Richter’s syndrome (RS) is a rare complication in which chronic lymphocytic leukaemia (CLL) or small lymphocytic lymphoma (SLL) transforms into a more aggressive type of lymphoma – diffuse large B cell lymphoma (DLBCL), or Hodgkin’s lymphoma (HL). The review describes the clinical case of a patient with CLL and RS diagnosis. A computed tomography (CT) scan of the abdominal cavity detected numerous normodense areas in the liver. Simultaneously, ultrasound examination (USG) of the thyroid revealed the presence of a solid hypoechogenic lump. The material sampled from closed biopsies of liver and thyroid in both cases allowed the diagnosis of diffuse large B cell lymphoma (DLBCL). The liver and the thyroid are particularly rare locations of RS. However, those cases allowed the conclusion that RS may occur even in a very unexpected and less probable location.

Introduction. Despite significant recent advances in the treatment of chronic lymphocytic leukaemia (CLL), most cases of the disease are still incurable. Treatment with monoclonal antibodies, such as ofatumumab, is one of the new therapeutic options. Objective. Retrospective analysis of the efficacy of ofatumumab in patients with chronic lymphocytic leukaemia (CLL) treated in the Haematooncology and Bone Marrow Transplantation Department of the Medical University of Lublin, Poland, during 2011–2013. Materials and method. The analysis included 5 patients (3 women and 2 men), aged 47–65, with Rai stage II-IV CLL, after a few lines of treatment. Three patients received ofatumumab in monotherapy and 2 patients received ofatumumab in combination with cyclophosphamide (50 mg/day) and dexamethasone (40 mg/day). All patients included in the study were diagnosed with an active form of leukaemia with symptoms such as lymphocytosis or massive lymphadenopathy. Results. All patients responded to the treatment. Within the first 8 weeks of the treatment, levels of white blood cells returned to normal in patients with baseline lymphocytosis (3 patients). An increase in platelet levels was reported in 3 patients. Haemoglobin levels were higher or comparable to the baseline values in all studied patients after the completion of immunotherapy. In the patient with massive lymphadenopathy and hepato- and splenomegaly, the size of the lymph nodes, spleen and liver decreased and neutrophil levels increased. Time of progression was 5–12 months, and in one patient partial remission has been maintained. The treatment was well-tolerated in most cases. Asymptomatic neutropenia and an infection with Candida glabrata were observed. Conclusions. Ofatumumab may be a new and safe therapeutic option for patients with CLL after a few lines of treatment.

Introduction. Prostate cancer (PC) is the most commonly diagnosed malignant tumour and the third cause of cancer deaths among men in Europe. The treatment of early-stage PC is very effective and in many cases allows achievement of a complete cure, whereas the treatment of metastatic prostate cancer (mPC) is still a huge challenge for clinicians. New therapeutic strategies for mPC are urgently needded. One of the most promising methods of treatment is anticancer immunotherapy including the monoclonal antibodies against immune checkpoint inhibitors. Objectives. To present the potential possibilities of using checkpoint inhibitors blockage in the treatment of mPC, and to overview the results of recent research on immune checkpoint inhibitors in patients with PC. State of knowledge. Recent studies suggest that monoclonal antibodies directed against immune checkpoint inhibitors in combination with traditional therapy may become a breakthrough in the treatment of mPC in the near future. Conclusions. The immunotherapy using monoclonal antibodies against immune checkpoint inhibitors seems to be a new opportunity for patients with advanced PC. The key to achieve the maximum anti-tumour response is to choose the best candidates for this therapy and determine the optimal sequence and combination of drugs. The introduction of immunotherapy as the standard treatment of patients with advanced PC requires further studies.

Multiple myeloma is a neoplastic disease which is characterised by proliferation of monoclonal plasmocytes in the bone marrow. It is the second most common hematologic cancer and it represents 1% of all cancer deaths. Despite enormous development in multiple myeloma biology and treatment over the last 30 years - it is still incurable disease with a median survival of 50 – 55 months. Currently, one of the most important goals in the treatment of multiple myeloma is to achieve long-term control of the disease, without negative impact on the patient’s quality of life. Thanks to therapeutic regimens based on new immunomodulatory drugs, this aim seems to be achievable. In this paper we present the case of a female patient living with multiple myeloma for 14 years. Initially patient was treated with standard VAD (vincristine, doxorubicin, dexamethasone) chemotherapy regimen. After a nearly complete remission of the disease, autotransplantation of hematopoietic cells was performed. One year after transplantation there was a relapse of the disease. In the treatment of relapse it was decided to use scheme based on lenalidomide and dexamethasone. After 4th cycle of treatment, a complete remission was achieved. So far, the patient received 149 cycles. In the evaluation of minimal residual disease still maintains a state of complete remission maintains. During over 12 years of treatment no complications in grade 3 and 4 of the CTCAE v.4 was observed. Currently the patient is 58 years old, she still receives lenalidomide and leads moderately active life.

This paper presents the issues related to the specific of market of regional and traditional products and shows how important may be the development of this market in the context of environmental protection. This is due to the fact that the idea of food production and consumption in the same region reduces the need for long-distance transport (this reduces fuel consumption and also helps reduce the greenhouse effect). Another issue is that the regional products are made from components having a common ecological character (this helps to reduce chemical treatments that may harm nature). Besides, in some cases, regional products can contribute to maintaining natural ecosystems and enhance natural values of a specific area. An example of this is described in detail in this study regional product “Sudeten Beef”, which is produced in the Polish part of the Sudetes Mountains in the voivodeship of Lower Silesia (south-western Poland). Regional products can play an important role in sustainable development, particularly in relations to the peripheral areas. This applies to environmental aspects as well as the typical economic. In this article defines sustainable development as a way of doing business, which on the one hand give a positive economic results, and at the same time will as far as possible to care for the environmental and does not cause over-exploitation of resources, so that future generations can also use them. In addition, sustainable development also means equalizing opportunities between regions with high growth potential (usually of a large urban areas) and areas with less growth potential (in Polish condition this is particularly true of rural areas far from big cities). According to such assumptions, the development of regional product’s market can contribute to sustainable economic development of rural areas, because it gives opportunities to create new jobs, increasing incomes and the stimulation of the local community. Regional product „Sudeten Beef”, which is the main focus in this article, has a good chance to market success. This product comes from meat obtained from cattle in meat and combined type, which is kept in Sudeten districts. This is due to both the natural character of the Sudetenland, and the potential of demand and supply in the study area. Placing on the market “Sudeten Beef” should enable the development of agricultural production in direction to which the region is especially predisposed because of natural conditions. This product may also be a source of additional income for local farmers and entrepreneurs. The benefits reach also the consumers who receive high quality and unique food product that will enrich the catering and tourist offer in the study area. Placing such type of product on the market can run non-agricultural activities in rural areas covered by the project. This type of activity can help in the integration of local communities related to agricultural, food production, catering and tourism. Finally it can help to preserve valuable environmental assets Sudeten Mountains (grazing cattle will help to maintain a balance in areas of high natural value, due to the act as “natural lawn”).

Przedstawiono możliwość wdrożenia produktu regionalnego, jakim jest wołowina sudecka. W tym celu dokonano analizy wielkości i struktury stad bydła utrzymywanego w powiatach sudeckich. Oszacowano wielkość i strukturę bazy hotelowo-gastronomicznej, w której można sprzedawać potrawy z wołowiny sudeckiej. Określono także wielkość popytu na ten produkt. Badania prowadzone były w latach 2010-2011 na terenie powiatów: kłodzkiego, jeleniogórskiego, lwóweckiego, lubańskiego, wałbrzyskiego, kamiennogórskiego, dzierżoniowskiego i ząbkowickiego. Z badań wynika, że 275 placówek hotelowo-gastronomicznych sprzedało w badanym okresie ponad 6 min posiłków głównych, natomiast szacowaną sprzedaż posiłków z udziałem wołowiny sudeckiej wyliczono na 851 200, a szacowany roczny popyt gastronomii na wołowinę sudecką określono na 85,1 t.
The paper presents the possibility of implementation of the regional product sudeten beef For this purpose it analyzes the size and the structure of herds maintained in sudeten counties. Moreover in the paper was estimated the size and the structure of accommodation and gastronomic facilities in which it is possible to sell meals of the sudeten beef. Volume of demand for this product was also determined. The study was conducted in the years 2010-2011 in the following districts: Kłodzko, Jelenia Góra, Lwówek, Lubań, Wałbrzych, Kamienna, Dzierżoniów and Ząbkowice. Research has shown that 275 objects sold during the period more than 6 million main dishes, while the projected sale of meals with Sudeten beef was calculated on 851 200 meals. The projected annual demand for catering to the sudeten beefissetat85.l t.
