Opis:
Invasive fungal infection (IFI) in patients with malignant solid tumours is rare, but it highly increases the risk of cancer recurrence because of prolonged discontinuation of cancer treatment. This paper presents a case of IFI induced by Candida glabrata in a 14-year-old girl with advanced Ewing’s sarcoma and metastases to the bone marrow. She was intensively treated with chemotherapy (CWS, CEVAIE, EWING 2008 + VIDE) and radiotherapy. As the treatment was ineffective, the tumour was surgically removed from the sacrum, and VAI chemotherapy was administered. Due to the symptoms of infection, antifungal treatment was initiated with caspofungin, followed by prophylaxis with posaconazole. In terms of anti-cancer treatment, the patient received megatherapy with auto-HSCT. Signs of infection and gastrotoxic complications developed, which is why broad-spectrum antibiotics and amphotericin B lipid complex were administered. Even so, a multisystem IFI appeared, causing the patient’s death. In multidrug chemotherapy with extended periods of agranulocytosis, primary prevention should be considered, similar to the one offered to patients with haematological malignancies.

The Coronavirus disease 2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has posed a public health system challenge across the globe. A retrospective analysis of COVID-19 globally surmises that the fungal co-infections associated with COVID-19 might be missed or misdiagnosed. However, data regarding all the signs and symptoms of COVID-19 are insufficient. The available few publications conclude that patients with COVID-19 have a higher susceptibility to fungal coinfections. Mucormycosis is a rare and often lifethreatening fungal disease characterized by vascular invasion by hyphae, resulting in thrombosis and necrosis. Based on the available data it seems COVID-19 patients, especially severely ill or immunocompromised, have a higher susceptibility to invasive mycoses. Therefore, it is important to assess the risk factors, the types of invasive mycosis, the strengths and limitations of diagnostic methods, clinical settings, and the need for standard or individualized treatment in COVID-19 patients. A 33 years old female operated case of laparoscopic ectopic removal with salpingectomy and tubectomy, at post-operative Day 5 had redness and pus discharge from the operative site and was diagnosed with abdominal wall cellulitis. She underwent local exploration and wound wash. At post operative day 5, the patient came to our emergency room with mild disorientation, cellulitis, and pain at the port insertion site. On examination, we highlight BP 90/50 mmg and blood test analysis with HB-8.3, leucocyte count 29.91×109/L, CRP 333mg/L. CT scan revealed necrotizing fasciitis. She then underwent wide local excision and debridement. Post debridement the next day during dressing, the wound showed a cotton fluffy appearance at the edges and part of the base with black necrotic areas. Wound swab was sent for fungal culture, KOH mount, blood culture, pus culture, and tissue for histopathology. In the meantime, she was started on empirical antifungal amphotericin B, meropenem and minocycline antibiotics. Covid antibodies test was done which were reactive: 1.96. Tissue histopathology revealed mucormycosis. A high degree of suspicion and promptness in starting antifungal prevented fatal outcome.

At present, the only active substance of Gentian Violet (GV) is methylrosaniline - a triphenylmethane dye of which amino group contains 2 methyl groups. GV can be used to treat uncomplicated bacterial and/or yeast infections, support antibiotic therapy of more severe infections, but also to protect medical equipment against colonization by microorganisms. In the light of recent studies, there are many new possibilities for GV application. It has been shown to be effective in the treatment of viral infections, some chronic skin diseases and oncology. GV can induce apoptosis of tumor cells among others by elevating caspase 8, inhibiting NADPH oxidases, decreasing mitochondrial thioredoxin 2 or inhibiting STAT3/SOX2 axis. Preclinical and in vitro studies have also demonstrated GV efficacy in the treatment of breast cancer, melanoma tumors and cutaneous T-cell lymphoma. There is no unambiguous evidence indicating the toxicity of GV, whereas its safety has been proven by its long history of use, its inclusion in numerous guidelines and its legal trade and distribution with no specific approval requested in many countries around the world. The article gathers the available knowledge about GV and its potential use in the future.

Invasive fungal infections (IFI) are devastating and life-threatening infections affecting especially immunocompromised patients. We report on a case of a 14-year-old boy with myelodysplastic syndrome (MDS), after haploidentical hematopoietic stem cell transplantation, in whom invasive pulmonary aspergillosis (IA) was diagnosed and successfully treated with subsequent (despite the secondary antifungal prophylaxis) IA or breakthrough infection development. Thanks to intensive and broad spectrum antifungal treatment (voriconazole upfront therapy, followed by a combination of voriconazole and micafungin, and eventually by the amphotericin B lipid complex), significant improvement was accomplished.

Background: Mucormycosis is a rare fungal infection affecting people with impaired immunity. The aim of this study is to shed light on the epidemiology, incidence, and outcome of patients with mucormycosis hospitalized at a tertiary care center in Pondicherry. Methods: We conducted a retrospective chart review between January 2008 and January 2018. All patients with proven or probable mucormycosis were included. Results: A total of 24 patients were included. Their median age was 49 years and the majority were males. Comorbidities included mainly hematologic malignancy and diabetes mellitus. A liposomal amphotericin B formulation alone or in combination with other antifungals was used as a first line agent in all patients. Conclusion: The incidence of mucormycosis has significantly increased over the past 10 years at our institution, most likely due to increased risk factors.

Opis:
Basing on 43 lectures and reports from the scope of current parasitological and mycological issues presented during the 50th Jubilee Clinical Day of Medical Parasitology (Lodz, 19–20 May 2011), the increasing importance of parasitic diseases and mycoses in medicine was presented. Difficulties in diagnosis and treatment of both imported parasitoses (malaria, intestinal amoebiosis, mansonelliasis) and native parasitoses (toxoplasmosis, toxocariasis, CNS cysticercosis), as well as parasitic invasions coexisting with HIV infection (microsporidiosis) have been emphasized. The possibility of human parasites transmission by vertical route and transfusion has been discussed. The important issue of diagnostic problems in intestinal parasitoses has been addressed, noting the increasing use of immunoenzymatic methods which frequently give false positive results. It was highlighted that coproscopic study is still the reference method for detecting parasitic intestinal infections. The mechanism of the immune reaction induced by intestinal nematodes resulting in, among others, inhibition of the host innate and acquired response was presented. Mycological topics included characteristics of various clinical forms of mycoses (central nervous system, oral cavity and pharynx, paranasal sinuses, nails and skin), still existing problem of antimicrobial susceptibility of fungal strains, diagnostic and therapeutic difficulties of zoonotic mycoses and the importance of environmental factors in pathogenesis of mycosis.

This paper reports on diagnostic and therapeutic management of pulmonary invasive fungal disease (IFD) in a child with relapsed acute myeloid leukaemia, undergoing chemotherapy followed by haematopoietic stem cell transplantation. Surgical management with resection of the involved lung tissue was based on the location of fungal infiltrates close to large circulatory vessels. After examination of resected pulmonary tissue, a diagnosis of proven IFD was done. This case report is an example that aspergillosis is usually the cause for pulmonary IFD. Pharmacotherapy of pulmonary IFD should be based on compounds with good penetration to lung tissue: amphotericin B lipid form or voriconazole.

Zinnia elegans is an annual plant recommended for cultivation for cut flowers as well as flowerbeds. This ornamental plant can be infected by: viruses, bacteria, phytoplasms and by a number of fungi species. Presented studies were conducted in 2006 in three localities of the Lublin region: in the vicinity of Zamość, near Krasnystaw and near Opole Lubelskie. Those studies included three cultivars of Zinnia elegans Jacq.: Golden Dawn, Lawa, Scarlet Flame as well as the mixed material of the cultivars belonging to the dahlia group. Six weeks after the sowing, the seedling’s healthiness was assessed. The proportion of seedlings with disease symptoms ranged from 7% to 44%. The main cause of root infection at the seedling stage proved to be the species of F. equiseti and S. sclerotiorum. Considerable amounts of B. cinerea and F. culmorum and A. alternata were also detected from diseased seedlings. Studies on the susceptibility of analyzed cultivars and the mixed material of elegans zinnia to infection by B. cinerea and F. avenaceum F. culmorum and F. equiseti were conducted in a growth chamber. Results obtained from this experiment confirmed considerable harmfulness of the species B. cinera and F. equiseti towards the seedlings of elegans zinnia, which can be reduced introducing less susceptible varieties to the cultivation. However, none of the studied varieties of elegans zinnia, only its mixed material, showed such properties.
Zinnia elegans polecana jest głównie do uprawy na kwiat cięty oraz na rabatach. Ta roślina ozdobna porażana jest przez: wirusy, bakterie, fitoplazmy i różne gatunki grzybów. Prezentowane badania przeprowadzono w 2006 r. w trzech lokalizacjach woj. lubelskiego: w okolicach Zamościa, w okolicach Krasnegostawu i w okolicach Opola Lubelskiego. Objęto nimi 3 odmiany cynii wytwornej (Golden Dawn, Lawa, Scarlet Flame) należące do grupy daliowatych oraz materiał mieszany odmian. Po 6 tygodniach od wysiewu przeprowadzono ocenę zdrowotności siewek. Udział siewek z objawami chorobowymi wahał się od 7 do 44%. Główną przyczyną porażenia korzeni w stadium siewki okazały się gatunki F. equiseti i S. sclerotiorum. Z chorych siewek w znacznych ilościach uzyskiwano także B. cinerea i F. culmorum oraz A. alternata. Badania podatności siewek analizowanych odmian i materiału mieszanego cynii wytwornej na porażenie przez B. cinerea oraz F. avenaceum, F. culmorum i F. equiseti przeprowadzono w fitotronie. Uzyskane wyniki potwierdziły szczególnie dużą szkodliwość dla siewek cynii wytwornej gatunków B. cinera i F. equiseti, którą można zmniejszyć wprowadzając do uprawy odmiany mniej podatne, jednak takiej właściwości nie wykazywała żadna z badanych odmian cynii wytwornej, jedynie materiał mieszany odmian.

Pneumonia is one of the most frequent cause of death after hematopoietic stem cell transplantation (HSCT). The objective of this review is to present various aspects of pneumonia in this group of patients, with focus on invasive pulmonary aspergillosis and cytomegalovirus disease, being the most frequent etiological causes of pneumonia after HSCT. The review is aimed at practical approach to diagnostic and therapeutic management of pneumonia after HSCT with special attention to: definitions of infections and level of diagnosis of upper and lower respiratory tract infections, including issues specific for invasive fungal disease, pneumocystosis, cytomegalovirus disease, community acquired respiratory viral infections and bacterial pneumonia. Another topics analyzed in the review are: epidemiology and risk factors for development of infection and risk of death due to pneumonia; invasive and non-invasive diagnostics, including imaging and laboratory biomarkers; methods of pharmacological and environmental prophylaxis and specific targeted therapy of pneumonia after HSCT.
