Occupational accidents on board criteria determining is a challenging procedure in shipping industry as the ideal safety ship management strategy depends on many factors involving in shipping transportation There are many legislations, agreements and practices to obtain series of security measures in order to ensure safety and security of seafarers. Causes of on-board occupational accidents need to be evaluated in a correct manner to regulate more functional practices and also to lower the on-board accident rates. However, causes of on-board accidents can be extremely complex. Therefore, scientific methods should be used to evaluate the causes and to determine the measures to be taken. The evaluation of the parameters is of great importance for the future of the maritime sector and in terms of development. In this study, factors have been identified that lead to seafarers? occupational accidents on board and we tried to present alternative solutions which can be applied on this issue. Severity of the reasons that led to the accidents and their relationships with each other are identified to be able to sort through the alternative solutions with a model using the fuzzy AHP (Analytic Hierarchy Process) method approach. Results of the study revealed that the most important criteria for the occupational accidents on board criteria selection are respectively; human factors, lack of management, ship-borne troubles, cargo troubles and environmental factors.

Progressive changes in the demographic structure of developed countries observed for many years have caused increasing disproportions in the workforce age distribution. Every year, the number of older employees is reported to be increasing as opposed to the number of younger employees, which is decreasing. This affects the sphere of occupational health and safety. A work accident comprises a number of interrelated adverse technical, organizational, environmental, and human events [1,2]. However, it has been claimed that the main contributing factor responsible for work accidents is a human error (disruption). The causes of human errors may include: incapacity, nature of tasks being performed as well as physical and social environment. It is important to note that the first element is strongly related to both age and seniority. Based on available literature, the impact of age on work accidents is not clearly defined. Advanced age of employees favors the acquisition and consolidation of professional knowledge, but it can also be associated with greater automation of behaviors in work environment. This can lead to an error resulting in a dangerous situation or accident. The analysis of the age structure of individuals employed in mining enterprises indicates the dominance of employees aged 36–45 between 2003–2017. Bearing in mind the above, this study examined the relationship between age and the number of work accidents among individuals employed in mining enterprises. The study was carried out using the Pearson’s correlation coefficient. The source of analysis included statistical data on the size of employment in individual age groups and the number of work accidents in mining enterprises. The period adopted for the analysis was between 2003–2017. The research showed a correlation between age and the number of work accidents in hard coal mines.
Postępujące od wielu już lat w krajach rozwiniętych zmiany struktury demograficznej ludności powodują coraz większe dysproporcje w rozkładzie wiekowym siły roboczej. Z roku na rok wzrasta liczba pracowników starszych, a maleje młodszych. Nie pozostaje to bez wpływu na sferę bezpieczeństwa i higieny pracy. Na zdarzenie wypadkowe składa się szereg powiązanych ze sobą niekorzystnych zdarzeń technicznych, organizacyjnych, środowiskowych i ludzkich [1,2]. Uznaje się jednak, że zazwyczaj czynnikiem inicjującym wypadek przy pracy jest błąd (zakłócenie) człowieka. Pośród przyczyn popełniania błędów można wyróżnić przede wszystkim: niesprawność człowieka, cechy realizowanych zadań, warunki fizycznego środowiska oraz wpływ środowiska społecznego, przy czym pierwszy z wymienionych elementów jest bardzo powiązany z wiekiem czy stażem pracy pracownika. W literaturze przedmiotu wpływ wieku pracowników na występowanie wypadków przy pracy nie jest jednoznacznie zdefiniowany. Wyższy wiek pracownika sprzyja nabywaniu i utrwalaniu wiedzy zawodowej, ale może również wiązać się z większą automatyzacją zachowań w środowisku pracy, skąd tylko krok do błędu skutkującego zaistnieniem sytuacji niebezpiecznej czy wypadku. Poddana analizie struktura wiekowa pracowników przedsiębiorstw górniczych wskazuje na dominację w analizowanym okresie (lata 2003–2017) grup pracowników w wieku 36–45 lat. Mając na uwadze powyższe, w publikacji zaprezentowano analizy związków wieku i liczby wypadków przy pracy pracowników przedsiębiorstw górniczych. Badanie zależności miedzy tymi wielkościami przeprowadzono wykorzystując współczynnik korelacji liniowej Pearsona. Jako źródło analizy przyjęto dane statystyczne dotyczące wielkości zatrudnienia w poszczególnych kategoriach wiekowych pracowników i liczby zaistniałych wypadków w przedsiębiorstwach górniczych. Przyjęty do analizy okres to lata 2003–2017. Przeprowadzone badania wykazały istnienie związku korelacyjnego między wiekiem pracowników a liczbą wypadków przy pracy w kopalniach węgla kamiennego.

Nowadays downhill skiing is one of the most popular form of winter recreation. Although skiing fulfills a number of contemporary health and leisure needs, even skiing on very well-prepared terrain poses several dangers and risks. Objective safety would be the state of being protected or free from all danger. The subjective aspect relates to one’s mental state and is based on their feeling of confidence of another person, organization, or situation. Subjective risk is connected with perception and is dependent not only on how one perceives a threat but also how can assess its possible outcomes. There are three factors composing the qualitative dimension of risk perception, being ‘the fear of risk’, ‘an unknown risk’, and ‘the level of risk’. The first factor is associated with such features as worrying about potential consequences, anxiety, negativity, voluntariness, and the ability to manage risk. Data was collected by use of a diagnostic survey designed by the study’s author (Risk Assessment Questionnaire). In total, 53 participants completed the survey (26 instructors, 27 beginners). Beginner skiers believed there to be a higher level of risk in skiing than ski instructors, especially among aspects that assessed their perception of risk that concerned themselves. Beginner skiers clearly assessed the risk of downhill skiing higher when it concerned their own welfare and declared higher levels of fear of being involved in an accident.

Identification of trends of the examined phenomenon plays a major role in taking decisions and allows conducting a deeper analysis of phenomena connected with the shaping of proper working conditions. One of the result indicators in the OSH system is the accident rate, whose existence is the result of a combination of various events. Seeking tangible economic benefits, decision makers in business entities who wish to improve activities protecting health and life of employees, pay great attention to using quantitative methods and drawing conclusions from them. This is conditioned by the fact that the analysis of the economic aspects of accidents is connected to a large extent with the cost of benefits the employer incurs for the benefit of accident victims. Therefore, the main goal of the article is to examine what impact on the cost of benefits due to accidents at work is exerted by such factors as: the number of related benefits, persons injured in accidents at work depending on the consequences and the number of days of inability to work caused by these accidents. Furthermore, this paper shows changes in costs of accident benefits of persons receiving those benefits due to inability to work caused by accident in business. In order to achieve this, it is proposed to present an econometric analysis based on the cross-section-time data with the dynamics of considered variables in voivodeships in Poland being examined. The study uses the annual data for the years 2010-2017. The data come from the CSO publications.

“Human error” is often cited as cause of occupational mishaps and industrial accidents. Human error, however, can also be seen as an effect (rather than the cause) of trouble deeper inside systems. The latter perspective is called the “new view” in ergonomics today. This paper details some of the antecedents and implications of the old and the new view, indicating that human error is a judgment made in hindsight, whereas actual performance makes sense to workers at the time. Support for the new view is drawn from recent research into accidents as emergent phenomena without clear “root causes;” where deviance has become a generally accepted standard of normal operations; and where organizations reveal “messy interiors” no matter whether they are predisposed to an accident or not.

Safety and health issues remain critical to the construction industry due to its working environment and the complexity of working practises. This research attempts to adopt 2 research approaches using statistical data and court cases to address and identify the causes and behavior underlying construction safety and health issues in Malaysia. Factual data on the period of 2000–2009 were retrieved to identify the causes and agents that contributed to health issues. Moreover, court cases were tabulated and analyzed to identify legal patterns of parties involved in construction site accidents. Approaches of this research produced consistent results and highlighted a significant reduction in the rate of accidents per construction project in Malaysia.

The main objective for reactor safety is to keep the fuel in a thermally safe condition with adequate safety margins during all operational modes (normal-abnormal and accidental states). To achieve this purpose an accident analysis of different design base accident (DBA) as loss of flow accident (LOFA), is required for assessing reactor safety. The present work concerns this transients applied to Egypt Test and Research Reactor ETRR-2 (new reactor). An accident analysis code FLOWTR is developed to investigate the thermal behaviour of the core during such flow transients. The active core is simulated by two channels: 1 - hot channel (HC), and 2 - average channel (AC) representing the remainder of the core. Each channel is divided into four axial sections. The external loop, core plene, and core chimney are simulated by different dynamic lumps. The code includes modules for pump coast down, flow regimes, decay heat, temperature distributions, and feedback coefficients. FLOWTR is verified against results from RETRAN code, THERMIC code and commissioning tests for null transient case. The comparison shows a good agreement. The study indicates that for LOFA transients, provided the scram system is available, the core is shutdown safely by low flow signal (496.6 kg/s) at 1.4 s where the HC temperature reaches the maximum value of 45.64°C after shutdown. On the other hand, if the scram system is unavailable, and at t=47.33 s, the core flow decreases to 67.41 kg/s, the HC temperature increases to 164.02°C, and the HC clad surface heat flux exceeds its critical value of 400.00 W/cm2 resulting from the fuel burnout.

Background The profession of a firefighter is fraught with a significant risk of injuries. Firefighters operate in hazardous zones to mitigate threats and combat fires. Aim of work is to evaluate the accident rate of the officers of the Polish State Fire Service in connection with their service, considering direct participation in rescue operations, and other official duties between the years 2015–2022. Material and Methods An 8-year analysis was conducted from reports covered the number of accidents, the number of injured firefighters, the age of the officers, the circumstances of the event, and the type and location of the injury. This includes fire and rescue operations, exercises, training, official trips, sports activities, and technical work related to equipment. Results Between 2015 and 2022, there were 12 588 individual and group accidents in various circumstances. Most firefighter accidents occurred during rescue and firefighting operations, as well as during sports activities which dominate as the cause of injuries among firefighters in the observed period (37.8%), and injuries related to rescue and fire-fighting activities constitute 28.5%. Other work activities of firefighters cause 33.7% of injuries. In 2020 was a clear reduction in the number of accidents per 1000 employed firefighters in each observed parameter (total, interventions, sport) while maintaining the average level of firefighter employment (M±SD 30 099±224). Conclusions The most frequent circumstances of injury are slippery and challenging spaces, surfaces, and carelessness. The most common type of body injury is a dislocation and sprain concerning the ankle joint, foot. The most frequent causes of work-related injuries for firefighters, as the results indicate, are participation in team sports and fire-fighting and rescue operations. The most commonly occurring medical consequences resulting from a firefighter’s injury are fractures, sprains, and contusions of various body areas. Firefighters are most burdened with injuries in the age group 26–35 years old.

A paradigm shift is presently underway in the shipping industry promising safer, greener and more efficient ship traffic. In this article, we will look at some of the accidents from conventional shipping and see if they could have been avoided with autonomous ship technology. A hypothesis of increased safety is often brought forward, and we know from various studies that the number of maritime accidents that involves what is called “human error” ranges from some 60‐90 percent. If we replace the human with automation, can we then reduce the number of accidents? On the other hand, is there a possibility for new types of accidents to appear? What about the accidents that are today averted by the crew? This paper will present a method to assess these different aspects of the risk scenarios in light of the specific capabilities and constraints of autonomous ships.

INTRODUCTION: Road accidents lead to injuries which are one of the most common causes of hospitalisation in trauma centres. Proper placement and number of EMS units determine quick arrival in the scene of accident and need for transport to the adequate healthcare entity. MATERIAL AND METHODS: An analysis was carried out of the profile of the patient, the distance to the scene of accident as well as the allocation of accident casualties to healthcare entities. A statistical analysis was conducted by means of the Chi-Square Test of Independence and the Kruskal-Wallis test by ranks. RESULTS: Distances between the scenes of accidents and places where EMS units were stationed was from 0.5 to 37 kilometres (mean: 9.8 km). 170 patients participated in the accidents. Most frequently, these were males at the age from 21 to 40 (mean: 35.77 ± 15.45). The most common diagnoses according to ICD-10 were injuries to the head (42.7%) and injuries of lower limbs (12%). In most cases, the accident casualties were transported to the Emergency Department in Łuków. 21 patients in total were placed in hospital facilities outside the catchment area. The Polish Medical Air Rescue transported the 10 patients in the most severe condition. CONCLUSIONS: Road accidents are most frequently experienced by middle-aged men. Injuries to the head and of lower limbs are the most common injuries. The placement of EMS units in the area under analysis is optimal, but the mean distance covered by the ambulance to reach to scene of accident is nearly 10 kilometres. Despite the presence of the Emergency Department in Łuków, one in ten injured patient warrants transport outside the catchment area and almost 6% of patients qualify for transport by air.

Zmiany przepisów, które wyeliminowały część zagrożeń w chemicznej ochronie roślin, przyniosły nowe zagrożenia takie jak: stosowanie środków ochrony roślin niezgodnie z rejestracją, rosnąca sprzedaż preparatów fałszywych, zagrożenia związane ze stosowaniem mieszanin zbiornikowych i obniżonych dawek, zbędna chemizacja środowiska wskutek rejestrowania dawek wyższych niż to potrzebne i lekceważenie zasad BHP przez rolników. Coraz bardziej realne stają się także zagrożenie gromadzeniem składowisk środków przeterminowanych oraz możliwe problemy ze zbiorem opakowań po środkach ochrony roślin.
The cases of acute poisoning with pesticides have been rare in last years in Poland and usually occurred as a consequence of gross violation of safety rules. The exact number of chronic poisoning cases with pesticides in Poland is unknown. The legal changes resulting in elimination of some hazards in chemical protection, resulted also in new risks: use of plant protection products contrary to the label, increase in sales of fake products, hazards related to use of tank mixtures and dose reduction, excessive environment chemicalisation due to registration of too high doses and negligence of safety rules by farmers. The more and more real risks become anew problems with accumulation of pesticides whose date expired and possible problems with package collection.

Error is a part of human. Despite that organizations are trying to reduce error to the zero level, this goal is unachievable. As far as human operation is taking place in a complicated environment, error will occurred, and its possibility would be increased under the conditions of stress, extra loading work, and fatigue. One of the most important transportation modes is marine transportations. The sea is an unsafe place that kills many by a simple event. Every year there are thousands of marine accidents that result in injuries, casualties, marine pollutions and also massive financial loss. To reduce the accidents, there should be more attention to the factors such as suitable training of human resource, proper implementation of national and international laws and regulations, vessels and the equipment on board them, port facilities, and also the utilities for marine search and rescue. In this research 1816 marine accidents have been studied in five Iranian shipping companies. 17 factors are known to be effective in occurrence of human error in these accidents. Four factors of the most influence are negligence, poor training, inadequate tools, and lack of skill and experience.
