The paper is the first part of the probabilistic general model of critical infrastructure accident consequences including the process of initiating events, the process of environment threats and the process of environment degradation models. Basic notions concerned with the events initiating dangerous for the environment after the critical infrastructure accident or its loss of safety critical level are introduced. The methods and procedures of estimating the process of initiating events unknown basic parameters and identifying the distributions of its conditional sojourn times at its states are proposed. Under these all assumptions from the constructed model and after its unknown parameters identification, the main characteristics of the process of the initiating events are predicted. Finally, the proposed model and methods are applied to modelling, identification and prediction of the process of initial events generated by the critical infrastructure defined as a ship operating in the Baltic Sea basin.

Their initiating events and environment threats coming from released chemical substances as well as environment degradations in the neighbourhood region of sea accident were analysed. Then, the process of initiating events, the process of environment threats and the process of environment degradation were analysed and their states are distinguised.

The paper presents a general approach to safety analysis of critical infrastructures that aims to suggest new and to develop existing methods and tools capable of supporting intelligent modeling and decision making in controlling and optimizing the safety of those systems and their accidents consequences risk. Its main focus is on the suggestions of the creation and usage of new techniques, procedures and strategies to improve and to optimize safety of real complex infrastructure systems related to the inside dependencies – among their subsystems and components and the outside dependencies – coming from their operation environment and from other dangerous events and natural hazards. The approach tries to create an original and coherent methodology of safety of critical infrastructures useful in ensuring and improving safety of those systems in various industrial sectors by providing an integrated package of solutions consisting of various packages of theoretical and practical tools ready for direct use by safety theoreticians and practitioners dealing with safety of real critical infrastructures.

Opis:
The paper presents research performed in order to indicate the threats posed to liquid petroleum gas (LPG) carriers maneuvering in the ports and fairway of Szczecin–Świnoujście. The effects of collision with another vessel, going aground, or striking a stationary object are taken into account. As a safety criterion, the possibility of damage to the cargo tanks is taken. As a result of the research, recommendations for ship movement in the ports and fairway were issued. The research method applied in this study consisted of several stages. In the first stage, experts determined possible scenarios of collision and grounding, taking into consideration local and navigational conditions. In the following steps, the external energy was calculated and an empirical model was used to determine the damage to the LPG carrier. In the last step, the necessary measures to be introduced port regulations are presented as conclusions of the research.

The safe operations of LNG terminal in Świnoujście mainly depends on safe operations of LNG tankers. Manoeuvring the LNG tanker at the terminal entrance and basin is always connected with a risk of accident. Areas where the risk of accident is the greatest are those in the vicinity of entrance heads and the turning basin. Accidents within these areas are burdened with the most serious consequences. This article presents possible scenarios of LNG tanker accident consequences in the LNG Terminal in Świnoujście.
Bezpieczna eksploatacja terminalu LNG w Świnoujściu w głównej mierze uzależniona jest od bezpiecznej eksploatacji gazowców LNG. Manewrowanie statkiem na wejściu do terminalu i w jego obszarze zagrożone jest możliwością wystąpienia awarii. Obszarami potencjalnie najbardziej narażonymi na awarie są główki wejściowe do terminalu oraz obrotnica. Wystąpienie awarii w tych obszarach obarczone jest największymi skutkami. W artykule zostały zaprezentowane możliwe warianty wystąpienia skutków awarii tankowca LNG na terminalu w Świnoujściu.

Background Due to the specifics of their work and to being exposed to a wide range of hazards, firefighters working for the State Fire Service (SFS) face the risk of work-related accidents more often than members of other occupational groups. The aim of this paper is to analyze the occurrence and consequences of accidents in the work environment of the SFS officers in Poland between the years 2008–2013. Material and Methods The material analyzed is based on aggregate data collected by the Headquarters of the State Fire Service. Figures regarding accidents in the period between 1 January 2008 and 31 December 2013 show that 8518 workrelated accidents occurred in that period and 8635 people were injured. Results The data shows that neither the number of accidents nor their frequency indicator underwent any significant fluctuations over the 6 years under consideration. The group that is most exposed to accidents on duty in the profession includes active firefighters serving in rescue and fire extinguishment divisions. According to the data, the greatest number of trauma incidents in the SFS between the years 2008–2013 occurred during sporting activities. The predominant cause of these was inappropriate behavior or the lack of proper care. The most frequent injuries sustained during the accidents were broken or fractured bones and sprained joints. Conclusions Accidents on duty occur significantly more often when firefighters are at their stations, during sporting classes, exercises or maneuvers, than in the course of actual rescue operations. Wstęp Funkcjonariusze Państwowej Straży Pożarnej (PSP) ze względu na specyfikę pracy i szeroki wachlarz zagrożeń częściej niż inne grupy zawodowe są narażeni na ryzyko wystąpienia wypadku przy pracy. Celem niniejszego opracowania jest analiza występowania i skutków wypadków w środowisku pracy funkcjonariuszy PSP w Polsce w latach 2008–2013. Materiał i metody Analizie poddano materiał bazujący na danych zagregowanych, zebranych przez Komedę Główną Państwowej Straży Pożarnej. Zgodnie z tymi danymi w okresie od 1 stycznia 2008 r. do 31 grudnia 2013 r. miało miejsce 8518 wypadków przy pracy, w których rannych zostało 8635 osób. Wyniki Zarówno liczba wypadków, jak i wskaźnik częstości wypadków przy pracy nie uległy zasadniczym zmianom w analizowanych 6 latach. Grupą najbardziej narażoną na wypadki w zawodzie strażaka są czynni strażacy, pracujący w jednostkach ratowniczo-gaśniczych. Najwięcej osób poszkodowanych w latach 2008–2013 w PSP miało wypadki w trakcie zajęć sportowych. Dominującą przyczyną zdarzeń było nieprawidłowe zachowanie lub nieostrożność. Najczęstszymi urazami w wyniku wypadków były liczne złamania, pęknięcia kości i zwichnięcia. Wnioski Państwowa Straż Pożarna przygotowuje funkcjonariuszy pod względem przestrzegania zasad bezpieczeństwa i higieny pracy podczas udziału w akcjach ratowniczych. Wypadki strażaków podczas służby zdecydowanie częściej miały miejsce w trakcie pobytu w jednostce – w czasie zajęć sportowych – niż rzeczywistych działań ratowniczych. Med. Pr. 2016;67(1):1–9

The basic aim of the study was to investigate the influence of alcohol consumption on the road traffic safety in Poland. For this purpose, statistical data were used, published. by the Road Traffic Department of the Civic Militia Headquarters in the years 1975-1984, as well as the data published in Statistical Yearbooks of the Central Statistical Office for these years.  It was impossible to include earlier periods in the analysis as in 1975 the rules of statistical registration of traffic accidents were changed, making the data from before 1975 uncomparable. When analysing the problem of the influence the extent of alcohol consumption has on road traffic safety, attention should be drawn to two problems. Firstly, accidents are caused not only by drunk drivers of vehicles but also by drunk pedestrians. Secondly, the drunkenness of the participants of traffic is not an isolated factor but it acts in correlation with other factors, such as e.g. faulty configuration of the road or wrong organization of road traffic. The taking into account of the above problems complicates the analysis of the discussed subject; however, they should not be left out of consideration. A collision of a car with a pedestrian has for many years now been the most frequent type of a traffic accident in Poland, amounting to 43-45 per cent of all traffic accidents. Moreover, traffic accidents caused by drunk pedestrians constitute 43-50 per cent of all accidents caused by drunk persons. Therefore, the contribution of drunk persons to traffic accidents in Poland is considerable; yet it is very difficult to find out how many drunk pedestrains participate in traffic and, in this connection, what is the relation between the number of such persons and that of accidents they cause. For that matter, such difficulties arise also , when one tries to find out the number of drunk drivers of vehicles as in Poland there are no data available which would make such an estimation possible. In spite of these difficulties, the basic hypothesis has been confirmed, according to which the greater the extent of alcohol consumption, the. Higher the frequency of traffic accidents caused by drunk persons. As shown by a statistical analysis by means of the correlation coefficient, there is a strong direct correlation between the two phenomena (r =0.680). This correlation was true for the years 1975-1980 only, while for the years 1981-1984 the smaller was the extent of alcohol consumption, the greater was the frequency of traffic accidents caused by drunk persons. It should be assumed that this absurd result was caused by the lowering of the official figures on the extent of alcohol consumption in Poland in the years 198l-1984 which did not take into account the consumption of illegally distilled alcohol. An estimation was made of the above figures and thus the approximate actual extent of alcohol consumption in Poland in the years 1981-1984 was established. Also the results of traffic accidents caused by drunk persons were studied. Judged by the number of deaths per accident, they .appeared to be much more serious than those caused by sober participants of traffic. The difference between the results of accidents caused by drunk as compared with sober persons was also found to vary according to whether the drunk perpetrator was a pedestrian or a driver, and what vehicle he was driving in the latter case. For instance, while 0.114 persons per accident died in traffic accidents caused in 1979-1984 b sober pedestrians, the ratio was 0.156 in the case of drunk pedestrians; the respective ratios were 0.116 and 0.191 in the case of sober vs. drunk drivers of passenger cars, and 0.132 and 0.320 in the case of sober vs. drunk drivers of tractors.             The distribution of traffic accidents caused by drunk persons according to the time of the day, day of the week and month of the year was also analysed. The yearly distribution of accidents caused by drunk pedestrians appeared to shape differently as compared with accidents caused by drunk drivers. Drunk drivers of vehicles cause the greatest number of accidents in the period from July till October. and drunk pedestrians - from October till January.             Also the analysis of the territorial distribution of traffic accidents caused by drunk persons brought interesting results, Drunk pedestrians cause traffic accidents first and foremost in towns (about two thirds of such accidents), in big cities in particular, whereas drunk drivers cause accidents mainly on roads out of towns (about 60 per cent of such accidents).             The lack of data concerning the extent of alcohol consumption in the separate voivodships greatly complicated the analysis of the territorial distribution of traffic accidents caused by drunk persons. However, it was possible to arrive at the following conclusion: the number of accidents caused by drunk participants of road traffic per 10.000 of the population of a given voivodship (the index of occurrence of such accidents) depends not only on the extent of alcohol consumption in the voivodship but also to some degree on its urbanization and development of road transport. Particularly in voivodships where a high level of urbanization (measured by the size of the urban population) and a highly developed road transport (measured by the number of persons per one passenger car) is not accompanied by an adequate quality of roads and good organization of road traffic; the large number of accidents caused by drunk persons cannot be explained by a high level of alcohol consumption. Bad road conditions exert the greatest influence on those of the participants of traffic who find it difficult to adapt their behavior1r to the complex situation on the road, i.e. on drunk persons, among others, whose intellectual and psychomotoric efficiency is handicapped due to the effects of alcohol. Bad road conditions may ,,produce" errors committed by drunk participants of traffic, and lead to traffic accidents caused by these persons, thus making the number of such accidents larger than that expected when considering the .number of drunk' participants of road traffic. However. there are voivodships where the factors leading to traffic accidents caused by drunk persons do not seem to be major and yet the number of such accidents is considerable. These are probably the voivodships where the extent of alcohol consumption is particularly high.             It should be added that the taking up of the subject included in the title was justified by the seriousness of the problem. In the years 1975-1984, the number of traffic accidents caused by drunk persons was on a constant increase in Poland. While accidents caused by drunk persons constituted 21.5 per cent of all accidents in 1975, their respective share was 26.5 per cent in 1984. Therefore, it is imperative to apply measures that could stop a further growth of this dangerous tendency.

Introduction. The majority of industrial accidents occur because of human errors. Human error has different causes, however, in all cases cognitive abilities and limitations of human play an important role. Occupational cognitive failures are cognitively-based human errors that occur at work. The aim of this study was to examine the relationship between occupational cognitive failures and safety consequences. Method. Personnel of a large industrial company in Iran filled out an occupational cognitive failure questionnaire (OCFQ) and answered questions on accidents. Univariate and multiple logistic regression analysis were used to determine the relationship between cognitive failures and safety consequences. Results. According to developed regression models, personnel with a high rate of cognitive failure, in comparison to low rate, have a high risk of minor injury involvement (OR 5.1, 95% CI [2.62, 10.3]); similar results were for major injury and near miss. Discussion. The results of this study revealed usefulness of the OCFQ as a tool of predicting safetyrelated consequences and planning preventive actions.

Opis:
The Baltic Sea basin is analyzed and Baltic Sea main geographical and climatological parameters are presented. The Baltic Sea environmental impacts of human activity and the ways of their consequences protection are discussed. The set of critical infrastructure networks at Baltic Sea and its seaside is identified and critical infrastructures and their operation environment methodology is introduced. The integrated management system of safety and security of Baltic Sea area critical infrastructure networks is proposed as a new research project continuing HAZAD project subjects and its core aims and description are presented. The project research team conception is created and partner cooperation added values are addressed. Seven main steps of project implementation are suggested. Moreover, the appropriate and wide references are given.
