Opis:
Breast cancer is the most frequently occurring cancer in women worldwide with more than one million new cases diagnosed each year. The objective of this study was to develop a Trastuzumab-dendrimer-fluorine drug delivery system by covalent attachment of Trastuzumab to a fluorinated PAMAM-G5 dendrimer. The Trastuzumab-dendrimer-fluorine drug delivery system was used to treat MCF-7 with Her-2 overexpression. The use of PAMAM-G5, which bears 128 primary amine surface groups, enables covalent attachment of both antibody and fluorinated functional groups for enhancement of cellular uptake. Thus, Trastuzumab was covalently attached to fluorinated PAMAM-G5 dendrimers and used as a vehicle for drug delivery to three dimensional (3D) cultured cells. The efficiency of Trastuzumab-dendrimer-fluorine drug delivery system binding to Her-2 receptors was measured by cell viability. The Trastuzumab-dendrimer-fluorine drug delivery system was found to have a higher efficiency in the treatment of Her-2 overexpressing MCF-7 cells than Trastuzumab alone. The incorporation of 19F by addition of heptafluorobutyric acid anhydride (HFAA) to PAMAM-G5 increased lipophilicity and hydrophobicity of drug delivery system.

Opis:
W niniejszym artykule autorzy podjęli próbę oceny przydatności systemów rezonansu magnetycznego o sile 1,5 Tesla do obrazowania małych struktur, jakimi są hodowle komórkowe.
In this article, the authors attempt to assess the usefulness of 1,5 Tesla magnetic resonance imaging systems for imaging of small structures such as cell cultures. The scope of imaging that has become the subject of research enters the area that is the domain of magnetic resonance microscopy (MRM). Nowadays, these tests are performed using high-field systems providing much higher spatial resolutions. The purpose of the work was to determine the conditions as well as scanning parameters that allow imaging of small objects using the OPTIMA MR360 production system. GE Healthcare. As a result of the work, it became necessary to develop an additional solenoid coil, which both dimensions, shape and design ensured optimal imaging quality possible based on the above MR system. This article is the first in a series of works showing the study of cell cultures using magnetic resonance imaging at the University of Rzeszów.

Opis:
Odkryte w połowie XX wieku zjawisko rezonansu magnetycznego stanowi do dnia dzisiejszego potężne narzędzie w rękach badaczy.

Imaging and quantification of MAbs distribution by MRI after administration in vivo can improve patient outcomes, although it is not routinely used in the clinic hence the motivation for development of new MAbs imaging methodologies. Improvements in imaging and quantification of antibody distributions can be made by conjugating MAbs to “hotspot” atoms such as 19F as 19F Currently, research investigating the incorporation of 19F to antibody delivery systems has been limited to trastuzumab perfluorocarbon emulsions and this method has not been extended to other clinically approved antibodies. The methodologies of 19F MRI can improve current limitations of antibody immunotherapy such as quantitative visualization of targeted surface antigens expressed on tumor cells.

Opis:
Zapotrzebowanie na jakościowe i ilościowe analizy zawartości aminokwasów w mediach i różnych płynach fizjologicznych i poznanie składu mieszanin aminokwasów białek spowodowało rozwój wielu technik spektroskopowych, m.in. MRS.
Magnetic Resonance Spectroscopy (MRS), despite its seventy years of history, is a field undergoing constant and uninterrupted development. This involves the possibility of comprehensive use of MRS measurements both experimentally for basic research in the field of biochemistry and physics (determination of certain kinetic and thermodynamic parameters) as well as clinically in medical practice. Cell cultures were chosen as the object of research and the results of spectroscopic examination in cancer cell cultures are presented in this study. The aim of the work was to optimize spectroscopic measurement in cell cultures on a medium-field nuclear magnetic resonance system. The need for qualitative and quantitative analysis of the content of amino acids in the media and various physiological fluids and knowledge of the composition of amino acid mixtures of proteins has resulted in the development of many spectroscopic techniques, among others MRS.

This paper shows one of the selected sources of errors that significantly increase the uncertainty of the result of determining the time T1. The authors focused on the influence of the center in which the studied area is located on the value of the relaxation time. It was shown on the example of a phantom how the value of the measured parameter can change depending on the properties of the medium components. The assumption of each tomography that allows imaging of any objects with finite spatial resolution is that the pixel has the same density or chemical composition on its entire surface. In the vast majority of cases, it is a far-reaching simplification which, omitted during the analysis, may lead to very serious errors in the interpretation of the results of the diagnostics or, more generally, tests.

The most common and most widely used examination methods for screening for and detecting breast cancer is presented herein. Material and methods. This review was performed according to systematic literature search of three major bibliographic databases. Results. Available data suggest that incidence and mortality in high-resource countries has been declining whereas incidence and mortality in low-resource countries has been increasing. Conclusion. The role of a physician is to select the most suitable one for each patient in order to obtain the best result. No matter the method however, between 2005 and 2011, the 5-year relative survival was found to be 89%. This is thought to be due to both the increase in utilization of population-wide screening, as well as advances in treatment.

Opis:
Functional Magnetic Resonance Imaging (fMRI) is a non-invasive research technique that gives insight into the processes in the patient’s brain. With its help, both the places responsible for motor skills and cognitive functions are assessed. This research method based on the magnetic properties of oxyhemoglobin and deoxyhemoglobin ensures identification of brain centers in a way that has not been possible so far. FRMI use is important not only in the cognitive process, but also in the treatment process, in particular in the process of preparing patients for surgery. FRMI belongs to the diagnostics particularly demanding cooperation on the part of the patient, because he most often has to carry out tasks imposed by the research staff. It is the proper design of these tasks and the time schedule of their implementation and evaluation of results that determine the correct diagnosis. The purpose of this work is to present selected paradigms used in FMRI.

Magnetic resonance imaging (MRI) has modified radiology in the field of brain research. This technique is based on the safe interaction between radio waves of a certain frequency and hydrogen nuclei in the human body or other object being tested. Particular progress has been observed in the quantitative and qualitative studies of metabolites which by definition are not disorders of the central nervous system, and which are not caused by structural abnormalities and therefore occur in small amounts enodogenously. The paper presents the view on the current use of MRI in brain research.

The three dimensional tumor environment can be mimicked with the use of cells cultured within a hollow-fiber bioreactor (HFBR). In this work, magnetic resonance imaging (MRI) was used to monitor changes in relaxation time in breast cancer cells following treatment with Trastuzumab in a HFBR system. Breast cancer cells were inoculated into the HFBR system and cultured over a period of 4 weeks. Relaxation time maps were generated according to MRI techniques in three dimensional (3D) cultures of MCF7/Her2 breast cancer and MCF7/Neo4 control cells. MRI measurements showed a variation in values of spin-lattice (T1) and spin-spin (T2) relaxation times related to cell density. Differences in both values (T1 and T2) were noted between untreated cells and cells treated with trastuzumab in the HFBR device. Additionally, 1H MRI was able to provide information about drug penetration in breast cancer cell culture organized in 3D. In conclusion, MRI in vitro can provide direct, noninvasive visualization of cell density in 3D geometry and cell viability as a function of drug uptake. Both T1 and T2 values were higher for lower cell density and accordingly both values, T1 and T2, were lower for higher cell density.

Introduction. The fraction of exhaled nitric oxide (FeNO) is used as a non-invasive biomarker that reflects inflammation in the airways. It is so versatile that it used to control asthma severity as well as to monitor response to treatment. However, the exact cut-off point of the nitric oxide level which allows one to make a precise diagnosis of asthma is unclear. Aim. To examine the possibility of using advanced statistical methods such as receiver operating characteristic for the analysis of FeNO concentrations for improving the diagnosis of asthma. Materials and methods. Receiver operating characteristic (ROC) was used for analyzing results to determine levels of nitric oxide which may be a prognostic indicator of asthma. The studied group consisted of 111 children including 69 asthmatic patients, and 42 age- and sex-matched healthy subjects. Measurement of exhaled nitric oxide was conducted in all subjects included in this study. Results. FeNO level was higher in asthmatic patients. The analysis of results showed that the cut-off point for the FeNO concentration is 11.5 ppb. Sensitivity and specificity with the FeNO level allowed us to determine a value of the diagnostic variable of FeNO concentration of 14.0 ppb. A comparison of FeNO level and sex of the subjects showed there is no correlation between these parameters of patients. Conclusions. Currently, the FeNO measurement provides complementary d

A wide spectrum of diagnostic methods are available in the diagnosis of nervous system diseases. Some are commonly performed, others only in specialist centers. In the study below, we discussed imaging tests used to diagnose neurological diseases. The broadly discussed diagnostics in the neurological context allow to highlight the needs that exist in this regard from the point of view of the neurologist. The values of various studies for various neurological diseases are also shown. This work cannot exhaust the topic of diagnostic imaging through the eyes of a neurologist due to the extent of the subject. Its assumption is to review the available imaging methods and show their value in treating patients.

An in vitro study was performed to determine cellular responses to fluorinated Trastuzumab conjugates by Magnetic Resonance Imaging (MRI). Breast cancer cells were cultured in 3D geometry. Proton (1H) MRI and Fluorine-19 (19F) MRI were used for visualization of cellular locations within a Hollow Fiber Bioreactor (HFBR) device and to monitor the cellular response to treatment. The results of this study confirm that cell growth is significantly decreased following treatment with Trastuzumab conjugates. The use of fluorinated Trastuzumab conjugates decreases breast cancer cell growth in 3D cultures and allows for tracking of drug delivery to cancer cells via 19F.

Magnetic Resonance Imaging functional Magnetic Resonance Imaging
Opis:
Magnetic Nuclear Resonance (MRI) is a non-invasive tissue imaging method. This technique is based on the influence of a strong magnetic field and electromagnetic wave of strictly defined frequency on the nucleus of elements with non-zero spin. The study describes one of the variants of functional MRI, (fMRI), which has become a key technique in brain imaging. This technique has excellent spatial and temporal resolution and involves a changing signal intensity depending on the degree of oxygenation of the blood. Blood oxygenation levels are known to vary in accordance with neural activity and these differences can be used to detect brain activity. This is due to increased demand for energy and oxygen in the area of increased neural activity. The basis of this imaging is the so-called Blood Oxygenation-Level Dependent (BLOD) effect. The aim of this paper is to present the scope of fMRI as a diagnostic method in neurology and in neurosurgery. This paper presents the principles of fMRI, methods of application, research result development, and suggests areas of possible medical applications. The limitations of fMRI as a clinical tool in medical applications will also be addressed. Studies presented in this paper are based on clinical fMRI experience and a literature review.

Magnetic resonance imaging is an essential tool in the research of today’s neurology. This study in a completely non-invasive way allows you to answer a number of questions related to both morphology as well as changes that occur in the CNS. This work presents a case report of sarcoidosis as a disease for which magnetic resonance imaging is the method of choice. This work aims to show resonance as a comprehensive diagnostic method in neurology. Sarcoidosis – a chronic disease of the immune system. At present, its etiology is not sufficiently explained. The possibility of occurring in almost all organs of the body and the need to differentiate with other disease entities makes this entity difficult to diagnose. This work presents sarcoidosis in magnetic resonance imaging (MR). The case report shows both clinical view and diagnostic difficulties.

Introduction. There is a need to develop a new imaging technique in medicine. Gastroenterology is the branch of medicine focused on the digestive system and its disorders therefore for this branch is needed to detect all problems affecting the gastrointestinal tract. Aim. The aim of this article is to complete discuss the possibility of the new fluorescent imaging technics in gastrology to use innovative screening to identify individuals at an early stage. Material and methods. We discuss here imaging techniques such as include x-rays, computed tomography, scans, and magnetic resonance imaging in gastrology. Spectroscopy is the study of the formation and interpretation of spectra resulting from the interaction of all types of radiation on matter understood as a community of atoms and molecules. Various spectroscopic techniques are obtained by combining different types of radiation with different ways of its interaction with the test sample. They provide the opportunity to obtain detailed information about the tested substance – from its atomic composition, through its chemical structure, to its surface structure. Analysis of the literatue. The tissue fluorescence spectrum can be obtained by: (1) autofluorescence, or natural or primary fluorescence, i.e. by direct irradiation of the tissue with laser radiation (laser-induced fluorescence – LIF) and (2) photodynamic diagnosis (PDD), where spectrum analysis is preceded by systemic or local administration of the photosensitizer. Conclusion. The use of fluorescence imaging in colon cancer patient has potential to improve quality of treatment and diagnosis.

Opis:
Currently, liver disease is widespread and the awareness of these diseases is low. Early symptoms of liver disease do not necessarily indicate problems with this organ and patients are usually informed of their problems when the stage of the disease is already advanced. Invasive biopsies are the clinical diagnostic method most commonly used in the evaluation of liver disease. A biopsy is associated with a high risk of false results and additional complications. Finding new non-invasive imaging methods has led to the discovery of a new method called Magnetic Resonance Elastography (MRE). This technique allows one to evaluate the mechanical properties of tissues and to distinguish between pathological states. Testing using this technique can be performed on a conventional magnetic resonance system by using few additional components and properly prepared software. Studies have shown that there is a strong correlation between MRE-measured liver stiffness and the degree of fibrosis. MRE is also useful in characterizing liver tumors. Studies show that this technique is highly credible in both health volunteers and patients with liver fibrosis. MRE has tremendous diagnostic potential. The described technique is not currently widely used and has the 127-132potential to serve as a safe and accurate alternative in clinical diagnostics in the future.

Introduction. The proper negative margins (R0) breadth in the breast - conserving surgery for invasive breast cancer (IBC) and ductal carcinoma in situ (DCIS) is very important. The presence positive surgical margins (R1) is associated with the necessity of reoperation. It delays the adjuvant therapy and psychologically burdens the patient. The re-operation increases the costs of treatment. The introduction of mammography (MMG) increased detection of DCIS by 20%. With the increase in malignancy, cancer detection decreases in MMG, inversely in MRI groving. Effective preoperative and intraoperative diagnosis aims to reduce the number of R1 resections. Aim. The size of the tumor next to its biology, determines the clinical course of the tumor. The accurate analysis of imaging tests is important. Material and methods. This analysis was performed using a systematic literature search. Results. Adequate surgical margins in breast cancer surgery for breast cancer have been reviewed. It is important to know if the cancer is multifocal and what the extent of the tumor is. Conclusion. The adequacy of margins is important for adjusting the volume of excision. It is avoiding unnecessary resection of healthy breast tissue. It is essential for a good cosmetic result and the local recurrence rate. The combination of breast MRI with conventional breast imaging resulted in the lover rate of the R1 resectios and the lower rate of the re-operation.

Opis:
Currently, new and easier ways of analyzing pharmaceutical drug forms and drug delivery mechanisms are being sought. Magnetic resonance imaging (MRI) is a non-invasive imaging technique that images drug forms such as tablets, liquids and topicals and drug form behavior in living organisms on both the tissue and cellular scale. The advantages of MRI include noninvasiveness, variable sample capacity and ease of transfer of phantom results to in vitro and in vivo studies. This review concerns the usefulness of clinical MRI that cannot be understated as this technique provides non-invasive and non-destructive insight into the properties of drug delivery systems. The research discussed here concerns the use of magnetic resonance, spectroscopy and chromatography to investigate selected pharmaceuticals and covers work of selecting drugs and antibodies for modification by synthesis for evaluation by MRI. Modifications have been aimed at improving therapeutic efficacy, delivery, and MRI. Modification conditions such as (pH, concentration, temperature, and the influence of other components present in the solutions) will be discussed to understand drug delivery system improvements and the reliability and repeatability of the results obtained. We hope to explore and expand the scope of pharmaceutical imaging with MRI for application in clinical medicine.

Opis:
Over the past few decades, magnetic resonance imaging (MRI) has proven to be extremely successful in medical applications. More recently, the biomedical applications of MRI have been gaining more use in the field of clinical pharmacy. In 1977, perfluorocarbon compounds (PFC), which form emulsions that can carry drugs, were analyzed by 19F MRI and emulsified PFC compounds have been investigated as potential blood substitutes since the early 1960s and now a wide variety of PFC compounds are currently available as 19F MRI biomarkers. Molecules with 19F substituents are particularly attractive for use in drug tracking by 19F MRI due to 100% 19F abundance, high 19F MRI sensitivity (0.83 relative to 1H MRI) and an impressively large chemical shift range (400 ppm). Another benefit in the use of 19F MRI is a zero background signal in biological samples due to lack of endogenous fluorine. Therefore, drugs containing fluorine atom have potential for 19F MRI imaging drug delivery to tissue. This article will review recent developments in the use of 19F MRI in imaging drug delivery to tissue and individual cells.

Introduction. Stroke is a common vascular disease encountered in the work of a practitioner. Despite this, his differentiation is difficult. This is due to a variety of diseases that can be a “mask” for stroke. These are neurological diseases other than vascular (brain tumors, epilepsy), head injuries and a number of metabolic disorders (fluctuations in glucose, electrolytes). Aim. This work aims to approximate the differentiation of this common disease entity. Case reports show both clinical view and diagnostic difficulties. Description of the cases. Case 1. The patient 59 years old treated for bipolar disorder, hypertension and alcohol abuse. Case 2. The patient 68 years old treated for type 2 diabetes, hypertension, ischemic heart disease, cholecystolithiasis – cholecystectomy. Conclusion. Stroke is a significant clinical and social problem both in Poland and in the world. Despite this, its diagnostics and especially differentiation is difficult. When analyzing the case of each patient with suspected sudden CNS vascular disease, all of them should be remembered. It should also be borne in mind that differential diagnosis cannot delay specific treatment for stroke

In the Podkarpackie voivodship up until 2014, the incidence rate of female breast cancer was rising and in 2015 it dropped by 68 cases. Even though the survival rate is increasing, it is still one of the leading causes of cancer deaths placing second after lung cancer. Conclusion. Mortality rate, unfortunately, rose gradually. This shows that even though there is a National Health Program aiming at breast cancer awareness and prevention, it is still not enough to reduce the number of breast cancer deaths.

Continuous scientific research and the increasing saturation of the medical market in Poland implements the possibilities of using advanced MR techniques including MRS in everyday practice. This method, which has so far been used primarily for research purposes, can bring measurable benefits to patients not only in terms of clarifying diagnosis and narrowing differential diagnosis, but also monitoring the course of various diseases and their treatment. Here we present the basic principles of performing and interpreting spectroscopic spectra and possible clinical applications and development prospects of MRS. The literature reviewed both Polish and foreign articles both historically and in the past 10 years. The paper presents methodological issues related to the proper performance of magnetic resonance spectroscopy (MRS) and spectral composition and the role of major metabolites, as well as current clinical applications and directions of MRS development.

Tobacco use is a leading cause of preventable mortality worldwide. New cost-effective smoking cessation treatments are needed especially in some low-to-middle income countries where smoking rates are rising, and current pharmacotherapy treatments remain cost-prohibitive. Since the 1960’s, cytisine has been used as an effective nicotine substitution agent to aid in smoking cessation albeit limited to a selected few Eastern/Central Europe and Central Asian countries. Cytisine is a biologically active alkaloid of plant origin and is known to be a ligand of nicotinic acetylcholinergic receptors (nAChRs). For several decades, the properties of cytisine have been investigated and reported in the biomedical and pharmaceutical literature. Due to the beneficial impact of cytisine on smoking cessation and its costly multistep synthesis, there is a growing interest in extraction from natural sources as well as in analytical identification and quantification for clinical medicine and forensic toxicology. In this paper, we present several current analytical approaches to cytisine extraction and identification from biological samples of plant and human origin. The development of extraction techniques will allow for the widespread use of the drug in experimental and clinical pharmacology, toxicology and forensic medicine.

Despite continuous scientific and technological advances in MR imaging, MR perfusion methods have not yet been widely deployed for routine clinical diagnostics. This is especially true for ASL (arterial spin labelling) methods used to evaluate cerebral perfusion. This method does not require a contrast agent, as new discoveries about gadolinium accumulation in the cerebellum and brain nucleus appear to be a valuable asset and provide the opportunity to be more widely deployed in clinical practice. The aim of this paper is to present the historical determinants of the development of MR perfusion techniques, the disadvantages and advantages and possible clinical applications and prospects of ASL development. Both historical articles published on MR in the 1990s and current research between 2006-2016 have been reviewed. The authors present in the work the MR perfusion method focusing on issues related to arterial spin labeling (ASL). Historically CASL (continuous ASL) and PCSL (pulsed ASL) techniques have been described and the pseudocontinuous ASL (pseudocontinuous ASL) 3D technique presents its technical and methodological considerations, advantages and disadvantages over previous methods. The methods of test protocol optimization and accompanying artifacts, as well as possible clinical applications and development perspectives, have been described.
