A high number of examples in today’s global industry prove that it is difficult to ensure a certain level of quality. One possible reason is the increasing complexity of modern mechatronic product systems. Interface problems due to several involved science disciplines and the lack of understanding subsystems, which are delivered by different suppliers, make it difficult to identify critical components and specifications of elements. Furthermore, it is even harder to estimate the effects of elements because of their unknown dependencies and relations. Therefore it becomes more and more improbable to develop a safe, sustainable and high quality product right from the beginning. In order to understand failures in complex system structures today’s engineering industries need a generic approach that increases the understanding of systems, identifies critical components and considers the different levels of a system. It shall examine failures that cause system errors but also failures that limit system functions. Thus, it is necessary to model and simulate the causally failure chains and analyse over several system levels and elements with their dependencies. This leads to a better understanding of the whole picture of cause and effect. This research paper points out which requirements this new model-based approach has to fulfill and how it is possible to link quality and reliability methods to achieve an overall understanding and analysis of a complex system to assure high quality and sustainability.

By example of steel 17G1S, the regularities in the impact fracture of Charpy specimens at normal and low temperatures are described. The relationship between the energy parameters of fracture (impact toughness) and the deformation response of the material (the height of shear lips) of the specimens from the pipe steel is established. The micromechanisms of impact fracture of the material are described. At 20 °C and –30 °C, focal splitting of the material was observed on the fracture surface of specimens; at –60 °C, the material failed in a brittle manner by the mechanism of cleavage.

This paper presents results of experimental vibration tests of the helicopter turbo-engine compressor blades. The blades used in investigation were retired from maintenance under technical inspection of engine. Investigations were conducted for selected undamaged blades, without existence of preliminary cracks or corrosion pits. The blades during experiment were entered into transverse vibration. The crack propagation process was conducted in resonance condition. During the fatigue test, the growth of crack was monitored. In the second part of work, a nonlinear finite element method was utilized to determine the stress state of the blade during vibration. In this analysis a first mode of transverse vibration were considered. High maximum principal stress zone was found at the region of blade where the crack occurred.

Opis:
The paper proposes a design procedure for the creation of a robust and effective hybrid algorithm, tailored to and capable of carrying out a given design optimisation task. In the course of algorithm creation, a small set of simple optimisation methods is chosen, out of which those performing best will constitute the hybrid algorithm. The simplicity of the method allows implementing ad-hoc modifications if unexpected adverse features of the optimisation problem are found. It is postulated to model a system that is smaller but conceptually equivalent, whose model is much simpler than the original one and can be used freely during algorithm construction. Successful operation of the proposed approach is presented in two case studies (power plant set-point optimisation and waveguide bend shape optimisation). The proposed methodology is intended to be used by those not having much knowledge of the system or modelling technology, but having the basic practice in optimisation. It is designed as a compromise between brute force optimisation and design optimisation preceded by a refined study of the underlying problem. Special attention is paid to cases where simulation failures (regardless of their nature) form big obstacles in the course of the optimisation process.

This work shows the results of the comparative study of characteristic frequencies in terms of Power Spectral Density (PSD) or RMS generated by a blower unit and the SKFNU322 bearing. Data is collected following ISO 10816, using Emonitor software and with speed values in RMS to avoid high and low frequency signal masking. Bearing failure is the main cause of operational shutdown in industrial sites. The difficulty of prediction is the type of breakage and the high number of variables involved. Monitoring and analysing all the variables of the SKFNU322 bearing and those of machine operation for 15 years allowed to develop a new predictive maintenance protocol. This method makes it possible to reduce from 6 control points to one, and to determine which of the 42 variables is the most incidental in the correct operation, so equipment performance and efficiency is improved, contributing to increased economic profitability. The tests were carried out on a 500 kW unit of power and It was shown that the rotation of the equipment itself caused the most generating variable of vibrational energy.

In this study, firstly, the behavior of a high steel frame equipped with tuned mass damper (TMD) due to several seismic records is investigated considering the structural and seismic uncertainties. Then, machine learning methods including artificial neural networks (ANN), decision tree (DT), Naïve Bayes (NB) and support vector machines (SVM) are used to predict the behavior of the structure. Results showed that among the machine learning models, SVM with Gaussian kernel has better performance since it is capable of predicting the drift of stories and the failure probability with R2 value equal to 0.99. Furthermore, results of feature selection algorithms revealed that when using TMD in high steel structures, seismic uncertainties have greater influences on drift of stories in comparison with structural uncertainties. Findings of this study can be used in design and probabilistic analysis of high steel frames equipped with TMDs.

This paper presents the process of the temporal and spatial functioning of the water distribution system in the mathematical modeling process. The research was carried out on the Bialystok water network model with particular emphasis on the efficiency and energy efficiency of the pumping units located on the analyzed water distribution system (SDW). The calculations and calibration of the model were performed using the ISYDYW software, with particular emphasis on time and spatial distribution. Particular situations were analyzed, especially when the system operated in exceptional conditions, which most often resulted in failure, leading to a shortage of water supply, and thus energy losses of the pumping units. The obtained results allowed demonstrating the auxiliary role of the simulation method in determining and predicting the effects on the existing emergency states. Possible scenarios were prepared for the observed changes, both in the entire network and at the selected points of the pump systems. Assuming the duration of the failure, its consequences were determined and presented in the form of the indicators discussed.

The producer focuses on producing parts that match the customer’s requirements during manufacturing the automotive engine parts. One of the essential automotive engine parts is a crankshaft used to translate movement from the pistons to the car axle. The crankshaft is a complex shape and difficult to produce accurate dimensions during the machining processes. Many machines are used to create the crankshaft. Therefore, many defects happen during the machining process, reducing reliability and increasing the manufacturing process’s production cost. This paper focuses on analyzing failure data and reliability of the crankshaft production line that occurs during the manufacturing process of one year. The common failure associated with the manufacturing process were ring screw, unbalanced crankshaft, broken drill screw, hub machining error, mains part machining error, and setup error. The paper aimed to determine and analyze the best failure fit between the distribution methods, such as Weibull, normal, lognormal, and exponential. Also, the reliability, hazard rate, surviving quantity, and failure density were calculated to evaluate the current situation and predict the reliability of the production line. Results proved the skewness of the data was positive equal to 3.33; the last months had the highest production failure rate, which is 53.8%, the normal method had a proper distribution of data depending on the Anderson-Darling (adj) values which is 1.367 when it compared with other methods, the normal method had the best fitting result depended on failure percentage, from 1% to 95% of the crankshafts production are expected to fail between 47.2676 and 1149.85 months respectively. The reliability of the production line decreased with manufacturing time increased. To reduce the failure and increase reliability, the maintenance system must be supported, analyze the sources that cause failure and downtime of the production line, continue the employee training system on an ongoing basis, and support the production line with modern technology. All analytical results and suggestions could be valuable to the production line to improve reliability and reduce the manufacturing process’s failure.

FMEAs have been prioritized using RPN; however, a new standard has introduced AP for prioritization. This study seeks to determine if the number of required improvement actions increases, decreases, or stays the same when using AP in place of RPN. Statistical software was used to simulate 10,000 combinations of severity, occurrence, and detection. Both AP and RPN were calculated for the 10,000 combinations. Statistical hypothesis testing was performed to determine if there was a difference between RPNs when sorted by AP and to determine if there was a difference in actions required using RPN or AP. There is a statistically significant difference between RPNs when sorted by high, medium, and low AP. Using an RPN threshold equal to or greater than 100 would result in no change in the number of actions required if prioritizing by high and medium, but would result in fewer actions required if only high is used.

In the analysis of the operation of critical infrastructure systems it is important to perform the analysis of the safety of the operation. The daily operation of such systems is inherently associated with the occurrence of various types of random undesirable events. Therefore, in the paper the matrix and logical trees methods used in the analysis of the risk of threats in critical infrastructure systems within the Baltic Sea, were presented. The analysis and assessment of the protection of technical system was performed using the FMEA method (Failure Mode and Effect Analysis). As to analyse the cause and effect of undesirable events the method of Bayes' theorem and Java Bayes program were implemented, which allows to identify the probability of the event occurrence.

Anticipatory Failure Determination (AFD) is a tool used in the TRIZ (Theory of Inventive Problem Solving) methodology. This article introduces its concept and describes the process of AFD in different versions of the method. The article presents the application of the AFD method at a very early state of a system’s development, i.e. its concept formulation stage, which corresponds to a technology readiness level (TRL) equal to 2. The system under analysis is a set of devices used to reduce displacement ship hull resistance. The system was modelled using functional analysis. An analysis of system resources was then carried out. Possible direct, indirect, and accident-related failures were identified. A multi-criteria analysis of the causes of system failures was conducted from which the top 10 potential failures were selected. Observations were made on the applicability of AFD in respect to systems not yet implemented.

Tensile strength of aluminum castings has been improved by employing surge and filter in a conventional non-pressurizing gating system. For this purpose, three non-pressurizing bottom-gating systems were designed where the first design was a simple design with no filter and no surge, in the second design filter and in the third one surge was added to the end of runner. Tensile strength, Weibull module, scanning electron microscopy, chemical analysis, and melt pattern during the mold filling were thoroughly analyzed to compare these three designs. it was observed that employing filter and surge in the gating system reduces flow kinetic energy and consequently avoid surface turbulence and air entrainment, which leads to castings with fewer defects and higher reliabilities. Finally, it found that appropriate use of surge in the running system can be as effective as employing a filter in reducing melt front velocity.

The effects of the miniature channel-shaped scratches not detectable by the present inline electromagnetic defect detection system employed for wires’ surface defect detection on the fracture behaviour of the wires for civil engineering applications were investigated numerically. Finite element analysis revealed that both miniature channel-shaped across-the-thickness and across-the-width scratches change the fracture behaviour of the wires in terms of the fracture initiation locations and fracture process sequence. However, miniature across-the-thickness scratches does not affect the fracture shape of the wire while miniature across-the-width scratches changed the wires’ cup and cone fracture to a fracture shape with a predominantly flat fracture. These results provide an understanding of the fracture behaviour of wires with miniature scratches and serve as an alternative or a complimentary tools to experimental or fractographic failure analysis of wires with miniatures scratches which are difficult to carry out in the laboratory due to the sizes of the scratches.
