at the neutral point and after power-up, the relationships with the surrounding anatomical structures as well as the points of insertion of the CFL are studied. Results. The calcaneofibular ligament was monofascicular in 91.6% of cases and bifascicular in 8.4%. Its average length in a neutral position was 26.85 ± 3.46 mm, and its width was 5.33 ± 1.45 mm. In dorsiflexion, the length was 27.10 ± 0.05 mm, and in forced inversion, it was 26.98 ± 0.07 mm. The distance between its fibular insertion and the apex of the lateral malleolus was 6.2 ± 0.9 mm. Conclusions. Injuries of the calcaneofibular ligament are common in lateral ankle sprains, which can often get complicated by chronic instability. The macroscopic and morphometric knowledge of this ligament can allow a more accurate anatomical reconstruction and a more accurate choice of graft.

The purpose of this study was to determine differences in force steadiness during submaximal plantar flexion (PF) and dorsiflexion (DF) of the dominant leg between individuals experienced and not experienced in resistance training. Forty males aged 18-32 were divided in two groups based on their experience in resistance training (experienced – not experienced). Evaluation of maximal voluntary isometric force was carried out on a Humac-Norm isokinetic dynamometer. The measurement of the maximal voluntary contraction (MVC) was measured in 3 isometric attempts. Then, the stability measurement of 10%, 30%, 50% and 70% of the MVC was assessed. The duration of the isometric contraction was 10 seconds but only 6 seconds were analyzed. T-test for independent samples and ANOVA was applied for the statistical analysis. The results showed that the individuals with more experience in resistance training presented a significantly higher MVC and force steadiness of the plantar and dorsiflexors muscles, compared with the individuals with less experience (p < 0.05). In conclusion, the years of practice with resistance training appear to have a beneficial effect on force steadiness.

Despite significant advances in the treatment of diabetic foot ulcers and below-the-knee critical ischemia, there are ongoing efforts to achieve a method with low complication, high success rate and persistence of long-term effects. The aim of the study was to examine the outcome of angioplasty in patients with below-the-knee critical ischemia referred to Hospital. Material and methods. This semi-experimental study conducted on diabetics patients treated with PTA (Percutaneous transluminal angioplasty) with critical ischemia of lower limbs referred to Sina Hospital. After discharge, the patients were followed weekly for the first month and then monthly up to 12 months. The procedure short-term effects were examined through evaluation of wound healing as well as patients' recovery and pain relief, after one month. Given the distribution type, parametric and non-parametric test were used to compare the results before and after treatment. Pearson's correlation coefficient was used to determine the correlation between variables. Results. Twenty four patients participated in this study. The mean ankle-brachial index (ABI) at baseline was 0.55±0.17. A month after angioplasty, the index increased statistically significant to 0.93±0.16. The mean health score expressed by the patients at baseline was 5.48±1.39. A month after angioplasty, it was significantly increased (6.32±1.24). The mean pain score before enrollment was 6.68±2.52 (according to VAS scale). There was a significant decrease over time (3.45±1.13). The overall mean score of all patients at Rutherford Classification was 3.88±0.63 at baseline. During the 1st month and 6th month follow-up, it was changed to Class 0 that was statistically significant in the first month. Conclusions. This study represents the mid-term outcomes of PTA. Although PTA treatment was associated with improved pain scores, satisfaction with health, classification of limb ischemia and diabetic foot ulcers, the effects only remain short-term and mid-term. However, long-term efficacy of PTA needs to be investigated further.

Ankle arthrodesis is still the gold standard in the treatment of end-stage degeneration in the ankle joint. The anatomical surgical approach, correct foot alignment and stable fixation are the factors determining the positive effect of surgery. The study presents the technique of ankle arthrodesis by modified Mann’s technique which meets the above-mentioned criteria with the economical use of implants for its stabilization.
Artrodeza stawu skokowo-goleniowego jest złotym standardem w leczeniu zmian zwyrodnieniowych w tym stawie. Dobry wynik leczenia zależy od dobrego pod względem anatomicznym dostępu do stawu, prawidłowej korekcji zniekształcenia oraz stabilnego zespolenia. Praca prezentuje technikę artrodezy stawu skokowo-goleniowego zmodyfikowanym sposobem Manna, która spełnia te kryteria, a dodatkowo nie wymaga stosowania drogich implantów.

Background. The purpose of this study was to collect current research lines in American football regarding the ankles and the foot. The specific goal was to determine how American football affects the ankle joints and the foot, i.e. the frequency, causes and types of injuries as well as various links between the footwear and failure of these structures. Material and methods. The study included 26 publications, drawn mainly from PubMed and related databases, meeting the search criteria. Results. As a result of the review of the publications, the frequency and types of ankle and foot injuries with regard to lower limbs were discussed along with the conditions predisposing for such injuries. Current works focus primarily on injury surveillance databases and on the research on the impact of footwear on the foot and ankle joint. There are works showing the relationship between ankle and foot injuries on the one hand, and the balance on the other. Additionally, some of the articles deal with the past treatments, the time of exclusion from the game and the use of stiffeners to protect the players against overloads and damage to the above-mentioned structures. Conclusions. Our research draws attention to the problem of foot and ankle injuries and shows that preventive, training and physiotherapeutic programs should still be improved due to their high level of trauma. More clinical study is needed to find the specific causes of injuries and prevent them effectively.
Wstęp. Celem badania było zebranie aktualnych kierunków badań w futbolu amerykańskim dotyczących stawów skokowych i stopy. Celem szczegółowym było określenie w jaki sposób futbol amerykańskiego wpływa na stawy skokowe i stopę, tj. częstość, przyczyny i rodzaje urazów oraz powiązań z obuwiem na niewydolność tych struktur. Materiał i metody. Do tego badania włączono 26 publikacji, głównie z bazy PubMed i powiązanych, spełniających kryteria wyszukiwania. Wyniki. W wyniku przeglądu publikacji wykazano częstość występowania urazów stawów skokowych i stopy na tle kończyn dolnych oraz ich rodzaje i warunki predysponujące do urazów. Analizowane prace skupiają się przede wszystkim na bazach nadzoru/ przeciwdziałaniu urazu oraz badaniach wpływu obuwia na stopę i staw skokowy. Pojawiają się prace wykazujące związek urazów stawów skokowych i stopy z równowagą. Dodatkowo niektóre prace dotyczą przebytych operacji, czasu wyłączenia z gry, stosowania usztywnień w celu zabezpieczenia zawodników przed przeciążeniami i uszkodzeniami wymienionych struktur. Wnioski. Badania zwracają uwagę na problem urazowości stóp i stawów skokowych oraz wykazują, iż nadal powinny być ulepszane programy profilaktyczne, treningowe i fizjoterapeutyczne w związku z dużą ich urazowością. Istnieje potrzeba większej ilości badań klinicznych, aby znaleźć szczegółowe przyczyny urazów i skutecznie im zapobiegać.

Numerical models represent a powerful tool for investigating the biomechanical behavior of articular cartilages, in particular in the case of complex conformation of anatomical site. In the literature, there are complex non-linear-multiphase models for investigating the mechanical response of articular cartilages, but seldom implemented for the analysis of high organized structure such as the foot. In the present work, the biomechanical behavior of foot cartilage is investigated by means of a fiber-reinforced hyperelastic constitutive model. The constitutive parameters are obtained through the comparison between in vitro experimental indentation tests on cartilage and numerical analysis data interpreting the specific experimental conditions. A finite element model of the hindfoot region is developed. Particular attention is paid to model cartilage in order to respect its morphometric configuration, including also the synovial capsule. The reliability of the procedure adopted is evaluated by comparing the numerical response of tibio-talar joint model with in vivo experimental tests mimicking the foot response in stance configuration.

Background: Ankle arthrodesis is probably still the safest method of treating painful, lesioned ankle in patients with rheumatoid arthritis or osteoarthritis with significant symptoms that do not respond to conservative treatment. The aim of this study was to present the effects of the ankle arthrodesis on correct functioning of the knee and hip joints during walking at natural velocity.Materials and methods: The study was conducted in the Biokinetics Laboratory of the Academy of Physical Education in Kraków. Fourteen patients with ankle arthrodesis, aged from 34 to 66 years (mean: 54 years), were enrolled into the study and participated in the testing sessions. Angular changes of the knees and hips in three planes of movement during particular gait phases were evaluated by a three–dimensional motion analysis system: Vicon®. The results of locomotion analysis were compared to a control group that consisted of thirty healthy persons aged 40–60 years.Results: Significant differences in the movement of the knees and hips in the three planes of motion were observed between the two groups. In the frontal plane, knees moved symmetrically, but during all the gait phases, knees were in a significant valgus deformity. In the transversal plane, knees were in a significant external rotation during all gait phases. In the saggital plane, hips moved with limited extension. There was also a significant external rotation, outside the biomechanical normal range, of both hips in the transversal plane.Conclusions: The most significant deviations from the biomechanical normal range were found in the frontal and transversal planes for knees and in the saggital and transversal planes for hips.

Celem pracy jest przedstawienie analizy błędów oraz wyników symulacji wartości momentów sił w stawie skokowym dla płaskiego modelu kończyny dolnej, wynikających z różnego przebiegu środka parcia stopy na podłoże (CoP - center of pressure). Składowa przednio-tylna CoP była przesuwana o wektory [a;b], gdzie a i b - dowolne liczby rzeczywiste. Wartości uzyskane dla symulacji wskazują, że zmiana położenia CoP może generować bardzo duże błędy w wartościach momentów. Jak wynika z symulacji dla badanego modelu zmiana położenia CoP o wektor np. [0;7] cm, generuje krzywą tożsamą z trajektorią przebiegu momentów w stawie skokowym w chodzie charakterystycznym dla osoby niepełnosprawnej.
The purpose of this study was to investigate the effect of errors in the center of pressure (CoP) locations on two dimensional lower limb joint moment during gait. Kinematics and kinetic data were synchronized in MatLab. Antero - posterior component of CoP was shifted by vectors [a;b], where a and b -real numbers. The study shows that change of the location of CoP can generate very large mistakes in calculation of joint moments. As results from simulation for studied model the change of CoP about the vector e.g. [0;7]cm, generate the curve the same like for the handicap person.

Opis:
This study developed an ankle rehabilitation device for post-stroke patients. First, the research models and dynamic equations of the device are addressed. Second, the Sliding Mode Controller for the ankle rehabilitation device is designed, and the device's response is simulated on the software MATLAB. Third, the ankle rehabilitation device is successfully manufactured from aluminum and uses linear actuators to emulate dorsiflexion and plantarflexion exercises for humans. The advantages of the device are a simple design, low cost, and mounts onto rehabilitative equipment. The device can operate fast through experiments, has a foot drive mechanism overshoot of 0°, and a maximum angle error of 1°. Moreover, the rehabilitation robot can operate consistently and is comfortable for stroke patients to use. Finally, we will fully develop the device and proceed to clinical implementation.
