Opracowanie przedstawia informacje z okresu 15 lat dotyczące dzieci, które na mocy decyzji sądowych o ingerencji we władze rodzicielską z powodu zagrożenia dobra dziecka zostały umieszczone w rodzinach zastępczych, placówkach opiekuńczych i rodzinach adopcyjnych. Niepokojącemu zjawisku wysokiej liczby będących pod nadzorem sądów małoletnich, których rodzice nie radzą sobie z wykonywaniem zadań opiekuńczo-wychowawczych, towarzyszy rosnąca liczba decyzji o pozbawieniu władzy rodzicielskiej. Na tle tendencji obserwowanych w ostatnich kilkunastu latach przedstawiono zmiany w systemie pieczy zastępczej od wprowadzenia ustawy w 2012 r. będące odzwierciedleniem założeń dotyczących ograniczenia liczby dzieci w instytucjach opiekuńczych i rozbudowywania różnorodnych form pieczy rodzinnej.
The study presents information from a fifteen-year period on children who have been placed in foster care, institutional care and adoptive families by court decisions regarding interference in parental authority taken when the child’s wellbeing was at risk. The alarming phenomenon of the high number of minors under the supervision of courts whose parents have failed to attend to the tasks of childcare and education is accompanied by an increasing number of decisions to deprive parents of their parental authority. Against the background of trends observed in recent years, the article presents changes in the system of alternative care implemented since the introduction of the Act in 2012, reflecting the assumptions about limiting the number of children in institutional care and developing various forms of family foster care.

Polish culture is strongly linked to both nuclear family and family networks, which are believed to safeguard stability and sense of security for individuals and communities. The migrations of young Polish men and women, which often scope to entire families, significantly alter the fundamentals of the above guarantees. The migration of Polish youth changes social expectations, possibilities of a family existing in an unchanged form, as well as provisions of care to those who need it, primarily children and elderly members of the kinship structure. For families affected by temporary migrations and experiencing increasingly settlement-oriented mobility, being “on the move” becomes “a way of life,” in which periods of “togetherness” are intertwined with much longer phases of separation. The practices and strategies employed by migrants in the hopes of overcoming the aforementioned challenges require thorough analysis. Therefore, a main goal of our article is to focus on multi-dimensional consequences of migration that pertain to the changes of ties and relationships in families, as well as the organization of child and elderly care.

The article presents the role of care in pedagogy and in the education of educators. The care is understood as the activity where a carer replaces a person (who, for some reason, cannot do it themselves) in their actions and life functions which enable them to live, let them stay healthy and contribute to their development. The considerations undertaken in the text lead to the conclusion about the continuous expansion of pedagogy tasks related to the issues connected with care. This expansion of the tasks is primarily due to the growing number of people who, for various reasons, require care. It also results from rapidly changing living conditions in the postmodernist world and the increasing risk of the occurrence of various forms of inadaptability. The article emphasises the interdisciplinary character of care pedagogy, showing its relationship with other subdisciplines of pedagogy as well as with human and social sciences.
Artykuł przedstawia miejsce, jakie w pedagogice a także kształceniu pedagogów, zajmuje opieka, rozumiana jako działalność, w której opiekun zastępuje osobę (która z jakiegoś powodu sama uczynić tego nie może) w tych jej działaniach i funkcjach życiowych, które umożliwiają jej życie, pozwalają zachować zdrowie i przyczyniają się do rozwoju. Rozważania podjęte w tekście, prowadzą do wniosku o nieustannym rozszerzaniu się zadań pedagogiki, odnoszących się do zagadnień związanych z opieką. To rozszerzanie się zadań wynika przede wszystkim ze wzrastającej liczby osób, które z różnych powodów wymagają opieki, jak również będącego skutkiem zmieniających się w szybkim tempie warunków życia w postmodernistycznym świecie, zwiększenia zagrożenia występowaniem różnych form nieprzystosowania. Artykuł podkreśla interdyscyplinarny charakter pedagogiki opiekuńczej, ukazując jej związki z innymi subdyscyplinami pedagogiki, jak również z innymi naukami: społecznymi, humanistycznymi i społecznymi.

The purpose of this paper is not to provide an overall picture of care ethics, but, rather, to reflect upon the concept of care, which has gained significance in particular scientific contexts. Undoubtedly, the importance of the subject of care represents a challenge on the level of fundamental philosophical positions and a diversified look into the occurring forms of the psychological and social suffering, dependency, and vulnerability. I will shed light on tenets that are considered central to the care ethics and that have led to the setting up of the relationship between care and democracy. By introducing the basic aspects of Carol Gilligan’s and Joan Tronto’s theories, I will emphasize their vocation to be politically relevant. A discussion on the relationship between care and justice on the one hand, and needs and rights on the other, will follow. Can hearing voices of care make democracy more just? How can care ethics provide a concrete support for democracy? On the political level, care ethics builds on the assumptions of the necessary relatedness and equality between individuals who are decisive in the light of the welfare state perspective. Upon closer examination, one may see that the setting up of these relationships is not new to the liberal perspective. Liberalism is historically rooted in the fundamental interconnection between freedom and equality. Virtues such as care and meekness can be formulated in the language of politics, but they still remain unpolitical. On the one hand, engaging in a critical reflection upon such virtues might be an indispensable measuring instrument for distinguishing a good democracy from a bad one. On the other hand, care ethics does not prove to be a real alternative to the ethics of justice. In this regard, care ethics seems to show its feet of clay.

The article is an attempt at diagnosing training needs of the employees of units operating in health care sector in the Podkarpacie Province. In times of permanent changes affecting each sphere of economy, providers of health care services cannot afford to remain outside this trend. Improving qualifications, adaptability of the offer, influencing its quality, and above all, the awareness of the necessity of these changes, have become an element which is fully integrated also with this sphere of public sector operations. Taking into account the above, the article verifies not only training needs articulated by employees of Health Care Centers (HCC) operating in the Podkarpacie Province, but also the way they are perceived by the managers of these centers, the ability to define training needs and their compatibility with characteristic features of analyzed HCCs. Therefore special emphasis has been placed on demonstrating the variety of diagnosed training needs with reference to such criteria as the size of analyzed centers, the market serviced by them, and their location. These determinants allowed us to conduct a complex analysis of conditions and structure of voiced need for subject training, and as a consequence, contributed to diagnosing the expectations of the health care sector concerning initiatives improving the quality of public services in the health care services area.

Both expenditure on healthcare and the functioning of the entire healthcare system in Poland stir up considerable controversy and are often discussed in the media. Hospital debts, the low quality of services, and the low availability of specialist medical services form the basis for the discussion of the effectiveness of the healthcare system. Statistical data are also bleak. Total health expenditure in Poland in 2019 amounted to 6.3% of GDP (estimated expenditure), whereas the average for health expenditure in the OECD countries was 8.8%. Therefore, Poland is below the average, and is placed last but four in the ranking (stat.oecd.org). The aim of this article is the presentation of public expenditure on healthcare in Poland from 2010 to 2020. In order to achieve this, the following research methods were used: a critical analysis of the literature, an analysis of statistical data, and - to make the research more transparent and the research results clearer - a tabular method was used. Also, widely accepted measurements were used, such as absolute values in domestic and international currencies, values per capita, and values in relation to the Gross Domestic Product (GDP).oduct (GDP).

The Study on the Role of Men in Gender Equality (2011–2012) shows a remarkable change in men’s partici- pation in care for children and domestic work in certain parts of Europe. Especially in Northern countries of Europe, men became more involved in care-giving roles between 2005 and 2010 while in the same period of time, decreases in the men’s share were reported, especially from some Southern and Post-socialist countries with low share rates. On the other hand, there has been a growing under-representation of men in professional care work. Data indicate that in the past decade this already heavily feminised sector has experienced a further decline in the participation of men. Based on intersectional analysis, the paper discusses the most relevant factors associated with men’s larger involvement in care for children and professional care. The variation of men’s share runs both between and within countries, with socio-economic position as one important variable.

This article presents a study conducted amongst young adults between the ages of 19–35 (N = 67). The aim was to determine the relationship between the about information about foster care families and the attitude of young adults towards them. For this purpose, one group (n = 35) was provided with information preceding the negative behavior of juveniles, and the other group received positive information about development opportunities (n = 32). Statistics indicate there is no relationship between the given information and the attitude of the respondents. The relationship between the attitude and general knowledge of young adults regarding foster parenthood was analyzed. In this case, the result also proved to be statistically insignificant. However, the results seem to indicate that there may be a relationship between the knowledge, attitude and age of the respondents. In addition, women had a more positive attitude towards foster care, and a wider knowledge about foster families compared to men. The data also indicate a fairly high level of knowledge of young adults about the foster care system in Poland. Respondents also have a positive attitude towards helping children and foster care families. The article discusses possible practical implications for further research and prevention.

Italy is one of the most important destination countries for Romanians. At the same time, the Italian care sector relies mainly on migrant labour, most of whom are Romanian women. Historically, Italy is considered one of the landmark countries for the southern or Mediterranean welfare state, characterised by its fragmented labour market, underdeveloped social protection system, informal economy and unpaid care work, usually done by the women in the family. Italy has one of the highest rates in Europe of both the elderly population and life expectancy at birth. In the last 20 years, the care work was gradually redistributed to migrant care workers, most of them women from former socialist countries, who often live in the household where they work. Migration from Eastern Europe, particularly Romania, has been facilitated, on the one hand, by rising unemployment and low-paid job opportunities in migrants’ countries of origin in the context of the deindustrialisation of state industry and, on the other, by the Italian elderly public-support system which is based on cash benefits granted to the family which can be redistributed to employ migrant care workers. In this paper we analyse three specific types of care work migration from Romania to Italy and the main challenges which they face, taking into account the specifics of the work and the type of migration chosen. The methodology is qualitative, based on 20 semi-structured online interviews with Romanian care workers and two interviews with stakeholders.

In recent years, there has been a maximum increase in admissions to the intensive care unit, culminating in an exponential increase in admissions during the COVID 19 pandemic. Many patients who survived and were discharged from the intensive care unit have cognitive, physical, and psychological disorders that are reflected in the term post-intensive care syndrome. Patients and their families show symptoms of anxiety, depression, post-traumatic stress, and sleep problems. The result is that they negatively affect their quality of life. Numerous risk factors contribute to the development of this syndrome, mainly the sedation, the duration of mechanical ventilation, and the length of stay in the intensive care unit. For this reason, it is necessary to take measures to prevent this syndrome including ABCDEFGH care plan, physical rehabilitation, nutritional support, and intensive care unit diaries. Care must also be given to the creation of Post Intensive care unit clinics where they have a diagnostic, therapeutic, counseling, and rehabilitation role that will act as assistants in the care of patients after discharge from the intensive care unit. Also, special care should be taken with patients who have recovered from COVID 19 whose needs are increasing and need immediate treatment. This review aims to analyze post-intensive care syndrome, prevention measures, and the impact of COVID 19. In conclusion, it is necessary to take measures to treat post-intensive care unit syndrome with early diagnosis and treatment, to reduce the adverse effects on both patients and their families.

Introduction and objective. Measuring the level of patient satisfaction is a useful tool in delivering quality care that is responsive to consumer preferences. Various socio-demographic factors might be considered as potential predictors of patient satisfaction. The aim of the study was to assess whether place of residence (rural/urban) affects patient satisfaction with hospital health care. Materials and method. Data were obtained using face-to-face questionnaire, administered in five large, urban hospitals in Podlaskie Province, north-eastern Poland, during 2014–2018. The study sample comprised of 1,624 participants (585 rural, 1039 urban) who assessed satisfaction with 28 hospital health care items. The means and standard deviations were presented to compare 28 satisfaction items between the rural and the urban samples. Regression analysis was used to determine whether location difference (rural vs urban) affected patient satisfaction with various domains of hospital health care. Results. The mean results of 28 satisfaction items on the 1–5 scale were similar among the rural and the urban samples, and generally skewed towards positive experiences. In the univariate analysis, significant associations between place of residence and patient satisfaction were identified with regard to three components of inpatient care: 1) hospital settings and staff care, 2) doctors’ professional skills, and 3) hospitalization outcomes. After adjusting for socio-demographic variables, the association remained significant only with respect to satisfaction with hospitalization outcomes (b = 0.121; SE = 0.055; p = 0.028). Conclusions. Some evidence for differences in patient satisfaction by place of residence was found. Study findings may be helpful in implementing care quality improvement programmes.

Population aging affects almost all areas of social life. In less than a quarter of a century, almost every third inhabitant of Poland will be 60 or over 60 years. Needs of society will change mainly due to the change in population age structure. The increasing need for care will be impossible to be satisfied only by the family. Greater involvement of informal support networks, NGOs, religious associations, local government and state will be required to ensure an adequate standard of care for seniors. The aim of this paper is to analyze the demographic, social and economic factors affecting the demand for nursing services provided in both the older person's living environment as well as in specialized institutions and the ability to meet the needs of care by shrinking and aging labor force. The paper first discusses the factors influencing the increasing demand for support. The second part discusses the factors affecting the ability to meet care needs.

Teorie etyki zwierzęcej głównego nurtu, reprezentowane np. przez Singera i Regana, opowiadają się za zachowaniami żywieniowymi wykluczającymi spożywanie jakichkolwiek produktów pochodzenia zwierzęcego. Zgodnie z takimi ujęciami tematu, w naszych społeczeństwach i w obecnych warunkach życia, rezygnacja z żywności pochodzenia zwierzęcego jest obowiązkiem w świetle wymogów normatywnych teorii etyki. W niniejszym artykule zaprezentuję inne spojrzenie na problem refleksyjnego wegetarianizmu moralnego (definiowanego w bardzo szerokim znaczeniu). Przechodząc od niektórych bardzo krytycznych sformułowań do argumentów reprezentowanych przez główny nurt, zaproponuję stwierdzenie, że bardziej celowym jest rozpatrywać wegetarianizm jako element samorozwoju i troski o samego siebie uprawianych przez ludzi będących uczciwymi czynnymi podmiotami moralnymi. Wegetarianizm, który jest owocem osobistych refleksji na temat moralności relacji między ludźmi i innymi istotami, powinien być rozpatrywany w taki sposób, w którym czynne podmioty moralne kształtują i kultywują swój własny charakter. Taki sposób ujmowania wegetarianizmu pozwala uniknąć trudności argumentów głównego nurtu i pozwala trafniej ujmować bogactwo i różnorodność rzeczywistych wyborów ludzi w kwestii wegetarianizmu w sferze zwyczajowych doświadczeń natury moralnej. W Niemczech dzieci i młodzież pozostające pod opieką oraz osoby opuszczające opiekę są nazywane angielskim terminem „wychowawca opieki”. To wyrażenie stało się kryptonimem dla tej grupy osób w społeczeństwie. Od dwudziestu lat przyciągają coraz większą uwagę decydentów i badaczy. Rozdział zawiera przegląd sytuacji dzieci pozostających pod opieką i opuszczających ją w Niemczech. Wprowadza statystyki w tej dziedzinie oraz ramy organizacyjne i prawne, opisuje drogę do niezależności po odejściu z opieki, a także obiecuje ostatnie osiągnięcia w zakresie wzmocnienia pozycji osób opuszczających opiekę zdrowotną, a także wspierania samoorganizacji.
Young people who cannot live with their birth families are provided with care through the child and youth welfare system. They are placed in foster, or residential care. In Germany, children and young people in care, and after leaving care are referred to with the English term “care leaver”. This phrase has become a code name for this group of persons in society. They have increasingly received attention from policy makers, and researchers over the last two decades. The chapter gives an overview to the German situation of children in and leaving care. It introduces the statistics in the field and the organizational, as well as legal framework, describes the path to independence when leaving care, and promising recent developments to empower care leavers, as well as to support self-organization.

Purpose: To assess the degree of independence in physical activities of daily living for patients under long-term in-home nursing care; to evaluate any changes in physical activities of daily living at entry and after 90 days of in-home nursing care. Materials and methods: InterRAI-HC (Residental Assessment Instrument – Home Care) questionnaire was used as a tool according to the protocol and assessed patients at entry and again after 90 days. The participants consisted of 100 consecutive patients who were newly admitted to long-term in-home nursing care.Results: The analysis of total dependence showed that more than 50% of patients were entirely dependent in eight out of ten specified physical activities of daily living (except mobility in bed and food consumption). Intention to treat analysis was performed; the percentages of patients totally dependent in performing eating and mobility in bed actions increased significantly by the 90-day follow-up. Also a comparison of the mean value of independence for analyzed activities did not change significantly over the 90 days, with the only exception being the mean value of independence for dressing up the upper part of the body, which significantly improved after 90 days per protocol analysis. A high mortality rate is noted among these patients (15% by the 90 day follow-up). This probably resulted from the restrictive qualification criteria used in Poland for patients to receive this form of in-home healthcare service.Conclusions: The majority of patients who are under long-term in-home nursing care in Poland are entirely dependent when it comes to the physical activities of daily living.

Stan polskiego systemu zdrowotnego nie jest zadowalający. W opracowaniu zwraca się uwagę na potrzebę wprowadzenia zmian instytucjonalnych w systemie ochrony zdrowia w Polsce w kontekściem różnych problemów i wyzwań. Do najważniejszych z nich należą: zmieniająca się sytuacja demograficzna (starzenie się społeczeństwa, migracje Polaków za granicę), niedobory kadr medycznych, różnice w ich terytorialnym rozmieszczeniu oraz problem dostępności usług medycznych i występowanie kolejek do specjalistów. Podjęta została również kwestia panującego w ochronie zdrowia konfliktu interesów oraz trudności w uzyskaniu porozumienia dotyczącego charakteru wprowadzanych zmian przez różne grupy interesariuszy. Dokonano przeglądu sposobów finansowania systemu zdrowotnego w Polsce w ujęciu historycznym – począwszy od zakończenia II wojny światowej do powstania w 2003 r. Narodowego Funduszu Zdrowia. Zwrócono też uwagę na problem braku spójności ustaw regulujących polski system ochrony zdrowia i występowanie zjawiska odpowiedzialności rozmytej. Następnie przeanalizowano planowane zmiany w ramach reformy proponowanej przez ministra zdrowia Konstantego Radziwiłła. Ich głównym założeniem jest zmiana modelu finansowania systemu ochrony zdrowia oraz likwidacja Narodowego Funduszu Zdrowia.
The state of the Polish health care system is not satisfactory. In the elaboration the author discusses the need for introduction of institutional changes in the health care system in Poland in the context of various problems and challenges. The most important of them are the following: the changing demographic situation (ageing population, migration of the Poles abroad), the shortage of medical personnel, the differences in their territorial placement and problem of availability of medical services and the occurrence of queues to specialists. Also the issue of the dominating conflict of interest in health care was taken and the difficulties in reaching an agreement concerning the nature of introduced changes by various groups of stakeholders. A review was made of the ways of financing the health system in Poland from the perspective of history – beginning from the end of the World War II to the creating of the National Health Fund in 2003. Also attention was paid to the problem of the lack of coherence of the laws regulating the Polish system of health care and the occurrence of the phenomenon of softened responsibility. Next the author analysed the planned changes within the framework of the reform proposed by the minister of health Mr. Konstanty Radziwiłł. Their main assumption is the change of the model of financing of the health care system and liquidation of the National Health Fund.

In this article I am focused on the functioning of the transnational intergenerational care system. This is dynamic,as it is bound up with the life cycle of the transnational family, and on the one hand, denotes practices associatedwith any assistance parents provide to their migrant children and on the other – in the event of elderly peoplebeing faced with health and basic living problems – with the phenomenon of migrants caring for their parentsin old age. The transnational system of care also incorporates the involvement (or lack of involvement, as faras this triggers consequences that are of relevance here) of relatively immobile people, for example the siblingsof migrants who provide (or not, as the case may be) domestic support for their elderly parents. In this articleI adopt the thesis that migrants who function in different care regimes change not only their own but also theirparents’ attitudes towards elderly care.

According to the Department of Foreign Affairs more and more people come to Poland in order to settle here. Although Poland is considered to be one of the least attractive countries in the European Union for incomers, many of them marry Polish citizens, find a job, study or start their own business in Poland. Such people live in big cities as well as in towns, becoming the members of local communities. Some of them are Catholics of various denominations, others are Christians from other Churches, some are non-believers or remain indifferent to any religion. They all, however, constitute a challenge for pastoral care of the family which aims at bringing them closer to Christ. Parish pastoral care priests, as it is indicated in the teaching of the Church, can undertake various initiatives according to the religious or denominational affiliation of the immigrants and their needs. The forms of assistance of pastoral care priests involve satisfying spiritual, psychological and material needs of incomers. However, the main aim of pastoral care is to integrate the newcomers with the local community.

Wsparcie społeczne jest udziałem wielu osób, przyjmuje różne formy, jest świadczone przez wiele podmiotów. Wyróżnia się w szczególności wsparcie dziecka, rodziny, ale też osób z niepełnosprawnościami, w podeszłym wieku oraz wymagających szczególnego zainteresowania z innych powodów. Celem opracowania jest analiza usług opiekuńczych i specjalistycznych usług opiekuńczych świadczonych w miejscu zamieszkania ich beneficjentów, w szczególności osób starszych. Poprzez podjęcie dyskursu nad tym zagadnieniem ukazano istotę, zakres, rodzaje usług opiekuńczych i specjalistycznych usług opiekuńczych. Ponadto wskazano również dane statystyczne ich dotyczące oraz znaczenie, zarówno dla osób starszych będących ich odbiorcami, jak i dla społeczności lokalnych.
Social support means contribution of many people, takes various forms, is provided by many entities. It includes, in particular, the support of a child, family, but also people disabilities, the elderly and those requiring special attention due to other reasons. The purpose of this study is to analyse care services and specialist care services provided at the place of residence of their beneficiaries, in particular the elderly. By undertaking a discourse on this issue, the essence, scope and types of care services and specialist care services were shown. In addition, it presents statistical data on the services and their significance were indicated, especially for the elderly beneficiaries as well as for local communities.

Introduction and aim. The incidence of cancer is increasing on a daily basis. One of the methods used for treatment is radiotherapy. Owing to interventions during the radiotherapy process, the patient may experience care dependency. In this study, the aim was to investigate care dependence and related factors in radiation oncology patients. Material and methods. This was a descriptive and cross-sectional study. Data were collected between September 2020 and September 2021. In the collection of data, a sociodemographic information form and a Care Dependency Scale were used. The sample consisted of 52 people. Results. Number of participants was 52, mean age was 60.25±11.715, mean care dependency score (initial) 66.19±18.966, mean care addiction score (final) 66.27±22.795. Conclusion. The care dependency of patients hospitalized in the radiation oncology clinic is moderate. The care dependency of these patients decreased partially during their stay in the clinic. The patient’s inability to walk, speak and the presence of a companion affected the patient’s condition. By evaluating the care dependency levels of the patients, the awareness of the nurses about their patients can be increased. In recent years, demographical changes have posed a significant interest for social work. The primary aim of social work as a science and as a profession is to ensure the social participation of all people on all levels of society: micro, meso and macro. Social work is therefore connected not just with individuals and families, but also with communities and society as a whole. This paper presents how this characteristic of social work in homes for the older people infl uence on the transformation of Slovenian institutional care from primary residential care to centres of care for the older people. Th e signifi cant impact is in the diff erence of social work between socially and medically oriented homes. Th e diff erence lies in social work methods as well as in the roles of the social worker in different areas of work with the residents, relatives and staff . This approach enables the provision of holistic care for the older people in institutions and in the community. Slovenian homes for the older people with existing and planned forms of assistance in the community demonstrate that institutional care is not necessarily linked only to the classical care in an institution. The transformation of homes enables the development of new forms of care for older adults both inside and outside each home for the older people

The article presents the evolution of cost accounting in the Polish health care system. There are presented the causal factors of poor quality cost information, and the characteristics of currently operating solutions is given. In the focus of considerations are: the nature and elements of the newly designed account system, based on the resources approach in which one of the main cost objects are DRG groups which, in turn, determines the considerable practicality of this system for the current medical entities management. The usefulness of cost accounting solutions in European countries, in the context of significant growth in the DRG funding, is also discussed.

With transition process in Slovenia, starting in 1991, also care for older people was considerably transformed. Policy measures and new services for older people living at home and their informal, mostly family, carers were the most essential contribution in this area. Social home care, as one of these services, is the focus of the paper. The first Slovenian representative survey of social home care users (2013) was used to assess the care arrangements among users of social home care. Multiple cluster analysis for symbolic data (no care, formal care only, informal care only, mixed care) carried out on 22 activities of daily living, discovered five groups of social home care users. Clusters were empirically aligned with availability of informal care network and need for care. Care arrangements empirically show compensatory and supplementary function of formal care in complementary model. Social home care was the most frequently selected additional source of care in assessment of future need for care across all clusters, with informal network being the second most frequently selected source.
W 1991roku w Słowenii rozpoczął się proces transformacji, któremu towarzyszyła spora zmiana modelu opieki nad starszymi osobami: nowe usługi dla osób starszych mieszkających w domu i ich nieformalnych, w wielu przypadkach spokrewnionych, opiekunów. Tematem artykułu jest opieka domowa oparta na zasadach pomocy społecznej, stanowiąca jedną z tych nowych usług. Pierwsze reprezentatywne badanie dotyczące użytkowników tej formy opieki domowej dla Słowenii (2013) zostało wykorzystane do przeprowadzenia oceny obejmującej osoby powiązane z opieką domową. Analiza wielu podzbiorów (Multiple cluster analysis) danych symbolicznych (brak opieki, wyłącznie formalna opieka, wyłącznie nieformalna opieka, opieka mieszana) przeprowadzona dla 22 codziennych czynności wykazała, że istnieje pięć grup osób działających w ramach usług społecznych opieki domowej. Podzbiory empirycznie przyporządkowano do dostępności sieci opieki nieformalnej i zapotrzebowania na opiekę. Ustalenia w zakresie opieki empirycznie wykazują funkcję kompensacyjną i uzupełniającą opieki formalnej w komplementarnym modelu. Opiekun domowy świadczący usługi społeczne był najczęściej wybieranym dodatkowym źródłem opieki w ramach oceny przyszłego zapotrzebowania na opiekę wśród wszystkich podzbiorów, natomiast sieć nieformalna stanowiła drugie najczęściej wybierane źródło.

Objective. To examine disparities in access to the public in Poland, in association of the resources of health care, socioeconomic development of Provinces and their level of urbanization. Materials and method. The Provinces were divided into four groups, using the quartile method, according to the urbanization and level of socio-economic development. The socio-economic development level was identified by the Human Development Index (HDI) for each Province. The urbanization level of each Province was determined by the Index of Urbanization (URBI). Disparities level to access to public health care was identified by taxonomic measure of accessibility (TMA), calculated using resources of health care data. TMA index was compared in the different regions, depending on the level of HDI and URBI. Results. There was no linear relationship between accessibility to public healthcare and socio-economic development of each tested region, nor between accessibility to public health care in the Provinces and their urbanization level. During the study, the correlation between the TMA and HDI and URBI alsdo produced a negative result. Conclusions. An insufficient number of physicians and the limited value of contracts within the public health service may cause a drop in the availability of the public medical service sector, regardless of regional level of urbanization and socioeconomic development.

The article addresses the parents’ anxiety during the separation from their child in the first days of stay in a nursery. For a parent, who is the first attachment figure for a child, separation is extremely difficult and stressful. The aim of presented research was to verify whether and to what extent, on the basis of such factors as the level of trait anxiety, gender, age of the parent or the number of children in the family, it is possible to predict the level of parents’ anxiety when entrusting children to nursery care. The study was done using State-Trait Anxiety Inventory [STAI] scale and short questionnaire regarding the age and family conditions. Results indicate that the factors predicting the level of state anxiety of the parents who entrust the care of their children to nurseries, were the parent’s gender and the level of their trait anxiety. Mothers had significantly higher levels of anxiety than fathers, and the level of anxiety as a state was higher in parents who generally experienced higher anxiety measured as trait anxiety.

This study is divided in three parts which intend to meet the following targets: Primarily to provide theoretical approaches toward the concept of equality in the healthcare. In the theoretical treatment of the concept of equality in health care the authors are cautious while making the difference between equality and justice; between equality in levels of health or health care and equity in financing etc. Secondly target it to assess the concept implemented in our legislation and make an overall assessment of whether this legal framework is comprehensive and convenient for the realization of the right to health care enshrined in the Constitution. The Albanian legislation stipulates that health care in the Republic of Albania is guided by very important principles. These principles embody the Constitution predictions from the formal point of view, primarily when declaring the right to health care as a key right of the individual, as well as when guaranteeing the equal rights based on non-discrimination. Thirdly to determine some of the specific areas of the legislation, which have to be improved and completed. After the analysis make in this paper, we found the need for some theoretical and practical improvements, which are reflected in the conclusions.

Introduction: Satisfaction with medical care is defined as the patient’s level of acceptance of healthcare, taking into account his or her needs and expectations. Purpose: Determining the level of satisfaction with care in the early postnatal period. Identification of factors affecting the level of satisfaction with care in the early postnatal period. Materials and methods: The study covered 100 women aged 15-45, patients of the Obstetrics and Gynecology Department. The study employed the diagnostic survey method, using a custom-designed questionnaire. Results: 68% of hospitalized patients rated the conditions in the unit as good and very good. According to 42% of them, both the doctors and the midwives provided the patients with exhaustive information and expressed empathy. 54% of the surveyed women had not been acquainted with the floor plan of the unit, and 75% of the personnel had not revealed their names or functions to the patients. Conclusions: More than a half of the respondents assessed the level of care as good or very good. Assessing the early postnatal care they received, they mostly considered the conditions ensured by the hospital, the level of care from the personnel, both with respect to the women and newborns, and the general atmosphere in the unit.

Intensive care units and well-qualified medical staff are indispensable for the proper functioning of every hospital facility. Due to demographic changes and technological progress having extended the average life expectancy, the number of patients hospitalized in intensive care units increases every year. Global shortages of nursing staff (including changes in their age structure) have triggered a debate on the working environment and workload the nursing staff are exposed to while performing their duties. This paper provides a critical review of selected methods for the measurement of the workload of intensive care nurses and points out their practical uses. The paper reviews Polish and foreign literature on workload and the measurement tools used to evaluate workload indicators.

The article attempts to determine basic organizational and legal changes concerning the functioning of educational care centres, which allows to show the main directions of deinstitutionalization of the system of foster care in Poland. New solutions, introduced in 2012, demonstrate a distinct paradigm shift of the pro-family policy in this area: preference for the work with the natural family in order to eliminate the dysfunction before the need for foster care emerges. It poins to the latest perspective on the care of orphaned children – moving away from institutional care towards family forms of foster care.
W artykule została podjęta próba określenia podstawowych zmian organizacyjnych i prawnych dotyczących funkcjonowania placówek opiekuńczo-wychowawczych, co pozwoliło na wskazanie głównych kierunków deinstytucjonalizacji systemu pieczy zastępczej nad dzieckiem w Polsce. Nowe rozwiązania wprowadzone od 2012 roku ukazują wyraźną zmianę paradygmatu polityki prorodzinnej w tym obszarze: prymat pracy z rodziną naturalną w celu wyeliminowania dysfunkcji – przed opieką zastępczą. Wskazuje to na nową perspektywę w opiece nad osieroconymi dziećmi – odchodzenie od instytucjonalnych na rzecz rodzinnych form pieczy zastępczej.

Niniejszy artykuł stanowi analizę ustawodawstwa regulującego organizację podstawowej opieki zdrowotnej, w szczególności podstawowej opieki zdrowotnej na rzecz dzieci i młodzieży. W pierwszym rozdziale przedstawiono dotychczasowe rozwiązania polskiego ustawodawstwa w zakresie podstawowej opieki zdrowotnej, w szczególności prze-pisy ustawy z dnia 27 sierpnia 2004 r. o świadczeniach opieki zdrowotnej finansowanych ze środków publicznych. Kolejny rozdział skoncentrowano na regulacjach krajowych oraz międzynarodowych dotyczących podstawowej opieki zdrowotnej na rzecz dzieci i młodzieży, w tym postanowieniach Konstytucji. W kolejnej części przedstawiono rozwiązania zawarte w ustawie o podstawowej opiece zdrowotnej, ze szczególnym uwzględnieniem opieki na rzecz dzieci i młodzieży. Regulacje te są rezultatem szerokich konsultacji, w szczególności ze środowiskiem medycznym. W artykule wskazano główne przyczyny uchwalenia ustawy o podstawowej opiece zdrowotnej. Obecnie najważniejszym wyzwaniem dla stworzenia skutecznego systemu podstawowej opieki zdrowotnej na rzecz dzieci i młodzieży jest opracowanie efektywnych sposobów wypracowania projektów regulacji ustawowych w tym zakresie.
The following article is an analysis of Polish legal acts concerning the organization of the primary health care, particularly the primary care for children and youth. The first chapter of the article presents the primary health care as a part of the Polish healtcare system. This article indicates that before the Polish parliament passed The Primary Health Care Act, this matter was regulated in several another legal acts, mostly in The Healthcare Benefits Act from 2004 year. The next chapter focuses on the primary care for children and youth. The healthcare for children and youth was regulated in the international law and The Constitution of the Republic of Poland. Therefore, the primary care for children and youth is a very important matter in the Polish legal system, which is confirmed in regulations of legal acts. The third chapter presents the main regulations in The Primary Health Care Act, particularly for children and youth. The passed act is a result of work of many lawyers and members of the medical environment. To summarise, this article indicates the main reason to establish The Primary Health Care Act, and discusses its regulations. The most important issue for the healthcare for children and youth is creating procedures for a discussion about its regulations.

Objectives There is a need for up-to-date research on health-care and social managers’ work well-being. The purpose was to develop a questionnaire for measuring health-care and social managers’ subjective work well-being and to determine whether their background factors are connected to their work well-being. Material and Methods The authors developed a questionnaire based on their previous health-care and social managers’ work well-being framework. It covers 5 separate categories: 1) individual factors, 2) social factors, 3) professional support from one’s own manager, 4) organizational factors, and 5) work-related factors. Using statistical methods, the authors examined the questionnaire’s internal validity, its fit with the framework, and the connections between several background factors and work well-being. The survey data (N = 281) were collected from South Osthrobothnia and Central Osthrobothnia in Finland. Results The questionnaire’s internal validity was good, and it fit rather well with the authors’ previous framework. Managers’ work well-being was highest for the category of “professional support from one’s own manager” and lowest for “organizational factors.” The authors found connections between different categories of work well-being and a) years of managerial experience, b) level of management, and c) occupational group. Conclusions The questionnaire gives a holistic view of managers’ work well-being and is suit- able for measuring work well-being in the social- and health-care context. An examination showed that there is a need to improve the individual situations of the social- and health-care managers. The questionnaire can be used to assess managers’ work well-being and to build a knowledge base for developing organizational policies.

has dramatically increased. The number of visits to EDs can be an important indicator of the quality of primary health care. Objectives. To analyse: 1) the frequency of admissions to EDs, 2) the type and number of medical procedures performed in EDs, and 3) the type and number of services provided by PHC in the Kedzierzyn-Kozle district. Material and methods. A retrospective analysis of the statistical data regarding services provided by PHC in the Kedzierzyn-Kozle district within the National Health Fund (NHF) and data from the hospital emergency department in Kedzierzyn-Kozle (Opole Province, Poland). Results. The results showed an annual increase in the number of hospitalisations (13,815 – 2012, 14,192 – 2013, 15,123 – 2014) and the number of medical procedures performed in the ED (mainly those from I–III categories), as well as the low rate of admission from the ED to ICU (Department of Anaesthesiology and Intensive Care). There was an increase in the overall number of consultations given by a primary health care doctor and the total number of healthcare services provided at night and during holidays. Conclusions. Reducing the number of non-urgent visits to EDs can be achieved by making efforts to meet the needs of patients at the level of primary health care, in particular through better coordination of services provided by EDs and PHC and better motivation of primary care doctors to perform the role of gatekeeper to the health care system

Despite the increasing popularity of the concepts known as person-centered care and the holistic approach, their implementation in real life is far from optimal. Patients’ priorities, preferences, and values are still too often neglected. The tendency to measure the outcomes of primary care just in terms of avoiding hospital admissions, reducing health care costs, and increasing adherence to treatment can cause problems and create distortion. Guidelines are too focused on single diseases and not patient-focused. Most guidelines have a “one-size-fits-all” mentality and do not build flexibility or contextualization into their recommendations. Quality indicators should be used with caution and wisdom, especially in primary care, as they are mainly related to a few common chronic diseases and this is not conducive to recognizing the vast range of health problems of our patients. Quality indicators can be useful as a starting point for discussions about quality in primary care but not all the data that we have in our electronic clinical records can be used to derive good quality indicators and they cannot reflect the broad scope of primary care. Some core values are difficult to measure because doctors and nurses are pushed to spend too much time on the registration and administration of the required data rather than dedicate this time to the actual care of the patient. Person-centered health care is certainly one the visions of primary care and primary care doctors need to step up and lead the change. Rural primary care doctors, who traditionally adopt a less biomedical and more holistic approach than their urban counterparts, could become the pioneers in the implementation of this process.
