Back pain is a symptom of a number of underlying diseases. The pain syndrome may be due to disorders of bone structures, apophyseal joints, ligaments, fascia, muscles or intervertebral discs, subsequently affecting or not the spinal cord and peripheral nerves. According to the pathophysiological mechanisms precipitating the pain, it can be classified as nociceptive or neuropathic pain. Nociceptive pain is precipitated by the mechanical irritation of nociceptors in ligamentous and muscular structures. Neuropathic pain is caused mostly by the compression of nerve roots. When acute nociceptive pain is not treated in a timely manner, it may develop into chronic neuropathic pain due to functional changes occurring in the central nervous system. Treatment of back pain involves a combination of myorelaxants, analgesics and non-steroidal anti-inflammatory drugs. It starts, as a first choice, with paracetamol because of a small number of its adverse effects. The combined administration of non-steroidal anti-inflammatory drugs and analgesics is preferable as it enhances the analgesic effect without increasing the toxicity. A similar combination is that of paracetamol with a mild opioid (tramadol). Neuropathic pain is influenced by several main groups of medications: antidepressants, anticonvulsants, membrane-stabilizing drugs, and GABA-B agonists. Early treatment of back pain is necessary in order to prevent pain chronification. Because of the different treatments of nociceptive and neuropathic pains, it is of particular importance to differentiate between them early.
Ból okolicy kręgosłupa to objaw towarzyszący wielu chorobom. Zespół bólowy mogą powodować nieprawidłowości struktur kostnych, stawów międzykręgowych, więzadeł, powięzi, mięśni czy krążków międzykręgowych, które następnie mogą negatywnie wpływać na rdzeń kręgowy i nerwy obwodowe. Na podstawie mechanizmów patofizjologicznych ból dzielimy na nocyceptywny i neuropatyczny. Ten pierwszy powstaje przez podrażnienie mechaniczne nocyceptorów w strukturach mięśniowych i więzadłowych. Z kolei ból neuropatyczny zwykle jest spowodowany uciskiem na korzenie nerwowe. Gdy ostry ból nocyceptywny nie jest leczony w odpowiednim czasie, może rozwinąć się przewlekły zespół bólowy spowodowany wtórnymi zmianami czynnościowymi w ośrodkowym układzie nerwowym. Leczenie zespołu bólowego kręgosłupa polega na podawaniu środków zwiotczających mięśnie, leków przeciwbólowych i niesteroidowych leków przeciwzapalnych. Lekiem pierwszego rzutu jest paracetamol ze względu na niewielką ilość reakcji niepożądanych. Preferowane jest leczenie skojarzone niesteroidowymi lekami przeciwzapalnymi i środkami przeciwbólowymi, ponieważ zwiększa efekt przeciwbólowy, nie nasilając toksyczności. W skojarzonym leczeniu stosowane jest połączenie paracetamolu z łagodnym opioidem (tramadolem). W przypadku bólu neuropatycznego podaje się różne grupy leków: leki przeciwdepresyjne, przeciwdrgawkowe, leki stabilizujące błonę komórkową i leki z grupy antagonistów receptora GABA-B. Ze względu na różne sposoby leczenia bólu nocyceptywnego i neuropatycznego ważne jest wczesne ustalenie rodzaju bólu, aby nie dopuścić do rozwoju przewlekłego zespołu bólowego.

Objectives: Low back pain in pilots of military helicopters is widely discussed in the world literature. Vibrations and improper seat configuration cause that cervical, thoracic and lumbar spine might be exposed to overloading. The aim of the study was to determine the incidence and intensity of pain in the spine as well as to identify subjective risk factors of back pain and its effect on the pilot’s actions while flying a helicopter. Materials and Methods: 112 pilots, aged 25−56 years (mean age: 34.8±6.3 years), actively flying helicopters, participated in the questionnaire survey. The questionnaire containing five groups of questions, elaborated by the authors, was used. Results: 70% of the pilots reported pain complaints. Pain was localized in different parts of the spine. Uncomfortable body posture during prolonged flights resulting from the lack of lumbar support exerted the highest effect on pain provocation. Conclusions: This questionnaire survey enabled to determine the prevalence of pain, its type and intensity. Knowledge on low back pain imposes the necessity to eliminate the negative environmental effect on helicopter pilotage. Elimination of risk factors is possible through appropriate physical exercises and adjustment of pilots’ seats. The authors plan to conduct – in the future – detailed annual examinations among pilots with back pain and, based on the results, to elaborate a prophylactic program containing simple forms of isometric and isotonic training involving paravertebral and deep abdominal muscles designed specifically for pilots of military helicopters.

Opis:
Non-specific lumbar spine pain syndromes (nslsp) are currently the most common ailment. There are many physiotherapeutic methods that reduce pain and improve the functional status of patients. One of them is Bowen Technique (BT), which is a little-known fascial therapy and is characterized by a holistic approach to the patient. The aim of the study was to assess the effectiveness of BT in patients with non-specific lumbar spine pain syndromes. We examined 50 patients aged 25-60 (the average age was 39.92, SD ± 9.510, median 37.0). An interview was conducted with each patient. Functional examination based was done four times. It included Revised Oswestry scale, Low Back Pain Disability Scale measurements of the range of motion of the lumbar spine and Visual analogue scale (VAS) was used to describe the pain of patients. Eeach patients attended into three procedures of Bowen Therapy. The pain in the lumbar spine decreased after the first TB and its level is constantly reduced with each subsequent procedure. Analyzing all subjects, statistically significant differences were visible between the state before therapy and each subsequent examination, as well as between the first and the third treatment. In the subjects, the health condition is improved, and more precisely, the intensity of pain, the frequency of its occurrence and the increase in independence were reduced. A study showed that there was a statistically significant improvement in patients' health due to therapy. The ranges of motion of the lumbar spine as a result of the therapy increased. The assessment of TB therapy after the first treatment is mostly good, and after the third 66% of respondents assess as very good. The number of patients who are very satisfied with the effectiveness of the treatment increases from the procedure to the procedure.

Pain is a complex and challenging problem for patients with low back pain and for the psychologists and physiotherapist. One of the most common musculoskeletal pain problems is low back pain. Literature data allow us to assume that possible cause of decreased multifidus size include reflex inhibition and disuse atrophy. Increased fatigability (decreased endurance) of the multifidus muscle has been demonstrated in low back pain (LBP) patients in several research studies. Taking up physical activity belongs to one of the fundamentals of the therapy Formal exercise programmes used in conjunction with other methods of treatment and psychological therapy are one the main forms treatments in low back pain. Lifestyle, physical activity in particular is one on the factors influencing the health of youth and adults. Passiveness and apathy do not facilitate the persistence in realizing the aims of treatment.

Objectives: This study aimed to investigate the prevalence of Low Back Pain (LBP) among female nursing staff and explore the potential risk factors associated with LBP. Methods: An analytical cross-sectional study was conducted on randomly selected female nurses using payroll as a sampling frame in all public hospitals in Sana'a City, Yemen. Data was collected through face-to-face interview using a structured, pre-coded questionnaire that was available in Arabic and English. Weight and height of the nurses were measured using weight and height scales and body mass index was calculated. Multiple logistic regression was used to identify the factors associated with LBP. Results: Out of 696 female nurses selected, 687 (98.7%) responded. The life-time, the 12-month and one-week prevalence rates of LBP among female nurses were 512 (74.5%; 95% CI: 71.1-77.7%), 411 (59.8%; 95% CI: 56.0-63.5%) and 249 (36.2%; 95% CI: 32.6-39.9%), respectively. The prevalence was significantly lower in Indian nurses compared to other nurses. Three out of every 10 nurses with LBP had sick leave because of LBP in the last 12 months. Factors that showed significant association with LBP among nursing staff in the multivariate analysis were age, nationality, menstrual disorders and stress level at work. Conclusion: LBP is common among female nurses in Yemen. The role of menstrual disorders in developing LBP among female nurses seems to be important. Although sharing the same working conditions, Indian nurses were less likely to report LBP, which highlight the importance of cultural differences in willingness to report LBP.

Background Spinal overload among blue-collar workers occurs most frequently in the lumbar spine. Long-lasting spinal overload results in pain syndromes leading to limitations in everyday and professional life. Material and Methods The research included 106 adult males working as farmers for at least 10 years. The control group included 50 male white-collar workers. Four research tools were used: an interview questionnaire, a questionnaire assessing pain intensity and limitations in everyday functioning of study participants, a Numeric Rating Scale (NRS) measuring pain intensity and Oswestry Disability Index. Results The research revealed that as many as 86% of the farmers (91 individuals) experienced low back pain. In the control group only 64% of all the white-collar workers complained of pain in this part of the spine. The farmers defined their low back pain as constant pain or stiffness significantly more often. In addition, they reported hip pain radiating to one or 2 legs and experienced chronic and acute back pain more frequently. The total time in which they experienced this pain was usually 11–20 years. The correlation between pain intensity measured on a numeric scale and the level of disability of farmers was revealed. Conclusions The research revealed that individual farmers complained of low back pain significantly more often. The pain was usually defined as constant pain, chronic pain, acute pain or hip pain radiating to legs. Med Pr 2018;69(4):355–364

Purpose: Low back pain (LBP) occurs among approximately 60-80% of the population, gradually contributing to long-term or recurring disability. We aimed to evaluate the efficiency of two physiotherapeutic treatments (namely, Träbert current and kinesiotherapy) among patients with LBP. Material and methods: Comprised of function tests and scales of pain and functionality, clinical tests were performed according to a prospective method in a regional hospital in Lomza. Forty patients randomly distributed into two groups: one group a subject to physical therapy with Träbert currents, the other group to kinesiotherapy following a proprietary programme. Physiotherapy was performed from Monday to Friday for two weeks in a series of ten treatments. Results: The treatment used in both groups significantly decreased pain and increased functionality of patients, evaluated by Laitinen’s survey, the Oswestry Disability Index (ODI), and the Roland Morris Disability Questionnaire. Results of the Schober’s and “fingertip-to-floor” tests also showed increased improvement. Conclusions: The physiotherapy facilitated a significant decrease in pain, an increase in the mobility in lumbosacral segment of the spine and a decrease in disability among patients with LBP.

The low back pain risks in a beef skinning operation at a high stand kill floor workstation was evaluated. The increases in compressive forces at lower back (L5/S1) between normal slump (back angle 25°, measured in the sagittal plane) and severe (45°) and between normal slump and very severe (70°) bent back postures were 387 N or 28% and 616 N or 45%, respectively. The high spine load coupled with high level of repetition can have a high probability of fatigue failure in the spine structural members. Non-neutral back posture for a large portion of the total work time can be a low back pain risk factor. The videotape analysis showed that the times involved during the task performance for the bent back (more than 25°) and severe bent back (more than 45°) were 48.4 and 33.5% of the total cycle time, respectively. The upper limit from OWAS (Ovako Working Posture Analysis System) for bent back posture is 30% of the total cycle time. The bent and twisted back posture (both more than 25°) time was 10.4% compared to OWAS limit of 5%. This indicated that actions are needed in the near future to alleviate the risk of low back pain. Ergonomics redesign of the workstation was recommended for the operation.

Objectives Increasing bilateral gluteus medius co-activation has been identified as one of the most important factors in developing low back pain due to prolonged standing in healthy people. This study aims to investigate the impact of an anti-fatigue mat on the bilateral gluteus medius co-activation pattern and to report the low back pain subjectively in 2 different standing positions on the normal rigid surface and on the anti-fatigue mat. Material and Methods While carrying out an easy simulated profession, 16 participants who had no low back pain background were requested to stand for 2 h in each position, with and without using the anti-fatigue floor mat, respectively. At the beginning of standing process and at every 15 min until the time of 120 min lapses, electric activities for the bilateral gluteus medius co-activation and subjective pain level in low back area were collected by the surface electromyogeraphy (EMG) and the visual analogue scale (VAS), respectively in each position. Results The obtained findings revealed that the anti-fatigue mat significantly decreased subjective pain level in low back area among 15 participants (p < 0.05). However, there was objectively no significant difference in the bilateral gluteus medius co-activation pattern among the participants between the position 1 and the position 2 (p > 0.05). The findings obtained under this study related to the impact of the anti-fatigue mat upon the low back pain based on the increase of > 10 mm on the VAS threshold, which showed that this intervention had no significant impact upon decreasing the number of patients suffering from the low back pain and also minimizing the bilateral gluteus medius co-activation in both pain developer groups (p > 0.05). However, 73% of the participants preferred to apply it. Conclusions It seems that the anti-fatigue mat may be useful in reducing the low back pain although it objectively didn’t significantly change the gluteus medius co-activation pattern related to the low back pain.

Objective: The purpose of this study was to examine the influence of gender and existing, recurrent low back pain (rLBP) on lower extremity and trunk mechanics, as well as neuromuscular control, during a lift task. Design: A multivariate design was used to examine the effects of gender and group on biomechanical and neuromuscular control variables in randomized symmetric and asymmetric lifting. Methods: 68 Males and females with rLBP and healthy performed symmetric and asymmetric weighted box lifting trials to a 1 meter height table. Results: Lifting style was different between gender and between the rLBP versus healthy groups during a 1m box lifting. A significant two-way interaction effect between gender and group was observed for multifidus muscle activity and knee rotation in asymmetric lifting. Several gender and group main effects were observed in pelvis obliquity, trunk flexion and side flexion, knee abduction angles in symmetric lifting; and in pelvis obliquity and rotation, trunk flexion and side flexion, hip abduction, knee abduction angles, external oblique and internal oblique muscles activity in asymmetric lifting. Conclusions: Females and individuals with rLBP appear to use different lifting styles that emphasize movement at the pelvis accompanied by poor kinematic control features at the hip, trunk and knee. Clinicians should be mindful of these changes when developing prevention and rehabilitation programs aimed at improving trunk control in preparation for lifting tasks during domestic and occupational activities.

The aim of this prospective cohort study was to identify modifiable protective factors of the progression of acute/subacute low back pain (LBP) to the persistent state at an early stage to reduce the socioeconomic burden of persistent LBP. Patients attending a health practitioner for acute/subacute LBP were assessed at baseline addressing occupational, personal and psychosocial factors, and followed up over 12 weeks. Pearson correlations were calculated between these baseline factors and the presence of nonpersistent LBP at 12-week follow-up. For those factors found to be significant, multivariate logistic regression analyses were performed. The final 3-predictor model included job satisfaction, mental health and social support. The accuracy of the model was 72%, with 81% of nonpersistent and 60% of persistent LBP patients correctly identified. Further research is necessary to confirm the role of different types of social support regarding their prognostic influence on the development of persistent LBP.

Background: Spinal pain is a common problem in developed societies. In recent years, more and more people have been found to complain about spinal pain. Therefore, it would seem that back pain remains a major problem, a burden for the patient, the family, and the economy. If the pain is severe, it can cause difficulties with performing even the most basic activities of everyday life. The quest for methods to reduce this complaint may contribute to improving the quality of life for many people. One of the more commonly chosen forms of therapy for back pain is therapeutic massage. Massage therapy has a long history with back pain treatment and has been found to produce various beneficial effects that are associated with endorphin release. It can also enhance local blood flow which could increase the efficacy of local pain mediators. Aim of the study: The aim of the study was to evaluate the effectiveness of therapeutic massage in the treatment of back pain in a 69-year-old woman. Case report: The subject was a 69-year-old woman reporting pain in the cervical and lumbar segments of the spine, BH = 154.2 cm, BM = 90.0 kg, BMI = 37.85 k/m² (obesity), waist circumference = 114.1, hip circumference = 118.2 cm, WHR = 0.96 (android obesity). The 11-point VAS scale was used in the assessment of her pain, where 0 means no pain, and 10 unbearable pain. The Laitinen Pain Scale (LPS) is a questionnaire which examines the nature of pain in four areas: the intensity of the pain, how frequently the pain occurs, the application of painkillers, and limiting motor activity. Before starting the therapy and immediately after the end of the therapy, researchers conducted a physical examination which included taking measurements of spinal mobility using the SFTR method. Before the first treatment, researchers conducted an interview was conducted about the need for the massage. 10 massages were performed after 60 minutes each, and were done every other day. Researchers performed a pain assessment with the VAS scale both before and after the procedures, which ultimately showed a reduction of the symptoms in the cervical and lumbar sections. The LPS scale showed a reduction of symptoms in three areas. The mobility of the spine had likewise increased. Conclusions: After a series of therapeutic massage treatments, there was a reduction in pain and improved spinal mobility.

Psychologically and physically straining work conditions increase the risk of low back pain (LBP) development. According to recent recovery theories, leisure time recovery can counteract the negative influences of work stress on health. Similarly, a recent empirical work has indicated a moderating role of recovery on the link between stress and health issues. In this paper, a theoretical model is deduced to integrate the moderating effects of recovery on work stress and LBP development. Based on theoretical and empirical results, 2 separate recovery paths are distinguished: on the one hand, recovery can prevent the experience of stress because a well-recovered person can cope more easily with work demands; while on the other hand, recovery refills the depleted resources after confrontation with work strain and reduces stress experiences. Given that work strains is a main risk factor for LBP development, recovery in leisure time seems to be a highly relevant aspect, which has not been investigated to date in the field of LBP.

Introduction and objective. Spinal pain (SP) has become a very common problem in contemporary societies and occurs in adults, youths, and even children. The aim of the study was to determine whether the prevalence and characteristics of SP, as well as the determinants leading to SP in Polish youths, vary depending on the place of residence. The determinants considered were out-of-school physical activity (PA) and physical work (PW), time spent at a computer and TV, and the level of psychosocial development expressed as the level of anxiety-trait. The restricting effect of the experienced SP on everyday functioning of youths was also analysed. Materials and method. A cross-sectional, population-based study comprising 502 village residents (VR) and 1,593 city residents (CR) aged 13–19, residing in 3 random Polish provinces. The tools used were a custom-designed survey and the State-Trait Anxiety Inventory (STAI) filled by students during classes. Results. The frequency and localisation of SP, the time and circumstances in which SP episodes occur, as well as the influence of the pain on everyday functioning, are the same in youths living in villages and youths living in cities. Slight differences in the correlation between the prevalence of SP and determinants of SP development related to lifestyle depending on the place of residence were observed. However, in both VR and CR the occurrence of SP is significantly correlated negatively with the level of psychosocial development. Conclusions. At present, the conditions of living in a rural and in an urban environment in Poland pose no specific threat determining the occurrence of SP in youths, and increasing the negative effect of the pain on the functioning of youths.

The aim of this study was to assess the quality of life and health behaviours and to analyse the relationship between the quality of life and health behaviours in the group of patients with low back pain (LBP). Material and methods: The research was conducted on a sample of 52 patients with LBP (29 female and 23 male, M(age) = 48.96±15.86). The tool used for assessing the quality of life was the Quality of Life Questionnaire (WHOQoL-BREF, Polish version), whereas the methodological basis for investigating health behaviours was the Health Behaviour Inventory (HBI). Results: The analysis revealed that the General Index of Intensity of Health Behaviours (GIIHB) was high in 30.8%, average in 40.4% and low in 28.8% of the respondents. Statistically significant positive relationships were observed between: (1) Positive Thinking (HBI) and: Overall Quality of Life (R=0.42), General Health (R=0.29), Psychological domain (R=0.46) and Environmental domain (R=0.44); (2) between Proper Nutritional Habits (HBI) and Overall Quality of Life (R=0.38), Psychological domain (R=0.28), Environmental domain (R=0.30); (3) between Health Practices (HBI) and: Overall Quality of Life (R=0.31) and Psychological domain (R=0.28); (4) between Preventive Behaviours (HBI) and two domains: Psychological domain (R=0.34) and Environmental domain (R=0.34). GIIHB for the respondents in general was a factor that significantly differentiated quality of life in Psychological domain (p=0.031) and Environmental domain (p=0.026). Conclusion: In general, positive correlations concerning quality of life and health behaviours of the respondents were observed between the Psychological domain and all categories of health behaviours (HBI). Furthermore, positive correlations were found for Overall Quality of Life and Environmental domain with most of HBI categories. In light of these findings, it should be indicated that the focus during therapies for patients with LBP aimed to improve their quality of life should be on education in the field of health behaviours.

Wstęp. Praca lekarza stomatologa często wymaga długotrwałego utrzymywania wymuszonej, nieergonomicznej pozycji ciała podczas pracy. Powoduje to, że przez wiele godzin kręgosłup stale narażony jest na przeciążenia. To z kolei może objawiać się bólami kręgosłupa. Od pewnego czasu stomatolodzy mają dwie możliwości sposobu pracy. Pracują z prawego boku pacjenta, co wymaga pochylenia się w przód i w prawo lub mają możliwość pracy zza głowy pacjenta wykorzystując specjalnie do tego przystosowane krzesło wymuszające prawidłowe ustawienie kręgosłupa. Materiał i metody. Badania, które zostały przeprowadzone, dotyczyły oceny występowania bólów kręgosłupa lędźwiowego u lekarzy stomatologów z podziałem na dwie grupy w zależności od przyjmowanej pozycji podczas pracy. Badaniami objęto 63 lekarzy. W celu oceny wykorzystano kilka prostych testów oraz ankietę utworzoną na potrzebę badania. Wyniki. W grupie lekarzy pracujących z boku pacjenta 18 badanych (54,5%) wskazywało na szósty stopień bólu, 11 badanych (33,3%) na siódmy oraz czworo badanych (12,1%) na piąty stopień bólu w skali VAS. U stomatologów z grupy pracujących w pozycji zza głowy pacjenta, skala bólu nie przekroczyła trzech. Wnioski. Pozycja podczas pracy lekarza stomatologa ma istotny wpływ na powstawanie dolegliwości bólowych kręgosłupa lędźwiowego. Pozycja lekarza pracującego zza głowy pacjenta jest bardziej ergonomiczna niż z boku pacjenta. Lekarze pracujący z boku pacjenta częściej zgłaszają dolegliwości bólowe kręgosłupa lędźwiowego.
Introduction. The work of a dental surgeon often entails remaining in a forced and non-ergonomic body posture for a prolonged time. Consequently, the spine is exposed to overstraining for many hours. This in turn may manifest in spine pain disorders. For some time now, dentists can choose between two working methods. They proceed at the patient’s right-hand side, which requires them to lean forward and rightward, or they may work from behind the patient’s head using a specially adjusted chair that enforces a proper position of the lumbar spine. Material and methods. The research was carried out to assess dentists’ lumbar spine pains. The subjects were divided into two groups depending on the posture they adopted while working. The research included 63 dentists. To analyse the results, a few simple tests and a questionnaire were carried out. Results. Among the dentists proceed at the patient’s right- hand side, 18 of them (54,5%) indicated on the sixth level of pain, 11 patients (33,3%) the seventh level of pain and four (12,1%) dentists suffered from pain at fifth level of the pain scale (0-10). In the group of dentists working in the position from behind the patient’s head pain scale did not exceed third level. Conclusion: dentists’ working posture has a significant impact on the formation of the lumbar spine disorders. Position of the dentists working from behind the patient’s head is more ergonomic than position while working on the patient’s one side. Dentists working on patient’s one side more often report the pain in a low back spine section.

The frequency in which back pain is diagnosed and treated has markedly increased in the recent years. At present, back pain and dysfunction syndrome is becoming a lifestyle disease, along with hypertension and diabetes. It affects more and more people. Low back pain is currently one of the most common complaints with which patients report to the doctor, including a primary care physician. Depending on the affected part of the spinal motion segment, we can distinguish: discogenic pain, radicular pain, facet joint pain and muscle pain. Unambiguous determination of its aetiology is generally difficult since, usually, several overlapping pathologies are involved. The aim of this paper is to draw attention to the problems associated with back pain and the complexity of its pathomechanism as treatment is adjusted to the site of pathology. This article is based on data from international, broadly cited literature on spinal dysfunctions. Back pain is not usually a very severe condition and can be treated conservatively in most cases. Despite this, it affects a vast number of people and constitutes a considerable financial burden for the state budget. This is associated with costs of medical procedures, but most of all, with absence at work and early opinions of incapacity for work.
W ostatnich latach częstotliwość diagnozowania i leczenia dolegliwości bólowych kręgosłupa znacząco się zwiększyła. W chwili obecnej zespół dysfunkcyjno-bólowy kręgosłupa staje się chorobą cywilizacyjną, tak jak nadciśnienie tętnicze czy cukrzyca. Problem ten dotyka coraz większej części społeczeństwa. Dolegliwości bólowe dolnego odcinka pleców należą obecnie do najczęstszych przyczyn zgłaszania się pacjentów do lekarza, w tym lekarza pierwszego kontaktu. W zależności od tego, jaka część jednostki ruchowej kręgosłupa zajęta jest procesem chorobowym, możemy wyróżnić: ból dyskogenny, ból korzeniowy, ból stawów międzywyrostkowych oraz ból mięśniowy. Z reguły trudne jest jednoznaczne ustalenie jego etiologii, najczęściej mamy bowiem do czynienia z sytuacją nakładania się kilku patologii jednocześnie. Celem artykułu jest zwrócenie uwagi na problematykę zespołów bólowych kręgosłupa oraz na złożoność patomechanizmu tych dolegliwości, ponieważ w zależności od wystąpienia lokalizacji patologii dobierana jest odpowiednia metoda leczenia. Praca opiera się na danych z międzynarodowej, szeroko cytowanej literatury z zakresu dysfunkcji kręgosłupa. Dolegliwości bólowe kręgosłupa zazwyczaj nie są bardzo poważnym schorzeniem i w większości przypadków mogą być leczone zachowawczo. Mimo to problem ten dotyka dużą grupę społeczną i stanowi ogromne obciążenie finansowe dla państwa. Dotyczy to kosztów związanych z procedurami medycznymi, ale przede wszystkim wiąże się z dużą absencją w pracy i wcześniejszym orzekaniem o niezdolności do dalszej aktywności zawodowej.

Objectives Housewives are nucleus of families but as the working, living, and social architecture in the rural areas significantly differ from developed or urban area, the results of urban population cannot be generalized. Hence the purpose of the present study is to evaluate the prevalence of low back pain in non working rural housewives. Also an attempt has been made to determine the impact of social burden on low back pain (LBP). Material and Methods A sample of 301 non working rural housewives of Kanpur, aged between 30–70 years was selected. Hindi version of 3 appropriate scales Nordic musculoskeletal questionnaire, Oswestry disability index and Zarit burden interview measuring musculoskeletal discomfort, low back disability and social burden were given to all the housewives. Results Analysis of data reveals that both recent and yearly prevalence of LBP in rural housewives is 83%. More than 50% housewives have severe disability due to their LBP. Correlational analysis has shown that there is a significant impact of social burden on their disability due to low back pain. Conclusions The findings of the present study suggest that 83% of the non working rural housewives have low back pain and activity restriction due to their pain. They have significant impact of social burden on their low back pain. High prevalence (83%) of low back pain among rural housewives is an alarming sign for our society. Better health-care measures to enhance rural housewives education about good posture, ergonomic measures, health schemes, health awareness, and activity pacing could help rural housewives.

Objectives Recovery processes in leisure time influence the effect of psychosocial work factors on health issues. However, this function of recovery has been neglected in research regarding the influence of work-related risk factors on low back pain (LBP) development. The aim of this prospective study was to examine the function of psychological detachment – a relevant recovery experience – concerning the influence of psychosocial work factors on LBP development. A moderating function of detachment for the interplay of work factors and LBP was assumed. Material and Methods Sixty pain-free administrative employees of German universities completed an online survey 3 times during a 6-month period. Generalized estimating equations were used to estimate risk-factors of LBP. Results Analyses revealed an increased chance of LBP development for smokers and a decreasing chance when work resources were high. Detachment had no direct influence on LBP development, although it moderated the influence of work stressors and work resources on LBP. On the one hand, high detachment values seem to protect against an increased chance of LBP development when employees were confronted with high work stressors, while on the other hand high detachment values enhance the protective effect of high work resources. Conclusions The results indicated a moderating role of detachment concerning the influence of psychosocial work factors on LBP development. Therefore, it is necessary to include recovery processes in future research regarding LBP development and consequently in LBP prevention concepts. Int J Occup Med Environ Health 2017;30(2):313–327

Cel badań. Celem badań była ocena związku między nadwagą, otyłością a bólami kręgosłupa oraz weryfikacja korelacji pomiędzy tymi zmiennymi w grupie młodych kobiet kończących studia na kierunku fizjoterapii. Materiał i metody. Grupę badaną stanowiło 41 losowo wybranych studentek fizjoterapii III roku studiów licencjackich. Dokonano pomiaru wysokości i masy ciała oraz obliczono wskaźnik BMI. Do oceny dolegliwości bólowych części lędźwiowej kręgosłupa zastosowano Kwestionariusz Oswestry. Dla analizowanych zmiennych ilościowych wyznaczono podstawowe statystyki opisowe. Odpowiedzi na pytania zawarte w kwestionariuszu zestawiono i skorelowano ze wskaźnikiem BMI. W celu znalezienia związku między wskaźnikiem BMI a niepełnosprawnością zastosowano analizę wariancji oraz Test Levene’a jednorodności wariancji. Wyniki. Analiza wskaźnika BMI wykazała, iż nadmiarem masy ciała charakteryzowało się 29% badanych studentek (nadwaga 17%, otyłość 12%). Średnia wartość BMI wynosiła 24,8 kg/m2. Jak wykazała analiza kwestionariusza Oswestry, 78% badanych doświadczyło bólów kręgosłupa, a dolegliwości o charakterze dużym i bardzo dużym odczuwało 22% studentek. U większości stwierdzono niewielki (61%) i mierny (27%) stopień niepełnosprawności. Analiza wskaźnika BMI względem stopnia niepełnosprawności wykazała związek istotny statystycznie (Fniepełnospr. (2, 38) = 8.72, p < 0,001). Wnioski. Większość badanych studentek doświadczyła bólów kręgosłupa, a znaczny odsetek (30%) charakteryzował się nadmierną masą ciała. Wykazano istotny statystycznie związek pomiędzy wskaźnikiem BMI a stopniem niepełnosprawności. Konieczne wydaje się położenie większego nacisku na profilaktykę zdrowotną w programie studiów.
Background. The aim of the study was to assess the relationship between overweight, obesity, and back pain, and to verify the correlations between these variables in a group of young female physiotherapy graduates. Material and methods. The study group comprised 41 randomly selected physiotherapy third year students. Body height and body mass were measured, and BMI was calculated. Pain in the lumbar spine was assessed with the Oswestry Low Back Pain Questionnaire. Basic descriptive statistics were determined for the analysed quantitative variables. The questionnaire answers were compared and correlated with BMI. We used analysis of variance and the Levene’s test for homogeneity of variance to establish correlations between BMI and disability. Results. BMI analysis demonstrated that 29% of the students were characterized by excess body mass (17% overweight, 12% obese); mean BMI equalled 24,8 kg/m2. Furthermore, 78% of the respondents experienced back pain, with high and very high intensity complaints reported by 22%. Most students presented minimal (61%) and moderate (27%) level of disability. A statistically significant correlation was found between BMI and the degree of disability (Fdisability (2, 38) = 8.72; p < 0.001). Conclusions. Most physiotherapy students in the study group experienced back pain, and a substantial percentage (30%) were characterized by excessive body mass. A statistically significant correlation was demonstrated between BMI and disability level. It seems necessary to put more emphasis on health prevention in university curricula.
