Opis:
Background: Acne vulgaris is a chronic inflammatory disorder of the skin. Low level of adherence of acne patients to treatment can lead to poor clinical efficacy, increased healthcare costs, and unnecessary treatments that can lead to adverse drug reactions. Objective: The aim of this study is to compare the level of adherence of acne patients between Greece and Bulgaria. Methods: A multicenter observational study utilized self-completed questionnaires to assess medication adherence among patients visiting 45 pharmacies throughout Bulgaria and Greece between May and December 2018. A total of 225 patients were included in the study. Subjects included acne patients undergoing treatment who had a medical consultation prior to the study. All patients had acne therapy prescriptions. The likelihood of adherence was assessed using the Elaboration d'un outil d'evaluation de l'observance (ECOB) Adherence Questionnaire. Adherence Questionnaire. The ECOB scale was translated into Bulgarian and standardized by forward translation, backward translation, and a pretest. The data collected were proceeded through SPSS ver. 22.0. Results: According to the ECOB scores, good adherence to treatment was documented in 42.0 % of the Bulgarian patients on topical treatment and 39 on oral medication. For the Greek patients good adherence to topical medication was judged to be good for 53% and for 40% on oral treatment. Conclusion. Greek and Bulgarian patients show similar level - of high rate of non-adherence to acne treatment. Perhaps patients could be monitored about their adherence and this can be performed in the dermatology clinic or in the pharmacy.

Information-Motivation-Behavioural Skills model
treatment
Opis:
According to Information-Motivation-Behavioural Skills model (IMB), accurate Information, strong Motivation and adequate Behavioural skills are prerequisites of treatment adherence. This model has been verified among chronically ills patients, usually suffering from one particular disorder. No studies report how the model fits data from a more diverse group. The aim of the study was to analyze adherence and its barriers in a group of three hundred patients suffering from various, common chronic disorders. They filled out IMB and Adherence Questionnaire. Information and Motivation correlated moderately (.35). Behavioural Skills was the strongest predictor of adherence (.24). An additional relationship was found: Information and Motivation interacted and the role of either of these dimensions vanished if the indices of the other were high enough. The relationships between IMB dimensions and adherence were confirmed in a varied clinical sample. Behavioural skills must be considered when working with non-adherent patients, as they are the strongest predictor of adherence. Other interventions may be limited to either Information or Motivation only if there is a chance of raising one of them to extreme levels. This may be useful with special cases, when increasing both is problematic.

Opis:
Introduction: Atopic diseases are mainly chronic disorders developing at the base of hypersensitivity reactions. Loratadine is an anti‑histamine drug with potential of inflammation and allergy symptoms minimizing. Reaching the best treatment results needs patient’s cooperation. The study was focused on loratadine treated patients’ adherence. Material and methods: In adult patients taking loratadine according to doctor’s recommendations minimum one month before enrolment the questionnaire survey was realized during two consecutive routine control visits in an outpatient clinic. The data of clinical indication for loratadine, other chronic diseases and prescribed medications, regularity of loratadine taking and the possible causes of doses omission as well as patient’s and doctor’s estimation of clinical course of atopic disease were collected. Results: In 36 960 adult patients (41.38±15.05 years old; 53.3% of women) treated by general practitioners (58.76% of doctors) and specialists, loratadine was recommended because of seasonal allergic rhinitis (44.82%), allergic conjunctivitis (25.25%), perennial allergic rhinitis (16.37%) and urticaria (13.56%). The majority of participants stated that loratadine treatment should not be ceased, but regular intake was realized only in about 40%. The irregular treatment was most frequent in patients beneath 30 years old (25.85% of study group) and in patients with concomitant diseases (37.71% of all participants). Predominant cause of irregular loratadine taking were organizational fails, symptoms relief and belief, that the disease is cured, and apprehension of drug side effects. In the end of the first visit 97% of patients were given additional information about atopic diseases and during the next one the percentage of organizational fails in treatment was significantly lower. Conclusions: Adherence of loratadine treated patients appeared far unsatisfied. Patients with atopic diseases should be offered more information about them and the rules of treatment.
Uzyskanie optymalnego efektu terapeutycznego wymaga współpracy pacjenta. Badanie zogniskowano na adherencji terapeutycznej pacjentów leczonych loratadyną. Materiał i metody: W grupie dorosłych pacjentów zażywających loratadynę zaleconą przez lekarza co najmniej jeden miesiąc przed włączeniem do badania przeprowadzono badanie ankietowe w ramach dwóch kolejnych rutynowych wizyt ambulatoryjnych. Zebrano dane dotyczące wskazania do włączenia loratadyny, schorzeń towarzyszących i przepisywanych z tego powodu leków, regularności zażywania loratadyny i prawdopodobnych przyczyn pomijania dawek, jak również oceny przebiegu klinicznego schorzenia atopowego dokonanej przez pacjenta i lekarza. Wyniki: U 36 960 dorosłych pacjentów (41,38±15,05 roku; 53,3% kobiet) leczonych przez lekarzy rodzinnych (58,76% lekarzy) i specjalistów loratadynę zlecono z powodu sezonowego alergicznego nieżytu nosa (44,82%), alergicznego nieżytu spojówek (25,25%), całorocznego alergicznego nieżytu nosa (16,37%) i pokrzywki (13,56%).

Objectives Non-adherence and non-compliance to pharmaceutical treatment is one of the most common causes of not effective management of patients suffering from ischemic heart disease (IHD). It is crucial to understand the reasons behind it but studies on this subject performed in the Polish population are still lacking. Material and Methods The 329 patients (160 male and 169 female) diagnosed with IHD who reported for follow-up appointments are examined. The following standardized questionnaires were used: Treatment Satisfaction with Medicines Questionnaire (SATMED-Q) and Adherence to Refills and Medication Scale (ARMS), which evaluates the patient’s compliance and adherence level, respectively. Results Patients with IHD showed moderate compliance with pharmacological recommendations and average satisfaction with treatment. Anemia, drugs side effects, and SATMED-Q total score were significant predictors of the overall ARMS score in the univariate analysis, whereas the male gender and satisfaction with treatment improves this results. In multivariate analysis, significant predictors of lower adherence included family history of IHD, anemia and drugs side effects, while higher education and SATMED-Q overall score increased adherence. Conclusions Treatment satisfaction is a significant predictor of increased overall treatment adherence as well as adherence in terms of drug intake and drug and prescription refills. Raising patient awareness should be an important goal of future educational activities.

The purpose of this study is to analyse the accuracy of the static schedule of bus transit network in Delhi using real-time data available from Delhi’s Open Transit Data (OTD) platform. To access and organise the data, an algorithm that can convert real-time data into a General Transit Feed Specification (GTFS) format, needs to be designed. Further, this study intends to develop a methodology, which can convert raw data of bus locations into link travel times, which consequently, helps in identifying problematic links. As researchers continue to make use of the data available via GTFS, they may well be aware of the fact that such data may differ systematically from actual transit operations. Continuous improvement of the accuracy of the GTFS static file would benefit its users.

Opis:
Insufficient treatment adherence after early childhood is frequently observed in patients with phenylketonuria. Assessment of these individuals' long-term metabolic control could enable early detection of the risk of intellectual deterioration resulting from high blood phenylalanine concentration. However, the predictive value of specific parameters related to individual dynamics of hyperphenylalaninemia is not clear. Here, we assessed the impact of blood phenylalanine fluctuations during the first 12 years of life on cognitive outcome in early and continuously treated teenagers with phenylketonuria. We have analyzed a total of 5141 results of blood phenylalanine measurements in 32 patients. The phenylalanine levels of these patients were usually acceptable during their early childhood, but the control of hyperphenylalaninemia worsened and the average treatment adherence dropped to 40% during the late primary school. Our analysis revealed a strong association between the Wechsler intelligence verbal scores and the mean of the yearly means of phenylalanine concentrations (r=-0.62). The correlations of IQ scores with median phenylalanine concentrations and the variability of blood phenylalanine levels gave weaker associations. The Wechsler verbal scores were also strongly correlated with the treatment adherence level during preschool and late primary school (r=0.61 and 0.72). The mean of the yearly means of blood phenylalanine concentrations appears to be a better predictor of cognitive outcome in children with phenylketonuria than other parameters related to phenylalanine fluctuations. The percentage of acceptable phenylalanine levels below 50-60% should be regarded as a "red flag" due to the risk of intellectual deterioration in patients.

Mortars are subjected to severe external stresses such as freezing, thawing, and drying during their lifetime. These stresses can lead to a loss of adhesion between the support and the mortar. The strength of the substrates with respect to their ability to receive a coating (mortar) is characterized in particular by the value of minimum tear resistance of the surface to be coated. In this work, the use of a non-destructive method which is both fast and easy to implement is employed to evaluate this support-mortar adhesion. The first method is based on the measurement of the velocities of the surface ultrasonic waves and the second by tearing tests using a specific dynamometer. The determination of the adhesion strength concerned two different supports (concrete beam and masonry block) coated with two types of mortar (a prepared cement mortar and a ready-to-use mortar) with two different thicknesses for each mortar (1 and 2 cm, respectively). The results of the two methods are then correlated for an estimation of the adhesion of the mortars.

Aim. The reality genre has gained much popularity in last few years across the globe. In India too, this genre has reportedly surpassed other genres in recognition and acceptance. However, its format and content intrigue controversies at both social and academic level, but its mass appeal gets bigger each day. The study examines gender, language and issues of gendered language in Indian reality show Bigg Boss. While keeping in view the format and claim of the genre, the study observes how under thorough surveillance, housemates negotiate with their real self and grapple with their language practice. Influence of neoliberalism on society, culture and identity has been very much discussed, the paper attempts to highlight how this transitions in gender identity is depicted in a show taking non-actors and common people. Concept. For the purpose of the study, scripted transcription of Bigg Boss (Season 11) has been used, wherein all the episodes of have been watched back and forth carefully, and detailed notes prepared for the analysis. Housemates’ language practice, voice pattern and preference of words and statements have been observed. Results and conclusion. The study finds that game reality show Bigg Boss substantially adheres to social stereotypes and standards and while doing so, it also imitates the language practice prevalent everywhere. Although active participation of women in the show is visible butt while exhibiting their true self, sometimes gender prejudice embedded deep inside also comes out, which in case of men is very frequent. The study concludes that as a globally acclaimed genre, reality show, like other genres, is very much commercialized and consumer oriented.   Originality. A number of studies related to reality shows have been conducted, but in Indian context, this genre has been inadequately explored. Also, it is very difficult to find studies focusing any specific season of an Indian reality show, so it would not be incorrect to mention that the present study is one of the primary works on this subject, which aims at making some significant contribution related to this genre, in academia.

W badaniu wykorzystano autorski kwestionariusz do pomiaru funkcjonowania młodzieży w czasie pandemii Covid-19 oraz skalę Short Grit Scale (SGS) A. Lee Duckworth w polskiej adaptacji P. Wyszyńskiej (Wyszyńska i in. 2017).
The article presents connections of grit and its two factors – perseverance of effort and consistency of interests, with a propensity to follow the rules and recommendations established by the government during the Covid-19 pandemic. The presented results are based on data collected during the study carried out in a group of 275 secondary school students from the Podlaskie voivodeship. Research tools included an original questionnaire for measuring the functioning of youth during the Covid-19 pandemic and a Polish adaptation of A. Lee Duckworth’s Short Grit Scale (SGS) by P. Wyszyńska (Wyszyńska et al. 2017). The obtained results indicate correlations of grit and, in particular, perseverance of effort with the propensity to comply with epidemic recommendations in the group of female respondents. No statistically significant correlations were found in the group of male respondents. This suggests that the grit factor may be among the important personal resources contributing to prosocial behavior, which should be capitalized in the course of taking preventive measures and resocialization practice addressed to the group of young women.

Opis:
Introduction. In Poland, it is estimated that HF is observed in approximately 0,7 million of patients. The aim of the study was to assess health behaviour in geriatric patients diagnosed with HF regarding pharmacotherapy and non-pharmacological treatment. Material and methods. The study group comprised 72 patients with diagnosed HF, over 65y. The study was performed using a questionnaire survey including 20 closed-ended questions. Results. The mean number of medicinal products prescribed by physicians for the treatment of HF and other diseases was 6.59±1.8. 50% of subjects were additionally taking medications and supplements for self-treatment, 63% declared they managed to take medications prescribed on their own. A diet is followed only by 50%, but only 18% limit their sodium intake. 46% still consume alcohol, and 10% of still smoke cigarettes. 46% declared the use of body weight monitoring, but none of them measured their body weight every day. Conclusion. Health behaviour of geriatric patients with HF in Poland indicates their low compliance with medical guidelines and it may explain a high hospitalisation rate and increasing costs of treatment.

Opis:
Objectives To assess the impact of patient education on medication adherence and quality-of-life (QoL) in Hungarian subjects with chronic obstructive pulmonary disease (COPD). Study Design Longitudinal, non-interventional study conducted at three pulmonology outpatient centers in and around Budapest, Hungary. Experimental Subjects visiting the center with COPD were invited to participate in the study. Data collected at baseline included subject demographics, medical history, and responses to the adherence (Morisky Medication Adherence Scale-8 (MMAS-8)) and QoL (EuroQoL-5D-5L (EQ5D), St. George’s Respiratory Questionnaire (SGRQ), and COPD Assessment Tool (CAT)) scales. Subjects were also provided with patient education designed with standardized content. Subjects were asked to provide responses to adherence and QoL scales again at the 3-month follow-up visit. Medication was left unchanged during the course of study. Statistical analysis included independent and paired-samples t-tests, one-way ANOVA, mixed-measures ANOVA and ANCOVA. Results Mean (± standard deviation (SD)) overall adherence score on MMAS-8 scale increased from 6.72 (± 1.46) at baseline to 7.01 (± 1.15) at follow-up (P = .040). A similar increase in mean (± SD) score was observed for question 4 on the MMAS-8 which deals with remembering to carry COPD medication when leaving house (baseline = 0.81 (± 0.40) versus follow-up = 0.89 (± 0.31); P = 0.018). Conclusions Patient education has a positive outcome on medication adherence in subjects with COPD. Further studies will be required to assess if these benefits are translated to patients’ QoL.
