Opis:
The aim of the study was to implement an integrally configured method of quality control of frictional welding of piston forgings intended for Diesel Man trucks. The use of the method (consisting of the 5W2H method, the Ishikawa diagram and the ABCD - Suzuki method) would contribute to the detection of the causes of non-compliance and consequently to the elimination of non-compliant products. Steel pistons have become the subject of research due to significant problems with maintaining their expected level of quality after the introduced structural and technological changes. The proposed method has helped to identify the root cause of non-compliance. It was: incorrect design of the combustion chamber in terms of diameter and depth. The study shows that it is advisable to use an integrated approach to finding the causes of quality problems on the example of the friction welding process. This was a new solution for the company, as no in-depth analyses of quality problems using a sequence of quality management techniques have been carried out so far.

Currently, effective quality management of manufactured products is a factor determining the development of manufacturing companies. However, the identification of the source of non-compliance and the analysis of its causes are sometimes underestimated and are not followed by appropriate methodologies. The study aimed to streamline and improve the production process of aluminium pistons for passenger cars by solving the problem related to a significant number of non-compliant products. The analysis of types of nonconformities identified through penetration testing was performed. The use of histogram, brainstorming session and Pareto-Lorenz diagram was proposed, which allowed identifying the causes of the problem. The presented solution shows the practical effectiveness of a sequence of selected instruments to solve production problems. The proposed sequence of methods can be implied in other qualitative analyses in different companies.

The state of the technical infrastructure determines the degree to which the quality requirements of products are met and has a significant impact on occupational safety. The purpose of this study is to build a universal model for supporting quality management, which allows the effective implementation of a wide-ranging research path supporting the evaluation of the relationship between the degree of modernity of product processing technology and the quality of the final product and the level of occupational safety. The developed model is verified by its implementation in one of the turning stations. A practical test of the quality management support model confirms that the practice of conducting analyzes of the level of modernity of infrastructure with its application contributed to identifying critical machine components, examining factors affecting the quality of technological operations, reducing uncertainty and the risk of risky events, and conducting activities in line with the concept of continuous improvement. The course of action detailed in the model makes it possible to determine the relationships that exist between key categories of factors and critical product defects, and accidents and near-misses. This allows for the proposal of adequate improvement measures. Further studies concern the implementation of the model at other workstations in the foundry company.

Opis:
Purpose: The main goal of the research was to determine the spectrum of instruments supporting management processes that are used to improve the quality of products in terms of sustainable development. We analyzed the degree of their use in the practical functioning of the surveyed enterprises. In addition, the aim of the research was to determine the scope of application of the indicated instruments or the possibility of their use in the decision-making process relating to the improvement of the broadly understood quality of products. Design/methodology/approach: The research method applied are: 1) standardized survey research making based on a survey conducted in contact and remote way conducted in 78 enterprises in south-eastern Poland; 2) analysis of the source documentation and in-depth interviews. It is a component of broader research. Findings: The following methods were used in decision-making processes with varying frequency: the Delphi method by 58% of respondents, Tree of Failure Analysis (by 52%), Event Tree Analysis (by 44%), and the QFD method (by 41% respondents). The surveyed companies that have implemented a vapour management system based on ISO standards tend to use 5 methods out of 25 cited in this study (these are a survey, brainstorming, 5Why and/or 5W2H, benchmarking, and the Delphi method). As a result of using mainly the above mentioned methods in decision-making processes, 74% (out of all) of the respondents indicated an increase in the quality level of products, and 39% of respondents indicated the enhancement of the pro-ecological impact of products on the natural environment. Research limitations/implications: Most of the analysed enterprises from SMEs are tried integrating qualitative-environmental actions as part of improving the quality of products. This approach is crucial when improving the quality of products in SMEs. Practical implications: It is necessary to develop a multidimensional modl enabling the optimization of decision-making processes with the possibility of making full use of the available instruments for improving the quality of the product in such a way as to meet the rapidly changing requirements of stakeholders. Originality/value: The article has cognitive value for development of knowledge, science, quality, and environmental in the area of management of products.

Purpose: The effectiveness of the NDT tests is to identify the incompatibility of the products without their destruction, but NDT tests do not point out the root of incompatibility. The aim of the study was to identify the incompatibility in the product (the mechanical sealer) using the fluorescent method (FPI) and to identify the root of this incompatibility using quality management techniques (5W2H method, Ishikawa diagram, 5Why method). Design/methodology/approach: To identify the root of incompatibility in the product the quality management techniques were used, i. e. 5W2H method, Ishikawa diagram and 5Why method. The subject (the mechanical sealer) on which being incompatibility (porosity cluster) was identified, was made from 410 steel. The problem in which the porosity cluster was identified after the fluorescent method (FPI). Findings: It has been shown that integrating NDT with the selected quality management techniques is an effective way to identify the problem and the root of its creation. Practical implications: This approach can be used in enterprises to analyze different types of products and incompatibilities. Originality/value: It was proposed to integrate the NDT tests with the selected quality management techniques. This integration allows identify the root of the problem and achieving the needed quality of the product.

Purpose: The basic purpose of research was to determine the causes of making the decision that manufacturers on their way to improve the quality of their products. Design/methodology/approach: The research method applied in article is mainly standardized survey research making based on survey conducted in contact and remote way conducted in 78 enterprises in south-eastern Poland. The research process also accompanied analysis of thesource documentation. Findings: It was concluded that decision during improving quality of the product were making mainly in simultaneously with considering customers' requirements of the customers about the quality of the products and the impact of products on the natural environment. Research limitations/implications: Most of the verified enterprises from SMEs are tried integrating qualitative-environmental actions as part of improving the quality of products. This is a favorable condition for further research, so that it is possible to adjust the quality and environmental approach when improving the quality of products in SMEs. Practical implications: Discussion of the results of research have a series of practical implications mainly for product management staff. Especially in organizations that designed new products or also in significant modification of these products. Social implications: Building awareness improves not only quality of products, but also simultaneously in line with sustainable development, including in improving environmental aspects. Originality/value: The article has cognitive value for development of knowledge, science, quality, and environmental in the area of management of products.

The analyses of the nonconformity of products are made in order to achieve the desired level of their quality. This is also the case in the analyzed enterprise located in south-eastern Poland. Due to production character in this enterprise to identify the incompatibilities of products the non-destructive tests are used. In the current approach to quality after identified the nonconformities the cause her arise were determined, it was noted in order to use by statistical analyzes. Unfortunately, no deeper qualitative analyzes were made which could specify the source of its creation. Therefore, it was considered that it is necessary to propose to use (near current actions according to nondestructive tests) the sequence of quality management instruments. In order to improve the quality control actions, the sequence of techniques which complement each other, i.e. non-destructive tests, Ishikawa diagram and 5Why method, was proposed. To demonstrate the effectiveness of the proposed sequence, the rear cover of the bearing turbine was selected as the subject of the analysis. The choice was conditioned by the unit character of production, which till now was discouraged a detailed analysis of sources of nonconformities. After analyzed the product with used the fluorescent method the nonconformities were identified on the rear cover of the bearing turbine, which was the porosity cluster. In order to identify the causes these nonconformities the Ishikawa diagram was drawn up. Next, the selected main causes (i.e. supplier of the product and nonconformities created during the production of the product) were analyzed the 5Why method in order to identify the source cause of the problem. In this case, it was the nonconformity material from the supplier. The proposed sequence, which uses the minimal resources let to show the wide range of information, which should be used to the improvement of quality. The presented the set of activities can be practised in each of the enterprises to analyze quality problems in order to identify the nonconformities and their causes.

Opis:
In an increasingly volatile environment, the importance of agile management, which should be based on flexible procedures and agile strategy, is increasing. The purpose of the study was to analyze the trend of development, level of knowledge and implications with the application of agile methods, techniques in project, program and manufacturing process management tools, as well as to identify areas of activity of foundry companies in the context of increasing the efficiency of their operations through the use of agile project approach. The completed research indicated that the knowledge and variety of application of agile methods, tools and techniques is at a relatively low level within the framework of project, program and manufacturing process management in the surveyed foundries. Among the most commonly used instrumentation are: PDCA, Kanban, SCRUM, SCRUMBAN, Lean Development, DMAIC and XP. The research showed that the use of agile instrumentation in the areas of projects, programs and manufacturing process management allows work to be done in an orderly and transparent manner. Foundries that consistently use agile instrumentation produce high-quality and non-conformity-free products with short time-to-market. Future research directions will be to perform a comparative analysis of the level of knowledge and implications of agile instrumentation in foundries located in Poland and selected neighboring countries.

Purpose: The aim of the work was to propose the integrated decision method with quality techniques to precision identify the root of the problem. Design/methodology/approach: The proposed method was integrated methods as the Ishikawa diagram, 5Why method and AHP method. An idea of the proposed method was improving the process of identifying the problem by using a new mixed-method, ie. after Ishikawa diagram the methods 5Why and AHP. Findings: It was demonstrated, that it is purposeful using the proposed mixed method (ie. qualitative and quantitative) to expand the horizon of identifying the root cause. Research limitations/implications: The limitation is the subjectivism of the entity using the method, which is revealed during assessing the causes of a problem. Therefore, future works will be based on the minimalization of subjectivism by using the fuzzy Saaty scale. Practical implications: The proposed method is a new approach to analyse different types of problems and support the process of solve decision problems. Therefore, this method can be practice in service and production enterprises to identify the root of product incompatibility. Social implications: Using the method allows improving the process of making the decision about causes of the problem, and limits the possibility of wrong decision about the source of the problem. Originality/value: The proposed method was used a new proposal of mixed-method, ie. it was used integrated the Ishikawa diagram, 5Why method and AHP method. The presented method is some kind of a new approach to identify the root of the problem.

Opis:
Dynamically identifying key product changes is a challenge for enterprises. It is even more complicated if companies strive for the sustainable development of their activities. Therefore, the aim of the article was to develop a method to help predict the direction of product improvement, taking into account its quality level and environmental impact during the life cycle (LCA). The method was based on the design phase of LCA and the process of obtaining and processing customer expectations. Techniques supporting the developed method were: a questionnaire, a seven-point Likert scale, a standardised list of criteria for assessing the product life cycle, the WSM method, and a scale of relative states. The product analysis was carried out according to modified criteria states, which were evaluated according to: i) customer satisfaction (quality criteria), ii) environmental impact of LCA (environmental criteria), and iii) importance of quality and environmental criteria for customers. The originality of the method is to support the product improvement process to make it environmentally friendly within LCA and, at the same time, satisfactory to customers in terms of quality. The method will be used mainly by SMEs that want to initially predict the environmental impact of a product, including taking into account customer expectations.

The purpose of the study was to identify and typify the main qualitative-environmental aspects of the success of foundry enterprises facing economic changes. The research method developed was based on methodological triangulation. This procedure influenced the credibility of the collected data by including a significant number of sources and reducing the measurement error. The study used: surveying, face-to-face interviews, multiple case studies and content analysis. The survey made it possible to categorize the attributes of success (quality, sustainability, resources, flexibility, environment, management strategy customer relations). Assuming that the frequency of occurrence of an attribute indicates the level of its effectiveness, the following were considered the main determinants: training/courses, supervision and control, reliability and durability of products and compliance with standards and requirements, waste management and waste disposal, core and reserve capital, and customer service.

Purpose: The analysis process of the causes of the problem by often used the Ishikawa diagram is not effective when knowledge is lacking with regard to the particular causes and their validity about the problem. Moreover, its subjectivity is affected by the people undertaking the analysis. The aim of this paper is to reveal ways of improving the analysis process of problem causes by utilising the new method of integrated techniques, i.e.: Ishikawa diagram and FAHP method. Design/methodology/approach: An analysis process of problem causes was made by utilizing the integrated techniques, i.e.: Ishikawa diagram and FAHP method. The problem was identified after the FPI method, and it was a linear indication on the bearing cover from 410 steel. Findings: It was shown that implementing the FAHP method in the Ishikawa diagram allows a numerical analysis of the main causes of the problem. Research limitations/implications: Future analysis should be undertaken by applying the 5Why method, in order to fully identify the root cause of the problem. Practical implications: A new method can be used to precisely and reliably analyse different types of problems. Social implications: This method is an effective tool that supports the process of analysing the causes of the problem, and thus the process of making multi-criteria decisions. Originality/value: The article proposes a new method for integrating analysis techniques, i.e. the Ishikawa diagram and the FAHP method, which can be effective in precisely and reliably assessing the causes of the problem.

The research of railway track surfaces is aimed at improving the quality of railway infrastructure and providing the chance of improving the safety of driving. Therefore, it is advisable to constantly monitor track surface, but the research itself is not innovative because it is based on a known procedure. The crucial element are the techniques that allow to delve into the sources of problems. The aim of the article is to use the selected instruments of quality management to analyze the causes of track twist at high (180 km/h) speed of driving. The analysis of the causes of track twist was done on the basis of results from the measurement of one kilometer track section of the Krakow Glowny - Medyka route, which was made by using the TEC measuring device in April 2018. It was inferred that it is impossible to reach 180 km/h without track twist. In order to identify the causes of track twist it was proposed to use the selected sequence of quality management instruments, i.e.: brainstorming, Ishikawa diagram and the 5Why method. The identified causes of track twist include the abrasion of rail, scratches and exploitation point. The analysis and conclusions drawn from it may be useful in the analysis of other problems in railway transport as well as production and service industries.

The aim of the study was to develop a model to define a manufacturing company’s current market position using KPIs in relation to a key product - gearbox casting. The company's position was defined in terms of the relationship occurring between technological capabilities and market position. An additional aim of the study was to identify critical determinants and, ultimately, to identify conditions for strengthening market position. As a test of the proposed model, the position of the analysed company (in terms of technological capabilities and market position) was defined as “Search for occasions”- box 9 within the 3x3 matrix. Technological determinants that weaken the company’s position (low level of maintenance capacity and long production cycle time) and determinants with a strong negative impact on market position (low level of human resource development) were identified. An element of novelty is the use of KPIs as variables determining the position of the company within the3x3 matrix, which is indicative of a specific technological position in the market. Further lines of research will concern the determination of appropriate KPIs in relation to the identified critical areas of the company. Subsequent steps will concern the implications of the model in relation to the company’s other key aluminium alloy castings.

As part of continuous quality improvement in well-managed enterprises, identifying unconformity should initiate actions to find their causes. Therefore, it was proposed to the enterprise located in Podkarpacie to use in the sequential way the Ishikawa diagram and 5Why method. The aim was to analyse of unconformity (porosity cluster) on the turbine outlet nozzle and identify the root of its creation. In the enterprise, the quality analysis of the products with a fluorescent method was carried out, but after identifying the unconformity, non-analysis of their reason for their occurrence was not practiced. Therefore, it was intentional to propose the use of sequence i.e. Ishikawa diagram and 5Why method to identify the root of unconformity. The subject of study was the turbine outlet nozzle, on which the fluorescent method the porosity cluster was identified. With the use of the Ishikawa diagram, the main cause of the problem was pointed (unconformity during production), and by the 5Why method the root cause of the problem, i.e. unconformity material from the supplier, was identified. The proposed method sequence is a simple and effective way to make analyses of unconformities and it can be used in different products and service enterprises.
