Introduction. High amputation of the lower limb not only causes immense physical disability but also the destruction of the patient’s mental state, and helps to shorten life of patients with diabetes. The incidence of amputations in diabetic patients is 10 times higher in comparison to non-diabetic subjects (2.8% vs. 0.29%). The purpose of the study is an analysis of the geographic variability of major non-traumatic lower limb amputation in diabetic and non-diabetic patients in Poland. Materials and method. All major non-traumatic lower limb amputations performed for the first time, in particular data between 1 January 2013 – 31 December 2013, and between 1 January 2014 – December 2014, were identified in the National Health Fund (NHF) database. In the presented study, the patients were grouped in relevant provincial departments of the NHF according to their place of residence, and not according to the hospital where lower limb amputation was performed. Results. In 2013 in Poland, 4,727 major non-traumatic lower limb amputation were performed in diabetic patients, and 4,350 in 2014. On the other hand, in non-diabetic patients, 3,469 major non-traumatic lower limb amputations were performed in 2013, and 3149 in 2014. The mean number indicator of major non-traumatic lower limb amputations in diabetic patients in Poland, compared to the average indicator of amputations in patients without diagnosed diabetes in Poland was 19.9-fold in 2013 and 19.4-fold higher in 2014. Conclusions. In populations of diabetic patients and individuals without diagnosed diabetes major non-traumatic lower limb amputations are performed over 19-fold more frequently.

The aim of the study was to examine the level of knowledge and sensitivity to dysmorphic features in a child with facial and body dysmorphia on the part of students of medicine and dentistry. We tested 70 students of medicine and 70 students of dentistry. A photograph of a child with craniofacial microsomia (CFM)was shown to all the tested students. Their task was to detect and name those facial deformities and describe the child in terms of selected features not related to the child’s health condition. As a tool was used the Overgeneralization Effect Scale and a questionnaire designed by the author relating to facial deformities. Significant differences were observed in the level of knowledge and sensitivity to dysmorphic features between students of medicine and dentistry. Future dentists detected more dysmorphic features in the face of the photographed child when compared to students of medicine. Interestingly, this sensitivity to abnormalities was found to noticeably increase with each subsequent year of study for dentistry students, while the opposite was observed for the stu dents of medicine. Importantly, a relationship was observed be tween the sensitivity to dysmorphic features and the general evaluation of the child in terms of non-medical aspects by the group of dentistry students. With the increase in the students’ skills to recognize dysmorphic features, the overall evaluation of the child tended to decrease. The results obtained indicate that the skills related to recognizing dysmorphic features are better in students of dentistry than in students of medicine. The sensitivity to abnormalities evidently increase with each subsequent year of study for dentistry students, while the opposite was observed for medical students.

When a child is born with a deformed face, his/her social environment is quickly confronted with a change which undoubtedly favours numerous over-generalizations. Children with craniofacial microsomia anomalies (CFCs) may experience less social support, more rejection and make them withdraw from their social life more frequently. The purpose of this research study is to show how children with craniofacial hypoplasia abnormalities is perceived by their close relatives. The sample of this study consisted of 26 participants (F=16; M=10). This research study was conducted using the following questionnaire methods: the Scale of Over-Generalization Effect by K. Milska and A. Mański (SOGE), modified Own Health Assessment Scale method (SOWC) and the Authors-Designed Questionnaire (ADQ) to obtain information on the issue. A child's illness always affects a variety of family life spheres. Most of the surveyed women did not experience any complication during pregnancy or childbirth. Unfortunately, after their child was diagnosed, most adults were not offered getting in contact with a psychologist / psychiatrist / psychotherapist. Some relatives - after the birth of a child with craniofacial microsomia (CFM) - reduced their working hours or gave up work completely in order to take care of the child. The most urgent needs for this child's illness reported by adults most often referred to educational and financial matters. At the time of this research study, most of the respondents (61%) - upon the birth of a child with CFM - considered it plausible to enlarge their family. As a result of the conducted research studies, new variable systems (a type and character of dysmorphia, closeness - distance) were identified, which may be a relevant element to facilitate research studies on the perception of children with body deformities by their environment. In the characteristics of a child with CFM, it was shown that his/her close relatives evaluate the child positively. Family members apply constructive strategies for coping with the child's illness, however, the research study results indicate the legitimacy of introduction of psychological and psychiatric consultations for close relatives to the standards of CFM child treatment.

Introduction. The article describes the process of converting a large multi-specialized hospital into one dedicated to COVID-19 patients, and present established standards of work organization in all the wards and training system of the medical and supporting staff. The several weeks pandemic of the COVID-19 disease has forced the healthcare systems of numerous countries to adjust their resources to the care of the growing number of COVID-19 patients. Managers were presented with the challenge of protecting the healthcare workers from transmission of the disease within medical institutions, and issues concerning the physical and psychological depletion of personnel. Materials and method. Based on analyses of the structure and work processes in Central Clinical Hospital (CCH) reconstructive strategic plan was developed. It included: division of existing wards into observation and isolation wards; installing locks; weekly plan for supplying personal protection equipment (PPE); designating new access to the hospital and communication routes; training of medical and supporting staff. The plan was implemented from the first days of conversion of the hospital. Results. The wards of the CCH were converted for observation and isolation, and each one was fitted with sanitary locks. There was a big improvement in the supply of PPE for the medical staff. Separation of the ‘dirty’ and ‘clean’ parts of the CCH were attained, and widespread intensive training not only protected personnel against infections, but also diminished unrest which was discernable at the beginning of conversion. Conclusions. The transformation efforts will ultimately be appraised at the end of the epidemic, but the data looks encouraging. Two weeks after conversion, the testing of hospital Staff was started and by the end of April, 459 tests were had been conducted, of which only 11 were positive.

Background: Multiple sclerosis is a chronic and highly debilitating disease with very high economic and social consequences. Designing changes to improve the functioning of the healthcare model primarily requires learning about the needs of beneficiaries. Aim of the study: The purpose of our study is the results’ description and summary of conclusions of the research conducted in the past years. These results are currently being used for the construction of a coordinated care model for MS patients. Material and methods: One questionnaire survey addressed to MS patients, carried out between 01.12.2013 and 01.02.2014 and the second one addressed to Polish and European medical professionals, carried out between 01.09.2016 and 04.10.2016 performed in 51 European centers and 2 branches of the Turkish Association of patients with MS. Results: In the first survey 84.4% patients declared that during the illness they received mental support mostly from their family: 48.5% received it from physicians; 42.1% from a nurse. 64.8% of the respondents declared that they received no support from social organizations and 77.6% received no support from religious organizations. According to the results of the survey for healthcare professionals in Poland and Europe, in 54.9% of the European institutions and in 22.4% of the Polish institutions, dedicated IT systems are used for processing MS patients’ data. Among institutions using IT, 52.9% of the European and 10.1% of the Polish ones keep the patient’s entire documentation, as well as a patient satisfaction survey, in an IT system. Conclusions: Based on our studies described above, the patients’ needs, resources and capabilities seem to indicate that the scale of system inefficiencies is such that remodeling care for this well-defined group of patients is justified and feasible.

Introduction. Several studies have suggested, that diabetes is very important factor of the risk of the chronic a renal disease. The paper aims to present a retrospective analysis of incidence of end-stage renal disease in subgroups of patients with and without diabetes in Poland. Material and methods. For assessing this problem an electronic search was performed using Polish National Health Fund data base from 01.01.2011 until 31.12.2013 in general population and from 01.01.2012 until 31.12.2012 in 2 populations: with diabetes and without diabetes mellitus. Results. The 43.70 % patients with end-stage renal disease was diabetic. The incidence rate in 2012 was: 292.48 ± 90.97 diabetic men / 100,000 diabetic population; 203.10 ± 90.97 diabetic women / 100,000 diabetic population; 23.44 ± 6.34 non-diabetic men / 100,000 non-diabetic population; 17.88 ± 6.33 non-diabetic women / 100,000 non-diabetic population. Conclusions. The incidence rate of end-stage renal failure in diabetics was multiple times higher than the incidence rate in non-diabetics. The incidence rate of new ESRD cases in Poland estimated to be 36.17 per 100,000 of general populations in 2011, 35.28% in 2012 and 30.46 per 100,000 of general populations in 2013. In 2012, the incidence rate of new ESRD cases in male diabetics was 292.48 ± 90.97 per 100,000 of diabetes men population, and in women diabetics 203.10 ± 66.06 per 100,000 of diabetes women population. In the same 2012 year, the incidence of new ESRD cases in men non-diabetics was 3.44 ± 6.34 per 100,000 of non-diabetes, and in women non-diabetes 17.58 ± 6.33 per 100,000 of non-diabetes women population.

Przeprowadzono badania usuwania oleju z wody. Zweryfikowano działanie dwóch komercyjnie oferowanych deemulgatorów, zmieniając ich stężenie oraz czas. W instalacji pilotażowej pracę separatora koalescencyjnego wspomagano dodatkiem wybranego deemulgatora w dawce określonej w badaniach laboratoryjnych. Uzyskano redukcję stężenia zemulgowanego oleju poniżej 10 mg/dm3, pozwalającego na bezproblemową pracę modułów membranowych w zasadniczej części instalacji oczyszczania wody po szczelinowaniu.
In this work the oil removal from water was studied. The effect of concentration and time was investigated for two commercial deemulsifiers. Then, selected product was used to improve the separation performance of coalescence separator in the pilot-scale rig. The oil concentration below 10 mg/dm3 was obtained, which enables a reliable long-time operation of membranes located downstream the coalescer in the cleaning system of water used for hydraulic fracturing.

In this work the esterification of diethyl tartrate was studied. The research was focused on the enhancement of reversible reaction yield, which is accomplished by dewatering of the reaction mixture. The removal of water shifts the equilibrium towards the main product. Pervaporation was applied for this purpose, and results were compared to distillation. The advantages and limitations of both processes are discussed. The experimental part consists of dewatering of mixture after the reaction had reached the equilibrium, and was subsequently fed to the test rig equipped with a single zeolite membrane purchased from Pervatech B.V. Results show a significant conversion increase as a result of water removal by pervaporation. Compared to distillation no addition of organics is necessary to efficiently remove water above the azeotrope. Nevertheless, some limitations and issues which call for optimisation are pointed out. A simple numerical model is proposed to support design and sizing of the pervaporation system. Various modes of integrated system operation are also briefly discussed.

Nowadays there is quite huge need for more and more precise and effective fast diagnostics methods in hematology diseases. One of the most important blood components are erythrocytes – RBCs (Red Blood Cells). Due to their size they are easy to observe using microscopy. It is commonly known that the shape and lifetime of RBCs allows for early disease identification. Authors present special measurement system for RBCs fluctuations observation using diffraction phase microscopy which is also described in theory.
Istnieje zapotrzebowanie na coraz bardziej precyzyjne i bardziej efektywne metody szybkiej diagnostyki w kierunku chorób hematologicznych. Jednym z głównych składników krwi są erytrocyty (RBC –Red Blood Cells), które z uwagi na swój rozmiar mogą być łatwo obserwowalne. Powszechnie wiadomo, że kształt i długość życia tych komórek pozwala na wczesną identyfikację stanów chorobowych. Najnowsze wyniki badań świadczą też, że oprócz tych cech istotną informację zawiera przebieg fluktuacyjny erytrocytów. Autorzy proponują układ do obserwacji fluktuacji czerwonych komórek krwi z wykorzystaniem dyfrakcyjnego mikroskopu fazowego.

The paper presents the results of the experimental study of the three-layer Al-Mg-Al sheets rolling process by the ARB method. The tests carried out were limited to single-pass symmetric and asymmetric rolling processes. An Al-Mg-Al package with an initial thickness of 4 mm (1-2-1 mm) was subjected to the process of rolling with a relative reduction of 50%. To activate the shear band in the strip being deformed, an asymmetry factor of av=2 was applied. From the test results, an increase in the tensile strength of the multi-layer Al-Mg-Al sheets obtained from the asymmetric process was observed. Microhardness tests did not show any significant differences in aluminium layer between respective layers of sheets obtained from the symmetric and the asymmetric process. By contrast, for the magnesium layer, an increase in microhardness from 72 HV to 79 HV could be observed for the asymmetric rolling. The analysis of the produced Al-Mg-Al sheets shows that the good bond between individual layers and grain refinement in the magnesium layer contributed to the obtaining of higher mechanical properties in the multi-layer sheets produced in the asymmetric process compared to the sheets obtained from the symmetric process.
W pracy przedstawiono wyniki badań doświadczalnych procesu walcowania metodą ARB trójwarstwowych blach Al- Mg-Al. W przeprowadzonych badaniach ograniczono się do jednoprzepustowego symetrycznego i asymetrycznego procesu walcowania. Procesowi walcowania poddano pakiet Al-Mg-Al o grubości początkowej 4 mm (1-2-1 mm) walcowany z gniotem względnym 50%. W celu aktywacji pasm ścinania w odkształcanym paśmie zastosowano współczynnik asymetrii av=2. Na podstawie wyników badań zaobserwowano wzrost wytrzymałości na rozciąganie wielowarstwowych blach Al- Mg-Al otrzymanych w procesie asymetrycznym. W wyniku przeprowadzonych badań mikrotwardości w poszczególnych warstwach dla blach otrzymanych w procesie symetrycznym i asymetrycznym nie stwierdzono istotnych różnic w wartościach mikrotwardości dla warstw aluminiowych. Natomiast dla warstwy magnezowej można było zaobserwować zwiększenie mikrotwardości z 72 HV do 79 HV - dla walcowania asymetrycznego. Analiza mikrostruktury otrzymanych blach Al-Mg- Al wskazuje, iż dobre połączenie pomiędzy poszczególnymi warstwami oraz rozdrobnienie ziarna w warstwie magnezu miały wpływ na uzyskanie wyższych właściwości mechanicznych w wielowarstwowych blachach otrzymanych w procesie asymetrycznym w porównaniu do blach wytworzonych w procesie symetrycznym.

Odwadnianie oleju napędowego z wykorzystaniem hydrofobizowanych spieków polimerowych

Opis:
W pracy przedstawiono badania odwadniania oleju napędowego z wykorzystaniem mediów porowatych uzyskanych przez spiekanie granulatów polimerowych. Opracowano procedurę testowania filtrów odwadniających w oparciu o istniejące metody standardowe. W celu uzyskania większej hydrofobowości spieków przeprowadzono próby modyfikacji powierzchni skutkujące powstaniem pokryć o odpowiednim składzie chemicznym i mikrochropowatości. Efekty tych działań zweryfikowano w obserwacjach mikroskopowych i doświadczalnie.
Dewatering of diesel fuel using sintered granular polymers is studied in the paper. A testing procedure was elaborated based on existing standard testing methods for fuel filters. To increase the hydrophobicity of polymer media a few types of surface modification, resulted in relevant chemical composition and morphology (roughness) of the surface, were performed. Effect of coating were verified experimentally and via SEM observations.
