In recent years, there has been a maximum increase in admissions to the intensive care unit, culminating in an exponential increase in admissions during the COVID 19 pandemic. Many patients who survived and were discharged from the intensive care unit have cognitive, physical, and psychological disorders that are reflected in the term post-intensive care syndrome. Patients and their families show symptoms of anxiety, depression, post-traumatic stress, and sleep problems. The result is that they negatively affect their quality of life. Numerous risk factors contribute to the development of this syndrome, mainly the sedation, the duration of mechanical ventilation, and the length of stay in the intensive care unit. For this reason, it is necessary to take measures to prevent this syndrome including ABCDEFGH care plan, physical rehabilitation, nutritional support, and intensive care unit diaries. Care must also be given to the creation of Post Intensive care unit clinics where they have a diagnostic, therapeutic, counseling, and rehabilitation role that will act as assistants in the care of patients after discharge from the intensive care unit. Also, special care should be taken with patients who have recovered from COVID 19 whose needs are increasing and need immediate treatment. This review aims to analyze post-intensive care syndrome, prevention measures, and the impact of COVID 19. In conclusion, it is necessary to take measures to treat post-intensive care unit syndrome with early diagnosis and treatment, to reduce the adverse effects on both patients and their families.

Intensive care units and well-qualified medical staff are indispensable for the proper functioning of every hospital facility. Due to demographic changes and technological progress having extended the average life expectancy, the number of patients hospitalized in intensive care units increases every year. Global shortages of nursing staff (including changes in their age structure) have triggered a debate on the working environment and workload the nursing staff are exposed to while performing their duties. This paper provides a critical review of selected methods for the measurement of the workload of intensive care nurses and points out their practical uses. The paper reviews Polish and foreign literature on workload and the measurement tools used to evaluate workload indicators.

Introduction: Corneal transplantation is a sight restoring procedure that enhances the recipient’s quality of life. Most deceased patients in Intensive Care Units (ICU) are potential eye donors, but the number of referrals from this group remains low. Understanding how nurses view eye donation and identifying their educational needs is vital in order to increase donation rates while offering choice to patients and families. Purpose: To assess nurses’ knowledge about the identification and referral of potential eye donors. Materials and methods: An online, 18-item, questionnaire survey assessing the knowledge, training and views towards eye donation was emailed to 100 ICU nurses working in a tertiary NHS Trust. Fifty-eight percent (n=58/100) of nurses completed the questionnaire. Results: The majority of nurses (n=50/58) could not identify the medical contraindications to eye donation, just one was aware of the referral process and only two reported that they received previous training on eye donation. Overall, participants were in agreement that corneal transplants improve the recipients’ quality of life (n= 50/58) and that offering eye donation should be considered as part of end-oflife care (n=43/58). However, only seven felt confident raising the subject with relatives. Conclusion: ICU nurses play a pivotal role identifying and discussing the potential for donation. However, the results of this survey showed that the majority of responding nurses lacked knowledge, training and confidence to effectively undertake this role. Therefore there is a need for educational programmes that focus on eye donation and communication skills to be able to offer the choice to patients and families. There is also a scope for research to evaluate the efficacy of educational programmes and their impact on donation rates

Introduction. Parental Stressor Scale: Neonatal Intensive Care Unit (PSS:NICU) by M. Miles et al. has been developed in order to assess the stress experienced by parents of infants being treated in Intensive Care Units. The measurement of parental stress enables the evaluation of nursing care effectiveness, as well as facilitating the determination of the level of progress made by parents in coping with the difficult situation they face. Objectives. The aims of the research include: (1) validation of the Parental Stressor Scale: Neonatal Intensive Care Unit Polish Version and (2) initial assessment of perceptions of parental stress in a group of 151 parents of infants treated in four NICUs in Poland. Materials and method. This quantitative cross-sectional study was performed among 151 parents (129 mothers and 22 fathers) of infants treated in four NICUs in central and eastern Poland. The respondents were asked to complete forms following the Parental Stressor Scale: Neonatal Intensive Care Unit, the Impact Event Scale – Revised (IES–R),and their demographics, which combined basic medical data along with socio-demographic data of both parents and children. Results. The three sub-scales distinguished on the basis of factor analysis (Infant Appearance, Parental Role Alteration, Sights and Sounds) explain in total 54.89% of variances. Cronbach’s alpha for the entire scale equals 0.92, while as follows for the particular sub-scales: Infant Appearance – 0.92; Parental Role Alteration – 0.86, and Sights and Sounds – 0.78. Conclusions. The Polish version of PSS:NICU is an accurate and reliable tool for the assessment of stress experienced by parents whose infants require treatment in NICUs.

INTRODUCTION: For many months now, the entire world experiences effects of the COVID-19 pandemic, and current data indicate that this condition will continue even longer. About 5% of patients who become infected with the SARSCoV-2 coronavirus may develop a critical form of severe respiratory failure, due to which they will undergo intubation and mechanical ventilation. Prolonged mechanical ventilation will be an indication for an open tracheostomy, burdened with an increased production of aerosol containing virus particles and an increased risk of transmitting the highly contagious pathogen to medical personnel. In this situation, surgical departments with a limited number of operating theaters may be dedicated only to COVID-19 patients. Their use for other procedures will be significantly reduced or even impossible to prevent transmission of infections and exposure to other people. Taking into account the well-being of patients and staff and limiting the transmission of the virus to other departments, other safe solutions are being sought to perform a tracheostomy. An alternative procedure may be to perform a tracheostomy directly at the bedside (point-of-care) of a patient hospitalized in an intensive care unit (ICU). PURPOSE: The purpose of the study is to present an effective performance of safe, open tracheostomy in the ICU. METHODS: The bedside tracheotomy as an alternative to the procedure performed in the setting of an operating theater has been presented. A case study was based on the analysis of the procedure performed in a 42-year-old patient who was installed an open tracheotomy tube on the twenty-ninth day of intubation. CONCLUSIONS: In order to limit the transmission of the SARS-CoV-2 coronavirus outside the intensive care unit, it is recommended to perform a tracheotomy in ICU, in patients requiring mechanical ventilation, as the method is effective and safe.

Introduction and aim. This study aimed to compare the difference in the number of microorganisms (microbial counts) between traditional and disposable bed baths. Material and methods. This study is quasi-experimental with two groups. The sample consisted of 30 respondents from the traditional bed bath and the disposable bed bath groups. Cultures of the groin were obtained to compare the number of microorganisms before and after bathing. Results. Bathing with disposable bed baths has proven to be more effective in reducing the number of microorganisms in both the control group and the intervention group on day I and day II with p=0.014 and p=0.033. Conclusion. Disposable bed baths are more effective in reducing the number of microorganisms on the skin than traditional bed baths.

Szpitalne zapalenie płuc jest najczęstszym rodzajem infekcji stwierdzanym w oddziałach intensywnej terapii (OIT), a wysokie odsetki śmiertelności i umieralności z jego powodu nakazują uznać to schorzenie za najbardziej poważne w środowisku szpitalnym. Z tego względu właściwe rozpoznanie i zrozumienie wpływu szpitalnego zapalenia płuc na stan chorych hospitalizowanych w OIT odrywa ważną rolę. Ryzyko zachorowania uzależnione jest od stanu ogólnego pacjenta, rodzaju biologicznego czynnika chorobotwórczego, inwazyjnych i związanych z immunosupresją procedur diagnostyczno-leczniczych. W związku z powyższym zaleca się wdrożenie wielokierunkowych działań, obejmujących systematyczne kształcenie personelu, eliminację czynników ryzyka oraz wprowadzanie rekomendacji opracowanych przez odpowiednie instytucje naukowe. W niniejszym artykule przedstawiono problematykę zapadalności, patogenezy, czynników ryzyka oraz metod zapobiegania szpitalnemu zapaleniu płuc w oddziałach intensywnej terapii.
Nosocomial pneumonia (NP) has been considered the most common infection in Intensive care units (ICUs) and due to its high morbidity and mortality rates it has been deemed the most serious hospital acquired infection. It is vital to recognize and understand the impact of nosocomial infections on ICU patients. The risk of NP is associated with patient’s condition, infectious agents, invasive and immunosuppressive procedures. A multiprong approach should include staf education, minimizing risk factors, and implementing guidelines established by national committees. The aim of this article is to discuss incidence, pathogenesis, risk factors and prevention of nosocomial pneumonia in intensive care units.

Background: Since 1950 intensive care has become a separate and independent specialty. Significant technological advances have allowed the Intensive Care Unit (ICU)s to be monitored through the centralized work of a multidisciplinary team of specialists. ICUs provide cares at different levels of support. To provide patient access to this highly specialized cares, the “Critical care without walls” or “Intensive Care without Borders” theories have emerged, involving reanimation nurses offering highly specialized care and support. Protocols for nurses have been developed, with the aim of facilitating their day-to-day activities, improving outcomes and safety of patients and all staff. Utilizing this concept, the role of intensive care has rapidly expanded over the past 20 years. Aim of the study: To understand the effectiveness and benefits derived from organisation of the working process. Material and methods: We used documentary and survey methods and analysed the data using the software package SPSS v. 21.0. Graphics were prepared using Microsoft Excel 2013. Numbers from 1 to 5 refer to the answers “no”, “not really”, “cannot judge”, “closer to yes” and “yes”, respectively. Results: Nurses’ performance, as an integral part of multidisciplinary ICU teams, is evidence that mortality and morbidity can be improved thanks to the early recognition of patient deterioration and rapid resuscitation. The better healing process is accomplished by optimising the content and evaluation of the desired results, in association with good doctor practices. Respondents’ opinions about outcomes of protocol use differed significantly between professions in regard to making their job easier, improving patient outcomes, providing consistency in care, and preventing patient harm. Conclusions: A more flexible and standard framework for nurses should be developed to improve quality of care. The rapidly growing lack of ICU nurses in Bulgaria is concerning.

Introduction and aim. In the COVID-19 pandemic, which is a global threat, the awareness levels of intensive care nurses who meet all the care needs of patients can affect the quality of care. In this study, pandemic awareness and care behaviors of nurses who undertook the patient’s care needs in intensive care units were examined. Material and methods. The research was carried out with 317 nurses working critical care units of 12 hospitals in different provinces. “Nurse Characteristics Form”, “Pandemic Awareness Scale” and “Caring Behaviors Inventory-24” were used as data collection tools. Data were collected between March and August 2022 in the middle of the COVID-19 pandemic process using an online survey. Results. It was determined that 75.4% of the participants were female and the mean age was 25.56±4.49. Pandemic awareness scale score was 3.04±0.62 (0.11-3.67) and caring behaviors inventory score was 5.48±0.84 (1.00-6.00). A significant, positive, weak relationship was found between nurses’ pandemic awareness and all sub-dimensions of caring behaviors (p≤ 0.05). Conclusion. It was seen that the caring behaviors of nurses with high pandemic awareness were also positively affected. Although it was the first time they had experienced the pandemic, nurses were found to have good caring behaviors.

Background: Craniocerebral injuries are one of the most common causes of mortality and disability in Poland. The treatment of patients who are in an intensive care unit is based primarily on stabilizing the patient’s general condition as well as basic duties according to the patient’s functioning. Aim of the study: The aim of this study is to demonstrate the importance of early rehabilitation and the role of physiotherapy in recovery after craniocerebral trauma. Case report: The subject was an 18-year-old patient who suffered craniocerebral trauma as a result of a road accident. After losing consciousness, he was in the intensive care unit, where he was placed on a medical ventilator. A properly selected physiotherapeutic procedure was performed. Passive exercises, contracture correction and appropriate positioning were used. To prevent pressure sores, anti-bedsore prophylaxis was implemented. Respiratory therapy played a key role. The goal of respiratory physiotherapy was to improve respiratory function by maintaining proper lung ventilation, increasing chest and diaphragm mobility along with maintaining the efficiency of respiratory muscles, as well as stimulating effective coughing and evacuation of secretions. The NDT-Bobath concept was used as therapy for spastic tension. The goal of the therapy was to get rid of pathological movement patterns and replace them with physiological patterns. The PNF method, classical and lymphatic massage, polysensory stimulation and music therapy were also used. Conclusions: Early and comprehensive rehabilitation in a patient after craniocerebral trauma is extremely important and determines therapeutic effectiveness. Comprehensive therapy and care are able to prevent a number of complications that threaten the patient as a result of immobilization.
