Opis:
Slipped capital femoral epiphysis in adolescents (SCFE) is a well known disease occurring mostly in the age between 9 and 16. Despite of proper diagnosis and treatment, it may cause permanent deformity of a hip joint and limitation in everyday life. The goal of this study is to evaluate the influence of underwent SCFE on young adults’ professional and everyday activity. Patients with SCFE treated surgically by stabilization with Kirchner wires were included in this study. There were inclusion criteria: answer to functional questionnaire and full pre- and post operative radiologic data. In the years 1985-2000 in our hospital 35 patients were operated on for SCFE, 13 of them responded to a questionnaire. There were 6 females and 7 males in the evaluated group, with mean age of 28.4 years. Mean preoperative slip angle in AP view was 10.9°, in axial view 27,5°. Patients underwent surgical stabilization with K-wires bundle in mean age of 13.4. The mean follow up was 14.9 years. Mean follow up slip angle in AP view was 7,6°, in axial view 22,1°. Among respondents, 6 patients completed high school, 7 – vocational education. All of them were satisfied with the underwent treatment. None of the patients was additionally operated on during the follow up period. Only 3 patients were complaining of hip pain, but none of them used analgetics. Limp occurred in 3 patients. Disease of the hip joint followed by surgery did not have any influence on patients’ present social, professional or sport activity in 11 cases, while 2 patients noted a small influence. In the field of physical activity, 8 patients play sport. The study did not show any correlation between the slip angle in both views and an appearance of any symptoms or functional restriction. In spite of a limited statistical value of our research, caused by a low rate of return of the questionnaire, the study shows that SCFE treated surgically does not result in early disablility or restrictions of daily activity.

Wstęp. Dzieci niepełnosprawne, podobnie jak ich pełnosprawni rówieśnicy, powinny mieć możliwość posiadania hobby. Poprzez niestandardowe formy wspomagania terapii można, realizując cele rehabilitacyjne, kształtować u dziecka zainteresowania, rozwijać pasje. Głównym celem poznawczym badań była ocena skuteczności zajęć dogoterapii jako metody wspomagania terapii dzieci z zespołem Downa. Materiał i metody. Badaniami objęto 36 dzieci z zespołem Downa. Średni wiek badanych to 11,2 lat. Ponadto 19 z nich uczęszczało na zajęcia dogoterapii w ramach wspomagania terapii. Większość dzieci charakteryzowała się umiarkowanym (n=19) lub znacznym (n=11) stopniem niepełnosprawności. Zastosowano metodę sondażu diagnostycznego. Narzędziem badawczym była autorska ankieta wypełniana przez rodziców dzieci z niepełnosprawnością intelektualną. Ankieta składała się z 21 pytań. Dotyczyły one oceny sprawności fizycznej dziecka, zmian w zakresie lokomocji, samoobsługi oraz koordynacji wzrokowo-ruchowej od momentu rozpoczęcia zajęć. Wyniki. Większość dzieci uczestniczących w zajęciach dogoterapii poczyniło znaczące postępy w zakresie komunikowania się z otoczeniem (95%) i pewności siebie (89%). Zaobserwowano znaczną poprawę samodzielności, trybu życia oraz uspołecznienia. W stosunku do grupy porównawczej różnice były istotne statystycznie (p<0,001). Wyraźnych różnic pomiędzy grupami nie stwierdzono w zakresie sprawności fizycznej. Wnioski. 1. Zajęcia dogoterapii dla dzieci z niepełnosprawnością intelektualną są formą terapeutyczną dotąd niedostatecznie poznaną, jednak wyniki osiągnięte przez dzieci z grupy badanej wskazują na jej wysoką skuteczność. 2. Zachodzi potrzeba poszukiwania i ewaluacji innych alternatywnych form wspomagania terapii dzieci niepełnosprawnych.
Introduction. Similarly to their healthy peers, handicapped children should have the possibility of fulfilling their hobbies. Apart from rehabilitating, non-standard forms of therapy help shape the child’s interests, develop the child’s passions. The main aim of this study was to evaluate the effectiveness of therapy dog sessions as a supplementary therapeutic method dedicated to children with Down Syndrome. Material and methods. The research was carried out on a group of 36 children with Down Syndrome. Their average age was 11.2. 19 of them attended dog therapy sessions conducted as part of their therapies. The majority of children were characterized by moderate (n=19) and severe (n=11) mental retardation. The diagnostic poll method was applied in the research. The research tool was an authorial survey filled in by the parents of children suffering from mental retardation. The survey comprised of 21 questions concerning the child’s physical fitness, locomotion lesions, self-care and visual-motor coordination from the beginning of the classes. Results. Majority of children attending dog therapy sessions made considerable progress in their ability to communicate with the environment (95%) and self-confidence (89%). Improvement was also noted in the area of lifestyle and sociality. Differences between the judo group and the control group were statistically significant (p<0.001). However, distinguishable differences were not recorded in the area of physical fitness. Conclusions. 1. The therapeutic value of dog therapy dedicated to children with mental retardation has not been recognized yet. However, the results achieved by a group of patients attending them clearly indicate their high effectiveness. 2. There is the need to look for and evaluate new form of supporting therapy children with mental retardation.

Cel pracy: Celem badań była ocena wpływu ćwiczeń relaksacyjnych i stabilizacyjnych na dolegliwości bólowe lędźwiowo- -krzyżowego odcinka kręgosłupa u kobiet w zaawansowanej ciąży. Materiał i metody: W badaniach wzięły udział 54 kobiety ciężarne (średnio 28,9 tydzień ciąży). Średnia wieku wynosiła 29,2 lat. Osoby badane podzielono na dwie grupy. Kobiety z Grupy 1 (n=25) wykonywały serie ćwiczeń relaksacyjnych i stabilizacyjnych (po wcześniejszym instruktażu podczas zajęć szkoły rodzenia). Osoby z Grupy 2 (n=29) nie zmieniły dotychczasowego stylu życia. Warunkiem zakwalifikowania do badań było występowanie dolegliwości bólowych L-S odcinka kręgosłupa. Narzędzia badawcze stanowiły: Wizualno- Analogowa Skala Bólu (VAS), Kwestionariusz Rolanda- -Morrisa, Kwestionariusz Spielbergera oraz autorska ankieta zawierająca pytania dotyczące wybranych elementów stylu i jakości życia. Wyniki: W Grupie 1 po czternastu dniach znacząco obniżyła się (p = 0,021) intensywność bólu. Nie zaobserwowano jednak istotnej różnicy w poziomie bólu po jednej sesji ćwiczeń (między pomiarem 1 i 2). W Grupie kontrolnej nasilenie bólu utrzymywało się na podobnym poziomie, różnice między pomiarami nie były istotne statystycznie. Regularne ćwiczenia w domu wpłynęły również na redukcję ograniczeń funkcjonalnych (mierzonych skalą Rollanda-Morrisa) u badanych kobiet (p = 0,044). Kobiety z Grupy 2 zgłosiły znaczne zwiększenie poczucia niepełnosprawności (p = 0,014). Wnioski: Samodzielne ćwiczenia relaksacyjne i stabilizacyjne wpływają na redukcję bólu dolnej części pleców u kobiet ciężarnych. Zasadne jest zatem stosowanie różnych form kinezyterapii w ramach profilaktyki i leczenia tego typu dolegliwości. Na skutek ćwiczeń zmniejszył się stopień ograniczenia funkcjonalnego badanych kobiet. Dowodzi to pozytywnego wpływu samodzielnych ćwiczeń w domu na sprawność fizyczną.
The aim of the work: The aim of the research was the evaluation of the influence of relaxation and stabilization exercises on low back pain at women in the advanced pregnancy. Material and methods: There were 54 pregnant women (averagely in 28.9 week of the pregnancy) who partook in the research. The average age was 29.2 years. The studied persons were divided into two groups. The women from the Group 1 (n=25) performed series of loosening and stabilizing exercises (after previous instruction at childbirth classes). The persons from the Group 2 (n=29) did not change the previously existing lifestyle. The condition of classifying to the research was the occurrence of pain complaints about L-S spinal section. The investigative tools were: Visual Analog Scale of Pain (VAS), the Questionnaire by Roland-Morris, the State-Trait Anxiety Inventory by Spielberger and the author’s questionnaire containing questions concerning of chosen elements of style and quality of life. Results: In the Group 1 after fourteen days the intensity of the pain dropped significantly (p = 0.021). There were not observed however the significant difference in the level of pain after one session of exercises (between the measurement 1 and 2). In the control group the intensification of the pain remained on the similar level, differences between measurements were not statistically essential. Regular exercises at home influenced also the reduction of functional limitations at the examined women (p = 0.044) (measured with the Rolland-Morris’ scale). Women from the Group 2 reported considerable increase of the feeling of being handicapped (p = 0.014). Conclusions: Individual relaxation and stabilization exercises bear on the reduction of pain of the lower back at pregnant women. Therefore, it is legitimate to use various forms of the kinesiotherapy as prophylaxis and treatment of this type of complaints. As the result of the exercises, the degree of functional limitation of the examined women was diminished. It proves the positive influence of individual exercises at home on physical fitness.

Introduction. The high requirements in terms of physical fitness of hockey players may be a factor predisposing to injuries. The purpose of the study was to determine the functional limitations of the locomotor system of children practicing ice hockey. Materials and met hods. 104 children took part in the study, including 16 girls and 88 boys, divided into two groups. The first group consisted of children practicing hockey (n=38). The second group consisted of children who do not practice hockey (n=66). The research tool was the FMS test consisting of seven movement activities graded on a 0 - 3 scale. The Mann - Whitney U test was used to evaluate the differences between particular groups, and the Wilcoxon’s test was used to evaluate the differences between the sides. The rela tionships between the variables were established based on the rho Spearmann correlation. The minimal statistical significance was set at p ≤ 0.05. Results. The boys practicing hockey obtained a significantly higher total result than the boys who are not hock ey players (p=0.008). The girls practicing hockey obtained a result close to their peers who do not play hockey. A significant positive correlation was observed between the age of the players and the result obtained in the FMS test in the group of hockey p layers (r=0.77; p<0.001), and between the training experience and the result of the test (r=0.49; p<0.01). Conclusion. The players obtained a significantly statistically higher result in the FMS test, which may indicate a higher level of functional fitness , resulting from a rational training and the acceleration of motor development.

The aim of this work was to estimate the effectiveness of kinesiotaping (KT) in the reduction of delayed-onset muscle soreness (DOMS) of the quadriceps femoris. 41 young women were examined. In the study, two groups applied kinesiotaping - the first with a relaxing application (Group 1); and the second with a lymphatic one (Group 2). Women from the control group used no treatment to help in the exertion restitution. All of the women performed a vertical jump test and training of their lower limbs (5 series of squat jumps). The research tools were the VAS scale (for the estimation of pain) and the Borg Scale (for a subjective estimation of the intensity of effort). The measurements (the vertical jump test and the intensification of the pain) were repeated 24, 48, 72 and 96 hours after the training where the exertion occurred. In all of the groups, the greatest regress of the vertical jump test was observed between the first and the second measurement - and this difference was statistically essential (p<0.01). The least fall in power was noticed in the women who had received the lymphatic KT application; while the greatest fall in results of vertical jump test was observed in the persons with the KT relaxing application. The greatest level of pain in Groups 1 and 2 was noted during the second measurement, which was 24 hours after the training; whereas in the control group, the greatest pain was observed in the third measurement. It can be concluded that the kinesiotaping has a bearing on the acceleration of the regeneration and increased the efficiency of the examined muscles.

The purpose of this study was to determine if physical activity of different intensity has an effect on reduction of delayed onset muscle soreness. Eighty women divided into two groups participated in the study. Subjects from Group 1 participated in the training of upper libs, whereas subjects from group 2 – in the training of lower limbs. Tests of power of upper limbs – the active overhang on the bar (Gr. 1) and power of lower limbs – the vertical jump test (Gr. 2), visual analog scale of pain (VAS) and the International Physical Activity Questionnaire (IPAQ) were used as investigative tools. The majority of studied women were characterized by the high level of physical activity. Physical activity level had strong positive correlation (r = 0.54; p < 0.001) with the reduction of pain. Taking into account the partition according to the kind of physical activity, there was observed some significant correlation between the diminution of delayed onset muscle soreness and the level of moderate activity and walking (relocating). Physical activity level positively correlated with the reduction of delayed onset muscle soreness. Moderate physical activity and walking proved to be the most beneficial from the reduction of delayed onset muscle soreness point of view.

Objectives of the research: The goal of the research was to identify the type and extent of activities that special education facilities engage in to teach and promote independence in people with profound disorders of intellectual development. Research methods: Empirical material was collected using the author’s original online questionnaire, which consisted of 30 questions measuring the extent to which students’ needs for developing independent living skills are met. The survey was conducted among special education teachers (N=642) from seven European countries (Portugal, Spain, Belgium, Poland, Bulgaria, Romania, and Turkey). A short description of the context of the issue: Independent living is widely seen as an immanent attribute of adulthood. Children and adolescents are prepared for independent living through the process of upbringing and socialization; people with disabilities are additionally prepared through rehabilitation. For people with a profound disorder of intellectual development, independent living is challenging but possible with the right support. Research findings: The results indicate that in the course of developing independent living skills in people with profound disorders of intellectual development, efforts are mainly made to improve their self-management skills and democratic management methods in both team work and individual work. Conclusions and recommendations: In the surveyed countries, there is a tendency to limit work for developing independent living skills to a single type of intervention. Rarely are two or three methods – not to mention four – simultaneously used to stimulate the development of independent living skills. It is therefore necessary to call for increased efforts to promote independent living among people with profound disorders of intellectual development and to focus this support on autonomy, self-determination, achieving quality of life goals, and, above all, the ability to make decisions – since this is what determines true independence in life.

The paper presents general features of the climate and bioclimate of Uniejów assessed from the point of view of the possibility to apply them to climate therapy. The authors have used multiannual data from meteorological stations located in the vicinity of Uniejów. It was found that the climate and bioclimate of the city are suitable for climate therapy. The most useful is the period from the beginning of May to the middle of October when the majority of patients can practice one of several forms of climate therapy (aerotherapy, heliotherapy, terrain therapy).

Introduction: Nosocomial (also known as hospital-acquired) infections are understood as infections which develop during patient’s stay in hospital, but are not the direct cause of hospitalisation. The aim of the study: The purpose of the paper was to determine the knowledge of the nursing personnel from Świętokrzyskie Voivodeship onnosocomial infections, prevention and methods of combating them. Other addressed issues included the analysis of the work yeas and education level as related to the state of nurses' knowledge on nosocomial infections, and what are the reasons for insufficient prevention of diseases. Material and methods: The study included 208 nurses from Świętokrzyskie Voivodeship. Out of 208 questionnaires, 180 were completed correctly, the remaining 28 were answered incorrectly or incompletely, which resulted in their rejection. The statistical analysis was conducted using the Pearson Chi-squared method, with the significance level p>0.01 assumed to be insignificant. The study involved 124 women and 56 men. The youngest respondent was 23 years old and the oldest was 59 years old. There were 36 respondents below 30 years of age(20%), 88 in the 31-40 age group (48.9%), 56 in the 41-60 age group (31.1%). Results 1. The knowledge of the intermediate personnel about hospital-acquired infections is satisfactory. 2. Job seniority is not statistically significant with regard to the knowledge level on nosocomial infections. 3. The research revealeda statistically significant correlation between the knowledge level of the secondary staff and their education. 4. The study results show that the most common causes of insufficient knowledge on preventing hospital-acquired infections are staff shortages and inadequatefinancial resources.

Nasal glial heterotopia is a rare congenital defect that is formed during embryonic development. This lesion is a part of a larger group of diseases, congenital midline nasal tumors, that occur with a frequency of one per 20,000-40,000 live births. Histologically, nasal glial heterotopia is a concentration of glial tissue which exhibits no malignant potential. We can differentiate three basic types of the disease based on the location of the lesion: 60% of nasal gliomas are extra nasal, 30% are intranasal and only 10% combine extra- and intranasal components. Because of its rare occurrence, correct diagnosis is quite difficult. We present a case of a 2.5-month-old male who was admitted to the Department of Pediatric Otolaryngology for the diagnosis and treatment of a tumor deforming the bridge of his nose. Initial differential diagnosis included encephalocele, glial heterotopia (nasal glioma), angioma, nasal dorsal cyst, rhabdomyosarcoma, lacrimal duct cysts. We performed endoscopic examination, radiological imaging (CT, MRI) and histological assessment of the lesion. The patient was diagnosed with nasal glial heterotopia (extra-and intranasal) based on diagnostic studies and clinical manifestation. The resection of the lesion was performed by means of dual access, external and endoscopic. The results of surgical treatment are satisfactory, control endoscopy showed no presence of residual tumor.

Heterotopowa tkanka neurogenna w obrębie struktur nosa jest rzadką wadą wrodzoną, która powstaje podczas rozwoju embriologicznego. Zmiana ta stanowi część większej grupy chorób wrodzonych guzów linii środkowej nosa, które występują z częstotliwością raz na 20 000–40 000 żywych urodzeń. Histologicznie heterotopowa tkanka neurogenna jest skupiskiem tkanki glejowej, która nie wykazuje potencjału złośliwego. Można wyróżnić trzy podstawowe typy w zależności od lokalizacji zmiany: 60% to zmiany zewnątrznosowe, 30% wewnątrznosowe, a tylko 10% łączy cechy obu typów i lokalizuje się zarówno podskórnie, jak i w jamie nosa. Prawidłowa diagnoza jest dość trudna ze względu na rzadkie występowanie. Przedstawiamy przypadek 2,5 miesięcznego chłopca, który został przyjęty do Oddziału Otolaryngologii Dziecięcej w celu diagnostyki i leczenia guza deformującego grzbiet nosa. Wstępna diagnostyka różnicowa obejmowała przepuklinę mózgową, hetertropową tkankę neurogenną (tzw. glejak nosa), naczyniaka, torbiel grzbietu nosa, mięśniakomięsaka prążkowanokomórkowego, torbiel przewodu łzowego. Wykonano badanie endoskopowe, obrazowanie radiologiczne (CT, MRI) i histologiczną ocenę zmiany. W oparciu o wyniki badań diagnostycznych i obraz kliniczny u pacjenta zdiagnozowano heterotopową tkankę neurogenną w obrębie nosa (postać mieszana). Resekcję zmiany przeprowadzono za pomocą podwójnego dostępu, zewnętrznego i endoskopowego. Wyniki leczenia chirurgicznego są zadowalające, kontrola endoskopowa nie wykazała obecności guza resztkowego.
