ObjectivesThe aim of the presented study was to compare the health locus of control (HLC) between employed and unemployed patients after kidney transplantation (KT), and to investigate the relationship between HLC and the quality of life (QoL).Material and MethodsThe study group consisted of 101 KT patients and the control group of 60 hemodialysis (HD) patients. The applied methods were: the Multidimensional Health Locus of Control Questionnaire (MHLC), the WHO Quality of Life Questionnaire (WHOQoL-BREF), and a survey collecting information on the socio-demographic status and work experience.ResultsOverall, 57.5% of KT patients were employed and 42.5% were unemployed. In the HD group, 25% were employed and 71% were unemployed, while 4% did not disclose their employment status. The unemployed KT patients, in comparison with the employed ones, presented a higher feeling of the impact of chance on their health (unemployed M±SD 23.68±6.59; employed M±SD 21.02±4.57) and a lower level of QoL on the Somatic Scale (unemployed Me = 14.00, IQR = 3.00; employed Me = 1450, IQR = 3.00) and the Environmental Scale (unemployed M±SD 15.39±2.83; employed M±SD 16.85±3.24). In the employed KT group, the Internal Control Scale (MHLC) correlated with all QoL scales (the Somatic Scale: r = 0.292, p = 0.036; the Psychological Scale: r = 0.455, p = 0.001; the Social Scale: r = 0.304, p = 0.029; and the Environmental Scale: r = 0.307, p = 0.027). In the unemployed KT group, the Internal Control Scale (MHLC) correlated significantly with the Somatic Scale (r = 0.396, p = 0.013) and the Psychological Scale (r = 0.374, p = 0.019).ConclusionsThe employed KT patients have a higher level of independence, with results indicating a strong internal type. Additionally, the working patients assess their QoL better, both in terms of their health condition and the organization of satisfying environment. The obtained knowledge about the psychological characteristics of KT patients may be useful for the occupational activation programs and psycho-education for those with weaker predispositions.

Autosomal dominant polycystic kidney disease is the most common genetic cause of renal failure. Apart from kidney involvement, patients are at risk of extra-renal manifestations, including vascular lesions. The etiology of vascular changes is diverse and depends, among other factors, on polycystin gene mutation, increased activity of the renin-angiotensin-aldosterone system and the occurrence of hypertension. The observed vascular system complications include cerebral artery aneurysms, cervico-encephalic arteries' dissection, aortic aneurysm and dissection and intracranial arterial dolichoectasia. This article discusses the etiopathogenesis, symptomatology, principles of prevention and treatment of the aforementioned diseases of the vascular system accompanying polycystic kidney disease.

Microbiota studies have uncovered numerous associations between human gut microbes and health-related outcomes. However, since most of these correlations were observed in cross-sectional studies, the causal infe rence is limited. The causal contribution of microbiota can be evidenced through disease induction or exacerbaiton in animal models aeftr fecal microbiota transplantation (FMT) from patients. In this article we present a protocol for a scoping review on the subject of FMT from humans to animals. Besides assessing how the published studies were conducted, in that scoping review we aim to find out whether enough literature exists to conduct a systematic review of the evidence for microbiota participation in the pathophysiology of any human non-infectious disease or phenotypic trait. We will conduct searches on the Web of Science plaotfrm and databases: MEDLINE, Scopus, EMBASE. Citation chasing of included studies will be done. We will include studies assessing the eefcts of FMT collected from people with certain medical conditions on animals. Studies that recruited only healthy humans or used other animals as donors will be excluded. The results of this literature search will be tabulated and discussed. Moreover, we will provide a short list of human non-infectious diseases or traits with the highest number of FMT patient-to-animal studies. In the horizontal line bisection task, patients with end-stage renal disease (ESRD) treated with dialysis typically have an increase in the normal leftward attentional bias. It is not clear, however, whether this increased bias is related to an increase of attention toward left hemispace or impaired disengagement from left-sided stimuli. Non-demented patients with ESRD who were being treated with dialysis and demographically matched healthy controls attempted to bisect series of horizontal lines composed of two segments of unequal length and width, with the longer segment placed to the right or left. To test for the presence of a global spatial attentional bias, participants attempted to find the middle of the whole line. To test for focal attentional engagement and attentional disengagement, participants were asked to bisect only the longer segment of these compound lines. For the global condition, both patients and healthy controls were biased toward the longer segment and more so to the left. In the focal condition, when the longer segment was on the right side, both groups deviated their bisections toward the shorter segment. However, when in the focal condition the longer segment was on the left healthy controls deviated their bisection significantly more rightwards than did the patients, whereas with the longer segment on the right, the patients and healthy controls did not differ in their bias. These results suggest that the previously recognized dysfunction of the right fronto-subcortical attentional network in dialyzed patients might account for the results of the current study.

Background: Incidence of morbid obesity is rising worldwide. Current clinical practice guidelines for the pre-transplant evaluation of end-stage kidney disease (ESKD) patients lack clear recommendations on morbid obesity. Material and methods: The aim of this review was to summarize the current guidelines on the role and treatment of obesity in kidney transplant recipients. Eight current national and international clinical practice guidelines were identified in a comprehensive literature search. Results: All guidelines underline early detection of obesity and obesity-related comorbidities in ESKD patients. Only two guidelines explored the role of weight-loss surgery, however due to the lack of sufficient evidence no formal recommendation of surgical procedure was given. Conclusions: Diagnosis and treatment of obesity remains underappreciated in the current guidelines, most of which do not include pharmacological and surgical interventions. High-quality evidence is warranted to assess the role of weight-loss including surgery in ESKD patients and to update the recommendations in future guidelines.

Przewlekła choroba nerek (PChN) od kilku lat jest wymieniana wśród chorób cywilizacyjnych podobnie jak otyłość, cukrzyca, choroby sercowo-naczyniowe i nadciśnienie tętnicze. Szacuje się, że w Polsce na różne stadia tej choroby może cierpieć ponad 4 mln ludzi. Może ona być również następstwem wszystkich pozostałych chorób cywilizacyjnych. Jednocześnie warto dodać, że problemy nefrologiczne są coraz częściej uwzględniane w nowoczesnym orzecznictwie lekarskim. Celem pracy jest m.in. poprawa bezpiecznego dostępu do rynku pracy pacjentów z chorobami nerek. Przewlekła niewydolność nerek w stopniu zaawansowania, który może stanowić zagrożenie w sytuacji kierowania pojazdami, jest wymieniana jako przeciwwskazanie zdrowotne do kierowania pojazdami w przepisach prawnych obowiązujących w naszym kraju od 2014 r. Również w załączniku do Rozporządzenia Ministra Zdrowia z dnia 9 grudnia 2015 r. w sprawie warunków zdrowotnych wymaganych od marynarzy do wykonywania pracy na statku morskim wymieniono kody N17–N19 według klasyfikacji ICD-10 (International Classification of Diseases – Międzynarodowej Klasyfikacji Chorób), odpowiadające ostrej i przewlekłej oraz nieokreślonej niewydolności nerek, które należy wziąć pod uwagę przy kwalifikacji pracowników do pracy na morzu. For a number of years chronic kidney disease (CKD) has been listed in the group of lifestyle diseases, such as obesity, diabetes, cardiovascular disease and hypertension. It is estimated that in Poland more than 4 million people may suffer from various stages of CKD. Chronic kidney disease may also be a consequence of all the other civilization diseases. At the same time it is worth noting that nephrological problems are increasingly being taken into account in modern medical certification. The aim of this work is, among other things, to improve safe access to the labor for patients with kidney diseases. In the legislation existing in our country since 2014 it is stated that chronic renal failure is a potential health contraindication to driving. Also in the annex to the Regulation of the Minister of Health dated 9 December 2015 on health conditions required for seafarers to work on a seagoing ship, it is said that ICD-10 codes (International Classification of Diseases) corresponding to acute and chronic renal failure (N17–N19) should be taken into account when qualifying employees to work at sea. Background. Dietary supplements (DS) are available over the counter, but patients with impaired renal function are specifically at risk for toxicity when consuming certain DS. The aim of this study was to evaluate the prevalence and characteristics of DS use in patients with chronic kidney disease (CKD). Material and methods. A cross-sectional, controlled DS use survey (22 questions) was conducted among 180 CKD patients (stage 1-5, dialysis, kidney transplant), with 60 patients without CKD serving as controls. Results. DS use did not differ significantly between subjects with and without CKD, unless the CKD patients were on dialysis. In the CKD group, 20% admitted to use DS regularly and 22% did not take the mat all. In the controls, DS consumption was 17% and 13%, respectively (NS). The DS use was higher among women ascompared to men (89% vs. 70%; p < 0.005), and people living in cities versus those living in the country side (81% vs. 63%; p < 0.05). DS most commonly used were: vitamins, minerals, and herbs. Major indications for DS use included: musculoskeletal issues, general health improvement and prevention of urinary tract infections. Subgroup analyses revealed that dialysis patients were characterized by a significantly higher DS use in comparison to CKD stage 1-5 subjects and renal transplant recipients. The decision to introduce DS was made by the physician in 54% of cases; by a pharmacist in 9% of cases, and by the patients themselves in 37%. Only 21% of patients with CKD, and 27% of subjects without CKD, declared knowledge of any possible side-effects associated with DS (NS). Conclusions. The use of DS among patients with CKD is similar to patients without CKD, with the exception of those on dialysis. Vitamins and minerals were the most commonly reported DS consumed. The knowledge on potential side-effectof DS was limited to approximately one-fourth of those surveyed.

Background: Analgesics can be sold following medical prescription, but also as over-the-counter (OTC) medications. In patients with chronic kidney disease (CKD), their use could potentially be associated with increased risk of side-effects, due to impaired renal elimination. The aim was to evaluate the epidemiology and indications for the use of OTC analgesics, and the knowledge of their side-effects in patients with CKD. Material and methods: A cross-sectional, controlled survey on the use of OTC analgesic drugs was conducted among 180 CKD patients (stage 1-5, dialysis, kidney transplant), compared to 60 controls. Results: The proportion of patients using OTC analgesics on a regular basis was higher in the CKD group, compared to controls (18.9% vs. 10.0%, p<0.02). The major indications included musculoskeletal issues, followed by headaches and other. Subgroup analysis revealed that analgesic use was lowest among transplanted patients, in comparison to CKD stage 1-5, and dialysis subjects (10%, 20%, 26%, respectively, p=0.06). Less than half of CKD patients and controls declared any knowledge on potential side-effects of analgesic drugs (45.6% vs. 40.0%, NS). Conclusions: The use of OTC analgesics among patients with CKD is higher than in subjects without CKD, with the exception of transplanted patients. The knowledge on the potential side-effect of analgesics is limited.

Background: The renoprotective effects of the direct renin inhibitor, aliskiren, in renal transplant recipients have been supposed, but not finally proven. We performed an exploratory double-blind, losartan controlled, cross-over study to evaluate the influence of aliskiren, direct renin inhibitor, on albuminuria and other surrogate markers of kidney injury in patients after renal transplantation. The safety of this therapy was also evaluated. Method: 16 of 18 patients (12 M, 4 F), 48.3 ± 9.0 years, 57.7 ± 9.1 months after kidney transplantation, with hypertension and stable serum creatinine 1.4 ± 0.08 mg/dl without proteinuria, completed the protocol. Each patient underwent two 8-week treatment periods (one with 150 mg of aliskiren, and one with 50 mg of losartan) in random order, allowing an 8-week placebo washout between them. Results: There were no differences in albuminuria, transforming growth factor β-1 and 15-F2t-isoprostanes urine excretion between aliskiren and losartan. Creatinine serum level, eGFR, 24 h systolic and 24 h diastolic blood pressure were stable through the study. There were no differences in haemoglobin and potassium serum concentration between studied drugs. Conclusion: Aliskiren decreases albuminuria in renal transplant recipients with clinically minimal side effects. The effect does not differ from that of losartan.

Chronically hemodialyzed (HD) patients are at a high risk of developing very severe forms of COVID-19 disease. In this article we describe three HD patients (all males, aged 70, 70 and 74 years) vaccinated intramuscularly with a two-dose mRNA BNT162b2 vaccine; BionTech/Pfizer Comirnaty, in whom subsequent breakthrough SARS-CoV-2 infections developed. All patients achieved post-vaccine seroconversion for anti-spike antibodies with IgG titers of 445, 227 and 92.5 AU/mL (cut-off, 13 AU/mL) case 1, 2 and 3 respectively. SARS-CoV-2 infection was diagnosed 44, 28 and 48 days after the second dose of BNT162b2 and confirmed with the polymerase-chain-reaction (PCR) test. Two asymptomatic patients underwent this test because of their direct contact with a person with confirmed COVID-19. The third patient reported only a non-significant drop in oxygen saturation, and was hospitalized (case 3). All these patients were characterized by a low post-vaccination neutralizing antibody titer and a high production of these antibodies after falling ill (795, 845 and 5770). Perhaps this production of antibodies is responsible for the mild course of the disease, and the likely reduction of mortality. These breakthrough cases in no way undermine the importance of the vaccinations, and on the contrary argue for their urgency.
