Atopic dermatitis is the most common skin disorder diagnosed in early childhood. Some children do not experience a relief of symptoms as they get older, and severe atopic dermatitis may develop, with manifestations including widespread skin lesions and unremitting itching. The disease is both physically and emotionally disabling, and significantly compromises the patient’s quality of life. Indications to intensify therapy include resistance to topical treatment and multidrug resistance. However, in many cases non-adherence to the treatment regimen, including inadequate skin care techniques, contribute to the development of severe or refractory atopic dermatitis. Persistent eczematous lesions may be a result of exacerbating environmental factors, secondary infection, and hypersensitivity reactions to topical treatments or other allergens. Wet-wrap treatment with topical corticosteroids, narrow-band UVB phototherapy and systemic immunosuppressive drugs, such as cyclosporine A, methotrexate, mycophenolate mofetil and azathioprine, are recommended for the treatment of severe atopic dermatitis in children. However, there are no evidence-based guidelines for using these agents. Systemic corticosteroids should be avoided, but they can be used for a short period of time for the immediate relief of acute flares before introducing other therapies. Patients need a holistic approach including education and modern biopsychosocial techniques. Paediatric studies are currently under way to test the safety and tolerability of dupilumab which was approved by the US Food and Drug Administration in 2017 for the treatment of adults with moderate-to-severe atopic dermatitis.
Atopowe zapalenie skóry jest najczęstszym schorzeniem skóry rozpoznawanym we wczesnym dzieciństwie. U części dzieci objawy nie łagodnieją w miarę dorastania, lecz rozwija się atopowe zapalenie skóry o ciężkim przebiegu z uogólnionymi zmianami skórnymi, uporczywym świądem; choroba przyczynia się do cierpienia fizycznego i psychicznego, znacząco pogarszając jakość życia. Sygnałem do intensyfikacji terapii powinna być oporność na leczenie zewnętrzne i wielolekowość. Jednak w wielu przypadkach przyczyną ciężkiego i opornego na terapię atopowego zapalenia skóry jest niedostosowanie się do zaleceń terapeutycznych (non-adherence), w tym nieprawidłowa pielęgnacja skóry. Uporczywość zmian wypryskowych może być także skutkiem narażenia na środowiskowe czynniki drażniące, wtórnej infekcji, reakcji nadwrażliwości na leki zewnętrzne czy inne alergeny. W ciężkim atopowym zapaleniu skóry u dzieci zaleca się stosowanie mokrych opatrunków z kortykosteroidami miejscowymi, fototerapii światłem wąskopasmowym UVB, ogólnoustrojowych leków immunosupresyjnych, takich jak cyklosporyna A, metotreksat, mykofenolan mofetylu i azatiopryna. Brak jest wytycznych opartych na dowodach naukowych dotyczących leczenia tymi lekami. Należy unikać kortykosteroidów ogólnych, ale można zastosować krótkotrwałą terapię w celu szybkiego opanowania dużych zaostrzeń przed wdrożeniem innych form leczenia. Pacjenci wymagają podejścia holistycznego, obejmującego edukację i stosowanie nowoczesnych technik biopsychospołecznych. W populacji pediatrycznej obecnie prowadzone są badania kliniczne oceniające bezpieczeństwo i tolerancję dupilumabu, leku biologicznego zaaprobowanego w 2017 roku przez amerykańską Agencję ds. Żywności i Leków w leczeniu umiarkowanego i ciężkiego atopowego zapalenia skóry u dorosłych.

Background: Acne vulgaris is a chronic inflammatory disorder of the skin. Low level of adherence of acne patients to treatment can lead to poor clinical efficacy, increased healthcare costs, and unnecessary treatments that can lead to adverse drug reactions. Objective: The aim of this study is to compare the level of adherence of acne patients between Greece and Bulgaria. Methods: A multicenter observational study utilized self-completed questionnaires to assess medication adherence among patients visiting 45 pharmacies throughout Bulgaria and Greece between May and December 2018. A total of 225 patients were included in the study. Subjects included acne patients undergoing treatment who had a medical consultation prior to the study. All patients had acne therapy prescriptions. The likelihood of adherence was assessed using the Elaboration d'un outil d'evaluation de l'observance (ECOB) Adherence Questionnaire. Adherence Questionnaire. The ECOB scale was translated into Bulgarian and standardized by forward translation, backward translation, and a pretest. The data collected were proceeded through SPSS ver. 22.0. Results: According to the ECOB scores, good adherence to treatment was documented in 42.0 % of the Bulgarian patients on topical treatment and 39 on oral medication. For the Greek patients good adherence to topical medication was judged to be good for 53% and for 40% on oral treatment. Conclusion. Greek and Bulgarian patients show similar level - of high rate of non-adherence to acne treatment. Perhaps patients could be monitored about their adherence and this can be performed in the dermatology clinic or in the pharmacy.

Purpose: To assess the knowledge of young adults (students of dietetics) of the health benefits of the Mediterranean diet (MedDiet) and the practical application of the principles of this diet. Materials and methods: The study was performed on 97 women, students of dietetics. Research on general information about the respondents and knowledge about the MedDiet was carried out using a questionnaire with 29 questions. To assess the nutritional value of the diet, a 3-day nutritional diary and computer programme Diet 5.0 were used. Adherence to the MedDiet was appraised according to the 9-point scale of aMED (alternate Mediterranean Diet Score). Results: The most students responded that they have high knowledge of the MedDiet, but do not use the MedDiet recommendations in their daily nutrition. Better adherence to MedDiet was significantly associated with the lower waist circumference of the women, higher intake of mono- and polyunsaturated fatty acids, omega3 fatty acids, fiber, vitamin C, folate, vitamin B1, vitamin E and magnesium, and the knowledge of participants of the diet and nutritional value of foods. Conclusions: The adherence to the MedDiet is significantly associated with the participants' knowledge about the diet, the higher nutritional value of the daily diet and lower waist circumference

Introduction. In Poland, it is estimated that HF is observed in approximately 0,7 million of patients. The aim of the study was to assess health behaviour in geriatric patients diagnosed with HF regarding pharmacotherapy and non-pharmacological treatment. Material and methods. The study group comprised 72 patients with diagnosed HF, over 65y. The study was performed using a questionnaire survey including 20 closed-ended questions. Results. The mean number of medicinal products prescribed by physicians for the treatment of HF and other diseases was 6.59±1.8. 50% of subjects were additionally taking medications and supplements for self-treatment, 63% declared they managed to take medications prescribed on their own. A diet is followed only by 50%, but only 18% limit their sodium intake. 46% still consume alcohol, and 10% of still smoke cigarettes. 46% declared the use of body weight monitoring, but none of them measured their body weight every day. Conclusion. Health behaviour of geriatric patients with HF in Poland indicates their low compliance with medical guidelines and it may explain a high hospitalisation rate and increasing costs of treatment.

Objectives To assess the impact of patient education on medication adherence and quality-of-life (QoL) in Hungarian subjects with chronic obstructive pulmonary disease (COPD). Study Design Longitudinal, non-interventional study conducted at three pulmonology outpatient centers in and around Budapest, Hungary. Experimental Subjects visiting the center with COPD were invited to participate in the study. Data collected at baseline included subject demographics, medical history, and responses to the adherence (Morisky Medication Adherence Scale-8 (MMAS-8)) and QoL (EuroQoL-5D-5L (EQ5D), St. George’s Respiratory Questionnaire (SGRQ), and COPD Assessment Tool (CAT)) scales. Subjects were also provided with patient education designed with standardized content. Subjects were asked to provide responses to adherence and QoL scales again at the 3-month follow-up visit. Medication was left unchanged during the course of study. Statistical analysis included independent and paired-samples t-tests, one-way ANOVA, mixed-measures ANOVA and ANCOVA. Results Mean (± standard deviation (SD)) overall adherence score on MMAS-8 scale increased from 6.72 (± 1.46) at baseline to 7.01 (± 1.15) at follow-up (P = .040). A similar increase in mean (± SD) score was observed for question 4 on the MMAS-8 which deals with remembering to carry COPD medication when leaving house (baseline = 0.81 (± 0.40) versus follow-up = 0.89 (± 0.31); P = 0.018). Conclusions Patient education has a positive outcome on medication adherence in subjects with COPD. Further studies will be required to assess if these benefits are translated to patients’ QoL.

In the last 60 years, the number of Muslims in the Netherlands has increased considerably from 300 to more than 827,000. Immigration, in particular from Turkey and Morocco, was the main cause. In the same time period, Dutch society was secularising to a large extent. About 60% of the Dutch population is no longer affiliated with a religion. This paper focus on two questions. Is the Muslim population secularising as well? Is a Muslim religious identity hindering integration in, or identification with, (secular or Christian) Dutch society? To answer these questions, the results of recent surveys, both on the national and on the local (Amsterdam) level, have been analysed after considering existing theories on secularisation and immigrant integration. The results show that after initial indications of secularisation, there is no secularisation among Muslims of Turkish and Moroccan origin in the last decade ; mosque attendance by the second generation has actually increased. Existing theories on secularisation, such as the classical secularisation paradigm and market theories can not explain these developments, perhaps with the exception of the theory of existential security. Also assimilation theories do not prove to be valid. It appears that there is a continuing religious vitality among these immigrant groups as a consequence of socialisation in immigrant families and communities with relatively strong intra-group ties. Also the possibility of a kind of “reactive religiosity” in confrontation with an increasing public hostility towards Islam cannot be excluded. Concerning the second research question, there proved to be a positive correlation between religious identification (with Islam) and ethno-national identification (with the country of origin), which could hinder integration in, and identification with, Dutch national society. However, it does not hinder identification with the local (Amsterdam) society, which is relatively strong and functions as a “ bridging identity ” with the national society.

Aim. The reality genre has gained much popularity in last few years across the globe. In India too, this genre has reportedly surpassed other genres in recognition and acceptance. However, its format and content intrigue controversies at both social and academic level, but its mass appeal gets bigger each day. The study examines gender, language and issues of gendered language in Indian reality show Bigg Boss. While keeping in view the format and claim of the genre, the study observes how under thorough surveillance, housemates negotiate with their real self and grapple with their language practice. Influence of neoliberalism on society, culture and identity has been very much discussed, the paper attempts to highlight how this transitions in gender identity is depicted in a show taking non-actors and common people. Concept. For the purpose of the study, scripted transcription of Bigg Boss (Season 11) has been used, wherein all the episodes of have been watched back and forth carefully, and detailed notes prepared for the analysis. Housemates’ language practice, voice pattern and preference of words and statements have been observed. Results and conclusion. The study finds that game reality show Bigg Boss substantially adheres to social stereotypes and standards and while doing so, it also imitates the language practice prevalent everywhere. Although active participation of women in the show is visible butt while exhibiting their true self, sometimes gender prejudice embedded deep inside also comes out, which in case of men is very frequent. The study concludes that as a globally acclaimed genre, reality show, like other genres, is very much commercialized and consumer oriented.   Originality. A number of studies related to reality shows have been conducted, but in Indian context, this genre has been inadequately explored. Also, it is very difficult to find studies focusing any specific season of an Indian reality show, so it would not be incorrect to mention that the present study is one of the primary works on this subject, which aims at making some significant contribution related to this genre, in academia.

Introduction and aim. To cure any disease, proper use of medicine or taking medicine in the correct order is required. Even patients from developed countries struggle to maintain their drug compliance. There is an odd parallel between underdeveloped, emerging nations and the so-called developed world in the West when it comes to improper medicine use. The understanding and perception of the disease is the most important factor influencing whether patients stick to their treatment plan. Material and methods. Prioritized databases for public health topics included PubMed, ALTAVISTA, Embase, Scopus, Web of Science, and the Cochrane Central Registers. Along with other online sources, journals from Elsevier, Springer, Willey Online Library, and Wolters Kluwer were thoroughly searched. Analysis of the literature. There is a thorough discussion of medication non-adherence issues and a few adherence-improving methods. Conclusion. Adherence to treatment guidelines is essential in the treatment of any disease.

Non-adherence regarding pharmacotherapy and lifestyle – is aware non-adherence of the therapeutic team recommendations (total or partial, permanent or temporary). This phenomenon presents many dangers, particularly in the treatment of chronic diseases. Appropriate treatment of diseases requires compliance with recommendations for lifestyle. The objective of the study was assessment of adherence among the participants of the Undying Youth University and training for patients and their families. The study was conducted among 66 participants in the of Undying Youth University and training for patients and their families in John Paul II Hospital in Krakow. To conduct a study there were used a Modifi ed Morisky Scale and authorship questions concerning lifestyle, causes of non-compliance and methods to improve adherence. The study group comprised 14 M and 52 F. Respondents rated their level of compliance on average 7.78 on a scale of 0 to 10. 66.66% of the study group forgot to take medication, 51.51% wasn’t adhere to the hours of medication, 28.71% skips a dose, if it feels good and 31.81% when feel bad. 66.66% were informed about the action of drugs and 81.81% do not forget to renew prescription for medication. BMI of the study group averaged 26.24. 50% at least once a week practicing physical activity. The majority reasons for non-adherence were: difficult contact with the doctor, concern about the adverse effects of the drug, concern about interactions with other drugs, the high price of the drug, lack of understanding of the principles of treatment. For better compliance the group would make: training for patients, personal beliefs, cheaper drugs. The study group had a low level of motivation (forgetfulness, failure to hours, and interruption of treatment) with a high level of knowledge. Therefore there is a need to change behaviour through training to patients for the scope of treatment compliance.
Niestosowanie się do zaleceń dotyczących farmakoterapii i trybu życia – to świadome nieprzestrzeganie zaleceń zespołu terapeutycznego (całkowite lub częściowe, stałe lub okresowe). Zjawisko to stwarza wiele niebezpieczeństw, szczególnie w leczeniu chorób przewlekłych. Właściwe leczenie chorób cywilizacyjnych wymaga również przestrzegania zaleceń dotyczących stylu życia. Celem badania była ocena stopnia przestrzegania zaleceń terapeutycznych wśród uczestników Uniwersytetu Niegasnącej Młodości i szkoleń dla pacjentów i ich rodzin. Badania przeprowadzono wśród 66 uczestników Uniwersytetu Niegasnącej Młodości i szkoleń dla pacjentów i ich rodzin w Krakowskim Szpitalu Specjalistycznym im. Jana Pawła II. Do ich przeprowadzenia wykorzystano Zmodyfikowaną Skalę Morisky’ego oraz pytania własnego autorstwa dotyczące stylu życia, przyczyn nieprzestrzegania zaleceń oraz metod poprawiających przestrzeganie zaleceń. Badaną grupę stanowiło 14 mężczyzn i 52 kobiet. Badani ocenili stopień przestrzegania zaleceń średnio na poziomie 7,78. 66,66% zapominało przyjąć leki, 51,51% nie przestrzegało godzin przyjmowania leków, 28,71% pomijało dawkę leku, jeżeli czuli się dobrze, a 31,81% kiedy czuli się źle. 66,66% zostało poinformowanych o działaniu leków; 81,81% nie zapominało o przedłużeniu recepty na leki. BMI badanej grupy wyniosło średnio 26,24. 50% przynajmniej raz w tygodniu uprawia aktywność fizyczną. Wśród przyczyn nieprzestrzegania zaleceń najczęściej wskazywano utrudniony kontakt z lekarzem, obawę o niepożądane działanie leku, obawę o interakcje z innymi lekami. Do przestrzegania zaleceń badaną grupę skłoniłyby: szkolenia dla pacjentów, osobiste przekonania, tańsze leki. W badanej grupie stwierdzono niski poziom motywacji (zapominanie, nieprzestrzeganie godzin, przerywanie leczenia) przy jednocześnie dużej wiedzy. Istnieje zatem potrzeba zmiany zachowań przez szkolenia dla pacjentów za zakresu przestrzegania zaleceń terapeutycznych.
