RESEARCH OBJECTIVE: The aim of this concise review is to both demonstrate the pros and cons in regards to personal health budgets (PHBs), as well as critically evaluate their performance and possibilities. Another purpose of this paper is to familiarize the wider public with the concept of PHBs. THE RESEARCH PROBLEM AND METHODS: The reason behind introducing PHBs and direct payments was to bring social care and long-term healthcare “closer” to the patients in order to personalize treatment and support. By “personalization” we understand the possibility of choosing services that best meet the unique and individual needs of a specific person. However, the tempting concept of PHBs carries with itself both pros and cons. This qualitative, comprehensive narrative review brings to light the current state of knowledge and different parties’ opinions on PHBs. THE PROCESS OF ARGUMENTATION: A PHB is an allocation of public/private financial resources used to identify and meet the health-related needs of a specific person. Such a resolution should potentially lead to new and innovative ways of spending the available funds, outside what traditional services offer, in order to personalize healthcare, increase its effectivity, and decrease the cost/benefit ratio. However, PHB’s are not all-inclusive but cater to needs easily overlooked in the traditional healthcare system. RESEARCH RESULTS: The described PHB organization combines the best available clinical experience with the cumulated health- and care-related experience of the patients. This way both long-term and new short-term needs can be addressed and the intervention that has begun can last as long as the patient needs it. CONCLUSIONS, INNOVATIONS AND RECOMMENDATIONS: The healthcare system faces difficult times, and PHBs may be a potential solution to at least a part of the problems. The only question is will they be implemented correctly becoming a positive driver of change or will they become the anchor that pulls down a sinking ship?

We live in the era of personalisation. Personalised advertisements attack us all around, we are proposed customised trainings and dietary plans. It is no different in medicine. Personalized medicine offers a special approach to the specific needs and symptoms of each patient. How is the future of medical cannabis drawn in the light of this concept? After all, the use of so-called "medical marijuana", although controversial, is the actual part of modern medicine. Approach to the principle: "one size fits all" does not work with therapeutic cannabis therapy. Cannabis is a complex herbal medicine, so far over five hundred compounds present in them have been identified and isolated - not only cannabinoids, but also terpenes, flavonoids and many other compounds. They are able to affect each other, causing so-called “entourage effect” (enhancing, synergetic effect). These interactions are responsible for specific action of different cannabis varieties. Choosing the right product is a huge challenge. There are hundred of different strains of cannabis. The global cannabis industry is full of different types and forms of products. Finding the right one for a patient struggling with specific condition can be a challenge. Gratefully, innovations allow higher levels of personalization while addressing challenges faced by a medical cannabis patient. Platforms using extensive data sets and artificial intelligence are being developed worldwide, which allow the development of personalized therapies based on cannabinoids, including for patients with cancer. However, there is a risk of abuse in this field. Especially since in Poland there is no proper law regarding genetic testing and biobanking... The other thing is that Cannabis flos, like other herbal medicinal products, can cause drug-drug interactions. As the medical cannabis market continues to evolve, we should all strive to ensure that the drug is adapted to the safety and therapeutic profile of the largest number of patients in need.
Żyjemy w erze personalizacji. Zewsząd atakują nas personalizowane reklamy, proponuje się nam dostosowane do naszych wymagań plany treningowe i dietę. Nie inaczej jest w medycynie. Medycyna spersonalizowana oferuje zindywidualizowane podejście do specyficznych potrzeb i objawów każdego pacjenta. Jak w świetle tej koncepcji rysuje się przyszłość konopi medycznych? W końcu stosowanie „medycznej marihuany”, choć budzi wiele kontrowersji, jest częścią współczesnej medycyny. Podejście w myśl zasady: „jeden rozmiar dla wszystkich” nie sprawdza się w przypadku terapii konopiami leczniczymi. Konopie siewne (Cannabis sativa L.) stanowią złożony lek roślinny, do tej pory zidentyfikowano i wyizolowano ponad 500 obecnych w nich związków – nie tylko kannabinoidów, ale również m.in. terpenów i flawonoidów. Mogą one między sobą oddziaływać, wywołując tzw. „efekt entourage”. Interakcje te odpowiadają za specyficzne działanie różnych odmian konopi. Wybór odpowiedniego produktu stanowi ogromne wyzwanie. Na świecie opracowuje się platformy wykorzystujące obszerne zbiory danych i sztuczną inteligencję, które pozwalają na opracowanie spersonalizowanych terapii opartych na kannabinoidach m.in. dla pacjentów z chorobą nowotworową. Istnieje jednak ryzyko nadużyć w tym zakresie. Ponieważ rynek medycznych konopi nadal ewoluuje, wszyscy powinniśmy dążyć do tego, aby lek był dostosowany pod kątem bezpieczeństwa i profilu terapeutycznego dla jak największej rzeszy potrzebujących go pacjentów.

Introduction and aim. Multiple primary tumors are defined as having more than one primary tumor in a different organ location in the same person. Therefore, it is important to know pathogenesis of multiple primary neoplasms to discover new forms of primary prevention and secondary prevention, especially connected with genetic tests which are important for the future of medicine as a part of personalized medicine. The aim of the study is to present selected aspects of the pathogenesis of multiple primary neoplasms. Material and methods. PubMed databases and Google Scholar were searched. Analysis of the literature. The rising risk of developing multiple primary cancers is a consequence of the progressive growth and ageing of the population and development of cancer in patients previously treated for cancer. The formation of secondary neoplasms may be multifactorial – to a large extent it is associated with genetic factors that may facilitate neoplastic transformation, for example as a result of radiation therapy, chemotherapy, inherited syndromes, environmental factors such as tobacco or alcohol, sometimes random somatic mutations. Conclusion. Knowledge of the pathogenesis of multiple primary tumors can contribute to a better understanding of the problem, as well as help in the prevention or early diagnosis of multiple primary tumors (primary and secondary prevention)

This paper presents the experimental results for stress index calculation using developed by the authors information technology for non-contact remote human heart rate variability (HRV) retrieval in various conditions from video stream using common wide spread web cameras with minimal frame resolution of 640x480 pixels at average frame rate of 25 frames per second. The developed system architecture based on remote photoplethysmography (rPPG) technology is overviewed including description of all its main components and processes involved in converting video stream of frames into valuable rPPG signal. Also, algorithm of RR-peaks detection and RR-intervals retrieval is described. It is capable to detect 99.3% of heart contractions from raw rPPG signal. The usecases of measuring stress index in a wide variety of situations starting with car and tractor drivers at work research and finishing with students passing exams are presented and analyzed in detail. The results of the experiments have shown that the rPPG system is capable of retrieving stress level that is in accordance with the feelings of experiments’ participants.

In the paper, we emphasize the idea of radical changes in contemporary medicine that have occurred as a result of the unprecedented development of scientific and technological progress. There is a growing need to analyse the current state and problems in the field of medicine, to consider its new goals and perspectives with an emphasis on the ethical dimension. From a wide range of questions and problems of contemporary medicine, we examine the ethical aspects of the patient's autonomy, commercialization of medicine and personalized medicine. We point out that applied occupational ethics in this field also provides a strategy for cultivating professional responsibility for doctors (healthcare professionals).

Consumer and system-wide gains remain limited by an outmoded policy regime. With scientific innovation running far ahead of public policy, physicians, researchers and patients are not receiving full advantage of the latest developments. European health systems require a seamless and rapid flow of digital information, including genomic, clinical outcome, and claims data. Research derived from clinical care must feed back into assessment, in order to advance care quality for consumers. National health systems are heterogeneous; the solutions and required fundamental approaches differ between the European member states and are not entirely portable and scalable. To date, this applies not only to general systemic aspects but particularly to cross-border reimbursement issues and the exchange of treatment and patient data. To answer those needs, an international consortium was established to implement the project “Interregional coordination for a fast and deep uptake of personalised health”: Regions4PerMed. A cycle of international events, such as conferences, in situ visits and workshops, has been planned. Interdisciplinary groups of experts will exchange thoughts and experiences to design solutions that could be implemented in the various healthcare systems. Regions4PerMed aims to coordinate regional policies and innovation programmes in personalised medicine and personalised health to accelerate the deployment of personalised health for patients. Key Action 2 is dedicated to health technologies and smart and integrated care.
