A high number of examples in today’s global industry prove that it is difficult to ensure a certain level of quality. One possible reason is the increasing complexity of modern mechatronic product systems. Interface problems due to several involved science disciplines and the lack of understanding subsystems, which are delivered by different suppliers, make it difficult to identify critical components and specifications of elements. Furthermore, it is even harder to estimate the effects of elements because of their unknown dependencies and relations. Therefore it becomes more and more improbable to develop a safe, sustainable and high quality product right from the beginning. In order to understand failures in complex system structures today’s engineering industries need a generic approach that increases the understanding of systems, identifies critical components and considers the different levels of a system. It shall examine failures that cause system errors but also failures that limit system functions. Thus, it is necessary to model and simulate the causally failure chains and analyse over several system levels and elements with their dependencies. This leads to a better understanding of the whole picture of cause and effect. This research paper points out which requirements this new model-based approach has to fulfill and how it is possible to link quality and reliability methods to achieve an overall understanding and analysis of a complex system to assure high quality and sustainability.

By example of steel 17G1S, the regularities in the impact fracture of Charpy specimens at normal and low temperatures are described. The relationship between the energy parameters of fracture (impact toughness) and the deformation response of the material (the height of shear lips) of the specimens from the pipe steel is established. The micromechanisms of impact fracture of the material are described. At 20 °C and –30 °C, focal splitting of the material was observed on the fracture surface of specimens; at –60 °C, the material failed in a brittle manner by the mechanism of cleavage.

This paper presents results of experimental vibration tests of the helicopter turbo-engine compressor blades. The blades used in investigation were retired from maintenance under technical inspection of engine. Investigations were conducted for selected undamaged blades, without existence of preliminary cracks or corrosion pits. The blades during experiment were entered into transverse vibration. The crack propagation process was conducted in resonance condition. During the fatigue test, the growth of crack was monitored. In the second part of work, a nonlinear finite element method was utilized to determine the stress state of the blade during vibration. In this analysis a first mode of transverse vibration were considered. High maximum principal stress zone was found at the region of blade where the crack occurred.

The paper proposes a design procedure for the creation of a robust and effective hybrid algorithm, tailored to and capable of carrying out a given design optimisation task. In the course of algorithm creation, a small set of simple optimisation methods is chosen, out of which those performing best will constitute the hybrid algorithm. The simplicity of the method allows implementing ad-hoc modifications if unexpected adverse features of the optimisation problem are found. It is postulated to model a system that is smaller but conceptually equivalent, whose model is much simpler than the original one and can be used freely during algorithm construction. Successful operation of the proposed approach is presented in two case studies (power plant set-point optimisation and waveguide bend shape optimisation). The proposed methodology is intended to be used by those not having much knowledge of the system or modelling technology, but having the basic practice in optimisation. It is designed as a compromise between brute force optimisation and design optimisation preceded by a refined study of the underlying problem. Special attention is paid to cases where simulation failures (regardless of their nature) form big obstacles in the course of the optimisation process.

This work shows the results of the comparative study of characteristic frequencies in terms of Power Spectral Density (PSD) or RMS generated by a blower unit and the SKFNU322 bearing. Data is collected following ISO 10816, using Emonitor software and with speed values in RMS to avoid high and low frequency signal masking. Bearing failure is the main cause of operational shutdown in industrial sites. The difficulty of prediction is the type of breakage and the high number of variables involved. Monitoring and analysing all the variables of the SKFNU322 bearing and those of machine operation for 15 years allowed to develop a new predictive maintenance protocol. This method makes it possible to reduce from 6 control points to one, and to determine which of the 42 variables is the most incidental in the correct operation, so equipment performance and efficiency is improved, contributing to increased economic profitability. The tests were carried out on a 500 kW unit of power and It was shown that the rotation of the equipment itself caused the most generating variable of vibrational energy.

In this study, firstly, the behavior of a high steel frame equipped with tuned mass damper (TMD) due to several seismic records is investigated considering the structural and seismic uncertainties. Then, machine learning methods including artificial neural networks (ANN), decision tree (DT), Naïve Bayes (NB) and support vector machines (SVM) are used to predict the behavior of the structure. Results showed that among the machine learning models, SVM with Gaussian kernel has better performance since it is capable of predicting the drift of stories and the failure probability with R2 value equal to 0.99. Furthermore, results of feature selection algorithms revealed that when using TMD in high steel structures, seismic uncertainties have greater influences on drift of stories in comparison with structural uncertainties. Findings of this study can be used in design and probabilistic analysis of high steel frames equipped with TMDs.

This paper presents the process of the temporal and spatial functioning of the water distribution system in the mathematical modeling process. The research was carried out on the Bialystok water network model with particular emphasis on the efficiency and energy efficiency of the pumping units located on the analyzed water distribution system (SDW). The calculations and calibration of the model were performed using the ISYDYW software, with particular emphasis on time and spatial distribution. Particular situations were analyzed, especially when the system operated in exceptional conditions, which most often resulted in failure, leading to a shortage of water supply, and thus energy losses of the pumping units. The obtained results allowed demonstrating the auxiliary role of the simulation method in determining and predicting the effects on the existing emergency states. Possible scenarios were prepared for the observed changes, both in the entire network and at the selected points of the pump systems. Assuming the duration of the failure, its consequences were determined and presented in the form of the indicators discussed.

The producer focuses on producing parts that match the customer’s requirements during manufacturing the automotive engine parts. One of the essential automotive engine parts is a crankshaft used to translate movement from the pistons to the car axle. The crankshaft is a complex shape and difficult to produce accurate dimensions during the machining processes. Many machines are used to create the crankshaft. Therefore, many defects happen during the machining process, reducing reliability and increasing the manufacturing process’s production cost. This paper focuses on analyzing failure data and reliability of the crankshaft production line that occurs during the manufacturing process of one year. The common failure associated with the manufacturing process were ring screw, unbalanced crankshaft, broken drill screw, hub machining error, mains part machining error, and setup error. The paper aimed to determine and analyze the best failure fit between the distribution methods, such as Weibull, normal, lognormal, and exponential. Also, the reliability, hazard rate, surviving quantity, and failure density were calculated to evaluate the current situation and predict the reliability of the production line. Results proved the skewness of the data was positive equal to 3.33; the last months had the highest production failure rate, which is 53.8%, the normal method had a proper distribution of data depending on the Anderson-Darling (adj) values which is 1.367 when it compared with other methods, the normal method had the best fitting result depended on failure percentage, from 1% to 95% of the crankshafts production are expected to fail between 47.2676 and 1149.85 months respectively. The reliability of the production line decreased with manufacturing time increased. To reduce the failure and increase reliability, the maintenance system must be supported, analyze the sources that cause failure and downtime of the production line, continue the employee training system on an ongoing basis, and support the production line with modern technology. All analytical results and suggestions could be valuable to the production line to improve reliability and reduce the manufacturing process’s failure.

FMEAs have been prioritized using RPN; however, a new standard has introduced AP for prioritization. This study seeks to determine if the number of required improvement actions increases, decreases, or stays the same when using AP in place of RPN. Statistical software was used to simulate 10,000 combinations of severity, occurrence, and detection. Both AP and RPN were calculated for the 10,000 combinations. Statistical hypothesis testing was performed to determine if there was a difference between RPNs when sorted by AP and to determine if there was a difference in actions required using RPN or AP. There is a statistically significant difference between RPNs when sorted by high, medium, and low AP. Using an RPN threshold equal to or greater than 100 would result in no change in the number of actions required if prioritizing by high and medium, but would result in fewer actions required if only high is used.

In the analysis of the operation of critical infrastructure systems it is important to perform the analysis of the safety of the operation. The daily operation of such systems is inherently associated with the occurrence of various types of random undesirable events. Therefore, in the paper the matrix and logical trees methods used in the analysis of the risk of threats in critical infrastructure systems within the Baltic Sea, were presented. The analysis and assessment of the protection of technical system was performed using the FMEA method (Failure Mode and Effect Analysis). As to analyse the cause and effect of undesirable events the method of Bayes' theorem and Java Bayes program were implemented, which allows to identify the probability of the event occurrence.

Anticipatory Failure Determination (AFD) is a tool used in the TRIZ (Theory of Inventive Problem Solving) methodology. This article introduces its concept and describes the process of AFD in different versions of the method. The article presents the application of the AFD method at a very early state of a system’s development, i.e. its concept formulation stage, which corresponds to a technology readiness level (TRL) equal to 2. The system under analysis is a set of devices used to reduce displacement ship hull resistance. The system was modelled using functional analysis. An analysis of system resources was then carried out. Possible direct, indirect, and accident-related failures were identified. A multi-criteria analysis of the causes of system failures was conducted from which the top 10 potential failures were selected. Observations were made on the applicability of AFD in respect to systems not yet implemented.

Tensile strength of aluminum castings has been improved by employing surge and filter in a conventional non-pressurizing gating system. For this purpose, three non-pressurizing bottom-gating systems were designed where the first design was a simple design with no filter and no surge, in the second design filter and in the third one surge was added to the end of runner. Tensile strength, Weibull module, scanning electron microscopy, chemical analysis, and melt pattern during the mold filling were thoroughly analyzed to compare these three designs. it was observed that employing filter and surge in the gating system reduces flow kinetic energy and consequently avoid surface turbulence and air entrainment, which leads to castings with fewer defects and higher reliabilities. Finally, it found that appropriate use of surge in the running system can be as effective as employing a filter in reducing melt front velocity.

The effects of the miniature channel-shaped scratches not detectable by the present inline electromagnetic defect detection system employed for wires’ surface defect detection on the fracture behaviour of the wires for civil engineering applications were investigated numerically. Finite element analysis revealed that both miniature channel-shaped across-the-thickness and across-the-width scratches change the fracture behaviour of the wires in terms of the fracture initiation locations and fracture process sequence. However, miniature across-the-thickness scratches does not affect the fracture shape of the wire while miniature across-the-width scratches changed the wires’ cup and cone fracture to a fracture shape with a predominantly flat fracture. These results provide an understanding of the fracture behaviour of wires with miniature scratches and serve as an alternative or a complimentary tools to experimental or fractographic failure analysis of wires with miniatures scratches which are difficult to carry out in the laboratory due to the sizes of the scratches.

The non-destructive tests (NDT) of the steel wire ropes put on the crane pulley blocks comprise more than a half of our NDT services. The intensive utilization of cranes often leads to critical situations endangering health and safety at work. The operators are forced to provide safety use of the cranes due to new cranes constructions, new construction of the block pulley and put on the new steel wire rope constructions. The NDT tests of the crane steel wire ropes are coordinate by the Standard ISO 4309. We have experienced only the crane steel wire ropes with an individual wire rope core during our NDT tests so far. The visual NDT test usually used is ineffective to the steel wire ropes with this construction. Based on our previous experience it can be confirmed that surface ruptures are produced after the disintegration of wires inside of a rope; it often leads to the rope abruption during its operation. The reduction of the pulley block dimensions often leads to the problem of the multi-axis rope bending. This kind of the rope line in the pulley block causes the increase of the fretting fatigue of the wires and the rope strands. The same type of the stress is created when the dead ends of the rope are badly constructed.

Opis:
Wear resistance is one of the main indicators of the reliability of machine parts. The selection of wear-resistant material should consider the operational environment and specific types of abrasive material. The steel abrasive wear resistance depends not only on its hardness and microstructure but also on the abrasive material's properties, such as hardness and particle morphology. This work aimed to determine abrasion wear mechanisms of a set of steels, i.e., S235, S355, C45, AISI 304 and Hardox 500, abraded by different types of grit i.e. garnet, corundum and carborundum. The abrasion tests were conducted using T-07 tribometer (rubber wheel method). Wear traces were examined with a scanning electron microscope (SEM), and a contact profilometer. SEM analysis revealed that apart from Hardox 500, ploughing and microfatigue were the dominant wear mechanisms. Microcutting was the main wear mechanism for Hardox 500 tested with carborundum (SiC). The highest mass loss was usually obtained for carborundum. The lowest wear resistance in garnet and carborundum was obtained for the S235JR and S235J2 steels and Hardox 500 tested with corundum. The effect of steel microstructure on the wear mechanism has been confirmed. AISI 304 austenitic steel abraded by carborundum grit, presented outstanding roughness parameters: Ra, Rz, RSm, Rk, Rvk and Rpk than other steels tested with carborundum. Steel hardness affects the morphology of the wear trace reducing the Ra and Rz roughness parameters. The effect of abrasive hardness and grain morphology on abraded surfaces has been stated. Contrary to fine grains of the hardest carborundum, coarse garnet grains caused high roughness parameters (Rk, Rpk and Rvk) determined in wear trace.

Opis:
Competitive economic conditions drive Continuous Improvement (CI) processes within organisations. CI systems and practices are easily accessible. However, questions have to be asked: Is the organization ready for necessary CI changes and is resistance to change an issue in the business? In the change management process the maturity of the organizational culture plays the key role. A significant set of requirements has to be fulfilled to facilitate transformation of the changes into constructive results. In order to assess the level of business advancement, and the maturity of those requirements, the Culture Failure Mode Effect Analysis Tool (CFMEA) has been created. CFMEA provides a practical system for assessing long–term changes, and, in the short term, will make the company fully aware of the critical cultural aspects that have to be developed. CFMEA is an original technique for identifying effects and causes of potential failures during the change implementation process and an organization’s readiness for change.

The aim of this study was to assess the innovation risk for an additive manufacturing process. The analysis was based on the results of static tensile tests obtained for specimens made of photocured resin. The assessment involved analyzing the measurement uncertainty by applying the FMEA method. The structure of the causes and effects of the discrepancies was illustrated using the Ishikawa diagram. The risk priority numbers were calculated. The uncertainty of the tensile test measurement was determined for three printing orientations. The results suggest that the material used to fabricate the tensile specimens shows clear anisotropy of the properties in relation to the printing direction.

This paper is devoted to the analysis of the stability and load-carrying capacity of thin-walled composite profiles in compression. The specimens reflect elements made of carbon fibre reinforced laminate (CFRP). Thin-walled columns with a square cross-section were made from 4 layers of composite in 3 different combinations of layer arrangements. Advanced numerical analyses have been carried out. In the first stage of the study, a buckling analysis of the structure was performed. In further numerical simulations, two advanced models were used simultaneously: the Progressive Failure Analysis (PFA) and the Cohesive Zone Model (CZM). The results showed significant differences between the critical load values for each layer configuration. The forms of buckling and the areas of damage initiation and evolution were also dependent on the applied layup.

This study investigates FMECA analysis and its potential use in improving the reliability and maintenance of the thermal deburring machine in a manufacturing company located in Slovakia. In the beginning, an overview of the FMEA/FMECA methods and their utilization for increased reliability are discussed. As a practical solution, the deburring machine EXTRUDE HONE TEM P-350 was chosen. Functions of its systems and components, thermal deburring process, its characteristics and in practice application are described. Maintenance policy in the company and the current state of maintenance of the machine are also presented. In addition, the study includes failure analysis which evaluates the riskiest systems of the machine. One element, hydraulic unit, is a subject for the FMECA analysis which envisions the assessment of the current failure modes using risk priority number (RPN) and proposals of prevention and detection action reducing the level of RPN. The final section focuses on influencing and improving maintenance and summarizing the potential benefits of FMECA Analysis for company and machine operation.

Responding to the needs of quality and robustness of analysis and management of degradation of equipment, to increase their life cycle and to expand these facilities to become more and more sophisticated and agronomic. This work proposes a contribution to increase the survival of a gas turbine, installed in a gascompression plant, with a comparative study between the two-parameter Weibull distribution. A new modified Weibull distribution was proposed also to reduce the risk of occurrence of failure in this rotating machine. A Statistical analysis and validation on the synthesis of turbine's reliability data and failures were considered, with a particular focus on the use of this data to increase the availability of this type of machine. So, developing a maintenance plan based on their reliability indices for scheduled inspections.

Opis:
Fatigue of a circular saw made of tool steel and used in metal industry to cut, in particular, metal bars and pipes is investigated in this study. Due to having a small tooth root radius, the circular saw is much more likely to get crack damage at the tooth root region. Radial and tangential forces are also effective at this region for fatigue crack initiation. In high cutting speeds and feed rates of the circular saw, higher stress concentration occurs particularly at that region. To examine the fatigue and failure in the circular saw, specimens used in an experiment are prepared from a damaged circular saw are subjected to different mechanical tests. In the theoretical study, stress and fracture behaviour of the saw is determined by the finite element method. Results and causes of the failure are assessed and compared.
Przedmiotem badań jest wytrzymałość zmęczeniowa piły tarczowej wykonanej ze stali narzędziowej i przeznaczonej do pracy w przemyśle metalurgicznym, w szczególności do cięcia prętów i rur. Z powodu niewielkiego promienia zaokrąglenia podstawy zęba tarczy, piły narażone są na pęknięcia właśnie w tym obszarze. Na rozwój procesu pękania mają wpływ obciążenia promieniowe i styczne zęba. Tu występuje największa koncentracja naprężeń przy znacznych prędkościach obrotowych tarczy i dużego tempa posuwu. Do przeanalizowania procesu zmęczenia piły użyto próbek wypreparowanych ze zniszczonego narzędzia i poddano je licznym testom mechanicznym. Równolegle przeprowadzono symulacje teoretyczne za pomocą Metody Elementów Skończonych. Przyczyny i wyniki ilościowe dotyczące zniszczenia zmęczeniowego wyznaczono na podstawie obydwu metod badawczych i następnie je porównano.

Sound visualization methods and techniques are currently applied in various fields to identify and locate individual sources of noise. Conveyor systems are used in various industrial plants. Conveyors require regular inspection and maintenance. In some operations, conveyor systems are difficult to access for the maintenance purposes. During their operation, conveyor systems contribute to increasing the noise levels. The increased noise emitted by a conveyor may signal the failure of a certain part of the system. Such failures can be identified by a sound visualization device and contribute to an effective maintenance of conveyor systems. The paper analyzes specific conveyor systems and identified potential failures of these conveyor systems by using sound visualization methods.

Opis:
The paper describes a numerical and experimental study investigating the load carrying capacity of thin-walled composite structure subjected to axial compression. Composite profile was made of carbon-epoxy laminate with symmetrical arrangement of the layers [90/-45/45/0]s. The experiment was performed on a universal testing machine (Zwick Z100) until total failure of the column. In case of experimental study, post-critical equilibrium paths in full range of loads of the structure were determined. The numerical analysis was performed by the finite element method using the Abaqus® software. Numerical analysis involved solving a nonlinear stability problem. The geometrically non-linear problem was solved by the Newton-Raphson method. The load carrying capacity of the composite structure was determined by the progressive failure analysis, which firstly estimates damage initiation load (Hashin criterion) and secondly estimates failure load (energy criterion). The numerical and experimental results show high agreement, which confirms the adequacy of the prepared numerical model of composite profile.

The scope of the article includes the analysis of the gas network failure based on a material obtained from field tests covering the years 2004-2014, conducted on the gas network of 120 thousand city, allowing to specify the failure rate of the gas network with division into material, pressure and pipelines diameter and indicate the main causes of failure on gas networks. On the base of the results of this analysis the Monte Carlo method to predict failures in gas pipe network has been presented.
Artykuł swoim zakresem obejmuje analizę awaryjności sieci gazowej na podstawie uzyskanego materiału z badań eksploatacyjnych obejmujących lata 2004-2014 prowadzonych na terenie Zakładu Gazowniczego w 120 tys. mieście, co pozwoliło na podanie intensywności uszkodzeń sieci gazowych z podziałem na materiał, ciśnienie i średnice rurociągów oraz podanie głównych przyczyn powstawania awarii na sieciach gazowych. Na podstawie wyników analizy zaprezentowano zastosowanie metody Monte Carlo do prognozowania awarii sieci gazowych.

Purpose – The purpose of this paper is to present practical and research lessons learned from analysis and the identification of failures which can occur. Failure mode and effects piping analysis (FMEPA) has been shown to be an effective way of improving piping design reliability. FMEPA is also employed for making sample control plans. Design/methodology/approach – To reduce project losses by using failure mode and effects piping analysis as a tool for analysis of the piping design department. The samples were selected from five projects. It was found that nine major points yielded a risk priority number (RPN) higher than 125. Findings – Results of RPN calculation concerning four topics revealed that the RPN value was reduced from 211 to 75, demonstrating a 64.4 percent improvement. Research limitations/implications – The study is limited to a planning and piping case study which considers RPN. Testing of the performance network regression model can be employed in companies, in which quality control has been implemented of solutions for failure prevention of piping design. Practical implications – This paper serves practitioners as a guideline and tool to understand and implement the FMEPA methodology. At this level, management sets the limits for determining measures. Management also decides whether a risk is acceptable or not. Management needs to clarify which risk priority number (RPN) represents the critical level above which requires risk reduction. Social implications – Conflicts and social unrest can cause costly delays to new projects and operations. Conflicts can also result in damage to a company’s reputation. This depends on the company’s responses to the conflict and the consequences or perceived consequences of its behavior and actions. Originality/value – This paper furnishes lessons learned for practitioners in various industrial sectors in preference to other methods of risk assessment and control activities.

In accordance with The European Programme for Critical Infrastructure Protection (EPCIP), infrastructure such as collective water supply system is required to be covered by a conservation plan including important resources identification, risk analysis based on the main threat scenarios as well as identification, selection and prioritization of procedures and security measures. The paper proposes a comprehensive method of analysis and assessment of water supply safety from the water consumer perspective. An innovative application of Failure Mode and Effects Analysis (FMEA) is presented, taking into account an uncertainty factor. The method is based on Total Quality Management (TQM). It adheres to the World Health Organization (WHO) recommendations in regard to water safety plan development based on a risk analysis. The method developed involves determination of a water supply safety measure taking account of a priority risk. It allows for a cause-and-effect analysis of all the event scenarios that might lead to the loss of water supply security. Additionally, it’s novelty comes from using the uncertain or incomplete databases. As an example, the paper presents the methodology application for the real water supply system.
System zbiorowego zaopatrzenia w wodę, zgodnie z europejskim programem ochrony infrastruktury krytycznej (EPCIP), stanowi taki rodzaj infrastruktury, która musi mieć plan ochrony, obejmujący identyfikację ważnych zasobów, analizę ryzyka opartą na scenariuszach głównych zagrożeń oraz identyfikację, wybór i ustalenie priorytetów procedur i środków ochrony. W pracy zaproponowano kompleksową metodę analizy i oceny bezpieczeństwa dostawy wody z punktu widzenia jej konsumentów. Wykorzystano metodę analizy przyczyn i skutków uszkodzeń (FMEA) z uwzględnieniem czynnika niepewności, co jest opracowaniem oryginalnym. Metoda ta została oparta na tzw. zasadzie zarządzania przez jakość (TQM). Wychodzi ona naprzeciw zaleceniom Światowej Organizacji Zdrowia (ONZ) w kontekście opracowywania planów bezpieczeństwa wody opartych na analizie ryzyka. Opracowana metoda polega na wyznaczeniu miary bezpieczeństwa dostawy wody z uwzględnieniem ryzyka priorytetowego. Pozwala ona na analizę przyczynowo-skutkową wszystkich scenariuszy zdarzeń mogących spowodować utratę bezpieczeństwa dostawy wody. Jej nowatorski charakter polega ponadto na wykorzystaniu baz danych o charakterze niepewnym lub niepełnym. Jako ilustrację metody podano przykład jej zastosowania w przypadku rzeczywistego systemu wodociągowego.

In a competitive environment, many production industries must reduce costs while maintaining asset value and reliability. In the manufacturing process, the machine is essential because downtime can inhibit and stop production. This study investigated the breakdown trend of a hard disk drive production line in the manufacturing industry to recommend applying Reliability-Centered Maintenance (RCM) for improved productivity, reliability, and availability. This study focused on breakdown analysis, identifying potential failures, and classifying the main components of screw-tightening machines. The RCM method was used based on several tools: failure mode and effects analysis (FMEA), risk priority number (RPN), mean time between failures (MTBF), and mean time to repair (MTTR). The study identified which production line had the lowest availability and productivity due to high downtime and failure rates. In addition, the top-five failures were identified that severely disrupted production These breakdowns were overcome and their occurrence reduced was by calculating and evaluating MTBF and MTTR to help manage failures and indicate the efficiency of corrective action. Thus, this industry and others can achieve better equipment availability and machine reliability using the RCM method.

In our laboratory we quite often experience problems with the operation of steel ropes. There is a range of causes related to lifetime problems of ropes. These problems are related to poor quality of input material used for wire production, mixing of different wire strengths in the rope construction, improper roping of the rope into the hoist system, improper rope operation, improper rope design for a particular type of operation as well as unfair practices of vendors. The subject of the article is the analysis of the rope showing the combination of a large number of the above factors leading to the damage of the rope. During the operation of the cargo cableway, the hauling ropes were damaged in a very short time. The Z wires from the surface layer of the rope started to release from the rope. Subsequently, the wires were unwinding from the layer, they created visible protuberant waves, they were braking and dropping out from the rope. The hauling rope had to be replaced. Than the samples were taken from the rope in order to find out the cause of the rope damage. Mechanical, metallographic and fractographic measurements were performed on the rope. The metallographic and fractographic measurements revealed the material defects of the wires of the rope caused during manufacturing of the Z wires from the surface layer of the hauling rope. The mechanical tests revealed differences in the tensile strength grade the operator required compering to the tensile strength grade the vendor declared. The above mentioned problems unambiguously show that it is appropriate to introduce a mechanical control of the rope properties in advance to its deployment. Checks must be performed by an independent testing laboratory. This procedure overprices inputs needed for the rope replacement. On the other hand, it can reduce the costs when a poor quality rope is deployed. Some of our customers have already adopted this approach. It helped to the increase in the safety of the operation of the mechanisms and it eliminated the problems with exchange and climes on bad quality ropes.

W artykule omówiono uszkodzenia powstające w trakcie eksploatacji podmorskich złóż ropy i gazu. Niniejsza praca opiera się na opisie i analizie rzeczywistych przypadków wypadków na morskich platformach stacjonarnych i pływających, łączy ona podstawową wiedzę z bieżącymi badaniami nad najnowszymi osiągnięciami w tej dziedzinie. Wykazano, że charakterystyki wytrzymałościowe elementów betonowych i stalowych konstrukcji morskich w trakcie eksploatacji zmieniają się i kumulują wady i uszkodzenia. Ustalono, że decydującymi przyczynami uszkodzeń elementów są zużycie korozyjne, procesy korozyjno-mechaniczne oraz defekty spękaniowe. Wykazano, że do 60–75% wszystkich uszkodzeń i wypadków stalowych urządzeń morskich powstaje w wyniku korozyjnego i mechanicznego działania agresywnego środowiska oraz obciążeń siłowych. Oznacza to, że kwestie korozyjnego mechanicznego uszkodzenia takich konstrukcji stały się problemem na skalę przemysłową. Na tej podstawie można wyciągnąć następujące wnioski: doskonalenie systemu utrzymania morskich stalowych platform wiertniczych wiąże się z opracowywaniem nowych modeli i metod zarządzania niezawodnością eksploatacyjną tych konstrukcji, ukierunkowanych na podejmowanie decyzji uwzględniających odporność na pękanie oraz odporność na korozję zmęczeniową stalowych platform wiertniczych stykających się ze środowiskiem korozyjno-aktywnym. Tylko w oparciu o tego typu, naukowo i ekonomicznie ugruntowane modele, można kształtować racjonalne strategie przeprowadzania przeglądów stanu technicznego morskich platform wiertniczych, zapewniające niezbędny poziom ich niezawodności w okresie eksploatacji. Badania te mogą być bardzo pomocne w ulepszaniu projektowania i budowy bardziej niezawodnych i trwałych morskich platform, zarówno stacjonarnych, jak i pływających.
: Failures caused by offshore oil and gas structures operations are investigated. This work is based on the description and analysis of real case studies of accidents on offshore stationary and floating platforms; it combines foundational knowledge and current research on the latest developments in the field. It was shown that strength characteristics of offshore reinforced concrete and steel elements change during operation and cause the accumulation of defects and damages. It was established that corrosive wear, corrosion-mechanical processes, and crack-like defects are the decisive causes of element failure. It was shown that up to 60–75% of all damages to and failures of offshore engineering facilities' steel structures occur due to the corrosion-mechanical influence of an aggressive environment and force loads. This means that issues of corrosion-mechanical failure of such structures have become an industrial-scale problem. It thus allows us to draw the following conclusions: improvement of steel offshore drilling platforms (ODPs) maintenance system involves the development of new models and methods of managing the operational reliability of these structures, aimed at making decisions that take into account the crack resistance and fatigue-corrosion strength of steel ODPs in contact with the corrosive-active environment. Only on the basis of such scientifically and economically grounded models can rational strategies be shaped for carrying out revisions of the ODPs technical condition, ensuring the necessary level of their reliability during the operation period. This investigation can be very helpful to improve the design and construction of more reliable and durable offshore stationary and floating platforms.

The purpose of this article is to alert the reader to the possibility of using Failure Mode and Effects Analysis (FMEA) in the implementation and operation of one of the contemporary concepts that is Reliability Centered Maintenance. The article discusses the scope of activities of Maintenance, briefly presents the evolution of the concept of Maintenance and role of FMEA as a tool to support the concept of RCM.

Opis:
A procedure of the modernisation of the maintenance system of the tramcars used in public transport was presented in this paper. The analysis is based on the maintenance information stored in the database of a city transport company. General and seasonal changes of the number of failures are analysed as well as their reasons and consequences. The effects of the failures are counted proportionally to the financial loss that is characteristic for each of tramcar. Using reliability indexes is essential for the proper description of the failure frequency of the tramcars were presented. The analysis allows one to evaluate the quality of the available information about maintenance process. Therefore, it may lead to decisions about improving the data storage system. The whole procedure provides information that is useful in choosing possible and effective ways of improving the maintenance system which will consequently lead to a reduction of the number of tramcars' failures and a better utilisation of financial resources. The results of the analysis are useful during planning the times of maintenance work, the extents of regular servicing and the supply of spare parts as well as the possibilities of modernising the rolling stock.
W opracowaniu przedstawiono procedurę modernizacji systemu obsługiwania wagonów tramwajowych funkcjonujących w komunikacji miejskiej. Działania podejmowane są w oparciu o informację eksploatacyjną zgromadzoną w bazie danych przedsiębiorstwa transportowego. Przeprowadzona analiza oraz zaproponowana metodyka ocenowa uwzględniają koszty ponoszone przez przedsiębiorstwo. Przedstawione charakterystyki niezawodnościowe stanowią podstawę działań modernizacyjnych w zakresie systemu obsługiwania. Zaproponowana procedura pozwala na ocenę procesu obsługiwana oraz realizację zmian mających na celu poprawę niezawodności. Zmiany powinny również dotyczyć funkcjonującego w przedsiębiorstwie systemu informacyjnego.

The article evaluates the failure rate of devices in the Mechanical Processing Plant using a traditional quality tool, the Pareto-Lorenzo diagram. A comparative analysis of failure rates was performed, recorded in repair, inspection and maintenance card. Then, changes were proposed in order to reduce failure rates and earlier detection of equipment malfunctions. In order to analyze the failure rate of grain washer equipment during thick coal preparation process, one quality management tools was used - the Pareto-Lorenzo diagram. In this case, it is important to assess the mechanical failure of devices, and demonstrating which devices generate most repairs. Based on materials made available by the mine, such as: Repair, inspection and maintenance cards, Device cards, DTR documentation. Quantitative and qualitative assessment the most frequently occurring mechanical failures during operation of devices in process enrichment of coarse sizes was carried out. An attempt was made to assess failure rate of devices on a grain washer. A comparative analysis of failure rates in 2015-2017 recorded in the repair, inspection and maintenance card was carried out. Then, changes were proposed to stand, in order reduce failure rates, and earlier detection equipment malfunctions.

Metal matrix composites (MMCs) are still scarcely described due to various combinations of used materials and a wide array of technologies. Applying the Failure Mode and Effect Analysis (FMEA) method to describe the quality of metal composite castings may contribute to eliminating specific (characteristic only to these materials) defects. This part of the analysis determines the criticality numbers, meaning the frequency of a given failure, detectability level and significance of a given failure to the group of specific composite casting failures. It contributes to establishing the priority number (P), which is a measure used to assess risk, a notion essential in discussing quality in a composite casting.

This article presents a theoretical analysis of a well-known method of FMEA as a tool to support quality management processes. On the basis of literature discusses the origin and nature of the method, as well as synthetic method presents examples of practical applications. An attempt was made to determine the possibility of further developments FMEA method in manufacturing companies. Based on many years of experience operating production company it has observed the need to implement quality management tools in one of the branches of the company, namely the Department of Maintenance of Machines and Equipment in one of the Podlasie power plants. Innovation actions taken manifests itself in the implementation of the industrial practice new solutions for an organization. The issue is so important that observed shortened life cycles of machines and devices operating in power plants. The article presents the problems associated with the use of modern methods of quality management system, combined heat and power. The intensity of the emerging new methods raises the question of how these methods are effective and what benefits the implemented procedures have company. An attempt was made to analyze this problem on the example of the energy industry. The implementation of this method should help to get certain benefits, and consequently improve the productivity of the production of energy in combined heat and power system. Unforeseen emergency downtime of machines operated in the power plants are expensive factor in the production process. FMEA is a method commonly known as analysis of cause - and effect, which allows the accurate formulation of the potential causes and the consequences of failure processes. Having regard to the foregoing attempt to analyze the possibility of implementing FMEA method in the enterprise, and especially a in a company characterized by a continuous type production.

This study uses statistical quality control (SQC) and overall equipment effectiveness (OEE) to examine quality at a porcelain production firm. The study is motivated by the most frequently broken machines in 2019, is the Jigger 01 machine. This paper aims to evaluate the machine’s effectiveness using the OEE method. The OEE determines the scope of the problem to be solved using the SQC method. The average OEE value in 2019 was 70%. Based on the SQC method, the product defect produced is still under control. However, the average defect is still above the company’s tolerance limit of 10%. Consequently, this study offers enhancements utilizing the Failure Mode Effect Analysis (FMEA) technique. The results indicate that human resources and machines caused defective products. This paper contributes to providing several improvements that the company can apply to maximize its quality control analysis. After implementing the improvement, the OEE value increases to 74%.

Opis:
A water distribution system (WDS) ought to be high reliable continuous operating system. Failure factors in WDS should be identified and prioritised, for example, the causing factors in the most frequent failures in water-pipe network. In this paper, the failure risk analysis of the WDS is presented, and accordingly, a New method consisting the failures index (FI) and the evaluation of risk of failure within the relevant area, based on the assumed categories (tolerable, controlled and unacceptable risk). It is expecting that the methodology for the WDS performance risk analysis would provide the city leadership for decision making support.

W artykule przedstawiono analizę awaryjności trzonów sterowych , jednego z najważniejszych elementów urządzeń sterowych na statkach morskich.. Analizę przeprowadzono na podstawie remontów urządzeń sterowych przeprowadzanych w Szczecińskiej Stoczni Remontowej SSR Gryfia w latach 2002-2007. Przedstawiono rozwój historyczny konstrukcji urządzeń sterowych. Przeprowadzono klasyfikację najczęściej spotykanych rodzajów mechanizmów sterowych na statkach morskich. Przeanalizowano awaryjność poszczególnych rozwiązań sterów i trzonów sterowych. Pokazano technologie napraw trzonów sterowych w SSR Gryfia.
The analysis of starring rods damaged problem, one of the most important elements of steering devices has been presented in the paper. The analysis was executed on the base of maintenance operations made in Szczecin Repair Shipyard Gryfia in 2002-2007 years. The history of the ships steering devices constructions was shown. The classification of different kinds of steering devices used on sea vessels was made. The analysis damages of different solutions of steering devices and steering rods was presented. Technology of steering rods maintenance used in Szczecin Repair Shipyard Gryfia was desriebed.

Failure Mode and Effect Analysis (FMEA) is a key risk management tool used in detecting and eradicating potential failure for the purpose of improving the reliability and safety of a system. However, the traditional FMEA in spite of its popularity has inadequacies that have hindered the effectiveness of the tool in analyzing risk of failure modes. Due to the shortcomings of the technique, different improved versions have been suggested in the literature in order for risk to be analysed more effectively but majority of these versions are computationally challenging. In this paper, a simple approach is in troduced for improving the risk analysis capability of the FMEA. The approach utilizes Evaluation based on Distance from Average Solution (EDAS) as an alternative to RPN of FMEA in analyzing risk of failure. A case study of the turbocharger system of a diesel engine is applied to demonstrate the usefulness of the method. The result obtained from the EDAS method were compared with approaches in the literature previously used to address risk analysis of a turbo charger. The result of the analysis indicated that the EDAS method is a feasible alternative technique for risk analysis.

The work describes the phenomenological model of cavitation erosion (CE) elaborated for MCrAlY (where M = Co, Ni or Co/Ni) and NiCrMoNbTa. Coatings were deposited via the HVOF method from CoNiCrAlY, NiCoCrAlY and NiCrMoNbTa feedstock powders. CE tests, conducted according to ASTM G32 standard, indicate that MCrAlYs have a 50% higher maximum erosion rate and twice lower CE resistance than the NiCrMoNbTa coating. CE kinetics of coatings were comparatively studied, combining the mass loss, erosion rate, roughness changes of the eroded surface with microstructure, and mechanical properties of the coatings. Results of SEM analysis of damaged coatings allow identifying the mechanism of CE. In the case of both types of coatings, the erosive damage is initiated at the removal of loose splats, cracking at the border splats and peeling off the coating material, and surface pitting. However, NiCrMoNbTa, due to higher ductility and microstructure homogeneity, presents lesser surface pitting contrary to the MCrAlYs, which have multiphase microstructure higher hardness and consequently was prone to cracking, resulting in the formation of craters and higher surface roughening. The CE mechanism of MCrAlYs was dominated by the brittle mode, while the NiCrMoNbTa coating has mainly a ductile damage behaviour.
W pracy opisano fenomenologiczny model erozji kawitacyjnej (EK) opracowany dla powłok MCrAlY (gdzie M = Co, Ni lub Co/Ni) i NiCrMoNbTa. Powłoki wytworzono metodą HVOF z komercyjnych proszków CoNiCrAlY, NiCoCrAlY i NiCrMoNbTa. Testy erozji kawitacyjnej, przeprowadzone zgodnie z normą ASTM G32, wskazują, że MCrAlY mają o 50% wyższą prędkość erozji i dwukrotnie niższą odporność EK niż powłoka NiCrMoNbTa. Kinetyka erozji kawitacyjnej powłok została poddana analizie porównawczej syntetyzującej utratę masy, szybkość erozji, zmiany chropowatości erodowanej powierzchni z mikrostrukturą oraz właściwościami mechanicznymi badanych powłok. Wyniki analizy SEM uszkodzonych powłok umożliwiły zidentyfikowanie mechanizmu EK. Uszkodzenie erozyjne powłok inicjowane jest w wyniku usuwaniu słabo umocowany cząstek materiału, pękaniu na granicach lameli następnie usuwaniu materiału i tworzeniu wżerów. Jednak NiCrMoNbTa ze względu na wyższą ciągliwość i jednorodność mikrostruktury wykazuje mniejsze wżery powierzchniowe w przeciwieństwie do MCrAlYs, które cechują się wielofazową mikrostruktura o wyższej twardości i w konsekwencji są podatne na pękanie, co powoduje powstawanie dużych wżerów i wyższe chropowacenie powierzchni. Mechanizm erozji kawitacyjnej powłok MCrAlY jest zdominowana przez kruche pękanie, natomiast w przypadku powłoki NiCrMoNbTa dominuje plastyczne odkształcenie.

Opis:
Machining center is the complex machinery, with high level automation and complicated structures, so there are lots of failures. When a random failure occurs, the failed machining center stops and causes a production line or even the whole workshop to stop functioning. The frequent failure leads to the low levels of reliability and production rate. In order to help users and manufacturers optimize maintenance policy to improve the reliability for machining center, this paper presents descriptive statistics of the failure data and develops the failure trend using power-law process, simultaneously establishes the routine inspection and regular inspection as well as the sequential preventive maintenance under maintenance cost constraints. The proposed model could be a useful tool to assess the current conditions, predict reliability and optimize the machining center maintenance policy.
Centrum obróbkowe to skomplikowany mechanizm o wysokim poziomie automatyzacji oraz złożonej konstrukcji, w związku z czym ulega licznym uszkodzeniom. Przy wystąpieniu przypadkowej awarii, uszkodzone centrum obróbkowe przestaje działać i powoduje zatrzymanie linii produkcyjnej a nawet całego oddziału produkcyjnego. Częste awarie obniżają poziom niezawodności oraz tempo produkcji. Aby pomóc użytkownikom i producentom zoptymalizować politykę utrzymania ruchu w celu poprawy niezawodności centrów obróbkowych, w niniejszym artykule przedstawiono statystyki opisowe dotyczące danych o uszkodzeniach i opracowano trend uszkodzeń w oparciu o proces spełniający prawo potęgowe. Jednocześnie ustalono zasady rutynowej inspekcji i okresowych przeglądów, jak również sekwencyjnej obsługi zapobiegawczej przy ograniczonych wydatkach na utrzymanie ruchu. Proponowany model może być użytecznym narzędziem dla potrzeb oceny aktualnych warunków oraz przewidywania niezawodności w celu optymalizacji polityki utrzymania ruchu centrum obróbkowego.

The paper concerns numerical study of the ultra-light seat frame serving transported patients in ambulance. The structure of seat was designed to be built of the carbon fibers, aluminium and steel. The present prototype distinguishes itself with low mass and high strength. During modelling, the stress state and displacement state were verified based on requirements according to regulation ECE14. In simulation, solid, beam and connection elements were employed to consider all the parts of structures. The analysis of the stress state verification based on the assumptions of boundary conditions close to regulation ECE14. The isotropic materials were considered to be in elastic range. In case of composite materials, TSAI-WU (TSW) criterion for assessment of strength was taken into account. Five different variants of seat were taken into consideration to indicate the differences between them. The paper includes the results of analysis of composite structure under static loads which were shown and discussed.

A failure analysis of Babar dam on the El Arab River was performed to highlight the impact of flood wave and velocities on the four villages downstream of the dam; Hella, Khérenne, Chebla and El Oueldja. The simulation of wave propagation along the El Arab River under several scenarios was performed by the hydraulic HEC-RAS model. This model is dedicated to the description of floods at the dam following a breach in the dike. The main factors considered in this simulation include the level of flood water, the flood hydrograph, and the typical scenario for this breach. The flood risk analysis revealed that the maximum of flood wave flow registered at the breach is (Qmax = 9253.02 m3∙s–1), and is beginning to mitigate downstream of the dam along the El Arab River where it reached at the last village with a low flow (Q = 1110.64 m3∙s–1). This simulation allowed drawing the risk map which showed the areas threatened by flood wave resulting from a total failure of the work, and consequently required a plan of security measures to moderate as much as possible the consequences of floods. A sensitivity analysis was conducted to approach the parameters of impact of the breach on the dam failure scenario. It was confirmed that these parameters as formulation time, breach width and side slope have a great influence on the dam failure scenario with the four adjustments (±20 and ±50).

Opis:
Obsługa urządzeń przemysłowych wymaga analizy zachowań układów i ich części składowych. W niniejszej pracy zoptymalizowano czynności konserwacyjne tak, aby wyeliminować występowanie uszkodzeń w kontrolowanym procesie przemysłowym. Celem prezentowanej pracy było poprawienie niezawodności elementów układu sprężania gazu poprzez zaplanowanie czynności konserwacyjnych w oparciu o analizę uszkodzeń z wykorzystaniem optymalizacji interwencji w przemysłowej sprężarce odśrodkowej. O skuteczności proponowanego podejścia świadczy poprawa parametrów niezawodności oraz gotowość omawianej instalacji olejowej.
The industrial maintenance implementation requires to the behaviour system analysis and their components. In this work, we optimize the maintenance actions to eliminate failures in the inspected industrial process. Our purpose in this work is to improve the components reliability in gas compression system, by the planning of the maintenance actions based on failure analysis using the intervention optimization in industrial centrifugal compressor plant. The finality of this proposed approach is proved by the improvement of the reliability performances and by the availability of this oil installation.
