Aim: The aim of the study was to retrospectively assess the relationship between the occurrence of polyps and colon cancer in patients with type 2 diabetes. Methods: In 2014–2015, 976 colonoscopies were performed in patients. We compared the number of polyps with high-grade dysplasia and colorectal cancers in patients with and without diabetes. In addition, in the diabetic group we documented the relationship between HbA1C and the occurrence of polyps with high-grade dysplasia, and colon cancer. The data were statistically analyzed. Results: 1. Patients with diabetes show a higher incidence of polyps with high-grade dysplasia/carcinoma – 32/91 (35.16%) in comparison to patients without diabetes – 136/885 (15.37%), P < 0.001; 2. Patients with diabetes show a higher incience of polyps with cancer – 9/91 (9.89%) as compared to patients without diabetes – 18/885 (2.03%), P < 0.001. 3) Colorectal cancer occurred significantly more often in uncontrolled diabetes (P = 0.022). Conclusion: The conducted study shows a significant association between type 2 diabetes and the incidence of colorectal adenomas. These findings may lead to a conclusion that diabetic patients are at a higher risk of developing colorectal cancer, thus are in higher need for controlled colonoscopy. Therefore, it may be worth considering a scheme for screening patients in the above-mentioned group with colonoscopy.

Cancer is a deadly disease that has gained a reputation as a global health concern. Further, lung cancer has been widely reported as the most deadly cancer type globally, while colon cancer comes second. Meanwhile, early detection is one of the primary ways to prevent lung and colon cancer fatalities. To aid the early detection of lung and colon cancer, we propose a computer-aided diagnostic approach that employs a Deep Learning (DL) architecture to enhance the detection of these cancer types from Computed Tomography (CT) images of suspected body parts. Our experimental dataset (LC25000) contains 25 000 CT images of benign and malignant lung and colon cancer tissues. We used weights from a pre-trained DL architecture for computer vision, EfficientNet, to build and train a lung and colon cancer detection model. EfficientNet is a Convolutional Neural Network architecture that scales all input dimensions such as depth, width, and resolution at the same time. Our research findings showed detection accuracies of 99.63%, 99.50%, and 99.72% for training, validation, and test sets, respectively.

Application of cells with high TAA (tumor associated antigen) presentation potential seems to be crucial in neoplasia immunotherapy. Such feature is distributed in dendritic cells, which present peptides from processed TAA - MHC molecules complex to the T cells of a host. The aim of the study was to assess the influence of colon neoplasia tissue lysate on functioning of generated autologous DC’s in the field of autologous CD4+ lymphocytes immunological response towards Th1/Th2 under in vitro environment together with comparison and assessment of DCs’ immunosuppressive properties acquired from patients with colon cancer. Material and methods. The population of this study consisted of 16 healthy- controls, 36colon cancer patients. Blood samples were collected 24h before planned surgery and preventive antibiotic therapy. Neoplastic tissue sample, was digested for cell lysates preparation. DC’s generation from PBMC was carried out in standard conditionsand medium enriched with rhGM-CSF and rhIL-4. Mature DC`s and cocultured autologous CD4 lymphocytes immunophenotype assessment was analyzed with flow cytometer. Intracellular and culture medium cytokines concentration was analyzed with ELISA and FACS method. Results. DC`s generated from colon cancer patients stimulated with lysates presented greater maturity, lower expression of CD206 antigen, significantly higher expression of HLA-DR, CD208 and CD209 and high intracellular expression of IL-12, compared to non-stimulated cells. Conclusions. The neoplastic tissue in vivo produces a number of substances having an unfavorable effect on immune system, our results suggests using lysates as good dendritic cells stimulators that possibly could have application in colon cancer immunotherapy

Colon cancer is one of the most frequent causes of death in the world. Despite the easily accessible diagnostic tools, the disease is still diagnosed at its advanced stage, when radical treatment is no longer possible and only palliative therapy can be provided. Cancer recurrence, which worsens the patient’s prognosis, is a separate problem. Currently, scientists are seeking modern, multidirectional treatment methods, which would enable the inhibition of neoplasia, elongation of patients’ lives and improvement in their quality. Immunotherapy and photodynamic therapy seem to be promising as they provide good results, also at advanced stages of cancer. Due to its cytotoxic and immunomodulating effect, photodynamic therapy exhibits an antineoplastic effect. Using monoclonal antibodies in the targeted therapy of colon cancer is a new, promising concept, which requires further studies.
Rak jelita grubego jest jedną z najczęstszych przyczyn zgonów na świecie. Pomimo łatwo dostępnych narzędzi diagnostycznych nadal często rozpoznawany jest w stadium zaawansowanym, kiedy niejednokrotnie nie ma już możliwości leczenia radykalnego i pozostaje jedynie terapia paliatywna. Odrębnym problemem jest wznowa nowotworu, pogarszająca rokowanie pacjenta. Współcześnie poszukuje się nowoczesnych, wielokierunkowych metod leczniczych, które pozwoliłyby zahamować proces nowotworzenia, wydłużyć życie chorych i poprawić jego jakość. Obiecującym kierunkiem badań okazują się immunoterapia oraz terapia fotodynamiczna, które dają pomyślne wyniki również w zaawansowanych stadiach raka. Terapia fotodynamiczna wykazuje działanie przeciwnowotworowe poprzez efekt cytotoksyczny oraz immunomodulujący. Zastosowanie przeciwciał monoklonalnych w celowanej terapii raka jelita grubego jest nową, dobrze rokującą koncepcją wymagającą dalszych badań.

Due to increased colorectal cancer incidence there is a necessity of seeking new both prognostic and prediction factors that will allow to evolve new diagnostic tests. K-ras gene seems to be such a factor and its mutations are considered to be an early marker of progression of colorectal cancer. The aim of the study was to find a correlation between K-ras gene mutation in patients with diagnosed colorectal cancer and selected clinical parameters. Material and methods. A total of 104 patients (41 women and 63 men) with diagnosed colorectal cancer were included in this study. The average age of male group was 68.3 and in female group – 65.9. Samples were taken from paraffine blocks with tissue from diagnosed patients and K-ras gene mutation were identified. Afterwards the statistical analysis was made seeking the correlation betweenK-ras gene mutation incidence and clinical TNM staging system, tumour localisation, histological type, sex, age. Results. K-ras gene mutations were detected in 20.1% of all colorectal cancers. Significantly higher rate of K-ras gene mutations were diagnosed among patients classified at stage I (40%), stage IIC (50%) and stage IV (50%) according to the TNM classification. Conclusions. The results of our study are compatible with other studies and indicate the correlation between K-ras gene mutation and colorectal cancer incidence. Identification of K-ras gene mutation may complement other diagnostic methods at early stage of colorectal cancer.

Introduction and objective: The risk of development colon cancer (CC) is increased significantly among patients with the type 2 diabetes (T2DM). A mechanism responsible for the higher prevalence of CC among diabetic patients may be associated with the immunity system. The aim of this study is to point out the differences in the immunity state in terms of interleukin 2 level among patients with T2DM suffering from CC, and patients without these diseases Material and Methods: 79 patients were included the tests, divided into 4 groups: Group 1–23 people with T2DM, Group 2–23 people with large intestine CC, Group 3–10 people with large CC and T2DM, and Group 4–23 people without T2DM or CC. Each patient had a colonoscopy and those with cancer were confirmed in a histopathological examination. Laboratory measurements included fasting glucose, insulin, C-peptide. The concentration of interleukin-2 in serum was determined with the immunoenzymatic (ELISA) method. Results: The results obtained showed that in patients with T2DM and CC the concentration of interleukin-2 was statistically higher than in the other groups.(4.21±1.61 pg/ml vs. Group 1 -1.64±0.44 pg/ml, Group 2–1.54±0.21 pg/ml, and Group 4–1.70±0.36 pg/ml; p<0.05). Insulin levels, C-peptide and HOMA-IR did not differ significantly between groups, but a tendency was observed to higher values of HOMA-IR and insulin levels in the groups with T2DM alone and T2DM with concomitant CC. Conclusions: The data show differences in the immunity state of patients with T2DM and CC, compared with people without those two diseases. Elevated level of interleukin 2 found in this group, after confirmation in other studies with more patients, could be used as a marker of an increased risk of CC in people with T2DM

Introduction. Colorectal cancer may be benign or malignant. According to the World Health Organization and CDC, it is the second most common cancer worldwide, after lung cancer. The mortality of colorectal cancer has been dropping for more than 20 years due to the improvements in screening techniques and treatments. Aim. The aim of this article is to discuss the role of new biomarkers for the diagnosis and treatment of colon cancer. Material and methods. This article is a review done in regards to discuss the role of new biomarkers for the diagnosis and treatment of colon cancer. Analysis of the literature. A review is discussed the role of new biomarkers for the diagnosis and treatment of colon cancer using current literature. Conclusion. The screening tests based on diagnostic new biomarkers may cause faster detection of cancer and risk factors, and provide prognostic information in order to adjust individual therapy.

Introduction. Abdominal ultrasound is considered an initial examination of the the gastrointestinal tract. The case is presented in which ultrasound (US) detected a ‘pseudokidney’ sign that enabled the tentative diagnosis of a colonic neoplastic lesion. Case report. A 78-year-old patient was admitted to the Department of Internal Diseases due to weight loss and anaemia. US demonstrated abnormalities in the form of intestinal wall thickening with a hyperechoic centre. This finding led to the tentative diagnosis of a neoplastic lesion. After further diagnostic procedures, i.e. colonoscopy and histopathological examination of biopsy specimens, the patient was diagnosed with colon adenocarcinoma. Conclusions. In many cases, experienced US radiologists can initially assess the intestinal wall and other organs, which effectively directs towards further diagnostic management, hence accelerates the diagnosis and decision to apply a given therapeutic option or otherwise.

Opis:
Adjuvant chemotherapy with 5-fluorouracil remains the basic treatment for patients with advanced colorectal carcinoma. The major obstacle in successful treatment is the ability of CRC cells to acquire chemoresistance. Here we examined the impact of ID1 silencing on the sensitivity of CRC cells to 5-FU. To suppress ID1 expression in HT-29 and HCT-116 cells the cells were transduced with a lentiviral vector carrying the ID1 silencing sequence. Cells with silenced ID1 showed altered expression of epithelial and mesenchymal markers and exhibited increased proliferation rate compared to the parental cells. HCT-116 cells with suppressed ID1 became sensitized to 5-FU and this was not observed in HT-29 cells. Silencing ID1 resulted in altered expression of genes encoding enzymes metabolizing 5-FU. HT-29 cells with suppressed ID1 had significantly reduced mRNA level for thymidine phosphorylase, uridine-cytydine kinase 2 and dihydropyrimidine dehydrogenase. ID1 suppression in HCT-116 cells resulted in an increase of mRNA level for thymidine phosphorylase, thymidine kinase and uridine-cytydine kinase 2 with concurrent drop of dihydropyrimidine dehydrogenase and thymidylate synthetase mRNA levels. In conclusion, ID1 expression impacts the sensitivity of colon cancer cells to 5-FU and may be considered as a potential predictive marker in CRC treatment.

Introduction and objective. Young green barley is the most valuable source of nutrients and bioactive substances. It has a broad spectrum of health-promoting properties such as antioxidant, anti-inflammatory, hypoglycaemic, anti-depressant, anti-atherosclerotic and anticancer. The presented study is an attempt to extend this knowledge with particular emphasis on the possibility of using green barley in colon cancer prevention. Materials and method. Extracts were prepared on the basis of two commercial products: ground dried barley grass (YGB INT) and powder of young green barley juice (YGB GW). Their influence on colon epithelial cells (CCD841 CoN) viability and proliferation were analyzed by LDH and MTT assays. Anticancer properties of extracts were screened on colon cancer cell lines (LS180, HT-29) by MTT and BrdU assays. Changes in cells morphology induced by extracts were investigated after May-Grünwald-Giemsa staining. Results. Tested extracts were not toxic against CCD841 CoN and did not affected their proliferation or morphology (LDH test, MTT test, microscopy observation). The MTT revealed that extracts significantly inhibited proliferation of colon cancer cells in a dose-dependent manner. Results of BrdU test confirmed antiproliferative properties of extracts, but opposite to MTT test, indicated YGB GW as a better anticancer agent. Light microscopy observation proved the data obtained from both MTT and BrdU tests and additionally suggested the ability of the extracts to induce necrosis in LS180 and HT-29 cells. Conclusions. The study demonstrated that YGB extracts specifically inhibit proliferation of colon cancer cells without any undesirable effect on colon epithelial cells. Obtained results will provide a rationele for the future development of dietary supplements which could be beneficial in colon cancer chemoprevention.

Patients with colorectal carcinoma showed statistically significant lower values of transferrin saturation, total iron binding capacity and serum iron level as compared with control group, while the level of ferritin and the size of labile iron pool in carcinoma patients were higher, although this difference was not statistically significant. Our observations are in favour of the hypothesis which suggests that changes in iron metabolism restrict iron availability for tumour cells and as consequence, slow their growth.

Cancers are among the most feared diseases of modern civilization. In Poland, colorectal cancer is one of the tumors with the worst prognosis. The ability to cure is primarily dependent on the stage of the disease at the time of diagnosis. The aim of the study was evaluate antioxidant response in patients with colorectal carcinoma. Material and methods. Twenty patients (14 men and 6 women) aged 61.9± 11.1 years with colorectal cancer were included in the study. Twenty healthy subjects (4 men and 16 women) aged 64 ± 15.3 years formed the control group. The erythrocyte activities of antioxidant enzymes, superoxide dismutase (SOD), and glutathione peroxidase (GPx), Results. A significant increase of GPx, and SOD (p < 0.05) were seen in patients compared to healthy controls. Conclusion. The results indicate that the tested antioxidant enzyme activity of glutathione peroxidase and superoxide dismutase is increased in patients diagnosed with colorectal cancer compared to the control group.

Dendritic cells are heterogeneous population of the leukocytes and most potent APC in activation of naive T lymphocytes. Therefore the DCs generated in vitro are under research for their application in anti-tumor immunotherapy. The aim of the study was generation of the immature dendritic cells from peripheral blood monocytes collected from colorectal cancer patients and comparison of their ability to endocytosis, cytokine production and immunophenotype to DCs generated from healthy donors. Material and methods. 16 adenocarcinoma stage II patients were included in the study. Dendritic cells were generated in the presence of rhGM-CSF and IL-4. PBMC were isolated from the blood of patients and 16 healthy donors - control group. Immunophenotype, ability of endocytosis of Dextran- FITC as well as intracellular IL-12 expression of the generated dendritic cells was measured using flow cytometry. The cytokines (IL-6, IL-10, IL-12p70, IFN-γ) concentration in the supernatants of DCs culture was measured by ELISA. Results. The percentage of the immature dendritic cells and expression of CD206 and CD209 antigens was significantly higher in patients group (p <0.05 and p <0.001 respectively). Significantly (p <0.001) higher expression of the antigens which initiate the Th2 immune response (CD80-/CD86 + and B7-H2 + / CD209 +) was in the patients group. There were no differences in endocytosis ability and the cytokines (IL-6, IL-10, IL-12p70, IFN-γ) concentration between investigated groups. Conclusions. High immature markers expression on the generated dendritic cells together with identical endocytosis ability in patients group is advantageous in antitumor autologous cells immunotherapy planning. However there is one troubling fact - high expression of markers, which may induce tolerance to particular antigen. It seems to be more reasonable to use the autologous DCs in the antitumor immunotherapy, especially due to the incompatibility in allogenic cells in the context of HLA complex.

Introduction:The presence of tumor budding, i.e.,single cancer cells or a nest of poorly differentiated cells at the front of tumor invasion appears to be a new histopathological indicator of increased aggressiveness of colorectal carcinoma. Purpose: The aim of this work was a retrospective evaluation of the invasion front (tumorbudding, vascular invasion,and lymphocytic infiltration) in postoperative biopsies of patients with colorectal carcinoma and analysis of the 5-year survival. Materialsand methods:The study was based on the material received after surgical treatment of 164 patients with colon cancer. Tissue was obtained directly following tumor resection, fixed in 10% formaldehyde and embedded in paraffin blocks using a routine method by melting with paraffin at a temperature of 56º C. These samples were then routinely stained with haematoxylin and eosin and underwent a histopathological evaluation, with particular attention being paid to the invasion front of the tumor. The immunohistochemical expression of cytokeratin 20 was also evaluated using anti-human CK20 monoclonal antibody (clone Ks.20.8, Dako, Poland). Results: Tumor budding was found in 124 out of 164 patients. Statistical analysis showed a correlation between the presence of tumor budding TB and depth of invasion (pT), lymph node metastasis, distant metastasis, lymphocytic infiltration,and vascular invasion. The cumulative five-year survival correlated with the lack of tumor budding and vascular invasion, as well as a decrease in lymphocyticinfiltration.

The study presented two cases of synchronous occurrence of colon and appendiceal adenocarcinoma. Both patients required surgical intervention, due to acute peritonitis during the course of acute appendicitis. In case of one patient we performed abdominal CT confirming the presence of sigmoid cancer. The patient was subjected to appendectomy and Hartmann’s operation. The second patient underwent an appendectomy, and colonoscopy performed two months later revealed the presence of rectal adenocarcinoma. The patient was subjected to low anterior rectal resection. The histopathological results considering both patients revealed the presence of synchronous colon and appendiceal adenocarcinoma.

Introduction: Expression of Epo, a glycoprotein secreted by the fetal liver and the adult kidney in response to cellular hypoxia and its receptor have been described in human solid tumors, such as colon and breast cancer. Purpose: Since activation of Epo-EpoR signaling pathway in erythroid progenitor and precursor cells leads to promotion of proliferation and differentiation or prevention of programmed cell death through Bcl-xl and Bcl-2 it was of interest to investigate expression of Epo, EpoR, apoptosis regulator – Bax and marker of proliferating cells - Ki-67 and assess correlation between them, with regard to clinicopathological variables of colorectal cancer. Materials and methods: The correlations between expression of Epo, EpoR, Bax and Ki-67 in colorectal cancer were analyzed in regard to patient age, sex, primary localization, histopathological type, grading, staging and lymph node invasion. Statistical analyses were performed by using the Spearman rank correlation test applying a significance level of p<0,05. Results: Correlation between Bax and EpoR is positive and statistically significant at all groups of patients except group pT1+pT2. Positive correlation between Bax and Epo is statistically significant at following groups of patients: all patients, age 60, age >60, male, female , primary localization in rectum, primary localization in colon, adenocarcinoma, G2, G3. Statistical analysis revealed no significant correlations between expression of neither Ki-67 with Epo nor Ki-67 with EpoR in all groups of patients. Conclusions: Epo seems to be a pleiotropic cytokine, which can exert its biological effect on several cell types, including neoplastic cells. The effect of Epo-EpoR signaling can differ in various cells and conditions.

Opis:
Detection of mutations in families with a hereditary predisposition to colon cancer gives an opportunity to precisely define the high-risk group. 36 patients operated on for colon cancer, with familiar prevalence of this malignancy, were investigated using the DNA microarrays method with the potential detection of 170 mutations in MLH1, MSH2, MSH6, CHEK2, and NOD2 genes. In microarrays analysis of DNA in 9 patients (25% of the investigated group), 6 different mutations were found. The effectiveness of genetic screening using the microarray method is comparable to the effectiveness of other, much more expensive and time-consuming methods.

Inula viscosa is a perennial herbaceous plant native to the Mediterranean Basin, which is used topically for the treatment of various diseases in folk medicine. This study aimed to evaluate the in vivo intestinal anti-inflammatory activity of the ethanolic extract of I. viscosa (EEIV) and to test its effect on a colorectal cancer cell line. EEIV was administered to rats orally and daily at 100 and 200 mg/kg body weight for 7 days, and then colitis was induced by intrarectal instillation of 2 ml of 4% (v/v) acetic acid (AA) solution. At the end of the experiment, clinical examinations of the rats were conducted by evaluating macroscopic and histological signs of colonic tissues and measuring erythrocyte sedimentation rate (ESR) and the levels of C-reactive protein, fibrinogen, myeloperoxidase (MPO), malondialdehyde (MDA) and nitric oxide (NO). Using MTS assay, the antiproliferative effect of EEIV against human colon carcinoma HT29 cells and cytotoxicity on nondifferentiated Caco-2 cell line was evaluated. EEIV significantly decreased the ESR and fibrinogen levels as compared to control colitic rats (P < 0.001). It also significantly decreased the NO, MDA, and MPO levels in the colon tissue compared with the untreated colitic group (P < 0.001). These results were confirmed by macroscopic and histological examination, which showed significant protection against AA-induced ulcerative colitis. Furthermore, EEIV at a concentration of 369.88 μg/ml did not show cytotoxicity on confluent Caco-2 cells, with significant inhibition of colorectal cancer cell (HT29) growth (EC50 = 62.39 μg/ml). These results demonstrate that EEIV plays a potential role as pharmacological tool in the management of inflammatory bowel disease and prevention of colorectal cancer.

Colorectal cancer has a high incidence rate with significant mortality and morbidity. The 15-lipoxygenase-1 (15-LOX-1) is a crucial enzyme as well as a tumor suppressor, especially in colon cancer. Bee venom, by the induction of apoptosis, is a promising new anti-cancer agent. Thus, the effects of bee venom on the expression of 15-LOX-1 m-RNA and its activities in the HT-29 cell line have been investigated in the current study. Human HT-29 colon cancer cells were treated with increasing concentrations of bee venom (1.87-30 µg/mL), and cisplatin (5 µg/mL) as the positive control for 24- and 48-hours. Then, cell viability and apoptosis were measured using the MMT assay and flow cytometry, respectively. Enzyme activity and expression was assayed using the kinetic method and real-time PCR, respectively. The results showed that the main content of bee venom was melittin with a retention time of 60 minutes. The IC50 values of bee venom at 24- and 48-hours were 6.01 µg/mL and 4.44 µg/mL, respectively. Expression of 15-LOX-1 in cancer cells treated with bee venom increased (p < 0.0005), as well as the activity of the enzyme in the presence of bee venom (p < 0.01). The current study revealed the apoptotic and cytotoxic effects of bee venom against the human colon cancer HT-29 cell line that was not seen in fibroblast cells. Findings suggest that bee venom may have therapeutic effects against the HT-29 colon cancer cell through the induction of the15-LOX-1 pathway. However, further studies are needed in this regard.

Trametes gibbosa, also referred to as lumpy bracket, is a fungus belonging to the Polyporaceae with natural bioactive compounds that have aroused great interest for their potential benefits in human health. This pilot study aimed to investigate the anti-cancer effect of Trametes gibbosa extract against the HCT-116 cell line (human colorectal cancer cells). The extract was obtained by macerating dry biomass with 3 solvents: acetone, isopropyl alcohol and DCM, isolating established anticancer agents –polysaccharide complexes (PSK and PSP) from the extract, and then being dissolved. Cellular toxicity was assessed by trypan blue staining, determining the effect of 1- and 24-hour continuous exposure to the extract of varying concentrations: 100 µl/mL, 200 µl/mL, 500 µl/mL, 1000 µl/ mL. Results have demonstrated a cytotoxic effect in time- and dose-dependent manners. The proposed cellular mechanisms behind the results are most likely apoptosis followed by secondary necrotic cell death, as the literature suggests, although that remains unclear and requires further experiments. Within the course of consecutive studies, it can be established whether the extract or the compounds contained may hold promise for clinical use, e.g., for adjuvant therapy.

Prebiotics are defined as selectively fermented food ingredients that induce specific changes in the composition and/or activity in the gastrointestinal microbiota beneficial to the host well-being and health. The aim of the presented experiment was to investigate the effect of a prebiotic applied alone or in combination with Hyppocastani extractum siccum, and Lini oleum virginale in rats with dimethylhydrazine induced colon cancer. Wistar albino rats were fed high fat diet supplemented with the prebiotic alone or in combination with Horse chestnut and flaxseed oil. The activity of faecal glycolytic enzymes, lipid parameters, bile acids, short chain fatty acids and counts of coliforms and lactobacilli were determined. Treatment with the prebiotic alone and in combination with selected substances significantly decreased the activity of glycolytic bacterial enzyme β-glucuronidase (P<0.001) and increased activities of β-galactosidase and β-glucosidase. Bile acids concentration was significantly decreased (P<0.01) except for the combination of the prebiotic with Horse chestnut. The prebiotic alone decreased the lipid parameters (P<0.001) and enhanced production of short chain fatty acids. Application of prebiotic and bioactive natural substances significantly reduced number of coliforms (P<0.05). Prebiotic alone significantly increased the count of lactobacilli (P<0.05). These results show that prebiotics have a protective effect and may be the useful for colon cancer prevention and treatment.

Introduction: Colorectal cancer (CRC) is the third most common malignancy in men and the second most common in women. The disease constitutes a significant civilization and social problem. The aim: The aim of the study is to assess the sudy group’s awareness and knowledge about CRC, as well as about its diagnostics and treatment. Material and methods: An online questionaire form was distributed in the study group regarding issues related to CRC, and followed by statistical analysis and interpretation of the obtained survey results. Results: After analysis, we found that a significant percentage of the surveyed sample group had basic knowledge and awareness in the area of CRC, whereas about half of the respondents did not consider themselves sufficiently informed about the disease. Conclusions: Considering the scale of the problem posed by CRC, it is necessary to undertake broader action to promote knowledge about this disease and to carry out this type of research on a larger and more socioeconomically diverse population.

In Poland there there are about 15‑16 thousand cases of colon cancer per year. The health care system allows the treatment of patients with colorectal cancer in highly specialized hospitals, oncology centers and district hospitals. The results of treatment within different reference level differ. The aim of the study was to evaluate the results of surgical treatment of patients with colorectal cancer at a district hospitals compared with the results of highly specialized center. Material and methods. A retrospective study. The material consisted of 171 consecutively operated patients diagnosed with colorectal cancer treated in the Department of Surgery, District Hospital in Wołomin. The control group consisted of 200 patients treated surgically at the Department of General and Colorectal Surgery, University Hospital in Łódź. In both centers, the patients were operated on by surgeons with experience in operations on the large bowel. The demographic data, information on the type of indication (elective vs emergent), and the severity of the disease by AJCC / TNM scale were collected. In the district hospital there were patients with more advanced disease (p <0.001), older (p = 0.0001), and often operated under emergent indication (p = 0.0001). The telephone survey collected data on survival or the date of death of the patient and set the percentage of five-year survival. Results. The proportion of five-year survival in the study group and control group was respectively 46% and 71% (p <0.0001). The percentage of five-year survival among patients undergoing elective procedure in both centers were respectively for Wołomin and Łódź 58% and 73% (p = 0.008). The proportion of 5-year survival among “younger” patients (<70) was respectively in Wołomin and Łódź 64% and 81% (p = 0.004) for “older” patients with (> 70) 50% and 60% (p = 0.6747) Conclusions. Overall results of surgical treatment of patients with colorectal cancer in the district hospital are inferior to treatment results in a highly specialized center. The population treated in the district hospital is statistically significantly different in comparison to patients treated in highly specialized center. The following differences were captured: severity of the disease, age and type of indication (elective vs emergent). The diffrences has an influence on the outcomes. The five years survival for patients > 70 years undergoing elective procedure is not statistically different between the district hospital and highly specialized center.

The aim of the study was to answer the question whether there are differences in the progression of the disease and its morphology, dependent of the patient’s age, as well as show colorectal cancer differences between young and elderly patients. Material and methods. During the period between 2009 and 2011, 747 patients with rectal carcinoma, and 478 with colon cancer underwent surgery at the Department of General and Colorectal Surgery. The study group comprised patients under the age of 40 years (56 patients) and >80 years (90 patients). The following were subject to analysis: gender, tumor location, percentage of radical and palliative procedures, clinical and histopathological staging, as well the differences in the morphology of the tumor. The χ2 test and Yates’ correction were used for statistical analysis. p<0.05 was considered as statistically significant. Results. A higher incidence of rectal carcinoma was observed in younger patients (p=0.004). Amongst the younger patients 30.3% were diagnosed with stage III cancer, while 35.7% with stage IV. In case of elderly patients 30.6% were diagnosed with stage III cancer, while 30.7% with stage IV. In 53.3% of young patients, local lymph node metastases were observed, while in the elderly 41.8%. No statistically significant difference was observed, considering the total number of lymph nodes metastases. However, in case of stage N2, results were unfavorable for young patients 31.8% vs 17% in case of the elderly. Mucogenic adenocarcinoma incidence was similar in both groups. In case of patients< 40 years we observed a higher incidence of poorly differentiated tumors, as compared to the elderly patients (>80 years). Conclusions. 1. It is necessary to create appropriate prevention programs for young community, and improvement of public awareness in this age group. 2. Colorectal cancer (CRC) in young patients is often recognized in advanced stage. 3. A high percentage of unresectable tumors in group of elderly patients shows that CRC is still diagnosed too late in Poland.

Introduction and objective. Barley (Hordeum vulgare L.) is known as a rich source of different bioactive compounds. At present, considerable attention of researchers is focused on young barley grass. It can be a good source of dietary minerals, vitamins, carbohydrates, amino acids, phenolic compounds and proteins. It is possible that the composition of chemical ingredients beneficial for health may induce an anticancer potential of young barley in human colon adenocarcinoma (HT-29) and human lung adenocarcinoma (A549) cell lines. Materials and method. Hordeum vulgare water extract (HWE) and Hordeum vulgare juice extract (HJE) were prepared. Cell proliferation and viability were examined with the use of MTT (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) and NR (neutral red) methods. Induction of necrosis was assessed by propidium iodide/Hoechst staining. Progress of the cell cycle involved in cell proliferation, apoptosis, and regulation of transcription was estimated using flow cytometry analysis. Additionally, the capability of free radical scavenging was evaluated with the DPPH assay. Results. The study revealed that extracts inhibited the proliferation of cancer cells. The NR study confirmed the low cytotoxic activity of the tested extracts to normal human colon epithelial cells (CCD 841 CoTr) and human skin fibroblasts (HSF). Furthermore, a dose-dependent cytotoxicity against HT-29 cells, but not A549 cells, has been reported. The free radical scavenging activity was observed in the case of the HWE but not the HJE. Conclusions. The obtained results indicate a cancer chemopreventive potential of young barley as a safe dietary agent in colon carcinoma.

Palliative chemotherapy in patients with thrombophilia is a challenge for clinical oncologists. On one hand the optimal treatment should be very safe for patient, on the other hand – as effective as possible. That therapy is also connected with proper thromboprophylaxis. The incidence of multiple cancers is a serious problem of modern oncology. In patients whose medical history includes two or more cancers, it is essential to obtain histopathological diagnosis before the administration of treatment of disseminated disease. It allows to avoid improper therapy which is often toxic. The article presents the case of a patient with antithrombin deficiency and two cancers in medical history: colon cancer and endometrial cancer. The treatment of the patient because of the metastatic cancer was started after the histopathological diagnosis. In metastatic lymph node the endometrial cancer was recognized. The carboplatin and paclitaxel were used in the therapy. The treatment was conducted in four course chemotherapy. It revealed considerable polyneuropathy and hematologic toxicity limiting from further therapy. Treatment allowed to obtain biochemical response, decrease of metastases and the condition of a patient improved. During the systemic therapy thromboprophylaxis with acenocumarol was used. The incidence of venous thromboembolism or bleeding complications were not noticed.

Colorectal cancer (CRC) is the fourth most commonly diagnosed cancer in females and the third in males. Every year there are more than a million new cases of colorectal cancer and more than six hundred thousand patients die. The risk factors of colorectal cancer include unhealthy lifestyle, genetic predisposition (found in about 25% of cases) and chronic colon inflammation i.e., ulcerative colitis and Crohn's disease. Many studies suggested the protective effect of nonsteroidal anti-inflammatory drugs, including mesalazine, on the chemoprevention of colorectal cancer. Studies have shown that the regular intake of mesalazine reduces the risk of developing colorectal polyps and cancer by its multidirectional action. Anti-inflammatory mechanisms include the modulation of inflammatory cytokine production, COX inhibition and impact on NF-κB and PPARδ pathways. Consequently, mesalazine inhibits, or at least delays, the processes of carcinogenesis.
Rak jelita grubego (RJG) jest czwartym najczęściej rozpoznawanym rakiem u kobiet i trzecim u mężczyzn. Każdego roku stwierdza się ponad milion nowych zachorowań i ponad sześćset tysięcy zgonów z powodu RJG. Do czynników ryzyka RJG zalicza się niezdrowy styl życia, predyspozycje genetyczne (stwierdzane w około 25% przypadków zachorowań) oraz choroby zapalne jelit. Wśród pacjentów z wrzodziejącym zapaleniem jelita grubego i chorobą Leśniowskiego-Crohna występuje podwyższone ryzyko zachorowania na RJG. Wiele badań wykazało ochronne działanie niesteroidowych leków przeciwzapalnych, w tym mesalazyny, w chemoprewencji raka jelita grubego. Badania wykazały, że regularne przyjmowanie mesalazyny - dzięki jej wielokierunkowemu działaniu - powodowało redukcję rozwoju polipów jelita grubego i transformacji nowotworowej komórek jelita grubego. Przeciwzapalne mechanizmy działania mesalazyny polegają na modulowaniu produkcji cytokin zapalnych, hamowaniu cyklooksygenazy oraz odziaływaniu na szlaki sygnałowe z udziałem czynnika transkrypcyjnego NF-κB i receptorów PPARδ. W konsekwencji mesalazyna hamuje, bądź przynajmniej opóźnia, procesy nowotworzenia.

Natural polyphenols and polyphenol-rich extracts have been found to possess preventive and therapeutic potential against several types of cancers, including colorectal cancer (CRC), which is an example of an inflammation-associated cancer. This study examines the chemopreventive effect of a Japanese quince (Chaenomeles japonica) fruit flavanol preparation (JQFFP) on colon cancer SW-480 cells. JQFFP, rich in procyanidin monomers and oligomers, was found to inhibit the SW-480 cell viability by 40% at 150 µM catechin equivalents (CE) after 72 h incubation when compared to control, but it was non-toxic to normal colon fibroblast CCD-18Co cells. Furthermore, 100 µM CE JQFFP suppressed COX-2 mRNA expression to 36.7% of control values and protein expression to 77%. In addition, JQFFP reduced the MMP-9 protein expression (to 24% vs. control at 100 µM CE) and caused inhibition of its enzymatic activity (to 35% vs. control at 100 µM CE). Not only did JQFFP inhibit the COX-2 and MMP-9 levels, but it also reduced the NF-κB protein expression (to 65% of control) and phosphorylation of its p65 subunit (to 51%) at 100 µM CE. These results provide the first evidence that JQFFP inhibits COX-2, MMP-9, and NF-κB expression, suggesting that it has cytotoxic, anti-inflammatory, and anti-metastatic activities towards the colon cancer SW-480 cells.

Rak jelita grubego jest drugą przyczyną zgonów w populacji pacjentów onkologicznych. Pomimo ciągłego rozwoju metod diagnostycznych, wprowadzania programów badań przesiewowych, systematycznej kontroli pacjentów będących już po interwencji chirurgicznej lub chemioterapii, osiągane finalne efekty przeżycia chorych z rakiem jelita grubego nie są zadowalające. Przyczyną tak niesatysfakcjonujących wyników jest częste wykrywanie choroby już w zaawansowanym stadium, gdyż u co trzeciego chorego w momencie rozpoznania raka jelita grubego potwierdzone są przerzuty. Powszechnie stosowane, celem detekcji zmian nowotworowych i późniejszej kontroli nawrotu choroby, markery CEA i CA 19-9 nie są wystarczające. Z tego powodu konieczne są dalsze poszukiwania znaczników, które w połączeniu z dotychczas stosowanymi elementami diagnostycznymi, umożliwią z wysoką czułością i swoistością wykrycie raka, wznowy i progresji choroby oraz pozwolą przewidzieć przebieg, a także prawdopodobną odpowiedź na standardowe leczenie. Oparcie podejmowanych decyzji na takich danych i kwalifikowanie pacjenta do niestandardowego schematu leczniczego umożliwi w większym stopniu wdrożenie zindywidualizowanej terapii, która powinna odgrywać podstawową rolę w leczeniu.
The colorectal cancer (CRC) is the second cause of death in the entire population of oncological patients. Despite of the development of diagnostic methods, screening programs, monitoring of patients after surgery or chemotherapy, results of survival rates are not satisfactory. The reason is that frequently the detection of the disease takes place already at the advanced stage, as every third patient at the time of diagnosis of CRC has already metastases. CEA and CA 19-9 markers are commonly used to detect cancer and to monitor the disease recurrence, are not sufficient. For this reason it becomes necessary to introduce new additional markers that will enable a higher sensitivity and specificity for detecting cancer, it’s recurrence and progression and will be helpful to predict the course of disease and response to standard therapy. Basing on an advanced diagnosis, an individualized therapy implementation will be possible. This approach should play a key role in the proper treatment.

BACKGROUND Series of evidences have shown that interleukin-6 (IL-6) could play an important role in the pathogenesis of colon cancer. Inositol hexaphosphate (IP6) is a naturally occurring polyphosphorylated carbohydrate, found in many plant sources and in certain high-fi ber diets. Over the past few years interest in IP6 has stemmed mostly from its potentially important antineoplastic activity against various types of cancer, including colon cancer. Moreover, it was pointed out that IP6 has ability to act on the host immune functions and infl ammatory processes by controlling the synthesis of cytokines such as IL-6. The aim of this study was to evaluate the infl uence of IP6 on IL-6 secretion by malignant epithelial colorectal Caco-2 cells. MATERIAL AND METHODS The Caco-2 cells were treated with IP6 at the concentrations of 1.0; 2.5; 5.0 mM for 1; 6; 12 and 24 hours. The level of IL-6 was measured by enzyme-linked immunosorbent assay. The concentration of IL-6 was related to the amount of total cell protein which was estimated by the Bradford method (pg/mg). RESULTS It was found that colorectal Caco-2 cells constitutively secreted IL-6 at the constant level during 24 hour culture. IP6 down-regulated IL-6 secretion in a dose and time dependent manner. Conclusion old The present findings demonstrate that IP6 in the lumen of the large intestine may play an immunoregulatory role by inducing changes in IL-6 secretion by epithelial colon cells.
WSTĘP Szereg danych wskazuje, że interleukina-6 (IL-6) może odgrywać potencjalną rolę w patogenezie raka jelita grubego. W ciągu ostatnich lat szczególną uwagę naukowców zwraca aktywność przeciwnowotworowa kwasu fitynowego (IP6), ufosforylowanego węglowodanu, składnika diety bogatobłonnikowej. Pojawiły się sugestie, że może on modulować przebieg reakcji odpornościowych i stanów zapalnych między innymi poprzez regulację syntezy cytokin takich jak IL-6. Celem pracy była ocena wpływu IP6 na sekrecję IL-6 przez komórki nowotworowe jelita grubego linii Caco-2. MATERIAŁ I METODY Komórki linii Caco-2 eksponowano na działanie IP6 w stężeniach 1,0; 2,5; 5,0 mM przez 1; 6; 12 i 24 godziny. Oznaczenie stężenia IL-6 w hodowlach przeprowadzono przy użyciu testu ELISA, natomiast do oznaczenia stężenia białka zastosowano metodę Bradforda. Stężenie IL-6 wyrażono w pg/mg białka komórek. WYNIKI Stwierdzono, że transformowane komórki nabłonkowe jelita grubego linii Caco-2 wykazują konstytutywną sekrecję IL-6, utrzymującą się na stałym poziomie w czasie trwania doświadczenia. IP6 posiada zdolność do zmniejszania tej sekrecji w sposób zależny od jego stężenia i czasu działania na komórki. WNIOSKI Wyniki badań sugerują, że IP6 obecny w świetle jelita może wywierać efekty immunoregulatorowe związane ze zmianami sekrecji IL-6 w komórkach nabłonka jelitowego.

The aim of this study was to analyze the molecular mechanism of inositol hexaphosphate (InsP6) action through which it may inhibit proliferation of colon cancer cells and cell cycle progression. A kinetic study of p53 and p21WAF1 mRNA increase was performed on human colon cancer HT-29 cells after treatment with 1, 5 and 10 mM InsP6 for 6, 12, 24 and 48 h. Real-time-QPCR based on TaqMan methodology was applied to analyze quantitatively the transcript levels of these genes. The transcription of β-actin and GAPDH genes was assessed in parallel to select the control gene with least variability. The 2-ΔΔCt method was used to analyze the relative changes in gene transcription. InsP6 stimulated p53 and p21WAF1 expression at the mRNA level, with the highest increase in p21WAF1 mRNA occurring at 24 h, i.e., following the highest increase in p53 mRNA observed at 12 h. Based on these studies it may be concluded that the ability of InsP6 to arrest the cell cycle may be mediated by the transcriptional up-regulation of the p53-responsive p21WAF1 gene.

Background: With the development of genetic research in oncology, it has become possible to track and identify early and preclinical forms of hereditary oncological diseases, which allows timely and effective preventive and therapeutic measures in relation to relatives at risk. Aim of the study: Assessment of genetically determined neoplasms in the region and the development of organizational forms and methods for early diagnosis. Material and methods: 10,727 residents of the Belarus-Poland border region were examined. Clinical and medical history data of 2,054 patients with tumors of the breast (1406), ovaries (239), and colon (409) were analyzed. As a result of the questionnaire, three main observation groups were formed: “high risk of hereditary cancer”, “hereditary cancer suspected”, and “no risk of hereditary cancer”. Results: Register and hospital screenings were the most informative types of screening. Of the 149 HBC patients who underwent molecular genetic testing, BRCA1 gene mutations were found in 5.37%, 5382insC in all cases. Seven mutations were detected in 77 individuals with a diagnosis of HOC and in 6 cases 5382insC and in 2 – 4145delA. Signs of hereditary ovarian cancer and suspicion of it were found in 1.12%, including people who were found to have a high risk of hereditary ovarian cancer. By their effectiveness, register and hospital screenings significantly exceeded the population, p<0.01. 1.67% of women suffering from this disease met the high clinical risk criteria for hereditary ovarian cancer. A high clinical risk of hereditary tumor genesis was established in 0.73% of cases among patients with a diagnosis of colon cancer. Conclusions: The results of assessing the clinical risk of hereditary cancer according to population screening indicates that approximately 1.2% of the population has an increased clinical risk of developing hereditary breast, ovarian, and colon cancer.

The case describes an 86-year-old man with thrombosis of the deep dorsal vein and corpora cavernosa of the penis who developed symptoms 4 months before hospitalisation. An accurate imaging diagnostics supplemented with a biopsy of the corpus cavernosum confirmed the initial diagnosis. Moreover, an adenocarcinoma of the hepatic flexion of the colon with numerous metastatic lesions, including those to the crus of penis were found. Diagnostic and therapeutic difficulties were described and the results obtained were discussed in the context of available literature.

Introduction: Colon cancer is a very serious medical problem in Europe and also in Poland. For many years fluorouracil has been used as a main agent in chemotherapy, but its effectiveness is not sufficient. Recently several modern drugs were introduced in the treatment of colon cancer and also various of therapeutic options have been tested but the survival time of patients with cancer has not been extended significantly. For that reason more efficient drugs have still been looked for. One of the potential anticancer drugs are thiazolidinediones (TZDs) – agonists of peroxisome proliferator-activated receptors gamma (PPARγ) which in the last years were used in diabetes treatment. Therefore, we decided to examine the effect of two TZDs – pioglitazone (PIO) and rosiglitazone (ROS) on the growth of murine colon cancer and to compare their action with the efficacy of routinely used fluorouracil (FU). Furthermore, we evaluated the PPARγ expression in colon cancer cells by using the immunocytochemical method. Material and methods: Cell line of murine cancer – Colon 38 was used in our experiment. The growth of cancer cells was assessed by using EZ4Y kit based on the modified colorimetric Mosmann method. In 24 and 48 h cell culture the effects of ROS and PIO at concentrations 10-4, 10-5, 10-6, 10-7 M and FU at concentrations 4 x 10-6 and 10-6 M were examined. Immunochistochemistry was performed with the use of murine specific polyclonal antibodies anti-PPARγ1,2 and the streptavidin-biotin-peroxidase method. Results: The immunopositive reaction for PPARγ was shown in the nuclei of Colon 38 cells. Both, the examined TZDs and fluorouracil significantly decreased the growth of colon cancer and their efficacy was dependent on the concentration of examined compounds and also the incubation time. Rosiglitazone, in all used concentrations, acted more strongly than PIO. Fluorouracil showed anti-cancer activity only in 48 h culture and its inhibitory effect was weaker than both TZDs at the highest concentration (10-4 M). Conclusions: Our data indicate that PPARγ agonists, especially rosiglitazone, inhibit the growth of Colon 38 cancer. However, the potential usefulness of rosiglitazone and pioglitazone for the chemoprevention and treatment of colon cancer requires further studies.
Wstęp: Rak jelita grubego stanowi poważny problem medyczny zarówno w Polsce, jak i w całej Europie. Przez wiele lat jako główny chemioterapeutyk stosowano fluorouracyl, niestety jego skuteczność okazała się niewystarczająca. Zastosowanie w terapii raka jelita grubego nowych leków i różnych opcji terapeutycznych nie wydłużyło istotnie czasu przeżycia pacjentów. Z tego powodu nadal poszukiwane są bardziej skuteczne formy leczenia. Związkami o potencjalnym działaniu przeciwnowotworowym są tiazolidinediony (TZDs) – agoniści receptorów gamma aktywowanych proliferatorami peroksysomów (PPARγ), które w ostatnich latach wykorzystywano w leczeniu cukrzycy. W związku z tym, w pracy oceniono wpływ dwóch TZDs – pioglitazonu (PIO) i roziglitazonu (ROS) na wzrost mysiego raka jelita grubego, a ich działanie porównano ze skutecznością fluorouracylu (FU). Ponadto w komórkach raka jelita grubego metodą immunohistochemiczną oceniono ekspresję PPARγ. Materiał i metody: Badanie przeprowadzono na linii komórkowej mysiego raka jelita grubego – Colon 38. Wzrost komórek nowotworowych mierzono za pomocą zestawu EZ4Y opartego na metodzie kolorymetrycznej Mosmann’a. W hodowli 24- i 48-godzinnej oceniono wpływ ROS i PIO w stężeniach 10-4, 10-5, 10-6, 10-7 M, a działanie fluorouracylu w stężeniach 4 x 10-6 i 10-6 M. W badaniu immunohistochemicznym wykorzystano mysie poliklonalne przeciwciała anty-PPARγ1,2 i streptawidynowo-biotynowo-peroksydazową metodę wizualizacji. Wyniki: W komórkach raka Colon 38 wykazano immunopozytywny jądrowy odczyn dla PPARγ. Badane TZDs i fluorouracyl znacząco zahamowały wzrost raka jelita grubego, a ich skuteczność zależna była od stężenia i czasu inkubacji. Działanie ROS było silniejsze niż PIO we wszystkich użytych stężeniach. Przeciwnowotworowe działanie fluorouracylu stwierdzono po 48 godzinach inkubacji, jednakże jego efekt był słabszy niż wpływ TZDs w najwyższym badanym stężeniu (10-4 M). Wnioski: Uzyskane wyniki wskazują, że agoniści receptorów PPARγ, a w szczególności roziglitazon, hamują wzrost raka Colon 38. Jednakże wykorzystanie roziglitazonu i pioglitazonu w leczeniu raka jelita grubego wymaga dalszych badań.

WSTĘP: Rak jelita grubego (colorectal cancer – CRC) jest jednym z najczęstszych nowotworów na świecie – odpowiada za 10% rocznej globalnej zapadalności na raka. Przez długi czas może pozostać bezobjawowy i mimo postępu medycyny zbyt często zostaje zdiagnozowany za późno. Białko YKL-40 jest czynnikiem wzrostu stymulującym migrację komórek śródbłonka, odgrywającym rolę w stanach zapalnych oraz nowotworowych. Celem pracy była ocena znaczenia YKL-40 jako biomarkera w CRC oraz określenie powiązań stężeń YKL-40 w surowicy u chorych na CRC z wybranymi parame-trami kliniczno-patologicznymi. MATERIAŁ I METODY: W badaniu prospektywnym, obejmującym 133 osoby powyżej 50 roku życia, oznaczono stężenie białka YKL-40 w surowicy metodą ELISA. Pacjentów podzielono na dwie grupy: 91 osób chorujących na CRC oraz 42 osoby zdrowe. W analizie statystycznej wykorzystano testy t-Studenta dla danych niezależnych, U Manna-Whitneya i regresję logistyczną.WYNIKI: Stężenie YKL-40 w surowicy było znacznie wyższe u chorych na CRC (163 ± 36 μg/l) niż u osób zdrowych (54 ± 20 μg/l; p < 0,0001). U osób z CRC nie wykazano zależności między parametrami klinicznymi, tj. płcią, wiekiem i wskaźnikiem masy ciała (body mass index – BMI), a stężeniem białka YKL-40 w surowicy. U chorych z CRC stężenie YKL-40 w surowicy statystycznie istotnie różniło się we wczesnym i średniozaawansowanym stadium nowotworowym. WNIOSKI: Białko YKL-40 wydaje się obiecującym biomarkerem CRC. Jego stężenie w surowicy jest skorelowane ze stopniem zaawansowania nowotworu zależnie od głębokości nacieku w stosunku do błony podśluzowej i może być czynnikiem rokowniczo niepomyślnym dla tego nowotworu.
INTRODUCTION: Colorectal cancer (CRC) is one of the most common cancers in the world, accounting for 10% of the annual global cancer incidence. This malignant neoplasm can remain asymptomatic for a long time and, despite medical advances, is too frequently diagnosed too late. The YKL-40 protein is a growth factor that stimulates endothelial cell migration, and plays a role in inflammation and neoplasia. The aim of the study was to assess the significance of YKL-40 as a biomarker in CRC and to determine the correlation between the serum YKL-40 levels in CRC patients and selected clinical and pathological parameters. MATERIAL AND METHODS: In a prospective study involving 133 patients over the age of 50, the serum YKL-40 protein level concentration was determined by the ELISA method. The patients were divided into two groups: 91 with CRC and 42 healthy. For the statistical analysis Student's t-test for independent data, the Mann-Whitney U test, and logistics regression were used. RESULTS: The YKL-40 serum concentration was significantly higher in the CRC patients (163 ± 36 μg/l) than the healthy patients (54 ± 20 μg/l; p < 0.0001). There was a statistically significant difference in the serum YKL-40 concentration between the early and intermediate stages of CRC. There was no correlation between the clinical parameters, i.e. sex, age and BMI and the serum YKL-40 protein concentration in the people with CRC. CONCLUSIONS: The YKL-40 protein seems to be a promising CRC biomarker. Its serum concentration is correlated with the stage of the cancer depending on the depth of the growth relative to the submucosa and may be a prognostic factor for an adverse prognosis of this cancer.

The aim of this paper is to check whether the information for colon cancer patients available on top websites devoted to this disease is comprehensible for the readers or whether, due to high saturation with special medical terms, it is beyond the recommended readability level of an average internet user. Two main criteria of analysis were involved in the study: readability and lexical sophistication. The methods used in the study include 8 readability tests (ARI, Colemen-Liau, New Dale-Chall, Flesch-Kincid, Fry, Gunning Fog, Raygor Estimate, and SMOG), TAALES software and Lexical Complexity Analyser used to examine syntactic and lexical parameters of texts, and a corpus-assisted web-based tool used for lexical sophistication called VocabProfile. The study has shown that none of the 30 websites under scrutiny meets the demand of the recommended readability level, and that higher lexical sophistication involves a lower readability level.
Celem artykułu jest sprawdzenie czy informacje na temat raka jelita grubego dostępne na najczęściej odwiedzanych stronach internetowych są zrozumiałe dla czytelników czy też, z uwagi na duże nasycenie terminami specjalistycznymi, informacje te są poza poziomem czytelności rekomendowanym dla przeciętnego użytkownika internetu. W analizie zastosowano dwa kryteria opisu: czytelność i trudność leksykalną. Metody badawcze polegały na wykorzystaniu 8 testów na czytelność (ARI, Colemen-Liau, New Dale-Chall, Flesch-Kincid, Fry, GunningFog, RaygorEstimate, SMOG) oraz kilku programów komputerowych, w tym TAALES i LexicalComplexityAnalyser do analizy syntaktycznych i leksykalnych cech tekstu oraz program VocabProfile, dostępny w internecie program oparty na analizie korpusowej pozwalający przeanalizować trudność leksykalną. Badanie wykazało, że wysoka wartość trudności leksykalnej skorelowana jest z poziomem czytelności.

Azot jest jednym z najważniejszych pierwiastków w przyrodzie, będąc głównym składnikiem atmosfery oraz składnikiem budulcowym DNA i białek wszystkich organizmów. Zawartość związków azotowych w glebie decyduje o jej żyzności. Dlatego więc związki azotu są szeroko stosowane w postaci sztucznych nawozów w celu zwiększenia plonów. Nadmiar zastosowanych nawozów, nie zbilansowany z intensywnością produkcji roślinnej, może stać się źródłem zanieczyszczenia azotanami wód powierzchownych oraz żywności. Związki azotu stosowane są również jako konserwanty żywności, zapobiegające rozwojowi bakterii jadu kiełbasianego (peklowanie). Podczas przetwarzania konserwowanych produktów azotany wchodzą w reakcję z zawartymi w mięsie aminami tworząc karcinogenne nitrozoaminy. Opisuje się związek nitrozoamin z rozwojem nowotworów przewodu pokarmowego (rak żołądka, rak jelita grubego, rak przełyku). Sygnalizowana jest również obecność w nawozach sztucznych soli metali ciężkich (ołów, arsen, kadm), które między innymi, mają wpływ na rozwój nowotworów przewodu pokarmowego i układu moczowego. Przedstawiono pokrótce sytuację epidemiologiczną i czynniki etiologiczne nowotworów żołądka, jelita grubego i przełyku.
Nitrogen, as the main component of the atmosphere, and a component of the construction of DNA and proteins of all organisms, is among the most important elements in nature. The content of nitrogen compounds in soil decides about its fertility. Therefore, nitrogen compounds are widely applied in the form of artifi cial fertilizers in order to increase yield. An excess of the fertilizers used, unbalanced with the intensity of plant production, may become the source of contamination of surface waters and food with nitrates. Nitrogen compounds are also applied as food preservatives, preventing the development of Clostridium botulinum bacteria in food products (pickling meat). While processing conserved food products, nitrates enter into the reaction with amines which are present in meat and form carcinogenic nitrosamines. There are reports describing the relationship between nitrosamines and the development of gastrointestinal cancer (gastric cancer, colorectal cancer, oesophageal cancer). In addition, the presence of heavy metals salts in artifi cial fertilizers is also signaled, such as lead, arsenic and cadmium which, among other things, exert an eff ect on the development of cancer of the gastrointestinal tract and urinary system. The epidemiological situation and etiologic factors of gastric, colorectal and esophageal cancer are briefly presented.

Organizacja czasu pracy coraz częściej obejmuje pracę w systemie zmianowym, co wpływa na zdrowie pracowników. Artykuł porusza ostatnie doniesienia na temat wpływu pracy zmianowej na ryzyko kancerogenezy. Podano różne definicje pracy zmianowej, a także pokrótce przedstawiono skalę problemu. Zaprezentowano najnowsze wyniki badań dotyczących związku ryzyka raka piersi, endometrium, prostaty i jelita grubego z pracą zmianową. Przedstawiono stanowisko Międzynarodowej Agencji Badań nad Rakiem IARC (International Agency for Research on Cancer), która uznała pracę zmianową powodująca zaburzenia rytmu okołodobowego za prawdopodobny czynnik kancerogenny, zaliczając ją do grupy 2A. Pomimo wystarczających dowodów na wpływ kancerogenny światła w czasie biologicznej nocy na zwierzęta eksperymentalne, wciąż istnieją ograniczone dowody na kancerogenność pracy zmianowej, w tym pracy nocnej dla ludzi. Wskazano kierunki dalszych badań oraz konieczność opracowania profilaktyki.
Nowadays organisation of work time often involves shiftwork , which affects workers’ health. This article analyses recent reports-on the influence of shiftwork on the risk of carcinogenesis. It presents various definitions of shiftwork and the scale of the problem. It also discusses the latest research on the association between risk of breast, endometrial, prostate and colon cancer, and shiftwork. According to the position of the International Agency for Research on Cancer IARC, shiftwork that involves night work resulting in disturbance in the circadian rhythm is a potential carcinogenic factor class 2A. Despite the sufficient evidence coming from experimental research performed on animals for. the cancerogenicity of light during work at night, there is still limited evidence for the cancerogenicity of shiftwork at night performed by humans. We conclude that further research should focus on preventive strategies.

Introduction and objective. Screening colonoscopy is a recommended tool, and the most sensitive and cost-effective method for reducing the incidence of colorectal cancer (CRC). Objective. The purpose of the study was to present the results of a 5-year screening for early detection of CRC carried out among the population of the central-eastern regions of Poland, primarily in Lublin Province. Materials and method. Screening colonoscopy was conducted in a group of 1,009 patients – 636 women and 373 men, aged 40–65 years. Results. Neoplastic polyps were found in 275 patients, advanced adenomas in 49 patients and adenocarcinoma in 13. 70.55% of neoplastic polyps was located in the distal colon, 18.9% in the proximal part and 10.55% in both regions, advanced adenomas in 79.59%, 8.16% and 12.25%, respectively. Adenocarcinoma was located in the proximal colon in 2 cases and in the distal region in 11 cases. Neoplastic polyps and advanced adenomas occurred significantly more frequently in smokers than in non-smokers. Neoplastic polyps were found statistically more frequent in males than in females, among the overweight and obese patients, than in subjects with normal BMI, and more frequently in the group of urban, compared to rural patients. However, the frequency of advanced adenomas and CRC was not statistically different in those groups. The incidence of CRC was statistically more frequent in males than in females. Smoking and male gender were significant risk factors for developing neoplastic polyps. Male gender seemed to predispose to CRC. Obesity was found to favour advanced adenomas. Conclusions. The results of screening found neoplastic polyps in every third person (mean) who did not have any symptoms suggestive of colon pathology. Advanced adenomas were found in 5% of the examined and CRC was detected in 1.29% of participants. Smoking, male gender and overweight were significant risk factors for developing neoplastic polyps. No correlation was found between gender and the location of neoplastic polyps and advanced adenomas in the colon.

Gelsolin, one of a major actin-binding proteins, is involved in the regulation of actin cytoskeleton organization by its severing and capping activity towards actin filaments. Human colon adenocarcinoma cell line LS180 and its selected variants of different metastatic potential were used to check for a correlation between gelsolin level, its subcellular localization and the invasive capacity of cells. Based on immunoblotting experiments, a decreased level of gelsolin was detected in the most invasive 5W subline when compared to the parental cell line LS180. The intracellular distribution of actin filaments and gelsolin in colon adenocarcinoma cells was examined by confocal microscopy. In the 5W subline, unlike in the other examined cells, gelsolin was colocalized with filamentous actin at the cell periphery. In summary, in human colon adenocarcinoma cells, gelsolin level and its subcellular distribution seem to correlate with their metastatic potential.

Introduction. Minimally invasive techniques in colorectal surgery have become increasingly popular and are considered a standard of care in most surgical cenres. Locating the tumour during laparoscopic procedure can be technically challenging. Incorrect localization of the primary lesion may lead to a non-radical resection margin. The technique of endoscopic tattooing (ET) prior to surgery or endoscopic treatment is considered a useful tool. Various dyes can be used for this purpose, such as: Indian ink, methylene blue, indigocarmine, toluidine blue, isosulfan blue, haematoxylin and eosin, indoxin green. This procedure is recommended by international scientific societies (ASGE and ESGE). Objective. The purpose of the study is to review the current literature on the use of ET in large intestine tumour lesions. Materials and method. A MEDLINE literature search of English language articles addressing the use of ET to enable intraoperative tumour localization in colorectal surgery was performed to evaluate and summarize the feasibility of this technique. Results. The use of ET enables the easy and safe localization of colorectal tumurs during minimally invasive colorectal procedures. The percentage of complications is insignificant. Conclusions. The available literature proves the safety and benefits of using the ET prior to surgical or endoscopic treatment. ASGE and ESGE recommend the use of ET in marking tumours before surgical treatment, and the area after endoscopic resection for further evaluation.

Earlier reports suggest that the endocannabinoids may play a role of endogenous tumor growth modulators. In this study, we investigated whether inhibition of the enzymes involved in the synthesis and degradation of endocannabinoids may reduce colorectal cancer cell invasion and migration. The human colon adenocarcinoma Colo-205 cells were incubated with PF-3845, JZL-184 and RHC-80267 (fatty acid amide hydrolase (FAAH), mono- (MAGL) and diacylglycerol lipase (DAGL) inhibitors, respectively) for 48 h. The MTT colorimetric assay was performed to quantify cell viability. Next, Colo-205 cells were incubated with PF-3845 alone or with PF-3845 together with selected antagonists: AM 251, AM 630, SB 366791, RN 1734 and G-15 (CB1, CB2, TRPV1, TRPV4 and GPR30 antagonists, respectively). Western blot assay was applied to identify the changes in CB1 and CB2 receptor expression. Migration and invasion assays were employed to characterize the effect of PF-3845 on colorectal cancer cell invasion. We found that of all the inhibitors used, the FAAH inhibitor PF-3845 reduced the Colo-205 cell line viability the most effectively (IC50=52.55 μM). We also showed that the effect of decreased cell viability was enhanced when Colo-205 cells were incubated with PF-3845 and RN-1734, a TRPV4 antagonist (IC50=30.54 μM). Western blot assay revealed significantly decreased CB1 receptor expression levels, while CB2 expression was increased in response to PF-3845 when compared to control. Furthermore, PF-3845 inhibited migration and invasion of Colo-205 cell line. These results suggest that pharmacological inhibition of FAAH and consequent enhancement of the endocannabinoid levels may reduce the colorectal cancer growth and progression.
